ABSTRACT
Brief historical review of drainage procedures of the wounds and abdominal cavity was adduced. The main mechanisms of the drainage process, from the hydrodynamics laws point, were discussed. There was shown, that drainage occurs due to hydrostatic tissue of liquid and the capillarity laws. The main modern draining gears were analyzed and a comparative characteristics of their efficacy was adduced, depending on the drain construction and clinical situation present.

The results of surgical treatment in 24 patients suffering chronic duodenal impassability (CDI), caused by various mechanical reasons, were analyzed. The most frequent of them were pancreatic cancer, arteriomesentery compression, pancreatic pseudocyst, volumic formations, adhesive deformity of duodenojejunal juncture, duodenal polyposis. There was proposed the method of duodenal arteriomesentery compression correction, which permits to restore effectively the duodenal motor-evacuatory function. While choosing the method of CDI it is necessary to pay attention to its etiologic cause.

Results of treatment of 2308 patients, suffering nonincarcerated hernia of abdominal wall of various localization, were analyzed. In 24 (1.1%) of them different pathologic changes of hernial sac were noted and in 17 (0.7%)--the diseases of organs, localized in hernial sac: an acute appendicitis, ileus, trauma, malignant tumor and others. The course of majority of the above-mentioned diseases was masked by incarcerated hernia signs, demanding the urgent operation performance. The operative intervention volume depended on the character of the organ pathological changes, localized in hernial sac, it was directed on the main disease elimination and on the abdominal wall defect closure.

The results of operation of revascularized osteotrepanation (ROT) in complex of treatment of 43 patients with chronic occlusion of the lower extremities arteries were analyzed. Stable clinical effect after application of ROT in patients with the ischemia II stage persisted during 40 months, III stage--25 months, "high" amputation of lower extremity in patients with IV stage ischemia was performed in 3.5-7 months. ROT is operation of choice in patients with distal form of ischemia of lower extremity, as well as while presence of contraindications for performance of reconstructive operation on main arteries.

Results of treatment of 84 patients with diabetic foot (DF) were analyzed, in whom, according to the radiologic investigation data, osteoporosis of the foot bones was revealed. Impact of osteochine was studied in 40 patients with DF and signs of the foot bones osteoporosis. Positive result of osteochine usage was noted in 90% patients, what had permitted to reduce duration of treatment in stationary by 8.3 days. Negative results of preparation application were not observed.

Comparative analysis of clinical course and the diagnostic methods of purulent-inflammatory disease of soft tissues was performed in 20 patients with drug abuse (the 1st group) and in 20 patients, who are keeping the normal way of living (2nd group). In 16 (80%) of patients of the 1st group the anaerobic non-clostridial infection (ANI) was diagnosed. The factors causing the ANI occurrence, were determined, the programme of treatment was introduced. In the 1st group 3 patients died. The duration of the stationary course in the 1st group was 18 days and in the 2nd one--3.8 days.
