ABSTRACT

Introduction: This case report presents the intentional periodontal maintenance of two periodontal hopeless lower central incisors with a multidisciplinary approach and 20-year follow-up. Case presentation: A 36-year-old male, in 2001, was diagnosed with aggressive periodontitis, gingival swelling, bleeding, and mandibular central incisors with mobility and poor prognosis. Following periodontal therapy (phase I), root canal treatment, and occlusal adjustment, #31 and #41 were gently extracted to remove the granulation tissues, calculus, and infected cementum from the root surface. Then, tetracycline-HCl was applied for 5 minutes on the root surfaces. The teeth were repositioned into the sockets and splinted with a lingual bar. At 3 months, the bar was removed, and a free gingival autogenous graft was done to improve the local keratinized tissue width. Mobility scores, pocket depths, and clinical attachment levels were recorded, and radiographs were taken at 1, 5, and 20 years. The 5-year follow-up showed that the teeth were clinically and radiographically in function. There was a reduction in probing depth and a gain in clinical attachment and radiographic alveolar bone levels. After 20 years, #41 was stable, but #31 had external root resorption, leading to a new treatment plan (dental implants) and extraction. Conclusion: The clinical result of this case was satisfactory for 20 years. Intentional periodontal maintenance of the teeth may be an alternative treatment, even considering the high level of complexity.

ABSTRACT

Abstract: Studies have found that blood flow to the renal medulla is an important determinant of pressure-natriuresis and the long-term regulation of arterial pressure. First, a brief review of methods developed enabling the study of the medullary circulation is presented. Second, studies performed in rats are presented showing medullary blood flow plays a vital role in the pressure-natriuresis relationship and thereby in hypertension. Third, it is shown that chronic reduction of medullary blood flow results in hypertension and that enhancement of medullary blood flow reduces hypertension hereditary models of both salt-sensitive rats and salt-resistant forms of hypertension. The key role that medullary nitric oxide production plays in protecting this region from ischemic injury associated with circulating vasoconstrictor agents and reactive oxygen species is presented. The studies cited are largely the work of my students, research fellows, and colleagues with whom I have performed these studies dating from the late 1980s to more recent years.

Resumen Introducción: Varias presiones antrópicas sufren los ecosistemas acuáticos del piedemonte llanero en Colombia. La respuesta a estresores ambientales aún se desconoce en organismos bioindicadores como Leptohyphidae. Objetivo: Determinar la diversidad de ninfas de Leptohyphidae del río Quenane-Quenanito, en dos periodos hidrológicos contrastantes y su relación con algunas variables fisicoquímicas. Métodos: En diciembre (2014) y febrero (2015) se recolectaron organismos con red Surber en seis estaciones a lo largo del río. Se analizó la diversidad alfa y beta y se aplicó análisis de redundancia y modelos lineales generalizados con el fin de establecer la relación entre los taxones y las variables ambientales. Resultados: Se identificaron 369 organismos pertenecientes a cuatro géneros (Amanahyphes, Traverhyphes, Tricorythopsis y Tricorythodes), dos especies y ocho morfoespecies. Se reporta por primera vez para el departamento del Meta Amanahyphes saguassu. Se registró la mayor diversidad de ninfas en la transición a la sequía y la mayor abundancia en sequía. La diversidad beta señaló que la configuración del ensamblaje cambia a nivel espacial y temporal. Conclusiones: Los organismos de Leptohyphidae prefieren hábitats de corrientes, particularmente en el periodo de sequía, donde hallan alimento (hojarasca, detritos) y refugio para establecerse exitosamente; actividades antrópicas como la urbanización afectan notablemente la diversidad. La alta diversidad registrada en este pequeño río de piedemonte llanero refleja la necesidad de incrementar este tipo de trabajos y esfuerzos de recolección de material de estudio en la región.


Abstract Introduction: Various anthropic pressures affect the aquatic ecosystems of the foothills of Colombia. The response to environmental stressors is still unknown in bioindicator organisms such as Leptohyphidae. Objective: To determine the diversity of Leptohyphidae nymphs of the Quenane-Quenanito river, in two contrasting hydrological periods and its relationship with some physicochemical variables. Methods: In December (2014) and February (2015), organisms were collected with a Surber net at six stations along the current. Alpha and beta diversity was analyzed and redundancy analysis and generalized linear model were applied to establish the relationship between taxa and environmental variables. Results: Were identified 369 organisms belonging to four genera (Amanahyphes, Traverhyphes, Tricorythopsis, and Tricorythodes), two species, and eight morphospecies. Amanahyphes saguassu is reported for the first time for the Meta department. High diversity of Leptohyphidae nymphs was recorded in the transition to drought season and greater abundance in drought. Beta diversity indicated that the configuration of the assemblage changes spatially and temporally. Conclusions: Leptohyphidae organisms prefer fast habitats, particularly in the dry period where they find food (leaf litter, detritus) and shelter to establish themselves successfully; anthropic activities such as urbanization notably affect diversity. The high diversity recorded in this small river in the foothills of the plains reflects the need to increase this type of works and collection efforts of study material in the region.

ABSTRACT

La linfohistiocitosis hemofagocítica (LHH) es una entidad rara que se caracteriza por un estado hiperinflamatorio secundario a la activación desregulada del sistema inmune con compromiso multisistémico. Puede ser primaria o hereditaria, o estar desencadenada por diversas enfermedades. La mortalidad sin tratamiento oportuno es del 50 % de los casos. Se presenta el caso de una paciente de 1 año y 8 meses con diagnóstico reciente de infección por virus de inmunodeficiencia humana en estadio sida. Cursó internación para estudio e inicio de tratamiento antirretroviral durante la cual presentó múltiples intercurrencias infectológicas e inmunológicas. Se destacan dos episodios de linfohistiocitosis hemofagocítica en contexto de inmunodeficiencia adquirida no controlada y coinfecciones oportunistas. El objetivo de este reporte es destacar la importancia de la sospecha de LHH para un diagnóstico y tratamiento pertinente


Hemophagocytic lymphohistiocytosis (HLH) is a rare condition characterized by a hyperinflammatory state secondary to dysregulated immune activity with multisystem involvement. HLH may be primary or hereditary, or triggered by various diseases. Mortality without a timely treatment reaches 50% of the cases. Here we describe the case of a 1-year and 8-month-old female patient with a recent diagnosis of human immunodeficiency virus infection in the AIDS stage. She was hospitalized for assessment and initiation of antiretroviral therapy during which she developed multiple intercurrent infectious and immune conditions. Two episodes of hemophagocytic lymphohistiocytosis in the setting of uncontrolled acquired immunodeficiency and opportunistic co-infections stand out. The objective of this case report is to highlight the importance of suspecting HLH for a relevant diagnosis and treatment.

ABSTRACT

Los postbióticos fueron definidos en 2021 por la Asociación Científica Internacional de Probióticos y Prebióticos (ISAPP) como "una preparación de microorganismos inanimados y/o sus componentes celulares capaces de conferir un efecto benéfico al hospedador". El campo de los postbióticos es un área nueva dentro de la familia de los bióticos; se han desarrollado ya numerosos productos con aplicaciones clínicas, como la estimulación inmunológica, el manejo de diarreas en niños y adultos, el abordaje del intestino irritable, además de tres fórmulas infantiles. En particular, las fórmulas infantiles con postbióticos obtenidos a partir de la fermentación de la leche con Bifidobacterium breve C50 y Streptococcus thermophilus O65, y sus metabolitos, incluido el oligosacárido 3'-GL, han demostrado seguridad y contribución al desarrollo de la microbiota intestinal y el sistema inmune asociado al intestino. Estas modificaciones contribuyen a la prevención y el manejo de los trastornos funcionales digestivos del lactante.


Postbiotics were defined in 2021 by the International Scientific Association for Probiotics and Prebiotics (ISAPP) as a "preparation of inanimate microorganisms and/or their cellular components that confers a health benefit to the host." The field of postbiotics is a new area within the biotics family; numerous products have already been developed for clinical applications, such as immune stimulation, the management of diarrhea in children and adults, the management of irritable bowel syndrome, and 3 infant formulas. In particular, infant formulas with postbiotics obtained from milk fermented with Bifidobacterium breve C50 and Streptococcus thermophilus O65 ­and their metabolites­, including the oligosaccharide 3'-GL, have demonstrated to be safe and to contribute to the development of the gut microbiota and the gutassociated immune system. These modifications help to prevent and manage functional gastrointestinal disorders in infants.

ABSTRACT

Introducción. La hipotermia terapéutica (HT) reduce el riesgo de muerte o discapacidad en niños con encefalopatía hipóxico-isquémica (EHI) moderada-grave. Objetivo. Describir una población de pacientes con EHI que requirió HT y su evolución hasta el alta hospitalaria. Población y métodos. Estudio descriptivo de cohorte retrospectivo. Se analizaron todos los pacientes que ingresaron a HT entre 2013 y 2022. Se evaluaron datos epidemiológicos, clínicos, de monitoreo, tratamiento, estudios complementarios y condición al alta. Se compararon los factores de riesgo entre pacientes fallecidos y sobrevivientes, y de estos, los que requirieron necesidades especiales al alta (NEAS). Resultados. Se incluyeron 247 pacientes. Mortalidad: 11 %. Evento centinela más frecuente: período expulsivo prolongado (39 %). Inicio del tratamiento: mediana 5 horas de vida. Convulsiones: 57 %. Eritropoyetina intravenosa: 66,7 %. Patrón anormal de monitoreo de función cerebral: 52 %. Normalización del monitoreo: mediana 24 horas. Resonancia magnética patológica: 42 %. Variables predictoras de mortalidad: Sarnat y Sarnat grave, y ecografía patológica al ingreso. Conclusión. La mortalidad global fue del 11 %. Las derivaciones aumentaron en forma más evidente a partir del año 2018. El horario de inicio de HT fue más tardío que en reportes anteriores. Los signos neurológicos de gravedad según la escala de Sarnat y Sarnat y la ecografía cerebral basal patológica fueron predictores independientes de mortalidad al alta. Los pacientes con NEAS presentaron normalización del trazado del electroencefalograma de amplitud integrada más tardío. El hallazgo más frecuente en la resonancia fue la afectación de los ganglios basales. No se encontraron diferencias clínicas ni de complicaciones estadísticamente significativas entre los pacientes que recibieron eritropoyetina.


Introduction. Therapeutic hypothermia (TH) reduces the risk of death or disability in children with moderate to severe hypoxic ischemic encephalopathy (HIE). Objective. To describe a population of patients with HIE that required TH and their course until discharge. Population and methods. Retrospective, descriptive, cohort study. All patients admitted to TH between 2013 and 2022 were studied. Epidemiological, clinical, monitoring, and treatment data were assessed, together with supplementary tests and condition at discharge. Risk factors were compared between deceased patients and survivors; and, among the latter, those requiring special healthcare needs (SHCN) at discharge. Results. A total of 247 patients were included. Mortality: 11%. Most common sentinel event: prolonged second stage of labor (39%). Treatment initiation: median of 5 hours of life. Seizures: 57%. Intravenous erythropoietin: 66.7%. Abnormal pattern in brain function monitoring: 52%. Normalization of monitoring: median of 24 hours. Pathological magnetic resonance imaging: 42%. Predictor variables of mortality: severe Sarnat and Sarnat staging and pathological ultrasound upon admission. Conclusion. The overall mortality rate was 11%. Referrals increased more markedly since 2018. The time of TH initiation was later than in previous reports. Severe neurological signs as per the Sarnat and Sarnat staging and a pathological baseline cranial ultrasound were independent predictors of mortality at discharge. Patients with SHCN at discharge showed a normalized tracing in the amplitude-integrated electroencephalography performed later. The most common finding in the magnetic resonance imaging was basal ganglia involvement. No statistically significant differences were observed in terms of clinical characteristics or complications among patients who received erythropoietin.

Introducción. El yodo desempeña un rol fundamental en el metabolismo, el crecimiento y el desarrollo humano. Durante el embarazo y la infancia, la demanda de este micronutriente aumenta considerablemente. La tirotropinemia neonatal (TSHn) aumentada, definida como TSHn ≥5 mUI/l, es un marcador que señala la deficiencia de yodo en una población cuando su prevalencia supera el 3 %. Objetivo. Determinar la prevalencia de TSHn ≥ 5 en La Pampa durante el período 2021-2022, analizar su correlación con diferentes variables y compararla con datos de una cohorte histórica. Población y métodos. Estudio transversal, de diseño descriptivo-analítico, sobre una población de neonatos nacidos en las cinco zonas sanitarias de la provincia de La Pampa durante los años 2021 y 2022. Resultados. De los 5778 neonatos evaluados, el 9,6 % presentó niveles de TSHn ≥5 mUI/l. El 70,4 % de estas mediciones fueron realizadas después del tercer día de vida. No se observaron diferencias significativas en la frecuencia de niveles elevados de TSHn según el año de nacimiento, peso al nacer o días hasta la extracción. Se registró una mayor prevalencia en el sexo masculino (10,6 % versus 8,5 %; p = 0,007) y entre los neonatos nacidos a término (9,8 % versus 6,6 %; p = 0,02). La prevalencia de hipertirotropinemia fue superior a la observada en una cohorte de 2001-2002. Conclusiones. La prevalencia de hipertirotropinemia neonatal en La Pampa durante los años 2021 y 2022 fue del 9,6 %, lo que indica un estado de deficiencia leve de yodo en la provincia, superior al reportado hace dos décadas.


Introduction. Iodine plays a key role in human metabolism, growth, and development. During pregnancy and childhood, the demand for this micronutrient increases notably. Increased neonatal thyroid stimulating hormone (nTSH) levels, defined as nTSH ≥ 5 mIUL, are a marker of iodine deficiency in a population if its prevalence is higher than 3%.Objective. To establish the prevalence of nTSH ≥ 5 in La Pampa in the 2021­2022 period, analyze its correlation with different variables, and compare it with data from a historical cohort.Population and methods. Cross-sectional, descriptive-analytical study in a population of newborn infants born in the 5 health regions of the province of La Pampa in 2021 and 2022. Results. Of  the  5778  assessed  newborn  infants,  9.6%  had  nTSH  levels  ≥  5  mIU/L. It was reported that 70.4% of these measurements were done after the third day of life. No significant differences were observed in the frequency of high nTSH levels by year of birth, birth weight, or days until samplecollection.A higher prevalence was observed among male infants (10.6% versus 8.5%; p = 0.007) and term infants (9.8% versus 6.6%; p = 0.02). The prevalence of high TSH levels was superior to that observed in the 2001­2002 cohort. Conclusions. The  prevalence of high nTSH levels in La Pampa during 2021 and 2022 was 9.6%, suggesting the presence of mild iodine deficiency in the population of this province, higher that what  had been reported 2 decades ago.

INTRODUÇÃO: O fechamento percutâneo transcateter do apêndice atrial esquerdo surgiu recentemente como uma alternativa segura e eficaz à terapia anticoagulante de longo prazo na prevenção de AVC e na redução dos riscos de sangramento em pacientes com FA não valvular. No entanto, o papel da combinação do fechamento percutâneo transcateter do apêndice atrial esquerdo e ablação por cateter em um único procedimento ainda não está claro. OBJETIVOS: Neste estudo, os autores buscaram comparar a segurança do fechamento percutâneo transcateter do apêndice atrial esquerdo, ablação por cateter e o procedimento combinando-os em pacientes com fibrilação atrial. MÉTODOS: As bases de dados MEDLINE, EMBASE e a Cochrane Central foram revisadas sistematicamente em busca de estudos que relatassem o desfecho de complicações periprocedimentos na ablação por cateter e oclusão do apêndice atrial esquerdo em uma abordagem "one-stop". O procedimento combinado dessas abordagens, o fechamento percutâneo transcateter do apêndice atrial esquerdo e a ablação por cateter isoladamente foram analisados. Foi realizada uma metaanálise em rede bayesiana para estimar os efeitos relativos entre tratamentos utilizando razão de risco (RR) e para classificar cada um de acordo com a "Surface Under the Cumulative Ranking Curve" (SUCRA). RESULTADOS: Identificamos 11 ensaios relevantes representando 3343 participantes. A avaliação dos valores SUCRA indicou que apenas o LAAC (0,84) emergiu como o tratamento mais eficaz para a redução das complicações periprocedimento, seguido por apenas ablação (0,60) e abordagem combinada (0,05). O risco de complicações periprocedimento foi maior para o grupo apenas ablação (RR 0,73; intervalo credível [CrI] 0,45, 1,1) e menor para o grupo LAAC (RR 0,62; CrI 0,35, 1,0) quando comparados ao grupo combinado. Na comparação indireta entre os grupos LAAC apenas e ablação apenas, não houve significância estatística (Odds Ratio [OR] 1,1; CrI 0,27, 4,4; p=0,60). CONCLUSÃO: Nesta metanálise de rede, a abordagem combinada do fechamento percutâneo transcateter do apêndice atrial esquerdo e da ablação por cateter foi associada a um aumento de complicações periprocedimentos quando comparado aos procedimentos isoladamente. Portanto, deve-se avaliar cuidadosamente os benefícios e riscos de realizar esses procedimentos em conjunto, levando em consideração a segurança do paciente.

ABSTRACT

INTRODUÇÃO: A ablação da fibrilação atrial (FA) é essencialmente baseada no isolamento das veias pulmonares (PVI), no entanto, a recorrência da FA devido à reconexão da VP é um desafio. O uso da técnica de alta potência de curta duração (HPSD; > 40 W) mostrou-se mais eficaz e seguro, proporcionando lesões uniformes. Recentemente, foi introduzida a técnica de muito alta potência de curta duração (vHPSD) (90 W, 4 s), no entanto, sua eficácia e segurança ainda são controversas quando comparadas à técnica de alta potência. OBJETIVO: Realizamos uma revisão sistemática e meta-análise para comparar a técnica de altíssima potência e curta duração (vHPSD; 70-90 W) e a técnica de alta potência e curta duração (HPSD; > 40 W) em pacientes com FA. MÉTODOS: Pesquisamos no PubMed, Embase e Cochrane Central. Os desfechos foram a recorrência da FA e as complicações. Um modelo de efeitos aleatórios foi usado para calcular as razões de chances (ORs) com intervalos de confiança (IC) de 95%. A análise estatística foi realizada utilizando o programa R (versão 4.3.2). A heterogeneidade foi avaliada com estatísticas I2. RESULTADOS: Incluímos pacientes de 7 estudos. Destes, 1 foi derivado de RCT e 6 foram estudos observacionais. Em comparação com a alta potência de curta duração, a técnica de altíssima potência e curta duração (vHPSD; 70-90 W) não demonstrou uma variação estatisticamente significativa nas taxas de recorrência da FA (OR 0,91; IC 95% 0,50 - 1,67; p=0,769; I2 = 48%) e complicações associadas (OR 1,04; IC 95% 0,38 - 2,86; p=0,933; I2 = 0%). CONCLUSÃO: Os dados desta metaanálise revelam que não houve diferença significativa na recorrência da FA e nas complicações entre a técnica de muito alta potência de curta duração e a técnica de alta potência de curta duração.

ABSTRACT

INTRODUÇÃO: A rápida restauração do fluxo sanguíneo em pacientes com infarto agudo do miocárdio com supradesnivelamento do segmento ST (IAMCSST) através da intervenção coronariana percutânea (ICP) é crucial para a sobrevivência desta população. As tentativas de diminuir o tempo desde o diagnóstico do IAMCSST até a chegada ao laboratório de cateterismo têm sido extensivamente investigadas. Contudo, faltam estratégias que visem reduzir o tempo intraprocedimento. OBJETIVO: Portanto, realizamos uma meta-análise para avaliar a revascularização do vaso culpado antes da angiografia completa como estratégia para minimizar atrasos na ICP primária em pacientes com IAMCSST. MÉTODOS: Pesquisamos na PubMed, Embase e Cochrane Central. Os desfechos de interesse foram: tempo acesso vascular-balão, tempo porta-balão, tempo primeiro contato médico-balão, mortalidade hospitalar, mortalidade em 30 dias, mortalidade em 1 ano, mortalidade cardíaca em 30 dias, reinfarto em 30 dias, sangramento BARC ≥ tipo 3, encaminhamento para cirurgia de revascularização do miocárdio e FEVE %. A análise estatística foi realizada no programa R (versão 4.3.2). A heterogeneidade foi avaliada com estatística I2. RESULTADOS: Incluímos 2.050 pacientes de 6 estudos, dos quais 2 eram ECRs e 4 estudos observacionais. A mediana de acompanhamento variou de 17 a 65 meses. A revascularização do vaso culpado antes da angiografia completa foi associada a uma diminuição estatisticamente significativa dos tempos: acesso vascular-balão (MD -6,79; IC 95% [- 8,00, - 5,58]; p<0,01; I2 = 82%) e porta-balão (MD -9,02; IC 95% [− 12,83, − 5,22]; p<0,01; I2 =93%). Além disso, a abordagem inicial da artéria culpada com a ICP foi associada ao aumento da FEVE (MD 1,90; IC95% 0,77 − 3,04]; p<0,01; I2 =82%). Não houve diferença significativa entre os grupos em termos de mortalidade hospitalar (RR 1,15; IC 95% 0,34 - 3,87; p=0,823; I2 =0%), mortalidade em 1 ano (RR 0,83; IC 95% 0,59 - 1,17; p =0,282; I2 =0%), mortalidade cardíaca em 30 dias (RR 0,77; IC 95% 0,38 - 1,57; p=0,472; I2 =0%) e reinfarto em 30 dias (RR 1,02; IC 95% 0,29 - 3,58; p=0,980; CONCLUSÃO: Nesta meta-análise abrangente de pacientes que apresentaram IAMCSST, a realização da ICP na lesão culpada antes da angiografia coronária completa levou a tempos de reperfusão significativamente mais curtos, sem diferenças discerníveis nas taxas de complicações.

ABSTRACT

INTRODUÇÃO: O reparo transcateter borda a borda utilizando o dispositivo MitraClip é uma ótima alternativa para tratar pacientes com insuficiência mitral e com alto risco cirúrgico. No entanto, a eficácia e segurança da nova geração de MitraClip versus as gerações anteriores não estão bem estabelecidas. OBJETIVO: Portanto, nosso objetivo foi realizar uma meta-análise explorando as gerações de MitraClip no reparo borda a borda da valva mitral por transcateter. MÉTODOS: Pesquisamos no PubMed, Embase e Cochrane Central estudos comparando dispositivos MitraClip de quarta geração com as gerações mais precoces em pacientes com regurgitação mitral (RM) tratados com reparo transcateter borda a borda. Os desfechos foram grau de RM, RM residual >2+, sucesso técnico, sucesso do dispositivo, número de clipes, NYHA III ou IV e mortalidade por todas as causas. A análise estatística foi realizada utilizando o programa R (versão 4.3.2). A heterogeneidade foi avaliada com estatística I2. RESULTADOS: Incluímos 2.123 pacientes de 6 estudos observacionais. Os resultados agrupados não revelaram diferença estatisticamente significativa no grau de RM ≤ 2+ após o procedimento (95,9% vs 95,7%; OR 1,05; IC 95% 0,68 - 1,62; p=0,839; I2 =0%), grau de RM ≤ 1+ após procedimento (OR 0,97; IC 95% 0,71 - 1,32; p=0,857; I2 =42%), sucesso do dispositivo (OR 1,20; IC 95% 0,87 - 1,68; p=0,275; I2 =0%), sucesso técnico (OR 0,98; IC 95% 0,66 - 1,46; p=0,919; I2 =0%) entre grupos de quarta geração e gerações iniciais. Entretanto, a opção pelo MitraClip de quarta geração em pacientes com insuficiência mitral foi associada a um maior número de pacientes que necessitaram de apenas 1 clipe durante o procedimento (OR 2,06; IC 95% 1,23 - 3,46; p = 0,007; I2 = 0%). CONCLUSÃO: Os dados desta meta-análise revelaram que não houve diferença significativa na redução da regurgitação mitral, na taxa de pacientes com NYHA III e IV e na mortalidade por todas as causas entre as gerações de MitraClip. Embora o uso da quarta geração estivesse associado a uma menor necessidade de clipes durante o procedimento.

ABSTRACT

BACKGROUND: Brain metastases (BMs) represent the most common intracranial neoplasms in adults, affecting up to 25% of patients with metastatic cancers. Primary cancers of the lung, melanoma and colorectal are responsible for the majority of diagnosed BMs. Current therapies for BMs include stereotactic radiosurgery, whole-brain radiotherapy, surgical resection, interstitial laser thermal therapy, systemic cytotoxic chemotherapy, targeted agents, and PD1/PDL-1 blockage, which play a crucial role in cancer immunotherapy. METHODS: A systematic search was conducted through PubMed, Embase and Cochrane databases for studies that assess the benefit of neoadjuvant treatment with PD-1/PD-L1 inhibitors plus steroids and the impact of this combination on treatment effectiveness. Hazard ratios (HRs) were computed for binary endpoints, with 95% confidence intervals (CIs). We performed the meta-analysis using RStudio v4.4.2 software. RESULTS: The systematic analysis, including 1,658 patients, assessed the impact of PD-1/PD-L1 inhibitors on overall survival (OS) and progression-free survival (PFS) in treating BMs from non-small cell lung cancer (NSCLC) and melanoma. For OS, eight studies with 848 patients indicated a significant improvement using steroids (HR: 1.978; 95% CI 1.308-2.992; I2 = 62%) compared to non-users. Regarding PFS, data from four studies involving 790 patients did not reach statistical significance (HR: 1.483; 95% CI: 0.843-2.608; I2 = 82%). The addition of steroids did not show a clear beneficial effect on the efficacy of PD-1/PD-L1 inhibitors in extending progression-free survival. CONCLUSIONS: The systematic analysis underscores the effectiveness of PD-1/PD-L1 blockage in improving overall survival in patients with BMs from NSCLC and melanoma, however, their impact on delaying disease progression, especially when combined with steroids, requires further investigation to clarify their role and optimize therapeutic strategies.

ABSTRACT

Hormonal protocols based on progestogens and equine chorionic gonadotrophin (eCG) are efficient for estrus and ovulation synchronization in ewes. Although eCG is indispensable during seasonal anestrus, it may not be necessary during the breeding season. Thus, we tested the hypothesis that GnRH is effective in replacing eCG during the breeding season allowing satisfactory ovulation rate, luteal function and conception rates after timed artificial insemination (TAI). Ewes (n = 134) with a minimum body condition score of 2.5 (0-5 scale) were treated with intravaginal devices (IVD) containing 60 mg of medroxyprogesterone acetate (MPA) for seven days and received 0.26 mg of sodium cloprostenol at the time of IVD removal. In Exp. 1, at IVD removal, ewes (n = 29) were allocated to three groups: eCG (200 IU at IVD removal; n = 10); eCG+GnRH (200 IU eCG at IVD removal and 4 µg of buserelin 36 h later; n = 10); or GnRH (buserelin 36 h after IVD removal; n = 9). Blood samples were collected 2, 6 and 12 days after TAI moment (54 h after IVD removal), for progesterone (P4) analysis. In Exp 2, the ewes were allocated to eCG (n = 10) or GnRH (n = 10) groups, as above described, and ovulation moment was evaluated 54, 66 and 78 h after IVD removal. In Exp 3, TAI was performed in ewes from eCG (n = 45) and GnRH (n = 40) groups using 100 × 106 motile spermatozoa from a pool of semen collected from four rams. In Exp. 1, based on P4 levels, we confirmed that all the ewes ovulated (29/29) and there was no significant effect of group (P = 0.89) or group x day (P = 0.18) on P4 concentration, being observed a significant effect of day (P = 0.0001). In Exp. 2, the maximum DF diameter (P = 0.26) and ovulation moment (P = 0.69) did not differ between groups. In Exp. 3, pregnancy rate was significantly lower (P = 0.02) in GnRH (22.5 %; 9/40) compared to eCG (46.7 %; 21/45). The results indicate that, although ovulation and luteal function were not altered after eCG, eCG+GnRH or GnRH treatment, GnRH alone before TAI cannot be used to replace eCG treatment during the breeding season.

ABSTRACT

BACKGROUND: Individuals who were formerly incarcerated have high tuberculosis incidence, but are generally not considered among the risk groups eligible for tuberculosis prevention. We investigated the potential health impact and cost-effectiveness of Mycobacterium tuberculosis infection screening and tuberculosis preventive treatment (TPT) for individuals who were formerly incarcerated in Brazil. METHODS: Using published evidence for Brazil, we constructed a Markov state transition model estimating tuberculosis-related health outcomes and costs among individuals who were formerly incarcerated, by simulating transitions between health states over time. The analysis compared tuberculosis infection screening and TPT, to no screening, considering a combination of M tuberculosis infection tests and TPT regimens. We quantified health effects as reductions in tuberculosis cases, tuberculosis deaths, and disability-adjusted life-years (DALYs). We assessed costs from a tuberculosis programme perspective. We report intervention cost-effectiveness as the incremental costs per DALY averted, and tested how results changed across subgroups of the target population. FINDINGS: Compared with no intervention, an intervention incorporating tuberculin skin testing and treatment with 3 months of isoniazid and rifapentine would avert 31 (95% uncertainty interval 14-56) lifetime tuberculosis cases and 4·1 (1·4-5·8) lifetime tuberculosis deaths per 1000 individuals, and cost US$242 per DALY averted. All test and regimen combinations were cost-effective compared with no screening. Younger age, longer incarceration, and more recent prison release were each associated with significantly greater health benefits and more favourable cost-effectiveness ratios, although the intervention was cost-effective for all subgroups examined. INTERPRETATION: M tuberculosis infection screening and TPT for individuals who were formerly incarcerated appears cost-effective, and would provide valuable health gains. ABSTRACT

Surface waters are vulnerable to contamination by human and animal feces, posing risks to human health due to potential exposure to enteric pathogens. This research developed a colorimetric loop-mediated isothermal amplification (cLAMP) assay to detect sewage associated Bacteroides dorei HF183/BacR287 (HF183) marker in wastewater and environmental water samples. The host sensitivity and host specificity of the assay were evaluated, and their performance was compared to the Bacteroides HF183 qPCR assay using control materials (gBlocks), environmental water samples seeded with untreated sewage, and ambient environmental water samples. In serial dilutions of control materials, qPCR produced quantifiable data across all dilutions, while cLAMP detected the marker down to 0.001 pg/µL of control materials, which was two orders of magnitude less sensitive than qPCR. All untreated sewage samples (n = 12) tested positive for HF183 by both the qPCR and cLAMP assays, demonstrating a host sensitivity value of 1.00 (maximum value of 1.00). The host specificity by analysing 70 non-human fecal nucleic acid samples revealed cLAMP's specificity value of 0.81 compared to qPCR's 0.64. When testing sewage-seeded environmental water samples, both methods detected HF183 for the lowest amount of sewage, indicating similar detection sensitivity. The application of cLAMP for tracking sewage pollution in environmental waters showed promising results, with moderate agreement between cLAMP and qPCR (κ = 0.510). However, cLAMP occasionally missed detections compared to qPCR, particularly in low-concentration samples. Overall, the cLAMP HF183 assay demonstrated promising potential as a rapid and sensitive method for detecting sewage pollution, offering a viable alternative to qPCR in certain environmental monitoring scenarios.

ABSTRACT

The consumption of a high-fat high-fructose diet partly resemble the western dietary patterns, which is closely associated with excessive body adiposity and metabolic disorders, such as obesity and type 2 diabetes. Moreover, this unhealthy regime produces unfavourable changes on the faecal microbiota, potentially interfering with microorganisms postbiotic function, such as spermidine, a natural polyamine that has been involved in the control of weight gain. The study aimed to analyse the repercussions of spermidine supplementation on somatic measurements, metabolic markers, and the faecal microbiota profile of rats fed a diet rich in fat and fructose. Indeed, Wistar males with oral administration of spermidine (20 mg/kg/day) for 6 weeks were evaluated for food and energy intake, biochemical markers, and faecal microbiota signatures. The daily use of spermidine decreased weight gain ( P < 0.01), reduced feed efficiency ( P < 0.01), and attenuated visceral fat deposition ( P < 0.01), although no effect on energy intake, hepatic weight, triglyceride and glucose index and atherogenic indexes. Similarly, the consumption of spermidine partially restored the presence of microbial species, notably Akkermansia muciniphila. Elevated concentrations of this species were linked to a decrease in triglycerides ( P = 0.04), indicating that the supplementation of spermidine might contribute to managing energy fuel homeostasis in association with an obesogenic diet.

ABSTRACT

OBJECTIVE: To describe independent factors related to the interaction of FTO rs9939609, TMEM18 rs6548238, leptin, and adiponectin in children/adolescents with asthma, under the influence of obesity. METHODS: The authors performed a cross-sectional study with 57 children/adolescents, ages 8-19 years, at a tertiary hospital, from 2017 to 2018. Participants were classified by nutritional status, performed spirometry with a bronchodilator test and completed an asthma questionnaire, higher scores indicated more asthma symptoms. Two asthma groups were formed: Group 1(G1)-normal-weight; Group 2(G2)-overweight/obese. Serum was collected for adipokines (n = 32) and genetic polymorphisms (n = 53) dosages. RESULTS: Age and body mass index (BMI) correlated directly in normal-weight (p = 0.009) and obese participants (p = 0.004). Girls reported more asthma complaints (p = 0.044). Participants with negative bronchodilator responses presented lower BMI (14.55-17.16) than responders (19.4-26.84) (p = 0.049). Leptin dosages are related directly to BMI (5,34-40 ng/ml in obese × 0,54-42 ng/ml in nonobese) (p = 0.003). Levels were high in girls (4.78-17.55 µg/ml) (p = 0.029) and low in nonobese boys (0.54-6.92 µg/ml) (p = 0.006). In obese, low leptin levels (< 10 ng/ml) were found in small airway dysfunction carriers (p = 0.025); elevated adiponectin (> 5 µg/ml) correlated with FEV1/FVC > 80 % (p = 0.035) and positive bronchodilator tests (8.84-13 µg/ml) (p = 0.039); and FTO A allele correlated with low adiponectin 0-8.84 µg/ml (p = 0.021) and low FEV1/FVC (46 %-88 %) (p = 0.023). CONCLUSION: BMI correlated directly with age and leptin levels. Obese participants presented high serum levels of leptin and FTO A allele correlated with low FEV1/FVC. Larger cohorts are necessary for better elucidation of the role of adipokines and polymorphisms in the pathophysiology of asthma and obesity.

BACKGROUND: The incidence of oral cancer has exhibited a rise within the young population. Considering that oral potentially malignant disorders (OPMDs) can precede the development of oral cancer, it is imperative to conduct studies in this particular younger population. This study aimed to evaluate the frequency and conduct a comparative analysis of the clinical-demographic characteristics of OPMDs in two distinct age groups. MATERIAL AND METHODS: A retrospective analysis was conducted with patients diagnosed with leukoplakia, erythroplakia, and leukoerythroplakia between 1965 and 2020. The individuals were categorized into two groups: those aged up to 40 years (Group Younger) and those aged 41 years and above (Group Older). RESULTS: A total of 640 lesions were subjected to analysis. Among these, patients aged up to 40 years constituted 10.63% of the sample, however, this proportion decreased significantly to 6.9% between 2010 and 2020. A predominant male representation was observed in both groups, with white lesions being the most common in both as well. However, the frequency of red or mixed lesions was significantly higher (p=0.034) in the older group, along with a higher prevalence of dysplastic lesions (26.9% versus 11.8%, p=0.01). Moreover, the older group exhibited a relatively higher percentage of smokers/ex-smokers (78.6%), compared to the younger group (61.5%, p=0.085) and alcohol consumers/ex-consumers (54.9% versus 22.7%, p=0.028). Elderly individuals exhibited an unfavorable progression (p=0.028). However, a logistic regression analysis identified as significant variables associated with malignant transformation, the presence of epithelial dysplasia, and red lesions diagnosed as erythroplakia. CONCLUSIONS: A declining frequency of OPMDs in young adults was observed over the years, whereas in older adults, these disorders exhibited an unfavorable progression.

ABSTRACT

A global rise in the prevalence of patients with chronic kidney disease (CKD) with end-stage kidney disease (ESKD) has led to a considerable and increasing burden to health systems, patients, and society. Sodium-glucose cotransporter 2 (SGLT2) inhibitors are proven to reduce incidence of cardio-renal outcomes, including onset of ESKD. Recent post hoc analyses of SGLT2 inhibitor trials extrapolate substantial delays in the average time to ESKD over a patient's lifetime. In this article, we explore the possible real-world effects of such a delay by considering the available evidence reporting outcomes following onset of ESKD. From the patient perspective, a delay in reaching ESKD could substantially improve health-related quality of life and result in additional life years without the need for kidney replacement therapies, a target relevant to all CKD subpopulations. Furthermore, should a patient initiate dialysis at an older age as a result of CKD progression, the time spent in receipt of dialysis, and therefore associated healthcare costs, may also be reduced. A delay in progression may also lead to changes in the management of ESKD, such as increased election of conservative care in preference to dialysis, particularly in elderly populations. For younger patients with CKD, those who reach ESKD while employed face considerable work impairment and productivity loss, as may families and care partners of working age. Therefore, a delay to the onset of ESKD will reduce the proportion of their working lives affected by productivity losses or unemployment due to medical reasons. In conclusion, optimised treatment of CKD may lead to a shift in treatment options, but proper and timely implementation is essential for the realisation of improved outcomes.
