INTRODUCTION: Perianal fistulizing Crohn's disease (CD) is a phenotype with a poor prognosis. There are no studies in our country. Our objective is to determine the clinical, sociodemographic and treatment characteristics of perianal fistulizing CD in a Colombian multicenter registry. MATERIALS AND METHODS: A retrospective, multicenter observational study was carried out, with prospective data collection, in the main reference centers for inflammatory Bowel disease (IBD) in the country. Continuous variables were expressed as medians and interquartile ranges. The categorical outcome variables were compared by the Chi-square test. RESULTS: Sixty-five patients with perianal fistulizing CD were documented, with a median age of appearance of perianal fistula of 31.0 years (range: 24-42), predominantly in men (61.5%; H:M ratio: 1.4:1). Complex perianal fistulas were more frequent than simple ones (75.35 vs. 24.6%). Regarding medical treatment, 66.2% of the patients received antibiotics, 64.6% steroids, 78.5% biological therapy, 47.7% non-cutting setons, and 46.2% required surgical management, other than seton placement. Only 29.2% achieved complete remission of the fistula, and 9.2% of the patients ended up in a definitive colostomy. CD patients with complex fistulas received more biological therapy, compared to CD patients with simple fistulas (84.8 vs. 56.3%; P: 0.038). CONCLUSIONS: Perianal fistulizing CD has a poor prognosis in our setting, only 3 out of 10 patients achieve complete remission despite treatment. A multidisciplinary management is essential for the comprehensive management of this difficult pathology.

Abstract Introduction: Inflammatory bowel disease (IBD) is an immune-mediated disease whose incidence in Latin America has increased in recent years. Aim: To analyze the demographic and clinical characteristics of patients with IBD treated in a university hospital and present the epidemiological data compared to other centers in Colombia. Patients and methods: Descriptive study of patients with IBD (1996-2019) at the Hospital Universitario Fundación Santa Fe de Bogotá. Analysis of data from centers in Medellín, Cali, Bogotá, and Cartagena. Results: Of 386 patients, 277 presented with ulcerative colitis (UC), 102 with Crohn's disease (CD), and seven with unclassifiable colitis. IBD was more frequent in women (53 %). Mortality was less than 1 %. The involvement of UC was mainly pancolitis (42.6 %). The greater the extent of the disease, the higher the hospitalization and surgery rates (OR 3.70, P < 0.01). Thirteen percent of patients with UC received biologics. Compromise due to CD was mainly ileocolonic (43.6 %) and ileal (43.6 %). The predominant clinical pattern of CD was structuring (50%). Forty-five percent received biologicals and 56 % surgery. Primary sclerosing cholangitis (PSC) was found in 4 % of patients (n = 15). Two patients with PSC developed colorectal cancer (OR 4.18; p 0.008), while 13 patients with UC developed colon cancer and seven dysplastic changes. Three patients with CD developed colon cancer. Conclusions: The results were compared to other reference centers. We found similar trends in the clinical behavior and treatment of IBD, with higher hospitalization and surgery rates in our cases.


Resumen Introducción: la enfermedad inflamatoria intestinal (EII) es una enfermedad inmunomediada, cuya incidencia en Latinoamérica ha aumentado en los últimos años. Objetivo: analizar las características demográficas y clínicas de los pacientes con EII tratados en un hospital universitario y presentar los datos epidemiológicos con respecto a otros centros en Colombia. Pacientes y métodos: estudio descriptivo de pacientes con EII (1996-2019) en el Hospital Universitario Fundación Santa Fe de Bogotá. Análisis de datos de centros de Medellín, Cali, Bogotá y Cartagena. Resultados: de 386 pacientes, 277 presentaron colitis ulcerativa (CU), 102 enfermedad de Crohn (EC) y 7 colitis no clasificable. La EII fue más frecuente en mujeres (53 %). La mortalidad fue menor de 1 %. El compromiso de la CU fue principalmente la pancolitis (42,6 %). Entre mayor la extensión de la enfermedad, más alta fue la tasa de hospitalización y cirugías (OR 3,70; p < 0,01). El 13 % de los pacientes con CU recibió biológicos. El compromiso por la EC fue principalmente ileocolónico (43,6 %) e ileal (43,6 %). El patrón clínico predominante de la EC fue estenosante (50%). El 45 % recibió biológicos y 56% cirugía. La colangitis esclerosante primaria (CEP) se encontró en 4 % de los pacientes (n = 15). Dos pacientes con CEP desarrollaron cáncer colorrectal (OR 4,18; p 0,008), mientras que 13 pacientes con CU desarrollaron cáncer de colon y 7 cambios displásicos. 3 pacientes con EC desarrollaron cáncer de colon. Conclusiones: se compararon los resultados en relación con otros centros de referencia. Encontramos tendencias similares en el comportamiento clínico y en el tratamiento de la EII, con mayores tazas de hospitalizaciones y cirugías en nuestros casos.

ABSTRACT

Celiac disease (CD) is an autoimmune enteropathy induced by the ingestion of gluten from wheat, barley, and rye in genetically susceptible individuals. The global prevalence of CD is 1.4%. However, most of the prevalence studies have been conducted in Caucasian populations; few studies have been performed in Latin America. The aim of this study is to determine the seroprevalence of auto-antibodies used as markers for CD in a Colombian cohort. In this cross-sectional study, the serum samples from Colombian donors of the National Red Cross Blood Bank were collected between June and September 2017 in Bogotá, Colombia. All sera were tested for IgA antitissue transglutaminase (TTG) by enzyme-linked immunosorbent assay. Seropositive sera were tested for IgA antiendomysium (EMA) using indirect immunofluorescence assay. The ancestral genetic composition was determined in donor samples with antibody assay reactivity. Those with two seroreactive assays were typed for HLA class II DQ2 and DQ8. In total, 228 blood donors participated in the study. Among them, 113 were females (49.56%) with an average age of 31.63 years (SD ± 12.99); males had an average of 34.71 years (SD ± 13.01). Only 3 (1.31%) donors reported chronic diarrhea and nonintentional weight loss; 11 (4.82%) had a family history of CD. For the serological assays, 11 donors (4.82%) were seroreactive to IgA anti-TTG: 3 had high reactivity and 8 had low reactivity. Of those seroreactive to IgA anti-TTG, 3 (1.32%) were also seroreactive to anti-EMA, and they were typed as HLA-DQ8 or HLA-DQ2. The baseline ancestral percentage of the seroreactive donors was higher for European and Native American than for African genes. The seroprevalence for anti-TTG and anti-EMA with the presence of HLA-DQ8 and HLA-DQ2 was 1.32%. Additionally, 4.82% donor participants were reactive only for anti-TTG. Compared with other studies, our findings suggest that Colombia has a high prevalence of CD markers.

Resumen Introducción: el cáncer colorrectal es un problema de salud pública; sin embargo, la detección temprana reduce su morbimortalidad. La colonoscopia es el procedimiento de elección para detectar lesiones premalignas y el éxito depende de una limpieza adecuada. El objetivo es evaluar el desempeño de dos preparaciones de bajo volumen empleados en un hospital de alto nivel. Materiales y métodos: estudio prospectivo en adultos que asistieran a colonoscopia en la Fundación Santa Fe de Bogotá, Colombia. Las preparaciones se evaluaron con la escala de Boston, con puntaje ≥ 6 puntos para una limpieza adecuada. Se realizó un análisis de regresión logística para establecer la efectividad de los medicamentos con un cálculo de no inferioridad del 3 %-5 %. Resultados: 598 pacientes fueron evaluados. El 49 % (293) fue expuesto al picosulfato de sodio/citrato de magnesio y el 51 % (305) fue expuesto al sulfato de sodio/potasio/magnesio. Con un promedio de Boston de 6,98 ± 1,86 (78 % con puntaje de Boston ≥ 6) y 7,39 ± 1,83 (83 %), respectivamente (p = 0,649). Según el análisis de la presencia y frecuencia de síntomas no deseados, el picosulfato fue mejor tolerado (p < 0,001). Conclusiones: los estudios de preparación intestinal en pacientes de un escenario real son muy escasos. Los medicamentos de bajo volumen obtuvieron una efectividad global y por segmento de colon similar, confirmando la no-inferioridad; el picosulfato de sodio/citrato de magnesio fue mejor tolerado. Un estudio de costo-efectividad podría definir esto según las necesidades de la población de estudio.


Abstract Introduction: Colorectal cancer is a public health problem; however, early detection reduces morbidity and mortality. Colonoscopy is the procedure of choice for detecting precancerous lesions, and success depends on proper bowel cleansing. Objective: To evaluate the performance of two low-volume agents used in a high-level hospital. Materials and methods: Prospective study in adults who underwent colonoscopy at the Fundación Santa Fe in Bogotá, Colombia. Preparations were evaluated using the Boston Bowel Preparation Scale. A score ≥6 points indicated adequate preparation. A logistic regression analysis was carried out to establish the effectiveness of the medicines with a non-inferiority ratio of 3-5%. Results: 598 patients were evaluated. 49% (293) received sodium picosulfate/magnesium citrate and 51% (305) received sodium sulfate/potassium/magnesium, with an average Boston score of 6.98±1.86 (78% Boston ≥6) and 7.39±1.83 (83%), respectively (p=0.649). According to the analysis of the presence and frequency of unwanted symptoms, picosulfate was better tolerated (p < 0.001). Conclusions: Bowel preparation studies in patients from a real-life scenario are scarce. Low-volume agents had similar overall and segmental effectiveness in the colon, confirming non-inferiority; sodium picosulfate/magnesium citrate was better tolerated. A cost-effectiveness study could establish the best option according to the needs of the study population.

Resumen Introducción: el esófago de Barrett es un trastorno en el que ocurre un cambio del epitelio escamoso estratificado del esófago por uno columnar especializado, lo cual se da como consecuencia del reflujo gastroesofágico crónico. En Colombia no se conoce la prevalencia actual de esta patología, ni se ha caracterizado a la población que la padece. El presente estudio tiene como objetivo conocer cuáles son las características demográficas principales de la población diagnosticada con esófago de Barrett en dos instituciones médicas de Bogotá. Material y métodos: se realizó un estudio de corte transversal multicéntrico, en el cual se evaluaron los reportes de endoscopias y de histopatología de 3000 pacientes que asistieron a estas instituciones por cualquier indicación. A partir de estos reportes se tomaron los datos requeridos. Asimismo, se realizó un análisis estadístico descriptivo de dichos datos. Resultados: la prevalencia del esófago de Barrett en la muestra es del 0,73 %. Se observó, además, que la correlación endoscópico-patológica es baja (28,5 %). De los casos diagnosticados, el rango de edad más frecuente se ubica entre los 60 y 80 años, con una edad promedio de 65,5 años. Asimismo, existe una predominancia de esta patología en el sexo femenino (63,6 %), en personas con un índice de masa corporal (IMC) >25 kg/m² y en aquellas con antecedentes de tabaquismo, sin historial de consumo de alcohol. En la mayoría de pacientes, se realizó la endoscopia por síntomas de reflujo gastroesofágico (50 %). La longitud del segmento observado no fue reportada en una gran cantidad de endoscopias. Conclusiones: en las instituciones analizadas, el esófago de Barrett es una patología de muy baja prevalencia y predominante en mujeres de edad avanzada con síntomas de reflujo gastroesofágico, sobrepeso y antecedente de tabaquismo.


Abstract Introduction: Barrett's esophagus occurs when the stratified squamous epithelium of the esophagus changes to a specialized columnar epithelium as a result of chronic gastroesophageal reflux. Its current prevalence in Colombia is unknown and the population suffering from it has not been characterized. The present study aims to determine the main demographic characteristics of the population diagnosed with Barrett's esophagus treated at two medical centers in Bogotá, Colombia. Materials and methods: A multicenter cross-sectional study was conducted to assess the endoscopy and histopathology reports of 3,000 patients who underwent this procedure for any reason. A descriptive statistical analysis of the data was performed. Results: The prevalence of Barrett's esophagus in the sample was 0.73%. The endoscopic-histology correlation was low (28.5%). Of the diagnosed cases, the most frequent age range was 60-80 years, with an average age of 65.5 years. This condition is predominant in the female sex (63.6%), in people with a BMI over 25 kg/m², with a history of smoking, and no history of alcohol consumption. Most patients underwent endoscopy for symptoms associated with gastroesophageal reflux (50%). The length of the observed segment was not reported in most endoscopies. Conclusions: In the medical centers included in this study, Barrett's esophagus is a rare pathology, found predominantly in elderly women with symptoms of gastroesophageal reflux, overweight, and with a history of smoking.

BACKGROUND: The effectiveness of Helicobacter pylori first-line treatment has decreased drastically with the rise of strains resistant to clarithromycin. Therapy failure has also been described in patients with infections by strains with dissimilar antimicrobial susceptibilities. The present study aims to estimate the prevalence of resistance and heteroresistance to clarithromycin in H. pylori isolates from antrum and corpus of Colombian patients. METHODS: The study material included 126 isolates from antrum and corpus biopsies from 63 symptomatic patients over 18 years old who had a gastric endoscopy performed on them between June 2014 to August 2016. PCR amplification and sequencing of the H. pylori 23S rDNA gene was performed to determine the presence of mutations associated with clarithromycin resistance. Random amplified polymorphic DNA analysis was implemented in cases of resistance and heteroresistance. RESULTS: The overall frequency of resistance to clarithromycin was 38.1% (24/63 patients), of which 19 patients had resistant isolates in both stomach segments (14 with A2143G mutation and 5 with A2142G mutation), and 5 patients had a heteroresistant status. The remaining 61.9% (39/63 patients) presented only susceptible isolates. DNA fingerprinting analysis showed different patterns in 4/22 paired isolates. CONCLUSIONS: The high prevalence of H. pylori clarithromycin-resistance obtained (> 15%) constitutes an alert for gastroenterologists and suggests the need for reconsideration of the current eradication regimen for H. pylori in the studied population. The data show that heteroresistance status is an additional factor to be considered in the assessment of resistance. In consequence, it is advisable to examine at least two biopsies from different gastric segments.

Resumen Objetivo: este estudio caracteriza la diversidad de los genes de virulencia cagA (gen asociado con la citotoxina A) y vacA (citotoxina vacuolizante) en pacientes colombianos para determinar posibles asociaciones entre estos 2 genes y la severidad de los hallazgos endoscópicos teniendo en cuenta todos los genotipos reportados para el gen vacA (s, m e i). Materiales y métodos: Helicobacter pylori fue detectado por cultivo y por métodos moleculares en biopsias de 62 pacientes. Los genotipos de cagA y vacA (m/i/s) se determinaron por reacción en cadena de la polimerasa (PCR) y secuenciación. Resultados: se aislaron 124 cepas de 62 pacientes; de estas, el 48,5% (n = 48) fueron vacA s2/m2/i2-cagA (-) presente en su mayoría en pacientes con gastritis folicular; mientras el 32,3% (n = 32) fueron vacA s1/m1/i1-cagA (+) presentes mayormente en pacientes con gastritis folicular, gastritis crónica y posible metaplasia. Se encontró una asociación significativa entre la presencia de cagA y el genotipo vacA s1/m1/i1 y la ausencia de cagA y el genotipo vacA s2/m2/i2 (p <0,001). No se encontró una asociación significativa entre la severidad de los hallazgos endoscópicos y el estatus cagA-vacA de las cepas. Conclusión: se encontró una baja prevalencia de cepas cagA (+), el estatus cagA-vacA no es un predictor de riesgo en la población estudiada y la presencia de infecciones heterogéneas sin tropismo sugieren la necesidad de tomar biopsias tanto del cuerpo como del antro del estómago en la práctica clínica rutinaria.


Abstract Objective: This study characterizes the diversity of cagA and vacA virulence genes in Colombian patients to determine possible associations between them and the severity of endoscopic findings. It considers all four genotypes reported for the vacA gene (s, m and i). Materials and methods: Helicobacter pylori was detected in biopsies of 62 patients through culturing and by molecular methods. Genotypes of cagA and vacA (m/i/s) were determined by PCR and sequencing. Results: One hundred twenty four strains from 62 patients were isolated. Of these, 48.5% (n = 48) were vacA s2/m2/i2 - cagA (-) which were mostly found in patients with follicular gastritis; 32.3% (n = 32) were vacA s1/m1/i1-cagA (+) which were mostly found in patients with follicular gastritis, chronic gastritis and possible metaplasia. Significant associations were found between the presence of cagA and the vacA s1/m1/i1 genotype and the absence of cagA and the vacA s2/m2/i2 genotype (p <0.001). No significant association was found between the severity of endoscopic findings and the cagA-vacA status of the strains. Conclusion: We found a low prevalence of cagA (+) strains, the cagA-vacA status is not a predictor of risk in this population. Moreover, the presence of heterogenous infections without tropism suggests a need for biopsies from both the corpus and the antrum of the stomach in routine clinical practice.

Achalasia is a motility disorder of the esophagus that might be secondary to a chronic Trypanosoma cruzi infection. Several studies have investigated esophageal achalasia in patients with Chagas disease (CD) in Latin America, but no related studies have been performed in Colombia. The goals of the present study were to determine the presence of anti-T. cruzi antibodies in patients with esophageal achalasia who visited a referral hospital in Bogotá, Colombia, and to detect the presence of the parasite and its discrete typing units (DTUs). This cross-sectional study was conducted in adult patients (18-65 years old) who were previously diagnosed with esophageal achalasia and from whom blood was drawn to assess antibodies against T. cruzi using four different serological tests. Trypanosoma cruzi DNA was detected by conventional polymerase chain reaction (cPCR) and quantitative polymerase chain reaction (qPCR). In total, 38 patients, with an average age of 46.6 years (standard deviation of ±16.2) and comprising 16 men and 22 women, were enrolled. Five (13.15%) patients were found to be positive for anti-T. cruzi antibodies by indirect immunofluorescence assay (IFA), and two patients who were negative according to IFA were reactive by both enzyme-linked immunosorbent assay and immunoblot (5.3%). Parasite DNA was detected in two of these seven patients by cPCR and in one of these by qPCR. The parasite DTU obtained was TcI. In summary, this study identified T. cruzi in Colombian patients with esophageal achalasia, indicating that digestive compromise could also be present in patients with chronic CD.

ABSTRACT

Problema: el reflujo gastroesofágico es un proceso fisiológico que en algunas personas puede tornarse patológico y que produce molestias y lesiones esofágicas y extraesofágicas que afectan la calidad de vida de los individuos que lo presentan. Actualmente no se cuenta con estadísticas sobre la frecuencia de esta condición en Colombia. Objetivo: cuantificar la prevalencia de síntomas de enfermedad por reflujo gastroesofágico (ERGE) en los adultos de cuatro ciudades principales de Colombia utilizando el cuestionario GERDQ, y explorar la asociación de variables sociodemográficas y antropométricas con esta enfermedad. Metodología: estudio de corte transversal de base poblacional. Se incluyeron 6842 personas, mayores de 18 años y menores de 80 años de edad, residentes en Bogotá, Cali, Medellín y Barranquilla. La muestra fue seleccionada por muestreo aleatorio estratificado. Se aplicó una encuesta telefónica que incluía el cuestionario GERDQ para la identificación de síntomas de reflujo en la última semana. Se calculó la prevalencia de reflujo por estratos teniendo en cuenta el punto de corte de la encuesta (8 o más puntos) y se realizaron comparaciones entre ellos utilizando la prueba de x2. Se exploró la asociación de variables sociodemográficas y antropométricas mediante análisis bivariados y modelos de regresión logística. Resultados: se obtuvieron 6842 encuestas. La prevalencia estimada de síntomas de reflujo en general fue del 11,98% (IC 95%: 11,05-12,97). En el análisis por ciudades, Barranquilla presentó la frecuencia más alta (16,22%; IC 95%: 14,58-18,01) y Bogotá, la más baja (10,75; IC 95%: 9,30-12,38). Para los síntomas evaluados con el cuestionario GERDQ, la prevalencia estimada fue: pirosis, 13,6% (IC 95%: 12,50-14,60); regurgitación, 16,9% (IC 95%: 15,74-17,99); epigastralgia, 16,67% (IC 95%: 15,54-17,80): náuseas, 11,4% (IC 95%: 10,46-12,35); dificultad para dormir por presentar pirosis o regurgitación, 8,17% (IC 95%: 7,36-8,97); y consumo de medicamentos adicionales a los formulados por el médico, 6,68% (IC 95%: 6,01-7,35). El sexo femenino, el vivir en Barranquilla o Medellín y el presentar una comorbilidad se asociaron estadísticamente con la presencia de reflujo. Conclusión: la prevalencia de síntomas de reflujo en cuatro ciudades capitales de Colombia, medida con el cuestionario GERDQ, de 11,98% (IC 95%: 11,05-12,97), es similar a la reportada en otros países de Latinoamérica, siendo las comorbilidades (particularmente hipertensión arterial HTA) el factor que más se asoció con esta condición en todos los estratos del estudio


Problem: Gastroesophageal reflux is a physiological process that can become pathological in some people. It can cause discomfort and esophageal and extra-esophageal injuries and can affect the quality of life of anyone affected by it. Currently there are no statistics on the frequency of this condition in Colombia. Objective: The objective of this study was to use the GerdQ questionnaire to quantify the prevalence of gastroesophageal reflux disease in adults in four major cities of Colombia and to explore the association of sociodemographic and anthropometric variables with this disease. Methodology: This was a cross-sectional population-based study of 6,842 people between the ages of 18 years and 80 in Bogotá, Cali, Medellín and Barranquilla. The sample was selected by stratified random sampling. A telephone survey was conducted that included the GERD Q questionnaire for the identification of reflux symptoms within the week prior to an interview. The prevalence of reflux by socioeconomic strata was calculated using a cut-off point of 8 or more points. Comparisons were made among strata using the χ2 test. Associations of sociodemographic and anthropometric variables were explored through bivariate analysis and logistic regression models. Results: We obtained 6,842 surveys. The overall estimated prevalence of reflux was 11.98% (95% CI = 11.05%-12.97%). The city of Barranquilla presented the highest frequency of 16.22% (95% CI = 14.58%-18.01%) while Bogotá had the lowest of 10.75% (95% CI = 9.30%-12.38%). For symptoms evaluated with GERD-Q, the estimated prevalences were: heartburn 13.6% (95% CI = 12.50% - 14.60%), regurgitation 16.9% (95% CI = 15, 74% - 17.99%), epigastralgia 16.67% (95% CI = 15.54% -17.80%), nausea 11.4% (95% CI = 10.46% - 12.35%), difficulty sleeping due to heartburn or regurgitation 8.17% (95% CI = 7.36% -8.97%) and consumption of medications additional to those formulated by the physician 6.68% (95% CI = 6, 01% - 7.35%). Women living in Barranquilla or Medellín, had statistically significant levels of comorbidities associated with reflux. Conclusion: The prevalence of reflux in four important Colombian cities measured with the GerdQ questionnaire was 11.98% (95% CI = 11.05-12.97) which is similar to prevalences reported in other Latin American countries. Comorbidities (particularly hypertension) are the factor that was most frequently associated with this condition in all strata of the study.

Introducción. El esófago de Barrett es una metaplasia intestinal especializada, cuyo diagnóstico se basa en la identificación de la mucosa de color salmón en el esófago, la apropiada ejecución de las biopsias y la confirmación histológica. Objetivos. Determinar la eficacia de la inspección endoscópica en comparación con los resultados de histopatología y la precisión de la descripción visual del esófago de Barrett en Colombia en comparación con otros países. Materiales y métodos. Se trata de un estudio internacional multicéntrico, con revisión prospectiva de bases de datos. Se evaluaron los hallazgos endoscópicos de 347 pacientes con sospecha de esófago de Barrett y se compararon con los reportes de histopatología. Además, se determinó la frecuencia de la adecuada descripción de los hallazgos endoscópicos y el apropiado protocolo de biopsias en tres países. Resultados. La presencia de esófago de Barrett se confirmó en 133 de 347 pacientes (38 %). Hubo metaplasia intestinal en 118 de 133 pacientes (89 %) y, displasia de bajo grado, en 15 (11 %). La morfología del esófago de Barrett se describió en 262 de 347 pacientes (76 %) y, la longitud, en 205 de ellos (59 %). La media de biopsias fue de 3,6±2,2. Conclusiones. Según los resultados, la inspección visual para el diagnóstico del esófago de Barrett es poco eficaz y se hizo una adecuada descripción de los hallazgos endoscópicos en cerca de la mitad de los pacientes con sospecha de esófago de Barrett en Colombia. Este estudio sugiere la necesidad de hacerle seguimiento a las guías de la American Gastroenterological Association para optimizar el diagnóstico del esófago de Barrett.


Background: Barrett esophagus is a specialized intestinal metaplasia. The diagnosis is based on visualization of a salmon colored mucosa, the properly executed biopsies and the pathology confirmation. Aims: The aims of this study were to determine the efficacy of the endoscopic inspection as compared with the histopathology report and the precision of the visual description of the Barrett's esophagus in Colombia in comparison with other nations. Methods: This is an international multicentric study with prospective review of the data bases. We evaluated the endoscopic findings of 347 patients in whom the endoscopist thought that Barrett´s esophagus was present, and compared them to the pathology results. In addition, we assessed the frequency of an adequate description of the endoscopic findings and the observance of a due biopsy protocol in three countries. Results: The presence of Barrett's esophagus was confirmed in 133 of the 347 patients (38%). Metaplasia was present in 118 of the 133 patients (89%) and low grade dysplasia in 15 patients (11%). The morphology of Barrett´s esophagus was described in 262 of the 347 patients (76%), and the length was reported in 205 of these patients (59%). An average of 3.6±2.2 biopsies were taken. Conclusions: According to our results, visual inspection for the diagnosis of Barrett's esophagus has a low efficacy, and a precise description of the Barretts esophagus was not done in around a half of the patients in Colombia. This study suggests the need for more strict observation of the American Gastroenterological Association and the American College of Gastroenterology guidelines by physicians in order to optimize the diagnosis of Barretts esophagus.

ABSTRACT

Presentamos el caso de una mujer de 78 años, que presentaba importante pérdida de peso y su endoscopia digestiva alta demostró pliegues gástricos gigantes. Se realizaron biopsias con asa de polipectomía en busca del diagnóstico y posible enfermedad neoplásica. La histopatología documentó gastropatía hipertrófica. La ecoendoscopia gástrica demostró una pared muy gruesa y aunque se sospechó neoplasia no podía ser conclusiva, solamente después de realizada la gastrectomía total se diagnosticó linitis plástica. Presentamos la revisión de pliegues gástricos gigantes así como las causas y clasificación de la gastropatía hipertrófica.


We present 78 year old Colombian lady who had lost weight and had giant gastric folds in her upper endoscopy. Intensive work up was done including extra large biopsies done with polipectomy snares. The pathology diagnosed hypertrophic gastropathy. Endoscopic ultrasound of the stomach diagnosed a very thick gastric wall. Only after total gastrectomy was done, Linitis Plastica was diagnosed. We review giant gastric folds as well as causes and classification of the hypertrophic gastropathy.

ABSTRACT

En la actualidad, la terapia endoscópica ha revolucionado el tratamiento del sangrado digestivo por úlcera péptica. El presente estudio tiene como objetivo demostrar la efectividad del tratamiento endoscópico con sustancias esclerosantes, en hemmorragia digestiva alta por úlcera péptica. Se realizó un estudio de casos y controles históricos con 96 pacientes en cada grupo; al grupo de los casos se les administró una solución de adrenalina alcohol absoluto. De acuerdo con la lacalización y las características de la úlcera, el 96 por ciento de los pacientes tratados con escleroterapia mostraron curación comparados con el 77 por ciento en el grupo control. La respuesta al tratamiento de la hemorragia digestiva alta secundaria a úlcera péptica, en mejor cuando se utiliza terapia endoscópica que cuando no se emplea dicho método.
