ABSTRACT

BACKGROUND@#Clinical outcomes of undifferentiated arthritis (UA) are diverse, and only 40% of patients with UA develop rheumatoid arthritis (RA) after 3 years. Discovering predictive markers at disease onset for further intervention is critical. Therefore, our objective was to analyze the clinical outcomes of UA and ascertain the predictors for RA development.@*METHODS@#We performed a prospective, multi-center study from January 2013 to October 2016 among Chinese patients diagnosed with UA in 22 tertiary-care hospitals. Clinical and serological parameters were obtained at recruitment. Follow-up was undertaken in all patients every 12 weeks for 2 years. Predictive factors of disease progression were identified using multivariate Cox proportional hazards regression.@*RESULTS@#A total of 234 patients were recruited in this study, and 17 (7.3%) patients failed to follow up during the study. Among the 217 patients who completed the study, 83 (38.2%) patients went into remission. UA patients who developed RA had a higher rheumatoid factor (RF)-positivity (42.9% vs. 16.8%, χ = 8.228, P = 0.008), anti-cyclic citrullinated peptide (CCP) antibody-positivity (66.7% vs. 10.7%, χ = 43.897, P < 0.001), and double-positivity rate of RF and anti-CCP antibody (38.1% vs. 4.1%, χ = 32.131, P < 0.001) than those who did not. Anti-CCP antibody but not RF was an independent predictor for RA development (hazard ratio 18.017, 95% confidence interval: 5.803-55.938; P < 0.001).@*CONCLUSION@#As an independent predictor of RA, anti-CCP antibody should be tested at disease onset in all patients with UA.

ABSTRACT

Objective To analyze the factors influencing the occurrence of macrosomia so as to provide a theoretical basis for the prevention of macrosomia.Methods The data were selected from the findings of Survey on Prevalence and Risk Factors for Birth Defects in Shaanxi Province.This study intended to select data of macrosomia in Xianyang City and its influencing factors for statistical analysis.The rates were compared by χ2 ;the influencing factors were analyzed by univariate and multivariate Logistic regression.Results The incidence of fetal macrosomia among single live birth neonates in Xianyang was 5.2%.The incidence of macrosomia was 6.5%in males and 4.2% in females,with a significant difference (P <0.001).From 2010 to 2013 the annual incidence rate of macrosomia was 5.4%,6.2%,5.9% and 4.4%,with no significant difference (P =0.151).The incidence rate of macrosomia in urban and rural areas was 6.3% and 5.5%,respectively,without significant difference (P =0.556).The incidence of macrosomia was 3.2% in Binxian County,5.6% in Changwu County,6.7% in Liquan County,5.4% in Qianxian County,8.0% in Wugong County,and 4.2% in Xunyi County,with significant differences (P <0.001).Logistic regression analysis showed that the occurrence of macrosomia was related to the baby's sex,mother's age,the times of pregancy,the time of antenatal care,alcohol intake during pregnancy and father's occupation.Conclusion The incidence of macrosomia of Xianyang from 2010 to 2013 was close to the national average.The sex of baby,mother's age,the times of pregnancy,the times of antenatal care,drinking history during pregnancy and father's occupation may affect the occurrence of macrosomia.

ABSTRACT

Studies on the whole herb of Urtica dioica L. grown in Nyingchi area, China's Tibet Autonomous Region, resulted in the isolated of nine compounds: beta-sitosterol, trans-ferulic acid, dotriacotane, erucic acid, ursolic acid, scopoletin, rutin, quercetin and p-hydroxylbenzalcohol. Dotriacotane, erucic acid, scopoletin, rutin and p-hydroxylbenzalcohol were obtained from Urtica L. for the first time. Their structures were confirmed by modem spectral analysis (NMR, MS, etc).

ABSTRACT

As a part of our systematic investigation of chemical constituents of Polyporaceous family fungi (Basidiomycetes), we studied the fungus of Fomes cajanderi P. Karst collected from Jiling province, China in 1997, and isolated 15 compounds, of which, compound 5 is a naturally occurring new metabolite, while 9, 11 and 12 are new derivatives. Their structures were identified by means of MS, NMR spectra and X-ray crystallographic analysis.

ABSTRACT

@#ObjectiveTo observe the effect of 99Tc-methylene diphosphate (99Tc-MDP, Yunke) combined with rehabilitation on ankylosing spondylitis (AS).Methods118 AS patients were randomly divided into the treatment group (n=58) and control group (n=60). The treatment group was treated with meloxicam, sulfasalazine and vein injecting Yunke combined rehabilitation. The control group was treated only with meloxicam and sulfasalazine. The changes of both clinical and experimental indexes of two groups before and after treatment were analyzed.ResultsAfter two therapeutic courses, a much better result was obtained in the treatment group compared with the control group at the respects of releasing arthralgia, shortening morning stiffness, improving range of spinal motion, and decreasing the levels of ESR, CRP, PLT and IgM, ect (P<0.05~0.01) without obvious side effects.ConclusionThe effect of Yunke combined with rehabilitation on AS is evident.

ABSTRACT

Bioactive natural products of novel structural types play a highly significant role in the drug discovery and development process. Of the 1,031 new chemical entities covering all diseases in the years 1981-2002, 48% were natural product mimics, 5% were the direct utility of natural products as sources of novel structures. Up to now, the main challenges are more efficient and rapid processes for discovery and isolation of bioactive natural products of novel structural types. Therefore, new strategies and ideas for chemical study on bioactive natural products of novel structural types should be applied. In order to discover a large number of bioactive natural products of novel structural types from naturally occurring sources and elucidate the intrinsic value of natural products as leads to new structures with different activities, new, more efficient and rapid processes for study on natural products will be established by use of modern chromatographies, techniques on line identification of natural products and screening methods based on molecular targets.

ABSTRACT

AIM: To provide basic data for the synthesis of new sinomenine derivatives. METHODS: The C ring in sinomenine was modified. RESULTS: Seven compounds were prepared and screened for anti-inflammatory and analgesic activities. Compounds 2 and 5 showed better activities. CONCLUSION: Modification of the C ring in sinomenine should be worthy to be studied further.

ABSTRACT

Three new naturally occurring bicyclic alkaloids, jenamidines A (1), B (2) and C (3), were discovered and isolated from the culture broth of Streptomyces sp. (strain HKI0297) via the chemical screening approach. Fermentation, isolation, structure and biological activities of these three new secondary metabolites are reported. The jenamidines have an unusual octahydro-pyrido[1,2-a]pyrimidine skeleton. Jenamidine A (1) shows antiproliferative effects against the chronic myeloid leukaemic cell line K-562. In addition, the new tricyclic sesquiterpenoid, africantriol (4) was isolated from the same strain.

ABSTRACT

Chemical screening with extracts of Actinomycetes strains resulted in the detection, isolation and structure elucidation of five new isoflavone glycosides from Streptomyces sp. (GT 51173): 5-O-alpha-L-rhamnopyranosyl genestein (genestein G1, 1), 5-O-alpha-L-rhamnopyranosyl-7-O-[2-O-(methyl)-alpha-L-rhamnopyranosyl] genestein (genestein G2, 2), 7,4'-O-di-[2-O-(methyl)-alpha-L-rhamnopyranosyl] daidzein (daidzein G1, 3), 6-O-methyl-7-O-[2-O-(methyl)-alpha-L-rhamnopyranosyl] daidzein (daidzein G2, 4) and 7-O-[2-O-(methyl)-alpha-L-rhamnopyranosyl] daidzein (daidzein G3, 5). As a striking structural feature, 2-5 bear one or two alpha-L-rhamnopyranosyl sugar moieties with methoxy groups in an axial position at C-2.

ABSTRACT

Investigation of the neutral ether extracts of the fungus Fomes cajanderi led to the isolation of three novel ketal lactones named fomlactones A (1), B (2), and C (3). These compounds were identified by analysis of IR, HRMS, CD, and 1D and 2D NMR data. The structure of fomlactone C (3) was confirmed by X-ray diffraction analysis.
