ABSTRACT

Objetivo principal: Identificar los modos en que las enfermeras quirúrgicas adquieren su conocimiento y competencia. Metodología: Estudio cualitativo descriptivo de carácter exploratorio, mediante entrevistas semiestructuradas a seis enfermeras quirúrgicas. Los datos obtenidos fueron codificados y analizados con el software MAXQDA v.18.1. Resultados principales: La práctica, formación, experiencia, intuición y compañerismo son conceptos relacionados con la adquisición de conocimientos de las enfermeras. El alcance del desarrollo de cada uno de ellos es diferente en relación con el nivel competencial en el que se encuentren las enfermeras, competente, eficiente o experta. Conclusión principal: El proceso de adquisición de conocimientos prácticos en el área quirúrgica es un proceso dinámico y flexible, que se nutre de varias fuentes de conocimiento. Destaca el compañerismo como elemento fundamental en la obtención de información y ayuda en la resolución de problemas, proporcionando la mayor satisfacción. (AU)


Objective: To identify the ways surgical Nurses acquire their knowledge and competence. Methods: Descriptive qualitative study of an exploratory nature, through semi-structured interviews with six surgical nurses. The data gathered were codified and analyzed using MAXQDA v.18.1 software. Results: The practice, training, experience, intuition and fellowship are concepts related to the acquisition of knowledge of the Nurses. The scope of development of each of them is different in relation to the level of competence, competent, efficient or expert. Conclusions: The protocol of acquiring practical knowledge in the surgical block is a dynamical and flexible process, that relies on several sources of knowledge. Fellowship stands out as a key factor of obtaining information, solving problems, and providing the greatest satisfaction. (AU)

ABSTRACT

RESUMEN Antecedentes: la existencia de la pandemia infectocontagiosa COVID-19 puede afectar a los equipos quirúrgicos y pacientes. Objetivo: describir los cambios introducidos en la estructura y los procesos de una planta quirúrgica a fin de adaptarla a la atención segura de pacientes positivos y sospechosos, así como los resultados iniciales de su implementación. Material y métodos: se realizó un estudio prospectivo, descriptivo, observacional entre el 1° de abril y el 31 de mayo de 2020. Fueron registrados los cambios estructurales y en los procesos de funciona miento adaptados a la atención de pacientes sospechosos y COVID-19 positivos, así como las activida des desarrolladas en dicha área. Resultados: se registró una disminución en el número de cirugías programadas y de urgencia en el período. Entre 173 cirugías de urgencia, hubo 17 pacientes sospechosos (9,8%) y 3 positivos (1,7%), confirmados por la prueba de PCR. No hubo pacientes con resultados ni sospechosos ni confirmados en 136 cirugías programadas. La adhesión al cumplimiento de la lista de verificación fue del 100%. No se registraron contagios entre el personal actuante. Conclusiones: los cambios implementados en la planta quirúrgica permitieron la atención adecuada de pacientes tanto sospechosos como confirmados durante el período, con completa adhesión a las recomendaciones y disminución en el riesgo de transmisión de la enfermedad para dar seguridad a los pacientes y al equipo de salud.


ABSTRACT Background: COVID-19 pandemic may affect the surgical teams and patients. Objective: The aim of this report was to describe the changes introduced in the structure and proces ses of a surgical facility for the safe care of suspected and positive COVID-19 patients, and to describe the initial results of their implementation. Material and methods: We conducted a prospective, descriptive and observational study between April 1 and May 31, 2020. The structural changes and the modifications introduced in the functioning processes within the surgical area of a university hospital adapted to the care of suspected and positi ve COVID-19 patients, and the activities developed in such area were documented. Results: There was a reduction in the number of scheduled and emergency surgeries performed du ring the study period. Of the 173 emergency surgeries, 17 (9.8%) were suspected cases and 3 (1.7%) resulted positive COVID-19 patients confirmed by PCR tests. None of the 136 patients undergoing scheduled surgeries were suspected or confirmed cases. Compliance with the checklist was 100%. There were no infections among the personnel working in the facility. Conclusions: The changes implemented in the surgical facility allowed for adequate care of suspected and confirmed COVID-19 patients during the period, with complete adherence to recommendations and reduced risk of disease transmission in order to provide safety to patients and the health care team.

ABSTRACT

RESUMEN Introducción: El personal que labora en el área quirúrgica de un hospital, está expuesto a riesgos biológicos que requieren más que un tratamiento médico. Para la prevención, además, de ocuparse de la integridad del paciente, se le ha agregado la de proteger la salud del equipo quirúrgico, susceptibles a contaminarse con objetos infectados. Estos aspectos no son de estricto cumplimiento por el personal que labora en los salones o salas quirúrgicas de hospitales y centros de salud. Objetivo: Realizar un análisis de riesgo biológico en el área quirúrgica de una instalación de salud. Métodos: Se realizó el análisis de riesgo, que consistió en tres procesos generales: evaluación del riesgo, gestión del riesgo y comunicación del riesgo. Resultados: Se identificaron y caracterizaron los peligros de contaminación a los que pueden estar sometidos los trabajadores. Se identificaron las vulnerabilidades presentes, empleándose para ello, una lista de chequeo diseñada, teniéndose en cuenta los principios de la bioseguridad y, se evaluaron los riesgos biológicos empleándose una matriz de estimación del riesgo (posibilidad por consecuencia). Conclusiones: Se discuten medidas aplicables para la gestión de la bioseguridad en áreas quirúrgicas de instituciones de salud cubanas(AU)


ABSTRACT Introduction: The personnel working in the surgical area of a hospital are exposed to biological risks requiring more than medical treatment. In view of prevention, apart from taking care of the integrity of the patient, they have been assigned protecting the health of the surgical team, which are susceptible to becoming contaminated with infected objects. These aspects are not strictly complied with by the personnel working in the operating or surgical rooms of hospitals and health centers. Objective: To assess the biological risk in the surgical area of a health facility. Methods: The risk analysis was carried out, which consisted of three general processes: risk assessment, risk management, and risk communication. Results: The contamination hazards to which workers may be subjected were identified and characterized. The existing vulnerabilities were identified using a checklist designed taking into account the biosecurity principles. The biological risks were evaluated, using a risk estimation matrix (possibility by consequence). Conclusions: Measures are discussed that can be applied for the management of biosecurity in surgical areas of Cuban health institutions(AU)

ABSTRACT

La comunicación efectiva dentro de las organizaciones es uno de los factores más importantes para lograr un trabajo positivo y eficaz. Se realizó una investigación cuyo objetivo fue identificar y describir las herramientas de comunicación en el área de Quirófano Central del Hospital Italiano de Buenos Aires y las distintas perspectivas de los instrumentadores quirúrgicos respecto de su utilización. Métodos: se realizó un estudio de corte transversal con un componente de observación participativa de los medios de comunicación y una encuesta a los instrumentadores quirúrgicos de la institución. Resultados: se identificaron ocho tipos de herramientas de comunicación en el área quirúrgica. El correo electrónico (e-mail) como herramienta de comunicación es muy utilizado según los instrumentadores quirúrgicos, pero estos sugirieron otras herramientas más directas, como reuniones y capacitaciones solas o en combinación para determinados tipos de información. Conclusiones: los instrumentadores quirúrgicos utilizan una amplia gama de medios de comunicación en el área quirúrgica. La distribución de preferencias según el tipo de información indica que la elección de estos medios debería ser personalizada. (AU)


Effective communication within organizations is one of the most important factors to achieve a positive and effective work. An investigation was carried out and its objective was to identify and describe the communication tools in the surgical area of the Hospital Italiano de Buenos Aires and the different perspectives of the surgical nurses regarding its use. Methods: a cross-sectional study was carried out with a component of participative observation of the communication tools and a survey of the surgical nurses of the institution. Results: eight types of communication tools were identified in the surgical area. The implementation of email as a communication tool is widely used by surgical nurses, but they suggested other more direct tools such as meetings and training sessions alone or in combination for certain types of information. Conclusions: Surgical nurses use a wide range of communication tools in the surgical area. The distribution of preferences according to the type of information indicates that the choice of these tools should be personalized. (AU)

INTRODUCTION: There is an increasing and more complex demand for sedation for procedures out of the operating room. For different reasons, nowadays the administration of sedation varies considerably. We believe that a patient safety approach rather an approach out of corporate or economic interests is desirable. METHOD: We created a working group of experts within the Spanish Anaesthesia and Reanimation Incident Reporting System (SENSAR) to prepare a series of recommendations through a non-systematic review. These recommendations were validated by an expert panel of 31 anaesthesiologists through two rounds of an adaptation of the Delphi Method where more than 70% agreement was required. RESULTS: The resulting recommendations include previous evaluation, material and staffing needs for sedation for procedures, post-sedation recommendations and activity and quality control advice. CONCLUSION: We present patient centred recommendations for the safe use of sedation for out of the operating room procedures from the point of view of the professionals with the most experience in its administration. We believe that these can be used as a guide to reduce variability and increase patient safety in the organisation of healthcare.
