ABSTRACT

BACKGROUND: Patients should be closely monitored during procedures under sedation outside the operating room, but it is unclear which type of monitoring is best. We investigated the efficacy and safety of BIS monitoring vs conventional monitoring for sedation during colonoscopy. METHODS: We performed a double-blind clinical trial in 180 patients undergoing elective colonoscopy. Patients were randomized to 1) the BIS group or 2) a control group, in which sedation was monitored with a BIS monitor or the Ramsay Sedation Score, respectively. The primary outcome was the rate of sedation-induced adverse events in both groups. Secondary outcomes were the characteristics of patients who developed adverse events, and time during colonoscopy when these events occurred, propofol and remifentanil dosage, and patient satisfaction. RESULTS: Univariate analysis showed fewer cardiopulmonary complications in the BIS group (41.11% vs 57.78% in controls; p = 0.02). Multivariate analysis found a significantly higher risk of adverse events in older patients (95% CI, 1.013-1.091; p = 0.0087) and in men (95% CI, 1.129-7.668; p = 0.0272). These events were observed at the hepatic flexure. No significant differences between propofol or remifentanil dosage, use of rescue medication, and patient satisfaction were observed between groups. CONCLUSIONS: Our data suggest that BIS monitoring during sedation in scheduled colonoscopies reduces adverse respiratory events. Although its routine use in sedation does not appear to be warranted, clinicians should take steps to identify patients with a higher risk of complications who might benefit from this type of monitoring.

Introducción y objetivos: La disfunción miocárdica contribuye a la mortalidad precoz (24-72 horas) de los supervivientes de parada cardiaca (PC). Actualmente, la decisión de implantar un dispositivo de soporte circulatorio en este contexto se toma con información limitada acerca del potencial de recuperación neurológica (PRN) del paciente, lo que en muchas ocasiones termina en infratratamiento. Por tanto, requerimos de herramientas accesibles y fiables que añadan información sobre el PRN y ayuden a establecer planes individualizados de escalada terapéutica. Métodos: Se recogieron valores de índice biespectral (BIS) y tasa de supresión (TS) en supervivientes de una PC sometidos a control de la temperatura corporal. La función neurológica se evaluó con la escala Cerebral Performance Category (CPC). Resultados: Se incluyeron 340 pacientes. En la primera evaluación neurológica completa, 211 (62,1%) alcanzaron buen pronóstico (CPC 1-2). Los valores de BIS fueron significativamente mayores y los de TS menores, en pacientes con CPC 1-2. Un BIS promedio> 26 en las primeras 12 horas predijo buena evolución neurológica (sensibilidad 89,5%; especificidad 75,8%; AUC=0,869), mientras que una TS promedio> 24 en las primeras 12 horas predijo mala evolución o CPC 3-5 (sensibilidad 91,5%; especificidad 81,8%; AUC=0,906). Los valores horarios de BIS/TS mostraron buena capacidad predictiva (AUC> 0,85) desde la 2.a hora para TS y 4.a para BIS. Conclusiones: El BIS/TS permiten estimar el PRN tras una PC. Este hallazgo puede contribuir a crear conciencia con respecto a evitar la limitación de escalada terapéutica en pacientes potencialmente recuperables.(AU)

At the first full neurological evaluation, 211 patients (62.1%) achieved good outcome or CPC 1-2. Mean BIS values were significantly higher and median SR lower in patients with CPC 1-2. An average BIS> 26 during first 12hours of TTM predicted good outcome with 89.5% sensitivity and 75.8% specificity (AUC of 0.869), while average SR values> 24 during the first 12hours of TTM predicted poor outcome (CPC 3-5) with 91.5% sensitivity and 81.8% specificity (AUC, 0.906). Hourly BIS and SR values exhibited good predictive performance (AUC> 0.85), as soon as hour 2 for SR and hour 4 for BIS. Conclusions: BIS/SR are associated with patients’ potential for neurological recovery after CA. This finding could help to create awareness of the possibility of a better outcome in patients who might otherwise be wrongly considered as nonviable and to establish personalized treatment escalation plans.(AU)

ABSTRACT

We present the case of an adult patient with drug-resistant epilepsy caused by extensive inflammation in the right cerebral hemisphere. She was scheduled to undergo right functional hemispherectomy, which is common in pediatric surgery, but about which few studies have been published with respect to adult patients. During the intraoperative period, the density spectral array of the bilateral bispectral index (BIS) VISTATM monitoring system was used. We observed a power increase in low frequency (0.1-4Hz) and alpha bands (8-12Hz) in the right hemisphere, where the epileptogenic focus was. During disconnection from the frontal lobe, there was a marked decrease of power in low frequency and alpha bands on the right side, with no changes during disconnection from other areas of the brain. We think that further studies are needed to determine whether the density spectral array can be a useful tool for monitoring the effectiveness of functional hemispherectomy.

ABSTRACT

Cerebral oximetry based on near infrared spectroscopy (NIRS) technology is used to determine cerebral tissue oxygenation. We hereby present the clinical case of a 12-month old child with right hemiparesis secondary to prior left middle cerebral artery stroke 8 months ago. The child underwent surgical enlargement of the right ventricular outflow tract (RVOT) with cardiopulmonary bypass. During cardiopulmonary bypass, asymmetric NIRS results were detected between both hemispheres. The utilization of multimodal neuromonitoring (NIRS-BIS) allowed acting on both perfusion pressure and anesthetic depth to balance out the supply and demand of cerebral oxygen consumption. No new neurological sequelae were observed postoperatively. We consider bilateral NIRS monitoring necessary in order to detect asymmetries between cerebral hemispheres. Although asymmetries were not present at baseline, they can arise intraoperatively and its monitoring thus allows the detection and treatment of cerebral ischemia-hypoxia in the healthy hemisphere, which if undetected and untreated would lead to additional neurological damage.

ABSTRACT

INTRODUCTION: Dexdor® do not include the possibility of loading dose, which could increase time to achieve adequate sedation for ambulatory procedures. The objective of this study was to evaluate the effect of several loading dose of dexmedetomidine in the time to achieve and maintain an optimal level of sedation and its clinical hemodynamic repercussion. MATERIAL AND METHODS: The IRB approved this observational study for patients that underwent oral and maxillofacial ambulatory surgery under dexmedetomidine at the University of Navarra Clinic from February 2013 to November 2014. According to the loading dose the patients were grouped into 3 categories:<0.5, 0.5, and>0.5µg/kg. Optimal level of sedation was defined as bispectral index<85. Data were analyzed using survival analysis techniques. Vasoactive drugs requirements was evaluated using exact logistic regression. RESULTS: Eighty-one patients were evaluated. Hazard ratios for patients in 0.5 and >0.5µg/kg loading dose categories for achieving a bispectral index<85 were 1.5 (95% CI 0.9, 2.6) and 1.8 (95% CI 0.8, 3.9), respectively, compared with the lowest category. Five patients (6.2%) required atropine for bradycardia. Patients in the group>0.5µg/kg showed greater risk of requiring atropine compared with the group<0.5µg/kg (odds ratio 2.2; 95% CI 0.03, 183). CONCLUSION: Loading dose of dexmedetomidine>0.5µg/kg appears minimize the time to achieve and maintain an optimal level of sedation during the first 60min of procedure. Further investigation to elucidate the association between loading dose of dexmedetomidine and subsequent atropine requirements may be warranted.

ABSTRACT

Monitoring the brain response to hypnotics in general anesthesia, with the nociceptive and hemodynamic stimulus interaction, has been a subject of intense investigation for many years. Nowadays, monitors of depth of anesthesia are based in processed electroencephalogram by different algorithms, some of them unknown, to obtain a simplified numeric parameter approximate to brain activity state in each moment. In this review we evaluate if spectral entropy suitably reflects the brain electric behavior in response to hypnotics and the different intensity nociceptive stimulus effect during a surgical procedure.

OBJECTIVES: 1) To assess the prevalence of pain during nursing care procedures, and 2) to evaluate the usefulness of certain vital signs and the bispectral index (BIS) in detecting pain. METHODS: A prospective, observational analytical study was made of procedures (endotracheal aspiration and mobilization with turning) in critically ill sedated patients on mechanical ventilation. The Behavioral Pain Scale was used to assess pain, with scores of ≥3 indicating pain. Various physiological signs and BIS values were recorded, with changes of >10% being considered clinically relevant. RESULTS: A total of 146 procedures in 70 patients were analyzed. Pain prevalence during the procedures was 94%. Vital signs and BIS values increased significantly during the procedures compared to resting conditions, but only the changes in BIS were considered clinically relevant. In the subgroup of patients receiving preemptive analgesia prior to the procedure, pain decreased significantly compared to the group of patients who received no such analgesia (-2 [IQR: {-5}-0] vs. 3 [IQR: 1-4]; P<.001, respectively). CONCLUSIONS: The procedures evaluated in this study are painful. Changes in vital signs are not good indicators of pain. Changes in BIS may provide useful information about pain, but more research is needed. The administration of preemptive analgesia decreases pain during the procedures.

ABSTRACT

The cardiopulmonary changes in propofol- or thiopental-anesthetized dogs induced to pulmonary hypertension (PH) were evaluated. Twenty adult animals were randomly assigned to two groups: propofol group (PG) and thiopental group (TG). In PG, propofol was used for induction (8(0.03mg.kg-1) and anesthesia maintenance (0.8mg.kg-1.minute-1), while, in TG, thiopental was used (22±2.92mg.kg-1; 0.5mg.kg-1.minute-1, respectively). Mechanical ventilation using time cycle was started. PH was induced by administration of serotonin (5HT) (10µg.kg-1 and 1mg.kg-1.hour-1) through a thermodilution catheter positioned in the pulmonary artery. The measurements were performed before administration of 5HT (T0), after 30 minutes (T30), then at 15-minute intervals (T45, T60, T75 and T90). No differences between groups were registered for systolic (sPAP) and mean pulmonary arterial pressure (mPAP), mean arterial pressure (MAP), total peripheral resistance index (TPRI) and pulmonary vascular resistance index (PVRI). In PG, sPAP and mPAP increased from T30. While in TG, sPAP and mPAP increased from T75. In PG, heart rate (HR) increased from T30, in which PG was higher than TG. The TPRI values decreased from T30 in PG, and in TG, at T45, T60 and T90. In PG, at T0, PVRI was lower than at other times. In PG, arterial partial pressures of oxygen (PaO2) decreased from T60 and alveolar-arterial oxygen gradient (PA-aO2) increased at T60. In TG, at T0 PaO2 was higher than at T30, T45, T60 and T90, while PA-aO2 at T0 was lower than at T90. From T30 to T90, TG showed higher PaO2 means and lower arterial partial pressures of carbon dioxide (PaCO2) values when compared to PG. In PG, from T30, PaCO2 increased, while in TG this parameter was stable. In conclusion, thiopental anesthesia attenuated the cardiopulmonary changes resulting from serotonin-induced PH, probably by attenuation of vasoconstriction and bronchoconstriction.


Avaliaram-se as alterações cardiopulmonares em cães anestesiados com propofol ou tiopental induzidos à hipertensão pulmonar (HP). Vinte animais adultos foram distribuídos aleatoriamente em dois grupos: grupo propofol (PG) e grupo tiopental (TG). No PG, o propofol foi usado para indução (8(0,03mg.kg-1) e manutenção da anestesia (0,8mg.kg-1minuto-1), enquanto no TG foi empregado o tiopental (22(2,92mg.kg-1; 0,5mg.kg-1.minute-1, respectivamente). Em seguida, a ventilação mecânica ciclada a tempo foi iniciada. A HP foi induzida pela administração de serotonina (5HT) (10µg.kg-1e 1mg.kg-1.hour-1) por meio de cateter de termodiluição posicionado na artéria pulmonar. As mensurações tiveram início antes da administração da 5HT (T0), depois de 30 minutos (T30), seguida de intervalos de 15 minutos (T45, T60, T75 e T90). Diferenças entre os grupos não foram registradas para pressões sistólica (PAPs) e média (PAPm) da artéria pulmonar, pressão arterial média (PAM), índices da resistência periférica total (IRPT) e da resistência vascular pulmonar (IRVP). A PAPs e a PAPm aumentaram a partir de T30, no PG, e a partir de T75, no TG. No PG, a frequência cardíaca (FC) aumentou a partir de T30, no qual PG foi maior que TG. O IRPT diminuiu no T45, T60 e T90, no TG, e a partir de T30 no PG. No PG, no T0, IRVP foi menor que nos outros momentos. No PG, a pressão parcial de oxigênio no sangue arterial (PaO2) diminuiu a partir de T60, e a diferença de tensão entre o oxigênio alveolar e arterial (PA-aO2) aumentou no T60. No TG, no T0, a PaO2 foi maior que no T30, T45, T60 e T90, enquanto a PA-aO2,, no T0, foi menor que no T90. Entre T30 e T90, TG apresentou maior PaO2 e menor pressão parcial de dióxido de carbono no sangue arterial (PaCO2) quando comparado ao PG. No PG, a partir de T30, a PaCO2 aumentou. A anestesia com tiopental abrandou as mudanças cardiopulmonares resultantes da indução da HP pela serotonina, provavelmente devido à atenuação da vasoconstrição e broncoconstrição.

ABSTRACT

OBJECTIVE: The bispectral index (BIS) values that predict appropriate anesthetic level to perform an upper gastrointestinal endoscopy in spontaneous breathing are not well established in Pediatrics. The objective of this study is to determine whether it is possible to find an appropriate, less profound, BIS level in the pediatric patient that would enable an upper gastrointestinal endoscopy (UGE) to be performed in spontaneous breathing without causing gag reflex or motor response. MATERIAL AND METHOD: A prospective study was designed and included 61 patients from 12-167 months old, and an ASAI-II who needed a diagnostic UGE. The study was conducted from October 2011 to March 2013. INTERVENTION: UGE performed with an anesthetic protocol using propofol. The vital signs measured were heart and respiratory rate, pulse oximetry, non-invasive blood pressure. The sedation level score (Ramsay scale) and BIS values were also measured. The first attempt was performed at BIS level 60-69, and this was not feasible, then the anesthetic was deepened and a second attempt made at BIS level 50-59. If this was still not possible a deeper anesthetic level was then achieved and a third attempt made at BIS level 45-49. Variables of interest were: effective BIS level (eBIS), BIS level at which UGE was performed without gag reflex or motor response; propofol total dose (mgkg(-1)), induction time (time from onset of sedation to effective start of UGE). A logistic regression analysis was performed to obtain an equation to estimate the possibility of UGE success. RESULTS: The distribution of the patient was: male 40%, female 60%, with 11 (18%) patients under 36 months. The statistical values are expressed as mean and standard deviation, with following results; age (months): 95.9±45.86; weight (kg): 30.5±14.68; effective BIS: 56.41±4.63; induction time (minutes): 11.07±2.69; total propofol dose (per kg): 4.86±1.21. An additional intra-procedure propofol bolus was given in 38 patients (62%), with 7/38 of them (18%) due to movement, and 31/38 (82%) due to BIS level increase. No statistical differences were found in effective BIS level between older and younger patients. CONCLUSIONS: According to the results, BIS levels below 59 predict UGE success, with 72.13% sensitivity and 88.06% specificity in the pediatric population studied.

Background and objectives: Esmolol is known to have no analgesic activity and no anaesthetic properties; however, it could potentiate the reduction in anaesthetic requirements and reduce postoperative analgesic use. The objective of this study is to evaluate the effect of intravenous esmolol infusion on intraoperative and postoperative analgesic consumptions as well as its effect on depth of anaesthesia. Methods: This randomized-controlled double blind study was conducted in a tertiary care hospital between March and June 2010. Sixty patients undergoing septorhinoplasty were randomized into two groups. History of allergy to drugs used in the study, ischaemic heart disease, heart block, bronchial asthma, hepatic or renal dysfunction, obesity and a history of chronic use of analgesic or β-blockers were considered cause for exclusion from the study. Thirty patients received esmolol and remifentanil (esmolol group) and 30 patients received normal saline and remifentanil (control group) as an intravenous infusion during the procedure. Mean arterial pressure, heart rate, and bispectral index values were recorded every 10min. Total remifentanil consumption, visual analogue scale scores, time to first analgesia and total postoperative morphine consumption were recorded. Results: The total remifentanil consumption, visual analogue scale scores at 0, 20 and 60 min, total morphine consumption, time to first analgesia and the number of patients who needed an intravenous morphine were lower in the esmolol group. Conclusions: Intravenous infusion of esmolol reduced the intraoperative and postoperative analgesic consumption, reduced visual analogue scale scores in the early postoperative period and prolonged the time to first analgesia; however it did not influence the depth of anaesthesia. .


Justificativa e objetivos: Esmolol é conhecido por não ter atividade analgésica e propriedades anestésicas; porém, pode potenciar a redução da necessidade de anestésicos e reduzir o uso de analgésicos no pós-operatório. O objetivo deste estudo foi avaliar o efeito da infusão de esmolol por via intravenosa sobre o consumo de analgésico durante os períodos intraoperatório e pós-operatório, bem como seu efeito sobre a profundidade da anestesia. Métodos: Este estudo randômico, controlado e duplo-cego foi conduzido em um hospital terciário entre março e junho de 2010. Foram randomicamente divididos em dois grupos 60 pacientes programados para serem submetidos à septorrinoplastia. História de alergia aos medicamentos usados no estudo, isquemia cardíaca, bloqueio cardíaco, asma brônquica, insuficiência hepática ou renal, obesidade e história de uso crônico de analgésicos ou β-bloqueadores foram os critérios de exclusão. Trinta pacientes receberam esmolol e remifentanil (grupo esmolol) e 30 receberam soro fisiológico e remifentanil (grupo controle) via perfusão intravenosa. Pressão arterial média, frequência cardíaca e valores do índice bispectral foram registrados a cada 10 minutos. Consumo total de remifentanil, escores da escala visual analógica, tempo para a primeira analgesia e consumo total de morfina no pós-operatório foram registrados. Resultados: O consumo total de remifentanil, os escores da escala visual analógica nos minutos 0, 20 e 60, o consumo total de morfina, o tempo para a primeira analgesia e o número de pacientes que precisaram de morfina intravenosa foram menores no grupo esmolol. Justificación y objetivos: El esmolol se conoce porque no posee actividad añalgésica ni propie-dades anestésicas; sin embargo, puede potenciar la reducción de la necesidad de anestésicos y disminuir el uso de analgésicos en el postoperatorio. El objetivo de este estudio fue evaluar el efecto de la infusión de esmolol por vía intravenosa sobre el consumo de analgésico durante los períodos intraoperatorio y postoperatorio, como también su efecto sobre la profundidad de la anestesia. Métodos: Este estudio aleatorizado, controlado y doble ciego fue llevado a cabo en un hospital terciario entre marzo y junio de 2010. Sesenta pacientes programados para someterse a la septorrinoplastia fueron aleatoriamente divididos en 2 grupos. El historial de alergia a los medicamentos usados en el estudio, isquemia cardíaca, bloqueo cardíaco, asma bronquial, insuficiencia hepática o renal, obesidad e historial de uso crónico de analgésicos o ß-bloqueantes, fueron los criterios de exclusión del estudio. Treinta pacientes recibieron esmolol y remifenta-nilo (grupo esmolol) y 30 pacientes recibieron suero fisiológico y remifentanilo (grupo control), vía perfusión intravenosa durante el procedimiento. La presión arterial media, frecuencia cardíaca y valores del índice biespectral fueron registrados cada 10 min. ABSTRACT

The Wada test is a procedure used in the preoperative assessment before epilepsy surgery in order to determine language lateralization, to assess the post-operative risk of an amnesia syndrome, and to evaluate the risk of material-specific memory deficits, in particular verbal memory deficits. This test involves inserting a cannula into the internal carotid artery via the femoral artery, and then to inject amobarbital to shut down brain function, usually in one of the brain hemispheres. The bilateral bispectral index (BIS) VISTA™ monitoring system (BVMS) was used to detect changes in EEG, and in the power spectrum distribution using the density spectral array (DSA) of both hemispheres. We describe a patient with an agenesis of the A1 segment of the right anterior cerebral artery, scheduled for a Wada test, in whom the BVMS demonstrated its potential value.

ABSTRACT

A sharp decrease in the values of the bispectral index (BIS), along with an increase in suppression rate, was observed in a patient after the removal of an epidermoid tumor in the cerebellopontine angle by right retrosigmoid access under general anesthesia. This was probably related to a frontal pneumocephalus. No accompanying neurological signs were observed. The patient was extubated in the Recovery Room with no further incidents, as the BIS increased again. The neurosurgeons chose conservative treatment, relying on the reabsorption and redistribution of the air.

ABSTRACT

A técnica de analgesia multimodal, por meio da infusão contínua de fármacos, pode ser empregada para diminuir a incidência de sensibilização central durante a anestesia. Avaliaram-se as características cardiorrespiratórias, durante o procedimento de artroscopia de joelho, em cães anestesiados com isofluorano e monitorados por meio do índice biespectral, submetidos à infusão contínua de morfina ou fentanil, associada à lidocaína e cetamina. Utilizaram-se 16 cães adultos, machos ou fêmeas, os quais foram distribuídos aleatoriamente em dois grupos, denominados MLK - que recebeu morfina (3,3μg/kg/min), lidocaína (50μg/kg/min) e cetamina (10μg/kg/min) ou FLK - em que foi substituída a morfina pelo fentanil (0,03μg/kg/min). Os cães foram pré-tratados com levomepromazina (0,5mg/kg IV), induzidos à anestesia com propofol (5mg/kg) e mantidos com isofluorano, ajustando-se a concentração para obterem-se valores de índice biespectral entre 55 e 65. As mensurações da frequência cardíaca (FC), dos parâmetros eletrocardiográficos (ECG), das pressões arteriais sistólica (PAS), diastólica (PAD) e média (PAM), da tensão de dióxido de carbono expirado (EtCO2), da saturação de oxi-hemoglobina (SpO2), da frequência respiratória (FR) e da temperatura esofágica (T) iniciaram-se 30 minutos após a indução (M0) e continuaram após o início da infusão das soluções, em intervalos de 15 minutos (M15 a M75). Diferenças entre os grupos foram registradas para duração do complexo QRS (M60), para FC e T, entre M30 e M75, com MLK apresentando médias maiores que FLK, que registrou médias maiores que MLK para a SpO2 (M60), para os intervalos QT (M30 e M75) e RR (M0, M60 e M75). Concluiu-se que o emprego de morfina ou fentanil, associados à lidocaína e cetamina, promove efeitos semelhantes e não compromete as características avaliadas.


The multimodal analgesia technique by continuous infusion of drugs can be used to decrease central sensitization during anesthesia. Cardiorespiratory parameters in isofluorane-anesthetized dogs during joint arthroscopy were evaluated. For this, 16 adult mongrel dogs were randomly divided into two groups, named MLK (morphine (3.3mg/kg/min), lidocaine (50μg/kg/min) and ketamine (10mg/kg/min)) or FLK (replacing morphine by fentanyl (0.03mg/kg/min). Levomepromazine (0.5mg/kg IV) was used as a preanesthetic medication, and propofol (5mg/kg IV) was used for induction and isoflurane was used for maintained general anesthesia, allowing the bispectral index to be maintained between 55 and 65. The measurements of heart rate (HR), eletrocardiographic, systolic (SAP), diastolic (DAP) and mean (MAP) arterial pressures, end-tidal carbon dioxide partial pressure (ETCO2), pulse oxygen saturation (SpO2) respiratory rate (RR) and esophageal temperature (T) were performed 30 minutes after induction (M0), and after the infusion of solutions, at 15 minute intervals (M15 to M75). Differences between groups were registered for the duration of the QRS complex (M60), for HR and T (from M30 to M75), with MLK recording a higher mean than FLK, which registered a lower value than MLK for SpO2 (M60), QT (M30 and M75) and RR (M0, M60 and M75) intervals. It was concluded that morphine or fentanyl, associated with lidocaine and ketamine, promotes similar effects and does not impair the parameters evaluated.

ABSTRACT

INTRODUCTION: The objective of this investigation is to determine whether bispectral index (BIS®) monitoring during intravenous anaesthesia with spontaneous breathing for upper gastrointestinal endoscopy (UGE) in a pediatric population is useful for: a) decreasing the amount of drug, b) decreasing the time for awakening, and c) improving patient safety. PATIENTS AND METHOD: A quasi-experimental case-control prospective study was conducted in the setting of a second level hospital pediatric intensive care unit. PATIENTS: Children aged 1-13 years. CASE: ASA I patient who needed a diagnostic UGE; eligible, 36, participants, 30. CONTROL: historical cohort of patients who needed UGE (years 2008-2010): 50 patients. INTERVENTION: UGE performed with anaesthetic protocol, vital signs monitoring, sedation level (Ramsay scale) and BIS monitoring. VARIABLES OF INTEREST: propofol total dose (mg/kg), induction time, time in performing the UGE, awakening time (min); initial BIS (iBIS), and BIS during the UGE; adverse effects. RESULTS: There were no significant differences in sex, age or weight between case (B) and control (C) population. No significant differences in total propofol doses: (B 4.9 ± 1.4 mg/kg; C 5.2 ± 1.6 mg/kg, P=.492), awakening time (B 12.2 ± 4.6 min; C 12.8 ± 4.4 min, P=.402), time for execution of UGE (B 9.5 ± 4.8 min; C 11.3 ± 6.5 min, P=.335) and induction time (B 11.1 ± 2.6 min; C 10.1 ± 4.2 min, P=.059), iBIS 55.4 ± 6.9. There were no significant differences in adverse effects: 2 patients suffered from mild desaturation in the control group. CONCLUSIONS: BIS monitoring for diagnostic UGE in spontaneous breathing in a pediatric population is feasible, but does not appear to decrease awakening time or the amount of propofol needed. Furthermore, there was no statistically significant decrease in the number of adverse effects.

ABSTRACT

We report the case of a patient with a cerebral aneurysm, located in the left middle cerebral artery. During the clipping of this aneurysm, the bispectral index (BIS) increased for no apparent reason. This was then interpreted as intraoperative non-convulsive status epilepticus. This clinical condition may have negative impact in the prognosis of the patient, so it is very important to be able to detect this conditions as early as possible. Measuring the BIS while the patient is anaesthetised could be useful in this situation, considering that an increase in values greater than 60, associated with acidosis and without any other peri-anaesthetic explanation, may provide evidence of a convulsive equivalent state, allowing appropriate action to be taken.

ABSTRACT

The effects of inspired oxygen fractions (FiO2) of 1 and 0.6 on bispectral index (BIS) in dogs with high intracranial pressure, submitted to a continuous rate infusion of propofol were evaluated. Eight dogs were anesthetized on two occasions, receiving, during controlled ventilation, an FiO2 = 1 (G100) or an FiO2 = 0.6 (G60). Propofol was used for induction (10mg.kg-1, IV), followed by a continuous rate infusion (0.6mg.kg-1.minute-1). After 20 minutes, a fiber-optic catheter was implanted on the surface of the right cerebral cortex to monitor the intracranial pressure, the baseline measurements of BIS values, signal quality index, suppression ratio number, electromyogram indicator, end-tidal carbon dioxide partial pressure, mean arterial pressure, intracranial pressure and cerebral perfusion pressure were taken. Then, the blood flow from the right jugular vein was interrupted in order to increase intracranial pressure and after 20 minutes additional recordings were performed at 15-minute intervals (T0, T15, T30, T45 and T60). The arterial oxygen partial pressure varied according to the changes in oxygen. For the other parameters, no significant differences were recorded. The BIS monitoring was not influenced by different FiO2.


Avaliaram-se os efeitos das frações inspiradas de oxigênio (FiO2) de 1 e 0,6 sobre o índice biespectral (BIS) em cães com pressão intracraniana elevada e submetidos a infusão contínua de propofol. Oito animais foram anestesiados em duas ocasiões, recebendo durante a ventilação controlada FiO2 = 1(G100) ou FiO2 = 0,6 (G60). Propofol foi usado para indução (10mg.kg-1) e seguido por infusão contínua (0,6mg.kg-1minuto-1). Após vinte minutos da implantação do cateter de fibra óptica do monitor de pressão intracraniana, na superfície do córtex cerebral direito, realizaram-se as mensurações basais de BIS, índice de qualidade de sinal, taxa de supressão, eletromiografia, pressão parcial de dióxido de carbono ao final da expiração, pressão arterial média, pressão intracraniana e pressão de perfusão cerebral. Em seguida, interrompeu-se o fluxo sanguíneo da veia jugular direita, para o aumento da pressão intracraniana e depois de 20 minutos, novas mensurações foram realizadas em intervalos de 15 minutos (T0, T15, T30, T45 e T60). A pressão parcial de oxigênio no sangue arterial variou conforme a FiO2. Para os outros parâmetros não foram registradas diferenças significativas. Conclui-se que o monitoramento pelo BIS não foi afetado pelo emprego de diferentes FiO2.

En los últimos años el despertar transoperatorio es uno de los principales temores de los enfermos que van a ser intervenidos quirúrgicamente y se relaciona a consecuencias como el síndrome de estrés postraumático y a las implicaciones legales resultantes. Diversos reportes que han enfatizado la elevada incidencia del despertar transoperatorio y sus efectos adversos se han diseñado diferentes técnicas para su detección temprana y monitorización; entre los cuales el más importante es el índice biespectral (BIS). El objetivo principal del presente trabajo es el de comparar el consumo de agentes anestésicos durante la anestesia general, monitorizando parámetros hemodinámicos vs. valores BIS. Se realizó un estudio prospectivo, longitudinal observacional y comparativo en la unidad de quirófano y salas de recuperación del Instituto Gastroenterologico Boliviano-Japonés desde junio-agosto 2008 ingresan a este estudio 40 pacientes, de ambos sexos, ASA l-ll, edad entre 20-60 años, programados para cirugía abdominal. Se aplico una ficha de recolección de datos aleatoriamente divididos en 2 grupos de estudio: Grupo I se dosifico los agentes anestésicos a la monitorización del grado de hipnosis mediante el BIS. Grupo II se dosifico los agentes anestésicos a la monitorización de signos clínicos y cambios hemodinámicos. Según los resultados obtenidos se puede observar menor dosificación de drogas durante la anestesia general, en el Grupo monitorizado con BIS con relación al Grupo que se monitorizó con cambios hemodinámicos, lo que significa mayor dosis de anestésicos en los monitorizados con cambios hemodinámicos.


In recent years, intraoperative awakening is one of the main fears of patients who will undergo surgery and is related to consequences such as posttraumatic stress syndrome and the resulting legal implications. Several reports have emphasized the high incidence of intraoperative awakening and adverse effects are designed techniques for early detection and monitoring, among which the most important is the bispectral Índex (BIS).The main objective of this study is to compare the use of anesthetics during general anesthesia, hemodynamic monitoring vs. BIS valúes. We performed a prospective, longitudinal observational and comparative unit of operating and recovery rooms Gastroenterological Bolivian-Japanese Institute from June to August 2008 entered this study 40 patients of both sexes.ASA l-ll, aged between 20 - 60 years, scheduled for abdominal surgery. It applied a data collection sheet randomly divided into 2 study groups: Group I was dosed anesthetic agents to monitor the degree of hypnosis through the BIS. Group II was dosed anesthetic agents to the monitoring of dinical and hemodynamic changes. According to the results you can see lower dosage of drugs during general anesthesia in the BIS monitored group compared with the group that was monitored with hemodynamic changes, which means higher doses of anesthetics on the hemodynamic changes monitored.

Foram avaliadas possíveis alterações cardiorrespiratórias e no índice biespectral em cães anestesiados pelo isofluorano, associado ou não ao tramadol. Utilizaram-se 16 animais, distribuídos em dois grupos denominados GC (grupo-controle) e GT (grupo tramadol). Todos os cães foram induzidos e mantidos sob anestesia com isofluorano. Os animais do GC receberam 0,05ml/kg de solução salina a 0,9 por cento e os do GT 2mg/kg de tramadol, ambos por via intramuscular. Foram avaliados: freqüência cardíaca, pressão arterial sistólica, diastólica e média, eletrocardiografia, freqüência respiratória, saturação de oxiemoglobina, concentração de dióxido de carbono ao final da expiração, índice biespectral e recuperação da anestesia. Concluiu-se que a administração de tramadol em cães anestesiados pelo isofluorano não produz alterações nas variáveis cardiorrespiratórias, no índice biespectral e no tempo de recuperação da anestesia, porém proporciona boa qualidade de recuperação anestésica.


It was studied fortuitous cardiorespiratory and bispectral index changes in dogs anesthetized with isoflurane associated or not to tramadol. Sixteen dogswere distributed in two groups named CG (control group) and TG (tramadol group). General anesthesia was induced in all animals with isoflurane via mask. After 10 minutes, the animals of CG received 0.05ml/kg of saline solution at 0.9 percent, and TG received 2mg/kg of tramadol, both via intramuscular. It was evaluated heart rate, systolic, diastolic and mean arterial pressures; electrocardiography; respiratory rate; oxihemoglobin saturation; end tidal carbon dioxide; bispectral index and recovery of anesthesia. The administration of tramadol in dogs anesthetized with isoflurane did not produce changes in cardiorespiratory variables, bispectral index and anesthetic recovery time. In addition, this association promoted good quality of anesthetic recovery.
