ABSTRACT

Management of severe joint involvement in rheumatoid wrist is controversial. The gold-standard is total wrist fusion, but total wrist replacement offers a motion-conserving alternative. The purpose of this study was to present the results of interposition arthroplasty with the Amandys® pyrocarbon implant in rheumatoid wrist. We performed a retrospective review of 28 arthroplasties for rheumatoid wrist arthritis. Eighteen females and 5 males were included, with a mean age of 55.7 years. Mean follow-up was 64 months. We measured range of motion, grip strength, and pain (on VAS). Function was evaluated preoperatively and at last follow-up with the DASH and PRWE scores. Mean range of motion in flexion-extension was maintained while mean inclination and rotational range of motion showed significant improvement. Mean grip strength increased from 10 kg to 17 kg. Mean pain score decreased from 6/10 to 2/10. Mean PRWE and QuickDASH scores decreased from 62/100 to 25/100 and from 62/100 to 36/100, respectively. Three patients underwent early reoperation to reposition a dislocated implant. No implants had to be removed. Amandys® pyrocarbon arthroplasty is a reliable alternative to total fusion or total replacement in rheumatoid wrist. Indications must be limited to well-aligned wrists with competent capsule-ligament structures.

ABSTRACT

Introducción: la artritis reumatoidea es una enfermedad sistémica, en la que son comunes las deformidades articulares, estas se pueden observar en el pulgar donde las de mayor frecuencia son las de Boutonniere y en cuello de cisne. Objetivo: el objetivo principal fue evaluar los resultados de la aplicación de las técnicas quirúrgicas en estas deformidades. Método: se realizó un estudio longitudinal prospectivo de intervención. Se trataron un total de 20 pacientes con este diagnóstico, atendidos en el Servicio de Miembro Superior Minifijación Externa y Microcirugía del Complejo Científico Ortopédico Internacional Frank País en el período comprendido desde enero 2011 a diciembre 2012. La evaluación de los pacientes se realizó según la escala confeccionada para este grupo particular en estudio. Resultados: se obtuvieron resultados excelentes en el 10 por ciento, buenos en el 75 por ciento y regulares en el 15 por ciento de los casos. Conclusiones: las técnicas quirúrgicas empleadas en el tratamiento de esta afección mostraron resultados alentadores(AU)


Introduction: Rheumatoid arthritis is a systemic disease, in which joint deformities are common; these deformities are observed in the thumb where the most frequent are Boutonniere and swan-neck deformities. Objective: Assess the results of the application of surgical techniques in these deformities. Method: A longitudinal prospective intervention study was conducted at Frank País International Orthopedic Scientific Complex from January 2011 to December 2012. Twenty patients with this diagnosis were treated in the Upper Limb Service for External Miniaturization and Microsurgery. The evaluation of the patients was performed according to the scale prepared for this particular study group. Results: 10 of the cases obtained excellent results, 75 percent obtained good results and 15 percent had moderate results. Conclusions: The surgical techniques used in the treatment of this condition showed encouraging results(AU)


Introduction: L'arthrite rhumatoïde est une maladie systémique caractérisée par des atteintes articulaires, surtout au niveau du pouce, telles que les déformations en boutonnière et en col de cygne. Objectif: Évaluer les résultats de l'application des techniques chirurgicales dans le traitement de ces déformations. Méthodes: Une étude longitudinale prospective interventionnelle a été réalisée. Un total de vingt patients, diagnostiqués d'arthrite rhumatoïde, ont été traités au Service d'orthopédie de membres supérieurs, mini-fixation externe et microchirurgie, au Complexe scientifique international d'orthopédie Frank Pais, dans la période comprise entre janvier 2011 et décembre 2012. Les patients de ce groupe ont été évalués selon une échelle élaborée pour cette étude. Résultats: On a obtenu des résultats excellents (10 pourcent), bons (75 pourcent), et passables (15 pourcent). Conclusions: Les techniques chirurgicales utilisées dans le traitement de cette affection ont montré des résultats encourageants(AU)

ABSTRACT

OBJECTIVES: The aim of this study was to investigate the role of IL-1ß (-511C>T), TNFα (-308 G>A), IL-10 (-1082 G>A) and IL-1RN VNTR polymorphisms in the susceptibility to rheumatoid arthritis (RA) in Tunisians. PATIENTS AND METHODS: Using PCR-based methods, 104 RA patients and 150 healthy controls were investigated. We compared allele and genotype frequencies in RA patients versus controls and analyzed their correlations with erosive form (EF). RESULTS: IL1-RN VNTR A1A3 genotype is associated with higher risk of RA (P=0.012, OR=4.31). Among the cases, males who carry this genotype were more exposed to RA (P=0.044, OR=8, 47). For IL1- ß gene, a significantly higher frequency of the -511C/C genotype was observed in RA patients in comparison to controls (P=0.013, OR=2.45). This higher frequency was especially observed in women (P=0,003, OR=3.42). In contrast, IL10-1082G/G genotype was less common in patients (P=0.046, OR=0.46). According to EF, men carrying IL1-RN VNTR A1A3 (P=0.005 OR=5.28) and IL1-ß-511C/C (P=0.015 OR=2.61) genotypes develop non EF of RA. Moreover, TNFα-308 A allele (P=0.024, OR=1.84) and A/A genotype (P=0.033, OR=3.1) were positively associated to EF of RA. However, G allele (P=0.024, OR=0.31) and GG genotype (P=0.31, OR=0.031) of the TNFα-308 were protectors. CONCLUSION: Our results indicated that IL-1RN VNTR, IL-1ß (-511C>T) and IL-10 (-1082 G>A) are associated with susceptibility to RA, and that IL-1RN VNTR, IL-1ß (-511C>T) and TNFα (-308 G>A) are associated with severity of RA.

ABSTRACT

Se estudiaron 44 pacientes adultos (6 del sexo masculino y 38 del femenino) con diagnóstico de artritis reumatoidea de la consulta externa del Hospital General Docente "Aleida Fernández Chardiet", de Güines, provincia de La Habana, para conocer la frecuencia, tipos y características de la enfermedad extraarticular asociada. Se encontró enfermedad extraarticular en el 66 % de los pacientes estudiados; las manifestaciones más frecuentes fueron los nódulos reumatoideos, la anemia y el síndrome del túnel carpiano. No se encontró relación entre la enfermedad extraarticular y el sexo de los pacientes afectados, pero sí con la forma poliarticular de la artritis reumatoidea (p < 0,001) y el tiempo de evolución de la enfermedad (p < 0,001). Los nódulos reumatoideos, la polineuropatía sensorial y las manifestaciones cardíacas se asociaron significativamente (p < 0,001) a pacientes con factor reumatoideo positivo.


44 adult patients (6 males and 38 females) with diagnosis of rheumatoid arthritis from the outpatient department of the "Aleida Fernández Chardiet" General Teaching Hospital in Güines, province of Havana, were studied to know frequency, types, and characteristics of the associated extraarticular disease. Extraarticular disease was found in 68 % of the patients studied. The most frequent manifestations were the reumatoid nodules, anemia, and the carpal tunnel syndrome. No relationship was found between the extraarticular disease and the sex of the patients; however, it was observed a relationship between the polyarticular form of the rheumatoid arthritis (p < 0,001) and the time of evolution of the disease (p < 0,001). The rheumatoid nodules, the sensorial polyneuropathy, and the cardiac manifestations were significantly associated (p < 0,001) to patients with a positive rheumatoid factor.


On a étudié 44 patients adultes (6 du sexe masculin et 38 du féminin) avec diagnostic d'arthrite rhumatoïde appartenant à la consultation externe de l'Hôpital Général Universitaire "Aleida Fernández Chardiet", à Güines, province de La Havane, afin de connaître la fréquence, types et caractéristiques de la maladie extraarticulaire associée. On a trouvé maladie extra-articulaire chez 68 % des patients étudiés; les manifestations les plus fréquentes furent les nodules rhumatoïdes, l'anémie et le syndrome du tunnel carpien. On n'a pas trouvé de rapport entre la maladie extra-articulaire et le sexe des patients affectés, mais on l'a trouvé avec la forme polyarticulaire de l'arthrite rhumatoïde (p < 0,001) et le temps d'évolution de la maladie (p < 0,001). Les nodules rhumatoïdes, la polyneuropathie sensorielle et les manifestations cardiaques ont été significativement associés (p < 0,001) aux patients avec facteur rhumatoïde positif.
