ABSTRACT

Acrochordons are polypoid, skin-colored lesions usually present at the site of skin folds. They are extremely rare in the preputial area of the penis and do not tend to grow. To the best of our knowledge, in English literature, this report presents the first case of an androgen receptor-positive penile acrochordon, which is located on the penis and showed rapid growth along with body development during puberty with no underlying causes such as acromegaly, diabetes, obesity, and trauma.

ABSTRACT

Objective: We sought to describe skin injuries associated with unapproved topical mole and skin tag removers containing concentrated salicylic acid, Sanguinaria canadensis, or other caustic agents. Methods: We identified skin injuries associated with unapproved non-device topical mole and skin tag removers reported to the US Food and Drug Administration (FDA) through October 30, 2021 or described in Amazon consumer product reviews between 2019 and 2021. Results: We identified 38 cases, including 30 from Amazon consumer product reviews and eight reported to the FDA. Twenty-eight were from 2021. The most common reason for use was for mole and/or skin tag removal. Listed ingredients included salicylic acid, Sanguinaria canadensis, botanicals (includes homeopathic products), and calcium oxide. Seven cases involved products without ingredients listed. Adverse events included burns, pain, and ulceration, some resulting in permanent scarring and disfigurement. There were 14 facial injuries, including four adjacent to the eye. Reported treatments included antibiotics, hospital care, wound care, and dermatology advice to have a skin graft. Limitations: Limitations include underreporting of adverse events to the FDA, limited clinical details and potential bias in consumer reviews, and poor replicability of review searches due to the dynamic nature of the Amazon website. Conclusion: Unapproved, non-device topical mole and skin tag removers are associated with serious skin injuries. We found Amazon consumer reviews to be a novel and useful data source for safety surveillance of these types of skin products. When dermatologists are consulted about skin injuries, exposure to these products should be considered in the differential diagnosis.

ABSTRACT

Fibroepithelial stromal polyps (FEPs) are benign tumors of the integumentary system with mesodermal origin. They are commonly found over the skin. Rarely, they can be found in the nipple. We report a case in a 62-year-old female with a large FEP emerging from the left breast nipple. It started as a "mole" 10 years ago and slowly progressed in size. The patient did not complain of pain but did report occasional bloody discharge. She has no family history of breast cancer. On physical exam, a pedunculated large polyp, with rubbery consistency, emerging from the left nipple was noted. The lesion consisted of hyper-pigmentation with scattered ulcerations and cauliflower-like growth. Surgical excision was performed and histopathologic analysis showed extensive fibrovascular tissue with dense collagen fibers deposition consistent with FEP. Surgical treatment is curative with minimal skin deformity. In order to make the diagnosis and exclude malignancy, histopathology is mandatory.

BACKGROUND: Cutaneous changes in obese adults have been investigated in numerous studies, but this issue has not been adequately investigated in children. OBJECTIVES: We aimed to determine the prevalence of skin manifestations in children with obesity by comparing them to normal-weight children. METHODS: A cross-sectional study was conducted between June 2017 and January 2019 in which 82 children with obesity and 86 normal-weight healthy control children were enrolled. Study participants had detailed full-body skin examinations performed by the same dermatologist; mycological and Wood's lamp examinations were performed when necessary. Sociodemographic and anthropometric measurements of the participants were recorded. RESULTS: Striae distensae were the most common skin manifestation in children with obesity; striae were significantly higher in the obese than in the control group (72% vs. 26.7%, p < .001). The anatomical distribution of the striae distensae in the groups differed significantly. Striae distensae were most commonly observed on the buttocks in the control group, while the thighs were the most common site in the obese group. Acanthosis nigricans (63.4% vs. 3.5%, p < .001), acrochordons (17.1% vs. 1.2%, p < .001), keratosis pilaris (32.9% vs. 17.4%, p = .021), intertrigo (11% vs. 0%, p = .001), folliculitis (31.7% vs. 5.8%, p < .001), seborrheic dermatitis (12.2% vs. 3.5%, p = .035) and facial erythema (7.3% vs. 0%, p = .012) were found to be statistically significantly more common in the obese group compared to the control group. CONCLUSIONS: Obesity in children is associated with numerous cutaneous manifestations. Further study is needed to identify whether weight loss can reduce skin manifestations in obese children.

ABSTRACT

Nevus lipomatosus cutaneous superficialis (NLCS) is a rare cutaneous hamartoma characterized by mature adipocytes in the dermis. Here, we present a unique case of NLCS in a 57-year-old female that strikingly resembled an acrochordon, demonstrating features of the classical and solitary form of NLCS. This presentation of NLCS has not been widely reported and emphasizes that the diagnosis of NLCS should be considered when evaluating suspected acrochordons.

ABSTRACT

INTRODUCTION: Skin tumors are relatively uncommon malignancies worldwide, but its incidence has been progressively increased over the last few decades. Skin tumor belongs to a diverse group of neoplasms arising from the epidermis, adnexal structures and dermis rendering the classification difficult. The study aims to find out the prevalence of benign skin neoplasm among the histopathological specimens of skin neoplasm of a teaching hospital. METHODS: A descriptive cross-sectional study among the hospital records of histopathological samples of skin neoplasm in the Department of Pathology of a tertiary care center from January 2017 to December 2020. Ethical approval was taken from the Institutional Review Committee (Ref: MEMG/IRC/427/GA). Convenient sampling was done. Data were entered in Microsoft Excel and analyzed using Statistical Package for the Social Sciences version 21 software. Point estimate at 95% Confidence Interval was calculated with frequency and descriptive statistics. RESULTS: Out of total skin neoplasm samples, 121 (57.34%) (50.67-64.01 at 95% Confidence Interval) benign skin neoplasms were present. Among them, the majority were keratinocytic tumor 81 (66.9%) followed by skin appendageal 23 (19.0%) and melanocytic tumors 17 (14.0%). Acrochordan 18 (14.9%) and pilomatricoma 12 (9.9%) were the predominant keratinocytic and appendageal neoplasms respectively. Most of the cases occurred in head and neck region 64 (52.9%). CONCLUSIONS: The study concluded that the prevalence of benign skin neoplasm was slightly lower compared to the other studies. Most of the benign skin neoplasms were keratinocytic tumors followed by appendageal and melanocytic tumors. Acrochordan was the commonest benign keratinocytic tumor.

BACKGROUND: Skin tags are common and mostly benign, but occasionally contain skin cancers. This study analysed skin tags by combining three advanced optical imaging technologies: reflectance confocal microscopy (RCM), optical coherence tomography (OCT) and multispectral optoacoustic imaging (MSOT) supplemented by dermoscopy MATERIALS AND METHODS: A prospective clinical study recruiting patients with skin tags from a university hospital clinic over a 2-week period. OCT, RCM and MSOT imaging were performed prior to excisional biopsies. Image features such as pigmentation, cell types and skin architecture, angiographic information demonstrating vascular pattern were captured, analysed, and compared to melanin and haemoglobin content in MSOT as well as histopathology. RESULTS: Six patients with dermal naevi (2); compound naevi (3); neurofibroma (1) were included. All skin tags except the neurofibroma were pigmented (5/6), with sparse (5/6) and dense (4/6) hyperreflective nests and band-like collagen in dermis in 6/6 lesions on RCM. Dermoscopy showed dots (5/6) and coiled vessels (5/6). Linear vertical vessels were present in all OCT images. MSOT images consisted of a compact shell-like superficial melanin area, same shape and size as the skin tag, dermal vessels were visible in 4/5 naevi, HbO2 signal clearly demarcated blood vessels located below the melanin signal. CONCLUSION: OCT showed linear vessels in all lesions. Pigmentation was identified by RCM as benign nests of melanocytes. MSOT supplemented with spatial distribution of melanin and HbO2 that indicated all skin tags were benign with no infiltration of vessels inside the melanin signal. Each advanced method proved indispensable for fast diagnosis. Larger studies are warranted for validation.

BACKGROUND: Acrochordons are benign hypertrophic lesions of the skin of which the pathophysiology is unclear. OBJECTIVE: This study aimed to examine the association of acrochordons with autoimmune disorders in patients with a poor obstetric history. METHODS: This retrospective cohort involved 350 female patients with poor obstetric history who were included in a preconceptional care program to investigate risk factors for obstetric complications. These patients were further investigated for the co-existence of autoimmune disorders (defined by either a diagnosis of autoimmune diseases or autoimmune antibody positivity) and acrochordons. RESULTS: An autoimmune disorder was present in 55.7% (195/350) of the patients. The rate of acrochordons was significantly higher in patients with autoimmune disorders (n= 195) compared to the control group (n= 155) (8.21% versus 2.58%, respectively) (p= 0.043). When the autoimmune disease positive (n= 58) and autoimmune antibody-positive (n= 137) groups were separately analyzed, acrochordons were found more frequently in the autoimmune disease group (p= 0.004). However, there was no statistically significant co-occurrence of autoimmune antibody positivity and the presence of skin tags (p= 0.135). CONCLUSION: There may be immune system-related biological mechanisms underlying the pathogenesis of acrochordons. Preconceptional counseling is beneficial for women with poor obstetric history and acrochordons.

ABSTRACT

@#Fibroepithelial stromal polyp, more commonly known as acrochordon, skin tag, or soft fibroma is a type of mesenchymal tumor occurring among women of reproductive age. Fibroepithelial polyp, although the most common cutaneous tumor, is rare in the vulvovaginal region and there is currently no established protocol in approaching these kinds of lesions. Presented here is a series of cases of gradually enlarging labial masses among reproductive age women. These giant vulvar masses presented as solitary, flesh-colored, polypoid masses, initially non-tender but later becoming associated with local pain. Diagnosis is mainly through history, clinical examination aided by ultrasonography, and histopathologic examination, which would show a central fibrovascular core covered with squamous epithelium. Surgical excision serves as both diagnostic and therapeutic modality for these lesions. Vulvar fibroepithelial polyp do not seem to be as rare as literature says, they are relatively easy to diagnose and presents with benign clinical course.

Contexto: los acrocordones son protrusiones benignas de piel presentes en zonas de roce; usualmente acompañan a enfermedades metabólicas. Objetivo: caracterizar la asociación entre la severidad de presentación de acrocordones y el riesgo cardiometabólico medido en función del índice de masa corporal y obesidad central en pacientes mayores de 18 años que acuden a la Consulta de Dermatología Hospital San Francisco de Quito (HSFQ). Sujetos y métodos: pacientes de ambos sexos, mayores a 18 años que acuden espontáneamente a Consulta Externa del Servicio de Dermatología del Hospital San Francisco de Quito, perteneciente al Instituto Ecuatoriano de Seguridad Social, durante los meses septiembre a octubre de 2015. Antecedentes familiares y personales de la enfermedad, medición de peso, talla, perímetro de cintura y presencia de acrocordones (coloración, número y localización) asociados a acantosis nigricans. Resultados: se estudiaron 111 sujetos, con una edad media de 47 años; predominaron pacientes del sexo femenino (60,7%). Se determinó en el 55,9% de casos un índice de masa corporal (IMC) superior a 30 Kg/m2, siendo el fototipo IV más común (59,4%) asociado a obesidad abdominal (69,3%). La zona más afectada es cuello (96,4%); se catalogaron como severos el 75,6% de casos. La prevalencia de acrocordones de coloración mixta y forma severa de la enfermedad estuvo presente en 39 pacientes (29%). Un IMC ≥30 Kg/m2 y la forma severa de acrocordones es considerado un factor riesgo, sin demostrarse asociación estadística (OR 1,83; IC 95% 0,76-4.41). Sujetos con IMC ≥ 30 Kg/m2 tiene mayor riesgo de presentar acrocordones de forma severa asociados a acantosis nigricans (OR 2,62; IC 95% 1,07-6,36). Conclusión: se demostró relación estadística en el grupo de pacientes con obesidad y acrocordones de presentación severa, que presentan acantosis nigricans asociada. (AU)


Context: acrochordons are benign skin protrusions present in areas of friction; they usually accompany metabolic diseases. Ojective: to characterize the association between the severity of the presentation of acrochordons and the cardiometabolic risk measured according to the body mass index and central obesity in patients over 18 years of age who attend the San Francisco de Quito Hospital Dermatology Consultancy (HSFQ). Subjects and methods: patients of both sexes, over 18 years of age, who spontaneously go to the External Consultation of the Dermatology Service of the San Francisco de Quito Hospital, belonging to the Ecuadorian Social Security Institute, from September to October 2015. Main measurements: Family and personal history of the disease, measurement of weight, height, waist circumference and presence of acrochordons (coloration, number and location) associated with acanthosis nigricans. Results: 111 subjects were studied, with an average age of 47 years; female patients predominated (60.7%). A body mass index (BMI) greater than 30 kg / m2 was determined in 55.9% of cases, with phototype IV being the most common (59.4%) associated with abdominal obesity (69.3%). The most affected area is neck (96.4%); 75.6% of cases were classified as severe. The prevalence of acrochordons of mixed coloration and severe form of the disease was present in 39 patients (29%). A BMI =30 Kg / m2 and the severe form of acrochordons is considered a risk factor, without showing a statistical association (OR 1.83, 95% CI 0.76-4.41). Subjects with BMI = 30 Kg / m2 have a higher risk of presenting acrochordons of severe form associated with acanthosis nigricans (OR 2.62, 95% CI 1.07-6.36). Conclusion: Statistical relationship was demonstrated in the group of patients with obesity and acrochordons of severe presentation, who presented associated acanthosis nigricans. (AU)

O acrocórdon é geralmente tumor benigno, peduncular, que acomete tanto o sexo feminino como o masculino e surge na idade adulta, sendo mais comum na meia idade. Alguns indivíduos podem ter mais probabilidade de serem acometidos, principalmente obesos e diabéticos. A maioria tem tamanho pequeno. Surgem mais facilmente em áreas de fricção, mas podem ocorrer em qualquer parte do corpo. Ocasionalmente pode exigir remoção devido a crescimento atípico, torção, irritação e dor, a depender do local acometido. Algumas condições comuns da pele podem imitar o acrocórdon ? a verruga de região genital é uma delas. Apesar de essa lesão ser acometida pelo papilomavírus humano, o acrocórdon algumas vezes pode estar associado a ela. Assim, é importante o exame histológico do material biopsiado.


Vulvar acrochordon: a report of two cases The acrochordon is usually a benign, stalk tumor, which affects both men and women. It is an adult-onset disease, being more common in middle-aged individuals. Some individuals may be more prone to the disease, especially those obese and diabetic. Most are small and develop more easily in areas of friction, but they may occur anywhere in the body. Occasionally removal may be required due to atypical growth, torsion, irritation or pain depending on the affected sites. Some common skin conditions may mimic acrochordon. Genital wart is one of these conditions, although it is affected by the human papiloma virus. The acrochordon may sometimes be associated with this infection. Therefore, histological examination of biopsy material is important.

BACKGROUND: Skin tags (ST) are common benign tumors of the skin but their etiopathogenesis is not well understood. STs arise in sites subjected to trauma. It was proved that mast cells are recruited to sites of skin trauma and increase their tumor necrosis factor-α (TNF-α) content. AIM: STs are linked to obesity and frictional sites, but this has not been studied at the molecular level. We hypothesized that mast cells, TNF-α and its family member, TNF-related apoptosis-inducing ligand (TRAIL) might play a role in the pathogenesis of STs as a response to trauma. MATERIALS AND METHODS: A study was done on 15 patients with STs. Two STs and a snip of normal skin were obtained in each subject. We counted the mast cells after Toluidine blue staining. Enzyme-linked immunosorbant assay was used to measure TNF-α level while reverse transcriptase polymerase chain reaction was used to evaluate the level of TRAIL mRNA expression. RESULTS: Mast cell count in all STs was significantly higher than that in control (P=0.0355). There was a highly significant increase in the level of TNF-α in all STs as compared to its level in controls (P<0.0001). Expression of TRAIL mRNA was significantly higher in STs as compared to its expression in controls (P<0.0001). CONCLUSION: Our study suggests that mast cells, TNF-α and TRAIL may play a role in the pathogenesis of STs.

ABSTRACT

Patients with Birt-Hogg-Dube syndrome have an increased risk of developing hamartomas of the pilosebaceous unit, renal tumors of various types, lung cysts, and spontaneous pneumothorax. We present the case of a 54-year-old woman with a long history of whitish papules in the central region of the face and a family history of similar lesions. Biopsy and genetic study revealed a new mutation of the gene involved in Birt-Hogg-Dube syndrome.

Acrochordons are usually small in size and are easily treated with electrofulguration under local anaesthesia. Here is a report of a giant acrochordon which posed problems in management. A technique to manage it under local anaesthesia in an office setting is described.

Acrochordons are commonly found in middle aged rnan. They are mostly a few millimeters in size. The authors report an exceptionally large(10x5cm) one with a 3cm pedicle, which is thought to be the largest one in the Korean literature.
