ABSTRACT

INTRODUCTION: Cystic fibrosis transmembrane regulator (CFTR) channel modulators (ivacaftor, lumacaftor, tezacaftor and elexacaftor) represent a major advance in the management of cystic fibrosis. However, few data are available on the real-life safety profile of these medications, in particular on adverse events that may lead to their discontinuation. The aim of this study is to describe the characteristics and evolution of adverse reactions to the tezacaftor/ivacaftor/elexacaftor combination that led to discontinuation and were reported to the Centre régional de pharmacovigilance (CRPV) in Rennes (France). MATERIALS AND METHODS: A retrospective study was conducted from December 2021 to May 2023, focusing on cases of discontinuation of the tezacaftor/ivacaftor/elexacaftor combination due to the occurrence of one or more adverse effects, and reported to the CRPV of Rennes, France. RESULTS: Ten cases of drug discontinuation were reported to the Rennes CRPV (6 women/4 men). Adverse effects mainly involved neuropsychiatric disorders (n=6), followed by liver disorders (n=2), ear, nose and throat disorders (n=1), and digestive disorders (n=1). The average duration of treatment at discontinuation was 339.8 [39-668] days. The drug was reintroduced in 7 patients on average 48.7 [7-123] days after discontinuation, with a dosage adjustments (n=4) consisting of changes in dosing times or a reduction in daily doses, with varying success in alleviating adverse symptoms depending on the case. CONCLUSION: This small case series suggests that neuropsychiatric adverse effects may occur more frequently than initially described after initiation of tezacaftor/ivacaftor/elexacaftor, and should be carefully screened and monitored. Dosage or administration schedule modifications may be considered for patients experiencing these adverse effects. Further pharmacovigilance studies are needed to better understand the adverse effect profiles of "caftors", their possible risk factors, and the impact of adjusting dosing modalities.

ABSTRACT

Abstract Sex-gender-diverse and non-monogamous strain cisnormativity and mononormativity. In scientific terms, the parenting arrangements of these people are uncertain. Thus, this ethnography aims to understand the perception of non-monogamous sex-gender-diverse people about parenting. The theoretical framework adopted is derived from non-monogamous studies, love and sexuality from the Social and Human Sciences in Public Health and the digital ethnography methodological framework. Fieldwork occurred from 2021 to 2022 through an online WhatsApp group. Participant observation was employed in the group, and semi-structured online interviews were held. Two categories emerged: a) The non-monogamous parenting nodes and b) Collective parenting. In the first, the importance of bonds in affective networks is explored, and the barriers to these family arrangements are exposed. The second describes the importance of living in a community, and Indigenous and Black ancestry is revived. The revived ancestry and ways of living in a community gain importance as we understand their relevance in the experience of parenting for sex-gender-diverse people who are non-monogamous.

ABSTRACT

Providing accurate and up-to-date practical tools enabling oversight of platelet-rich plasma (PRP) legislation and of the appropriate standards to be implemented for its manufacture and use in Europe is a demanding task. This is due to rapid medico-technological advancements, slowness and disparity in legislation updates and enforcement between member states, and many reported gray-zone practices, notably for autologous PRP use. The levels of risk associated with blood manipulation processes generally dictate the manufacturing requirements for PRP preparations, which have gradually shifted toward good manufacturing practices (GMP) for standardization and overall quality enhancement. This work firstly outlines Western European and Swiss legislation for PRP products/preparations, providing key simplified information and recommendations for medical doctors seeking to implement this biological-based therapy for safe use in hospital settings, clinics, or private offices. This work secondly shows the importance of PRP-based product manufacturing standardization, which subsequently enables sound clinical evaluation of therapeutic interventions. Although the applicable legal bases provide guidelines for GMP manufacturing infrastructure and basic process design, paramount importance is set on the definition of workflows, technical specifications, and key parameters for PRP preparation and delivery. Overall, the development of simple and robust technologies and processes for PRP preparation is critical for guaranteeing the high therapeutic quality of the intervention, in collaboration with qualified GMP manufacturing platforms. Importantly, this work aims to serve as a practical tool for clinicians based in Western Europe who are willing to appropriately (i.e., administratively and technically) implement autologous PRP treatments in musculoskeletal regenerative medicine workflows, to ensure they make informed and optimal regulatory or process-based decisions.

ABSTRACT

The aim of this study was to verify the associations between passion for work, well-being and occupational self-efficacy in sports coaches. 139 coaches of both sexes participated (71.2% male). Positive effects of harmonious passion have been observed on positive affects and occupational self-efficacy, and negative effects on negative affects. Obsessive passion positively predicted negative affects. Still, the mediating role of positive affects in the relationship between harmonious passion and occupational self-efficacy was identified. Corroborating the theoretical hypotheses, the results suggest the importance of harmonious involvement with professional activity, its potential on positive emotional experiences and perception of competence to face professional challenges.

ABSTRACT

(AU)

ABSTRACT

INTRODUCTION: Fertility problems are the main reasons for culling dairy cows. Diseases of the female genital tract are also often the cause of antibiotic or hormonal treatments in bovine practices. The use of medicinal plants could expand the available therapeutics. The aim of the work was to analyze historical literature before the introduction of antibiotics in veterinary reproductive medicine. Five books in German language, published in Germany and Switzerland between 1878 and 1921, and one handwritten therapy booklet by the rural veterinarian Carl Ammann-Honegger (1879-1960) were systematically examined regarding the descriptions (AW) on gynecological diseases. The herbal and additional ingredients of the recipe, the target animal species, the type of administration and the indication were recorded in detail for each AW. The six literature sources contained a total of 103 AW (79 administered orally, 13 locally, and 11 both orally and locally). Almost two thirds of the AW (61) were based on a mixture of different plants (two to seven plants), and one third of the AW (31) on a single plant. A total of 55 plants were recorded. The most frequently mentioned medical plants were plants of the genus Juniperus (J. communis L. (19 AW), J. sabina L. (13 AW)) and Linum usitatissimum L. (18 AW), Matricaria chamomilla L. (13 AW) and Gentiana lutea L. (12 AW). The treatment of the Retentio secundinarum was the most frequently mentioned indication (44 AW), followed by parturition preparation (17 AW) and endometritis treatment (15 AW). The most frequently recorded plants can be divided based on their ingredients and their effect into (a) energy- and protein-rich forage plants, (b) generally appetizing, digestive- and metabolism-enhancing plants, (c) medical plants with a specific gynecological organotrophic effect and (d) according to current knowledge, predominantly toxic plants. Besides the antimicrobial active immunity to defence against bacterial infections, a stable barrier funcion of the endometrium contributes to uterine health. The plants classified under (a) - (c) have at least the potential for a positive effect on the immune system and the endometrial barrier function and thus contribute indirectly to the uterine health.

ABSTRACT

I present a methodological experience of doing research guided by affections, inspired by the "escrevivência" of the writer, academic and activist Conceição Evaristo. In this article, I will try to expose the methodological flow that took place when I was writing my thesis believing that this was an affective-methodological path. This methodological flow proves to be relevant as it shows another form of knowledge construction, founded on counter-hegemonic bases, namely: the choice of a subjective analysis category (affections) and the development of a subjective analysis; the integration between the person who researches and the researched material; the interdisciplinarity as a way of expanding the look and, therefore, the research; the connection between affections and writing

ABSTRACT

The Network of Rural Women's Groups and the Agroecology Network of Uruguay are presented, analyzed as affective and supportive community networks that burst into the public sphere during the last decades, in search of collective solutions for common needs. Both networks are characterized by claiming rights and questioning instituted, particularly in relation to environmental and gender issues. From qualitative action research methodologies, a score of individual and collective in-depth interviews, six workshops, participant observation in daily spaces of the networks and the collection of secondary information were carried out. The article analyzes the tensions between instituted and instituting in both networks, from a point of view of the affects in the production of the common. The relevance of the study of this type of networks acquires particular relevance in the current context of economic, social and health crisis, since among the main findings we find that participation allows to politicize existence and enables alternative forms of subjectivation to the hegemonic ones.

ABSTRACT

Background: Known in the past for its toxic aspect as the main urban pollutant, in the last few decades, ozone has been gaining greater visibility for its possible antimicrobial, antiviral, and antioxidant effects when used in human dermatological pathologies. Despite the reports of clinical benefits, the standard dosage for clinical efficacy and safety are yet not clear, nor are its means of application and its true acting mechanism. Objective: We conducted a review to determine the efficacy and safety of ozone therapy for a variety of dermatological conditions. Methods: We considered clinical trials (both randomized and non-randomized) published between December 2020 and March 2021 as long as they provided some PICO information, i.e., population (P), intervention (I), and study design. The skin dermatological conditions researched were: acne, dermatitis, psoriasis, systemic sclerosis, herpes, aging, ulcers, and skin scarring. Results: A total of 326 articles were identified and 150 remained after duplicates were removed. After titles, abstracts and full articles were read, 17 articles were included in the systematic review (with 643 patients). Conclusion: Ozone therapy seems promising for some dermatological conditions; however, the articles included in this review had methodological limitations and did not sufficiently demonstrate sound evidence for safe therapy. Therefore, more studies with better methodological standards and longer-term assessments of side effects should be conducted to achieve better standards and safety in ozone therapy for dermatological conditions.

ABSTRACT

ABSTRACT Introduction: COVID-19 brought with it multiple effects, especially in the field of health. These manifest progressively in those who suffer from it in relation to the symptoms presented, which become visible in the short and long term at a physical and mental level. Objective: To identify the physical and psychological effects caused by COVID-19 in physically active people before the disease, during the period from January to May 2021. Method: Was used a descriptive-interpretative qualitative methodology. A sample of 5 adults from 27 to 66 years of age from Juchitán, Oaxaca, Mexico was used on the study; who were selected based on the following criteria: having been COVID-19 carriers, level of severity (mild/moderate) and have performed constant physical activity before the infection. The techniques applied for collecting information were: the interview and the questionnaire. Results: A drastic decrease was evidenced in relation to the components of physical activity (type, frequency, intensity and duration), as well as the prevalence of psychological affectations: stress, depression, anxiety and fear. Regarding the effects at the physical level, weight loss and fatigue (100%) stood out, followed by hair loss (80%) in those studied. Conclusions: The decrease in physical activity is a consequence of the disease itself, while the psychological effects are the result of the uncertainty generated by the disease.

ABSTRACT

Over the last 10 years, there has been a growing interest in the relationship between gut microbiota, the brain, and neurologic-associated affections. As multiple preclinical and clinical research studies highlight gut microbiota's potential to modulate the general state of health state, it goes without saying that gut microbiota plays a significant role in neurogenesis, mental and cognitive development, emotions, and behaviors, and in the progression of neuropsychiatric illnesses. Gut microbiota produces important biologic products that, through the gut-brain axis, are directly connected with the appearance and evolution of neurological and psychiatric disorders such as depression, anxiety, bipolar disorder, autism, schizophrenia, Parkinson's disease, Alzheimer's disease, dementia, multiple sclerosis, and epilepsy. This study reviews recent research on the link between gut microbiota and the brain, and microbiome's role in shaping the development of the most common neurological and psychiatric illnesses. Moreover, special attention is paid to the use of probiotic formulations as a potential non-invasive therapeutic opportunity for prevention and management of neuropsychiatric-associated affections.

ABSTRACT

Human fetal progenitor tenocytes (hFPT) produced in defined cell bank systems have recently been characterized and qualified as potential therapeutic cell sources in tendon regenerative medicine. In view of further developing the manufacture processes of such cell-based active pharmaceutical ingredients (API), the effects of hypoxic in vitro culture expansion on key cellular characteristics or process parameters were evaluated. To this end, multiple aspects were comparatively assessed in normoxic incubation (i.e., 5% CO2 and 21% O2, standard conditions) or in hypoxic incubation (i.e., 5% CO2 and 2% O2, optimized conditions). Experimentally investigated parameters and endpoints included cellular proliferation, cellular morphology and size distribution, cell surface marker panels, cell susceptibility toward adipogenic and osteogenic induction, while relative protein expression levels were analyzed by quantitative mass spectrometry. The results outlined conserved critical cellular characteristics (i.e., cell surface marker panels, cellular phenotype under chemical induction) and modified key cellular parameters (i.e., cell size distribution, endpoint cell yields, matrix protein contents) potentially procuring tangible benefits for next-generation cell manufacturing workflows. Specific proteomic analyses further shed some light on the cellular effects of hypoxia, potentially orienting further hFPT processing for cell-based, cell-free API manufacture. Overall, this study indicated that hypoxic incubation impacts specific hFPT key properties while preserving critical quality attributes (i.e., as compared to normoxic incubation), enabling efficient manufacture of tenocyte-based APIs for homologous standardized transplant products.

ABSTRACT

This research proposes to investigate the concept of the body according to the philosopher Gilles Deleuze and its possible interfaces with the clinic, using as intercessors some of his dialogues with Nietzsche and Espinosa. For this, we sought the definitions of body for Nietzsche and Espinosa. For the first, the body is a relation of forces, and for the second, it is characterized by its power of affection. Based on Deleuze's reading of them, corporality can be understood as processuality, permeated by the intensive states that pass through bodies. Deleuze's theoretical encounters make the clinic think about how the body can be an instrument of self-invention, as this is where the processes of subjectivity occur. Experimentation, therefore, constitutes an ethical compromise of a clinic that is invented at each meeting. Therefore, the potency to cast the bodies for the exercise of creation is problematized.(AU)

ABSTRACT

Abstract This article reflects on the importance of affections in pedagogical action, to promote emotional education. In the educational perspective, it is expected that the school, in its praxis in the different teaching spaces, can promote excellence on learning through processes in which the student appropriates, in a self-regulated way, the cognitive contents. Based on both Vygotsky and Walloon's theories, this reflective contribution emphasizes the scope and relevance of this topic as part of the professional agenda of all those involved in educational issues, promoting the coexistence of affect in teaching, and learning process. Behaviors and attitudes are the essence of the pedagogical relationship and convey a great affective impact on students, either positive or negative, depending on the way these interactions are experienced. Considering the significance of self-regulated learning in the school setting, it can be concluded that affection is the main foundation of pedagogical relationship, constituting an essential dimension of education for developing emotional skills in a future society.

ABSTRACT

ABSTRACT

INTRODUCTION: During the Coronavirus (COVID-19) pandemic, a set of daily stressors are being experienced, all this affects people's mental health, leading them to have a set of emotional disturbances. Little is known about how people's age can influence their emotional well-being in the face of prolonged stress generate by the pandemic. OBJECTIVE: To clarify the presence of emotional aspects such as emotional expressiveness and the frequency of positive and negative affections in people with different age in times of crisis. METHODS: The final sample included 297 Chileans between 22 and 68 years old (M = 38.51, SD = 13.85), recruited through an online survey with the appropriate written informed consent. The study was carried out when the pandemic was spreading in Chile. RESULTS: The findings revealed age differences in emotional expressivity and the type of affections experienced. The expression of emotions was more affected by negative affections, the age and the gender of the people. While the avoidance of this emotional expression, by age and affections both positive and negative. Age was a significant predictor of emotional expressiveness. DISCUSSION: Findings suggests that the associations between both variables, varied according to the age group of the people. Furthermore, this finding proposes that although older people are facing the persistent and serious threat of COVID-19, they show better emotional functioning. Which would help to better understand the interaction of both positive and negative life experiences in times of crisis.

ABSTRACT

The irradiation of non-malignant diseases, essentially for anti-inflammatory purpose, have been largely proposed and performed worldwide until the 1970-80s. At that time, the better assessment of the radio-induced malignancies, essentially in children and young patients, as well as the efficacy of the new anti-inflammatory drugs (steroids and non-steroids), led to the almost disappearance of those techniques, at least in France. In contrast, our German colleagues are still going on treating about 50,000 patients per year for non-malignant (more or less severe) diseases. After a short historical overview, the present article suggests that we were possibly going too far in the rejection of those low-dose irradiations for benign lesions. The recent emergence of new preclinical data, the better understanding of the risk of radio-induced secondary tumours (almost nil in the elderly), and the severity of some situations, such as the cytokine storm of the COVID-19, should probably lead us to reconsider those low - and sometimes very low (less than 1Gy) - irradiations for well-selected indications in the elderly.

ABSTRACT

OBJECTIVES: To evaluate the prevalence and factors associated with dermatological conditions and to study the peculiarities of comorbidity between psychiatric disorders and dermatological conditions among medical students in Morocco. METHODS: This is a multicenter, descriptive, and analytical cross-sectional study over a period of 4 weeks during the month of February 2019. RESULTS: Six hundred and five students from the seven faculties of medicine of Morocco were included. Dermatological conditions were present in 15.5% (94) of students, acne being the most common condition (10.7%), followed by psoriasis in 1.65% of students as well as urticaria in 1.65%. Psychiatric disorders were present in 28% (168) of the students depression being the most common disorder (10.4%), followed by anxiety disorders (7.8%), comorbidity between depression and anxiety disorder (6.94%), and psychotic disorders (2.46%). Dermatological diseases were significantly and independently associated with the presence of a psychiatric disorder (0.0000) and marital status (P=0.0103). Suicide attempts and substance use are more common among medical students who have an association between psychiatric disorder and dermatological pathology compared to students with isolated dermatological conditions or students with isolated psychiatric disorders. CONCLUSION: Dermatological and psychiatric conditions are frequent and underestimated among medical students in Morocco. Dermatologic and psychiatric comorbidity is associated with a higher prevalence of psychoactive substance use and suicide attempts.
