ABSTRACT

In this case series, the simultaneous occurrence of Wernicke's encephalopathy (WE) and dry beriberi was reported in three patients who underwent vertical sleeve gastrectomy (VSG) between May 2021 and May 2023. All patients were obese women who underwent vertical sleeve gastrectomy (VSG) without immediate postoperative complications, but two weeks later, hyperemesis and subsequent encephalopathy with ocular movement abnormalities and weakness were observed over the following thirty days. Patients were referred to neurology, where due to the high suspicion of WE, thiamine replacement therapy was initiated; meanwhile, diagnostic neuroimaging and blood tests were conducted. Neurological and psychiatric evaluations and neuroconduction studies were performed to assess the clinical evolution and present sequelae. One year after diagnosis, all patients exhibited affective and behavioral sequelae, anterograde memory impairment, and executive functioning deficits. Two patients met the criteria for Korsakoff syndrome. Additionally, peripheral nervous system sequelae were observed, with all patients presenting with sensorimotor polyneuropathy. In conclusion, Wernicke's encephalopathy requires a high diagnostic suspicion for timely intervention and prevention of irreversible sequelae, which can be devastating. Therefore, raising awareness among medical professionals regarding the significance of this disease is essential.

ABSTRACT

BACKGROUND: A progressive decrease in spontaneous locomotion with repeated exposure to a novel environment has been assessed using both within and between-session measures. While both are well-established and reliable measurements, neither are useful alone as methods to concurrently assess treatment effects on acquisition and retention of habituation. NEW METHOD: We report a behavioral method that measures habituation by combining the within and between measurements of locomotion. We used a 30 min session divided into 6 five min blocks. In the first novel environment session activity was maximal in the first 5 min block but was reduced to a low level by the sixth block, indicative of within-session habituation. Using 8 daily sessions, we showed that this terminal block low level of activity progressed incrementally to the first block to achieve complete habituation. RESULTS/COMPARISON WITH EXISTING METHODS: Within-session activity across sessions was used to identify different stages of between session habituation. It was then possible to assess drug treatment effects from partial to complete habituation, so that treatment effects on retention of the previously acquired partial habituation, expressed as a reversion to an earlier within session habituation pattern (retrograde amnesia assessment), as well as the effects on new learning by the failure in subsequent sessions to acquire complete between-session habituation (anterograde amnesia assessment). CONCLUSIONS: The use of spontaneous motor activity to assess learning and memory effects provides the opportunity to assess direct treatment effects on behavior and motor activity in contrast to many learning and memory models.

ABSTRACT

Introducción: La edad es el factor de riesgo más importante asociado al deterioro cognitivo. Al respecto, la pérdida de la memoria resulta un motivo frecuente y creciente de atención en las consultas médicas del nivel primario. Sin embargo, la disfunción cognitiva en los ancianos suele ser una de las afecciones que transcurren usualmente sin un diagnóstico oportuno. Objetivo: Determinar la prevalencia del deterioro cognitivo leve de tipo amnésico en ancianos de un área de salud. Métodos: Se realizó un estudio observacional, descriptivo y transversal de 203 ancianos pertenecientes al área de salud del Policlínico Docente Ramón López Peña de Santiago de Cuba, desde junio hasta diciembre de 2022. Para diagnosticar a los pacientes con disfunción cognitiva leve se utilizaron los criterios de Petersen. Resultados: Se halló que 36,9 % de los ancianos presentaron deterioro cognitivo leve de tipo amnésico, con predominio del sexo masculino (43,6 %) y del nivel de escolaridad primario (50,0 %), seguido del medio (40,3 %). Conclusiones: El deterioro cognitivo leve de tipo amnésico prevaleció en más de un tercio de la muestra estudiada; frecuencia que aumentó según la edad. Asimismo, se detectó una mayor incidencia del sexo masculino y de los ancianos con escolaridad primaria y media finalizadas.


Introduction: Age is the most important risk factor associated with cognitive deterioration. In this regard, loss of memory is a frequent and growing reason for attention in medical consultations of primary care. However, cognitive dysfunction in the elderly is one of the affections that usually go without an opportune diagnosis. Objective: To determine the prevalence of the amnestic mild cognitive impairment in elderly of a health area. Methods: An observational, descriptive and cross-sectional study of 203 elderly, belonging to the health area of Ramón López Peña Teaching Polyclinic was carried out in Santiago de Cuba, from June to December, 2022. To diagnose the patients with light cognitive dysfunction the Petersen approaches were used. Results: It was found that 36.9% of the elderly presented amnestic mild cognitive impairment, with a prevalence of male sex (43.6%) and the primary school level (50.0%), followed by the middle level (40.3%). Conclusions: Amnestic mild cognitive impairment prevailed in more than a third of the studied sample; frequency that increased according to the age. Likewise, a higher incidence of male sex and the elderly with concluded primary and middle schooling was detected.

ABSTRACT

c-Jun N-terminal kinase (JNK) phosphorylates the transcription factor c-Jun in response to stress stimuli and contributes to both hippocampal synaptic plasticity and memory processing in mammals. Object recognition memory (ORM) is essential for remembering facts and events. In rodents, ORM consolidation and reconsolidation require a functional hippocampus. However, the possible involvement of hippocampal JNK on ORM processing has not yet been studied. Here we show that when injected into dorsal CA1 5 min, but not 6 h, after training adult male rats in the novel object recognition learning task, the JNK inhibitor SP600125 impaired ORM for at least 7 days without affecting exploratory activity, short-term ORM retention, or the functional integrity of the hippocampus. SP600125 did not hinder ORM retention when given in CA1 after a memory reactivation session carried out 24 h post-training in the presence of the same two objects presented during the training session, but caused time-dependent amnesia when one of the objects presented at training was replaced by a different but behaviorally equivalent novel one. Taken together, our results indicate that hippocampal JNK activity is necessary for ORM consolidation and reconsolidation but not for ORM recall or short-term retention.

ABSTRACT

Infantile amnesia, the inability to form long-lasting episodic memories, is a phenomenon extensively known but with no clear understanding of its origins. However, a recent study showed that high rates of hippocampal postnatal neurogenesis degrade episodic-like memories in infants a few days after memory acquisition. Additionally, new studies indicate that exposure to an enriched environment in mice leads to high hippocampal neurogenesis in their offspring. Nevertheless, it is still unclear how this intergenerational trait affects the persistence of hippocampal memories. Therefore, we evaluated spatial memory retention in the offspring of enriched female mice after weaning to address this question. Ten days after spatial learning, we tested memory retention, observing that the offspring of enriched dams increased spatial memory failure; this finding correlates with high proliferation rates in the hippocampus. Furthermore, we evaluated the causal relationship between postnatal hippocampal neurogenesis and memory failure using the antiproliferative drug Temozolomide (TMZ), which rescued spatial memory retrieval. Finally, we evaluated neuronal activity in the hippocampus quantifying the cells expressing the immediate early gene c-Fos. This evaluation showed engram modifications between groups. This neural activity pattern indicates that the high neurogenesis rates can modify memory engrams and cognitive performance. In conclusion, the inherited increase of hippocampal neurogenesis by enriched dams leads to plastic changes that exacerbate infantile amnesia in a spatial task.

Studies investigating amnesic patients have shown the involvement of the medial temporal lobe during working memory (WM) tasks, especially when multiple items or features have to be associated. However, so far, no study has examined the relationship between episodic memory and WM components in patients with amnesia for comprehensive neuropsychological evaluation. Objective: The objective of this study was to investigate whether the null retention relates to deficits in the episodic buffer (EB) or the central executive (CE) components of WM. Methods: This study included 15 amnesic patients with mixed etiologies and 13 matched healthy controls. These 15 amnesic patients with mixed etiologies were divided into two subgroups: NUL subgroup (n=7) patients whose raw score was 0 (zero) on the Logical Memory delayed recall test and MOR subgroup (n=8) patients who recalled at least 1 item. The EB was assessed by complex span tasks, and the CE was assessed by random number generation (RNG) test. Results: EB tasks were impaired in both subgroups compared with controls. RNG was impaired in NUL (p=0.03), but not in MOR (p=0.99), subgroup. Conclusions: CE impairment hampers the retrieval mode action, preventing it from initiating the mental reconstruction of the context in which the to-be-remembered information was presented minutes ago.


Estudos que investigaram pacientes amnésicos demonstraram envolvimento do lobo temporal medial durante tarefas de memória de trabalho, especialmente quando vários itens ou características devem ser associados. No entanto, até o momento, não há estudos que tenham examinado a relação entre memória episódica e os subcomponentes da memória de trabalho em pacientes com amnésia por meio de avaliação neuropsicológica ampla. Objetivo: Investigar se a retenção nula está relacionada a déficits no buffer episódico ou nos componentes do executivo central da memória operacional. Métodos: Quinze pacientes amnésicos com etiologias mistas foram divididos em dois subgrupos: subgrupo NUL (n=7), de pacientes cuja pontuação bruta foi 0 (zero) na memória lógica tardia, e subgrupo MOR (n=8), de pacientes que recordaram pelo menos um item; além de 13 controles saudáveis pareados. O retentor episódico foi avaliado por tarefas de spam complexo e o executivo central com geração aleatória de números. Resultados: As tarefas do retentor episódico estavam prejudicadas em ambos os subgrupos em comparação com os controles. O teste de geração aleatória de números foi prejudicado em NUL (p=0,03), mas não no subgrupo MOR (p=0,99). Conclusões: O comprometimento do executivo central dificulta a ação do modo de recuperação, impedindo-o de iniciar a reconstrução mental do contexto em que a informação a ser lembrada foi apresentada, minutos antes.

ABSTRACT

Theta is one of the most prominent extracellular synchronous oscillations in the mammalian brain. Hippocampal theta relies on an intact medial septum (MS) and has been consistently recorded during the training phase of some learning paradigms, suggesting that it may be implicated in hippocampus-dependent long-term memory processing. Object recognition memory (ORM) allows animals to identify familiar items and is essential for remembering facts and events. In rodents, long-term ORM formation requires a functional hippocampus but the involvement of the MS in this process remains controversial. We found that training adult male Wistar rats in a long-term ORM-inducing learning task involving exposure to two different, but behaviorally equivalent novel stimuli objects increased hippocampal theta power, and that suppressing theta via optogenetic MS inactivation caused amnesia. Importantly, the amnesia was specific to the object the animals were exploring when the MS was inactivated. Taken together, our results indicate that the MS is necessary for long-term ORM formation and suggest that hippocampal theta activity is causally linked to this process.

BACKGROUND AND AIMS: Transient global amnesia (TGA) is a clinical syndrome characterized by sudden anterograde amnesia not accompanied by other neurological symptoms. There is no consensus on the underlying pathophysiological mechanism. However, diffusion-weighted imaging (DWI) of the magnetic resonance imaging (MRI) has demonstrated hippocampal lesions in as many as 50% of cases. This paper describes a series of patients with TGA and hippocampal lesions. METHODS: This study assessed vascular risk factors in patients older than age 18 admitted to the Hospital Universitario San Ignacio, Bogota, Colombia, from May 2017 to June 2020 with a diagnosis of TGA and evidence of hippocampal ischemic lesion on 3 Tesla brain MRI. RESULTS: The authors identified 36 patients, 72.2% female, with mean age 62 years. Cardiovascular risk factors, most frequently high blood pressure, carotid disease, and dyslipidemia, were present in 75% of these patients. Hippocampal lesions were unilateral in 80% of cases, with median size 2.5 mm, most frequently located at the hippocampal body. Approximately 14% of patients also presented acute ischemic lesions in locations other than the hippocampus. CONCLUSIONS: TGA is a clinical entity previously considered to have undetermined etiology. The present study used brain MRI to identify a group of patients with hippocampal ischemic lesions, finding associated vascular risk factors in a high proportion of them.

ABSTRACT

RESUMEN Se señala la importancia de reconsiderar el trabajo de Pierre Janet en trauma y amnesia disociativa, como elementos importantes en el estudio actual del trastorno de estrés postraumático (TEPT). Los objetivos de esta revisión histórica no sistemática y comparativa son: a) explicar, desde la perspectiva de Janet: el concepto de Idea Fija, que hoy denominamos trauma b) describir los diferentes tipos de amnesia en las cuales se basa el DSM para su clasificación actual y c) a través de un caso clínico descrito por Janet, mostrar su concordancia con lo que actualmente diagnosticamos como TEPT complejo, que denominó en su época: "una de las formas que puede tomar la histeria después de un accidente emocional". Esta revisión se basa principalmente en algunos capítulos originales de Janet publicados en Francés.


The present article gives emphasis in reconsidering the work of Pierre Janet in trauma and dissociative amnesia, as important aspects for the contemporary study of posttraumatic stress disorder (PTSD). The purpose of this non systematic historical review is to explain the following ideas under Janet´s perspective: a) the concept of fixed idea, denominated in our contemporary nomenclature as trauma, b) to describe the different types of amnesia described by Janet and its relation with DSM clasiffication for this penomenon, and c) to show through the analysis of a clinical case described by Janet, how a concordance exists between the current denomination of complex PTSD and what Janet nominated "one of the forms in which hysteria can presents after an emotional accident". This article is mainly based in some original chapters from Janet´s work published in French.

ABSTRACT

The aim of the present study was investigate the binding affinity of 5-((4-methoxyphenyl)thio)benzo[c][1,2,5]thiadiazole (MTDZ) with acetylcholinesterase (AChE). We also evaluated the effect of MTDZ against scopolamine (SCO)-induced amnesia in mice and we looked at the toxicological potential of this compound in mice. The binding affinity of MTDZ with AChE was investigated by molecular docking analyses. For an experimental model, male Swiss mice were treated daily with MTDZ (10 mg/kg, intragastrically (i.g.)) or canola oil (10 ml/kg, i.g.), and induced, 30 min later, with injection of SCO (0.4 mg/kg, intraperitoneally (i.p.)) or saline (0.9%, 5 ml/kg, i.p.) daily. From day 1 to day 10, mice were submitted to the behavioral tasks (Barnes maze, open-field, object recognition and location, Y-maze and step-down inhibitory avoidance tasks), 30 min after induction with SCO. On the tenth day, the animals were euthanized and blood was collected for the analysis of biochemical markers (creatinine, aspartate (AST), and alanine (ALT) aminotransferase). MTDZ interacts with residues of the AChE active site. SCO caused amnesia in mice by changing behavioral tasks. MTDZ treatment attenuated the behavioral changes caused by SCO. In ex vivo assay, MTDZ also protected against the alteration of AChE activity, reactive species (RS) levels, thiobarbituric acid reative species (TBARS) levels, catalase (CAT) activity in tissues, as well as in transaminase activities of plasma caused by SCO in mice. In conclusion, MTDZ presented anti-amnesic action through modulation of the cholinergic system and provided protection from kidney and liver damage caused by SCO.

ABSTRACT

INTRODUCTION: Dissociative amnesia (DA) is a retrograde amnesia characterized by an alteration in episodic memory. AIM: Establish the neural bases which underlie the development of dissociative amnesia. METHODS: Systematic and evaluative bibliographic review of qualitative type. RESULTS: The bibliography found suggested functional inhibition in the hippocampus, amygdala, temporal lobes, prefrontal cortex and thalamus. Also, hypoglycemia was observed in right cerebral cortex, at the fronto-temporal junction. An inhibition in the potential action P300 was also stated. CONCLUSIONS: There is enough evidence to say that dissociative amnesia is an objectifiable biologically based pathology. There is a need to review the current conceptualization of this syndrome and to establish new criteria that would allow us to distinguish DA from organic amnesias.

ABSTRACT

The incidence of peri-procedural amnesia following procedural sedation in children is unclear and difficult to determine. This study aimed to apply quantitative and qualitative approaches to better understand amnesia following dental sedation of children. After Institutional Review Board Approval, children scheduled for sedation for dental procedures with oral midazolam (OM), oral midazolam and ketamine (OMK), or intranasal midazolam and ketamine (IMK) were recruited for examination of peri-procedural amnesia. Amnesia during the dental session was assessed using a three-stage method, using identification of pictures and an animal toy. On the day following the sedation, primary caregivers answered two questions about their children's memory. One week later, the children received a semi-structured interview. Behavior and level of sedation during the dental session were recorded. Quantitative data were analyzed using descriptive statistics and comparison tests. Qualitative data were analyzed using content analysis. Triangulation was used. Thirty-five children (age: 36 to 76 months) participated in the quantitative analysis. Most children showed amnesia for the dental procedure (82.9%, n = 29/35) and remembered receiving the sedation (82.1%, n = 23/28 for oral administration; 59.3%, n = 16/27 for intranasal administration). The occurrence of amnesia for the dental procedure was slightly higher in the oral midazolam group compared with the other groups (44.8%, n = 13/29 for OM, 13.8%, n = 4/29 for OMK, and 41.4%, n = 12/29 for IMK). Twenty-eight children participated in the qualitative approach. The major theme identified was that some children could remember their procedures in detail. We conclude that peri-procedural amnesia of the dental procedure was common following sedation.

OBJECTIVE: Acute administration of benzodiazepines is considered a pharmacological model of general organic anterograde amnesias (OAA). We sought to determine which type of amnesia these drugs best model by comparing the effects of diazepam with those reported in amnesiacs regarding working memory capacity (WMC), susceptibility to retroactive interference (RI), and accelerated forgetting. METHODS: In this double-blind, parallel-group design study, 30 undergraduates were randomly allocated to acute oral treatments with 15 mg diazepam or placebo. WMC and story recall were assessed pre- and post-treatment. Story presentation was succeeded by 10 min of RI (spotting differences in pictures) or minimal RI (doing nothing in a darkened room). Delayed story recall was assessed under diazepam and 7 days later in a drug-free session to assess accelerated forgetting. RESULTS: Recall of stories encoded under diazepam, whether reactivated or not, was severely impaired (anterograde amnesia). However, diazepam did not impair WMC, increase susceptibility to RI, or accelerate forgetting. CONCLUSIONS: Diazepam's amnestic effects mirror those in patients with probable severe medial temporal damage, mostly restricted to initial consolidation and differ from other OAA (Korsakoff syndrome, frontal, transient epileptic, posttraumatic amnesia, and most progressive amnesias) in terms of WMC, susceptibility to RI and accelerated forgetting.

RESUMEN Los medios de contraste iodados pueden presentar diferentes efectos adversos, siendo los más frecuentes daño renal y reacciones de hipersensibilidad. La ceguera cortical y amnesia global transitoria son complicaciones neurológicas raras descritas tras la administración de medios de contraste. Reportamos el caso de un paciente de 63 años, hipertenso que acude por cianosis, frialdad del segundo y tercer dedo de mano izquierda. Es sometido a arteriografía de miembro superior, usando 50 mL de iopamidol e inmediatamente desarrolla cefalea occipital asociada a visión borrosa bilateral que progresa rápidamente a ceguera; la tomografía cerebral reveló hiperdensidades subaracnoideas occipitales, sin efecto de masa. A las dieciocho horas del evento, el paciente presenta amnesia anterógrada con persistencia de amaurosis. Su tomografía cerebral de control no muestra trastornos focales. El paciente cursa con evolución favorable, la amaurosis bilateral y la cefalea ceden a las treinta horas del evento y es dado de alta sin mayores eventualidades a los diez días.


ABSTRACT Iodinated contrast substances may cause different adverse events, most frequently renal involvement and hypersensitivity reactions. Transient cortical blindness and global amnesia are rarely described neurological complications after the administration of contrast substances. We report the case of a 63-year old patient with high blood pressure who presented with cyanosis and cold sensation affecting the second and third digits of his left hand. The patient underwent an arteriography of the left arm, where 50-mL of iopamidol was used as a contrast substance, and he immediately developed occipital headache associated to bilateral blurred vision that rapidly progressed to blindness. A cerebral computed tomography (CT) scan showed occipital subarachnoid hypodense areas, with no mass effect. Eighteen hours after the event, the patient showed retrograde amnesia with persistent amaurosis. His control cerebral CT scan did now show any focal disorder. The patient had a good progression, both bilateral amaurosis and headache subsided thirty hours after the event, and the patient was uneventfully discharged after ten days.

ABSTRACT

ABSTRACT The current article describes the case of a man who claimed amnesia in relation to a sexual crime he had allegedly committed. Psychiatric examination concluded that the individual was feigning amnesia. Claimed amnesia of a criminal offense is one of the most commonly feigned symptoms in the forensic medical setting. It is thus necessary to rule out organic or psychogenic causes of amnesia and always consider feigned amnesia in the presence of psychopathological alterations that do not reflect classically known syndromes.


RESUMO O presente artigo descreve o caso de um homem que alegou amnésia ao fato da denúncia de crime sexual que lhe foi imputada. A perícia psiquiátrica concluiu tratar-se de simulação. A alegação de amnésia da ofensa criminosa é um dos sintomas mais comumente simulados no ambiente pericial. Portanto, devem-se excluir as causas de amnésia orgânica ou psicogênica e sempre considerar a amnésia simulada na presença de alterações psicopatológicas que não configuram quadros sindrômicos classicamente conhecidos.

ABSTRACT

Objetivo: identificar a causa da dor psicopatológica antes do desenvolvimento da doença de Alzheimer. Método: estudo observacional e descritivo, do tipo relato de experiência, realizado no Hospital Escola Portugal Ramalho, entre 2016 e 2019. Agrupou-se os pacientes conforme as causas dos tipos de esquecimento, com construção de gráficos, de acordo com as principais teorias acerca da gênese da doença de Alzheimer. Resultados: os grupos de pacientes foram submetidos à anamnese e ao teste de Folstein. Percebeu-se que os tratamentos dependem se a amnésia é relacionada ao humor; se ocorre por sequelas vasculares e endocrinológicas; ou pacientes com demência de Alzheimer diagnosticada. Conclusão: a causa da dor psicopatológica se baseia no esquecimento involuntário que gera a não aceitação ou no medo de relembrar situações traumáticas. Embora essas causas variam de acordo com as hipóteses que levam ao dano cognitivo e mnêmico.


Objective: to identify the cause of psychopathological pain before the development of Alzheimer's disease. Method: observational and descriptive study, experience report type, carried out at the Portugal Ramalho Teaching Hospital, between 2016 and 2019. The patients were grouped according to the causes of the types of forgetfulness, with construction of graphs, according to the main theories about the genesis of Alzheimer's disease. Results: the groups of patients were submitted to anamnesis and Folstein's test. It was noticed that the treatments depend on whether the amnesia is related to mood; if it occurs due to vascular and endocrinological sequelae; or patients with diagnosed Alzheimer's dementia. Conclusion: the cause of psychopathological pain is based on involuntary forgetfulness that generates non-acceptance or on the fear of remembering traumatic situations. However these causes vary according to the hypotheses that lead to cognitive and mnemic damage.

ABSTRACT

The isoquinoline 7-fluoro-1,3-diphenylisoquinoline-1-amine (FDPI) has been studied due to its multitarget properties, such as modulation of GABAergic and glutamatergic systems, antioxidant, and anti-inflammatory. This study investigated the contribution of oxidative stress, nuclear factor (erythroid-derived 2)-like 2 (Nrf2)/heme oxygenase (HO-1) signaling, and the cholinergic system to the anti-amnesic action of FDPI in mice. Adult male Swiss mice received FDPI for 5 days (5-25 mg/kg, i.g.); the animals received scopolamine (1 mg/kg, i.p) from day 3-5. The vehicle-control group was carried out. Afterward, mice performed object recognition tests (ORTs). Scopolamine induced amnesia and cholinergic dysfunction by increasing the acetylcholinesterase (AChE) activity and content, decreasing the muscarinic M1 receptor levels in the prefrontal cortex and hippocampus of mice. This study reveals that scopolamine altered oxidative stress parameters differently in the prefrontal cortex and hippocampus of mice. Whereas the prefrontal cortex was susceptible to oxidative stress, none of the parameters evaluated was altered in the hippocampus of scopolamine-treated mice. FDPI at doses of 10 and 25 mg/kg had an anti-amnesic effect in the ORT tests. FDPI 10 mg/kg reversed the increase in the AChE activity and content, oxidative stress parameters, and modulated Nrf2/HO-1 signaling in the prefrontal cortex of scopolamine-exposed mice. Pearson's correlation analyses reinforced the contribution of the prefrontal cortical cholinergic system, oxidative stress as well as Nrf2/HO-1 signaling in the anti-amnesic effect of FDPI. Considering FDPI effects on the hippocampus, it was effective against the cholinergic dysfunction, AChE activity and content, and M1 receptor levels, which collectively could contribute to its anti-amnesic effect.
