ABSTRACT

Resumen El empiema necessitatis (EN) constituye una muy rara complicación de un empiema pleural en el cual la infección se extiende a los tejidos blandos adyacentes. La etiología por anaerobios es muy infrecuente y se da en el curso de infecciones crónicas. Se presenta el primer caso de empiema necessitatis por Campylobacter rectus. La identificación de este agente se efectuó por espectrometría de masas (MALDI-TOF MS) y su sensibilidad antimicrobiana se determinó por el método epsilométrico.


Abstract Empyema necessitatis (EN) is a very rare complication of a pleural empyema, in which the infection extends to adjacent soft tissues. Anaerobic bacteria are very rare etiologic agents of EN, which occurs in the course of chronic infections. We present the first case of empyema necessitatis caused by Campylobacter rectus. ABSTRACT

ABSTRACT Osteoarticular infections due to anaerobes are very rare in children, with the Fusobacterium genus being the most frequently isolated. The course is usually subacute and, although there are predisposing factors described, most patients do not present with them. Generally, joint fluid cultures are sterile since these microorganisms are very sensitive to contact with oxygen, so they require specific culture media. All of the above causes the diagnosis to be delayed, increasing the risk of long-term sequelae. However, the prognosis improves when treatment is started early. The case is presented of a 10-year-old patient who was admitted for 30 days due septic arthritis of the right hip caused by Fusobacterium nucleatum During the admission, he required three surgical interventions, and completed 6 weeks of effective antibiotic therapy, with a good outcome and remaining asymptomatic at the current time.


RESUMEN Las infecciones osteoarticulares por anaerobios son muy raras en los niños, siendo el género Fusobacterium el que se aisla con más frecuencia. El curso suele ser subagudo y, aunque hay factores predisponentes descritos, la mayoría de los pacientes no los presenta. Generalmente, los cultivos de liquido articular son estériles ya que estos microorganismos son muy sensibles al contacto con el oxigeno, por lo que precisan medios de cultivo específicos. Todo lo anterior hace que el diagnóstico se retrase y que el riesgo de secuelas a largo plazo aumente. Sin embargo, el pronóstico mejora cuando el tratamiento se inicia de modo precoz. Por todo ello, presentamos el caso de un paciente de 10 anos con una artritis séptica de cadera derecha por Fusobacterium nucleatum que permaneció ingresado 30 dias. Durante el ingreso precisó 3 intervenciones quirúrgicas y cumplió 6 semanas de antibioterapia efectiva, con buena evolución; permanece asintomático en el momento actual.

ABSTRACT

Las adenopatías cervicales benignas en lactantes son relativamente frecuentes, se definen como el aumento de volumen ganglionar de más de 1 cm, sin síntomas sistémicos y cuando están presentes, el término correcto es adenitis. Para su estudio, las adenitis se dividen en: locales, sistémicas, unilaterales, bilaterales, agudas, crónicas, y por edad, con diferentes etiologías. Se presenta el caso clínico de un lactante de 11 meses de edad con diagnóstico de adenitis cervical abscedada unilateral aguda, con cuadro de 72 h de evolución, con crecimiento constante a nivel cervical derecho, compromiso del estado general, fiebre y anorexia, por lo que se inician antibióticos de primera línea para los agentes bacterianos más frecuentes (Staphylococcus aureus y Streptococcus pyogenes), con evolución tórpida a las 48 h, por lo que se solicita ultrasonido cervical, ya que la familia no contaba con recursos para solicitar cultivo o tomografía, reportando el ultrasonido ganglio cervical de 3,5 cm de diámetro abscedado, por lo que se agrega cobertura para anaerobios, con respuesta muy favorable a las 24 h. Queda la duda del origen de los anaerobios en la paciente, sin antecedentes de importancia y en grupo etario diferente al afectado por esos gérmenes. Consideramos este caso interesante por su comportamiento atípico, para el enriquecimiento del ejercicio de la otorrinolaringología, recalcando el invaluable apoyo de la clínica y solo con un ultrasonido, ya que no siempre se tendrán todos los recursos disponibles, pero siguiendo las pautas de lo reportado en la literatura, se tuvo una resolución exitosa.


Benign cervical lymphadenopathies in infants are relatively frequent, they are defined as an increase in lymph node volume of more than 1 cm, without systemic symptoms, and when they are present, the correct term is adenitis. For its study, adenitis is divided into: local, systemic, unilateral, bilateral, acute, chronic, and by age, with different etiologies. An 11-month-old infant with a diagnosis of acute unilateral abscessed cervical adenitis, with a 72 h evolution, with constant growth at the right cervical level, fever and anorexia, for which first-line antibiotics were started to the most frequent bacterial agents (Staphylococcus aureus and Streptococcus pyogenes), with torpid evolution at 48 h, for which only cervical ultrasound is requested, since the family did not have the resources to request culture or tomography, reporting the cervical ganglion ultrasound of 3.5 cm of abscessed diameter, so coverage for anaerobes is added, with a very favorable response at 24 hrs. There remains the doubt of the origin of the anaerobes in the patient, without important antecedents and in an age group different from that affected by these germs. We consider this case interesting due to its atypical behavior, for the enrichment of the otolaryngology exercise, emphasizing the invaluable support of the clinic, and only with an ultrasound, since other clinical tools were not available, but following the guidelines of what is reported in literature, there was a successful resolution.

ABSTRACT

RESUMO O estudo da atividade metanogênica específica (AME) constitui uma importante ferramenta para o controle operacional de reatores anaeróbios, pois apresenta parâmetros de monitoramento da eficiência e estabilidade de um reator biológico. Nesse sentido, o presente trabalho visou determinar a AME de lodos provenientes de reatores anaeróbios de fluxo ascendente (UASB - upflow anaerobic slugde blanket) de duas indústrias cervejeiras (lodos A e B), uma de processamento de grãos (lodo C), uma leiteira (lodo D) e uma de processamento de couro (lodo E). Os ensaios foram conduzidos em batelada, em ambiente aclimatado a 35°C, utilizando um equipamento para monitorar a geração de biogás em função do tempo de digestão. Também foi realizada a qualificação do gás metano presente no biogás gerado pelos diferentes lodos. O maior valor de AME obtido e a maior porcentagem do gás metano produzido, para uma concentração de biomassa de 5,0 gSTV.L-1 e 4,0 gDQO.L-1 de glicose, foram para o lodo B: 0,36 gDQO-CH4.gSTV-1.d-1 e 58,9%, respectivamente.


ABSTRACT The study of Specific Methanogenic Activity (SMA) is an important tool for operational control of anaerobic reactors, as it presents parameters to monitor the efficiency and stability of a biological reactor. For this reason, the present article aimed to determine the SMA of sludges from UASB reactors, of two different brewing industries (sludges A and B), a grain processing industry (sludge C), a dairy industry (sludge D), and a leather processing industry (sludge E). Assays were performed in batch, at an acclimatized environment (35°C), using a device equipment to monitor the generation of biogas due to digestion time. Also, the quantity of methane gas was measured in the biogas generated by the different sludges. The best SMA value obtained and the greater percentage of methane gas generated, with a biomass concentration of 5.0 gSTV.L-1, and a glucose concentration of 4.0 gCOD.L-1, were for sludge B, with 0.36 gCOD-CH4.gSTV-1.d-1 and 58.9%, respectively.

Resumen Objetivo. Describir la microbiota que se encuentra en la cavidad bucal de caninos en condición de abandono de la Fundación Razas Únicas en el municipio de Chía -Cundinamarca. Métodos. Para el estudio se tomaron 29 muestras orales con escobillón a 23 caninos de la Fundación Razas Únicas del municipio de Chía - Cundinamarca. 23 muestras se recolectaron para identificación de bacterias aerobias y anaerobias facultativas, las cuales se transportaron en medio líquido tripticasa soya y 6 muestras para recuperación de bacterias anaerobias estrictas transportadas en medio VMGA-III. El aislamiento de los microorganismos se realizó en medios selectivos y la identificación con el sistema BD BBL™ Crystal™. Resultados. De las 29 muestras analizadas se aislaron 59 bacterias, entre ellas 15 géneros y 15 especies diferentes como; Escherichia coli, Proteus mirabilis, Citrobacter freundii, Enterobacter sakazakii, Enterobacter cloacae, Enterococcus durans, Enterococcus faecalis, Eikenella corrodens, Porphyromonas endodontalis, Fusobacterium spp y Capnocytophaga spp. De acuerdo con la revisión de la literatura, las bacterias anaerobias encontradas están principalmente relacionadas con enfermedad periodontal y las enterobacterias con contaminación oro-fecal.


Abstract Objective. To describe the microbiota found in the oral cavity of canines in condition of abandonment of the Razas Únicas Foundation in the municipality of Chía - Cundinamarca. Methods. For the study, 29 oral samples were taken with a brush from 23 canines from the Razas Únicas Foundation of the municipality of Chía - Cundinamarca. 23 samples were collected for identification of facultative aerobic and anaerobic bacteria, which were transported in soybean tripticase liquid medium and 6 samples for recovery of strict anaerobic bacteria transported in VMGA-III medium. The isolation of the microorganisms was carried out in selective media and identification with the BD BBL™ Crystal™ system. Results. From the 29 samples analyzed, 59 bacteria were isolated, including 15 genera and 15 different species such as Escherichia coli, Proteus mirabilis, Citrobacter freundii, Enterobacter sakazakii, Enterobacter cloacae, Enterococcus durans, Enterococcus faecalis, Eikenella corrodens, Porphyromonas endodontalis, Fusobacterium spp and Capnocytophaga spp. According to the review of the literature, the anaerobic bacteria found are mainly related to periodontal disease and the enterobacteria with oral-fecal contamination.

Periodontitis is an inflammatory response in a susceptible host caused by complex microbiota, predominantly composed of Gram-negative anaerobic bacteria. Aiming to characterize the subgingival bacterial microbiota associated with ovine periodontitis, polymerase chain reaction (PCR) was performed in subgingival periodontal pocket samples of 14 sheep with severe periodontitis and in subgingival sulcus biofilm of 14 periodontally healthy sheep in search mainly of Gram-negative and Gram-positive microorganisms considered important periodontopathogens. The most prevalent bacteria in the sheep with periodontal lesions were Tannerella forsythia (78.6%), Treponema denticola (78.6%), Fusobacterium nucleatum (64.3%), and Porphyromonas gingivalis (50%), whereas in the healthy sheep, F. nucleatum (42.8%) was the most often detected bacterium. Statistically significant differences were observed for Campylobacter rectus, Enterococcus faecium, Prevotella nigrescens, T. forsythia, and T. denticola (p<0.05) in the sheep with periodontitis in the comparison between groups. Aggregatibacter actinomycetemcomitans, Enterococcus faecalis, and Porphyromonas gulae were not detected in any of the samples analyzed. In conclusion, C. rectus, E. faecium, P. nigrescens, T. forsythia, and T. denticola were associated with severe lesions caused by ovine periodontitis, and F. nucleatum was the most prevalent microorganism in the subgengival sulcus biofilm of healthy sheep.(AU)

Com o objetivo de caracterizar a microbiota bacteriana subgengival associada à periodontite ovina foi realizada a reação em cadeia da polimerase (PCR) de amostras de biofilme subgengival de 14 ovinos com a enfermidade e 14 ovinos periodontalmente saudáveis, com destaque para micro-organismos Gram-negativos e Gram-positivos considerados importantes periodontopatógenos. As bactérias mais prevalentes em 14 animais com lesões periodontais foram Tannerella forsythia (78,6%), Treponema denticola (78,6%), Fusobacterium nucleatum (64,3%) e Porphyromonas gingivalis (50%). Entretanto, nos 14 ovinos sem lesões periodontais, F. nucleatum (42,8%) foi a bactéria mais detectada. ABSTRACT

RESUMO Nesta pesquisa, estudou-se o desempenho de três filtros biológicos anaeróbios com novos e diferentes meios de suporte em tempos de detenção hidráulica (TDH) distintos (5,5 e 11,1 horas), objetivando a remoção de algas de efluentes de lagoas de estabilização. O considerável desempenho no pós-tratamento (remoções de até 77,4%) pode ser atribuído à atuação dos mecanismos de sedimentação, retenção física e assimilação biológica. Foi desenvolvido um modelo de decaimento para clorofila com base na simplificação da equação de transporte de massa. O coeficiente global de decaimento foi descrito como uma função dos números de Reynolds e Weber, obtida por meio de ajuste aos dados experimentais com um coeficiente de determinação superior a 0,96. Para confirmação de que a função de Reynolds e Weber descreve bem o comportamento de k, foram realizadas simulações do impacto das seguintes variáveis na eficiência de remoção de algas: comprimento do filtro (altura do leito), diâmetro do filtro, diâmetro do meio suporte, porosidade, vazão, tensão superficial e da viscosidade do líquido. As simulações desenvolvidas demonstraram coerência nos resultados, reforçando que os números de Reynolds e Weber, assim como o modelo de fluxo em pistão, podem ser usados para descrever o processo de remoção de algas em filtros anaeróbios.


ABSTRACT In this research we studied the performance of three anaerobic biological filters with new and different support media in different hydraulic retention time (HRT): 5.5 and 11.1 hours, aiming at the removal of wastewater stabilization ponds of algae. The considerable performance in the post-treatment (removal of up to 77.4%) can be attributed to the actions of sedimentation mechanisms, physical restraint and biological assimilation. It has developed a model for chlorophyll decay from the simplification of mass transport equation. The overall decay coefficient was described as a function of Reynolds and Weber numbers, which was obtained through adjusting the experimental data with a determination coefficient greater than 0.96. To confirm that the Reynolds and Weber function describes well the k behavior, simulations were made of the impact of these variables on algae removal efficiency: filter length (bed height), filter diameter, the diameter of the support means, porosity, flow, surface tension and viscosity of the liquid. Simulations show consistency in the results, reinforcing that the Reynolds and Weber numbers and the flow model of piston may be used to describe the process of removing algae in anaerobic filters.

Introducción: el absceso de pulmón es una infección pulmonar supurada y circunscrita que provoca la destrucción del parénquima pulmonar, habitualmente aparece uno solo e infrecuentemente, múltiples. Puede presentar variadas etiologías, siendo los microorganismos que se aíslan con más frecuencia: Staphylococcus aereus, Streptococcus pneumoniae y gérmenes anaerobios.Objetivo: presentar un caso de un paciente inmunocompetente, que a partir de una celulitis abscedada en rodilla izquierda presentó abscesos pulmonares múltiples. Presentación del caso: se presenta el caso de un paciente masculino de 21 años, criador de palomas, con antecedentes de ser asmático desde niño. A los 21 días de una celulitis abscedada en rodilla izquierda, comienza con fiebre elevada, dolor torácico y tos con expectoración blanquecina. Se determinó la presencia de abscesos pulmonares múltiples. Se aisló Staphylococcus aureus en el lavado bronquial. Conclusiones: se presenta un caso muy infrecuente en la práctica clínica. Un paciente inmunocompetente con abscesos pulmonares múltiples causado por Stafilococcus aureus, a partir de una celulitis abscedada en rodilla izquierda. El diagnóstico se realizó por estudio microbiológico del lavado bronquial. Evolucionó favorablemente con mejoría clínica e imagenológica(AU)

It has several etiologies and the most frequently isolated microorganisms are: Staphylococcus aureus, Streptococcus pneumoniae and anaerobic germs. Conclusion: iIt presented a case of a patient immune- competent with multiple lung abscesses due to Stafilococcus aureus, to leave abscessed cellulitis at the left knee. The diagnosis was performed by microbiological study of the bronchial washed. It was applied triple antimicrobial therapy for three weeks and concluded with vancomicina. ABSTRACT

O objetivo do presente estudo foi analisar a validade dos parâmetros de potência absolutos e relativos do Running-based Anaerobic Sprint Test (RAST) com base no teste de Wingate (WAnT). Doze voluntários do sexo masculino com idade de 25,50 anos (± 2,32) realizaram os testes RAST e WAnT. Foram observados valores significativos (p < 0,01) de correlação para a potência máxima absoluta (r = 0,76) e para a potência média absoluta (r = 0,74) dos testes, entretanto, os valores relativos apresentaram correlações não significativas (r = 0,54 e 0,27 respectivamente). Todos os parâmetros analisados apresentaram diferenças significativas (p < 0,01) na comparação entre WAnT e RAST. Desta forma, os resultados sugerem que apenas os parâmetros de potência absoluta do RAST são válidos. Assim ao aplicar o RAST para avaliação e/ou prescrição de treinamento, ou em pesquisas científicas, devem ser usados preferencialmente os parâmetros absolutos do teste.


The aim of the present study was to analyze the absolute and relative parameters validity for Running-based Anaerobic Sprint Test (RAST) based on Wingate test (WAnT). Twelve male volunteers aged 25.50 ± 2.32 years old performed both RAST and WAnT. Significant (p < 0.01) correlation values for the absolute maximum power (r = 0.76) and mean absolute power (0.74) was found but the relative mean values did not showed significant correlations for both maximum (r = 0.54) and average power output (r = 0.27). Significant differences (p < 0.01) between WAnT and RAST were found for all assessed parameters. Therefore, the results of the present study suggest that the RAST should be used with caution for prescription training and evaluation, and acquisition of scientific data. The results also suggest that only the absolute parameters of the test are good predictors of anaerobic power.

O presente trabalho teve como objetivo avaliar a influência da velocidade ascensional de recirculação e da estratégia de alimentação na eficiência de um reator anaeróbio com biomassa imobilizada operado em bateladas seqüenciais tratando efluentes provenientes de um laticínio de pequeno porte. Avaliaram-se três condições operacionais com valores de velocidade de 0,2 m.h-1, 3,8 m.h-1 e 6,4 m.h-1, com tempo de ciclo de 48 horas. Finda esta etapa, avaliou-se a influência do volume tratado na eficiência do sistema. Nesta avaliação, o tempo de ciclo foi de 24 horas e estudaram-se duas condições experimentais, nas quais diariamente o reator era alimentado com 50% e 67% de seu volume. [...] Não foram verificadas diferenças significativas entre as eficiências de remoção de matéria orgânica à medida que a velocidade ascensional aumentou, todavia percebeu-se diminuição no desvio padrão de tal parâmetro, favorecimento no processo de geração e consumo de ácidos e aumento na velocidade de conversão da matéria orgânica. Os resultados das análises de biologia molecular demonstraram que a biomassa se modificou adaptando-se ao efluente, mas apresentando pouca variação entre as duas últimas condições operacionais. Quando o tempo de ciclo foi diminuído para 24 horas, e o sistema alimentado com 50% e 67% de seu volum e reacional, também não se verificou diferenças significativas em relação à eficiência de remoção de matéria orgânica, todavia a última condição operacional a que proporcionou maior volume de efluente tratado por dia. Conclui-se que o ASBBR foi eficiente, robusto e confiável para o tratamento de efluentes de laticínios nas condições de operações testadas


The aim of the present study was to evaluate the influence of the recirculation rate and feed strategy on the efficiency of an Anaerobic Sequencing Batch Biofilm Reactor - ASBBR -treating effluents from a small dairy industry. Three operational conditions were studied in which the recirculation rate was 0.2 m.h-1, 3.8 m.h-1 e 6.4 m.h-1 and the cycle time was 48 h. When this assay was finished, the cycle time was decreased to 24 hours and two operational conditions were studied, in which the reactor was fed with 50% and 67% of its reaction volume, respectively. [...] Statistic differences were not verified; nevertheless it was possible to observe that the values of standard errors decreased, the production and consumption of the volatile acids improved and the rate of organic matter removal increased when the velocity was rose. The results from the molecular biology analyses showed that the biomass changed and adapted to the treatment conditions, presenting low variation along the last condition. When the cycle time was decreased to 24 hours, and the system was fed with 50% and 67%, statistic difference among the efficiencies were again not observed, nevertheless, the latter condition resulted in the highest daily treated volume. It was possible to conclude that the ASBBR was efficient, robust and reliable in treating dairy effluents at the practiced conditions

ABSTRACT

Fundamento: en ambientes subgingivales de pacientes con periodontitis crónica se han encontrado microorganismos inusuales que incluyen bacilos entéricos gramnegativos y levaduras. En estos pacientes la terapia mecánica o antimicrobiana puede ser ineficiente para tratar o controlar la enfermedad periodontal. La característica que comparten los microorganismos inusuales es la de ser patógenos oportunistas, ya que aprovechan las condiciones de inmunodepresión para originar o agravar una enfermedad. Objetivo: presentar la prevalencia de microorganismos inusuales en placa subgingival de pacientes con periodontitis crónica en una población colombiana. Método: se evaluaron 76 pacientes sistémicamente sanos que asistieron a las clínicas odontológicas de la Facultad de Odontología de la Universidad de Antioquia. Las muestras subgingivales se procesaron mediante cultivo. Resultados: se observaron bacilos entéricos gramnegativos en 20 (26.31 %) pacientes y levaduras en 10 (13.2 %). Conclusiones: estos hallazgos tienen implicaciones terapéuticas importantes que deben tenerse en cuenta para el manejo de los pacientes con enfermedades periodontales.


Background: subgingival environment of patients with chronic periodontitis have been found unusual microorganisms including Gram-negative enteric bacilli and yeasts. Mechanical or antimicrobial therapy in these patients may be inefficient to treat or to control periodontal disease. The common feature unusual microorganisms share is to be opportunistic pathogens, taking advantage of immunosuppression conditions to cause or aggravate the disease. Objective: to present the prevalence of unusual microorganisms in subgingival plaque of patients with chronic periodontitis in a Colombian population. Method: 76 systemically healthy patients who were presented in the dental clinic at the Dental School of Antioquia University were evaluated. Subgingival samples were processed by cultivation. Results: enteric Gram-negative bacilli in 20 patients and yeasts in 10 were observed. Conclusions: these findings have important therapeutic implications that must be taken into account for patients management with periodontal diseases.

Esta nota técnica busca consolidar as principais características, vantagens e desvantagens dos métodos usualmente empregados para o controle de emissões odorantes, discorrendo criticamente acerca da aplicabilidade de cada um dos métodos para o tratamento de odores em estações de tratamento de esgoto doméstico. Verificou-se que vários métodos são sofisticados, de elevado custo e, muitas vezes, não aplicáveis ao tratamento de odores emitidos em reatores anaeróbios tratando esgoto doméstico. Uma análise qualitativa dos vários métodos indica que a combustão direta e os processos bioquímicos são os que reúnem o maior conjunto de vantagens para o tratamento de gases residuais emitidos em reatores anaeróbios.


This technical note aims at consolidating the main characteristics, advantages and disadvantages of the methods usually applied to the control of odorous emissions, with a critical analysis upon the applicability of each method to the treatment of odours in treatment plants of domestic wastewater. It was verified that several methods are sophisticated, present high cost, and are often non-applicable to the treatment of odour emissions from anaerobic reactors treating domestic wastewater. The qualitative analysis of the various methods points out that direct combustion and biochemical processes gather more advantages for the treatment of waste gases produced in anaerobic reactors.

ABSTRACT

Osteomyelitis of the mandible and maxilla are common in developing countries and their treatment may belong-standing and difficult. Thus, the aim of this study was to discuss the main biological aspects of the chronic osteomyelitis of the jaws of especial interest for dental team. These infections are associated with a complex microbiota composed mainly by anaerobic bacteria, sometimes associated with microorganisms originated from the skin and digestive tract. These data suggest that chronic osteomyelitis of the mandible and maxilla should be treated as anaerobic infections in most cases. In addition, local surgical treatments are relevant in the therapy outcome, associated to the use of antimicrobial agents, and the failure to accomplish them is a major cause of treatment failure.


La osteomielitis maxilar y mandibular son comunes en países en desarrollo y su tratamiento puede ser difícil y de larga duración. Así, el objetivo del presente estudio fue discutir los principales aspectos biológicos de especial interés odontológico de la osteomielitis crónica del maxilar. Estas infecciones están asociadas a una microbiota compleja compuesta principalmente de bacterias anaerobias, algunas veces asociadas a microorganismos oriundos de la piel y del tracto digestivo. Estos datos sugieren que la osteomielitis crónica maxilar y mandibular deben ser tratadas como infecciones anaerobias en la mayoría de los casos. Además, tratamientos quirúrgicos locales son relevantes en el éxito de la terapia, asociados al uso de agentes anti-microbianos, e cuyos resultados no satisfactorios representan una de las principales causas de un tratamiento fallo o no adecuado.

ABSTRACT

A osteomielite crônica de maxila e mandíbula é rara em países industrializados e sua ocorrência, nos países em desenvolvimento, está associada a trauma e procedimentos cirúrgicos, sendo que sua etiologia não foi estudada profundamente. O objetivo desse estudo foi avaliar a microbiota associada com a osteomielite de mandíbula e maxila em pacientes brasileiros. Após exames clínicos e radiográficos, amostras de seqüestros ósseos, secreção purulenta e biopsias de tecido granulomatoso de vinte e dois pacientes com osteomielite crônica da mandíbula e maxila foram cultivadas e submetidas à detecção de um conjunto de patógenos através do método do PCR. O isolamento bacteriano foi realizado em ágar “fastidious anaerobe” suplementado com hemina, menadiona e sangue de cavalo, para os microrganismos anaeróbios, e em ágar de tripticaseína de soja suplementado com extrato de levedura e sangue de cavalo, para os aeróbios e anaeróbios facultativos. Cada paciente apresentava uma única lesão. As placas foram incubadas em anaerobiose e aerobiose, a 37oC, por 14 e 3 dias, respectivamente. Bactérias foram cultivadas de 10 amostras de pacientes e os gêneros Actinomyces, Fusobacterium, Parvimonas e Staphylococcus foram os mais freqüentes. Através do PCR, DNA bacteriano foi detectado em amostras de 20 pacientes. Os resultados sugerem que as osteomielites crônicas dos maxilares geralmente são infecções anaeróbias mistas, reforçando o conceito de que estão relacionadas principalmente aos microrganismos do ecossistema bucal e que as infecções periapicais e periodontais podem atuar como fatores predisponentes


Chronic osteomyelitis of the maxilla and mandible is rare in industrialized countries and its occurrence in developing countries is associated with trauma and surgery, and its microbial etiology has not been studied thoroughly. The aim of this investigation was to evaluate the microbiota associated with osteomyelitis of the mandible and maxilla from some Brazilian patients. After clinical and radiographic evaluation, samples of bone sequestra, purulent secretion, and biopsies of granulomatous tissues from twenty-two patients with chronic osteomyelitis of the mandible and maxilla were cultivated and submitted for pathogen detection by using a PCR method. Each patient harbored a single lesion. Bacterial isolation was performed on fastidious anaerobe agar supplemented with hemin, menadione and horse blood for anaerobes; and on tryptic soy agar supplemented with yeast extract and horse blood for facultative bacteria and aerobes. Plates were incubated in anaerobiosis and aerobiosis, at 37oC during 14 and 3 days, respectively. Bacteria were cultivated from ten patient samples; and genera Actinomyces, Fusobacterium, Parvimonas, and Staphylococcus were the most frequent. By PCR, bacterial DNA was detected from thirteen patient samples. The results suggest that cases of chronic osteomyelitis of the jaws are usually mixed anaerobic infections, reinforcing the concept that osteomyelitis of the jaws are mainly related to microorganisms from the oral environment, and periapical and periodontal infections may act as predisposing factors

ABSTRACT

Introdução: Foi feita uma comparação na frequencia com que os patogenos infectantes foram detectados na supuração causada pelo colesteatoma e pela otite crônica simples no período de 2006 a 2008. Objetivo: Fazer um estudo comparativo entre os achados bacterianos encontrados na secreção da otite crônica simples e a colesteatomatosa. Método: Foram estudados a bacterioscopia de 83 pacientes (125 orelhas) portadores de otite média crônica, sendo 43 (52 orelhas) com colesteatoma e 40 (73 orelhas) com otite crônica simples, com predominância de idade dos 16 aos 20 anos. A duração da otorreia variou entre 2 meses e 10 anos. Para a colheita do material utilizamos um equipamento bastante pratico com caldo de tioglicolato dentro e fora enviados ao laboratório por um período máximo de até 18 horas. Resultados: O S. aureus foi mais frequente na otite crônica simples, enquanto que os anaeróbios foram mais frequentes no colesteatoma. A P. aeruginosa foi mais frequente na otite crônica simples e o Corynebacterium sp. apresentou maior frequencia no colesteatoma. o S. epidermidis apareceu com frequencias iguais em ambas as doenças otológicas. Conclusão: Não encontramos mudanças notáveis na bacteriologia dessas duas doenças. Na otite crônica simples os achados mais frequentes foram S. aureus, Pseudomonas sp. e fungos. No colesteatoma os achados mais frequentes foram os Anaeróbios e o Corynibacterium sp. A frequencia de aparecimento para S. epidermidis, Klebisiela sp. e Streptococcus sp. foi igual em nosso estudo.


Introduction: This study carried out a comparison in the frequency with which the infecting pathogens were detected in the suppuration caused by cholesteatoma and simple chronic otitis media in the period from 2006 to 2008. Objective: To carry out a comparative study between the bacterial findings found in the simple and cholesteatomatous chronic otitis media secretion. Method: We studied the bacterioscopy of 83 patients (125 ears) with chronic otitis media, 43 (52 ears) with cholesteatoma and 40 (73 ears) with simple chronic otitis, and age prevalence from 16 to 20 years. The duration of otorrhea ranged between 2 months and 10 years. For collection of the material we used very practical instrument with tioglicolate broth inside and outside sent to the laboratory for a maximum period of until 18 hours. Results: The S. aureus was more frequent in the simple chronic otitis, and the anaerobic were more frequent in the cholesteatoma. The P. aeruginosa was more frequent in the simple chronic otitis and the Corynebacterium sp. presented a higher frequency in cholesteatoma. The S. epidermidis appeared with the same frequencies in both otologic diseases. Conclusion: We did not find any critical changes in the bacteriology of either disease. In the simple chronic otitis, the most frequent findings were S. aureus, Pseudomonas sp. and fungi. In the cholesteatoma, the most frequent findings were the Anaerobios and Corynibacterium sp. The frequency of S. epidermidis, Klebisiela sp. and Streptococcus sp. was the same in our study.

ABSTRACT

O principal objetivo do presente estudo foi determinar os parâmetros anaeróbios obtidos através do RAST (Running-based Anaerobic Sprint Test) e o limiar anaeróbio de 12 atletas filiados a Federação Paulista de Handebol. Além disso, também procuramos verificar as correlações entre as variáveis do RAST, o limiar anaeróbio e a resposta lactacidemica obtida no início, no intervalo e ao final de uma partida oficial de handebol. As avaliações foram conduzidas em 2 dias. No primeiro dia, os atletas foram submetidos ao RAST e em seguida foi determinado o limiar anaeróbio de cada atleta através de um protocolo adaptado ao de Tegtbur et al. (1993). No segundo dia, durante a disputa de uma partida oficial de handebol, foram coletadas amostras de sangue para determinação das concentrações de lactato no início, no intervalo e ao final do jogo. Foi utilizado o teste Anova para dados repetidos, seguido pelo post hoc de Newman-Keuls quando necessário, com o intuito de comparar as concentrações de lactato obtidas ao longo da partida de handebol e a correspondente ao limiar anaeróbio. A análise de correlação de Pearson foi utilizada para verificar as relações entre os parâmetros anaeróbios, o limiar anaeróbio e as concentrações de lactato obtidas durante uma partida oficial de handebol. Para todos os casos o nível de significância foi pré-fixado em 5 por cento. Não foram verificadas correlações dos parâmetros anaeróbios do RAST e do limiar anaeróbio com as respostas lactacidemicas durante a partida de handebol. De acordo com os resultados obtidos no presente estudo, podemos concluir que embora o protocolo proposto para a avaliação do limiar anaeróbio também forneça parâmetros anaeróbios e se aproxime das situações reais do jogo de handebol, não foram encontradas correlações significativas entre as variáveis determinadas na avaliação com as concentrações de lactato obtidas durante a partida oficial de handebol.


The main purpose of the present study was to determine the anaerobic parameters obtained by the RAST (Running-based Anaerobic Sprint Test) and the anaerobic threshold of twelve handballers affiliated with the Handball Federation of São Paulo. Moreover, we aimed to study the relationship of the RAST variables, the anaerobic threshold and the lactacidemia response obtained at the beginning, at the interval and at the end of a an official handball match. Measurements were carried out in two days. On the first day, the athletes performed the RAST and then, the anaerobic threshold was obtained by a protocol adapted from Tegtbur et al. (1993). On the second day, the athletes had blood samples collected to determine the blood lactate responses during the three different times of the handball game. Repeated measurements Anova test followed by post hoc Newman-Keuls test whenever needed, were used to compare the blood lactate concentrations during the handball match as well as that one corresponding to the anaerobic threshold. Pearson product-moment coefficient analysis was used to verify the relationships of the RAST variables, the anaerobic threshold and the blood lactate obtained at the beginning, in the middle and at the end of an official handball match. A significance level of 5 percent was chosen for all cases. Correlations between the RAST parameters and the anaerobic threshold with the blood lactate responses during the handball match have not been observed. According to our results, it is possible to conclude that the variables obtained by the protocol proposed to determine the anaerobic threshold did not present significant correlations with the blood lactate concentrations obtained during the official handball match.
