ABSTRACT

La evidencia actual confirma la alta comorbilidad entre el trastorno por déficit de atención con hiperactividad (TDAH) y trastorno por uso de sustancias (TUS). Esta revisión resume las intervenciones farmacológicas y psicosociales que se han evaluado en pacientes con TDAH y TUS, y ofrece recomendaciones mediante el enfoque GRADE. Nuestros resultados sugieren: 1) En pacientes con TDAH y trastorno por uso de alcohol, la atomoxetina es recomendable para reducir los síntomas de TDAH (recomendación débil) y el craving de alcohol (recomendación débil). 2) En pacientes con TDAH y trastorno por uso de cannabis, la atomoxetina es recomendable para mejorar los síntomas de TDAH (recomendación débil), no para reducir el uso de cannabis (recomendación débil). 3) En pacientes con TDAH y trastorno por uso de cocaína, el metilfenidato no es recomendable para mejorar los síntomas de TDAH o para reducir el uso de cocaína (recomendación débil). 4) En pacientes con TDAH y trastorno por uso de nicotina, es recomendable el metilfenidato para mejorar los síntomas de TDAH (recomendación débil). Los psicoestimulantes, como metilfenidato o lisdexanfetamina, no son recomendables para reducir el uso de nicotina (recomendación débil). 5) Respecto de los pacientes con TDAH y cualquier TUS, el uso de los psicoestimulantes es recomendable para mejorar los síntomas de TDAH (recomendación débil), no para reducir el uso de sustancias (recomendación débil) o para mejorar la retención del tratamiento (recomendación fuerte).(AU)


Substantial evidence has confirmed the high comorbidity between Attention-Deficit/Hyperactivity Disorder (ADHD) and a substance use disorder (SUD). This review synthesizes the pharmacological and psychosocial interventions conducted in ADHD and SUDs, and provides clinical recommendations using the GRADE approach. Our results suggest: 1) In patients with ADHD and alcohol use, atomoxetine is recommended to reduce ADHD symptoms (weak recommendation) and alcohol craving (weak recommendation). 2) In patients with ADHD and cannabis use disorder, atomoxetine is recommended to improve ADHD symptoms (weak recommendation), not to reduce cannabis use (weak recommendation). 3) In patients with ADHD and cocaine use disorder, methylphenidate is not recommended to improve ADHD symptoms or to reduce cocaine use (weak recommendation). 4) In patients with ADHD and comorbid nicotine use disorder, methylphenidate is recommended to improve ADHD symptoms (weak recommendation). Psychoestimulants, such as methylphenidate or lisdexamfetamine dimesylate, are not recommended to reduce nicotine use (weak recommendation). 5) Regarding patients with ADHD and any SUD, the use of psychostimulants is recommended to improve ADHD symptoms (weak recommendation), not to reduce substance use (weak recommendation) or to improve retention to treatment (strong recommendation).(AU)

ABSTRACT

El trastorno por déficit de atención e hiperactividad afecta al 5 % de los niños en edad escolar. Se presenta una serie de 82 niños con este trastorno no asociado a enfermedades neurológicas ni a discapacidad intelectual o trastorno del espectro autista, atendidos durante un período de 8 meses en Neuropediatría: 57 casos de tipo combinado, 23 de tipo inatento y 2 de predominio hiperactivo. Tiempo medio de seguimiento: 7 ± 2,8 años (rango: 4-14,6). Compartían seguimiento con Psiquiatría 16 pacientes. Nunca recibieron tratamiento por decisión parental 12 pacientes. De los 70 que recibieron, en 20, hubo demora en el inicio del tratamiento. Tiempo medio de demora: 20 meses ± 1,6 años (rango: 1 mes y 6 años). Tiempo medio de tratamiento: 44 meses ± 2,6 años (rango: 1 mes y 10,5 años). El 90 % de los pacientes (63) que iniciaron tratamiento continuaban tomándolo en la última revisión


Attention deficit disorder with hyperactivity has a high prevalence affecting 5 % of school-age children. We present a case series of 82 children with said disorder not associated with neurological diseases or intellectual disability or autism spectrum disorder, treated during a period of 8 months in a neuropediatrics clinic: 57 cases of combined type, 23 of inattentive type and 2 of overactive predominance. Average follow-up time: 7 ± 2.8 years (range: 4-14.6); 16 patients shared follow-up with Psychiatry; 12 patients never received treatment by parental decision. Of the 70 who received it, in 20 there was a delay in the start of treatment. Average delay time: 20 months ± 1.6 years (range: 1 month and 6 years). Average treatment time: 44 months ± 2.6 years (range: 1 month and 10.5 years); 90 % of the patients (63) who started treatment were under treatment at the last control

ABSTRACT

INTRODUCTION: Approved drugs for attention deficit hyperactivity disorder (ADHD) in Spain are methylphenidate, lisdexamphetamine, atomoxetine and guanfacine. Due to adverse cardiovascular effects, mainly increased blood pressure and heart rate, its use in patients with known or undiagnosed heart disease may be controversial. OBJECTIVE: To obtain a consensus document from the Spanish Society of Paediatric Cardiology and Congenital Heart Diseases (SECPCC) and experts from other Agencies and Societies as a guide for the paediatric cardiologist and physicians who treat children and adolescents with ADHD. METHODOLOGY: An analysis was performed on the bibliography and Clinical Practice Guidelines, technical data sheets approved by the Spanish Agency of Medicines and Health Devices, and the Spanish Ministry of Health Guidelines. A Working Group was formed, with a Coordinator, as well as members of the Clinical Cardiology Working Group and Arrhythmia Group of the SECPCC. This Group produced a preliminary document that was reviewed by a group of external experts and a group of internal experts of the SECPCC with a consensus being reached on the final document. RESULTS: The recommendations of the SECPCC and the group of experts are presented on cardiovascular evaluation prior to treatment in children and adolescents with no known cardiovascular disease and with known cardiovascular disease. The recommendations of the SECPCC and the group of experts are also presented on the use of medications for ADHD in children and adolescents with cardiological symptoms with no evidence of heart disease, congenital heart disease, cardiomyopathy, Marfan syndrome and other aortic diseases, hypertension, and arrhythmias.

ABSTRACT

El trastorno por déficit de atención e hiperactividad (TDAH) es un trastorno del neurodesarrollo complejo y heterogéneo, de carácter crónico, de etiología multifactorial, principalmente debida a factores genéticos y ambientales. Realizamos un estudio analítico retrospectivo del tratamiento de niños diagnosticados de TDAH. Se estudió una muestra de 82 niños diagnosticados de TDAH (74.4% niños y 25.6% niñas). El 96.3% de los casos presentaba algún trastorno asociado. El tratamiento farmacológico fue el tratamiento de elección (90.2%). El 46.0% recibía metilfenidato de liberación inmediata, un 51.4% metilfenidato de liberación sostenida y la atomoxetina solo se recetó en un 2.7% de los casos. El 20.3% de la muestra abandonó en algún momento el tratamiento farmacológico. El tratamiento farmacológico fue la opción más utilizada en nuestra muestra, y el metilfenidato de liberación inmediata el fármaco de elección para inicio del tratamiento. Se utilizan poco las alternativas a los estimulantes. No se encontraron diferencias significativas entre el tipo de tratamiento y el subtipo de TDAH o el género, aunque sí en cuanto a la edad de inicio del tratamiento.


Attention deficit hyperactivity disorder (ADHD) is a complex and heterogeneous neurodevelopmental disorder, of a chronic nature, of multifactorial etiology, mainly due to genetic and environmental factors. We conducted a retrospective analytical study of the t herapeutic management of children diagnosed with ADHD. A sample of 82 children diagnosed with ADHD (74.4% children and 25.6% girls) was studied. 96.3% of the cases presented some associated disorder. Pharmacological treatment was the treatment of choice (90.2%). 46.0% received immediate release methylphenidate, 51.4% sustained release methylphenidate and atomoxetine was only prescribed in 2.7% of patients. 20.3% of the sample abandoned pharmacological treatment at some point. Pharmacological treatment was the most frequent option in our sample, and methylphenidate immediate release the drug of choice for treatment initiation. The alternatives to stimulants are used in very low percentage of the patient. No significant differences were found between the type of treatment regarding the subtype of ADHD or gender, but we found significant difference in relation with the age of onset of treatment.

ABSTRACT

RESUMEN El trastorno por déficit de atención e hiperactividad (TDAH) es la alteración neuroconductual más frecuente en la consulta pediátrica y su tratamiento comprende la terapia comportamental y el empleo de fármacos. Existe una preocupación latente por el eventual desarrollo del trastorno por el uso de sustancias (TUS) en los pacientes con TDAH tratados con estimulantes. La evidencia médica sugiere un efecto protector con respecto al consumo de sustancias psicoactivas, pero también alerta sobre el potencial abuso por parte de los pacientes y las personas alrededor. En este artículo se revisan sistemáticamente las virtudes y los riesgos de desarrollar TUS en los pacientes con TDAH en tratamiento con estimulantes.


SUMMARY The attention deficit with hyperactivity disorder is the most frequent disorder in pediatric outpatient services and its treatment involves behavioral therapy and drugs. However, there is concerning about develop substances use disorder once upon treatment with stimulants. Medicine based evidence suggests a preventive effect about psychoactive substance consumption but also warns potential abuse by patients and people around. In this paper, we systematically review worths and risks of SUD in stimulant - treated ADHD patients.

INTRODUCTION: Attention-deficit/hyperactivity disorder (ADHD) is one of the most common behavioural disorders of childhood; its prevalence in Spain is estimated at 5-9%. Available treatments for this condition include methylphenidate, atomoxetine, and lisdexamfetamine, whose consumption increases each year. MATERIAL AND METHODS: The prevalence of ADHD was estimated by calculating the defined daily dose per 1,000 population per day of each drug and the total doses (therapeutic group N06BA) between 1992 and 2015 in each of the provinces of Castile-La Mancha (Spain). Trends, joinpoints, and annual percentages of change were analysed using joinpoint regression models. RESULTS: The minimum prevalence of ADHD in the population of Castile-La Mancha aged 5 to 19 was estimated at 13.22 cases per 1,000 population per day; prevalence varied across provinces (p<.05). Overall consumption has increased from 1992 to 2015, with an annual percentages of change of 10.3% and several joinpoints (2000, 2009, and 2012). methylphenidate represents 89.6% of total drug consumption, followed by lisdexamfetamine at 8%. CONCLUSIONS: Analysing drug consumption enables us to estimate the distribution of ADHD patients in Castile-La Mancha. Our data show an increase in the consumption of these drugs as well as differences in drug consumption between provinces, which reflect differences in ADHD management in clinical practice.

Introducción. El trastorno por déficit de atención e hiperactividad (TDAH) es la psicopatología diagnosticada con mayor frecuencia en la edad pediátrica, la cual consiste en la presencia de grados variables de inatención, hiperactividad e impulsividad. El clorhidrato de atomoxetina es una molécula estable que ofrece una eficacia similar a los estimulantes pero con un patrón de seguridad más amplio para su uso pediátrico. Objetivo: establecer la eficacia y seguridad del clorhidrato de atomoxetina en el tratamiento del TDAH en un grupo de pacientes pediátricos. Material y métodos. Se seleccionaron pacientes pediátricos de ambos sexos, con TDAH, captados en la consulta externa de Neurología del Hospital Infantil de México Federico Gómez, a los cuales se les inició un tratamiento con clorhidrato de atomoxetina, empezando con una dosis inicial de 0.5 mg/kg/día y realizando ajustes según su evolución. Resultados. Se evaluaron 67 pacientes (femeninos 21 y masculinos 46). La edad promedio fue de 8 años. El subtipo más frecuente de TDAH fue el combinado, en 54% de la muestra. Un 23% de los pacientes habían utilizado medicamentos previos al inicio del estudio; 78% presentó comorbilidad, la más frecuente el trastorno negativista-desafiante. Con una dosis promedio de atomoxetina a 1.2 mg/kg/día, se observó una eficacia promedio en 78% de los pacientes. Los principales efectos secundarios observados fueron cefalea, náusea y disminución del apetito, los cuales descendieron durante las primeras 4 semanas de tratamiento. Conclusión. La utilización de clorhidrato de atomoxetina demostró ser efectiva y segura en el control de los síntomas cardinales del TDAH en niños mexicanos.


Introduction. Attention deficit consists of the presence of variable grades of inattention, hyperactivity and impulsivity disorder and is one of the most frequent psychopathologic disorders seen in pediatric population. Atomoxetine is a new molecule that offers a similar efficacy to other available drugs but with a broad safety for pediatric use. Objective: to establish the efficacy and safety of the atomoxetine for the treatment of the attention deficit disorder in a pediatric population. Material and methods. Pediatric patients of both sexes, with attention deficit/hyperactivity disorder, seen in the department of neurology of the Hospital Infantil de Mexico in the period between 2003-2004 were selected, prior written informed consent by parents or guardians.The initial doses of atomoxetine were 0.5 mg/kg/day, and adjusting the doses depending on evaluation.The evaluation was made with a clinical and a functioning scale like ADHD-RS, KDSAD-S and CGI.The temporally related adverse effects were measure by anthropometry, and vitals signs. Results. A total of 67 patients, 21 females and 46 males with a mean age of 8 years.The more frequent subtype was the combined in (54%) of the population.Twenty three percent of the patients had been previously treated for ADHD. A 78% had some type of comorbidity, the most frequent beign the defiant-oppositive disorder. The average dose of atomoxetine was 1.2 mg/kg/day, with an efficacy of 78%. The major secondary effects observed were headache, nausea and decreased of appetite, which diminished by the fourth week of treatment. Conclusions. In these open clinical study, the atomoxetine demonstrated efficacy and safety in the control of the cardinal symptoms of ADHD in pediatric patients.
