ABSTRACT

BACKGROUND: Breast hyperplasia is a condition with a significant impact on the quality of life and psychological well-being of patients. Over time, various surgical techniques have been developed to reduce breast mass, which have proven effective in improving the quality of life of women undergoing the procedure. It is not yet well described how various techniques may influence the improvement in QoL (Quality of Life). The aim of this study is to assess the impact of different techniques on the improvement of quality of life. MATERIAL AND METHODS: We analyzed the responses to the Breast-Q questionnaire from 95 patients undergoing breast reduction surgery between 2021 and 2023. The questionnaires were completed both before the procedure and during the 6-month follow-up. We assessed the questionnaire responses and overall satisfaction with the results, categorizing the data based on the specific surgical technique used. RESULTS: A statistically significant improvement in the quality of life was observed in the postoperative period, as well as a remarkable satisfaction with the outcome (mean score of 81 out of 100). We did not detect statistically significant differences in the levels of quality of life and satisfaction with the outcome among the various techniques. CONCLUSION: The breast reduction surgery procedure continues to prove its ability to significantly improve the quality of life for patients with breast hypertrophy, regardless of the specific surgical technique employed. ABSTRACT

OBJECTIVE: The aim: To determine the role of infectious diseases as the cause of the Cervical, Ovarian and Breast hyperplasia in Ukraine. PATIENTS AND METHODS: Materials and methods: We conducted a retrospective multicenter cohort study from January 1st, 2020 to December 31st, 2022. This study included patients aged 20-59 years with a diagnosis of hyperproliferative pathology of the women reproductive organs without atypia, who sought medical care for hyperplastic processes admitted to the 12 hospitals from 9 regions of Ukraine. RESULTS: Results: We had examined 4,713 women; out of which 81.1% met the clinical definition of female reproductive organs hyperplasia. Of all hyperplasia cases, most frequently recorded types were breast hyperplasia (41,7%), followed by cervical hyperplasia (31,1%) and ovarian hyperplasia (27,2%). History of Cervicitis (p<0.001), Vaginal cuff infection (p<0.001), Oophoritis (p<0.001), and Mastitis (p<0.001) were identified as independent risk factors of Cervical, Ovarian and Breast hyperplasia. CONCLUSION: Conclusions: This study showed that surgical site infections after obstetric and gynecological operations are is the cause of Cervical, Ovarian and Breast hyperplasia. Therefore, early detection and treatment SSIs can reduce the risk of hyperplasia these organs.

ABSTRACT

This study aimed to explore the mechanism of Xiaoyao San(XYS) in the treatment of three diseases of liver depression and spleen deficiency, ie, depression, breast hyperplasia, and functional dyspepsia, and to provide a theoretical basis for the interpretation of the scientific connotation of "treating different diseases with the same method" of traditional Chinese medicines. Traditional Chinese medicine systems pharmacology database and analysis platform(TCMSP) was used to screen the active components of XYS which underwent principal component analysis(PCA) with the available drugs for these three diseases to determine the corresponding biological activities. The targets of XYS on depression, breast hyperplasia, and functional dyspepsia were obtained from GeneCards, TTD, CTD, and DrugBank databases. Cytoscape was used to plot the "individual herbal medicine-active components-potential targets" network. The resulting key targets were subjected to Kyoto encyclopedia of genes and genomes(KEGG) pathway analysis and gene ontology(GO) enrichment analysis. A total of 121 active components of XYS and 38 common targets in the treatment of depression, breast hyperplasia, and functional dyspepsia were collected. The key biological pathways were identified, including advanced glycation and products(AGEs)-receptor for advanced glycation and products(RAGE) signaling pathway in diabetic complications, HIF-1 signaling pathway, and cancer-related pathways. The key targets of XYS in the treatment of depression, breast hyperplasia, and functional dyspepsia included IL6, IL4, and TNF, and the key components were kaempferol, quercetin, aloe-emodin, etc. As revealed by the molecular docking, a strong affinity was observed between the key components and the key targets, which confirmed the results. The therapeutic efficacy of XYS in the treatment of diseases of liver depression and spleen deficiency was presumedly achieved by reducing the inflammatory reactions. The current findings are expected to provide novel research ideas and approaches to classify the scientific connotation of "treating different diseases with the same method" of Chinese medicines, as well as a theoretical basis for understanding the mechanism of XYS and exploring its clinical applications.

ABSTRACT

In the present study, a rat model of breast hyperplasia was established via the administration of estradiol benzoate and progesterone. Subsequent changes associated with breast hyperplasia were then investigated by measuring the diameter and height of the nipples and by staining breast tissue with hematoxylin and eosin. The proliferation and apoptosis of hyperplastic cells in the breast tissue were then determined by analyzing the expression of proliferating cell nuclear antigen (PCNA) and cleaved­caspase­3 by immunohistochemistry and TUNEL staining. We also determined the expression of proteins associated with the phosphatase and tensin homolog (PTEN)/protein kinase B (AKT) signaling pathway by western blotting. Melatonin treatment led to a significant reduction in the degree of breast hyperplasia (P<0.05), a significant reduction in PCNA, a significant increase in the level of apoptosis (P<0.05), a significant increase in PTEN (P<0.05), and a significant reduction in AKT/p­AKT (P<0.05). Furthermore, melatonin significantly decreased the aggravation of breast hyperplasia induced by application of a PTEN inhibitor. Melatonin reduced the degree of breast hyperplasia, reduced the proliferation of hyperplastic breast tissue cells, and promoted cell apoptosis in hyperplastic tissue. These effects were achieved by the specific regulation of proteins in the PTEN/PI3K/AKT axis.

ABSTRACT

Pseudoangiomatous stromal hyperplasia (PASH) is a benign hyperplastic condition of the breast that can lead to macromastia. The standard treatment for PASH is focal excision or rarely reduction mammoplasty. We present a rare case of postpartum bilateral rapid breast enlargement and axillary growth that was refractory to reduction mammoplasty. Ultimately, the patient required bilateral mastectomy and two-stage implant-based breast reconstruction. This more extensive form along with its management represents one of the few reported cases in the literature. The decision to pursue bilateral mastectomy was undertaken after exhausting more conservative options. Excellent aesthetic outcome and pain relief was obtained following definitive extirpative and reconstructive surgery.

ABSTRACT

Objective To analyze the effects of Xiaogan capsule, Xiaoyao pill combined with Shu er capsule on hyperplasia of mammary gland and its effect on vascular endothelial growth factor ( VEGF ) and basic fibroblast growth factor ( bFGF ) .Methods 106 cases of patients with breast hyperplasia treated in our hospital from April 2013 to August 2016 were randomly divided into experimental group and control group according to the order of admission, with 53 cases in each group.The control group was treated with Xiaojin Capsule and Xiaoyao Pills.The experimental group was treated with Xiaojin Capsule, Xiaoyao Pill and Shuerjing Capsule.Analyze the total effective rate of clinical treatment, incidence of adverse reactions, visual analogue pain score and the influence on the expression of VEGF and bFGF in the two groups.Results The total effective rate was 96.23%(51/53) in the experimental group and 86.79%(46/53) in the control group,the total effective rate in the experimental group was much higher than that in the control group (P<0.05).The incidence of adverse reactions was 11.32%(6/53) in the experimental group and 13.21%(7/53) in the control group,there was no significant difference in adverse reactions between the two groups.The visual analogue pain score of the experimental group was (2.85 ±0.41) points, the visual analogue pain score of the control group was (5.25 ±1.84),the visual analogue pain score of the experimental group was significantly lower than that of the control group ( P <0.05 ) .After treatment, the expression of VEGF and bFGF in the patients with hyperplasia of mammary gland decreased, and the levels of VEGF and bFGF in the patients with hyperplasia of mammary glands were significantly higher than those in the untreated group (P<0.05),the expression of VEGF and bFGF in the control group was enhanced.Conclusion Xiaojin capsule,Xiaoyao pill and Shu er jing capsule have higher application value in the patients with hyperplasia of mammary gland , and significantly lower the expression of VEGF and bFGF .

ABSTRACT

To explore the efficacy of psychological interventions (PI) in patients with breast hyperplasia (BH). In total 120 BH patients who were treated in the Third Affiliated Hospital of the Third Military Medical University were randomly divided into PI group (n = 40; treated with oral XiaoYao Pill and psychological interventions), anti-anxiety/depression medication (AADM) group (n = 40; treated with oral XiaoYao Pill and paroxetine), and control group (n = 40; treated with oral XiaoYao Pill) and the treatment lasted for 1 year. Before the treatment and 4, 8, and 12 months after the initiation of treatment, the changes in the psychological indicators were measured using Toronto Alexithymia Scale (TAS), Hamilton Anxiety Scale (HAMA), Hamilton Depression Scale (HAMD), and Ways of Coping Questionnaire (WCQ), as well as the physiological indicators including estradiol, prolactin, and progesterone were determined. The overall response rates were evaluated at the end of the treatment, and the relapse rates were calculated during the 1-year follow-up. The HAMD and HAMA scores were declined in all three groups. The scores of TAS and WCQ negative coping subscales showed a declining trend after treatment for the AADM and PI groups. Compared to the control and PI groups, the HAMA and HAMD scores were significantly lower in the AADM group 4, 8, and 12 months after the initiation of treatment (P < 0.05). The scores of TAS and WCQ negative coping subscales were significantly lower in AADM group but were significantly higher than those in PI group and lower than the control group 4, 8, and 12 months after the initiation of treatment (P < 0.05). The HAMA and HAMD scores were significantly lower in PI group than in control group 4, 8, and 12 months after the initiation of treatment (P < 0.05). After the initiation of treatment, the estradiol and prolactin levels decreased while the progesterone levels increased in all three groups. Compared with the control group and AADM group, the PI group had significantly higher estradiol and prolactin levels and higher progesterone levels 4, 8, and 12 months after the initiation of treatment (P < 0.05). Compared with the control group, the AADM group had significantly lower levels of estradiol and prolactin and higher progesterone levels 4, 8, and 12 months after the initiation of treatment (P < 0.05). The overall response rate was not significantly different for both PI group and AADM group (P > 0.05), while the relapse rate was significantly lower in the PI group than the control and AADM groups (P < 0.05). However, the relapse rate did not significantly differ between the control group and AADM group (P > 0.05). PI can effectively improve the psychological status of BH patients and restore the disordered endocrine system. The efficacy lasts for long and the relapse rate is lower. Therefore, it could be an effective method for treating BH.

ABSTRACT

OBJECTIVE:To observe the clinical efficacy and safety of Guizhi fuling capsule combined with Tamoxifen citrate tablet in the treatment of breast hyperplasia. METHODS:216 female patients with breast hyperplasia were randomly divided into control group and observation group. Control group was orally treated with Tamoxifen citrate tablet 10 mg,bid. Observation group was additionally orally treated with Guizhi fuling capsule 3 granules,tid. The treatment course was 3 months. Drug withdrawal was conducted in menstrual period. The clinic data was observed,including clinical efficacy,the maximum mass diameter and breast pain scores before and after treatment,and incidence of adverse reactions. RESULTS:The total effective rate in observation group was significantly higher than control group,the difference was statistically significant(P0.05). CONCLUSIONS:Guizhi fuling capsule combined with Tamoxifen citrate tablet has more significant efficacy than only Tamoxifen citrate tablet in the treatment of breast hyperplasia,with similar safety.

Objective To analyze the clinical diagnosis value of Color Doppler Flowing Imaging in the diag-nosis of female breast mass ,discuss sonographic features of breast hyperplasia and maeexy dyeplesle .Methods The data of 20 patients with breast disease which diagnosed by Color Doppler Flowing Imaging were retrospectively ana -lyzed,some cases were conducted postoperative pathological examination .Results In all cases,13 cases were breast fibroadenoma(65.0%),6 cases were maeexy dyeplesle (30.0%),1 case was metaplastic squamous cell carcinoma (5.0%).The typical fibroadenoma had regular shape ,clear boundary and homogeneous internal echo ,and were mar-ginated smoothly with a fine echoic capsule and a lateral wall shadowing .Conclusion Color Doppler ultrasonography is an approach for noninvasive and simple examination of breast lesion ,and worthwhile for spreading in clinical prac-tice .Analysis of the ultrasonographic performance of breast fibroadenoma is helpful to the diagnosis and differential di -agnosis of breast disease ,which can also improve the level of diagnosis .

ABSTRACT

This study was aimed to reveal the medication patterns of Chinese patent medicine in the treatment of breast hyperplasia based on association rules and clustering of data mining technology. Articles on Chinese patent medicine in the treatment of breast hyperplasia were retrieved from databases. External formulas or those mixed with western drugs were excluded. Terminologies in the selected formulas were standardized. Information was extracted to build excelltables. Association rules analysis and cluster analysis were used to reveal the medication patterns of Chi-nese patent medicine in the treatment of breast hyperplasia with software R2.15.2 and Cytoscape 2.8.3. The results showed that Chinese medicinals with higher frequency in 195 formulas were Radix bupleuri, Rhizoma cyperi, Chinese Angelica, Selfheal, White Peony Root, Trichosanthes kirilowii Maxim et al. Chinese medicinals with lower frequency were Chinese Alangium leaf, Rhinacanthus nasutus et al. The commonly used herbal pairs were Radix bupleuri -Chinese A ngelica, Rhizoma curcumae - Rhizoma sparganii, frankincense - myrrh et al. The strong association rules indicated that Xiaoyaosan, which takes Radix bupleuri and Chinese A ngelica as its core, is the key character of Chinese patent medicine in the treatment of breast hyperplasia. The cluster analysis revealed several Chinese medicine functional modules, such as Fructus forsythiae - Rhizoma smilacis glabrae - Evodia lepta - Chinese mahonia stem - Lignum millettiae et al. It was concluded that the herbal frequency, herbal pairs, strong associa-tion rules and cluster analysis can reveal the medication patterns of Chinese patent medicine in the treatment of breast hyperplasia in order to provide reference evidences in the optimization of clinical treatment for mula and im-provement of therapeutic efficacy.

ABSTRACT

PURPOSE: The need for surgical excision in patients with ultrasound-guided core needle biopsy (CNB)-diagnosed atypical ductal hyperplasia (ADH) remains an issue of debate. The present study sought to validate a scoring system (the U score, for underestimation) that we have previously developed for predicting malignancy in CNB-diagnosed ADH. METHODS: The study prospectively enrolled 85 female patients with CNB-diagnosed ADH who underwent subsequent surgical excision. Underestimation was defined as a surgical specimen having malignant foci. RESULTS: The overall underestimation rate was 37% (31/85). Multivariate analysis showed that a clinically palpable mass, microcalcification on imaging, size >15 mm and a patient age of ≥50 years were independently associated with underestimation. When applied to the scoring system, the validation score was significant (p<0.001; area under the curve, 0.852). No patient with a U score <3.5 had an underestimated lesion. CONCLUSION: The present study successfully validated the efficacy of our scoring system for predicting malignancy in CNB-diagnosed ADH. A U score of ≤3.5 indicates that surgical excision may not be necessary.

PURPOSE: Our purpose was to identify the factors in a breast core needle biopsy (CNB) of atypical ductal hyperplasia (ADH) that are predictive for carcinoma in the subsequent excision specimens. METHODS: We performed a retrospective pathologic review of 50 cases that were diagnosed as ADH via the CNB and that had the corresponding excision specimens. RESULTS: The size of the largest ADH foci in the CNBs was 0.8+/-0.6 mm (mean+/-SD) for benign proliferative disease (BPD, n=12), 1.0+/-0.5 mm (mean+/-SD) for ADH (n=9) and 1.3+/-0.8 mm (mean+/-SD) for malignant lesions (n=29) in excision specimens (p=0.105). Among the 30 cases showing stromal alterations around the ducts with ADH in the CNBs, 9 cases (30.0%) were BPD or ADH and 21 cases (70.0%) were malignant lesions in the excision specimens (p=0.004). CONCLUSION: As the stromal alterations around the ADH were noted in the CNB, there was an increased likelihood that more advanced lesions would be identified in the surgical excision specimens.

Objective To investigate the expression and role of Skp2 and P21 in the process of breast hyperplasia cancerizing.Methods The expression of Skp2 and P21 protein in normal breast tissue,adenosis of breast,breast atypical hyperplasia,breast hyperplasia cancerization,and breast cancer were detected by immunohistochemical staining SP method.Results(1) The expression of Skp2 and P21 in normal breast tissue and adenosis of breast was negative,the positive staining rate of Skp2 and P21 in breast atypical hyperplasia was 25.71% and 37.14%,in breast hyperplasia cancerization 63.16% and 73.68%,breast cancer,75% and 100% respectively.The contradistinction between breast atypical hyperplasia and breast hyperplasia cancerization was of significant statistically.But there was no statistical significance between breast hyperplasia cancerization and breast cancer.(2) The expression of P21 could lead to accelerated expression of Skp2.(?2= 10.024,P=0.002).There was a significant positive correlation between Skp2 and P21(r=0.563,P=0.000).Conclusion Skp2 and P21 are involed in the early stage of breast cancer and contribute to the maligant phenotype without affecting infiltration.Both of them are danger factors for breast hyperplasia cancerizing and contribute to malignant process from normal breast tissue and atypical hyperplasia to breast cancer.They are effective indicators for prognosis of breast hyperplasia.

ABSTRACT

Objective To explore the quantitative expression of estrogen receptor(ER),progesterone receptor(PR)in breast hyperplasia and its Clinical significance.Methods The expression of ER and PR was examined by immunohistochemical S-P method and quantitative analysis technique in 20 cases a piece of epithelial hyperplasia of usual type,atypical hyperplasia(mild,moderate,severe)and intraductal carcinoma,which contrasted with 20 cases of usual breast.Results The difference of the expression of ER and PR between usual breast or epithelial hyperplasia of usual type and atypical hyperplasia or intraductal carcinoma was significant(P50%,there was significant difference between moderate dysplasia and mild dysplasia,but no significant difference between moderate dysplasia and severe dysplasia,intraductal carcinoma.Conclusions The abnormal expression of ER and PR may be an early event in the progression of breast carcinoma,it could be used for early diagnosis of breast premalignancy and as a effective mark to monitor the prognosis.
