ABSTRACT

Objetivo: analisar a incidência e os fatores relacionados à obstrução de cateter intravenoso periférico em adultos hospitalizados. Método: uma coorte prospectiva, realizada a partir da observação de 203 cateteres, entre fevereiro de 2019 e maio de 2020, em hospital público e de ensino brasileiro. Foram consideradas variáveis clínicas e do cateter. Os dados foram analisados descritivamente e por estatística inferencial. Resultados: o tempo de permanência variou entre um a 15 dias e a obstrução ocorreu em 7,5% das observações. Houve aumento do risco de obstrução em relação ao sexo (RR=0,49 / p=0,186), à idade (RR=1,20/ p=0,732), aos cateteres de maior calibre (RR=0,53/ p=0,250), à inserção no dorso da mão até antebraço (RR=2,33/ p=0,114) e ao tempo do cateter in situ (RR=033/ p=0,433). Conclusão: O cuidado diário e observação do cateter intravenoso periférico são importantes para minimizar o surgimento de complicações locais e sistêmicas e manter a patência do dispositivo.


Objective: to analyze the incidence and factors related to peripheral intravenous catheter obstruction in hospitalized adults. Method: a prospective cohort, based on the observation of 203 catheters, between February 2019 and May 2020, in a Brazilian public teaching hospital. Clinical and catheter variables were taken into account. The data was analyzed descriptively and using inferential statistics. Results: the length of stay ranged from one to 15 days and obstruction occurred in 7.5% of the observations. There was an increased obstruction risk in relation to gender (RR=0.49 / p=0.186), age (RR=1.20/ p=0.732), larger catheters (RR=0.53/ p=0.250), insertion in the back of the hand up to the forearm (RR=2.33/ p=0.114) and the time length the catheter was in situ (RR=033/ p=0.433). Conclusion: Daily care and observation of the peripheral intravenous catheter is important to minimize the appearance of local and systemic complications and maintain the patency of the device.

ABSTRACT

Resumo O cateter vesical de longa permanência pode ser indicado em situações clínicas, como nas doenças crônicas do sistema genitourinário ou neurológico. Além dos riscos de infecção, traumas e sangramentos, a permanência do cateter pode afetar dimensões psicoemocionais e socioeconômicas. Objetivamos compreender como a necessidade de uso do cateter urinário por um longo prazo afeta a autopercepção, as interrelações e o autocuidado deste paciente. Realizamos um estudo qualitativo, descritivo, a partir da entrevista de 17 pacientes, e aplicamos a análise temática e o pensamento complexo. Os diferentes prognósticos e as expectativas em relação ao cateter influenciaram a autopercepção, a adaptação, sua aceitação ou negação. A presença do cateter, seja como medida curativa ou para conforto, pode afetar a autoimagem e a sexualidade, gerar inseguranças e incertezas, que requerem compreensão da multidimensionalidade das situações, que sofrem interferências do meio pessoal, familiar e social, bem como da capacidade dos sistemas de saúde para o seu enfrentamento. Apesar dos desafios, a maioria dos participantes relatou disposição favorável para o autocuidado, seja para viabilizar retirada do cateter, ou para prevenir agravos em indicações vitalícias.


Abstract A long-term indwelling catheter may be indicated in clinical situations, such as chronic diseases of the genitourinary or neurological systems. In addition to the risks of infection, trauma, and bleeding, a catheter's permanence can affect psycho-emotional and socioeconomic dimensions. We aimed to understand how the need to use a long-term indwelling catheter affects this patient's self-perception, interrelationships, and self-care. We carried out a qualitative, descriptive study based on interviews with 17 patients, and applied thematic analysis and complex thinking. The different prognoses and expectations regarding the catheter influenced self-perception, adaptation, acceptance, or denial. The presence of a catheter, whether as a curative measure or for comfort, can affect self-image and sexuality, and generate insecurities and uncertainties, which require understanding the multidimensionality of situations that suffer interference from the personal, family, and social environment, as well as health systems' capacity to deal with it. Despite the challenges, the majority of participants reported a favorable disposition towards self-care, whether to enable catheter removal or to prevent injuries in lifelong indications.

ABSTRACT

Introducción. La infección asociada a catéter venoso central (CVC) es la principal complicación que presentan los pacientes en hemodiálisis en los que se usa este tipo de acceso. Objetivo. Estimar la incidencia de bacteriemia asociada a CVC no tunelizado, analizar la frecuencia de agentes causales y explorar factores de riesgo asociados en niños en hemodiálisis. Población y métodos. Estudio retrospectivo realizado en niños en hemodiálisis por CVC no tunelizado entre el 1 junio de 2015 y el 30 de junio de 2019. Para evaluar factores de riesgo predictores de bacteriemia asociada a CVC, se realizó regresión logística. Los factores de riesgo independiente se expresaron con odds ratio con sus respectivos intervalos de confianza del 95 %. Se consideró estadísticamente significativo un valor de p <0,05. Resultados. En este estudio se incluyeron 121 CVC no tunelizados. La incidencia de bacteriemia fue de 3,15 por 1000 días de catéter. El microorganismo aislado con mayor frecuencia fue Staphylococcus epidermidis (16 casos, 51,5 %). La infección previa del catéter fue el único factor de riesgo independiente encontrado para el desarrollo de bacteriemia asociada a CVC no tunelizado (OR: 2,84; IC95%: 1,017,96; p = 0,04). Conclusiones. El uso prolongado de los CVC no tunelizados para hemodiálisis crónica se asoció con una incidencia baja de bacteriemia. Los gérmenes grampositivos predominaron como agentes causales. La presencia de infección previa del CVC aumentó en casi 3 veces el riesgo de bacteriemia asociada a CVC en nuestra población pediátrica en hemodiálisis.


Introduction. Central venous catheter (CVC)-related infection is the main complication observed in patients undergoing hemodialysis with this type of venous access. Objective. To estimate the incidence of non-tunneled CVC-related bacteremia, analyze the frequency ofcausative agents, and explore associated risk factors in children undergoing hemodialysis. Population and methods. Retrospective study in children receiving hemodialysis via a non-tunneled CVC between June 1 st, 2015 and June 30 th, 2019. A logistic regression was carried out to assess risk factors that were predictors of CVC-related bacteremia. Independent risk factors were described as odds ratios with their corresponding 95% confidence interval (CI). A value of p < 0.05 was considered statistically significant. Results. A total of 121 non-tunneled CVCs were included in this study. The incidence of bacteremia was 3.15 per 1000 catheter-days. The most commonly isolated microorganism was Staphylococcus epidermidis(16 cases, 51.5%). Prior catheter infection was the only independent risk factor for the development of bacteremia associated with non-tunneled CVC (OR: 2.84, 95% CI: 1.01­7.96, p = 0.04). Conclusions. Prolonged use of non-tunneled CVCs for chronic hemodialysis was associated with a low incidence of bacteremia. Gram-positive microorganisms prevailed among causative agents. A prior CVC infection almost trebled the risk for CVC-related bacteremia in our pediatric population receiving hemodialysis.

Resumen Objetivo: Presentar la experiencia en un país andino con el dispositivo OcclutechTM Duct Occluder para el cierre del conducto arterioso persistente. Método: Estudio observacional, retrospectivo, de corte transversal con análisis estadístico básico. Periodo: diciembre/2014 a diciembre/2022. Datos: historia clínica, informes de laboratorio de cateterismo. Resultados: Cuarenta y seis pacientes; de sexo femenino 71.3%, de sexo masculino 28.7%; edad: 0.6-38 años (mediana [Me]: 5.2); peso: 6.3-60 kg (Me: 16.5). Procedencia: andina 91.3%, costa 8.7%. Tipos de conducto arterioso persistente: E 54.4%, A 32.6%, D 13%. Diámetro ductal mínimo: 1.8-11.8 mm (Me: 3.5). Presión media de la arteria pulmonar previo a la oclusión: 14-67 mmHg (Me: 27). Índice de resistencias vasculares pulmonares previo a la oclusión: 0.28-4.9 UW/m2 (Me: 1.3). Fueron catalogados como conductos arteriosos persistentes hipertensivos seis de ellos. Tasa de oclusión: inmediata el 47.8%, a las 24 horas el 81%, a los seis meses el 100%. Tiempo de fluoroscopia: 2-13.8 minutos (Me: 4). Complicaciones: un dispositivo migrado. Seguimiento: 1-6.5 años. Conclusiones: El dispositivo OcclutechTM Duct Occluder fue efectivo y seguro para el cierre de conducto arterioso persistente tipo E, A y D en habitantes de baja y alta altitud, ya sea que estos hubieran sido niños o adultos, incluso cuando estos conductos arteriosos fueron hipertensivos.

Types of patent ductus arteriosus: E 54.4%, A 32.6%, D 13%. Minimum ductal diameter: 1.8-11.8 mm (Me: 3.5). Mean pulmonary artery pressure prior to occlusion: 14-67 mmHg (Me: 27). Pulmonary vascular resistance index prior to occlusion: 0.28-4.9 WU/m2 (Me: 1.3). Six of them were classified as hypertensive patent ductus arteriosus. Occlusion rate: 47.8% immediate, 81% at 24 hours, 100% after six months. Fluoroscopy time: 2-13.8 minutes (Me: 4). Complications: a migrated device. Follow-up: 1-6.5 years. Conclusions: OcclutechTM Duct Occluder device was effective and safe for the closure of patent ductus arteriosus type E, A and D in low-altitude and high-altitude dwellers, whether they were children or adults, even when these ductus arteriosus were hypertensive.

INTRODUCTION: Pulmonary hypertension (PH) is a serious disease that requires early diagnosis to achieve a better patient prognosis. Right heart catheterization (RHC) has become the main diagnostic test for this disease, measuring the pressures from the right heart chambers invasively, using a catheter placed through venous access. Nursing performance has an important role in RHC through peripheral venous access due to its well-done skills for canalization and knowledge about the management and care of these accesses. RHC performed through peripheral venous access provide advantages over those performed through central venous access. OBJECTIVE: To analyze the benefits from RHC through peripheral venous access compared to those performed through central venous access, as well as highlighting the role of nursing during this type of procedures. METHOD: A retrospective, descriptive, and observational study was performed for patients who underwent RHC in our center between January 2019 to January 2023. We analyzed clinical characteristics, access, fluoroscopy parameters, periprocedural complications, and hospital admissions. RESULT: A total of 115 patients were included. The average age was 65±12 years, with 58.1% of females. Risk stratification of PH was the reason for conducting RHC in 82.9%. The anterocubital veins became the main approach (72.2%), performed by hemodynamics nurses, while the central venous ones composed the rest (27.8%), which were done by cardiology specialized doctors. We observed a significant reduction in radiation dose in RHC via anterocubital route compared to central venous access (4.4Gycm2 vs 12.5Gycm2 [IQR: 4.5]; P<.001), and it does also in fluoroscopy times (2.3minutes vs 4.6minutes [IQR: 2.6]; P<.001). No complications were recorded, independently of the approach. Patients who underwent a scheduled catheterization were discharged more frequently on the same day of the procedure whether a peripheral approach was performed (77.2%, 44 of 57 patients), in comparison with the central one (28.6%) (P=.001). CONCLUSIONS: The RHC is an essential tool for the diagnosis of PH, achieving nursing such an important role for those performed by peripheral venous access. Peripheral venous access provides benefits and advantages like the reduction of radiation exposure and scan times, reduced hospital stay. All this could bring greater comfort, safety and better quality of care to the patient.

O Estágio de Natureza Profissional é um período fundamental do segundo ciclo de estudos pois visa completar a formação académica da componente de especialização, onde o estudante, integrado num contexto profissional, imerge em ambiente e situações clínicas complexas, desenvolvendo atividades que lhe permitam adquirir e aprimorar competências comuns e específicas do Enfermeiro Especialista em Enfermagem Médico-Cirúrgica. Pretende-se com este relatório evidenciar as oportunidades de aprendizagem e atividades realizadas no âmbito deste espaço e tempo formativo, no serviço de urgência de um hospital do norte do país, compreendendo a importância e apropriando a intervenção do Enfermeiro Especialista em Enfermagem Médico-Cirúrgica na área da Pessoa em Situação Crítica e as suas competências para gerir cuidados de enfermagem, intervir na formação de equipas de saúde e prestar cuidados altamente qualificados à pessoa doente e família. A investigação que integra este relatório assenta num estudo descritivo-correlacional com o objetivo de analisar as atitudes e práticas dos enfermeiros do serviço de urgência, de um hospital do norte de Portugal, na utilização do acesso vascular intraósseo. Utilizou-se o questionário como instrumento de recolha de dados a uma amostra de 76 enfermeiros, distribuído e preenchido via on-line, entre 4 e 15 de maio de 2023. Os resultados evidenciaram que 97,4% dos participantes reconhecem o acesso intraósseo como importante no contexto do serviço de urgência, embora 85,5% nunca o utilizassem, sendo o acesso por veia central privilegiado por 90,8%, como alternativa à falha da inserção da veia periférica. A insatisfação com os conhecimentos sobre cateterização intraóssea é de 89,5%. Fatores como a falta de treino/formação na realização do procedimento são considerados pela maioria, como os mais condicionantes à realização do procedimento. A principal conclusão é que os enfermeiros do serviço de urgência reconhecem a importância do acesso intraósseo, no entanto, afirmam que é necessário mais formação e treino para que o número de cateterizações intraósseas possa aumentar nas situações que se justifiquem. Deste percurso formativo, destaca-se a importância da intervenção diferenciada do Enfermeiro Especialista na melhoria contínua da qualidade de cuidados, num contexto complexo como é o serviço de urgência. Salienta-se, a nível pessoal, o desenvolvimento de competências especializadas comuns e específicas, na interação com o ambiente clínico envolvente.


The Professional Internship is a fundamental period of the second cycle of studies because it aims to complete the academic training of the specialisation component, where the student, integrated in a professional context, immerses himself in complex clinical environment and diferent situations, developing activities that allow him to acquire and improve common and specific skills of the Specialist Nurse in Medical-Surgical Nursing. This report aims to highlight the learning opportunities and the activities carried out within this space and time training, in the emergency service of a hospital in the north of the country, understanding the importance and appropriate intervention of the Nurse Specialist in Medical-Surgical Nursing in the area of the Person in a Critical Situation and their skills to manage nursing care, intervene in the training of health care teams and provide highly qualified care to the sick person and their families. The research that integrates this report is based on a descriptive-correlational study with the objective of analysing the attitudes and practises of nurses in the emergency service, of a hospital in the north of Portugal, in the use of intraosseous vascular access. The questionnaire was used as a data collection tool for a sample of 76 nurses, distributed and completed online, between May 4 and 15, 2023. The results showed that 97.4% of the participants recognise intraosseous access as important in the context of the emergency service, although 85.5% never used it, and access by central vein was privileged by 90.8%, as an alternative to the failure of the insertion of the peripheral vein. Dissatisfaction with the knowledge of intraosseous catheterisation is 89.5%. Factors such as the lack of training in the realisation of the procedure are considered by most, as the most conditioning to the realisation of the procedure. The main conclusion is that the nurses of the emergency service recognise the importance of intraosseous access, however, they say that more training is necessary so that the number of intraosseous catheterisations can increase in situations that are justified. From this training path, the importance of the differentiated intervention of the Specialist Nurse in the continuous improvement of the quality of care is highlighted, in a complex context such as the emergency service. It is noteworthy, at a personal level, the development of common and specific specialised skills in the interaction with the surrounding clinical environment.

Durante décadas el estudio de las enfermedades cardiacas ha sido una de las principales preocupaciones de los profesionales del área de la salud, más aún aquellos casos donde la patología es de tipo congénita, debido a las múltiples complicaciones que se pueden generar en la calidad de vida de los pacientes diagnosticados, por falta de un manejo adecuado. Tal es el caso de la fistula coronaria, que es una anomalía de este tipo de arterias, los síntomas más frecuentes son la dificultad respiratoria, ángor, taponamiento cardiaco y muerte súbita, por ello, entre más precoz sea el diagnostico existirán mayores posibilidades de la reducción de los riesgos agravados de la enfermedad. Se presenta el caso de un paciente pediátrico cuyos estudios demostraron la presencia de una fistula coronaria proveniente de la arteria coronaria izquierda hacia la aurícula derecha, cuya frecuencia no es muy alta y por lo cual puede ser considerada como una enfermedad rara, así que también se describe el tratamiento recibido para dicha malformación congénita y los resultados obtenidos luego del manejo dado. Objetivo: Describir el caso de un paciente pediátrico con falla cardiaca secundario a una fistula coronaria. Conclusiones: La fistula coronaria es una cardiopatía congénita poco frecuente, sin embargo, la presencia de esta anomalía pone en riesgo la vida de los pacientes que tienen esta afectación en cuanto al funcionamiento o estructura de su corazón, la descripción de este caso clínico aporta información relevante para el conocimiento científico del área de la salud, en lo que respecta al diagnóstico y manejo temprano de esta afectación en la función cardiaca de los pacientes pediátricos, ya que, se logró tener un estatus exitoso post cierre endovascular de la fistula coronaria identificada, que favorecerá al comportamiento normal de las funciones del corazón y por consiguiente la calidad y el pronóstico de vida del paciente tratado. For decades, the study of heart disease has been one of the main concerns of health professionals, especially in cases where the pathology is congenital, due to the multiple complications that can be generated in the quality of life of diagnosed patients, for lack of proper management. Such is the case of coronary fistula, which is an anomaly of this type of arteries, the most frequent symptoms are respiratory distress, angina, cardiac tamponade and sudden death, therefore, the earlier the diagnosis, the greater the possibilities of reducing the aggravated risks of the disease. We present the case of a pediatric patient whose studies showed the presence of a coronary fistula from the left coronary artery to the right atrium, whose frequency is not very high and therefore can be considered a rare disease, and we also describe the treatment received for this congenital malformation and the results obtained after the management given. Objective: To describe the case of a pediatric patient with heart failure secondary to a coronary fistula. Conclusions: Coronary fistula is a rare congenital heart disease, however, the presence of this anomaly puts at risk the life of patients who have this affectation in terms of the function or structure of their heart, the description of this clinical case provides relevant information for the scientific knowledge of the health area, regarding the diagnosis and early management of this affectation in the cardiac function of pediatric patients, since a successful status was achieved after endovascular closure of the identified coronary fistula, which will favor the normal behavior of the heart functions and consequently the quality and prognosis of life of the treated patient. ABSTRACT

El hiperaldosteronismo primario (HAP) es una causa importante de hipertensión arterial (HTA) secundaria. El estudio del mismo precisa de una alta sospecha clínica, además de un estudio hormonal que confirme la hipersecreción hormonal. Es importante iniciar el tratamiento adecuado una vez se confirma el diagnóstico, y para ello es preciso demostrar si la hipersecreción hormonal es unilateral (pacientes que podrían ser tributarios a tratamiento quirúrgico) o bilateral (pacientes que son tributarios a tratamiento únicamente farmacológico). En el Hospital del Mar desde el año 2016 existe un equipo de trabajo multidisciplinar en el que participan nefrólogos, endocrinólogos, radiólogos y cirujanos para evaluar los casos con sospecha de hiperaldosteronismo y consensuar el mejor abordaje diagnóstico-terapéutico de estos pacientes, incluyendo la necesidad de cateterismo venoso adrenal, que es una técnica que en los últimos años se ha erigido como gold standard para el estudio del HAP. En el presente estudio recogemos la experiencia de nuestro centro en la realización de cateterismo venoso adrenal y en la utilidad de este para el manejo de tales pacientes (AU)


Primary hyperaldosteronism (PAH) is an important cause of secondary hypertension (HTN). The study of the same requires a high clinical suspicion in addition to a hormonal study that confirms hormonal hypersecretion. It is important to start the appropriate treatment once the diagnosis is confirmed, and for this is necessary to demonstrate whether the hormonal hypersecretion is unilateral (patients who could be candidates for surgical treatment) or bilateral (patients who are candidates for pharmacological treatment only). At the Hospital del Mar since 2016 there has been a multidisciplinary work team in which Nephrologists, Endocrinologists, Radiologists and Surgeons participate to evaluate cases with suspected hyperaldosteronism and agree on the best diagnostic-therapeutic approach for these patients, including the need for adrenal vein sampling, which is a technique that in recent years has become the gold standard for the study of PAH. In the present study we collect the experience of our center in performing adrenal vein catheterization and its usefulness for the management of these patients (AU)

Introduction and objectives Chronic thromboembolic disease refers to the presence of chronic thrombotic pulmonary vascular thrombosis without pulmonary hypertension (PH) at rest but with exercise limitation after pulmonary embolism (PE). Our aim was to evaluate the hemodynamic response to exercise in these patients and its correlation with the values reached in cardiopulmonary exercise testing. Methods We included symptomatic patients with persistent pulmonary thrombosis after PE. We excluded patients with left heart disease or significant PH (mean pulmonary arterial pressure [mPAP] >25mmHg, pulmonary vascular resistance >3 WU, and pulmonary capillary wedge pressure [PCWP] >15mmHg). Cardiopulmonary exercise testing and exercise right heart catheterization were performed. Exercise-induced precapillary PH was defined as mPAP/CO slope >3 and PCWP/CO slope <2mmHg/l/min. The hemodynamic response and the values obtained in cardiopulmonary exercise testing were compared between patients with and without exercise-induced precapillary PH. Results We studied 36 patients; 4 were excluded due to incomplete hemodynamic data. Out of the 32 patients analyzed; 3 developed a pathological increase in PCWP. Among the remaining 29 patients (mean age, 49.4±13.7 years, 34.5% women), 13 showed exercise-induced PH. Resting mPAP was higher in those who developed exercise-induced PH (23.3±5.4 vs 19.0±3.8mmHg; P=.012), although CO was similar in the 2 groups. Patients with exercise-induced PH exhibited data of ventilatory inefficiency with reduced values of end-tidal CO2 pressure at the anaerobic threshold (32.8±3.0 vs 36.2±3.3mmHg; P=.021) and a higher Ve/VCO2 slope (34.2±4.8 vs 30.7±5.0; P=.049). Conclusions Exercise limitation and ventilatory inefficiency could be attributable to exercise-induced precapillary PH in a subgroup of patients with persistent pulmonary thrombosis and dyspnea (AU)


Introdución y objetivos La enfermedad tromboembólica crónica se define como la trombosis crónica de la vasculatura pulmonar y disnea, sin hipertensión pulmonar (HP) en reposo tras una embolia de pulmón. El estudio evaluó la hemodinámica al esfuerzo en estos pacientes y su relación con la ergoespirometría. Métodos Se incluyó a pacientes sintomáticos con trombosis pulmonar crónica tras una embolia de pulmón. Se excluyó a los pacientes con cardiopatía izquierda o HP significativa en reposo (presión arterial pulmonar media [PAPm] >25mmHg, resistencia vascular pulmonar >3 UW y presión capilar pulmonar [PCP] >15mmHg). Se realizó una ergoespirometría y un cateterismo derecho de ejercicio. La HP precapilar al ejercicio se definió como las pendientes PAPm/gasto cardiaco >3 y PCP/gasto cardiaco <2mmHg/l/min. Se comparó la respuesta hemodinámica y ergoespirométrica entre pacientes con y sin HP precapilar al ejercicio. Resultados Se estudió a 36 pacientes, excluyéndose 4 por calidad subóptima en el registro hemodinámico. Tres pacientes presentaron una elevación patológica de la PCP. De los 29 restantes (edad, 49,4±13,7 años; el 34,5% mujeres), en 13 se halló HP precapilar al ejercicio. La PAPm basal fue mayor en aquellos con HP al ejercicio (23,3±5,4 frente a 19,0±3,8mmHg; p=0,012), con similar gasto cardiaco. Aquellos con HP al ejercicio exhibieron valores reducidos de la presión de CO2 exahalada en el umbral anaeróbico (32,8±3,0 frente a 36,2±3,3mmHg; p=0,021) y mayor pendiente Ve/VCO2 (34,2±4,8 frente a 30,7±5,0; p=0,049). Conclusiones La HP precapilar al ejercicio se asocia con limitación funcional e ineficiencia ventilatoria en un subgrupo de pacientes con trombosis arterial pulmonar crónica y disnea persistente (AU)

OBJETIVO: Mapear e sintetizar as evidências sobre os cuidados realizados por enfermeiros para a manutenção do cateter central de inserção periférica neonatal. MÉTODO: O protocolo foi construído seguindo as recomendações do Instituto Joanna Briggs (JBI), atendendo às diretrizes Preferred Reporting Items for Systematic Reviews and Meta-Analyses Extension for Scoping Reviews (PRISMA-ScR). A revisão de escopo descreverá e mapeará as evidências científicas relativas à manutenção do cateter central de inserção periférica neonatal presentes nas literaturas nacionais e internacionais.


OBJECTIVE: To map and synthesize the evidence regarding the care provided by nurses for the maintenance of neonatal peripherally inserted central catheters. METHOD: The protocol was developed following the Joanna Briggs Institute (JBI) recommendations, adhering to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses Extension for Scoping Reviews (PRISMA-ScR) guidelines. The scoping review will describe and map the scientific evidence of maintaining neonatal peripherally inserted central catheters from the national and international literature.

ABSTRACT

Evaluation and staging of liver disease is essential in the clinical decision-making process of liver tumors. The severity of portal hypertension (PH) is the main prognostic factor in advanced liver disease. Performing an accurate hepatic venous pressure gradient (HVPG) measurement is not always possible, especially when veno-venous communications are present. In those complex cases, a refinement in HVPG measurement with a thorough evaluation of each of the components of PH is mandatory. We aimed at describing how some technical modifications and complementary procedures may contribute to an accurate and complete clinical evaluation to improve therapeutic decisions.

ABSTRACT

Primary hyperaldosteronism (PAH) is an important cause of secondary hypertension (HTN). The study of the same requires a high clinical suspicion in addition to a hormonal study that confirms hormonal hypersecretion. It is important to start the appropriate treatment once the diagnosis is confirmed, and for this is necessary to demonstrate whether the hormonal hypersecretion is unilateral (patients who could be candidates for surgical treatment) or bilateral (patients who are candidates for pharmacological treatment only). At the Hospital del Mar since 2016 there has been a multidisciplinary work team in which Nephrologists, Endocrinologists, Radiologists and Surgeons participate to evaluate cases with suspected hyperaldosteronism and agree on the best diagnostic-therapeutic approach for these patients, including the need for adrenal vein sampling, which is a technique that in recent years has become the gold standard for the study of PAH. In the present study we collect the experience of our centre in performing AVC and its usefulness for the management of these patients.

INTRODUCTION AND OBJECTIVES: Chronic thromboembolic disease refers to the presence of chronic thrombotic pulmonary vascular thrombosis without pulmonary hypertension (PH) at rest but with exercise limitation after pulmonary embolism (PE). ABSTRACT Background: Newborn infants admitted to the neonatal intensive care unit require arterial cannulation for hemodynamic monitoring and blood sampling. Arterial access is achieved through catheterization of umbilical or peripheral arteries. Peripheral artery cannulation is performed in critically ill newborns, but artery localization and cannulation is often challenging and unsuccessful. Therefore, increasing the internal diameter and preventing vasospasm are important for successful peripheral artery cannulation in neonates. Topical glyceryl trinitrate has the potential to increase cannulation success by relaxing arterial smooth muscles and thus increasing the internal diameter. We aim to conduct a pilot randomized controlled trial to evaluate the efficacy and safety of topycal glyceryl trinitrate in increasing the diameter of the radial artery in neonates. Methods/Design: This study will be a single-center, observer-blind, randomized, placebo-controlled trial conducted in the neonatal intensive care unit of Perth Children's Hospital, Western Australia. A total of 60 infants born at >34 weeks of gestation who are admitted for elective surgery or medical reasons and for whom a peripheral arterial line is needed for sampling or blood pressure monitoring will be recruited after informed parental consent is obtained. The primary outcome will be the change in radial arterial diameter from baseline to postintervention. Secondary outcomes will be the absolute and percentage change from baseline in the radial arterial diameter in both limbs and safety (hypotension and methemoglobinemia). Discussion: This will be the first randomized controlled trial evaluating the use of topical glyceryl trinitrate to facilitate peripheral artery cannulation in neonates. If our pilot randomized controlled trial confirms the benefits of glyceryl trinitrate patches, it will pave the way for large multicenter randomized controlled trials in this field.


RESUMO Histórico: Os recém-nascidos internados na unidade de terapia intensiva neonatal necessitam de canulação arterial para monitoramento hemodinâmico e coleta de sangue. O acesso arterial é obtido por meio de cateterização das artérias umbilicais ou periféricas. A canulação da artéria periférica é realizada em recém-nascidos em estado grave, mas a localização e a canulação da artéria são, muitas vezes, desafiadoras e sem sucesso. Assim, o aumento do diâmetro interno e a prevenção do vasoespasmo são importantes para o sucesso da canulação da artéria periférica em recém-nascidos. O trinitrato de glicerilo tópico tem o potencial de aumentar o sucesso da canulação ao relaxar a musculatura lisa arterial e, dessa forma, aumentar o diâmetro interno. Nosso objetivo é realizar um ensaio piloto controlado e randomizado para avaliar a eficácia e segurança do trinitrato de glicerilo tópico no aumento do diâmetro da artéria radial em recém-nascidos. Métodos/Desenho: Este estudo será um estudo de centro único, cego para o observador, randomizado, controlado por placebo, conduzido na unidade de terapia intensiva neonatal do Perth Children's Hospital, Austrália Ocidental. Serão recrutados 60 bebês nascidos com mais de 34 semanas de gestação, internados para cirurgia eletiva ou por razões médicas e para os quais é necessária a colocação de um acesso arterial periférico para coleta de amostras ou monitoramento da pressão arterial, após a obtenção do consentimento informado dos pais. O desfecho primário será a mudança no diâmetro arterial radial basal e pós-intervenção. Os desfechos secundários serão a alteração absoluta e percentual basal no diâmetro arterial radial em ambos os membros e a segurança (hipotensão e metemoglobinemia). Discussão: Este será o primeiro estudo controlado e randomizado a avaliar o uso de trinitrato de glicerilo tópico para facilitar a canulação da artéria periférica em recém-nascidos. Se nosso estudo-piloto randomizado e controlado confirmar os benefícios dos adesivos de trinitrato de glicerilo, ele abrirá caminho para grandes estudos multicêntricos randomizados e controlados nesse campo.

ABSTRACT Objective: to describe scientific evidence on good practices for peripheral arterial catheter management. Method: this is an integrative review, carried out through a search in the Latin American and Caribbean Literature in Health Sciences, Virtual Health Nursing Library, National Library of Medicine, Cochrane Library, Cumulative Index to Nursing & Allied Health, Excerpta Medica dataBASE, SciVerse Scopus TopCited and Web of Science databases in March 2021. Articles in Portuguese, English and Spanish, without time limits in the search, were included. Results: forty-nine articles were found. At insertion, measures involved hand hygiene, skin preparation, no-touch technique, aseptic technique and protective barrier, sterile components and transducers, peripheral arterial catheter insertion, insertion attempts, ultrasound and comfort measure use. During maintenance, issues regarding insertion site, invasive blood pressure circuit, connectors, dressing and stabilization were identified, and, during removal, aspects such as local and systemic complications after peripheral arterial catheter removal. Conclusion: the study provides crucial information for the effective management of peripheral arterial catheters, contributing to the reduction of complications and improvement of clinical results. By updating their practices, healthcare professionals can ensure greater safety and well-being for patients, always seeking to provide excellent care.


RESUMEN Objetivo: describir la evidencia científica sobre buenas prácticas para el manejo de catéteres arteriales periféricos. Método: revisión integradora, realizada a través de una búsqueda en las bases de datos Literatura Latinoamericana y del Caribe en Ciencias de la Salud, Biblioteca Virtual en Enfermería en Salud, National Library of Medicine, Cochrane Library, Cumulative Index to Nursing & Allied Health, Excerpta Medica dataBASE, SciVerse Scopus TopCited y Web of Science en marzo de 2021. Se incluyeron artículos en portugués, inglés y español, sin límite de tiempo en la búsqueda. Resultados: se encontraron 49 artículos. En la inserción, las medidas incluyeron higiene de manos, preparación de la piel, técnica de no contacto, técnica aséptica y barrera protectora, componentes y transductores estériles, Inserción de catéter arterial periférico, intentos de inserción, uso de ultrasonido y medidas de comodidad. Durante el mantenimiento se identificaron problemas relacionados con el sitio de inserción, circuito de presión arterial invasiva, conectores, vendaje y estabilización y, durante el retiro, aspectos como complicaciones locales y sistémicas, luego del retiro del catéter arterial periférico. Conclusión: el estudio proporciona información crucial para el manejo eficaz de los catéteres arteriales periféricos, contribuyendo a la reducción de complicaciones y mejora de los resultados clínicos. Al actualizar sus prácticas, los profesionales de la salud pueden garantizar una mayor seguridad y bienestar a los pacientes, buscando siempre brindar una excelente atención.

Conclusão: o estudo fornece informações cruciais para o eficaz manejo do cateter arterial periférico, contribuindo para a redução de complicações e aprimoramento dos resultados clínicos. Objetivo: identificar o conhecimento e sentimentos de pessoas sobre o cateterismo cardíaco. Método: estudo descritivo, abordagem qualitativa, realizado com 12 pacientes em hospital referência em cardiologia, no município de João Pessoa, no período de novembro a dezembro de 2021. Os dados foram coletados através de entrevista gravada e semiestruturada, os dados analisados pela Análise de Conteúdo de Bardin. Resultados: a partir das entrevistas foram construídas três categorias temáticas: C1 - Conhecimento do procedimento de cateterismo cardíaco, C2 - Falta de orientação dos cuidados que envolvem o cateterismo cardíaco e C3 - Sentimentos à realização do cateterismo cardíaco. Os dados evidenciam um conhecimento insuficiente e limitado que podem se relacionar com o surgimento de sentimentos negativos. Considerações finais: Percebe-se a necessidade de orientações e informações, bem como o planejamento e construção de tecnologias educativas para as pessoas que irão realizar o cateterismo cardíaco, com a finalidade de promover um pré, intra e pós-exame seguro


Objective: to identify people's knowledge and feelings about cardiac catheterization. Resumo Objetivo Analisar e sintetizar as evidências científicas disponíveis sobre o cateterismo urinário e as técnicas utilizadas para prevenir a hematúria no esvaziamento da bexiga, na retenção urinária aguda e crônica, em adultos e idosos. Métodos Revisão integrativa realizada nas bases de dados Pubmed, LILACS, Embase, Cochrane, Web of Science e Scopus; e busca manual nas listas de referências dos estudos incluídos. Um período ilimitado foi usado para revisar estudos em português, inglês, francês, alemão e espanhol. Aplicado estratégia PICOS na elaboração da pergunta de pesquisa e instrumento Joanna Briggs Institute (JBI) para avaliar qualidade metodológica dos estudos. Resultados Foram incluídos 11 estudos, com um total de 659 pacientes. A retenção urinária ocorreu principalmente em homens, tendo como principal causa a hiperplasia prostática benigna. As técnicas utilizadas para descompressão vesical, por cateterismo urinário, foram a descompressão rápida e a gradual, sendo as principais complicações, independente da técnica, hematúria e hipotensão. Os resultados mostraram que não houve diferença significativa entre as duas técnicas quanto ao desfecho prevenção da hematúria. Conclusão O esvaziamento gradual da bexiga não previne a hematúria em comparação com o esvaziamento rápido e completo. Mas os estudos, principalmente os relatos de casos, descrevem quadros clínicos que merecem atenção diante da ocorrência de hematúria após cateterismo urinário e as implicações no tratamento dos pacientes. Destaca a importância da enfermagem na identificação e prevenção do diagnóstico de Retenção Urinária, a fim de evitar intervenções posteriores e complicações clínicas, inclusive hematúria pós-cateterismo.

Se utilizó un período ilimitado para revisar estudios en portugués, inglés, francés, alemán y español. Se aplicó la estrategia PICOS en la elaboración de la pregunta de investigación y el instrumento Joanna Briggs Institute (JIB) para evaluar la calidad metodológica de los estudios. Resultados Se incluyeron 11 estudios con un total de 659 pacientes. La retención urinaria ocurrió principalmente en hombres, principalmente a causa de la hiperplasia prostática benigna. Las técnicas utilizadas para la descompresión vesical por cateterismo urinario fueron la descompresión rápida y la gradual, y las principales complicaciones, independientemente de la técnica, fueron hematuria e hipotensión. Los resultados mostraron que no hubo diferencia significativa entre las dos técnicas respecto al resultado de prevención de la hematuria. Conclusión El vaciado gradual de la vejiga no previene la hematuria en comparación con el vaciado rápido y completo. Sin embargo, los estudios, principalmente los relatos de casos, describen cuadros clínicos que necesitan atención ante los episodios de hematuria después del cateterismo urinario y las consecuencias en el tratamiento de los pacientes. Se destaca la importancia de la enfermería en la identificación y prevención del diagnóstico de retención urinaria, a fin de evitar intervenciones posteriores y complicaciones clínicas, inclusive hematuria poscateterismo.

An unlimited period was used to review the studies published in Portuguese, English, French, German, and Spanish. The PICOS strategy was applied to develop the research question and the Joanna Briggs Institute (JBI) instrument was used to assess the methodological quality of studies. Results Eleven studies were included, with a total of 659 patients. Urinary retention occurred mainly in men and the main cause was benign prostatic hyperplasia. Rapid and gradual decompressions were the techniques used for bladder decompression by urinary catheterization, and hematuria and hypotension were the main complications regardless of the technique. The results showed that there was no significant difference between the two techniques in terms of the outcome of hematuria prevention. Conclusion Gradual bladder emptying does not prevent hematuria compared to rapid and complete emptying. Case reports describe clinical conditions that deserve attention when hematuria occurs after urinary catheterization and the respective implications in the treatment of patients. We highlight the importance of nursing in identifying and preventing the diagnosis of Urinary Retention to avoid subsequent interventions and clinical complications, including post-catheterization hematuria.

ABSTRACT Objective: to assess the incidence of complications related to peripherally inserted central catheters in hospitalized adult patients with Covid-19 and to discuss the potential benefits of employing insertion technologies to prevent complications. Method: a descriptive, exploratory and cross-sectional study was conducted from March 2020 to December 2021 at a high-complexity hospital. The study included patients over 18 years old with a positive diagnosis for Covid-19 who made use of peripherally inserted central catheters for venous infusion. Data collection included sociodemographic and clinical information regarding catheter insertion and use. The analysis involved Chi-square and Fisher's Exact tests, with a significance level of 0.05. Results: a total of 123 inserted catheters were analyzed. The patients' mean age was 50 years old (SD=16.37), most of them male and in the acute phase of infection (59.3%). The following significant complications related to the insertion process were identified: catheter material (p=0.01); use of Sherlock (p=0.03); need for traction (p<0.001); number of punctures (p<0.001); and difficulty in catheter progression (p<0.001). Conclusion: the study identified the main complications related to the insertion and use of PICCs and showed that employing vascular visualization technologies such as ultrasound and Sherlock 3CG® can mitigate complications, as well as maximize patient comfort, experience and safety. The research provides support for the implementation of protocols for insertion and management of peripherally inserted central catheters, thus avoiding the occurrence of adverse events.


RESUMEN Objetivo: evaluar la incidencia de complicaciones relacionadas a catéteres centrales de inserción periférica en pacientes adultos hospitalizados con Covid-19 y debatir el potencial de emplear tecnologías de inserción en la prevención de complicaciones. Método: estudio descriptivo, exploratorio y transversal, realizado entre marzo de 2020 y diciembre de 2021 en un hospital de alta complejidad. En el estudio se incluyó a pacientes mayores de 18 años con diagnóstico positivo de Covid-19 y que utilizaran catéteres centrales de inserción periférica para infusiones venosas. Se recolectaron datos sociodemográficos y clínicos sobre la inserción y el uso de los catéteres. El análisis implicó las pruebas de Chi-cuadrado y Exacta de Fischer, con 0,05 como nivel de significancia. Resultados: se analizó un total de 123 catéteres insertados. La media de edad de los pacientes fue de 50 años (DE=16,37), con mayoría del sexo masculino y en la fase aguda de la infección (59,3%). Se hicieron evidentes las siguientes complicaciones significativas vinculadas al proceso de inserción: material del catéter (p=0,01), uso de Sherlock (p=0,03), necesidad de tracción (p<0,001), cantidad de punciones (p<0,001) y dificultad de avance del catéter (p<0,001). Conclusión: el estudio identificó las principales complicaciones relacionadas a la inserción y el uso de catéteres PICC y demostró que utilizar tecnologías de visualización vascular como ultrasonido y Sherlock 3CG® puede mitigar las complicaciones, además de maximizar la comodidad, experiencia y seguridad del paciente. El trabajo de investigación presenta aportes para implementar protocolos de inserción y manejo de los catéteres centrales de inserción periférica, evitando así la incidencia de eventos adversos.

Foram incluídos no estudo pacientes maiores de 18 anos com diagnóstico positivo para Covid-19 e que fizeram uso do cateter central de inserção periférica para infusão venosa. Fez-se coleta de dados sociodemográficos e clínicos sobre a inserção e uso do cateter. A análise envolveu os testes qui-quadrado e exato de Fischer, com nível de significância de 0,05. Resultados: analisou-se um total de 123 cateteres inseridos. A média de idade dos pacientes foi de 50 anos (DP=16,37 anos), sendo a maioria do sexo masculino e na fase aguda da infecção (59,3%). Foram evidenciadas as seguintes complicações significativas atreladas ao processo de inserção: material do cateter (p=0,01), uso de Sherlock (p=0,03), necessidade de tração (p<0,001), número de punções (p<0,001) e dificuldade de progressão do cateter (p<0,001). Conclusão: o estudo identificou as principais complicações relacionadas à inserção e uso do PICC e mostrou que o uso de tecnologias de visualização vascular como o ultrassom e Sherlock 3CG® pode mitigar complicações, além de maximizar conforto, experiência e segurança do paciente. A investigação apresenta subsídios para implementação de protocolos de inserção e manejo do cateter central de inserção periférica, evitando a ocorrência de eventos adversos.

ABSTRACT Objective: to translate and adapt the I-DECIDED® Tool to Portuguese and Brazilian context. Method: this methodological research used Beaton's framework for the translation and cross-cultural adaptation process, which occurred in five stages: initial translation; synthesis of translations; back-translation; committee of experts; and pre-testing. It was carried out from July to December 2022. For data analysis, the Content Validity Index and Cronbach's alpha were used. Results: from a committee of experts, the adapted version of I-DECIDED® obtained satisfactory semantic, idiomatic, experiential and conceptual equivalence when compared to the original version, reaching a Content Validity Index of 0.94. In pre-testing, 60 nurses participated, and the reliability of the adapted tool was 0.83. Conclusion: the translation and cross-cultural adaptation process of I-DECIDED® was carried out and provides Brazilian professionals with an assessment and decision-making tool in relation to peripheral intravenous catheters aligned with patient safety.


RESUMEN Objetivo: traducir y adaptar la herramienta I-DECIDED® al idioma portugués y al contexto brasileño. Método: esta investigación metodológica utilizó el marco de Beaton para el proceso de traducción y adaptación transcultural, que ocurrió en cinco etapas: traducción inicial; síntesis de traducciones; traducción inversa; comité de expertos; y prueba previa. Se realizó de julio a diciembre de 2022. Para el análisis de los datos se utilizó el Índice de Validez de Contenido y el alfa de Cronbach. Resultados: con base en el comité de expertos, la versión adaptada de I-DECIDED® obtuvo equivalencia semántica, idiomática, experiencial y conceptual satisfactoria respecto al original, alcanzando un Índice de Validez de Contenido de 0,94. En el pretest participaron 60 enfermeros y la confiabilidad de la herramienta adaptada fue de 0,83. Conclusión: se realizó el proceso de traducción y adaptación transcultural de la I-DECIDED® que brinda a los profesionales brasileños una herramienta de evaluación y toma de decisiones en relación a los catéteres intravenosos periféricos alineados con la seguridad del paciente.

Método: pesquisa metodológica que utilizou o referencial de Beaton para o processo de tradução e adaptação transcultural, que ocorreu em cinco etapas: tradução inicial, síntese das traduções, tradução reversa, comitê de especialistas e pré-teste. Realizado no período de julho a dezembro de 2022. Para análise de dados, foram utilizados o Índice de Validade de Conteúdo e Alpha de Cronbach. Resultados: a partir do Comitê de Especialistas, a versão adaptada da Ferramenta I-DECIDED® obteve satisfatória equivalência semântica, idiomática, experiencial e conceitual quando comparada à original, atingindo o Índice de Validade de Conteúdo de 0,94. No pré-teste, participaram 60 enfermeiros e a confiabilidade da Ferramenta adaptada foi de 0,83. Conclusão: o processo de tradução e adaptação transcultural da Ferramenta I-DECIDED® foi realizado e disponibiliza aos profissionais brasileiros uma ferramenta de avaliação e tomada de decisão em relação ao cateter intravenoso periférico alinhada à segurança do paciente.
