ABSTRACT

Introducción y objetivos: Existe controversia sobre la mejor estrategia de revascularización en la enfermedad coronaria avanzada, incluidas la enfermedad del tronco coronario y la enfermedad multivaso. Varios metanálisis han comparado resultados a 5 años, pero no hay resultados después del quinto año. Se realizaron una revisión sistemática y un metanálisis de ensayos clínicos aleatorizados para comparar los resultados después del quinto año entre la cirugía de revascularización coronaria (CABG) y la intervención coronaria percutánea (ICP) con stents farmacoactivos.MétodosSe analizaron los ensayos clínicos publicados entre 2010 y 2023. El objetivo primario fue la mortalidad por cualquier causa. Las bases de datos originales se reconstruyeron a partir de las curvas de Kaplan-Meier simulando un metanálisis individual. Se realizaron comparaciones en ciertos puntos de corte (5 y 10 años). Se calculó la diferencia del tiempo medio de supervivencia restringida. Se aplicó el modelo de efectos aleatorios y de DerSimonian-Laird.ResultadosSe analizó a 5.180 pacientes. Durante los 10 años de seguimiento, las ICP muestran una mayor incidencia de mortalidad (HR=1,19; IC95%, 1,04-1,32; p=0,008). La ICP muestra un mayor riesgo de mortalidad a 5 años (HR=1,2; IC95%, 1,06-1,53; p=0,008), mientras que no hubo diferencias de 5 a 10 años (HR=1,03; IC95%, 0,84-1,26; p=0,76). La esperanza de vida de los pacientes sometidos a CABG fue ligeramente mayor (2,4 meses más).ConclusionesEntre los pacientes con enfermedad coronaria avanzada, incluidas la enfermedad del tronco coronario y la enfermedad multivaso, hubo mayor mortalidad tras una ICP que tras la CABG a los 10 años de seguimiento. En concreto, la ICP tiene mayor mortalidad durante los primeros 5 años y un riesgo comparable de 5 a 10 años. (AU)

ABSTRACT

INTRODUCTION AND OBJECTIVES: To evaluate the impact of dexmedetomidine impact on cardiac surgery-associated acute kidney injury (CSA-AKI), kidney function, and metabolic and oxidative stress in patients undergoing coronary artery bypass grafting with heart-lung machine support. METHODS: A randomized double-masked trial with 238 participants (50-75 years) undergoing coronary artery bypass grafting was conducted from January 2021 to December 2022. The participants were divided into Dex (n=119) and NS (n = 119) groups. Dex was administered at 0.5 mcg/kg over 10minutes, then 0.4 mcg/kg/h until the end of surgery; the NS group received equivalent saline. Blood and urine were sampled at various time points pre- and postsurgery. The primary outcome measure was the incidence of CSA-AKI, defined as the occurrence of AKI within 96hours after surgery. RESULTS: The incidence of CSA-AKI was significantly lower in the Dex group than in the NS group (18.26% vs 32.46%; P=.014). Substantial increases were found in estimated glomerular filtration rate value at T4-T6 (P<.05) and urine volume 24hours after surgery (P<.01). Marked decreases were found in serum creatinine level, blood glucose level at T1-T2 (P<.01), blood urea nitrogen level at T3-T6 (P<.01), free fatty acid level at T2-T3 (P<.01), and lactate level at T3-T4 (P<.01). CONCLUSIONS: Dex reduces CSA-AKI, potentially by regulating metabolic disorders and reducing oxidative stress.

ABSTRACT

RESUMEN La decisión sobre la mejor estrategia de revascularización para los pacientes con enfermedad de múltiples vasos se ha tornado una tarea compleja a medida que la angioplastia coronaria ha mejorado sus resultados. En la siguiente revisión nos propusimos evaluar las variables que en nuestra experiencia definen el beneficio de una técnica sobre la otra, entendiendo que de esta manera la decisión del médico tratante se hace más sencilla y objetiva. Por otro lado, y festejando el saludable protagonismo que se le da al paciente, creemos que esta evaluación permite ofrecer argumentos sólidos para ayudarlo en la toma de la decisión.


ABSTRACT The decision on the best revascularization strategy for patients with multivessel disease has become a complex task as coronary angioplasty has improved its results. In the following review, we set out to evaluate the variables that, in our experience, define the benefit of one technique over the other, understanding that in this way the treating physician's decision will become simpler and more objective. On the other hand, and celebrating the healthy prominence given to patients, we believe that this evaluation allows solid arguments to help them in decision making.

Introducción y objetivos: La enfermedad coronaria (EC) es frecuente en pacientes con estenosis aórtica; sin embargo, la estrategia terapéutica óptima sigue siendo objeto de debate. Investigamos los resultados periprocedimiento en pacientes sometidos a implante percutáneo de válvula aórtica con intervención coronaria percutánea (TAVI/ICP) frente al recambio valvular aórtico con injerto de derivación de arteria coronaria (RVAo/CABG) en pacientes con estenosis aórtica con EC.Métodos: Con los datos de alta del Sistema Nacional de Salud Español, se identificaron 6.194 pacientes (5.217 RVAo/CABG y 977 TAVI/ICP) entre 2016 y 2019. Se realizó un análisis emparejado por puntuación de propensión ajustado por características basales. El objetivo primario fue la mortalidad hospitalaria, Los objetivos secundarios fueron las complicaciones hospitalarias y rehospitalización cardiovascular a 30 días.Resultados: Tras el emparejamiento, se seleccionaron 774 parejas de pacientes. La mortalidad total hospitalaria fue más frecuente en el grupo quirúrgico (3,4 frente a 9,4%, p <0,001), al igual que el ictus periprocedimiento (0,9 frente a 2,2%, p=0,004), fallo renal agudo (4,3 frente a 16,0%, p <0,002), transfusión (9,6 frente a 21,1%, p <0,001) y neumonía intrahospitalaria (0,1 frente a 1,7%, p=0,001). La implantación de marcapasos permanente fue más frecuente en el tratamiento percutáneo (12,0 frente a 5,7%, p <0,001). Los centros de menor volumen (< 130 procedimientos por año) tuvieron mayor mortalidad hospitalaria para ambos procedimientos: TAVI/ICP (3,6 frente a 2,9%, p <0,001) y RVAo/CABG (9,3 frente a 6,8%, p <0,001). La rehospitalización cardiovascular a 30 días no difirió entre los grupos.(AU)


Introduction and objectives: Concomitant coronary artery disease (CAD) is prevalent among aortic stenosis patients; however the optimal therapeutic strategy remains debated. We investigated periprocedural outcomes among patients undergoing transcatheter aortic valve implantation with percutaneous coronary intervention (TAVI/PCI) vs surgical aortic valve replacement with coronary artery bypass grafting (SAVR/CABG) for aortic stenosis with CAD.Methods: Using discharge data from the Spanish National Health System, we identified 6194 patients (5217 SAVR/CABG and 977 TAVI/PCI) between 2016 and 2019. In-hospital all-cause mortality was more common in the SAVR/CABG group (3.4% vs 9.4%, P <.001) as was periprocedural stroke (0.9% vs 2.2%; P=.004), acute kidney injury (4.3% vs 16.0%, P <.001), blood transfusion (9.6% vs 21.1%, P <.001), and hospital-acquired pneumonia (0.1% vs 1.7%, P=.001). Permanent pacemaker implantation was higher for matched TAVI/PCI (12.0% vs 5.7%, P <.001). Lower volume centers (< 130 procedures/y) had higher in-hospital all-cause mortality for both procedures: TAVI/PCI (3.6% vs 2.9%, P <.001) and SAVR/CABG (8.3 vs 6.8%, P <.001). Thirty-day cardiovascular readmission did not differ between groups.Conclusions: In this large contemporary nationwide study, percutaneous management of aortic stenosis and CAD with TAVI/PCI had lower in-hospital mortality and morbidity than surgical intervention. Higher volume centers had less in-hospital mortality in both groups. Dedicated national high-volume heart centers warrant further investigation.(AU)

ABSTRACT

Introducción: los dispositivos liberadores de energía permiten la hemostasia de los vasos mediante generación de calor y la coagulación de las proteínas de la pared. Sin embargo, se desconoce su comportamiento a medio plazo en la cirugía arterial con injertos venosos. Objetivos: desarrollar un modelo animal que permita evaluar la eficacia y seguridad del sellado a medio plazo tras el proceso de cicatrización. Comparar y evaluar qué modelo in vivo presenta menor morbilidad y mayor supervivencia a las 4 semanas. Material y métodos: estudio experimental animal de 16 conejos New Zealand a los que se interpuso un fragmento de vena safena humana (VS) con una colateral. Se desarrollaron dos modelos arteriales: bypass termino-terminal de VS en aorta infrarrenal (n = 5) y plastia de aorta con parche de VS (n = 11). La colateral venosa fue sellada, previa aleatorización, con electrocoagulación bipolar controlada por temperatura (EB) o bisturí armónico (BA). Todos los animales recibieron inmunosupresión y profilaxis antitrombótica. Se registró la tasa de paraplejia, de infección, de hemorragia y de supervivencia. Resultados: la supervivencia a los 7 días fue del 50 % (2/4) en el modelo de injerto de interposición. Sin embargo, ningún animal sobrevivió a las 4 semanas de seguimiento en este modelo. En el grupo de plastia de aorta, la supervivencia a los 7 días fue del 55,56 % (5/9) y del 44,44 % (4/9) a las 4 semanas (p = 0,5). La tasa de paraplejia en el grupo de interposición fue del 100 % e inferior en el modelo de plastia de aorta (25 %) (p = 0,03). El tiempo medio de isquemia en el modelo de plastia de aorta (37,11 ± 8,1 min) fue inferior al del grupo del bypass (42 ± 10,61 min) (p = 0,414). En ningún caso se objetivó hemorragia intraabdominal ni reacción adversa a la inmunosupresión. Conclusiones: el modelo arterial de plastia de aorta con parche de VS presentó menor tasa de paraplejia, así como menor mortalidad posoperatoria a los 7 días...(AU)


Introduction: energy sealing devices achieve hemostasis of the vessels through the heat generated and coagula-tion of the vascular wall proteins. However, the mid-term efficacy profile for venous graft sealing in arterial bypasssurgery remains unknown.Objectives: to create an animal model to compare the mid-term efficacy and safety profile at the sealing areaafter the healing process. To compare and assess which in vivo arterial models show lower morbidity and highersurvival rates after 4 weeks.Material and methods: this was an in vivo experimental study of 16 New Zealand rabbits. In each rabbit a humansaphenous vein (SV) with, at least, 1 venous collateral was implanted. Two arterial models were developed: infrarre-nal aorta bypass with SV (n = 5) and aortoplasty with SV patch (n = 11). In both models the collateral was randomizedand sealed with either 1 these 2 energy sealing devices: electrothermal bipolar vessel sealing (EBVS) or Harmonicscalpel (HS). Every animal was treated with antithrombotic prophylaxis and immunosuppressive medication. Therates of intraoperative mortality, paraplegia, infection, bleeding, and survival were all studied.Results: two animals (50 %) survive 7 days after surgery in the bypass model. However, no animal survived 4 daysafter surgery in this model. In the aortoplasty group, the 7-day survival rate was 55.56 % (5/9) while the 4-weeksurvival rate was 44.44 % (4/9) (p = 0.05). The rate of paraplegia was 100 % for the bypass model and much lowerfor the patch group (25 %) (p = 0.03). The mean ischemic time was lower for the aortoplasty model (37.11 ± 8.1 min)compared to the bypass group (42 ± 10.61 min) (p = 0.414). We investigated periprocedural outcomes among patients undergoing transcatheter aortic valve implantation with percutaneous coronary intervention (TAVI/PCI) vs surgical aortic valve replacement with coronary artery bypass grafting (SAVR/CABG) for aortic stenosis with CAD. METHODS: Using discharge data from the Spanish National Health System, we identified 6194 patients (5217 SAVR/CABG and 977 TAVI/PCI) between 2016 and 2019. Propensity score matching was adjusted for baseline characteristics. The primary outcome was in-hospital all-cause mortality. ABSTRACT

RESUMEN Introducción: Los ensayos clínicos aleatorizados que compararon la cirugía de revascularización miocárdica (CRM) con la angioplastia transluminal coronaria (ATC) incluyeron todo tipo de técnicas quirúrgicas (con y sin bomba de circulación extracorpórea) y diversos conductos (arteriales y venosos). ¿Es razonable suponer que todas las técnicas quirúrgicas son iguales en términos de mortalidad tardía? Objetivos: Evaluar si la CRM sin circulación extracorpórea y con el empleo de ambas arterias mamarias tiene un beneficio adicional a la revascularización convencional utilizando una sola arteria mamaria en términos de sobrevida a largo plazo para la enfermedad del tronco de la coronaria izquierda (TCI). Material y métodos: Estudio observacional retrospectivo comparativo (n = 723) ajustado por riesgo. Se realizó análisis estratificado según el uso de arteria mamaria interna única (SITA, n = 144) o ambas arterias mamarias internas (BITA, n = 579). Se analizó la sobrevida a los 10 años de la intervención. Resultados: La supervivencia a los 10 años fue significativamente mayor en el grupo en que se utilizaron ambas arterias mamarias (79,0% ± 3,4% vs 67,0% ± 4,9%, log-rank test, p <0,01). Este beneficio también se observó en el análisis ajustado por riesgo (93,0% ± 4,6 vs 69,0% ± 5,7 respectivamente, p = 0,03). El uso de ambas arterias mamarias fue un predictor independiente de sobrevida a 10 años (HR 0,57, IC 95% 0,37-0,87; p = 0,01). Conclusión: El uso de ambas arterias mamarias internas en pacientes con enfermedad del tronco coronario izquierdo sometidos a revascularización coronaria sin circulación extracorpórea se asoció con mayor sobrevida a los 10 años.


ABSTRACT Background: The randomized controlled trials comparing coronary artery bypass graft (CABG) surgery versus percutaneous coronary intervention (PCI) included all types of surgical techniques (on-pump and off-pump) and different conduits (arterial and venous). Is it reasonable to assume that all surgical techniques are equal in terms of late mortality? Objectives: The aim of this study was to evaluate whether off-pump CABG surgery using both mammary arteries provides additional benefit over conventional revascularization using single mammary artery in terms of long-term survival for left main coronary artery (LMCA) disease. Methods: We conducted a retrospective, observational and comparative study (n=723) adjusted for risk. A stratified analysis was performed according to the use of single internal thoracic artery (SITA, n=144) or bilateral internal thoracic arteries (BITA, n=579) analyzing survival at 10 years after the intervention. Results: Survival at 10 years was significantly higher in BITA group (79.0%±3.4% vs 67.0%±4.9%, log-rank test, p <0.01). This advantage was also observed in the risk-adjusted analysis (93.0%±4.6 vs 69.0%±5.7 respectively, p=0.03). The use of BITA was an independent predictor of 10-year survival (HR 0.57, 95% CI 0.37-0.87, p=0.01). Conclusion: The use of bilateral internal mammary arteries in patients with left main coronary artery disease undergoing off-pump coronary artery bypass grafting was associated with higher survival at 10 years.

Introducción: la enfermedad renal crónica (ERC) acelera el proceso de arterioesclerosis, lo que incrementa la prevalencia de enfermedad arterial periférica (EAP). El objetivo de esta revisión sistemática es evaluar los resultados del trasplante renal en pacientes sometidos a cirugía abierta de revascularización del sector aortoilíaco por arteriopatía crónica de las extremidades inferiores. Material y métodos: revisión sistemática según las recomendaciones QUOROM de artículos en PubMed y Cochrane, en español e inglés, que incluían pacientes con ERC y cirugía abierta de revascularización del sector aortoilíaco en el pasado o como indicación previa a un trasplante renal. Los artículos incluidos describían complicaciones inmediatas, supervivencia del injerto y supervivencia del paciente después del trasplante renal. Resultados: la búsqueda generó 253 artículos. Tras la revisión se seleccionaron 8 artículos que incluían 101 pacientes, de los que el 84,2 % fueron trasplantados. La incidencia de complicaciones tras la revascularización fue del 25,9 % y la incidencia de complicaciones tras el trasplante renal fue del 28,2 %. La mediana de seguimiento fue de 22 meses (rango: 6,7 a 71). La supervivencia del injerto renal fue del 80 % y la del paciente fue del 90,5 % al final del seguimiento. Conclusión: la coexistencia de enfermedad arterial y renal no debe considerarse un obstáculo para la realización de un trasplante renal. Los datos publicados hasta el momento muestran resultados satisfactorios en la supervivencia del injerto y del paciente.(AU)


Introduction: chronic kidney disease (CKD) increases and accelerates the arterial calcification process, increasing the prevalence of peripheral arterial disease in these patients. The aim of this systematic review is to evaluate the results of kidney transplantation in patients who have undergone open revascularization surgery in the aorto-iliac sector for chronic lower limb arteriopathy. Material and methods: systematic review, following QUORUM recommendations, of articles in PubMed and Cochrane, in English or Spanish, which include patients with CKD, who have undergone open revascularization surgery of the aorto-iliac sector in the past or as a prior indication to kidney transplantation. Articles included described immediate complications, graft survival and patient survival after kidney transplantation. Results: the search generated 253 articles and after the systematic review, 8 articles that included, 101 patients were selected, 84.2 % of whom were transplanted. Complications of revascularization surgery were 25.9 % and complications of kidney transplantation were 28.2 %. Median follow-up was 22 months (range: 6.7 to 71). Graft survival was 80 % and patient survival was 100 % at the end of follow-up. Conclusion: the coexistence of vascular and kidney disease should not be an obstacle to performing a kidney transplantation. Since the data published so far shows satisfactory results in graft and patient survival.(AU)

ABSTRACT

OBJECTIVE: The objective of the study was to systematically evaluate the effect of coronary artery bypass grafting (CABG) or CABG combined with mitral valve surgery (cMVS) on post-operative survival in patients with moderate ischemic mitral valve regurgitation. MATERIALS AND METHODS: Databases including PubMed, Web of Science, COCHRANE LIBRARY, WanFang Data, and CNKI Data were searched from inception to January 2020. According to the inclusion criterion, relevant articles were screened. After that we extracted data, assessed quality, and performed meta-analysis using RevMan 5.2. RESULTS: A total of 4 randomized controlled trial and 14 retrospective study involving 4476 patients were included in the study. The CABG group was 2278 and the cMVS group was 1698. The results of meta-analysis showed that compared with CABG group, there were no statistically significant differences in the recent mortality (odds ratio [OR] = 0.88, p = 0.62), 1-year survival (OR = 1.03, p = 0.82), 1-year survival (OR = 1.07, p = 0.62), and long-term survival (OR = 0.95, p = 0.61) of the cMVS group. CONCLUSION: Current evidence indicates that patients in the cMVS group did not benefit from CABG group in survival after surgery.


OBJETIVO: . Evaluar sistemáticamente el efecto del injerto de derivación de la arteria coronaria (CABG) o el injerto de derivación de la arteria coronaria combinados con la cirugía de la válvula mitral (cMVS) sobre la supervivencia posoperatoria en pacientes con insuficiencia valvular mitral isquémica moderada. MATERIAL Y MÉTODOS: . Se realizaron búsquedas en bases de datos que incluyen Pubmed, Web of Science, COCHRANE LIBRARY, WanFang Data y CNKI Data desde el inicio hasta enero de 2020. De acuerdo con el criterio de inclusión, se seleccionaron los artículos relevantes. Después de eso, extrajimos los datos, evaluamos la calidad y realizamos el metanálisis con RevMan 5.2. RESULTADOS: . Se incluyó un total de 4 ensayos controlados aleatorios (ECA) y 14 estudios retrospectivos con 4476 pacientes. El grupo CABG fue 2278, el grupo cMVS fue 1698. Los resultados del metanálisis mostraron que, en comparación con el grupo CABG, no hubo diferencias estadísticamente significativas en la mortalidad reciente (OR = 0.88, p = 0.62), supervivencia a 1 año (OR = 1.03, p = 0.82), supervivencia a 1 año (OR = 1.07, p = 0.62) y supervivencia a largo plazo (OR = 0.95, p = 0.61) del grupo cMVS. CONCLUSIÓN: . La evidencia actual indica que los pacientes del grupo cMVS no se beneficiaron del grupo CABG en la supervivencia después de la cirugía.

INTRODUCTION: Moyamoya disease (MD) is a progressive, occlusive disease of the arteries of the anterior cerebral circulation that may cause ischaemia or haemorrhage. Patient management aims to prevent new cerebrovascular events through surgical revascularisation and/or pharmacological treatment. MATERIALS AND METHODS: We studied a series of 17 patients with MD (n = 14) or moyamoya syndrome (MS; n = 3), who were evaluated between January 1989 and December 2016; 11 patients were women and 6 were men. Thirteen patients had definitive MD (76%), one had unilateral MD (5.2%), and 3 had MS (18%). The condition manifested as intraparenchymal haemorrhage (in 35.2% of patients), brain ischaemia (29.4%), subarachnoid haemorrhage (17.6%), seizures (11.7%), and headache with no associated haemorrhage (1 patient). RESULTS: Ten patients (58.8%) underwent revascularisation and 7 (41.2%) received pharmacological treatment. All patients were evaluated with the modified Rankin Scale (mRs) at admission and at the last consultation; mRs scores were significantly lower in the group undergoing surgery (P <  .04). During follow-up, none of the patients undergoing revascularisation experienced recurrences, whereas 2 patients receiving pharmacological treatment did experience a new vascular event (one ischaemic and one haemorrhagic) (P <  .05). No significant differences were observed between the treatment outcomes of different revascularisation techniques. CONCLUSIONS: Although our population has different demographic characteristics from those of other non-Asian populations, ours is the largest published series of Hispanic individuals with MD. Our results support the use of revascularisation procedures to improve these patients' neurological status and to prevent new cerebrovascular events.

Introducción: La enfermedad moyamoya (EM) es una arteriopatía oclusiva y progresiva de la circulación cerebral anterior que puede producir eventos isquémicos o hemorrágicos. El objetivo terapéutico es la prevención de nuevos eventos a través de tratamiento médico o cirugía de revascularización.MétodosSe estudió a 17 pacientes con EM y síndrome de moyamoya, atendidos de enero de 1989 a diciembre del 2016. Trece pacientes tenían EM definitiva (76%), un paciente EM unilateral (5,2%) y 3 pacientes síndrome de moyamoya (18%). Once pertenecían al sexo femenino y 6 al masculino. Su forma de presentación fue hemorragia intraparenquimatosa (35,2%), isquemia cerebral (29,4%), hemorragia subaracnoidea (17,6%), crisis convulsivas (11,7%) y un paciente empezó con cefalea sin hemorragia.ResultadosEn 10 pacientes se realizó una cirugía de revascularización (58,8%) y 7 se manejaron médicamente (41,2%). La valoración neurológica, empleando la escala modificada de Rankin al ingreso y en su última consulta, mostró una diferencia significativa (p < 0,04) a favor del grupo quirúrgico. Ningún paciente del grupo quirúrgico presentó nuevos eventos vasculares, mientras que 2 pacientes del grupo médico desarrollaron uno nuevo (uno isquémico y otro hemorrágico). En nuestro estudio no hubo una diferencia significativa entre los métodos de revascularización, pero sí en comparación con los pacientes que se trataron médicamente (p < 0,05).ConclusionesSi bien nuestra población muestra diferencias con otras series reportadas fuera de Asia, esta es la serie reportada más numerosa para una población hispana. Nuestros resultados indican que los procedimientos de revascularización son útiles para prevenir nuevos eventos vasculares isquémicos o hemorrágicos. (AU)

Abstract Introduction: Ankle-brachial pressure index is an objective, noninvasive test for predicting subclinical atherosclerotic diseases. We investigated the role of ankle-brachial pressure index measured with automated sphygmomanometer devices in the prediction of the development of acute kidney injury in patients undergoing major cardiac surgery. Methods: This single-centered, cross-sectional, and observational study was performed on 80 (66 males and 14 females, 58 ± 10 years) patients undergone cardiac surgery. Complete anamnesis, laboratory tests, intravenous fluids, medications, blood products, and all perioperative procedures were recorded in all patients before the surgery. Two automated sphygmomanometer devices giving equivalent results were used for measuring Ankle-brachial pressure index. The data in the first two days after the surgery were used for analysis. The criteria of AKIN were used in the diagnosis of acute kidney injury. Results: Twenty-one (23%) patients developed acute kidney injury in the postoperative period. None of the patients needed renal replacement therapy or died. There was no significant difference between mean ankle-brachial pressure index levels of patients with and without acute kidney injury (1.04 ± 0.17 and 1.06 ± 0.19, respectively, p=0.554). The mean ankle-brachial pressure index was significantly lower in patients with perioperative complications that cause hemodynamic instability (1.07 ± 0.14, 0.96 ± 0.13, p=0.016). On the multivariate analysis model, only perioperative hemodynamic complication development was found to be related to postoperative acute kidney injury. Conclusion: Ankle-brachial pressure index may have a role in predicting perioperative hemodynamic complications, which may cause acute kidney injury in patients undergoing major surgery. Simple automatic blood pressure devices can be used in daily practice for ankle-brachial pressure index measurement instead of complex and expensive doppler devices.


Resumen Introducción: El índice tobillo-brazo (ITB) es una prueba objetiva y no invasiva para diagnosticar la aterosclerosis asintomática. Investigamos el papel del índice tobillo-brazo medido a través de esfigmomanómetros automáticos para pronosticar el desarrollo de insuficiencia renal aguda en pacientes que se sometieron a una cirugía cardíaca mayor. Material y métodos: En este estudio observacional, transversal y unicéntrico, se incluyó a 80 pacientes (66 hombres y 14 mujeres de 58 ± 10 años) que se sometieron a una cirugía cardíaca. Se registraron los siguientes datos de todos los pacientes antes de la cirugía: anamnesis completa, análisis clínicos, líquidos intravenosos, medicamentos, productos hemoderivados e intervenciones perioperatorias. Para medir el ITB, se utilizaron dos esfigmomanómetros automáticos que arrojaron resultados similares. Se analizaron los datos recogidos los primeros dos días luego de la cirugía. Se siguieron los criterios de la AKIN para diagnosticar la insuficiencia renal aguda. Resultados: Veintiún pacientes (23 %) sufrieron insuficiencia renal aguda en el postoperatorio. Ninguno de los pacientes necesitó tratamiento renal sustitutivo ni falleció. No hubo diferencias significativas entre los valores medios del índice tobillo-brazo en pacientes con insuficiencia renal aguda y sin ella (1,04 ± 0,17 y 1,06 ± 0,19, respectivamente; p=0,554). El valor medio del ITB fue significativamente menor en pacientes con complicaciones perioperatorias que causan inestabilidad hemodinámica (1,07 ± 0,14; 0,96 ± 0,13; p=0,016). En el modelo de análisis multivariado, solo se encontró que la aparición de complicaciones hemodinámicas perioperatorias estaba relacionada con la insuficiencia renal aguda luego de la operación quirúrgica. Conclusión: Es posible que el ITB desempeñe un papel en la predicción de complicaciones hemodinámicas perioperatorias, que pueden causar insuficiencia renal aguda en pacientes sometidos a cirugía mayor. En la práctica diaria, pueden utilizarse dispositivos automáticos simples que calculan la tensión arterial para medir el índice tobillo-brazo, en lugar de dispositivos Doppler complejos y costosos.

INTRODUCTION AND OBJECTIVES: To analyze the association between volume and outcomes in coronary artery bypass grafting (CABG) in the Spanish National Health System. METHODS: We analyzed CABG episodes from 2013 to 2015. The selected outcome variables were in-hospital mortality in the index episode, 30-day cardiac-related readmissions, and mortality during readmission. Risk-adjusted rates of in-hospital mortality (RAMR) and 30-day readmissions (RARR) were calculated using multilevel logistic regression. High- and low-volume hospitals for CABG were identified by a nonconditioned analysis (k-means) and by compliance with the volume recommendation of clinical practice guidelines. RESULTS: A total of 17 335 CABG index episodes were included, with a crude in-hospital mortality rate of 5.0%. Episodes attended in low-volume centers for CABG (< 155 CABG per year) showed 17% higher RAMR (5.81%±2.07% vs 4.96%±1.76%; P <.001) and a negative linear correlation between volume and RARR (r=-0.318; P=.029), as well as a higher percentage of complications during the episode. The same association between volume and more favorable outcomes was found in isolated CABG. CONCLUSIONS: The mean CABG volume is low in Spanish National Health System hospitals. Higher volume was associated with better outcomes in CABG, both total and isolated. The findings of this study indicate the need for a higher concentration of CABG programs, as well as the publication of risk-adjusted outcomes of coronary intervention.

Resumen Antecedentes: El mejor método para el tratamiento de la revascularización de la lesión del tronco no protegido de la coronaria izquierda es un tema de creciente interés. Método: Se realizaron un total de 2,439 intervenciones coronarias percutáneas (ICP) durante 3 años; se incluyeron los pacientes con lesión del tronco no protegido de la coronaria izquierda (TCI; n = 48) y se compararon con pacientes a los que se realizó cirugía de revascularización coronaria (CRVC; n = 50). Se analizaron los eventos cardiovasculares en fase hospitalaria y extrahospitalaria. El promedio de seguimiento fue de 16 meses. Resultados: El riesgo cardiovascular fue mayor en pacientes con ICP; log EuroSCORE (16 ± 21 vs. 5 ± 6, p = 0.001), Syntax clínico (77 ± 74 vs. 53 ± 39, p = 0.04). Los pacientes del grupo de ICP presentaron al ingreso con más frecuencia infarto con elevación del ST (IMCEST) y con choque cardiogénico. Los eventos hospitalarios fueron similares en ICP y CRVC (14% vs. 18%, p = 0.64). El IMCEST fue menos frecuente en el grupo de ICP (0% vs. 10%, p = 0.03. Los eventos cardiovasculares fueron menores en el grupo de ICP (2.3% vs. 18%, p = 0.01) y hubo una disminución de la muerte general y cardiaca (2.3% vs. 12%, p = 0.08 y 2.3% vs. 8%, p = 0.24), al excluir los pacientes con choque cardiogénico como presentación. En la fase extrahospitalaria los eventos fueron similares en ICP y CRVC (15% vs. 12%, p = 0.46). La supervivencia sin eventos cardiovasculares, muerte general y cardiaca fueron comparables entre los grupos (log rank, p = 0.38, p = 0.44 y p = 0.16). INTRODUCTION AND OBJECTIVES: The predictive value of the SYNTAX score (SS) for clinical outcomes after transcatheter aortic valve implantation (TAVI) is very limited and could potentially be improved by the combination of anatomic and clinical variables, the SS-II. We aimed to evaluate the value of the SS-II in predicting outcomes in patients undergoing TAVI. METHODS: A total of 402 patients with severe symptomatic aortic stenosis undergoing transfemoral TAVI were included. Preprocedural TAVI angiograms were reviewed and the SS-I and SS-II were calculated using the SS algorithms. Patients were stratified in 3 groups according to SS-II tertiles. The coprimary endpoints were all-cause death and major adverse cardiovascular events (MACE), a composite of all-cause death, cerebrovascular event, or myocardial infarction at 1 year. RESULTS: Increased SS-II was associated with higher 30-day mortality (P=.036) and major bleeding (P=.015). The 1-year risk of death and MACE was higher among patients in the 3rd SS-II tertile (HR, 2.60; P=.002 and HR, 2.66; P<.001) and was similar among patients in the 2nd tertile (HR, 1.27; P=.507 and HR, 1.05; P=.895) compared with patients in the 1st tertile. The highest SS-II tertile was an independent predictor of long-term mortality (P=.046) and MACE (P=.001). CONCLUSIONS: The SS-II seems more suited to predict clinical outcomes in patients undergoing TAVI than the SS-I. Increased SS-II was associated with poorer clinical outcomes at 1 and 4 years post-TAVI, independently of the presence of coronary artery disease.

MACCE were similar in both groups in the out-patient phase (15% vs. 12%, P=.46). Survival without MACCE, general and cardiac death were comparable between groups (log rank, P=.38, P=.44 and P=.16, respectively). CONCLUSION: Even though the clinical and peri-procedural risk profile of the PCI patients were higher, the in-hospital and out-hospital efficacy and safety were comparable with CABG.

ABSTRACT

Resumen Las mujeres diabéticas con la enfermedad arterial coronaria sometidas a revascularización tienen mayor riesgo de reestenosis debido a las alteraciones fisiopatológicas a nivel vascular propias de la diabetes, a la disminución del efecto protector de los estrógenos después de la menopausia y porque el calibre de sus arterias es menor que el de las no diabéticas y menor que el de los hombres. El objetivo de este artículo es comparar la tasa de reestenosis en la arteria coronaria descendente anterior en las mujeres diabéticas que se sometieron a intervención coronaria percutánea más el stent, frente a la cirugía de revascularización. Para ello se realizó una revisión de la literatura utilizando artículos publicados en bases de datos científicas, que compararan las dos técnicas, haciendo énfasis en la población diabética femenina. Encontrando que hubo una ventaja de la cirugía de revascularización sobre la intervención coronaria percutánea en los eventos de revascularización de la lesión diana (OR 0,253, IC 95% 0,092-0,703, p = 0,008) y revascularización del vaso diana (OR 0,185, 95% CI 0,079 a 0,432, p < 0,001), correspondiente a una ventaja en la presentación de evento cardiovascular y cerebrovascular (OR 0,429, CI 95% 0,254-0,723, p = 0,001). Finalmente, se hizo un análisis crítico, concluyendo que la cirugía de revascularización es la técnica de elección en las pacientes diabéticas puesto que tiene una tasa de reestenosis menor y con ello menos eventos adversos que de esta se derivan.


Abstract Women with diabetes and coronary artery disease who undergo revascularization have a higher risk of restenosis due to the pathophysiological alterations at vascular level inherent to diabetes, to the decrease of the protective effect of estrogens after menopause and to the caliber of their arteries being smaller than women without diabetes or men. The goal of this study is to compare the rate of restenosis in anterior descending coronary artery in women with diabetes who underwent percutaneous coronary intervention and the stent, compared to revascularization surgery. For that a literature review was carried out using medical articles published in scientific databases where both techniques were compared, and focusing on female population with diabetes. Results showed there was an advantage for the revascularization surgery versus percutaneous coronary intervention in the revascularization events of the target lesion (OR 0.253, IC 95% 0.092-0.703, p = 0.008) and revascularization of the target vessel (OR 0.185, 95% CI 0.079 a 0.432, p <0.001), corresponding to an advantage in the presentation of the cardiovascular and cerebrovascular event (OR 0.429, CI 95% 0.254-0.723, p = 0.001). Finally, a critical analysis was conducted, thus concluding that revascularization is the chosen technique for female patients with diabetes because of the lower restenosis rate, avoiding its adverse events.
