ABSTRACT

We aimed to determine the association between the duration of widowhood and cognition decline. We compared the decline observed in widowed people compared with married, single, or separated persons using the scores obtained in the cognitive assessment of memory, learning, and visual exploration by adults and older adults in Mexico. The Mexican Health and Aging Study (MHAS) provides the base for this paper. This study is an analysis of the fourth data wave (2015), except for the independent variable: marital status. Marital status was built longitudinally with information from the four surveys (2001, 2003, 2012, and 2015). The sample comprised 6898 adults aged 50 and over. Cognition was assessed with an adapted Cross-Cultural Cognitive Examination (CCCE). Confounders include sociodemographic characteristics (sex, age, schooling, self-perception of economic status, and whether the individual worked or not), multimorbidity, functionality, support networks, and psychological characteristics. Of the total sample, 4094 (59.3%) were women. The mean age was 70.86 years (SD = 7.4). The baseline of the study is 2001. In 2001, 8.7% (n = 600) were widows or widowers. People widowed by 2001 scored -0.158 points in cognition while divorced participants scored - 0.095 points.

ABSTRACT

Surrogacy has been prohibited in France since the Court of Cassation condemned it on the grounds that 'only merchandise can be the object of contracts' (in 1991), and decided that 'any contract concerning procreation or gestation on behalf of a third party is void' (in 1994). French people who undertake surrogacy abroad act knowingly against French law. Once a child is born through surrogacy on foreign soil, except in rare cases, the birth is not included in the French register and the newborn is thus held at the gates of the national community. Most of these children have a foreign birth certificate and the nationality of their land of birth. This paper presents research currently in progress, and reports on interviews conducted in France with 28 families, 16 of which were formed by gay male couples, 11 by heterosexual couples and one by an unpartnered gay man. One surrogacy arrangement took place in Russia, one in Poland and one in India; the others were completed in North America (mostly in the USA but also in Canada). The research method and the characteristics of the families are described briefly, in addition to the legal, ethical and political context concerning surrogacy in France. The situation of the children born abroad through surrogacy is then analysed, to demonstrate that it is reminiscent of the historical assimilation of filiation with transmission of French nationality, from which children considered illegitimate have been excluded.

ABSTRACT

Introducción: debido a sus conocidas y múltiples virtudes, la OMS y la UNICEF recomiendan la lactancia materna como alimento insustituible en los primeros seis meses de vida. No obstante, los resultados en cuanto a su utilización no son siempre los esperados. Objetivos: evaluar la relación entre la edad de las gestantes, el estado civil, la ontogenia de las familias, los conocimientos que manifestaban poseer sobre lactancia materna y los que realmente poseían. Métodos: se realizó un estudio analítico y transversal de 192 gestantes en su tercer trimestre, entre marzo de 2010 y marzo de 2011 en los Policlínicos "Turcios Lima" y "Mártires del 9 de Abril" del Municipio San José de las Lajas. Se aplicaron 22 preguntas mediante entrevista y se estableció un sistema de puntaje para evaluar el nivel de conocimientos. Resultados: predominaron las gestantes de 19 a 35 años que mantenían una unión consensuada, pertenecían a familias extensas, deseaban lactar y se consideraban con los conocimientos necesarios para lograrlo, aunque realmente dichos conocimientos eran insuficientes para ese proceder. Las pacientes con mayores deficiencias fueron las adolescentes y las solteras, que estaban enmarcadas en familias extensas y que a priori habían reconocido que no poseían los conocimientos adecuados sobre lactancia materna. Conclusiones: aunque la mayoría de las gestantes consideró que tenía los conocimientos necesarios para la lactancia materna, no demostró tener los indispensables para asumirla exitosamente(AU)


Introduction: Due to its known and many virtues, breastfeeding is recommended by WHO and UNICEF as irreplaceable food in the first six months of life. However, the results in terms of use are not always as expected. Objectives: Evaluate the relationship between ages of pregnant women, marital status, ontogeny of families, and their knowledge about breastfeeding. Methods: A cross-sectional study of 192 pregnant women in their third trimester was conducted from March 2010 to March 2011 at the polyclinics "Turcios Lima" and "Martyrs of the April 9" in San Jose de las Lajas the municipalities. A 22 question- interview was applied and a point system was established to assess the level of knowledge. Results: Pregnant women aging 19-35 years prevailed. They had a consensual union and lived in extended families. They wished to breastfeed and considered themselves to have the knowledge to do it, although that knowledge was not sufficient to proceed. Patients with major deficiencies were adolescents and unmarried, which lived in extended families and who had previously acknowledged that they did not have the proper knowledge about breastfeeding. Conclusions: Although most pregnant women saw the skills needed for breastfeeding, they did not demonstrate the necessary to successfully do it(AU)

ABSTRACT

BACKGROUND: Emergency presentation affects up to every fourth patient with colon cancer, and is associated with worse outcomes. The aim of this study was to investigate any association between socioeconomic status (SES) and mode of presentation in colon cancer. MATERIALS AND METHODS: Individually attained data on civil status, education and income were linked to quality registries for colon cancer in two large Swedish regions 1997-2006 (n = 12 293) and analyzed by logistic regression, adjusting for age, sex, stage, region and socioeconomic variables. RESULTS: The frequency of emergency presentation was 23%; 27.8% among patients above the age of 80, and 20.0% among patients aged 70-79 (p < 0.001). There was no difference between men and women (22.6% vs. 23.8%; p = 0.1). Among patients with stage IV colon cancer, 34.6% presented as emergencies. Odds ratio for an emergency presentation in unmarried patients was 1.24 (96% CI 1.04-1.48), and for unmarried patients above the age of 80, OR was 1.45 (95% CI 0.98-2.13). Among patients below the age of 70 with compulsory education only, OR was 1.22 (95% CI 0.98-1.48). For patients within the lowest income quartile (Q1), OR was 1.24 (95% CI 1.04-1.49). This was most pronounced in men (OR 1.34; 95% CI 1.40-1.72), in patients below the age of 70 (OR 1.36; 95% CI 1.02-1.82), and above the age of 80 (OR 1.41; 95% CI 1.00-1.98). CONCLUSION: Emergency presentation of colon cancer is consistently associated with socioeconomic factors, and this must be considered in efforts aimed at reducing the overall frequency of emergency cases.
