ABSTRACT
Background: Cenurosis is a parasitic disease caused by Coenurus cerebralis, an intermediate form of Taenia multiceps multiceps, causing a fatal disease in production animals. Its adult form (Taenia) lodges in the small intestine of canids and can infect several intermediate hosts. Sheep are the main species affected by the disease, having nervous symptoms as one of its manifestations. This study aimed to describe the changes observed in computed tomography, as well as the clinical findings of a case of Cenurosis in a sheep on the western border of Rio Grande do Sul. Case: A ram was referred to the Centro Universitário da Região da Campanha (URCAMP) with neurological signs. Clinical, hematological, radiographic, tomographic and necroscopic evaluation of the animal was performed. There were no significant hematological and radiographic changes. During the neurological examination, corneal opacity was found in the right eyeball, associated with a visual deficit in the same eye. Also, when stimulated to move, it was possible to observe ataxia with ambulation to the left side, with right lateral displacement of the head. The tomography showed a hypodense area of approximately 3 cm at the base of the brain, in the region of the thalamus and third ventricle. Macroscopically, a translucent spherical cyst with approximately 4 cm in diameter was observed, containing the protoescolex/scolices of Taenia multiceps in the same region. Discussion: Computed tomography identified the presence of an apparently circular volume of approximately 4 cm in diameter, causing ventricular dilatation. This evidence of ventricular dilation corroborates aspects described in the literature, which found bilateral dilation of the ventricles by compression promoted by a cyst in the 4th ventricle. As the location of the cyst was located at the base of the brain, in the region of the third ventricle, it would be difficult to perform the surgery in the treatment of this case. Signs of ataxia, walking movements, decreased reflexes, nystagmus, unilateral blindness and lateral decubitus are commonly observed. Reports described that the main neurological alterations observed in 20 sheep with cenurosis were postural deficit, locomotion alteration, visual deficit and behavioral alterations, with more than 50% of the animals showing at least one of these signs. The main neurological signs observed in the case described were changes in gait, with walking, blindness, lateral head deviation and ataxia. These signs guide the clinical diagnosis of cenurosis. At necropsy, a spherical translucent cyst with approximately 4 cm in diameter was observed, containing the protoescolex/scolices of Taenia multiceps, which extended dorsally from the third ventricle to the base of the thalamus, laterally displaced to the right side. Rostrocaudal the lesion extended rostrally across the entire length of the thalamus to the beginning of the caudate nucleus and caudally to the base of the caudal colliculus, causing compression of the adjacent parenchyma, enabling the diagnosis of cenurosis. Similar findings are described as Coenurus cerebralis. This lesion pattern macroscopically characterizes the parasite lesions. Thus, it is concluded that the visualization of a hypodense area in CNS regions of sheep with neurological signs allows the diagnosis of cenurosis and precise location of the lesion. The description of this case adds information so that other professionals in the field can be successful in diagnosing the disease.

Deaths of 130 (16.6%) adult native goats in a herd (n=780) were reported from January to June 2017. Affected goats showed various nervous signs ended by death. Investigations for thiamine deficiency, polioencephalomalacia, caprine arthritis encephalitis, and listeriosis were negative. Upon necropsy, multiple (1-4) thin-walled cysts 2-3.5 cm in diameter containing clear fluid with numerous clusters of protoscolices in the cerebrum and cerebellum had replaced the brain parenchyma, causing space-occupying lesions. Parasitologically, the recovered cysts were Coenurus cerebralis, based on the arrangement of protoscolices, and the number and size of their hooks. Morphologically, each protoscolex had four suckers and a rostellum with double-crown hooks. The large and small hooks were 157.7±0.5 µm and 115±0.6 µm in length, respectively. Histopathologically, the parasite destroyed the affected tissues associated with multifocal to diffuse lymphocytic, non-suppurative meningoencephalitis; ischemic neuronal necrosis; and malacia. This is the first report of cerebral coenurosis in livestock in Oman, which should alert the local public health authorities for the application of prevention and control measures.
Resumo Este estudo documentou o primeiro surto de coenurose cerebral em cabras em Salalah, Oman. A morte de 130 (16,6%) caprinos adultos nativos (n=780) foi relatada de janeiro a junho de 2017. As cabras afetadas mostraram distúrbios neurológicos, que culminaram em óbito. Investigações para deficiência de tiamina, polioencefalomalácia, encefalite por artrite caprina e listeriose foram negativas. Na necropsia, múltiplos (1-4) cistos de paredes finas com 2-3,5 cm de diâmetro contendo líquido claro com numerosos aglomerados de protoescólices no cérebro e no cerebelo haviam substituído o parênquima cerebral, causando compressão nas estruturas adjacentes. Os cistos recuperados foram identificados como sendo de Coenurus cerebralis, com base no arranjo dos protoescólices, e no número e tamanho de seus ganchos. Morfologicamente, cada protoescólice tinha quatro ventosas e um rostelo com dupla coroa de ganchos. Os ganchos grandes e pequenos tinham 157,7±0,5 µm e 115±0,6 µm de comprimento, respectivamente. Histopatologicamente, o parasita causou a destruição dos tecidos afetados associada à meningoencefalite linfocítica não-supurativa, que variou de multifocal a difusa, necrose neuronal isquêmica e malacia. Este é o primeiro relato de coenurose em ruminantes no Oman, o que deve servir de alerta para as autoridades locais da área de saúde para a aplicação de medidas de prevenção e controle.

The aims of this study were to diagnose coenurosis by means of computerized tomography (CT) scan imaging and molecular characterization of the CO1 gene using the polymerase chain reaction (PCR). Sheep and calves were necropsied, and CT scans on the cephalic region were performed on the animals. Sections of brain tissue infected with parasites were then stained with hematoxylin and eosin for microscopic examination. Material collected from brain cysts was fixed in 70% ethanol. PCR amplification was carried out using the CO1 mitochondrial gene. A total of 60 calves and 80 sheep were examined clinically and, of these, 15 calves and 38 sheep showed signs of depression, with counterclockwise circling movements and altered head carriage. Four sheep and one calf were necropsied, and C. cerebralis cysts were detected in all of them. A hypodense cyst was monitored in the right cerebellar hemisphere on a CT scan on one sheep. A cyst was found in the left frontal lobe on a CT scan on one calf. Microscopically, C. cerebralis cysts were surrounded by a fibrous or epithelial wall that presented necrosis on cerebral sections of both the sheep and the cattle. The CO1-PCR assay yielded a 446 bp band, which was sequenced and phylogenetically analyzed: the results confirmed the presence of T. multiceps. This study reports the first use of CT imaging on naturally infected calves and sheep for diagnosing coenurosis.(AU)
Os objetivos deste estudo foram diagnosticar cenurose por tomografia computadorizada (CT) por imagem de digitalização e caracterização molecular do gene CO1, usando a Reação em Cadeia da Polimerase (PCR). Ovelhas e bezerros foram necropsiados, e uma tomografia computadorizada da região cefálica foi realizada nos animais. Em seguida, cortes microscópicos de cérebro infectado com parasitas foram corados com hematoxilina e eosina e posterior avaliação ao microscópio de luz. Em seguida, o material recolhido de cada cisto cerebral foi fixado em etanol a 70%. A amplificação pela PCR foi realizada utilizando-se o gene mitocondrial CO1. Um total de 60 bezerros e 80 ovelhas foram clinicamente examinados e, desses, 15 bezerros e 38 ovelhas apresentaram sinais de depressão, com movimentos circulares em sentido anti-horário, e desvio da cabeça. Quatro carneiros e uma vitela foram necropsiados, e cistos de C. cerebralis foram detectados nos animais. Um cisto hipodenso foi monitorado no hemisfério cerebelar direito por imagem do CT de um carneiro. O cisto foi encontrado no lobo frontal esquerdo por imagem do CT de um bezerro. Microscopicamente, cistos de C. cerebralis foram envolvidos por uma parede fibrosa ou epitelial, apresentando necrose em ambos os cortes cerebrais de ovinos e de bovinos. O ensaio CO1-PCR produziu uma banda de 446 pb, sequenciado e submetido à filogenia, confirmou ser T. multiceps. Here, a total number of 60 sheep obtained from a private farm in Suez province, Egypt, were examined postmortem to detect visible parasitic cysts, and microscopically to detect small-sized entities. Necropsy revealed bladder-like cysts measuring 0.5-6.5 cm in diameter that were filled with a translucent fluid containing a large number of protoscolices. Accordingly, the cysts were identified as the metacestode Coenurus cerebralis. Among the sheep examined, 11 animals (7 males and 4 females) (18.3%) were infected. Most of the cysts were located in the cerebral hemispheres, with numbers ranging from one to three per infected animal. The effect of the presence of cysts in the brain tissue was evaluated. Histopathologically, pseudocysts of the apicomplexan Toxoplasma gondii were found in two animals with no detectable inflammatory cell reactions. In conclusion, coenurosis and toxoplasmosis are serious parasitic problems that play a significant role in sheep management in Egypt, as a result of close contact between livestock and dogs and cats, which play a critical role in the life cycle of these parasites.

Neste trabalho são descritos casos de doença neurológica causada por Coenurus cerebralis em 16 ovinos provenientes de nove propriedades rurais do Rio Grande do Sul. Os casos ocorreram entre janeiro de 1990 e dezembro de 2006. A evolução clínica variou de 30-90 dias e os ovinos afetados apresentavam depressão (9/16), isolamento do rebanho (8/16), andar cambaleante (7/16), cegueira (4/16), desvio da cabeça (3/16), incoordenação (3/16), movimentos de pedalagem (2/16), quedas (2/16), andar em círculos, deficiência proprioceptiva nos membros torácicos e pélvicos, estrabismo, midríase, opistótono, tremores e rigidez dos membros (1/16 cada). Os achados macroscópicos foram restritos ao sistema nervoso central e caracterizados por cistos de 2 a 9cm de diâmetro preenchidos por líquido translúcido e revestidos por fina membrana. Na parte interna da membrana, havia numerosas estruturas levemente alongadas e brancas de aproximadamente 1mm de diâmetro (escólices). Na maioria dos ovinos, os cistos eram localizados no telencéfalo (12/16); em três, os cistos estavam no cerebelo e em um ovino havia um cisto no cerebelo e um na medula espinhal. Em todos os casos havia acentuada compressão e deslocamento do tecido nervoso adjacente aos cistos. Histologicamente, os cistos parasitários eram formados por dupla membrana fracamente eosinofílica da qual evaginavam múltiplos escólices esféricos acelomados. Adjacente à parede da vesícula eram observadas sucessivas camadas compostas por uma zona de necrose e mineralização, circundadas por infiltrado inflamatório composto por macrófagos epitelióides e células gigantes multinucleadas, com uma cápsula externa de tecido fibrovascular e infiltrado inflamatório linfoistioplasmocitário perivascular. Adicionalmente, havia compressão e atrofia das substâncias branca e cinzenta adjacentes. O diagnóstico de cenurose foi realizado com base nos achados epidemiológicos, clínicos e macroscópicos, e confirmado pela histopatologia.(AU)
Sixteen cases of neurologic disease caused by Coenurus cerebralis occurring in sheep from nine farms in Rio Grande do Sul, Brazil, from January 1990 to December 2006, are described. Clinical courses varied from 30 to 90 days and affected sheep developed depression (9/16), isolation from the herd (8/16), staggering (7/16), blindness (4/16), head tilt (3/16), incoordination (3/16), paddling (2/16), falls (2/16), circling, proprioceptive deficits in fore and hindlimbs, strabismus, mydriasis, opisthotonus, trembling and rigidity of the limbs (1/16 each one). Macroscopic findings were restricted to central nervous system and consisted of 2 to 9cm fluid filled cysts with numerous slightly elongated 1 mm white scolices attached to the inner wall surface of the capsule. Cysts were located in the telencephalon (12/16); in the cerebellum (3 sheep); and in the cerebellum and spinal cord (1 sheep). All cases showed severe compression and displacement of the adjacent nervous tissue. Histologically, the cyst walls were characterized by a thin walled eosinophilic vesicle in which evaginated multiple spherical acelomated scolices. The cyst wall was surrounded by successive layers consisting of an internal necrotic and mineralized band, a layer of epithelioid macrophages with abundant number of multinucleated giant cell and an external fibrovascular capsule with perivascular lymphohistioplasmacytic infiltrate. Adjacent white and gray matter were compressed and atrophied. The diagnosis was based on epidemiologic, clinical and pathologic findings and confirmed by histopathology.(AU)

Os achados macroscópicos foram restritos ao sistema nervoso central e caracterizados por cistos de 2 a 9cm de diâmetro preenchidos por líquido translúcido e revestidos por fina membrana. Na parte interna da membrana, havia numerosas estruturas levemente alongadas e brancas de aproximadamente 1mm de diâmetro (escólices). Na maioria dos ovinos, os cistos eram localizados no telencéfalo (12/16); em três, os cistos estavam no cerebelo e em um ovino havia um cisto no cerebelo e um na medula espinhal. Em todos os casos havia acentuada compressão e deslocamento do tecido nervoso adjacente aos cistos. Histologicamente, os cistos parasitários eram formados por dupla membrana fracamente eosinofílica da qual evaginavam múltiplos escólices esféricos acelomados. Adjacente à parede da vesícula eram observadas sucessivas camadas compostas por uma zona de necrose e mineralização, circundadas por infiltrado inflamatório composto por macrófagos epitelióides e células gigantes multinucleadas, com uma cápsula externa de tecido fibrovascular e infiltrado inflamatório linfoistioplasmocitário perivascular. Adicionalmente, havia compressão e atrofia das substâncias branca e cinzenta adjacentes. O diagnóstico de cenurose foi realizado com base nos achados epidemiológicos, clínicos e macroscópicos, e confirmado pela histopatologia.
Sixteen cases of neurologic disease caused by Coenurus cerebralis occurring in sheep from nine farms in Rio Grande do Sul, Brazil, from January 1990 to December 2006, are described. Clinical courses varied from 30 to 90 days and affected sheep developed depression (9/16), isolation from the herd (8/16), staggering (7/16), blindness (4/16), head tilt (3/16), incoordination (3/16), paddling (2/16), falls (2/16), circling, proprioceptive deficits in fore and hindlimbs, strabismus, mydriasis, opisthotonus, trembling and rigidity of the limbs (1/16 each one). Macroscopic findings were restricted to central nervous system and consisted of 2 to 9cm fluid filled cysts with numerous slightly elongated 1 mm white scolices attached to the inner wall surface of the capsule. Cysts were located in the telencephalon (12/16); in the cerebellum (3 sheep); and in the cerebellum and spinal cord (1 sheep). All cases showed severe compression and displacement of the adjacent nervous tissue. Histologically, the cyst walls were characterized by a thin walled eosinophilic vesicle in which evaginated multiple spherical acelomated scolices. The cyst wall was surrounded by successive layers consisting of an internal necrotic and mineralized band, a layer of epithelioid macrophages with abundant number of multinucleated giant cell and an external fibrovascular capsule with perivascular lymphohistioplasmacytic infiltrate. Adjacent white and gray matter were compressed and atrophied. The diagnosis was based on epidemiologic, clinical and pathologic findings and confirmed by histopathology.
