ABSTRACT

OBJECTIVES: We assessed the prevalence of anti-hepatitis C virus (HCV) antibodies and active HCV infection (HCV-RNA-positive) in people living with HIV (PLWH) in Spain in 2019 and compared the results with those of four similar studies performed during 2015-2018. METHODS: The study was performed in 41 centres. Sample size was estimated for an accuracy of 1%. Patients were selected by random sampling with proportional allocation. RESULTS: The reference population comprised 41 973 PLWH, and the sample size was 1325. HCV serostatus was known in 1316 PLWH (99.3%), of whom 376 (28.6%) were HCV antibody (Ab)-positive (78.7% were prior injection drug users); 29 were HCV-RNA-positive (2.2%). Of the 29 HCV-RNA-positive PLWH, infection was chronic in 24, it was acute/recent in one, and it was of unknown duration in four. Cirrhosis was present in 71 (5.4%) PLWH overall, three (10.3%) HCV-RNA-positive patients and 68 (23.4%) of those who cleared HCV after anti-HCV therapy (p = 0.04). The prevalence of anti-HCV antibodies decreased steadily from 37.7% in 2015 to 28.6% in 2019 (p < 0.001); the prevalence of active HCV infection decreased from 22.1% in 2015 to 2.2% in 2019 (p < 0.001). Uptake of anti-HCV treatment increased from 53.9% in 2015 to 95.0% in 2019 (p < 0.001). CONCLUSIONS: In Spain, the prevalence of active HCV infection among PLWH at the end of 2019 was 2.2%, i.e. 90.0% lower than in 2015. Increased exposure to DAAs was probably the main reason for this sharp reduction. Despite the high coverage of treatment with direct-acting antiviral agents, HCV-related cirrhosis remains significant in this population.

ABSTRACT

ABSTRACT OBJECTIVE Analyze, systematize, and compile social, individual, and programmatic vulnerability factors associated with tuberculosis and HIV in homeless persons. METHODS This is a systematic literature review assessing quantitative studies, published between 2014 and 2020, on the prevalence of tuberculosis in homeless persons. Our review grouped studies according to vulnerabilities, followed the PRISMA recommendation guide, and used the Joanna Briggs Institute Critical Appraisal tool for bias analysis. RESULTS Of the 372 publications found, 16 were selected according to our eligibility criteria. In total, 10 studies assessed tuberculosis and HIV. The most commonly described factors for individual, social, and programmatic vulnerability were drug use, HIV coinfection, and tuberculosis treatment failure, respectively. The literature also claims that average homelessness length related to a higher frequency of tuberculosis and latent tuberculosis infection. CONCLUSION All reviewed studies described how homeless persons suffer with stigma and dehumanization, which are important barriers to their access to health services. Homelessness enhances the risks of chronic and infectious diseases and prioritizes issues which are more pragmatic for the maintenance of life, such as safety and food, to the detriment of health. The results can be used to support hypotheses for future research and to reinforce and direct existing public health and social policies to cope with tuberculosis and HIV in homeless persons.


RESUMO OBJETIVO Analisar, sistematizar e compilar os fatores de vulnerabilidade (social, individual e programática) associados à tuberculose e HIV em pessoas em situação de rua. MÉTODOS Revisão sistemática de literatura de publicações quantitativas sobre tuberculose em pessoas em situação de rua entre os anos de 2014 e 2020, seguindo o guia de recomendações PRISMA e, para análise de viés, a ferramenta Joanna Briggs Institute Critical Appraisal. Agrupou-se as publicações segundo as vulnerabilidades. RESULTADOS Entre 372 publicações encontradas, selecionaram-se 16 segundo os critérios de elegibilidade. Em 10 estudos, foi descrita ocorrência de tuberculose e HIV. Os fatores de vulnerabilidade individual, social e programática mais descritas foram uso de drogas, coinfecção com HIV e falha no tratamento da tuberculose, respectivamente. A média de tempo em situação de rua também se mostrou relacionada à maior frequência de tuberculose e da infecção latente da tuberculose segundo literatura. CONCLUSÃO O estigma e a desumanização associados às pessoas em situação de rua foram descritos em todos os estudos revisados, sendo importantes barreiras no acesso aos serviços de saúde. A vivência na rua potencializa os riscos para a ocorrência de doenças crônicas e infecciosas, bem como a priorização de questões mais pragmáticas à manutenção da vida, como segurança e alimentação, em detrimento à saúde. Os resultados encontrados podem ser utilizados para embasar hipóteses para futuras pesquisas e para reforçar e direcionar políticas públicas de saúde e sociais já existentes para o enfrentamento da tuberculose e HIV na pessoa em situação de rua.

ABSTRACT OBJECTIVE To evaluate the prevalence and factors associated with HIV/syphilis co-infection in people initiating antiretroviral therapy in Belo Horizonte, capital of the state of Minas Gerais. METHODS A sectional section of a prospective cohort study was carried out with people living with HIV, treatment-naive, initiating antiretroviral therapy, older than 16 years, and in follow-up treatment at specialized HIV/Aids care services in Belo Horizonte. Sociodemographic, behavioral, clinical, laboratory and pharmacological treatment-related data were obtained through interviews, medical records, and information systems for logistical control of antiretroviral medications and laboratory tests. The dependent variable was the first episode of active syphilis, recorded by the physician in clinical records, within 12 months after beginning of the antiretroviral therapy. Factors associated with HIV/syphilis co-infection were assessed using binary multiple logistic regression. RESULTS Among the 459 individuals included, a prevalence of 19.5% (n = 90) of sexually transmitted infections (STI) was observed, with syphilis (n = 49) being the most frequent STI in these individuals. The prevalence of HIV/syphilis co-infection was 10.6% (n = 49), and the associated independent factors were alcohol use (OR = 2.30; 95%CI: 1.01-5.26), and having a diagnosis of other sexually transmitted infections (OR = 3.33; 95%CI: 1.24-8.95). CONCLUSIONS There was a high prevalence of HIV/syphilis co-infection in people living with HIV initiating antiretroviral therapy in Belo Horizonte. HIV/syphilis co-infection was associated with behavioral and clinical factors, such as alcohol use and diagnosis of other sexually transmitted infections. Prior knowledge about the factors associated with this co-infection may support the decisions of health professionals engaged in the care to people living with HIV, with regard to timely diagnosis, guidance, follow-up and adequate treatment, both for syphilis and HIV.


RESUMO OBJETIVO Avaliar a prevalência e os fatores associados à coinfecção HIV/sífilis em pessoas no início da terapia antirretroviral no município de Belo Horizonte, Minas Gerais. MÉTODOS Foi realizado um corte seccional de um estudo de coorte prospectivo, com pessoas vivendo com HIV, sem tratamento prévio da infecção, em início da terapia antirretroviral, maiores de 16 anos e em acompanhamento em serviços de assistência especializada em HIV/aids de Belo Horizonte. Dados sociodemográficos, comportamentais, clínicos, laboratoriais e relacionados ao tratamento farmacológico foram obtidos por meio de entrevistas, coleta em prontuários clínicos e nos sistemas de informação de controle de medicamentos antirretrovirais e exames laboratoriais. A variável dependente foi o primeiro episódio de sífilis ativa, registrado pelo médico em prontuário clínico, em um período de 12 meses após início da terapia antirretroviral. Os fatores associados à coinfecção HIV/sífilis foram avaliados por meio de regressão logística binária múltipla. RESULTADOS Dentre os 459 indivíduos avaliados, observou-se uma prevalência de 19,5% (n = 90) de infecções sexualmente transmissíveis, sendo a sífilis (n = 49) a infecção sexualmente transmissível mais frequente nesses indivíduos. A prevalência da coinfecção HIV/sífilis foi de 10,6% (n = 49) e os fatores independentes associados foram o uso de álcool (OR = 2,30; IC95% 1,01-5,26) e ter diagnóstico de outras infecções sexualmente transmissíveis (OR = 3,33; IC95% 1,24-8,95). CONCLUSÕES Houve alta prevalência de coinfecção HIV/sífilis em pessoas vivendo com HIV em início de terapia antirretroviral em Belo Horizonte. A coinfecção HIV/sífilis foi associada a fatores comportamentais e clínicos, como uso de álcool e diagnóstico de outras infecções sexualmente transmissíveis. O conhecimento prévio sobre os fatores associados à essa coinfecção pode subsidiar as decisões dos profissionais de saúde inseridos no cuidado às pessoas vivendo com HIV, no que diz respeito ao diagnóstico oportuno, orientações, acompanhamento e tratamento adequado, tanto da sífilis quanto do HIV.

BACKGROUND: HIV-positive individuals may acquire HCV via injection drug use (IDU) and condomless anal sex. HIV care provides opportunities for HCV testing and cure with direct-acting antiviral agents (DAAs). METHODS: We analyzed data from the Ontario HIV Treatment Network Cohort Study. Among those not HCV-positive or diagnosed previously (n = 4586), we used Cox regression to test the rates of ever HCV testing (serological or RNA) in HIV care by DAA era (pre-DAA: 2000-2010; after DAA: 2011-2015) and compared the proportion diagnosed with HCV. We identified correlates of annual proportions of serological testing using Poisson generalized estimating equations. RESULTS: After DAA vs pre-DAA, the hazard rate ratio (95% CI) of ever HCV testing was 1.70 (1.59, 1.81). The proportion (95% CI) tested annually increased from 9.2% (8.0%, 10.7%) in 2000 to 39.1% (37.1%, 41.1%) in 2015 (P < 0.0001). The proportion diagnosed with HCV declined by 74% pre-DAA to 11% after DAAs. Annual testing increased per calendar year (16% steeper slope after DAA vs pre-DAA) and was more common among men who have sex with men; those more educated (post-secondary vs ≤ high school); and those positive for syphilis or reporting any IDU. Annual testing decreased per decade of age and time since HIV diagnosis. DISCUSSION: Annual HCV testing increased over time with higher testing among those reporting sexual or IDU risk factors, but fell short of clinical guidelines. Targeted interventions to boost testing may be needed to close these gaps and reach WHO 2030 HCV elimination targets.

Background: Hospital-acquired infections are associated with increased morbidity, mortality, costs and length of stay. Prior studies have linked increased hand hygiene compliance with reduced hospital-acquired infection rate. With the increased vigilance for personal and institutional hygiene practices during the COVID-19 pandemic, we hypothesized increased hand hygiene compliance and, as a result, decrease in hospital-acquired infections in our hospital. Methods: Hand hygiene compliance data was provided by the hospital's Quality Department. We queried and empirically analyzed local hospital-acquired infection data obtained from our Quality Department and the National Healthcare Safety Network. We compared local hand hygiene compliance rates before and after the implementation of increased infection prevention and control measures in March 2020 and correlated various hospital-acquired infection rates with hand hygiene compliance. Results: Our results showed a statistically significant and sustained increase in compliance with hand hygiene at our hospital after implementation of hospital-wide infection control measures. We demonstrated a downward trend in all hospital-acquired infections, which was not statistically significant. A strong statistically significant negative correlation was found between hand hygiene compliance and the rate of Clostridium difficile hospital-acquired infection. Conclusion: Hand hygiene adherence has increased since the beginning of COVID-19 pandemic in our hospital, and a noticeable, although not statistically significant, downward trend in most of the analyzed hospital-acquired infections was identified. A significant limitation to our study was small sample size. Future studies are warranted to further analyze the impact of increased hygiene practice on the incidence of hospital-acquired infections.

Introducción: La malaria continúa siendo un importante problema de salud pública en todo el mundo. Las coinfecciones son un factor de riesgo que incrementa la mortalidad de esta enfermedad. En Venezuela no existen estudios que describan la presencia de coinfecciones en pacientes con malaria. Nosotros determinamos las características clínicas y epidemiológicas de los pacientes con malaria y la presencia de coinfecciones en Ciudad Bolívar, Estado Bolívar, Venezuela. Metodología: Se realizó un estudio descriptivo, correlacional y transversal, que incluyó pacientes diagnosticados con malaria por prueba rápida y/o gota gruesa y extendido de sangre periférica que consultaron en tres centros médicos de Ciudad Bolívar, estado Bolívar, entre junio y noviembre de 2018. Se realizó una evaluación clínica y de laboratorio de cada paciente, las coinfecciones con Dengue (VD), Hepatitis viral (HV) (A, B y C), Leptospirosis (LP), y Chikungunya (VCHIK) fueron evaluadas mediante la técnica de ELISA. Resultados: Un total de 161 pacientes fueron estudiados, 106 (65,8 %) presentaron infección por P. vivax, 43 (26,7 %) por P. falciparum y 12 (7,4 %) tenían malaria mixta (Pf/Pv). La media de edad fue 33,8 (±13,43) años; 103 (63,9 %) fueron hombres, la raza más frecuente fue mestiza (94,4 %); la mayoría de los pacientes (37,3 %) practicaban la minería ilegal. Los síntomas más frecuentes fueron fiebre, escalofríos y cefalea. Anemia leve, trombocitopenia moderada, y compromiso de la función hepática fueron los hallazgos de laboratorio más relevantes en todas las especies parasitarias. Se encontró coinfección en 55/161 (34,2 %) pacientes, siendo más frecuente entre los pacientes con P. falciparum (48,8 %). La coinfección más frecuente fue con VD (14,9 %), seguida de VHA (11,8 %)VHB (6,2 %), VCHIK (5,5 %) y LP (3,7 %). En el grupo de coinfectados fue más frecuente la malaria complicada (56,36 %) que la no complicada (43,63 %) con una diferencia estadísticamente significativa (P=0,018). Conclusión: Se encontró una alta prevalencia de coinfecciones en los pacientes con malaria, y su asociación con la severidad de la Malaria, estos datos epidemiológicos influyen de manera directa en el curso clínico, así como en la mortalidad de la enfermedad. Estos hallazgos deben darse a conocer al personal de salud para la identificación oportuna de coinfecciones en estos pacientes.


Introduction: Malaria continues to be a major public health problem worldwide. Co-infections are a risk factor that increases the mortality of this disease. In Venezuela there are no studies describing the presence of coinfections in patients with malaria. We determine the clinical and epidemiological characteristics of patients with malaria and the presence of coinfections in Ciudad Bolívar, Bolívar state, Venezuela. Methodology: A descriptive, correlational and cross-sectional study was carried out, which included patients diagnosed with malaria by rapid test and / or thick and extended peripheral blood drop that they consulted in three medical centers in Ciudad Bolívar, Bolívar state, between June and November 2018 A clinical and laboratory evaluation of each patient was performed, coinfections with Dengue (DV), viral hepatitis (HV) (A, B and C), Leptospirosis (LP), and Chikungunya (VCHIK) were evaluated using the technique of ELISA Results: A total of 161 patients were studied, 106 (65.8 %) had P. vivax infection, 43 (26.7 %) due to P. falciparum and 12 (7.4 %) had mixed malaria (Pf / Pv ). The mean age was 33.8 (± 13.43) years; 103 (63.9 %) were men, the most frequent race was mestizo (94.4 %); the majority of patients (37.3 %) practiced illegal mining. The most frequent symptoms were fever, chills and headache. Mild anemia, moderate thrombocytopenia, and hepatic function impairment were the most relevant laboratory findings in all parasitic species. Coinfection was found in 55/161 (34.2 %) patients, being more frequent among patients with P. falciparum (48.8 %). The most frequent coinfection was with RV (14.9 %), followed by HAV (11.8 %) HBV (6.2 %), HCV (5.5 %) and LP (3.7 %). Complicated malaria (56.36 %) was more frequent than uncomplicated (43.63 %) with a statistically significant difference (P = 0.018). Conclusion: A high prevalence of coinfections was found in patients with malaria, and its association with the severity of Malaria, these epidemiological data directly influence the clinical course, as well as the mortality of the disease. These findings should be made known to health personnel for the timely identification of coinfections in these patients.

BACKGROUND: We assessed the prevalence of antibodies against hepatitis C virus (HCV-Abs) and active HCV infection in patients infected with human immunodeficiency virus (HIV) in Spain in 2016 and compared the results with those of similar studies performed in 2002, 2009, and 2015. METHODS: The study was performed in 43 centers during October-November 2016. The sample was estimated for an accuracy of 2% and selected by proportional allocation and simple random sampling. During 2016, criteria for therapy based on direct-acting antiviral agents (DAA) were at least significant liver fibrosis, severe extrahepatic manifestations of HCV, and high risk of HCV transmissibility. RESULTS: The reference population and the sample size were 38904 and 1588 patients, respectively. The prevalence of HCV-Abs in 2002, 2009, 2015, and 2016 was 60.8%, 50.2%, 37.7%, and 34.6%, respectively (P trend <.001, from 2002 to 2015). The prevalence of active HCV in 2002, 2009, 2015, and 2016 was 54.0%, 34.0%, 22.1%, and 11.7%, respectively (P trend <.001). The anti-HCV treatment uptake in 2002, 2009, 2015, and 2016 was 23.0%, 48.0%, 59.3%, and 74.7%, respectively (P trend <.001). In 2016, HCV-related cirrhosis was present in 7.6% of all HIV-infected individuals, 15.0% of patients with active HCV, and 31.5% of patients who cleared HCV after anti-HCV therapy. CONCLUSIONS: Our findings suggest that with universal access to DAA-based therapy and continued efforts in prevention and screening, it will be possible to eliminate active HCV among HIV-infected individuals in Spain in the short term. However, the burden of HCV-related cirrhosis will continue to be significant among HIV-infected individuals.

Background. The purpose of this study was to assess the prevalence of anti-hepatitis C virus (HCV) antibodies (Abs) and active HCV infection in human immunodeficiency virus (HIV)-infected (HIV+) patients in Spain in 2015. This was a cross-sectional study. Methods. The study was performed in 41 centers in 2015. Sample size was estimated for an accuracy of 2%, the number of patients from each hospital was determined by proportional allocation, and patients were selected using simple random sampling. Results. The reference population was 35 791 patients, and the sample size was 1867 patients. Hepatitis C virus serostatus was known in 1843 patients (98.7%). Hepatitis C virus-Abs were detected in 695 patients (37.7%), in whom the main route of HIV acquisition was injection drug use (75.4%). Of these 695 patients, 402 had HCV RNA, 170 had had a sustained viral response (SVR) after anti-HCV therapy, and 102 cleared HCV spontaneously. Hepatitis C virus-ribonucleic acid results were unknown in 21 cases. Genotype distribution (known in 367 patients) was 1a in 143 patients (39.0%), 4 in 90 (24.5%) patients, 1b in 69 (18.8%) patients, 3 in 57 (15.5%) patients, 2 in 5 (1.4%) patients, and mixed in 3 (0.8%) patients. Liver cirrhosis was present in 93 patients (23.1%) with active HCV infection and in 39 (22.9%) patients with SVR after anti-HCV therapy. Conclusions. The prevalence of HCV-Abs and active HCV infection in HIV+ patients in Spain is 37.7% and 22.1%, respectively; these figures are significantly lower than those recorded in 2002 and 2009. The predominant genotypes in patients with active HCV infection were 1a and 4. A high percentage of patients had cirrhosis. Cirrhosis is also common in patients with SVR after anti-HCV therapy.
