ABSTRACT

El nuevo coronavirus denominado SARS-COV2 causa diversas manifestaciones clínicas englobadas bajo el término COVID-19. El sistema de informe y datos conocidos por sus siglas en inglés como RADS, determinadas a partir del consenso de expertos producido por un grupo de trabajo multidisciplinario con el objetivo de mejorar la comunicación de los resultados, establece un enfoque estándar para la notificación de manifestaciones imagenológicas. El presente trabajo presenta el prototipo de aplicación CO-RADS que propone una herramienta digital sobre la base de una revisión actualizada del papel y la idoneidad de los estudios de imagenología para el diagnóstico y seguimiento de pacientes con sospecha o infección conocida de COVID-19, proporcionando terminología estandarizada de imágenes para lograr comunicar los resultados al médico de asistencia de manera clara y consistente. The new coronavirus called SARS-COV2 causes various clinical manifestations encompassed under the term COVID-19. The data and reporting system known by its acronym in English as RADS, establishes a standard approach for the notification of imaging manifestations with the aim of improving the communication of results; was determined from the consensus of experts produced by a multidisciplinary working group. This paper presents the CO-RADS application prototype that proposes a digital tool based on an updated review of the role and suitability of imaging studies for the diagnosis and follow-up of patients with suspected or known COVID-19 infection, providing standardized imaging terminology to achieve clear and consistent communication of results to the attending physician. ABSTRACT

Introducción: El método clínico por sí solo no es definitivo para diagnosticar una enfermedad cerebrovascular, por lo que es importante el uso de medios de diagnóstico imagenológicos como la tomografía axial computarizada, que permite investigar los aspectos morfológicos y funcionales del cerebro. Objetivo: Describir las características clínicas y tomográficas de pacientes con infarto cerebral. Métodos: Se efectuó un estudio descriptivo y transversal de 269 pacientes con infarto cerebral, atendidos en el Hospital Provincial Docente Clinicoquirúrgico Saturnino Lora Torres de Santiago de Cuba, desde enero hasta diciembre de 2019. Resultados: Predominaron el sexo masculino (54,6 %), el grupo etario de 70-79 años, las afectaciones motoras (76,5 %), la alteración de la conciencia (51,3 %) y el trastorno del lenguaje (50,5 %). En 38 pacientes los resultados de la tomografía fueron negativos antes de las 24 horas de evolución y 64,0 % se consideraron mejorados con secuelas. Conclusiones: La tomografía antes de las 24 horas demuestra una baja positividad y los hallazgos evolucionan con el tiempo, aunque es posible reconocer varios signos precoces de ictus agudo en menos de 4-6 horas.


Introduction: The clinical method by itself is not definitive to diagnose a cerebrovascular disease, reason why the use of imaging diagnostic means, as the computerized axial tomography is important, as it allows to investigate the morphological and functional aspects of the brain. Objective: To describe the clinical and tomographic characteristics of patients with complete stroke. Methods: A descriptive and cross-sectional study of 269 patients with complete stroke, assisted in Saturnino Lora Torres Teaching Clinical Surgical Provincial Hospital was carried out in Santiago de Cuba, from January to December, 2019. Results: There was a prevalence of the male sex (54.6 %), 70-79 age group, motor disorders (76.5 %), alteration of the conscience (51.3 %) and language disorder (50.5 %). In 38 patients the results of the tomography were negative before the 24 hours of clinical course and 64.0 % were better with aftereffects of the disease. Conclusions: Tomography before the 24 hours demonstrates a low positivity and findings have a favorable clinical course as time goes by, although it is possible to recognize several early signs of acute ictus in less than 4-6 hours.

ABSTRACT

An 82-year-old male was transferred for an abdominal CT scan for chronic cutaneous fistulation at the level of the right abdominal wall. Previous CT and ultrasound imaging described recurrent collections in the right abdominal wall, requiring CT guided abscess drainage. The abdominal CT scan revealed an abscess in between the internal oblique and transversus abdominis muscle layers of the right flank, with significant fat stranding and loss of the intermuscular fat planes ( Figure 1 ). Inside this abscess, we notice a spontaneous hyperdense nodular lesion (Hounsfield Units 130), which doesn't enhance after contrast injection ( Figure 1 arrow). Looking back at the previous CT scans we discern the presence of this hyperdense lesion, which tends to migrate over time over a small distance along the abdominal wall ( Figure 2 A-D arrow). We can trace this back on the numerous previous scans, with different local tissue reactions over time. The first performed CT 8 years prior reveals a perforated calculous cholecystitis, containing multiple cholecystolithiases with the same density as our previously mentioned hyperdense lesion ( Figure 3 arrow). Thus, raising the suspicion of a biliary origin of this corpus alienum. TEACHING POINT: Spilled gallstones during laparoscopy may lead to late abscess.

ABSTRACT

El tratamiento con radioterapia, consiste en irradiar de forma homogénea el tumor, evitando irradiar los órganos cercanos. El desarrollo de la tecnología en el campo de la medicina, ha permitido que se cumpla este principio, con buenos resultados de respuesta objetiva, que se traduce en el control de la enfermedad de los pacientes con cáncer. La Tomografía axial computarizada por emisión de positrones (PET-CT) consigue una mayor precisión en el delineado del volumen blanco o tumoral. El propósito de este estudio fue analizar la influencia del uso del PET-CT en el delineado de volúmenes a tratar en la planificación del tratamiento con radiaciones.Se diseñó un estudio piloto con dos pacientes preparados para radioterapia por neoplasias malignas, a los cuáles se les realizó un TAC simple y un PET-CT y observadores independientes realizaron la delimitación del tumor. Se examinó la consistencia entre los observadores y las mediciones en las imágenes.El uso del PET-CT favoreció la delimitación del volumen a irradiar lo que disminuye el riesgo para los órganos vecinos.


The treatment with radiotherapy consists on irradiating in a homogeneous way the tumor, avoiding to irradiate the near organs. The development of the technology in the field of the medicine, it has allowed that this principle is completed, with good results of objective answer what allows the control of the illness of the patients with cancer. The computerized axial tomography by positrons emission (PET-CT) it gets a bigger precision in the one delineated of the white volume or tumoral. The purpose of this study was to analyze the influence of the use of the PET-CT in the one delineated of volumes to try in the planning of the treatment with radiations.A study pilot was designed with two prepared patients for radiotherapy for malignant diseases, to those which they were carried out a simple TAC and a PET-CT and independent observers carried out the delimitation of the tumor. The consistency was examined between the observers and the mensuration in the images.The use of the PET-CT favors the delimitation of the volume to irradiate what diminishes the risk for the neighboring organs.

ABSTRACT

La inestabilidad patelofemoral es una entidad que afecta principalmente a adolescentes y adultos jóvenes. En su diagnóstico se consideran elementos clínicos e imagenológicos, en especial para medir la distancia entre la tuberosidad anterior de la tibia y el surco intercondíleo, que permite la selección de la técnica quirúrgica en cada paciente, en específico la transferencia de la tuberosidad anterior de la tibia. En este artículo se exponen brevemente algunos aspectos de interés sobre el tema: métodos imagenológicos empleados en estos pacientes (radiografía simple, tomografía axial computarizada, imagen por resonancia magnética) y valores de referencia considerados como normales; también se describe por pasos cómo medir la distancia entre la tuberosidad anterior de la tibia y el surco intercondíleo.


The patellofemoral instability is an entity that mainly affects adolescents and young adults. In its diagnosis clinical and imaging elements are considered, especially to measure the tibial-tuberosity to trochlear groove distance that allows the selection of the surgical technique in each patient, in specific the transfer of the tibial-tuberosity. In this work some aspects of interest on the topic are shortly exposed: the imaging methods used in these patients (simple x-rays, computerized axial tomography, magnetic resonance imaging) and the reference values considered as normal; it is also described step by step how to measure the tibial-tuberosity to trochlear groove distance.

ABSTRACT

BACKGROUND: Displaced odontoid fractures that are irreducible with traction and have cervicomedullary compression by the displaced distal fracture fragment or deformity caused by facetal malalignment require early realignment and stabilization. Realignment with ultimate solid fracture fusion and atlantoaxial joint fusion, in some situations, are the aims of surgery. Fifteen such patients were treated with direct anterior extrapharyngeal open reduction and realignment of displaced fracture fragments with realignment of the atlantoaxial facets, followed by a variable screw placement (VSP) plate in compression mode across the fracture or anterior atlantoaxial fixation (transarticular screws or atlantoaxial plate screw construct) or both. OBSERVATIONS: Anatomical realignment with rigid fixation was achieved in all patients. Fracture fusion without implant failure was observed in 100% of the patients at 6 months, with 1 unrelated mortality. Minimum follow-up has been 6 months in 14 patients and a maximum of 3 years in 4 patients, with 1 unrelated mortality. LESSONS: Most irreducible unstable odontoid fractures can be anatomically realigned by anterior extrapharyngeal approach by facet joint manipulation. Plate (VSP) and screws permit rigid fixation in compression mode with 100% fusion. Any associated atlantoaxial instability can be treated from the same exposure.

RESUMEN Fundamento: Los estudios imagenológicos son importantes en la evaluación del paciente con la Covid-19, las publicaciones sobre la aplicación de estos crecen y evolucionan cada día, lo cual trae consigo la inabarcable búsqueda de información verosímil. Objetivo: Analizar información actual disponible de hallazgos imagenológicos de neumonía por SARS-CoV-2, así como el papel y la idoneidad de los estudios de imagen para el mejor diagnóstico y seguimiento de pacientes en diversos contextos clínicos. Metodología: Se hizo una recopilación y revisión de la bibliografía nacional e internacional en un período de febrero a agosto de 2020. La estrategia de búsqueda adoptada fue la utilización de palabras clave o descriptores en español e inglés. Se revisaron 71 artículos de diferentes bases de datos y se utilizaron 42. Desarrollo: Se describen los signos imagenológicos en pacientes sospechosos y diagnosticados con neumonía por SARS-CoV-2, así como las ventajas y desventajas de los estudios imagenológicos más utilizados. Conclusiones: Los estudios de imagen juegan un papel relevante para la identificación e intervención temprana en pacientes de todas las edades con neumonía por SARS-CoV-2. Se propone valorar la utilización de la ecografía pulmonar en el contexto nacional a partir de las evidencias científicas encontradas.


ABSTRACT Background: Imaging studies are important in the evaluation of patient with Covid-19, publications on its application increase and evolve every day, so it results in the endless search of credible information. Objective: To analyze current available information on imaging findings of pneumonia by SARS-CoV-2, as well as the role and imaging studies suitability for the best diagnosis and monitoring of patients in several clinical contexts. Methodology: A compilation and review of the national and international bibliography from February to August 2020 was made. The search strategy was the use of keywords or descriptors in Spanish and English. 71 articles from different databases were reviewed and 42 were used. Development: Imaging signs were described in suspicious patients diagnosed with pneumonia by SARS-CoV-2, also the advantages and disadvantages of the most widely used imaging studies. Conclusions: Imaging studies play a relevant role in the early identification and intervention in patients of all ages with pneumonia by SARS-CoV-2. Based on the scientific evidence, it is proposed to assess the use of pulmonary ultrasound in the national context.

BACKGROUND: Inflammation is a recognized concomitant of hepatocellular carcinoma (HCC) and its indices are prognostically useful. AIMS: To evaluate two commonly used inflammatory indices, neutrophil-to-lymphocyte ratio (NLR) and the platelet-to-lymphocyte ratio (PLR), to examine their relationship to maximum tumor diameter (MTD) and to survival. METHODS: A database of 1024 prospectively-accrued HCC patients was examined, who had full baseline tumor parameter data, including CT scan information on HCC size and whose survival was known. Analyses of NLR and PLR were correlated to MDT and to survival. NLR and PLR cutoffs were calculated from receiver operator characteristic curves. RESULTS: Every MTD pair had significantly different PLR values, for MTD groups of groups <2/≥2, <3/≥3, <4/≥4, <5/≥5 cm. However there were few significant differences in NLR values. Logistic regression models of different MTD groups likewise showed significance for PLR. Patients with both low NLR and low PLR had the longest overall survival compared to all the other 3 combinations of NLR and PLR. In a Cox regression analysis, univariate models on NLR (≤3.02/>3.02) and PLR (≤6.82/>6.82) groups, showed significance for PLR, p = 0.034 and approaching significance for NLR, p = 0.057. CONCLUSIONS: MTD pairs down to <2/≥2 cm showed significance for PLR, survival showed significance for PLR and almost for NLR.

Introducción: El cáncer de páncreas se origina en la glándula pancreática y es una de las neoplasias más invasivas debido a su rápida diseminación, la falta de síntomas específicos en sus inicios y su diagnóstico tardío. Objetivo: Caracterizar a pacientes con cáncer de páncreas según variables de interés. Métodos: Se efectuó un estudio observacional, descriptivo y transversal en el Servicio de Imagenología del Hospital Provincial Clinicoquirúrgico Docente Saturnino Lora de la provincia de Santiago de Cuba, desde enero de 2013 hasta diciembre de 2017, de 70 pacientes con diagnóstico tomográfico sugestivo de dicha neoplasia maligna. Resultados: En la serie el tumor exocrino pancreático primó en pacientes mayores de 60 años de edad (37,1 %), del sexo masculino (54,2 %) y de la raza mestiza (61,4 %). El diagnóstico histológico predominante fue el de adenocarcinoma ductal poco diferenciado (47,2 %), en tanto, los factores de riesgo mayormente asociados al proceso neoplásico fueron las comidas grasas (68,6 %), el tabaquismo (61,4 %) y el alcoholismo (50,0 %), y el antecedente patológico personal más frecuente, la diabetes mellitus (37,1 %). Conclusiones: La tomografía axial computarizada proporciona una descripción detallada de las neoplasias pancreáticas y su extensión, lo cual es de gran utilidad para la estadificación de estas y, además, determina la conducta terapéutica a seguir.


Introduction: The pancreas cancer originates in the pancreatic gland and it is one of the more invasive neoplasms due to its quick dissemination, lack of specific symptoms in its beginnings and its late diagnosis. Objective: To characterize patients with pancreas cancer according to variables of interest. Methods: An observational, descriptive and cross-sectional study was carried out in the Imaging Service of Saturnino Lora Teaching Provincial Clinical Surgical Hospital in Santiago de Cuba, from January, 2013 to December, 2017 to 70 patients with suggestive tomographic diagnosis of this malignancy. Results: In the series the pancreatic exocrine tumor prevailed in patients older than 60 years (37.1 %), the male sex (54.2 %) and mixed race (61.4 %). The predominant histologic diagnosis was the ductal adenocarcinoma hardly differentiated (47.2 %), as long as, the risk factors mostly associated with the neoplasm were fatty foods (68.6 %), nicotine addiction (61.4 %) and alcoholism (50.0 %), and the most frequent personal pathological history was diabetes mellitus (37.1 %). Conclusions: The computerized axial tomography provides a detailed description of the pancreatic neoplasms and their extension, which is very useful for the staging of these malignancies and, also, it determines the therapeutic behavior we should follow.

ABSTRACT

Testicular neoplasms have been extensively described and characterized in domestic animals, but reports in wildlife species, including marine mammals, are scarce. This case report describes a testicular seminoma in an adult striped dolphin Stenella coeruleoalba stranded along the coasts of the Canary Islands. Post-mortem computerized tomography (CT) showed a prominent enlargement of the cranial pole of the right testicle, displacing the intestinal loops. Necropsy gross findings confirmed the presence of a testicular mass, bulging at the cut surface, with multiple well-delimitated whitish nodular lesions. Histologically, intratubular and diffuse neoplastic germinative cell proliferation was described. Complementary immunohistochemical assessments for vimentin and CD117 antibodies were negative. To the authors' knowledge, this is the first seminoma described in a S. coeruleoalba. We suggest that post-mortem (PM) pre-necropsy CT in dolphins is a useful tool for anatomic-guided pathology in such cases.

ABSTRACT

La mala alineación patelofemoral tiene múltiples causas anatómicas y una de ellas es la displasia de la tróclea del fémur distal, por lo cual su tratamiento depende de varios factores. Para profundizar en los elementos esenciales de la técnica de trocleoplastia, se realizó una revisión bibliográfica exhaustiva donde se analizaron algunos aspectos de interés, a saber: clasificación más empleada para las displasias de la tróclea, elementos imagenológicos más importantes, indicaciones de la técnica quirúrgica, contraindicaciones, principales modalidades de trocleoplastia y vías de acceso quirúrgico. Se concluyó que este es un proceder con indicaciones muy específicas y puede realizarse mediante cirugía abierta o por vía artroscópica.


The poor patellofemoral alignment has multiple anatomical causes and one of them is the dysplasia of the distal femur trochlea, reason why its treatment depends on several factors. To deepen in the essential elements of the trochleoplasty technique, an exhaustive literature review was carried out where some aspects of interest were analyzed, that is: most used classification for the trochlear dysplasias, most important imagenologic elements, indications of the surgical technique, contraindications, main modalities of trochleoplasty and surgical approaches. It was concluded that this is a procedure with very specific indications and can be carried out by means of open surgery or arthroscopic way.

ABSTRACT

Pediatric Obstructive Sleep Apnea (OSA) is a condition that may lead to a variety of comorbidities in adolescence and adulthood. The gold standard of diagnosing OSA is polysomnography (PSG). Over the past fifteen years numerous publications have explored how to better visualize the upper airway to further assess OSA in the pediatric population, and eventually institute personalized treatment. Lateral neck radiograph, cephalometry, computed axial tomography, and magnetic resonance imaging are all unique imaging techniques that are used in the diagnosis of OSA. Drug Induced Sleep Endoscopy is a direct visualization technique that is gathering momentum in pediatrics. Each approach has respective benefits and weaknesses. However, none of them at this time can replace PSG. They are a helpful supplement in those patients with complicated upper airway anatomy and in those with residual OSA.

Degenerative sacroiliac joint disease is a cause of lumbosacral pain in dogs; however, published information on cross-sectional imaging characteristics is limited. Objectives of this retrospective, secondary analysis, methods-comparison study were to test hypotheses that CT lesions reported in humans with degenerative sacroiliac joint disease are also present in dogs, and that CT is comparable to gross pathology for detecting these lesions. Matched CT and gross pathology slice images of 30 sacroiliac joints were retrieved from a previous prospective, canine cadaver study. A veterinary radiologist interpreted randomized CT images for each joint based on previously published CT characteristics of lesions in humans with degenerative sacroiliac joint disease. A veterinary pathologist independently interpreted randomized gross pathology images using the same criteria. All joints had at least one CT lesion consistent with degenerative sacroiliac joint disease. A new CT lesion was also identified and termed "subarticular cleft." The CT and gross pathology methods agreed for detecting joints with subchondral sclerosis, subchondral erosion, and intra-articular ankylosis lesions (P > .05, McNemar's test), but disagreed for detection of joints with subchondral cyst, para-articular ankylosis, and subarticular cleft lesions (P ≤ .05). Using gross pathology as the reference standard, CT had 100% sensitivity for detection of subarticular cleft and subchondral cyst lesions, with 56% and 22% specificity, respectively. Para-articular ankylosis lesions were detected by CT but not by gross pathology. Findings supported the hypothesis that CT lesions reported in humans with degenerative sacroiliac joint disease are also present in dogs, and partially supported the hypothesis that CT is comparable to gross pathology for detecting joints with these lesions.

We reviewed computerized axial tomography of 28 patients with hamate hook fractures who had surgical resection of the hook. We analysed the relationship between the fragment height ratio, fragment gap, and intraoperative findings of the tendons. We determined whether parameters in the images can predict complication of tear or disruption of the flexor tendons to the ring or little fingers. Of 28 patients, 16 had fragment height ratios between 50-74; ten among them had worn (eight patients) or ruptured (two patients) flexor tendons. Nine of the ten patients had fragment gaps greater than 2 mm. The remaining 12 patients had fragment height ratios between 75-100 and had intact tendons. We conclude that a fragment height ratio greater than 75 and fragment gap less than 2 mm in computer tomography may rule out tear or disruption of the flexor tendons of the ring and little fingers after hamate hook fractures, and a fragment height ratio between 50-74 with fragment gap greater than 2 mm indicates a high risk of flexor tendon tear or disruption. ABSTRACT

Se estudió un paciente masculino de 30 años de edad, con antecedentes de salud; que sufrió una caída desde una altura. Fue atendido en el Cuerpo de Guardia del Hospital Clínico-Quirúrgico de Holguín, donde se presentó con un trauma torácico cerrado. En estudios radiológicos de urgencia, no se evidenciaron lesiones óseas, pleuroparenquimatosas ni mediastinales; sin embargo, por la evolución clínica posterior, se le realiza un interrogatorio y examen físico exhaustivo, y se indica una tomografía axial computarizada de tórax, en la que aparecen lesiones contusionales bibasales postraumáticas. Se impone tratamiento medicamentoso y, a su término, se reevalúa clínica y radiológicamente, con una evolución satisfactoria del cuadro.


A 30-year-old male patient with medical records, with a closed thoracic trauma, who was treated at the Emergency Room, after falling from a height, at Holguin´s Clinical Surgical Hospital. Radiological studies not showed pleuroparenchymal and mediastinal lesions. Due to clinical evolution, a thorough physical examination was computerized. Thorax tomography was indicated. Bibasal contusional posttraumatic lesions were detected. Medication, clinical and radiological re-evaluation led to the satisfactory evolution of patient´s clinical picture.

ABSTRACT

BACKGROUND: Colonoscopy has a reported localization error rate as high as 21% in detecting colorectal neoplasms. Preoperative repeat endoscopy has been shown to be protective against localization errors. There is a paucity of literature assessing the utility of staging computerized tomography (CT) and repeat endoscopy as diagnostic tools for detecting localization errors following initial endoscopy. The objective of this study is to determine the diagnostic characteristics of staging CT and repeat endoscopy in correcting localization errors at initial endoscopy. METHODS: A retrospective cohort study was conducted at a large tertiary academic center between January 2006 and August 2014. All patients undergoing surgical resection for CRC were identified. Group comparisons were conducted between (1) patients that underwent only staging CT (staging CT group), and (2) patients that underwent staging CT and repeat endoscopy (repeat endoscopy group). The primary outcome was localization error correction rate for errors at initial endoscopy. RESULTS: 594 patients were identified, 196 (33.0%) in the repeat endoscopy group, and 398 (77.0%) patients in the staging CT group. Error rates for each modality were as follows: initial endoscopy 8.8% (95% CI 6.5-11.0), staging CT 9.3% (95% CI 6.5-11.0), and repeat endoscopy 2.6% (95% CI 0.3-4.7); p < 0.01. Repeat endoscopy was superior to staging CT in correcting localization errors for left-sided / rectal lesions (81.2% vs. 33.3%; p < 0.01), right-sided lesions (80.0% vs. 54.5%; p = 0.21), and overall lesions (80.8% vs. 42.3%; p < 0.01). Repeat endoscopy compared to staging CT demonstrated relative risk reduction of 66.7% (95% CI 22-86%), absolute risk reduction of 38.5% (95% CI 14.2-62.8%), and odds ratio of 0.18 (95% CI 0.05-0.61) for correcting errors at initial endoscopy. CONCLUSIONS: Repeat endoscopy in colorectal cancer is superior to staging CT as a diagnostic tool for correcting localization-based errors at initial endoscopy.

BACKGROUND: It is not easy to suspect whether newly born infants diagnosed as hypoxic ischemic encephalopathy (HIE) will develop impairment of renal function, so there is an urge to scientifically research about correlations of severity of HIE, which is represented by Sarnat scoring and brain computerized axial tomography (CAT) and serum creatinine level in these newly born infants. AIM: To evaluate renal function in the form of serum creatinine levels in full-term neonates with HIE and their correlation with severity degree of HIE, which is represented by Sarnat scoring and CAT. AIM: To evaluate renal function in the form of serum creatinine levels in full-term neonates with HIE and their correlation with severity degree of HIE, which is represented by Sarnat scoring and CAT. SUBJECTS AND METHODS: This study was a case-control type. It was conducted on 72 full-term neonates who were classified into group 1, which included 36 full-term neonates who were diagnosed as HIE according to the definition of the World Health Organization and group 2, which included 36 full-term neonates who were matched for age and sex and who served as the control group. Serum creatinine levels were measured at days 1 and 7 postnatally. CAT scans were carried out for cases only. RESULTS: Serum creatinine levels were elevated in group 1 when compared to the control group at days 1 and 7 postnatally. They were significantly correlated to the Sarnat scoring system of HIE, meaning that serum creatinine levels gradually increased with the increase in severity of HIE according to Sarnat and Sarnat staging. A statistically significant difference was observed between serum creatinine levels in patients with different findings of brain CAT, meaning that more elevation in serum creatinine levels were reported with more severe cases represented by marked changes in brain CAT. CONCLUSION: Serum creatinine levels correlate with the severity of HIE of neonates according to Sarnat scoring and brain CAT.

ABSTRACT

OBJECTIVES/HYPOTHESIS: There is little evidence to support the removal of thyroid tissue during total laryngectomy. Although oncological control of the tumor is the priority, thyroidectomy can lead to hypothyroidism and hypoparathyroidism. This study aimed to test the usefulness of preoperative computed tomography in predicting histological invasion of the thyroid. STUDY DESIGN: Ambispective cohort study. METHODS: All patients undergoing total laryngectomy for squamous cell carcinoma at one center from 2006 to 2016 were included. Data were recorded prospectively as part of the patients' standard care, but were collated retrospectively, giving this study an ambispective design. The histology report for thyroid invasion was taken as the gold standard. The computed tomography report was categorized by invasion of tumor into intralaryngeal, laryngeal cartilage involvement, and extralaryngeal tissues. RESULTS: Seventy-nine patients were included. Nine patients had thyroid involvement on histology, translating to an incidence of 11.29% in this population. The positive predictive value for cartilage involvement on computed tomography for thyroid invasion was 52.9% (95% confidence interval [CI]: 28.5%-76.1%) and the negative predictive value was 100% (95% CI: 92.7%-100%).The positive predictive value for extralaryngeal spread on computed tomography for thyroid involvement was 100% (95% CI: 62.9%-100%), and the negative predictive value was also 100% (95% CI: 93.5%-100%). CONCLUSIONS: This study has shown that preoperative computed tomography is an effective method of ruling out thyroid gland invasion. The absence of extralaryngeal spread on computed tomography has been shown to be the most useful finding, with a high negative predictive value and a narrow 95% CI. LEVEL OF EVIDENCE: 4. ABSTRACT

La celulitis orbitaria se define como la inflamación de los tejidos de la órbita, es una infección con riesgo vital de las partes blandas por detrás del tejido orbitario. En la patogenia más frecuente se cita la sinusitis etmoidal. En el lactante, el seno maxilar no está neumatizado. El Streptococcus pneumoniae, Staphylococcus aureus e Haemophilus influenzae, son microorganismos frecuentes. La tomografía axial computarizada y la resonancia magnética son necesarios para detectar alteraciones de las estructuras orbitarias, los senos paranasales y el cerebro. Se presentó un cuadro de celulitis orbitaria y osteomielitis, secundaria a sinusitis etmoidal y maxilar en una lactante femenina de dos meses de edad, dolencia infrecuente que fue atendida en el Hospital Pediátrico Provincial Octavio de la Concepción de la Pedraja, de Holguín. Evolucionó satisfactoriamente, por la intervención precoz y adecuada de un equipo multidisciplinario.


Orbital cellulitis, is the inflammation of the orbital tissue, it is an infection with vital risk of the soft parts behind the orbital tissue. Ethmoidal sinusitis is reported as the more frequent pathogenic factor. The maxillary sinus is not pneumatizaised in nursling. Streptococcus pneumoniae, Staphylococcus aureus and Haemophilus influenzae are frequent microorganism. Computer axial tomography and magnetic resonance are necessary to detect orbital, paranasal sinus, and brain structures changes. A female patient of two months of age was reported in this paper, the patient presented an orbital cellulite picture, caused by ethmoidal and maxillary sinusitis, who was attended at Octavio de la Concepción de la Pedraja pediatric hospital. She had a satisfactory evolution due to the early and adequate care of a multidisciplinary team.
