ABSTRACT

Introducción El umbral de confort máximo o maximum comfort level (MCL), umbral eléctrico o threshold level (THR) e impedancia eléctrica cambian en el postoperatorio del implante coclear durante meses hasta estabilizarse. El objetivo de este artículo es establecer la variación durante cinco años posquirúrgicos de la impedancia, y su relación con MCL en adultos implantados unilateralmente. Métodos Estudio retrospectivo a cinco años, con 78 pacientes adultos implantados con MED-EL en un hospital terciario desde el año 2000 hasta 2015. Se analizó la variación de impedancia, MCL y relación entre ellos, en electrodos basales (9-12), medios (5-8) y apicales (1-4), realizando análisis inferencial ANOVA de medidas repetidas con comparaciones entre tiempos consecutivos, corregidas con criterio Bonferroni. Resultados Treinta y tres hombres (42,3%) y 45 mujeres (57,7%), con edad media 52,7 ± 14,6 años. Se consideró «estabilidad» el momento del seguimiento sin diferencias estadísticamente significativas entre una visita y la siguiente. Los cambios en la impedancia en electrodos medios dejaron de ser estadísticamente significativos a los tres meses, y en apicales a los seis meses, con valores medios de 5,84 y 6,43 kohms. MCL se estabilizó a los dos años en electrodos basales y apicales, y a los tres años en medios, con valores medios de 24,9, 22,7 y 25,6 qu. Hubo correlación entre MCL e impedancia en electrodos medios hasta 3 meses y en apicales hasta un año. Conclusiones La impedancia eléctrica desciende significativamente en electrodos medios y apicales hasta tres y seis meses. El MCL aumenta significativamente hasta dos años. La impedancia se relaciona con MCL hasta seis meses. (AU)

Analisar comparativamente o nível de conforto de familiares que participaram da visita ampliada e da visita social em Unidade de Terapia Intensiva. Método: Estudo transversal, desenvolvido com 57 familiares dos sujeitos internados na Unidade de Terapia Intensiva, entre 2019 e 2020. O conforto foi avaliado pela Escala de Conforto para Familiares de Pessoas em Estado Crítico de Saúde. Os dados foram analisados descritivamente e a análise bivariada pelo teste de T Student. O nível de significância estatística foi de 5%. Resultados: Os familiares inclusos na visita ampliada dispõem de um nível de conforto (4,00 ±0,53) maior que os da visita social (3,57 ±0,51), com (p=0,004). Observa-se melhor comunicação entre a equipe e os familiares da visita ampliada e um maior conhecimento por parte desses sobre o quadro de saúde do enfermo. Conclusão: A visita ampliada tem proporcionado maior nível de conforto nos familiares quando comparadas com a visita social.(AU)


To comparatively analyze the comfort level of family members who participated in the extended visit and the social visit in the Intensive Care Unit. Method: A cross-sectional study was carried out with 57 family members of patients admitted to the Intensive Care Unit between 2019 and 2020. Comfort was assessed using the Comfort Scale for Relatives of People in a Critical Health Condition. Data was analyzed descriptively and bivariate analysis using the Student's t-test. The level of statistical significance was 5%. Results: Family members included in the extended visit have a higher level of comfort (4.00 ±0.53) than those in the social visit (3.57 ±0.51), with (p=0.004). There was better communication between the team and the relatives during the extended visit and greater knowledge on the part of the latter about the patient's health condition. Conclusion: The extended visit has provided a greater level of comfort for family members when compared to the social visit.(AU)


Analizar comparativamente el nivel de confort de los familiares que participaron en la visita prolongada y la visita social en la Unidad de Cuidados Intensivos. Método: Se realizó un estudio transversal con 57 familiares de pacientes ingresados en la Unidad de Cuidados Intensivos entre 2019 y 2020. Se evaluó el confort mediante la Escala de Confort para Familiares de Personas en Estado Crítico de Salud. Los datos se analizaron de forma descriptiva y análisis bivariante mediante la prueba t de Student. El nivel de significación estadística fue del 5%. Resultados: Los familiares incluidos en la visita ampliada tienen un mayor nivel de confort (4,00 ±0,53) que los de la visita social (3,57 ±0,51), con (p=0,004). Hubo mejor comunicación entre el equipo y los familiares durante la visita ampliada y mayor conocimiento por parte de estos últimos sobre el estado de salud del paciente. Conclusiones: La visita ampliada ha proporcionado un mayor nivel de confort a los familiares en comparación con la visita social.(AU)

Este grupo es producto del acuerdo de colaboración firmado por la Sociedad de Medicina Intensiva de Madrid (SOMIAMA) y la Sociedad de Anestesiología, Reanimación y Terapéutica del Dolor de Madrid (SAR MADRID), por el que las organizaciones acordaron crear grupos de trabajo conjuntos para mejorar la atención al paciente crítico.El dolor, el malestar, la agitación y el delirio causan sufrimiento, retrasan el alta y pueden provocar complicaciones graves en los pacientes ingresados en las unidades de cuidados críticos médicos y quirúrgicos y en las unidades de cuidados postanestésicos. Los principales objetivos en este tipo de unidades incluyen: asegurar el confort de los pacientes que sufren o se recuperan de una enfermedad crítica. Evitar las complicaciones asociadas a las medidas, sobre todo farmacológicas, adoptadas para asegurar ese confort. (AU)

INTRODUCTION: Patients often perceive flexible bronchoscopy as an unpleasant procedure. The aim of this study was to investigate the effect of virtual reality (VR) hypnosis on tolerance to flexible bronchoscopy. MATERIALS AND METHODS: We conducted a prospective, randomized, controlled, monocentric study comparing flexible bronchoscopy with VR-induced hypnosis to the usual procedure. Patient tolerance was evaluated using a visual analogue scale (VAS), the state-trait anxiety inventory (STAI) before and after the procedure and, finally, willingness to repeat the examination under the same conditions (WTR). RESULTS: Among the 70 patients included, 34 were randomized to the VR hypnosis group and 36 to the control group. There was no difference between the 2 groups in terms of modification of the pre-/post-bronchoscopy VAS for anxiety, pain, cough, choking, nausea and overall discomfort, or modification of the STAI score and WTR. Subgroup analysis among patients who were more anxious before the procedure revealed a trend toward reduced anxiety in the VR hypnosis group. CONCLUSION: This study did not observe any effect of VR hypnosis on the tolerance of patients during routine flexible bronchoscopy. However, VR hypnosis may be beneficial in patients with higher anxiety score before bronchoscopy, a hypothesis that needs to be confirmed by further studies with a larger number of subjects.

These questions pose difficulties, insofar as the act of eating and drinking as a vital act has a symbolic, cultural and social dimension. They must be clarified with family members or even caregivers who are not familiar with these types of care.

Introducción: Los cirujanos informaron esfuerzo y dolor musculoesquelético durante o después de un procedimiento. Modificando que la inserción del trócar localizado en línea media clavicular se coloque en línea media, puede mejorar la postura del cirujano. En el Hospital Universitario de Caracas, la técnica de colecistectomía laparoscópica modificada se realiza con frecuencia por disminuir la sensación de esfuerzo y dolor durante el acto quirúrgico. Se plantea el estudio de los elementos de confort para el cirujano con esta técnica modificada. Métodos: Se realizó un estudio prospectivo, descriptivo, comparativo y de corte transversal en cirujanos de pacientes con litiasis vesicular que se resolvieron con colecistectomía laparoscópica, en el Hospital Universitario de Caracas, período enero-agosto 2022. Resultados: Se realizaron 77 colecistectomías laparoscópicas, conformadas por: técnica americana 32 (41,6%) y la técnica modificada 45 (58,4%). La técnica americana evidenció como zona dolorosa la muñeca izquierda (62,5%), y en la técnica modificada reportan en 91,1% sin zona dolorosa. El 43,8% de los cirujanos no se sienten cómodos con la técnica americana, mientras que en la modificada el 97,8% reportaron la técnica cómoda. Los cirujanos refirieron un esfuerzo difícil en el 56,3% con la técnica americana y con la modificada el 82,2% refieren un esfuerzo leve. El grado de dolor en las extremidades fue mayor en los que realizaron la técnica americana en comparación con la modificada, siendo todos los resultados estadísticamente significativos. Conclusión: La técnica modificada tiene ventajas en cuanto a la comodidad del cirujano y su equipo de trabajo y puede emplearse desde el inicio de la formación de cirujanos(AU)


Surgeons reported exertion and musculoskeletal pain during or after a procedure. Modifying the insertion of the trocar located in the clavicular midline to be placed in the midline, to improve the surgeon's posture. At the Hospital Universitario de Caracas, the modified laparoscopic cholecystectomy technique is frequently performed to reduce the sensation of effort and pain during the surgical act. the study of comfort elements for the surgeon with this modified technique is proposed. Methods: A prospective, descriptive, comparative and cross-sectional study was carried out in patients with gallbladder lithiasis at the Hospital Universitario de Caracas, from January to August 2022. Results: 77 laparoscopic cholecystectomies were performed, consisting of: American technique 32 (41, 6%) and the modified technique 45 (58.4%). The American technique showed the left wrist as a painful area (62.5%), and in the modified technique they reported no painful area in 91.1%. 43.8% of the surgeons did not feel comfortable with the American technique, while in the modified one 97.8% reported the comfortable technique. the surgeons reported a difficult effort in 56.3% with the American technique and with the modified one, 82.2% reported a light effort. the degree of pain in the extremities was greater in those who performed the American technique compared to the modified one, all of the results being statistically significant. Conclusion: the modified technique has advantages in terms of comfort for the surgeon and his team and can be used from the beginning of surgeon training

Despite the available scientific evidence, there is still controversy regarding the use of enteral tube nutrition in people with advanced dementia. This document aims to reflect on the key aspects of advanced dementia, tube nutritional therapy and related ethical considerations, as well as to respond to several frequent questions that arise in our daily clinical practice.

As they near the end of life, long term care (LTC) residents often experience unmet needs and unnecessary hospital transfers, a reflection of suboptimal advance care planning (ACP). We applied the knowledge-to-action framework to identify shared barriers and solutions to ultimately improve the process of ACP and improve end-of-life care for LTC residents. We held a 1-day workshop for LTC residents, families, directors/administrators, ethicists, and clinicians from Manitoba, Alberta, and Ontario. The workshop aimed to identify: (1) shared understandings of ACP, (2) barriers to respecting resident wishes, and (3) solutions to better respect resident wishes. Plenary and group sessions were recorded and thematic analysis was performed. We identified four themes: (1) differing provincial frameworks, (2) shared challenges, (3) knowledge products, and 4) ongoing ACP. Theme 2 had four subthemes: (i) lacking clarity on substitute decision maker (SDM) identity, (ii) lacking clarity on the SDM role, (iii) failing to share sufficient information when residents formulate care wishes, and (iv) failing to communicate during a health crisis. These results have informed the development of a standardized ACP intervention currently being evaluated in a randomized trial in three Canadian provinces.

INTRODUCTION: There are clinical and sociodemographic factors that have an impact on the comfort of the critically ill paediatric patient. The main aim of this study was to determine the level of discomfort of paediatric patients admitted to different national hospitals, and to analyse its correlation with sociodemographic and clinical variables, analgosedation, and withdrawal syndrome. METHODS: An observational, analytical, cross-sectional, and multicentre study was conducted in five Spanish hospitals. The level of analgosedation was assessed once per shift over a 24 h period, using a BIS sensor, and pain with scales adapted to paediatric age population. The intensity of withdrawal syndrome was determined using the Withdrawal Assessment Tool (WAT-1) scale once per shift for 3 days. Discomfort level was simultaneously assessed using COMFORT Behaviour Scale-Spanish version (CBS-S). RESULTS: A total of 261 critically ill paediatric patients with median age of 1.61 years (IQR = 0.35-6.55) were included. An overall discomfort score of 10.79 ±â€¯3.7 was observed during morning compared to 10.31 ±â€¯3.3 during the night. When comparing analgosedation and non-analgosedation groups, statistical differences were found in both shifts (χ2: 45.48; P = .001). At the same time, an association was observed (P < .001) between low discomfort scores and development of withdrawal syndrome development assessed with WAT-1. CONCLUSIONS: As there is a percentage of the studied population with discomfort, specific protocols need to be developed, guided by valuated and clinically tested tools, like the COMFORT Behaviour Scale-Spanish version.

ABSTRACT

O estresse térmico é um dos principais problemas relacionados ao decréscimo do bem-estar animal em vacas leiteiras. Entre as regiões do Brasil, a Amazônia se destaca em suas características climáticas como os altos índices de temperatura e umidade atmosférica oriundos de uma região de clima equatorial. Nesse cenário, o estresse térmico é um dos fatores que prejudicam a criação, a produtividade, os parâmetros fisiológicos e o comportamento dos animais. Nesse sentido, o presente artigo de revisão visa contextualizar as características regionais com a produção leiteira na Amazônia, destacando o estresse térmico, seus efeitos e as estratégias de conforto térmico. Dessa forma, o emprego de medidas como o melhoramento genético, adoção de sistemas de produção silvipastoris e implantação de tecnologias são maneiras eficazes de melhoria das condições climáticas para os animais. No entanto, salienta-se a importância da realização de estudos específicos para determinação dos parâmetros de conforto térmico das vacas leiteiras na Amazônia.(AU)


Heat stress is an important issue related to decrease in animal welfare in dairy cows. The Amazon region stands out in Brazil due to its diverse climatic characteristics, with high temperature and atmospheric humidity resulting from its equatorial climate. In this context, heat stress is one of the factors that impair the breeding, productivity, physiological parameters, and behavior of those animals. This article aims at performing a literature review in order to contextualize the regional characteristics with dairy production in the Amazon, with focus on heat stress, its effects, and thermal comfort strategies. The use of measures such as genetic improvement, adoption of production using silvopastoral system and the implementation of technologies are effective ways to improve climatic conditions for animals. However, the importance of conducting specific studies to determine the parameters of thermal comfort of dairy cows in the Amazon is also emphasized.(AU)


El estrés por calor es uno de los principales problemas relacionados con la disminución del bienestar animal en vacas lecheras. Entre las regiones de Brasil, la Amazonia se destaca por sus características climáticas, como los altos niveles de temperatura y humedad atmosférica que surgen de una región con clima ecuatorial. En este escenario, el estrés por calor es uno de los factores que inciden en la creación, productividad, parámetros fisiológicos y comportamiento de los animales. Este artículo de revisión tuvo como objetivo contextualizar las características regionales de la producción lechera en Amazonía, destacando el estrés térmico, sus efectos y estrategias de confort térmico. Así, el uso de medidas como el mejoramiento genético, la adopción de sistemas de producción silvopastoril y la implementación de tecnologías son formas efectivas de mejorar las condiciones climáticas para los animales. Sin embargo, se destaca la importancia de realizar estudios específicos para determinar los parámetros de confort térmico de las vacas lecheras en Amazonía.(AU)

Resumen Introducción. En este artículo se presentan los resultados finales de la investigación Análisis del confort térmico de conjuntos habitacionales realizada en la ciudad de Portoviejo, Ecuador. Objetivo. Analizar la percepción del confort térmico de las viviendas del conjunto habitacional Fuentes del Río de la ciudad de Portoviejo y reconocer los elementos que favorecen o no la calidad climática del interior de sus espacios, mediante técnicas de investigación y herramientas tecnológicas, evidenciando el valor de orientar las edificaciones de carácter residencial de acuerdo a las condicionantes climáticas que influyen en ellas. Materiales y Métodos. Se presenta una investigación bibliográfica y de campo, apoyada en el diagnóstico del conjunto arquitectónico en estudio. Resultados. Una vez obtenidas las valoraciones, se procedió mediante estrategias de diseño a idear elementos que mitiguen las incidencias climáticas en el interior de viviendas además de elaborar un listado de parámetros bioclimáticos que aporten en la proyección de futuros conjuntos urbanísticos en el área urbana de la ciudad de Portoviejo. Conclusiones. los elementos analizados permiten reconocer la importancia de emplear parámetros bioclimáticos o principios de arquitectura sustentable y el uso de tecnología constructiva que faciliten la creación de programas habitacionales sustentables, contribuyendo en forma positiva con el medio ambiente y mejorando la habitabilidad de los espacios.


Abstract Introduction. This article presents the final results of the research Analysis of the thermal comfort of residential complexes carried out in the city of Portoviejo, Ecuador. Objective. Analyze the perception of thermal comfort of the housing units of the Fuentes del Río housing complex in the city of Portoviejo and recognize the elements that favor or not the climatic quality of the interior of their spaces, through research techniques and technological tools, demonstrating the value of orienting the buildings of a residential nature according to the climatic conditions that influence them. Materials and methods. A bibliographic and field research is presented, supported by the diagnosis of the architectural complex under study. Results. Once the valuations were obtained, we proceeded by means of design strategies to devise elements that mitigate the climatic incidences inside homes, in addition to preparing a list of bioclimatic parameters that contribute to the projection of future urban complexes in the urban area of the city of Portoviejo. Conclusions. The elements analyzed allow us to recognize the importance of using bioclimatic parameters or principles of sustainable architecture and the use of constructive technology that empower the creation of sustainable housing programs, contributing positively to the environment and improving the habitability of the spaces.


Resumo Introdução. Este artigo apresenta os resultados finais da pesquisa Análise do conforto térmico de complexos residenciais realizada na cidade de Portoviejo, Equador. Objetivo. Analisar a percepção de conforto térmico das unidades habitacionais do conjunto habitacional Fuentes del Río, na cidade de Portoviejo, e reconhecer os elementos que favorecem ou não a qualidade climática do interior de seus espaços, por meio de técnicas de pesquisa e ferramentas tecnológicas, demonstrando o valor da orientação dos edifícios. de natureza residencial de acordo com as condições climáticas que os influenciam. Materiais e métodos. É apresentada uma pesquisa bibliográfica e de campo, apoiada no diagnóstico do complexo arquitetônico em estudo. Resultados. Uma vez obtidas as avaliações, procedemos por meio de estratégias de projeto para conceber elementos que mitigam os incidentes climáticos dentro das casas, além de elaborar uma lista de parâmetros bioclimáticos que contribuem para a projeção de futuros complexos urbanos na área urbana da cidade de Portoviejo. Conclusões. Os elementos analisados permitem reconhecer a importância do uso de parâmetros ou princípios bioclimáticos da arquitetura sustentável e o uso de tecnologia construtiva que possibilite a criação de programas habitacionais sustentáveis, contribuindo positivamente para o meio ambiente e melhorando a habitabilidade dos espaços.

ABSTRACT

The aim of this study was to compare the acute cardiometabolic and perceptual responses between local and whole-body passive heating. Using a water-perfused suit, 10 recreationally active males underwent three 90 min conditions: heating of the legs with upper-body cooling (LBH), whole-body heating (WBH) and exposure to a thermoneutral temperature (CON). Blood samples were collected before and up to 3 h post-session to assess inflammatory markers, while a 2 h oral glucose tolerance test was initiated 1 h post-session. Femoral artery blood flow and perceptual responses were recorded at regular intervals. The interleukin (IL)-6 incremental area under the curve (iAUC) was higher for LBH (1096 ± 851 pg/mL × 270 min) and WBH (833 ± 476 pg/mL × 270 min) compared with CON (565 ± 325 pg/mL × 270 min; p < 0.047). Glucose concentrations were higher after WBH compared with LBH and CON (p < 0.046). Femoral artery blood flow was higher at the end of WBH (1713 ± 409 mL/min) compared with LBH (943 ± 349 mL/min; p < 0.001), and higher in LBH than CON (661 ± 222 mL/min; p = 0.002). Affect and thermal comfort were more negative during WBH compared with LBH and CON (p < 0.010). In conclusion, local passive heating elevated blood flow and the IL-6 iAUC. However, while resulting in more positive perceptual responses, the majority of the included cardiometabolic markers were attenuated compared with WBH. Novelty: The increase in the IL-6 iAUC in response to passive heating is not reduced by upper-body cooling. Upper-body cooling attenuates the plasma nitrite, IL-1ra and femoral artery blood flow response to passive heating. Upper-body cooling leads to more positive perceptual responses to passive heating.

ABSTRACT

Objetivo identificar as dificuldades na abordagem dos cuidados paliativos e da terminalidade na percepção de acadêmicos de Enfermagem e Medicina de uma universidade pública. Método trata-se de estudo exploratório, descritivo, com abordagem qualitativa, cuja coleta de dados foi realizada entre setembro de 2019 e janeiro de 2020, por meio de entrevista semiestruturada. Resultados emergiram duas categorias temáticas principais: Desafios da comunicação em cuidados paliativos e Despreparo para lidar com o morrer e a morte. Considerações finais identificou-se que os acadêmicos de Enfermagem e Medicina apresentam dificuldades relacionadas à comunicação na assistência em cuidados paliativos e em situações de fim de vida. Demonstrou-se ainda o seu despreparo em lidar com a morte, o sofrimento humano e os conflitos morais decorrentes de crenças religiosas.


Objetivo identificar las dificultades para abordar los cuidados paliativos y la terminalidade en la percepción de los estudiantes de Enfermería y Medicina de una universidad pública. Método se trata de un estudio exploratorio y descriptivo con un enfoque cualitativo, cuya recogida de datos se llevó a cabo entre septiembre de 2019 y enero de 2020, a través de entrevistas semiestructuradas. Resultados surgieron dos categorías temáticas principales: Desafíos de la comunicación en los cuidados paliativos y Falta de preparación para lidiar con el morir y la muerte. Consideraciones finales se identificó que los estudiantes de Enfermería y Medicina presentan dificultades relacionadas con la comunicación en cuidados paliativos y situaciones al final de la vida. También se demostró su falta de preparación al tratar la muerte, el sufrimiento humano y los conflictos morales derivados de las creencias religiosas.


Objective to identify the difficulties in addressing palliative care and terminality in the perception of Nursing and Medical students from a public university. Method this is an exploratory, descriptive study with a qualitative approach, whose data collection was carried out between September 2019 and January 2020, through semi-structured interviews. Results two main thematic categories emerged: Challenges of communication in palliative care and Unpreparedness to deal with dying and death. Final Considerations Nursing and Medical students present difficulties related to communication in palliative care and end-of-life situations. This unpreparedness was also present in dealing with death, human suffering and moral conflicts arising from religious beliefs.

Resumo Objetivo: Identificar, nas bases de dados científicas, as evidências que comprometem o sono e as intervenções de enfermagem promotoras do sono em Unidades de Terapia Intensiva. Métodos: Revisão integrativa da literatura, realizada nas bases de dados MEDLINE, CINAHL e Cochrane Database of Systematic Reviews, para responder às questões norteadoras do estudo, de acordo com o formato PICo, nos meses de maio e junho de 2019. Os idiomas considerados foram o inglês, português e espanhol. Resultados: Identificou-se 110 estudos e 12 foram considerados elegíveis para a revisão. A análise reporta que o comprometimento do sono é multicausal, sendo este influenciado sobretudo por desconforto ambiental e físico do paciente. As intervenções de enfermagem, tais como a gestão do ambiente, no âmbito do ruído e da luminosidade, o agrupamento da prestação de cuidados, o controle da dor e da ansiedade do paciente internado, são fundamentais para a qualidade do sono. Conclusão: A produção científica evidencia que as intervenções de Enfermagem identificadas, sobretudo no âmbito da promoção do conforto ambiental e físico do paciente, contribuem para a qualidade do sono em Unidades de Terapia Intensiva, contudo a promoção do conforto psicoespiritual e social do paciente ainda é superficial, tendo em conta o foco e as prioridades nesta tipologia de internamento.

Métodos: Revisión integradora de literatura, realizada en las bases de datos MEDLINE, CINAHL y Cochrane Database of Systematic Reviews para responder las preguntas orientadoras del estudio, de acuerdo con el formato PICO, en los meses de mayo y junio de 2019. Los idiomas considerados fueron inglés, portugués y español. Resultados: Se identificaron 110 estudios y 12 fueron considerados elegibles para la revisión. El análisis reporta que el comprometimiento del sueño es multicausal y lo que más influye en el sueño es la incomodidad ambiental y física del paciente. Las intervenciones de enfermería, tales como el manejo del ambiente respecto al ruido y a la luminosidad, el agrupamiento de la prestación de cuidados y el control del dolor y de la ansiedad del paciente internado, son fundamentales para la calidad del sueño. Conclusión: La producción científica evidencia que las intervenciones de enfermería identificadas, sobre todo respecto a la promoción del bienestar ambiental y físico del paciente, contribuyen con la calidad del sueño en unidades de cuidados intensivos. No obstante, la promoción del bienestar piscoespiritual y social del paciente aún es superficial, teniendo en cuenta dónde se centra la atención y cuáles son las prioridades en este tipo de internación.


Abstract Objective: To identify, in the scientific databases, evidence of factors that impair sleep and nursing interventions to promote sleep in Intensive Care Units. Methods: An integrative literature review was conducted across the databases MEDLINE and CINAHL and the Cochrane Database of Systematic Reviews between May and June 2019 using the PICO framework, seeking to answer the guiding questions of the study. The languages included in the literature review were English, Portuguese, and Spanish. Results: One hundred and ten studies were identified, twelve of which were considered eligible for review. The analysis of the articles found that sleep impairment is multicausal, being influenced mainly by the patient's environmental and physical discomfort. Nursing interventions, such as environmental management of noise and light, grouping of care procedures, and control of pain and anxiety in the hospitalized patient, are fundamental for sleep quality. Conclusion: The academic literature shows that the nursing interventions identified, particularly those that promote the patient's environmental and physical comfort, contribute to the quality of sleep in Intensive Care Units. However, promotion of the patient's psychospiritual and social comfort is still lacking, given the focus and priorities of this type of hospitalization.

ABSTRACT

INTRODUCTION: There are clinical and sociodemographic factors that have an impact on the comfort of the critically ill paediatric patient. The main aim of this study was to determine the level of discomfort of paediatric patients admitted to different national hospitals, and to analyse its correlation with sociodemographic and clinical variables, analgosedation, and withdrawal syndrome. METHODS: An observational, analytical, cross-sectional, and multicentre study was conducted in five Spanish hospitals. The level of analgosedation was assessed once per shift over a 24h period, using a BIS sensor, and pain with scales adapted to paediatric age population. The intensity of withdrawal syndrome was determined using the Withdrawal Assessment Tool (WAT-1) scale once per shift for 3 days. Discomfort level was simultaneous assessed using COMFORT Behaviour Scale-Spanish version (CBS-S). RESULTS: A total of 261 critically ill paediatric patients with median age of 1.61 years (IQR=0.35-6.55) were included. An overall discomfort score of 10.79±3.7 was observed during morning compared to 10.31±3.3 observed during the night. When comparing analgosedation and non-analgosedation groups, statistically differences were found in both shifts (χ2: 45.48; P=.001). At the same time, an association was observed (P<.001) between low discomfort scores and development of withdrawal syndrome development assessed with WAT-1. CONCLUSIONS: As there is a percentage of the studied population with discomfort, specific protocols need to be developed, guided by valuated and clinically tested tools, like the COMFORT Behaviour Scale-Spanish version.

ABSTRACT

INTRODUCTION: These guidelines deal with the parturient wellbeing in terms of hydration and regional and systemic pain management during labour. METHOD: Guidelines were established based on literature analysis and experts consensus. RESULTS: Clear liquids consumption is permitted all along labor and postpartum, without volume limitation, in patients at low risk of general anesthesia (grade B). The consumption of solid foods is not recommended during the active stage of labor (consensus agreement). It is recommended to promote on regional analgesia to prevent inhalation (grade A). Pain relief using regional analgesia is a part of normal childbirth. It is recommended to provide regional analgesia to parturient who wish these technics. Regional analgesia is the safest and most effective analgesic method for the mother (grade A) and the child (grade B). It is recommended to inform women on the analgesic technics, to respect their choice and consider the right for a parturient to change her strategy in obstetrical circumstances or in cases of untractable pain (consensus agreement). It is recommended to perform a "low-dose" regional analgesia that respects the experience of childbirth (grade A) and maintain it with a patient controlled epidural analgesia technics (grade A). There is no minimum cervical dilation to allow epidural analgesia (grade A). In cases of rapid labor or after delivery for revision, spinal or combined spinal epidural can be used (grade C). Epidural has not to be ended before birth (consensus agreement). Blood pressure and fetal heart rate must be monitored every 3minutes after induction and/or each 10mL bolus then hourly (consensus agreement). Systematic and preventive fluid loading is not needed if only due to regional analgesia (grade B). Deambulation or postures are allowed in the absence of motor block and must be traced and do not alter the distribution of the regional analgesia (grade C). The postures of childbirth do not alter regional analgesia spread (NP2). There is no effect low dose regional analgesia on the duration of obstetric labor, nor the rate of instrumental births or caesarean section (NP1). Systematic use of oxytocin due to epidural analgesia is neither useful nor recommended (AE). Regional analgesia has no side effect on the fetus or newborn (NP1). If regional analgesia is contraindicated or during the waiting time, alternatives analgesic drugs (entonox, nalbuphine and tramadol or pudendal block) can be used but their analgesic efficiency remains mediocre to moderate and they are associated with adverse maternal and especially neonatal side effects (NP2). Remifentanil, ketamine and volatile anesthetics are excluded from these recommendations. CONCLUSION: The present guidelines were established to update wellbeing of normal parturient during normal labor: hydration is recommended and low dose patient-controlled regional (epidural and spinal) analgesia is the most effective and safest analgesic method.
