ABSTRACT

Introduction: Family Medicine is a growing specialty in the medical world. While it is expected that the specialty should have its own unique and established core values and tasks, the breath of the practice in several countries of the world has made this a daunting task. Core values and Tasks have far reaching effects on professions. They guide development of curricula, methods of instruction, standards of performance and even the culture of the profession. We aimed to explore the core value system of Family Physicians and General Practitioners practicing in Africa. Methods: Using the Delphi technique, a purposive selection of African Family Medicine practitioners in academia, public service, private practice and clinical training across Central, East, North, South and West Africa was conducted. Participants were asked to select five core values from an alphabetically collated global list of 29 core values in an online survey. The five most selected core values were collated and sent out in the second round to the participants to rank in order of importance. Results: Practitioners from nine African countries in three out of the five United Nations subregions of Africa completed the study. The first round of the study saw participation of a team of nineteen experts who selected the following five core values-Comprehensive care, Continuity of Care, Collaborative Care, Patient centered care, and Life-long learning. Discussion/Conclusion: The core values selected were not very different from global literature. These core values should guide the development of curricula, standardization of training methods and creation of benchmarks for standards of practice for the specialty in Africa.

ABSTRACT

Abstract An unsteady plan for curriculum development and supportive issues in physical education (PE) has created confusion among professionals. The purpose of this research is to investigate the factors perceived as important in the development of quality physical education (QPE) by professionals in Latin American (LA) countries. A questionnaire consisting of 24 items based on QPE was responded by 468 professionals collected from 6 LA cities. An exploratory factor analysis (EFA) of the 24 items using ML extraction and direct oblimin rotation were applied, and the retained 17 items were clustered in a three factor solution referred to as, Development and Supportive Elements for QPE in School (DSFQPE) (α = .935), Core Value of QPEtabl (CVPE) (α = .890), and Curriculum Arrangement of Physical Activities (CAPA) (α = .850). The retained items indicated excellent properties and the basic framework as perceived by professionals from PE in LA countries as important in the investigation of QPE.


Resumo Um plano instável para o desenvolvimento de currículos e questões de apoio na educação física (PE) criou confusão entre os profissionais. O objetivo desta pesquisa é investigar os fatores percebidos como importantes no desenvolvimento da educação física de qualidade (QPE) por profissionais de países da América Latina (AL). Um questionário composto por 24 itens com base no QPE foi respondido por 468 profissionais coletados em 6 cidades da América Latina. Uma análise fatorial exploratória dos 24 itens usando extração ML e rotação obliminar direta foram aplicados, e os 17 itens retidos foram agrupados em uma solução de três fatores denominada Elementos de Desenvolvimento e Suporte para QPE na Escola (DSFQPE) (α = 0,935), Valor essencial do QPE (CVPE) (α = 0,890) e Arranjo Curricular das Atividades Físicas (CAPA) (α = 0,850). Os itens retidos indicaram propriedades excelentes e o referencial básico percebido pelos profissionais de EF em países da América Latina como importante na investigação do PEQ.


Resumen Un plan inestable para el desarrollo curricular y aspectos de apoyo para la educación física (EF) han creado confusión entre los profesionales. El propósito de esta investigación es investigar los factores percibidos como importantes en el desarrollo de la educación física de calidad (EFC) por profesionales en países de América Latina (AL). Un cuestionario que consta de 24 ítems basados en EFC fue respondido por 468 profesionales recolectados en 6 ciudades de LA. Se aplicó un Análisis factorial exploratorio de los 24 ítems que utilizan extracción ML y rotación oblicua directa, y los 17 ítems retenidos se agruparon en una solución de tres factores denominada Elementos de desarrollo y de apoyo para EFC en la escuela (EDAEFC) (α = 0,935); Valor principal de EFC (CVPE) (α = 0,890) y Arreglo del plan de actividades físicas (CAPA) (α = 0,850). Los ítems retenidos indicaron excelentes propiedades y el marco básico es percibido por los profesionales de EF en los países de AL como importante en la investigación de EFC.

ABSTRACT

Since laparoscopy was first used in cholecystectomy in 1987, it has developed quickly and has been used in most fields of traditional surgery. People have now accepted its advantages like small incision, quick recovery, light pain, beauty and short hospital stays. In early times, there are still controversies about the application of laparoscopy in malignant tumor treatments, especially about the problems of oncology efficacy, incision implantation and operation security. However, these concerns have been fully eliminated by evidences on the basis of evidence-basis medicine. In recent years, new minimally invasive technologies are appearing continually, but they still have challenges and may increase the difficulties of radical dissection and the risks of potential complications, so they are confined to benign or early malignant tumors. The core value of the laparoscopic technique is to ensure the high quality of tumor's radical resection and less complications. On the basis of this, it is allowed to pursue more minimally invasive techniques. Since the development of laparoscopic colorectal surgery is rapid and unceasing, we have reasons to believe that laparoscopic surgery will become gold standard for colorectal surgery in the near future.

ABSTRACT

An analysis of the core value of 38 tertiary hospitals in the country identified the humanistic spirit as the lifeline of their core value, comprising such factors as benevolence,professionalism and innovation. It also found that upholding of such a spirit is in line with medical model reform, with improvement of hospital management system, and with the upgrading of hospital staff's humanistic competence. The author pointed out that the atmosphere building of such spirit is a systematic process, and that such a spirit can only exert its entire potential by identifying the key person(s) of education and the point of force application, and igniting the inherent innovation power.
