ABSTRACT

Introducción : El dengue es en la actualidad la más importante arbovirosis que afecta al hombre, es un problema de salud pública para Cuba y el mundo, a pesar de los esfuerzos por parte de los estados miembros, para contenerlo y mitigar el impacto de las epidemias. Objetivo: Determinar la comprensión sobre control epidemiológico del dengue en estudiantes de medicina. Métodos : Se realizó un estudio observacional descriptivo de corte trasversal, el universo de estudio lo constituyó los 368 estudiantes matriculados en el quinto año de medicina de la Universidad de Ciencias Médicas de Camagüey luego de culminada la rotación de Salud Pública. Se calculó una muestra de 188 estudiantes. Se aplicaron métodos y técnicas de la investigación científica en las ciencias médicas. Resultados : Solo el 9 % de los estudiantes identificaron el tipo de mosquito que interviene en la transmisión del dengue; el 69,7 % desconoce el horario de máxima circulación del vector, el 75 % desconoce el cálculo e interpretación del índice casa y Breteau; el 75,5 % no conoce las medidas de control en tanques bajos y el 72,3 % tiene desconocimiento de la información en lo relacionado con el vector. Conclusiones : Se evidenció un bajo conocimiento sobre aspectos importantes relacionados con el vector transmisor del dengue, el manejo epidemiológico y control entomológico, así como la información recibida sobre este tema en la educación en el trabajo. Se estableció la correspondencia con los resultados de otros investigadores cubanos y extranjeros y se constató la efectividad, objetividad y actualidad del estudio.


Introduction: Dengue is currently the most important arbovirosis that affects man and it is a public health problem for Cuba and the world, despite the efforts of the member states to contain it and mitigate the impact of epidemics. Objective: To determine the understanding of dengue epidemiological control in medical students. Methods: A cross-sectional descriptive observational study was carried out; the universe of study was made up of the 368 students enrolled in the fifth year of medicine at the University of Medical Sciences of Camagüey after completing the Public Health rotation. A sample of 188 students was calculated. Methods and techniques of scientific research in the medical sciences were applied. Results: Only 9% of the students identified the type of mosquito involved in dengue transmission; 69.7% do not know the time of maximum circulation of the vector, 75% do not know the calculation and interpretation of the casa and Breteau index; 75.5% do not know the control measures in low tanks and 72.3% are unaware of the information related to the vector. Conclusions: There was evidence of low knowledge about important aspects related to the transmitting vector of dengue, epidemiological management and entomological control, as well as the information received on this subject in education at work. Correspondence with the results of other Cuban and foreign researchers was established and the effectiveness, objectivity and topicality of the study was verified.

A dengue é uma doença dolorosa e debilitante transmitida por insetos da espécie Aedes aegypti. Ela é definida como uma doença viral que, nos últimos anos, se espalhou vertiginosamente por todas as regiões tropicais e subtropicais do planeta. Este estudo teve como objetivo identificar e discutir o número e a taxa de incidência de casos de dengue no estado do Paraná utilizando-se dos boletins emitidos por semana epidemiológica nos anos de 2016 a 2021, considerando a sazonalidade da doença. Também se objetivou debater a incidência por macrorregional, as possíveis causas de períodos epidêmicos e ações de combate vetorial para redução dos casos da patologia. Foram utilizados como fonte de informações o banco de dados da Dengue/SVS/SESA, por meio de informes técnicos, disponibilizados pelo portal online de Boletins da Dengue Paraná da Secretaria de Estado de Saúde do Paraná. Conclui-se que o ano epidemiológico de 2019/2020 foi o de maior incidência e os anos epidemiológicos 2016/2017 e 2017/2018 apresentaram os menores casos durante todo período analisado. Dessa forma, a vigilância epidemiológica é muito importante para avaliação espacial da distribuição de casos para execução de ações estratégicas para redução da infestação do vetor. As políticas públicas e a disponibilização de inseticidas para aplicação também são essenciais para o combate da Dengue.


Dengue is a painful and debilitating disease transmitted by insects of the Aedes aegypti species. It is defined as a viral disease that, in recent years, has spread vertiginously throughout the tropical and subtropical regions of the planet. This study aimed to identify and discuss the number and incidence rate of dengue cases in the state of Paraná using the bulletins issued by epidemiological week in the years 2016 to 2021, considering the seasonality of the disease. The aim was also to discuss the incidence per macro-region, the possible causes of epidemic periods, and vectorial combat actions to reduce the cases of the pathology. The Dengue/SVS/SESA database was used as a source of information, through technical reports, made available by the online portal of Dengue Paraná Bulletins of the Paraná State Health Department. It is concluded that the epidemiological year 2019/2020 was the one with the highest incidence and the epidemiological years 2016/2017 and 2017/2018 had the lowest cases during the entire period analyzed. Thus, epidemiological surveillance is very important for the spatial assessment of the distribution of cases to carry out strategic actions to reduce vector infestation. Public policies and the availability of insecticides for application are also essential to combat Dengue.


El dengue es una enfermedad dolorosa y debilitante transmitida por insectos de la especie Aedes aegypti. Se define como una enfermedad viral que, en los últimos años, se ha extendido vertiginosamente por las regiones tropicales y subtropicales del planeta. Este estudio tuvo como objetivo identificar y discutir el número y la tasa de incidencia de los casos de dengue en el estado de Paraná utilizando los boletines emitidos por la semana epidemiológica en los años 2016 a 2021, considerando la estacionalidad de la enfermedad. También se pretendía discutir la incidencia por macrorregiones, las posibles causas de los periodos epidémicos y las acciones de control de vectores para la reducción de los casos de la enfermedad. Se utilizó como fuente de información la base de datos de Dengue/SVS/SESA, por medio de informes técnicos, puestos a disposición por el portal online de Boletines de Dengue Paraná de la Secretaría de Salud del Estado de Paraná. Se concluye que el año epidemiológico 2019/2020 fue el de mayor incidencia y los años epidemiológicos 2016/2017 y 2017/2018 presentaron los menores casos durante todo el periodo analizado. Por lo tanto, la vigilancia epidemiológica es muy importante para la evaluación espacial de la distribución de los casos para la implementación de acciones estratégicas para reducir la infestación del vector. Las políticas públicas y la disponibilidad de insecticidas para su aplicación también son esenciales para combatir el dengue.

ABSTRACT

The mean age of dengue hemorrhagic fever (DHF) cases increased considerably in Thailand from 8.1 to 24.3 y between 1981 and 2017 (mean annual increase of 0.45 y). Alternative proposed explanations for this trend, such as changes in surveillance practices, reduced mosquito­human contact, and shifts in population demographics, have different implications for global dengue epidemiology. To evaluate the contribution of each of these hypothesized mechanisms to the observed data, we developed 20 nested epidemiological models of dengue virus infection, allowing for variation over time in population demographics, infection hazards, and reporting rates. We also quantified the effect of removing or retaining each source of variation in simulations of the age trajectory. Shifts in the age structure of susceptibility explained 58% of the observed change in age. Adding heterogeneous reporting by age and reductions in per-serotype infection hazard to models with shifts in susceptibility explained an additional 42%. Reductions in infection hazards were mostly driven by changes in the number of infectious individuals at any time (another consequence of shifting age demographics) rather than changes in the transmissibility of individual infections. We conclude that the demographic transition drives the overwhelming majority of the observed change as it changes both the age structure of susceptibility and the number of infectious individuals. With the projected Thai population age structure, our results suggest a continuing increase in age of DHF cases, shifting the burden toward individuals with more comorbidity. These insights into dengue epidemiology may be relevant to many regions of the globe currently undergoing comparable changes in population demographics.

Dengue is a significant health concern in Sri Lanka, but diagnosis of the infecting dengue virus (DENV) serotype has hitherto been largely restricted to the Colombo district in the western province. Salinity tolerant Aedes vectors are present in the island's northern Jaffna peninsula, which is undergoing rapid groundwater salinization. Virus serotypes were determined by RT-qPCR in 107 and 112 patients diagnosed by NS1 antigen positivity from the Jaffna district in 2018 and 2019, respectively, and related to clinical characteristics. DENV1 and DENV2 were the most common serotypes in both years. Infections with multiple serotypes were not detected. DENV1 was significantly more prevalent in 2019 than 2018, while DENV3 was significantly more prevalent in 2018 than 2019 among the Jaffna patients. Limited genomic sequencing identified DENV1 genotype-I and DENV3 genotype-I in Jaffna patients in 2018. Dengue was more prevalent in working age persons and males among the serotyped Jaffna patients. DENV1 and DENV2 were the predominant serotypes in 2019 in the Colombo district. However, DENV1 and DENV3 were significantly more prevalent in Colombo compared with Jaffna in 2019. The differences in the prevalence of DENV1 and DENV3 between the Jaffna and Colombo districts in 2019 have implications for dengue epidemiology and vaccination. Salinity-tolerant Aedes vector strains, widespread in the Jaffna peninsula, may have contributed to differences in serotype prevalence compared with the Colombo district in 2019. Significant associations were not identified between virus serotypes and clinical characteristics among Jaffna patients.

El dengue es una enfermedad viral aguda transmitida a través de la picadura del mosquito (artrópodo) Aedes aegypti infectado con cualquiera de los serotipos de virus dengue. Son virus envueltos, de 40 a 50 nm de diámetro, con cápside icosaédrica y genoma de ácido ribonucleico (ARN) monocatenario, no segmentado, de polaridad positiva perteneciente al género flavivirus de la familia Flaviviridae. El análisis filogenético de las distintas cepas del virus dengue indica que la diseminación global ha dado lugar a distintos genotipos dentro de cada serotipo. En el quinquenio 2015-2020, en Ecuador, se notificaron en toda la región 76.085 casos de dengue. La atención unitaria hospitalaria para el tratamiento del dengue de las unidades de atención sanitaria hospitalarias (A, B, C), generaron un costo total promedio de 498,04 $ con una desviación estándar de ± 40,36, en el Ecuador para el año 2020. Cabe considerar, por otra parte, que el costo unitario directo promedio en ($) de la atención sanitaria en las entidades hospitalarias estudiadas en la presente investigación, generanron un gasto total para el tratamiento hospitalario de 9.585.114,02. Al mismo tiempo en los 2.135 pacientes diagnosticados con dengue que presentaron signos de alarma y requirieron cuatro días de hospitalización, señalando un gasto total para el tratamiento de 1.471.385,06. Por su parte los 51 pacientes que fueron diagnosticados con dengue grave, generaron un costo unitario total en las unidades hospitalarias de 134.152,54. Los resultados son conservadores, porque algunos componentes importantes no se incluyeron en los costos relacionados con el dengue.En ausencia de vacunación, y siendo los programas de control del vector la estrategia básica para mitigar la propagación del dengue, esta enfermedad seguirá produciendo una carga económica y social considerable en el Ecuador, lo que se refleja en el costo total de la enfermedad(AU)


Dengue is an acute viral disease transmitted through the bite of the mosquito (arthropod) Aedes aegypti infected with any of the dengue virus serotypes. They are enveloped viruses, 40 to 50 nm in diameter, with an icosahedral capsid and a single-stranded, non-segmented ribonucleic acid (RNA) genome of positive polarity belonging to the genus Flavivirus of the Flaviviridae family. Phylogenetic analysis of the different dengue virus strains indicates that global spread has given rise to different genotypes within each serotype. In the five-year period 2015-2020, in Ecuador, 76,085 cases of dengue were reported throughout the region. The hospital unit care for the treatment of dengue in the hospital health care units (A, B, C), generated an average total cost of $ 498.04 with a standard deviation of ± 40.36, in Ecuador for the year 2020. It should be considered, on the other hand, that the average direct unit cost in ($) of health care in the hospital entities studied in the present investigation, generated a total cost for hospital treatment of 9,585,114.02. At the same time, in the 2,135 patients diagnosed with dengue who presented alarm signs and required four days of hospitalization, indicating a total cost for treatment of 1,471,385.06. On the other hand, the 51 patients who were diagnosed with severe dengue, generated a total unit cost in the hospital units of 134,152.54. The results are conservative, because some important components were not included in the costs related to dengue. In the absence of vaccination, and with vector control programs the basic strategy to mitigate the spread of dengue, this disease will continue to produce an economic burden and considerable social in Ecuador, which is reflected in the total cost of the disease(AU)

Variation and clade shifts in dengue virus (DENV) genotypes are responsible for numerous dengue fever outbreaks throughout Latin America in the past decade. Molecular analyses of dengue serotypes have revealed extensive genetic diversification and the emergence of new genotypes in Brazil (DENV-4 genotype I) and elsewhere in tropical and subtropical America. The goal of the present study is to assess the extent to which the adventitious introduction of DENV genotypes and their increasing genetic diversity affects dengue epidemiology in Mexico. A nuanced sequence inspection and phylogenetic analysis of the C-prM nucleotide region of DENV was performed for specimens collecting in 2009 from the Veracruz State, Mexico. Findings were contrasted with specimens collected in adjacent years and analysed based on the epidemiological patterns reported between 1990 and 2019. Additionally, the identification process of various DENV genotypes was assessed, including: (1) DENV-1, genotype V, (2) the DENV-2 Asian/American and Asian II genotypes (3) DENV-3, genotype III, and (4) DENV-4, genotype I. This resulted in the discovery of a distinct genetic cladistic pattern for serotype DENV-2. Lastly, study findings suggest that a correlation exists between the emergence of novel genotypes and genetic diversification, with the increasing incidence of DENV infections in Mexico in 2009.

RESUMEN Fundamento: el dengue es una enfermedad infecciosa muy extendida en varias regiones del planeta. Objetivo: caracterizar el comportamiento de algunos parámetros clínicos y de laboratorio de pacientes con sospecha de dengue. Métodos: se realizó un estudio analítico longitudinal prospectivo en el Hospital Clínico Quirúrgico Docente Amalia Simoni desde de junio de 2016 a junio de 2017. El universo estuvo compuesto por 151 pacientes de los cuales se seleccionó una muestra de forma probabilística aleatoria simple compuesta por 90 pacientes. Los datos se recogieron en un formulario y se procesaron en una base de datos creada con SPSS v23. Resultados: los resultados agrupados en forma de tablas, mostraron predominio de los pacientes del sexo masculino, los síntomas iniciales más frecuentes estuvieron dados por fiebre asociada a cefalea y artralgia. Conclusiones: entre los síntomas iniciales encontrados predominaron los pacientes que no presentaron signos de alarma de dengue, la aparición de estos signos se asoció de forma significativa a leucocitosis, hematocrito elevado y trombocitopenia.


ABSTRACT Background: dengue is an infectious disease widespread in several regions of the planet. Objective: to characterize the behavior of some clinical and laboratory parameters of patients with suspected dengue. Methods: a prospective longitudinal analytical study was carried out at the Amalia Simoni Teaching Surgical Clinical Hospital in the period from June 2016 to June 2017. The universe consisted of 151 patients from which a sample of simple probabilistic form composed of 90 patients was selected randomly. The data was collected in a form and processed in a database created with SPSS v23. Results: the results grouped in the form of tables, showed predominance of male patients, the most frequent initial symptoms were due to fever associated with headache and arthralgia. Conclusions: among the most frequent initial symptoms, patients with no signs of dengue alarm predominated, the appearance of these signs was significantly associated with leukocytosis, elevated hematocrit and thrombocytopenia.

ABSTRACT

Dengue fever, the most notorious of vector borne diseases is under global resurgence. Incidence has increased 30 fold with global expansion. It is thus imperative to review the origin, history and current epidemiology of dengue, its transmission, factors responsible for resurgence, surveillance and the treatment options available. India being hyperendemic, national level comprehensive studies to estimate the true burden of dengue along with its geographical mapping is the need of the hour. Through integrated and combined efforts from various sectors and policy makers, prevention of dengue must be identified and intensified to control further disease transmission as there is no specific antiviral treatment or vaccine against dengue is available in India.

ABSTRACT

Dengue virus (DENV), an arbovirus transmitted by mosquitoes, has become a major threat to American human life, reaching approximately 23 million cases from 1980 to 2017. Brazil is among the countries most affected by this terrible viral disease, with 13.6 million cases. DENV has four different serotypes, DENV1-4, which show a broad clinical spectrum. Dengue creates a staggering epidemiological and economic burden for endemic countries. Without a specific therapy and with a commercial vaccine that presents some problems relative to its full effectiveness, initiatives to improve vector control strategies, early disease diagnostics and the development of vaccines and antiviral drugs are priorities. In this study, we present the probable origins of dengue in America and the trajectories of its spread. Overall, dengue diagnostics are costly, making the monitoring of dengue epidemiology more difficult and affecting physicians' therapeutic decisions regarding dengue patients, especially in developing countries. This review also highlights some recent and important findings regarding dengue in Brazil and the Americas. We also summarize the existing DENV polymerase chain reaction (PCR) diagnostic tests to provide an improved reference since these tests are useful and accurate at discriminating DENV from other flaviviruses that co-circulate in the Americas. Additionally, these DENV PCR assays ensure virus serotyping, enabling epidemiologic monitoring.

ABSTRACT

Using the dengue surveillance program, we prospectively collected data on all the suspected and confirmed cases of dengue in Barbados from 2006 to 2015. Data were analysed for demographic, seasonal and temporal dynamics of this disease in this country. The overall mean annual incidence rate of suspected and confirmed dengue over the study period was 0.49% (range 0.15%-0.99%) and 0.16% (range 0.05%-0.48%), respectively. There was a significant correlation between the mean monthly number of confirmed cases, the mean monthly rainfall and the mean monthly relative humidity percentage. Dengue in this population is predominantly an infection affecting children and young adults. The median age of the patients with both, suspected and confirmed dengue was 25 years and the highest proportion of cases was seen in the age group 0-15 years. The annual incidence rates of both the suspected and the confirmed cases showed an upward trend during the study period and this upward trend was more pronounced among children.

ABSTRACT OBJECTIVE To describe the epidemiology of dengue in a medium-sized city in the state of São Paulo. METHODS Data, such as circulating serotypes, severe cases and deaths, age group, sex, among others, were obtained on reported and confirmed dengue cases in Araraquara, state of São Paulo, between 1991 and 2015. Climatic and infestation data were also analyzed. These variables were evaluated descriptively, using statistical measures such as frequencies, averages, minimum and maximum. Dengue incidence rates were calculated according to month, year, age and sex, and time series of dengue cases, infestation, and climatic variables. RESULTS Approximately 16,500 cases of dengue fever were reported between 1991 and 2015. The highest number of reports was recorded in 2015 (7,811 cases). In general, the age group with the highest number of reports is between 20 and 59 years old. The highest incidences, generally between March and May, occurred after the increase in rainfall and infestation in January. CONCLUSIONS Increased levels of infestation due to rainfall are reflected in incidence rates of the disease. It is fundamental to know the epidemiology of dengue in medium-sized cities. Such information can be extended to diseases such as Zika and Chikungunya, which are transmitted by the same vector and were reported in the city. The intensification of surveillance efforts in periods before epidemics could be a strategy to be considered to control the viral spread.


RESUMO OBJETIVO Descrever a epidemiologia da dengue em cidade de médio porte do estado de São Paulo. MÉTODOS Foram obtidas informações sobre os casos notificados e confirmados de dengue em Araraquara, SP, entre 1991 e 2015, como sorotipos circulantes, casos graves e óbitos, faixa etária, sexo, entre outras. Também foram levantadas as informações climáticas e de infestação pelo vetor. Essas variáveis foram trabalhadas descritivamente, utilizando-se medidas estatísticas como frequências, médias, mínimo e máximo. Calcularam-se taxas de incidência de dengue segundo mês, ano, idade e sexo, e série histórica de casos de dengue, infestação e variáveis climáticas. RESULTADOS Foram confirmados 16.431 casos de dengue entre 1991 e 2015. O maior número de notificações foi registrado em 2015 (7.811 casos). De forma geral, a faixa etária com o maior número de notificações encontra-se entre 20 e 59 anos. As maiores incidências, geralmente entre março e maio, ocorreram após o aumento da pluviosidade e infestação, em janeiro. CONCLUSÕES Os altos níveis de infestação que aumentam com as chuvas refletem-se nas taxas de incidência da doença. É fundamental conhecer a epidemiologia da dengue em cidades de médio porte. Seus resultados podem ser estendidos para doenças como Zika e Chikungunya (transmitidas pelo mesmo vetor e notificadas na cidade). A intensificação dos esforços de vigilâncias em períodos que precedem as epidemias poderia ser uma estratégia a ser considerada para o controle da dispersão viral.

INTRODUCTION: Dengue is a serious public health problem in Colombia; it is prevalent in 90% of the municipalities in Quindío. Studies on its seroprevalence are required to address public health interventions. OBJECTIVE: To establish the seroprevalence of dengue infection in neighborhoods with high incidence in the municipalities of Armenia, Calarcá, La Tebaida and Montenegro, Quindío, in 2014. MATERIALS AND METHODS: We conducted a probabilistic, stratified, two-stage prevalence study. We interviewed 658 residents in the urban area of the selected municipalities. After they signed the informed consent, we took a blood sample to determine dengue IgG and IgM antibodies. RESULTS: Seroprevalence of IgG in Quindío was 89,4%; in Armenia it was 88,7%, in Calarcá, 81,5%, in Montenegro, 91,8% and in La Tebaida 97,8%. IgM was 14, 2% in Quindío; in Armenia it was 11,5%, in Calarcá, 13,0%, in Montenegro, 13,1% and in La Tebaida, 28,9%. CONCLUSIONS: We found a high prevalence of both IgG and IgM in the four municipalities. We had positive results for IgM in all age groups, which suggests recent infection. We also found simultaneous seropositivity for IgG and IgM (12.9%), which may indicate infection by another serotype or presence of infection in the past three months. A multisectoral approach is necessary for dengue control in Quindío.

ABSTRACT

Dengue is an arboviral disease that is a serious public health concern, especially due to its increasing incidence in the past decades. This study aimed to characterize the epidemiological incidence of dengue in 2008­2012 in Southern Mato Grosso, Brazil. This is an epidemiological and descriptive study of dengue in the 19 municipalities of the Rondonópolis Regional Health Office using data obtained from the Information System of Diseases Notification. An analysis of the number of confirmed cases of dengue, according to year, age, and gender was performed. Data was associated with the rainfall of the region as well. There were 13,580 reported dengue cases; 2010 had the highest number of cases, with 7,244 dengue cases. In 2011, the number fell dramatically to only 236 notified cases. With regard to age, the most often affected were young adults (20-39 years of age); no difference was noted regarding gender. As expected, dengue cases were higher during the rainy season. In 2009 and 2010, in Southern Mato Grosso, there was a significant increase in the proportion of dengue cases compared with other years during the period evaluated. However, a rapid decline in the subsequent year was noted, possibly because of the intensification of preventive actions in vector control. Interestingly, 2012 presented an increasing number of dengue cases. This fact highlights the importance of actively combating dengue in order to prevent future epidemics.

ABSTRACT

BACKGROUND: The 2009 revised World Health Organization (WHO) guidelines for dengue describe fever as the core symptom. Accordingly, the diagnosis of non-febrile patients is complicated. The aim of this study was to evaluate the importance of fever in patients with dengue according to the 2009 revised WHO classification. METHODS: In this study, we assessed 30,670 dengue cases using enzyme-linked immunosorbent assay, detection of the non-structural protein 1, or polymerase chain reaction for diagnostic confirmation. Fisher's exact test was used to evaluate associations between fever and related clinical manifestations. The Mann-Whitney U test was used to assess the association of dengue classification with fever and time to treatment. The effects of fever and time to treatment on the risk of progression were analyzed using an ordinal logistic regression to stereotype the model. RESULTS: Disease classification was found to associate significantly with both fever and time to treatment (both P < 0.001). Non-febrile patients were nearly four-fold more likely to exhibit "dengue without warning signs" than "severe dengue" (odds ratio [OR] = 3.74; 95% confidence interval [CI]: 3.20-4.36). Patients who received treatment within 7 days were twice as likely to have "dengue without warning signs" as opposed to "severe dengue" when compared to those who waited >7 days (OR = 2.23; 95% CI: 1.78-2.80). However, this difference was negligible in the multivariate analysis (OR = 1.02; 95% CI: 0.98-1.07). CONCLUSIONS: Fever is a risk factor for disease progression in patients with dengue. However, non-febrile patients should not be neglected because this may delay treatment and could lead to more severe disease.

Resumen Introducción: El dengue representa un grave problema de salud pública para Colombia y, en el departamento del Quindío, afecta el 90 % de los municipios. Se necesitan estudios actualizados sobre la seroprevalencia en la población general para reforzar las acciones de salud pública. Objetivo: Determinar la seroprevalencia de la infección por dengue en barrios con alta incidencia de dengue en cuatro municipios del departamento del Quindío: Armenia, Calarcá, La Tebaida y Montenegro, en 2014. Materiales y métodos: Se hizo un estudio de prevalencia mediante muestreo probabilístico estratificado y bietápico. Se hizo una encuesta a 658 sujetos residentes del área urbana de los municipios seleccionados y se les tomó una muestra de sangre por venopunción para determinar anticuerpos IgG e IgM contra el virus del dengue. Resultados: La seroprevalencia de anticuerpos IgG en el Quindío fue de 89,4 %; en Armenia fue de 88,7 %, en Calarcá, de 81,5 %, en Montenegro, de 91,8 %, y en La Tebaida, de 97,8 %. La seroprevalencia de anticuerpos IgM en Quindío fue de 14,2 %; en Armenia, de 11,5 %, en Calarcá, de 13,0 %, en Montenegro, de 13,1%, y en La Tebaida, de 28,9 %. Conclusiones: Se evidenció una alta prevalencia de anticuerpos IgG e IgM en los cuatro municipios. En todos los grupos de edad se encontraron personas seropositivas para IgM, lo cual indicaría infección reciente. La seropositividad simultánea para IgM e IgG (12,9 %) puede indicar infección secundaria por otro serotipo del virus o una infección en los tres meses anteriores. Es necesario impulsar estrategias multisectoriales para el control de la transmisión del dengue en el Quindío.


Abstract Introduction: Dengue is a serious public health problem in Colombia; it is prevalent in 90% of the municipalities in Quindío. ABSTRACT OBJECTIVE Analyze the spatial distribution of classical dengue and severe dengue cases in the city of Rio de Janeiro. METHODS Exploratory study, considering cases of classical dengue and severe dengue with laboratory confirmation of the infection in the city of Rio de Janeiro during the years 2011/2012. The georeferencing technique was applied for the cases notified in the Notification Increase Information System in the period of 2011 and 2012. For this process, the fields "street" and "number" were used. The ArcGis10 program's Geocoding tool's automatic process was performed. The spatial analysis was done through the kernel density estimator. RESULTS Kernel density pointed out hotspots for classic dengue that did not coincide geographically with severe dengue and were in or near favelas. The kernel ratio did not show a notable change in the spatial distribution pattern observed in the kernel density analysis. The georeferencing process showed a loss of 41% of classic dengue registries and 17% of severe dengue registries due to the address in the Notification Increase Information System form. CONCLUSIONS The hotspots near the favelas suggest that the social vulnerability of these localities can be an influencing factor for the occurrence of this aggravation since there is a deficiency of the supply and access to essential goods and services for the population. To reduce this vulnerability, interventions must be related to macroeconomic policies.


RESUMO OBJETIVO Analisar a distribuição espacial dos casos de dengue clássico e dengue grave no município do Rio de Janeiro. MÉTODOS Estudo exploratório, considerando casos de dengue clássico e de dengue grave com comprovação laboratorial da infecção, ocorridos no município do Rio de Janeiro nos anos de 2011/2012. Foi aplicada a técnica de georreferenciamento dos casos notificados no Sistema de Informação de Agravos de Notificação, no período de 2011 e 2012. Para esse processo, utilizaram-se os campos "logradouro" e "número". Foi realizado o processo automático da ferramenta Geocoding do programa ArcGis10. A análise espacial foi feita a partir do estimador de densidade Kernel. RESULTADOS A densidade de Kernel apontou áreas quentes para dengue clássico não coincidente geograficamente a dengue grave, estando localizadas dentro ou próximas de favelas. O cálculo da razão de Kernel não apresentou modificação significativa no padrão de distribuição espacial observados na análise da densidade de Kernel. O processo de georreferenciamento mostrou perda de 41% dos registros de dengue clássico e 17% de dengue grave devido ao endereçamento da ficha do Sistema de Informação de Agravos de Notificação. CONCLUSÕES As áreas quentes próximas às favelas sugerem que a vulnerabilidade social existente nessas localidades pode ser um fator de influência para a ocorrência desse agravo, uma vez que há deficiência da oferta e acesso a bens e serviços essenciais para a população. Para diminuir essa vulnerabilidade, as intervenções devem estar relacionadas a políticas macroeconômicas.

INTRODUCTION: Little is known about the prevalence and clinical course of dengue infection in elderly patients living in endemic areas; it is presumed that there is a lower prevalence but higher severity, complications and mortality.  OBJECTIVE: To describe the prevalence and clinical course of dengue infection in elderly patients who were admitted to a referral care center for infectious diseases in an endemic region.  MATERIALS AND METHODS: We conducted an observational and descriptive study between 2011 and 2014, using a cohort of elderly patients with serological diagnosis of dengue.  RESULTS: A total of 235 febrile elderly patients were assessed, of which 43 patients (18.3%) were found to have dengue. The median age was 71 years; 48.7% were female, and 89% of patients had at least one comorbid condition. According to the serological tests, 51.4% of cases were positive for NS1 Ag, 27% for IgM and 54.1% for IgG, while 64.8% were secondary infections. Dengue was diagnosed in 13 patients (35%), dengue with warning signs in 16 cases (43%), and severe dengue in 8 cases (22%). Nearly 56.7% of patients were admitted to hospital and 21.6%, to the intensive care unit. None died.  CONCLUSION: We found dengue infection to be more frequent than expected in this sample of elderly patients, due to acute febrile syndrome. Elderly patients also required higher rate of hospitalization and had more complications, however there were no deaths due to good management.

INTRODUCTION: Dengue has a seasonal behavior associated with climatic changes, vector cycles, circulating serotypes, and population dynamics. The wavelet analysis makes it possible to separate a very long time series into calendar time and periods. This is the first time this technique is used in an exploratory manner to model the behavior of dengue in Colombia.  OBJECTIVE: To explore the annual seasonal dengue patterns in Colombia and in its five most endemic municipalities for the period 2007 to 2012, and for roughly annual cycles between 1978 and 2013 at the national level.  MATERIALS AND METHODS: We made an exploratory wavelet analysis using data from all incident cases of dengue per epidemiological week for the period 2007 to 2012, and per year for 1978 to 2013. We used a first-order autoregressive model as the null hypothesis.  RESULTS: The effect of the 2010 epidemic was evident in both the national time series and the series for the five municipalities. Differences in interannual seasonal patterns were observed among municipalities. In addition, we identified roughly annual cycles of 2 to 5 years since 2004 at a national level.  CONCLUSIONS: Wavelet analysis is useful to study a long time series containing changing seasonal patterns, as is the case of dengue in Colombia, and to identify differences among regions. These patterns need to be explored at smaller aggregate levels, and their relationships with different predictive variables need to be investigated.

INTRODUCTION: Dengue is the fastest spreading disease in the world and a permanent threat to global public health. It is a viral illness for which approximately 2.5 million people are at high risk of infection. Given the severity of the disease at national and global levels, new predictive methodologies need to be generated to facilitate decision-making in public health.  OBJECTIVE: To characterize cases of dengue reported from 2009 to 2013 in Valle del Cauca department, Colombia, and to establish a methodology to develop endemic channels that can be applied to this event.  MATERIALS AND METHODS: This was a retrospective descriptive study. Notification forms were used as a secondary database to characterize dengue cases from 2009 to 2013. Two endemic channels were developed, one using running means and the other through exponential smoothing.  RESULTS: Dengue in the department of Valle del Cauca showed a positive tendency, indicating that the number of cases had increased in the last five years. An important variation was observed that could be explained by a three-year cycle beginning in the first epidemiological period of the year.  CONCLUSION: The development of the dengue endemic channel for Valle del Cauca illustrates the importance of applying these monitoring methodologies to events of public health interest. As can be seen from the results, there were some years in which the number of cases was very low and others in which the epidemic reached very high levels.

Introducción. El dengue es una enfermedad infecciosa sistémica, viral transmitida por el mosquito “Aedes”. La OMS calcula que cada año se producen entre 50 y 100 millones de infecciones por este virus en el mundo. En México en el transcurso de la última década ha habido un incremento de casos, especialmente de tipo hemorrágico.Métodos. Se realizó una búsqueda sistemática de artículos en cuatro etapas, retrospectiva a 10 años relacionada a dengue grave en PubMed, BVS, Tripdatabase y sitios Web; obteniendo 149 resultados, siendo útiles 40 para el desarrollo de esta guía: revisiones sistemáticas, meta análisis, ensayos clínicos aleatorizados, estudios observacionales y Guías de Práctica Clínica.Resultados. Revisiones sistemáticas mencionan que la desnutrición grado 2 o 3 proteico calórica severa protege contra la vasculopatía del dengue y que la fase crítica comienza alrededor del período de desaparición de fiebre, la leucopenia avanza y hay una rápida disminución de plaquetas, esto precede a la manifestación más específica y potencialmente mortal de esta fase: incrementa la permeabilidad capilar conduciendo a la pérdida de plasma y aumento en él hematocrito; un estudio transversal prospectivo encontró que infecciones sucesivas del virus del dengue aumentan el riesgo de dengue grave y un estudio multicéntrico refiere que las fallas orgánicas más desarrolladas fueron respiratorias, cardiovasculares, neurológicas y hepáticas.Conclusiones. La implementación de las recomendaciones están dirigidas principalmente a las intervenciones de enfermería que contribuyen a la prevención, recuperación, disminución de complicaciones y limitación de daños para el paciente con dengue grave.

In Mexico over the course of the last decade, there has been an increase in cases, especially of hemorrhagic type. Methods. A systematic search of articles was carried out in four stages, back at 10 years related to severe dengue in PubMed, VHL, Tripdatabase and Web sites; 149 results, being useful 40 for the development of this guide: systematic reviews, meta analysis, clinical trials randomized, observational studies and clinical practice guidelines. Results. Systematic reviews mentioned malnutrition grade 2 or severe caloric protein 3 protects against vascular disease of dengue and the critical phase begins around the time of the disappearance of fever, leukopenia advances and there is a rapid decrease in platelets, this precedes the manifestation more specific and potentially deadly this phase: increases capillary permeability, leading to the loss of plasma and increase the hematocrit; a prospective cross-sectional study found that successive infections of the dengue virus increases the risk of serious dengue and a multicenter study concerns that more developed organic failures were respiratory, cardiovascular, neurological and hepatic.Conclusions. The implementation of the recommendations are directed mainly to the nursing interventions that contribute to the prevention, recovery, reduction of complications and limitation of patient with severe dengue.
