ABSTRACT

Eccrine hidrocystomas (EHs) are benign tumors, which arise as cystic dilatation of the eccrine sweat duct. The lesions of EH have a chronic course with periodic flares in summer months, associated with exacerbation in sweating. Diagnosis is mainly clinical with histopathology being confirmatory. Dermoscopy is a noninvasive tool, which may confirm diagnosis of EH without subjecting the patient to a biopsy. We report two representative cases of EH, with emphasis on dermoscopic features and which well responded to topical botulinum toxin-like peptide.

ABSTRACT

Eccrine hidrocystomas are benign, cystic tumors that are most commonly found on the central face in middle-aged females. Their dermoscopic findings are rarely described in the literature, with only seven cases currently reported to date. We present the case of an elderly man with an unusual location of an eccrine hidrocystoma of the central chest and its associated dermoscopic findings. Characterizing the dermoscopic features of eccrine hidrocystomas may allow for better differentiation of these lesions from cutaneous malignancies and may minimize unnecessary biopsies, treatment, and scarring.

ABSTRACT

Hidrocystoma is a cystic tumor originating from a dilated sweat gland. The tumor is typically small and located predominantly on the face, particularly in the eyelid margins. They are two classical types of hidrocystomas i.e., Apocrine and Eccrine. Apocrine hidrocystomas are cystic lesions that arise from the apocrine secretory coil and Eccrine hidrocystomas represent retention cysts of the eccrine duct. Multiple apocrine hidrocystomas confined to the eyelids have been recognized as a feature of a new variant of ectodermal dysplasia. Here we present a rare case of Apocrine hidrocystoma of supraorbital area for which excisional biopsy of the lesion was performed. Histopathological evaluation proved the lesion was a unilocular cyst lined by a double-layered epithelium of the apocrine type. The purpose of this paper is to report the unique occurrence of a Supra orbital apocrine hidrocystoma which was clinically as a sebaceous cyst and to discuss its differential diagnosis with other more common supraorbital and face lesions.

ABSTRACT

Eccrine hidrocystomes (EH) are benign cystic tumors of dermal eccrine ducts. There is still no gold standard treatment option for EH because of the large number of lesions, the risk of cicatrization and the different treatment options offered. We offer combined oral isotretinoin and following erbium-yttrium aluminum garnet laser treatment as an alternative treatment option in difficult patients with EH.

ABSTRACT

BACKGROUND: Multiple eccrine hidrocystomas (MEH) are benign cystic lesions of the sweat gland ducts. They are characterized by translucent, skin-colored or bluish dome-shaped papules on the face, causing cosmetic concern. Recently, botulinum toxin-A, because of its antiperspirant properties, has been used to treat facial MEH. However, there are only some case reports in the literature. OBJECTIVE: Here, we conducted a prospective study to assess the efficacy and safety of intralesional injection of botulinum toxin-A (Dysport) for the treatment of MEH. MATERIAL AND METHODS: Twenty patients (3 men and 17 women), aged from 31 to 75 years old, participated in this study. A 300 unit vial of botulinum toxin-A (Dysport, Ipsenn Biopharm, United Kingdom) was diluted with 4 ml of saline solution without preservative to achieve a concentration of 7.5U/.1 ml. Up to 1.5 unit of botulinum toxin was injected intradermally at the base of each lesion to raise a visible wheal. The patients were evaluated 7 days later, and any clinical changes or adverse effects were recorded. RESULTS: In all patients, more than 75% of eccrine hidrocystoma lesions resolved without any scaring. The result sustained for 2-5 months. In two patients mild smile asymmetry and in one patient lagophthalmos were noted approximately 5-7 days after injection that gradually resolved in 3 weeks. CONCLUSION: Intralesional botulinum toxin-A for treatment of multiple hidrocystomas is a simple and well-tolerated procedure. It is accompanied by excellent results, a good postoperative course, and no risk of scarring. It can be considered in patients who did not respond to other treatment or even as the first line therapy.

ABSTRACT

Multiple eccrine hidrocystoma (EH) has been treated with topical atropine with variable results. However, in rare cases, anticholinergic side effects have been seen after the use of the topical form of this drug. We presented a 50-year-old woman who developed recent onset of visual disturbance and photophobia from 2 weeks prior. The diagnosis of topical atropine-induced bilateral mydriasis was made. We reported a recognized but often overlooked case of bilateral mydriasis caused by application of topical 1% atropine for treatment of multiple EH.

ABSTRACT

Eccrine hidrocystomas are benign tumors of the sweat glands that arise from cystic dilatation of the excretory sweat duct. Exact diagnosis requires biopsy and histopathologic examination, from which arises risk of scarring. Dermoscopy could be a useful tool in diagnosing eccrine hidrocystomas. Herein we aim to present dermoscopic features in three female patients with multiple lesions on the face consistent with eccrine hidrocystomas.

ABSTRACT

A 2-year-old boy with an upper-eyelid tumor in his right eye underwent complete resection. Pathological examination showed that the tumor was composed of a thin wall, lined by a single cuboidal epithelium, and contained a transparent liquid. The epithelium of the tumor wall did not display decapitation and the tumor was diagnosed as eccrine hidrocystoma. After resection, obvious improvement of the tumor-caused blepharoptosis was observed and the visual acuity of the patient recovered.

A 45-year-old female patient known case of sarcoidosis presented in skin department with chief complaint of skin-colored papulonodular lesions over face with exacerbation in summer season since last 4 years. On examination lesions were multiple, firm and skin colored. Biopsy was done showing features of eccrine hidrocystoma. Patient was treated with electrocautery and showed improvement.

ABSTRACT

Eccrine hidrocystomas are benign cystic dilatations with eccrine differentiation. The clinical presentation usually occurs as a solitary tense vesicle or translucent papule located predominantly on the face, whereas multiple eccrine hidrocystomas are unusual. Histologically, eccrine hidrocystoma consists of one or more partially collapsed unilocular cysts in the dermis, which show flattened epithelium that is partially lined by two layer of cuboidal epithelium. We report here on a case of the patient with multiple eccrine hidrocystomas that showed an especially generalized presentation that included the face, neck, anterior chest and both arms.

ABSTRACT

We report a case of intratumoral fat in neurofibroma and coexistance of eccrine hidrocystoma on the occipital area of the scalp for one year duration. There are several diseases showing fatty change histopathologically. Among them, few cases of neurofibroma showing fatty change had been reported. The mechanism of fatty change is unclear, but several hypotheses are proposed. Here we report a case of neurofibroma showing fatty change and coexistence of eccrine hidrocystoma, and discuss the pathomechanism of fatty change and its relationship with disease.

ABSTRACT

BACKGROUND: Eccrine hidrocystoma (EH) represents benign cystic dilatations of previously normal dermal eccrine ducts. It is characterized by tense vesicles or translucent papules most often involving the face, predominantly in a periorbital distribution. There were no data on the clinical and histologic features of EH in Koreans. OBJECTIVE: The purpose of this study is to investigate clinical and histopathological features of EH in Koreans. METHOD: We reviewed the medical records and biopsy slides of 19 cases diagnosed previously as EH in 3 hospitals of Catholic Medical Center from August 1991 to May 2003. RESULT: We could compile clinical data and histologic informations on 15 of the total 19 patients with EH. 1. The mean age at diagnosis was 51.6 years, 54.0 years in females and 32.5 years in males. EH developed predominantly in females as the sex ratio of 1: 6.5. 2. Most patients were asymptomatic, but 4 and I patients complained of mild itching and one of a mild burning sensation. The predilection sites were the lower eyelids, nose and lateral canthus. Only one patient had a concomitant Graves' disease. 3. The ratio of Robinson to Smith types was 14: 1. The Robinson type had multiple, pinhead-sized, translucent skin-colored lesions, whereas one smith type had two, pea-sized, dark brownish lesions. The former only had seasonal variation, aggravated in warm, humid weather and improved by a cold environment. 4. Histopathologic examination revealed all cases had a single cystic cavity located in the dermis. In 12 of 15 patients, there were eccrine secretory tubules and ducts in close approximation to a cystic cavity, but neither papillary projections of cyst wall nor connections between the epidermis and the cystic cavity could be found in all cases. 5. Various treatments, such as CO2 laser, atropine ointment, cryotherapy or punch excision were tried, but all failed. CONCLUSION: EHs in this study appeared on the face, especially on the lower eyelids of middle-aged women. In contrast to the western literatures in which most EHs are presented as a solitary lesion, Robinson type is much more common in Koreans. There have been no effective treatments for EHs.

ABSTRACT

Eccrine hidrocystoma is a cyst origined from the secretory portion of the eccrine sweat gland and tends to occur as a solitary facial lesion. Clinically, an eccrine hidrocystoma presents as a small, translucent and cystic swelling(often having a bluish hue) which measures 1 to 3 mm in diameter. In some patients, the mass is appeared with numerous lesions. The mass has the tendency that the number of cyst increases in warm weather and decreases during winter. The histologic finding is an intradermal cyst with a thin wall composed of two layers of eosinophilic- staining cuboidal cells. Eccrine hidrocystoma has not yet reported in the journal of korean society of plastic and reconstructive surgeons. Thus, we report a case of 65-years-old female patient with eccrine hidrocystoma, which presents 2.1 x 1.6 x 1.6 cm in size and bluish hue color, occurring on the nasal tip.

ABSTRACT

We report a case of multiple eccrine hidrocystoma (MEH) in a 57-year-old woman who had asymptomatic, discrete, skin-colored, shiny, firm papulovesicles on the face. These lesions enlarged during summer or when the patient did housework, and decreased in sue during winter or rest. Histologic examination showed a dilated, unilocular, cystic, invaginated structure within the middle dermis. The wall of the cyst generally consisted of two layers of Rat or cuboida1 epithelial cells. Decapitation secretion and myoepithelial cells were not observed. The lesions improved markedly with 1% topical atropine sulfate.

ABSTRACT

Eccrine hidrocystoma is a benign eccrine duct-derived cyst which may be solitary or multiple. We report two cases of multiple eccrine hidrocystoma, which have two points of sameness in middle aged women and worsening of the condition in summer and improvement in winter. Recently, the incidence of multiple eccrine hidrocystoma seems to be decreasing due to well-developed cooling devices in summer. In spite of its decreasing incidence, we think that it is still noteworthy to keep in mind the possibility of eccrine hidrocystoma when a patient presented with multiple facial papules in hot season.
