ABSTRACT

The eruptive deviation of the permanent maxillary canine must be identified early, at the beginning of the second transitional period of the mixed dentition. Acting at this moment is fundamental for its spontaneous redirection and eruption. The aim of the paper is to present a case where the extraction of the deciduous canines was the choice for redirecting the maxillary permanent canines with eruptive palatal deviation, as well as to suggest a sequence of procedures for diagnosis and choice of treatment. The success of deciduous canine extraction in redirecting the ectopic permanent canines is discussed in this case, as well as aspects that might lead to failure. Still, it highlights the importance of computed tomography for choosing conservative treatment or traction, including it in the sequential guideline for acting in these cases.

ABSTRACT

INTRODUCTION: The management of eruption disturbances in orthodontics may be challenging and requires a careful diagnosis and treatment planning. This case report discusses the challenges of a two-phase orthodontic treatment of a patient presenting with a dental eruption pattern anomaly. PATIENT CONCERNS: A 10-year-old boy was presented with no complaints for a routine orthodontic evaluation during mixed dentition. PRIMARY DIAGNOSES: The patient was diagnosed with a skeletal Class I malocclusion with unilateral posterior crossbite, incomplete mandibular lateral incisor-canine transposition and a unilateral maxillary ectopic canine. INTERVENTIONS: Phase 1 started with rapid maxillary expansion to correct maxillary constriction and the ectopic eruption of the right maxillary canine. In the mandibular arch, phase 1 included the extraction of the left primary lateral incisor and canine, alignment of the left permanent lateral incisor and orthodontic traction of the left permanent canine. The duration of phase 1 was 14 months. Phase 2 involved a comprehensive course of orthodontic treatment and started when the patient was aged 13 years. This phase lasted 18 months. RESULTS: An adequate dental occlusion was obtained, and the treatment results were stable after an 18-month follow-up. CONCLUSION: In this case, the early diagnosis of the dental anomalies was valuable as it allowed an early intervention to be undertaken, which resulted in overall treatment simplification and potentially minimised the adverse effects. This case report reinforces the importance of a careful follow-up during mixed dentition.

Objectives: An ectopic tooth is a rare eruption of a tooth out of the normal dental apparatus and occurs commonly with the third molar. Thus, in this study, we reported a case series of ectopic teeth in rare jaw locations and highlight the associated pathology and our experience in the surgical management. Patients and. Methods: All cases of ectopic tooth managed at the Department of Oral and Maxillofacial Surgery, University of Maiduguri Teaching Hospital from January 2011 to December 2020 were reviewed. The information retrieved includes biodata, location of the ectopic tooth, signs, symptoms, type of tooth and associated pathology, surgical approach and complications. Results: Ten cases of ectopic teeth were identified over the study period. This comprised 80.0% males with a mean age was 23.3 years. The antrum and lower border of the mandible accounted for 50.0% and 40.0% of the ectopic locations, respectively. Dentigerous cyst was the most associated pathology (70%) and usually presented with pain and swelling. Surgical intervention predominantly via the intraoral route was performed if indicated. Conclusion: Ectopic teeth are rare and not always associated with pathology. A high index of suspicion and radiological investigation are necessary for diagnosis. A more extensive multi-center study is however recommended to determine the prevalence of ectopic teeth other than the third molar.

ABSTRACT Permanent canines are essential for the preservation of the stomatognathic system, as they are part of the occlusal and aesthetic function and balance. During the normal development of occlusion, canines follow a long and complex trajectory until their eruption, that is, they are more likely to suffer disturbances during this process. Removal of the permanent canine should only be considered in extreme cases, where the canine itself is not susceptible to orthodontic traction. Orthodontic traction is the most indicated treatment due to the high morphofunctional and aesthetic value of the permanent canine. The aim of this study is to report a clinical case of traction surgery of the superior permanent canine included in the maxilla. A 13-year-old female patient attended the dental clinic at the Nilton Lins University accompanied by her guardian, referred by an orthodontistfor canine traction surgery. during the examIn the clinical analysis, tooth 13 was absent from the dental arch. The patient had a computed tomography scan, where it was possible to verify the actual position of the toothimpacted with the partially erupted crown on the alveolar ridge. After planning, surgery was performed to install an orthodontic button and adapt the ligature with a 0.30 mm wire. After the 10-month follow-up period, the presence of the tooth in arch, completely erupted, was observed, showing effectiveness in the proposed treatment. Therefore, the procedure for bonding the orthodontic device was performed satisfactorily without complications or intercurrences, making it possible to continue the orthodontic treatment. The patient remains under follow-up.


RESUMO Os caninos permanentes são essenciais para a preservação do sistema estomatognático, visto que, fazem parte da função e equilíbrio oclusal e estético. Durante o desenvolvimento normal da oclusão, os caninos seguem uma longa e complexa trajetória até seu irrompimento, ou seja, estão mais propensos a sofrerem distúrbios no decorrer desse processo. A extração do canino permanente só deve ser considerada em casos extremos, onde o próprio não se encontra suscetível à tração ortodôntica. O tracionamento ortodôntico é o tratamento mais indicado devido ao alto valor morfofuncional e estético do canino permanente. Sendo assim, o objetivo deste estudo é relatar um caso clínico de cirurgia de tracionamento de canino permanente superior incluso em maxila. Paciente de 13 anos, gênero feminino, compareceu a clínica odontológica da Universidade Nilton Lins acompanhado de seu responsável, encaminhada por um ortodontista para cirurgia de tracionamento de canino. Durante o exame clínico observou-se ausência do dente 13 na arcada dentária. A paciente apresentou a tomografia computadorizada, onde foi possível verificar o real posicionamento do dente impactado com a coroa parcialmente irrompida no rebordo alveolar. Após o planejamento, realizou-se a cirurgia destinada a instalação de botão ortodôntico e adaptação da amarrilha com fio 0,30 mm. Posteriormente ao período de acompanhamento de 10 meses, observou-se a presença do dente em arco, totalmente erupcionado, mostrando efetividade no tratamento proposto. Portando, o procedimento de colagem de dispositivo ortodôntico ocorreu de forma satisfatória sem complicações ou intercorrências, tornando possível a continuidade do tratamento ortodôntico. A paciente permanece em acompanhamento.

Resumo Este artigo possui como objetivo relatar 3 casos clínicos de pacientes com Síndrome de Down (SD) com anomalias dentárias associadas, incluindo agenesia dentária, irrupção ectópica e dentes inclusos. O padrão de anomalias dentárias (PAD) é um assunto amplamente descrito na população sem síndromes. No entanto, o PAD associado à Síndrome de Down ainda é pouco abordado na literatura. O tratamento ortodôntico em pacientes com Síndrome de Down deve ser cuidadosamente monitorado durante o desenvolvimento dentário, considerando a herança genética e o aumento do risco de distúrbios de irrupção dentária e outras anomalias dentárias. Intervenções interceptivas podem contribuir para simplificar o tratamento ortodôntico e reduzir os efeitos adversos (AU)


Abstract Individuals with Down Syndrome (DS) have a higher prevalence of dental anomalies than non-syndromic patients. This series of cases aim to report 3 patients seeking orthodontic treatment with several associated dental anomalies, including tooth agenesis, ectopic eruption and impacted teeth. The dental anomaly pattern (DAP) is well described subject in non-syndromic population. However, DAP in association with DS is still uncovered in the literature. The orthodontic treatment in patients with Down Syndrome should be carefully monitored during dental development, considering the genetic background and the increased risk for dental eruption disturbances and other dental anomalies. Interceptive interventions might contribute to simplify orthodontic treatment and reduce adverse effects (AU)

Resumo Introdução: O traumatismo dental na dentição decídua pode influenciar o desenvolvimento dos sucessores permanentes e causar um deslocamento da coroa em relação à raiz dental, gerando uma curvatura definida como dilaceração, que em grande parte dos casos impede a irrupção normal do dente permanente. Diversas são as possibilidades de tratamento, desde exodontia ao tratamento ortocirúrgico. Objetivo: O presente trabalho tem o objetivo de relatar um caso clínico de incisivo central superior esquerdo (21) com dilaceração radicular cujo tratamento envolveu procedimento cirúrgico e tracionamento ortodôntico ancorado no disjuntor maxilar. Resultado: O desfecho do tratamento foi satisfatório, visto que o dente incluso foi reposicionado e a má oclusão da paciente foi corrigida. Conclusão: O tracionamento ortocirúrgico de incisivo superior permanente com dilaceração radicular é desafiador e com prognóstico incerto. Contudo, seu reposicionamento é possível quando se realiza um diagnóstico, planejamento periodontal e biomecânico adequado (AU)


Abstract Introduction: Dental trauma in the primary dentition can influence the development of permanent successors and cause a displacement of the crown in relation to the dental root, generating a curvature defined as laceration, which in most cases prevents the normal eruption of the permanent tooth. There are several treatment possibilities, from extraction to orthosurgical treatment. Objective: The present study aims to report a clinical case of a left upper central incisor (21) with root laceration whose treatment involved a surgical procedure and orthodontic traction anchored in the maxillary breaker. Result: The treatment outcome was satisfactory, since the included tooth was repositioned and the patients' malocclusion was corrected. Conclusion: Orthosurgical traction of the permanent upper incisor with root dilaceration is challenging and has an uncertain prognosis. However, its repositioning is possible when a diagnosis, periodontal and biomechanical planning is performed.(AU)

ABSTRACT

This case report describes a simple alternative for treatment of mesioangulation of mandibular second molars. An 11-year-old girl was diagnosed with unilateral posterior crossbite, moderate incisor crowding and mesioangulation of the right mandibular second molar. The ectopic second molar was uprighted using a modified lingual arch with a distal hook and elastic chains. Orthodontic traction began after appliance installation and was activated once per month using 120 g of force. After six months, the right mandibular second molar was in an upright position. Orthodontic treatment continued with teeth levelling and alignment until acceptable occlusal and aesthetic results were achieved. Adequate stability was observed one year after debonding. The modified mandibular lingual arch with a distal hook is a simple and effective alternative to manage mesioangulation of mandibular second molars with minimum adverse effects and stable outcomes.

ABSTRACT

Maxillary canine and first premolar transposition is a complicated dental anomaly to treat, especially if the clinician's goal is to orthodontically move the canine into its normal position. Early diagnosis with cone-beam computed tomography simplifies the treatment of this pathology. This case report describes a patient with bilateral transposition, one complete and the other incomplete, involving the maxillary canine and the first premolar (Mx.C.1P). The orthodontic treatment involved the correction of both transpositions. In the complete transposition, the traction was mesial and upward to move the canine into a more apical position with a wider dentoalveolar process for easier crown interchange.

ABSTRACT Introduction: Invasive cervical resorption (ICR) is a relatively rare type of ERR (External Root Resorption), in which a localized resorption begins in the cervical area of the tooth, below the epithelial junction and above the ridge crest. Objective: Describe the clinical case of an 11-year-old boy with no dental trauma history, presenting moderate crowding and ectopic eruption of the maxillary right central incisor. He had been undergoing orthodontic treatment elsewhere, and his family was dissatisfied with the results. Description: A new treatment was indicated, which included rapid maxillary expansion followed by extraction of four premolars. During routine panoramic evaluation, a radiolucid image was detected and a periapical radiograph was requested. At this point, an ICR of the maxillary right central incisor was found. The treatment was cautiously finalized and despite the use of light forces, central incisor was severally compromised by ICR and was therefore extracted. Conclusion: This clinical example discusses the importance of routine radiographs for the early diagnoses of ICR.


RESUMO Introdução: A reabsorção cervical invasiva (RCI) é um tipo relativamente raro de reabsorção radicular externa (RRE), no qual uma reabsorção localizada começa na região cervical do dente, abaixo da junção epitelial e acima da crista marginal. Objetivo: Descrever o caso clínico de um menino com 11 anos de idade, sem histórico de trauma dentário, apresentando apinhamento moderado e erupção ectópica do incisivo central superior. Ele realizava tratamento ortodôntico em outro local, mas sua família estava insatisfeita com os resultados. Descrição: Foi iniciado um novo tratamento, que incluiu a expansão rápida da maxila, seguida pela extração de quatro pré-molares. Durante a avaliação de rotina da radiografia panorâmica, uma imagem radiolúcida foi detectada, e uma radiografia periapical da região foi solicitada, sendo encontrada uma RCI no incisivo central superior direito. O tratamento foi finalizado com cautela e, apesar do uso de forças leves, o incisivo central estava seriamente comprometido pela RCI e, portanto, precisou ser extraído. Conclusão: Esse exemplo clínico discute a importância das radiografias de rotina para o diagnóstico precoce da RCI.

ABSTRACT

The presence of supernumeraries may lead to irruptive problems, such as impacted teeth, functional and aesthetic disorders. The present study describes two clinical cases, with anterior teeth eruption disorders caused by additional dental elements, treated early. Patients presented with aesthetic involvement, with delay in the emergence of the upper incisors. The clinical and radiographic examinations detected the presence of obstacles to the teeth eruption. Early treatment with 2x4 appliance was realized, followed by final corrective orthodontics. After restoration of the dimension for retained teeth, there was spontaneous eruption in clinical case 1 and need of traction in clinical case 2. When patients reached the complete permanent dentition, a corrective phase was performed with the objective of detailing the dental occlusion and finishing the treatment. The early diagnosis of teeth impacted and supernumerary, provide the achievement of adequate occlusion and positive aesthetic of patients, with good prognosis, avoiding psychological and functional problems caused by absence of anterior teeth. A presença de supranumerários pode ocasionar problemas irruptivos, como impacção de dentes, desordens funcionais e estéticas. O presente trabalho descreve dois casos clínicos abordados precocemente, com alterações na irrupção de dentes anteriores devido a presença de supranumerários. Os pacientes apresentavam queixas estéticas, com atraso no aparecimento dos incisivos superiores. Os exames clínico e radiográfico detectaram a presença de barreiras irruptivas. Realizou-se tratamento precoce com auxílio da mecânica 4x2, seguido pela ortodontia corretiva final. Após etapa de restabelecimento da dimensão para os dentes retidos, houve irrompimento espontâneo no caso clínico 1 e necessidade de tracionamento no caso clínico 2. Quando pacientes atingiram a dentição permanente completa, realizou-se uma fase corretiva com o objetivo de detalhamento da oclusão dentária e finalização do tratamento. O diagnóstico precoce de impacção dentária e detecção de supranumerários favoreceu a restituição de oclusão adequada e estética dos pacientes, com prognóstico muito favorável, evitando problemas psicológicos e funcionais provocados pela ausência de dentes anteriores. ABSTRACT

This article presents a case report of an adult patient with chronic sinusitis related to the presence of two erupted ectopic teeth located atypically in the maxillary sinus roof/orbital floor after a long latency period associated with childhood facial trauma. This article aims to show the treatment of chronic sinusitis of odontogenic origin by surgical removal of ectopic teeth in an unusual position by direct visualization. This case report discusses the signs and symptoms of chronic sinusitis linked to the presence of ectopic elements and associated with an inflammatory cyst, the choice of complementary tests for diagnosis and surgical treatment through the Caldwell-Luc procedure. After a 2-year follow-up period, the patient presently shows clinical improvement, thus demonstrating the success of the chosen treatment.

Tooth transposition is a rare dental anomaly that often represents a challenge for the clinician. The case of a girl with skeletal Class III malocclusion and concomitant maxillary canine-first premolar bilateral transposition, followed from 7 to 17 years of age, is presented. After a first phase of treatment aimed at resolving the Class III malocclusion, the transposition was maintained and the case finalized with a multibracket appliance.

INTRODUCTION: Maxillary canine-first premolar transposition (Mx.C.P1) is an uncommon dental positional anomaly that may create many orthodontic problems from both esthetic and functional points of view. OBJECTIVE: In this report we show the orthodontic management of a case of Mx.C.P1 associated with bilateral maxillary lateral incisor agenesis and unilateral mandibular second premolar agenesis METHODS: The patient was treated with a multibracket appliance and the extraction of the lower premolar. RESULTS: treatment was completed without the need for any prosthetic replacement. .


INTRODUÇÃO: a transposição entre canino e primeiro pré-molar superiores (Mx.C.P1) é uma anomalia de posição dentária rara, que pode causar muitos problemas ortodônticos, não só com relação à estética, mas também com relação à função do paciente. OBJETIVO: no presente artigo, relatamos o manejo ortodôntico de um caso de Mx.C.P1 associada à agenesia bilateral de incisivos laterais superiores e agenesia unilateral do segundo pré-molar inferior. MÉTODOS: o paciente foi tratado com aparelho ortodôntico fixo e extração do pré-molar inferior. RESULTADOS: o tratamento foi concluído sem a necessidade de reposição protética. .

METHODS: We describe a case of transposition between the upper left canine and lateral incisor associated with impaction of the central incisor on the same side, in a 12-year-old patient. Conservative treatment involving surgical-orthodontic correction of transposed teeth and traction of the central incisor was conducted. CONCLUSIONS: The option of correcting transposition and orthodontic traction by means of the segmented arch technique with devices such as cantilever and TMA rectangular wire loops, although a complex alternative, was proved to be esthetically and functionally effective.

INTRODUÇÃO: transposição e impacção dentárias são distúrbios relacionados à erupção ectópica ou à falha na erupção dentária, que podem afetar o desenvolvimento físico, psíquico e social da criança, e que podem ser de difícil resolução clínica. MÉTODOS: é descrito um caso clínico de transposição entre o canino e o incisivo lateral superior esquerdo, associado à impacção do incisivo superior, do mesmo lado, em uma paciente de 12 anos de idade. Optou-se pela realização de tratamento ortodôntico-cirúrgico conservador, envolvendo a correção ortodôntica dos dentes transpostos e o tracionamento do incisivo central impactado. CONCLUSÃO: a opção pela correção da transposição e tracionamento ortodôntico com a utilização da técnica do arco segmentado com uso de dispositivos como cantiléver e alças em fios TMA retangulares, apesar de ser uma alternativa de execução complexa, mostrou-se efetiva do ponto de vista estético e funcional. .

INTRODUCTION: Nasal teeth eruption is a rare phenomenon. The variability of symptoms and generic history makes the diagnosis difficult. This difficulty is more challenging when the tooth is placed in the depth of septum. CASE REPORT: Our case is an example of this problem. Herein, we present a case of intraseptal tooth with nasal obstruction and septal deviation and recurrent sinusitis. We present preoperative imaging. CONCLUSION: Great suspicion may helpful for preoperative diagnosis and good deciding.

Ectopic eruption is a concept including those clinical cases in which teeth show abnormal eruption pattern and erupt ectopically, in an incorrect position. This abnormality has been pointed out in the literature for the first permanent molars, mainly the maxillary ones. There is no specific etiological factor for this abnormality in the first permanent molar, but different factors are reported. The early approach of the ectopic eruption of the first permanent molar can prevent effects such as the early root resorption of the adjacent deciduous second molar and the loss of space for eruption of the premolar successor as well. The purpose of this paper was to report a ectopic eruption of a maxillary first permanent molar diagnosed in a nine-year-old patient with severe root resorption of the adjacent deciduous second molar. A simple and effective therapeutic approach was implemented to treat such abnormality. This clinical case was followed until complete eruption of the premolar successor.


Erupção ectópica é um conceito utilizado para designar aqueles casos em que os dentes apresentam algum desvio em seu padrão normal de erupção, ou seja, erupcionam numa posição atípica. Esta condição tem sido descrita na literatura para os primeiros molares permanentes, principalmente os superiores. Não há um fator etiológico específico para a erupção ectópica do primeiro molar permanente, onde diferentes fatores são relatados. A abordagem precoce da erupção ectópica do primeiro molar permanente poderá evitar sequelas, tais como a reabsorção radicular precoce do segundo molar decíduo adjacente, bem como a perda de espaço para erupção do pré-molar sucessor. O objetivo deste trabalho é descrever um caso de erupção ectópica de um primeiro molar permanente superior, em uma paciente de nove anos de idade, com reabsorção radicular severa do segundo molar decíduo adjacente. Uma abordagem terapêutica simples e efetiva foi adotada para o tratamento da erupção ectópica. O caso foi acompanhado até a completa erupção do segundo pré-molar sucessor.
