ABSTRACT

The impact of bacterial pneumonia on patients with COVID-19 infection remains unclear. This prospective observational monocentric cohort study aims to determine the incidence of bacterial community- and hospital-acquired pneumonia (CAP and HAP) and its effect on mortality in critically ill COVID-19 patients admitted to the intensive care unit (ICU) at University Hospital Olomouc between 1 November 2020 and 31 December 2022. The secondary objectives of this study include identifying the bacterial etiology of CAP and HAP and exploring the capabilities of diagnostic tools, with a focus on inflammatory biomarkers. Data were collected from the electronic information hospital system, encompassing biomarkers, microbiological findings, and daily visit records, and subsequently evaluated by ICU physicians and clinical microbiologists. Out of 171 patients suffering from critical COVID-19, 46 (27%) had CAP, while 78 (46%) developed HAP. Critically ill COVID-19 patients who experienced bacterial CAP and HAP exhibited higher mortality compared to COVID-19 patients without any bacterial infection, with rates of 38% and 56% versus 11%, respectively. In CAP, the most frequent causative agents were chlamydophila and mycoplasma; Enterobacterales, which were multidrug-resistant in 71% of cases; Gram-negative non-fermenting rods; and Staphylococcus aureus. Notably, no strains of Streptococcus pneumoniae were detected, and only a single strain each of Haemophilus influenzae and Moraxella catarrhalis was isolated. The most frequent etiologic agents causing HAP were Enterobacterales and Gram-negative non-fermenting rods. Based on the presented results, commonly used biochemical markers demonstrated poor predictive and diagnostic accuracy. To confirm the diagnosis of bacterial CAP in our patient cohort, it was necessary to assess the initial values of inflammatory markers (particularly procalcitonin), consider clinical signs indicative of bacterial infection, and/or rely on positive microbiological findings. For HAP diagnostics, it was appropriate to conduct regular detailed clinical examinations (with a focus on evaluating respiratory functions) and closely monitor the dynamics of inflammatory markers (preferably Interleukin-6).

Background: The epidemiologic transition in Mexico has generated a change of paradigm in public health. Morbidity is characterized by infectious diseases and the mortality is due to chronic degenerative diseases. The three most important infectious diseases in the country are: respiratory infections, diarrhea, and urinary tract infections. Method: The objective of this work was to build a tool to monitor the presence of health risks in the environment in a timely manner and to demonstrate its application in different sicknesses, especially those that are water related. In this study, we analyzed water samples from five cenotes with high tourist flow in the State of Yucatan. We developed a DNA microarray for the adequate and prompt detection of viruses, bacteria, fungi, and parasites. This microarray could be used in samples of different origin including air, water (fresh, brackish and saltwater), food, inert surfaces or wounds. Clinically, it would allow prompt and precise detection of etiological agents of infectious diseases to prevent outbreaks. It would also be useful for the identification of those agents that cannot be detected in our laboratories with the traditional methods. It includes 38,000 probes that detect 252 etiological agents of diseases in humans and antimicrobial resistance genes. Results from DNA samples can be obtained in 24 h, which would be difficult or impossible using other technologies. Results: The results are readily available within 24 h. Samples from five cenotes (sinkholes) with high flow of people, were analyzed with the microarray. The water samples analyzed detected 228 different bacteria, viruses, fungi, and protozoa. They are amongst the most important etiological agents for infectious diseases in Mexico. Conclusions: The microarray provides the opportunity for precise and early detection of various infectious agents in individuals, hospitals and natural environments. This could help reduce the global burden of diseases, the severity of outbreaks, and reduce antibiotic resistance.

ABSTRACT

Mycetoma is a chronic, disabling infection caused by fungi or actinomycetes that affects the disadvantaged rural populations of arid tropical regions. The identification of etiological agents is long, difficult, and often imprecise or unsuccessful. Recently developed molecular methods can be used to identify causal agents at the species level. However, diagnosis can only be implemented in specialized laboratories. For these reasons, the distribution of causal agents in endemic African countries remains approximate. It is known that the pathogenic organisms of mycetoma are present in the environment, introduced as a result of injuries or trauma. There are still unknowns concerning the natural habitats of agents and the mode of infection. A potential association between mycetoma and acacia was uncovered in Sudan, allowing the elaboration of a risk map of the country. A new hypothesis for the mode of contamination involves the intervention of an intermediate host. The first surveys in Sudanese endemic villages gave a higher prevalence than the previous estimates, indicating that the prevalence of mycetoma in endemic African countries has previously been underestimated.

ABSTRACT

Lung diseases continue to draw considerable attention from biomedical and public health care agencies. The lung with the largest epithelial surface area is continuously exposed to the external environment during exchanging gas. Therefore, the chances of respiratory disorders and lung infections are overgrowing. This review has covered promising and opportunistic etiologic agents responsible for lung infections. These pathogens infect the lungs either directly or indirectly. However, it is difficult to intervene in lung diseases using available oral or parenteral antimicrobial formulations. Many pieces of research have been done in the last two decades to improve inhalable antimicrobial formulations. However, very few have been approved for human use. This review article discusses the approved inhalable antimicrobial agents (AMAs) and identifies why pulmonary delivery is explored. Additionally, the basic anatomy of the respiratory system linked with barriers to AMA delivery has been discussed here. This review opens several new scopes for researchers to work on pulmonary medicines for specific diseases and bring more respiratory medication to market.

BACKGROUND: Fungal infection of animals or humans are common all over the world. Some of microorganisms like fungi, exist on the skin and can be transmitted onto other individuals, other animal species or even humans and cause skin infections. Moreover, they can be the causative agents of severe generalized infections especially in immunocompromised individuals. The study aimed to evaluate the most frequent etiological agents of skin diseases and to compare the prevalence of animal fungal infections in Poland, and to discuss the possibility of transmission to humans in Poland. RESULTS: The obtained results are culture based. The fungi most frequently isolated from group of animals with skin lesions were Malassezia pachydermatis (29.14%), and Candida yeasts (27.07%), and dermatophytes (23.5%), including Microsporum canis as majority of them (59.25%), and Trichophyton genus (40.7%), most of them T. mentagrophytes, while Malassezia pachydermatis represented (80%) of isolates in animals with otitis externa. In over 19% of positive fungal cultures obtained from external ear canals Candida yeasts, mainly C. albicans, were identified. CONCLUSIONS: Dermatomycoses in companion animals are caused by both, mycelial fungi and yeasts. Most frequently isolated were Malassezia pachydermatis and Candida spp. Dermatophytes (Trichophyton, Microsporum), were also cultured, but the total number of these isolates seems to decrease. We have not found Cryptococcus neoformans in tested clinical samples.

ABSTRACT

Neonatal calf diarrhea (NCD) is one of the most serious health challenges facing the livestock industry and has caused substantial economic losses due to increased morbidity and mortality rates. The present study investigated the main infectious pathogens causing NCD among cattle in Yangxin County, China. Sixty-nine fecal samples were collected from diarrheic newborn cattle and tested for infectious agents, including bovine rotavirus, bovine coronavirus, Escherichia coli K99, Cryptosporidium parvum, and Giardia lamblia, that cause NCD, as determined by rapid kit analysis and polymerase chain reaction (PCR) amplification. The PCR results showed that the percentages of samples that were positive for C. parvum, bovine rotavirus A, bovine coronavirus, and G. lamblia were 44.93, 36.23, 17.39, and 13.04%, respectively. The rapid kit analysis results showed that the prevalence of C. parvum, rotavirus, coronavirus, and G. lamblia was 52.17, 31.88, 28.98, and 18.84%, respectively. No E. coli K99 was detected by either method. The total positivity of the samples, as determined by PCR and rapid kit analysis, was 80.00 and 81.16%, respectively. No significant difference between the two methods was observed. The results of this study may help to establish a foundation for future research investigating the epidemiology of NCD in cattle and may facilitate the implementation of measures to control NCD transmission to cattle in Yangxin County, Shandong Province, China.

ABSTRACT Mastitis is one of the most important diseases for the dairy industry worldwide due to the great economic losses it causes, and the bacterial agents that cause it vary from one herd to another. The objective of the investigation was to determine the infectious agents involved in the development of mastitis in specialized dairies herds in the Highlands of Boyacá, Colombia. Was performed CMT tests in 4432 teats to determine the positive results for inflammation (CMT + 2), sampling in aseptic material of positives quarters, identification and differentiation through microbiological cultures, Gram staining, and biochemical tests of bacterial and fungal agents present in milk samples. It was determined that the total prevalence varied in the three years of the study as follows: in 2016, 45.19%; in 2017, 51.06% and in 2018, 39.76%. The months of April, May, and September, October presented the highest number of teats affected with mastitis during the three years of study. Streptococcus agalactiae and Staphylococcus aureus were the most prevalent microorganisms that cause mastitis in the three study years. There was an increase in the prevalence of infected teats corresponding to the months (April, May, September, and October) due to rainfall increases. The contagious pathogens were most prevalent that environmental pathogens.


RESUMEN La mastitis es una de las enfermedades más importantes para la industria lechera a nivel mundial debido a las grandes pérdidas económicas que genera; además, los agentes que la generan varían de un rebaño a otro. El objetivo de esta investigación fue determinar los agentes infecciosos involucrados en el desarrollo de mastitis en lecherías especializadas del departamento de Boyacá (Colombia). Se realizaron pruebas de CMT a 4432 cuartos para determinar los positivos a inflamación (CMT+2), toma de muestras en material aséptico de cuartos positivos, identificación y diferenciación a través de cultivos microbiológicos, tinción de Gram y pruebas bioquímicas de los agentes bacterianos y micóticos presentes en las muestras de leche. Se determinó que la prevalencia total varió en los tres años de estudio así: en el año 2016, 45,19%; en el 2017, 51,06%, y en el 2018, 39,76%. Los meses de abril-mayo y septiembre-octubre presentaron la mayor cantidad de cuartos afectados con mastitis a lo largo de los tres años de estudio. Finalmente, se determinó que Streptococcus agalactiae y Staphylococcus aureus, microorganismos que generan mastitis, fueron los de mayor prevalencia durante los tres años de estudio debido a un aumento en las lluvias. Los patógenos contagiosos causantes de mastitis fueron los más prevalentes, por encima de los medioambientales.

BACKGROUND: Acute encephalitis syndrome (AES) surveillance in India has indicated that Japanese encephalitis virus (JEV) accounts for 5-35% of AES cases annually; the etiology remains unknown in the remaining cases. We implemented comprehensive AES surveillance to identify other etiological agents of AES, with emphasis on dengue virus. METHODS: Serum and cerebrospinal fluid (CSF) specimens were collected from patients enrolled prospectively in AES surveillance from 2014-2017 at selected sites of three high burden states of India. All samples were initially tested for JEV IgM. Specimens negative for JEV by serology were tested for IgM to scrub typhus, dengue virus (DEN), and West Nile virus; all JEV IgM-negative CSF samples were tested by PCR for S. pneumoniae, N. meningitidis, H. influenzae, herpes simplex virus type 1, enteroviruses and DEN. RESULTS: Of 10,107 AES patients, an etiology could be established in 49.2% of patients including JEV (16%), scrub typhus (16%) and DEN (5.2%) as the top three agents. Amongst the DEN positive cases (359/6892), seven (2%) were positive only for dengue virus RNA: one in serum and six in CSF. CONCLUSION: Amongst the pathogens identified, dengue accounted for 5% of all AES cases and was one of the three common etiological agents. These results underscore the importance of including dengue virus in routine testing of AES cases.

INTRODUCTION: Ventilator-associated pneumonia (VAP) is one of the most common healthcare-associated infections (HAI) in neonates admitted in neonatal intensive care units (NICUs). METHODS: We did a systematic review using PRISMA methodology to identify the main etiological agents in Brazilian NICUs. Eligible studies published without period restriction were identified in PUBMED, SCIELO, LILACS and DOAJ. Studies were included if they were conducted in neonates hospitalized at NICU. Studies done in outpatient care, neonates outside NICU, emergency department, primary care, long-term care facilities or a combination of these were excluded. RESULTS: We identified 6384 articles in the initial search and four papers met the inclusion criteria. In all studies included, rates of device-associated infections were described, including VAP rates. The VAP incidence density, in exclusively Brazilian NICU, ranged from 3.2 to 9.2 per 1000 ventilator-days. Pneumonia was described as the main HAI in NICU in one article, as the second type of HAI in two other articles and as the fourth type of HAI in the last one. The main pathogens causing all HAI types were described in three of four articles, but, none of the articles reported which pathogens were related or associated to VAP. CONCLUSION: Etiological agents causing VAP in Brazilian NICUs are, until the present time, not known.

INTRODUCTION: Occupational asthma (OA) is the most common occupational lung disease in industrialized countries; however, in Western Europe, a decline in the prevalence of OA and changes in etiological factors has been recorded. Data from Central Europe, where in the past healthcare and economy sectors were different, are rare. OBJECTIVE: The aim of this study was to discover the changes related to OA during a four decade long period in the Slovak Republic. MATERIAL AND METHODS: The study included patients whom OA was reported during 1980-2016. All cases of OA were divided into four decades. The prevalence of OA, changes in etiological factors and key diagnostic methods were evaluated. All decades were analysed separately and compared to one another. RESULTS: During 1980-2016, OA was reported in 155 patients. OA was most often found in the second decade (6.4 cases per year), the lowest number of cases was diagnosed in the third decade (2.1 cases per year). Since 1988, an increasing prevalence of OA was observed with the peak in 1992, followed by the significantly progressive decrease during subsequent years. In the fourth decade, a mild increase in OA causes was recorded again. In the first and second decades, agricultural allergens were the dominant agents, which were replaced by chemical factors in the last decade. The most frequently used key diagnostic methods in 1980-2009 were skin tests, during the fourth decade the importance of this method declined. However, specific BPT a serial BPT were more frequently used in the fourth decade. CONCLUSION: The number of OA cases induced by agricultural allergens declined, which was connected with a decrease of total OA cases. The current problem is the OA induced by the chemicals, which increased significantly in the last decade, even the possibility of underdiagnosed OA cases still exists.

ABSTRACT

In this chapter, we provide an overview of the basic principles of teratology, beginning with its definition, the critical point for teratogenesis to occur and the most evident etiological agents to improve the understanding of this science.Teratology is a recent science that began in the early twentieth century, and has greatly improved over the recent years with the advancements in molecular biology, toxicology, animal laboratory science, and genetics, as well as the improvement on the knowledge of the environmental influences.Nevertheless, more work is required to reduce the influence of hazardous products that could be deleterious during pregnancy, thus reducing teratogenic defects in the newborn. While some teratogenic defects are attributed to their agents with certainty, the same for a lot of other such defects is lacking, necessitating consistent studies to decipher the influence of various teratogenic agents on their corresponding teratogenic defects. It is here that the laboratory animal science is of great importance both in the present and in the future.

BACKGROUND: Mycetoma is a chronic inflammatory process caused either by fungi (eumycetoma) or bacteria (actinomycetoma). In this retrospective study, we report epidemiologic and histopathological data of mycetoma observed in the Lome Hospital, Togo in a 25-year period (1992-2016). METHODOLOGY: This is a retrospective study, over a period of 25 years, to analyze epidemiological and etiological findings of mycetomas seen in the single laboratory of pathological anatomy of the Lomé, Togo. RESULTS: A total of 61 cases were retrieved from which only 33 cases were included which where clinically and microbiologically confirmed. The mean age of the patients was 29.7 ± 1.34 and a sex ratio (M/F) of 1.5. The majority of patients were farmers (n = 23 cases; 69.7%). Diagnosed etiologic agents were fungal in 24 cases (72.7%) and actinomycotic cases in 9 cases (27.3%). The fungal mycetomas consisted of Madurella mycetomatis (black grains) and Falcifomispora senegaliensis (black grains). The actinomycotic agents were represented by Actinomadura madurae (white grains), Actinomadurae pelletieri (red grains) and Nocardia sp. (yellow grains). CONCLUSION: This report represents a single-center study which provides epidemiologic and histopathological data of mycetoma cases in Togo.

OBJECTIVES: To determine the etiological agents by Latex Particle Agglutination (LPA) and Polymerase Chain Reaction (PCR) in patients admitted with Cerebrospinal Fluid (CSF) culture negative bacterial meningitis Methods: This descriptive case series was conducted at National Institute of Child Health, Karachi from January 2010 to December 2012. Patients meeting the WHO case definition of suspected meningitis from one month to 59 months of age were included in the study. CSF examination and culture was carried out on every patient and CSF culture negative patients were enrolled. Demographic data, clinical signs & symptoms and laboratory findings were entered into the proforma. Data was analyzed using statistical package for social sciences (SPSS) version 17. P-value <0.05 was taken as significant. RESULTS: A total of 166 patients were included. Male were 96 and female were 76 with the male to female ratio of 1.26. The mean age of patient was ± SD 14.6 ± 14.5 months. The etiological agents identified by LPA were in 26/166 (15.66%) cases and the organisms were H. influenzae type b 10 cases, streptococcus pneumoniae 15 cases and meningococcus only one case respectively. The organisms identified by PCR were in 65/166 (39.15%) cases and the isolates were H. influenzae type b 16 cases, streptococcus pneumoniae 48 cases and meningococcus 01 case respectively. CONCLUSION: LPA and PCR are superior and useful diagnostic tools in microbiology. They can be used for rapid etiological diagnosis of bacterial meningitis for the early administration of proper antibiotic. ABSTRACT

La dacriocistitis representa la infección más frecuente del sistema lagrimal. El estudio microbiológico de las dacriocistitis es importante para la adecuada selección de los antibióticos. El objetivo del estudio es determinar la frecuencia y sensibilidad antimicrobiana de los agentes causantes de dacriocistitis en pacientes que acudieron a la Cátedra de Oftalmología del Hospital de Clínicas de mayo de 1998 a abril de 2007. Los pacientes no recibieron antibióticos por lo menos por una semana antes de la toma de muestra, que fueron tomadas por aspiración del contenido del saco lagrimal por el canalículo inferior o punción directa del saco lagrimal, y cultivadas en aerobiosis y anaerobiosis siguiendo métodos convencionales. De 41 pacientes (23 adultos y 18 niños), 30 (73,1%) presentaron cultivos positivos; 16 (53.4%) monomicrobianos y 14 (46,6%) polimicrobianos. Se aislaron 51 microorganismos, 37 en adultos y 14 en niños. El 54% de los microorganismos aislados en muestras de adultos correspondió a bacilos gramnegativos (enterobacterias, Haemophilus sp, anaerobios, y bacilos gramnegativos no fermentadores); 41% a cocos grampositivos (Staphylococcus aureus, Streptococcus pneumoniae, Streptococcus viridans, y estafilococo coagulasa negativo); el 72% de los micro-organismos aislados en niños correspondió a cocos grampositivos (Staphylococcus aureus y Streptococcus pneumoniae), y en una muestra se aisló Candida sp. La sensibilidad del Streptococcus pneumoniae a la penicilina y la del Staphylococcus aureus a la meticilina fue de 100%. Los microorganismos aislados presentaron buena sensibilidad a los antimicrobianos, pero por la gran variedad de microorganismos y la naturaleza polimicrobiana de los cultivos se debería ampliar el esquema inicial de tratamiento y de la profilaxis de la dacriocistorrinostomía y otros procedimientos intraoculares. Una opción es el uso de aminopenicilinas con un inhibidor de betalactamasa; asociado con colirio de tobramicina en niños y ciprofloxacina en adultos.


Dacryocystitis is the most frequent infection of the lacrimal sac. The microbiological evaluation of dacryocystitis is important to make an adequate choice of antibiotics. The objective of this study is to determine the frequency and antibiotic susceptibility of the etiological agents of dacryocystitis in patients attending the Ophthalmology Department of the Teaching Hospital of the National University of Asunción, Paraguay from May 1998 to April 2007. All patients were antibiotic-free for at least one week prior to sampling. Samples were taken by aspiration of the lacrimal sac obtained via canalicular reflux or incision of the lacrimal sac and cultured aerobically and anaerobically following standard methods. Out of 41 patients (23 adults and 18 children), 30 (73.1%) showed positive cultures, 16 (53.4%) of them were pure and 14 (46.6%) mixed cultures, yielding 51 microorganisms. Of the 37 microorganisms 54% were isolated in adults and corresponded to gram negative rods (enterobacterias, Haemophilus sp, anaerobic and non fermenting gram negative rods) while 41% to gram positive cocci: (Staphylococcus aureus, Streptococcus pneumoniae, Streptococcus viridans, coagulase negative Staphylococcus). In children, 72% of the micro-organisms isolated were gram positive cocci (Staphylococcus aureus and Streptococcus pneumoniae) and Candida sp was found in one sample. The susceptibility patterns of the Streptococcus pneumoniae to penicillin and Staphylococcus aureus to methicillin were 100%. All isolates showed high susceptibilities to the tested antibiotics. However, due to the great variety of species and the polymicrobial nature of the cultures, the initial scheme and prophylaxis of the dacryocystorhinostomy and other intraocular procedures should be expanded. One option is the use of aminopenicillins with a betalactamase inhibitor associated with tobramycin in children and ciprofloxacin in adults.

BACKGROUND: Onychomycosis manifests itself in various forms, notably onychodystrophy, onycholysis, subungual hyperkeratosis, or nail-plate discoloration. Not necessarily nail changes mentioned here should always be of fungal origin. OBJECTIVE: The present study is planned to get an idea about etiological agent and clinical correlation in fingernail onychomycosis. MATERIALS AND METHODS: Nail-clipping and subungual debris of patients with above mentioned nail changes were subjected to KOH preparation. Culture was done on SDA and SDCCA media. Species identification was done by colony character, pigment production, LCB staining, and some special tests like germ tube test, etc. RESULTS: Out of 85 cases, 44 cases showed the growth of fungus, amounting to 51.76% positivity. Among those 44 cases, the infective fungal agents were predominantly dermatophytes (50%), and the rest were due to yeasts (27.27%) and moulds (22.72%). Among the different species, Trichophyton rubrum (29.54%) accounted for the majority of dermatophytes; Candida albicans (11.78%) was the predominant yeast; and Aspergillus niger (18.18%) the commonest mold. No significant association could be established between the different fungal species and various clinical manifestations. Positive results were found more with fungal culture (95.45%) than KOH preparation (63.64%). CONCLUSION: The results show that nail changes are not always a reliable marker for predicting the causative organism, and relying only on the clinical manifestation (i.e., pattern of nail changes) in the diagnosis of onychomycosis is often misleading. The present study highlights the need for microbiological confirmation in case of onychomycosis.

Objetivo - analisar as ocorrências, os agentes etiológicos e a apresentaçäo clínica referentes às infecçöes bacterianas diagnosticadas em grupo de pacientes submetidos a transplante (Tx) cardíaco. Métodos - foram considerados 100 doentes, observados consecutivamente, após Tx cardíaco. O período de seguimento variou de 3 a 90 (média 25,38 + ou - 25,96) meses. O reconhecimento das infecçöes bacterianas levou em conta os critérios estabelecidos pelos Centers for Disease Control. Resultados - as infecçs pulmonares bacterianas comparecem em maior número, havendo dificuldade para diagnosticá-las depois do Tx. Ocorreram comprometimentos motivados por bactérias em pele, mucosas, partes moles, ferida operatória, pericárdio, pleura, loja do marcapasso e vias urinárias, tendo também sido constatadas bacteremias e endocardites. Os agentes etiológicos, quando reconhecidos, ficaram devidamente especificados. Conclusäo - o período pós-operatório inicial é crítico, pois nos 30 dias subsequentes a ele as infecçöe acterianas surgem com maior frequência. Elas também suscitam maior preocupaçäo nas fases de tratamento dos episódios de rejeiçöes. Diagnóstico precoce e rápida adoçäo de medidas coercitivas podem evitar gravidade e evoluçäo para óbito


Purpose - To evaluate clinical findings and etiology of bacterial infections diagnosed in 100 consecutive heart transplantations. Methods - One hundred consecutive heart transplant patients were studied. Follow-up after heart transplantation varied from 3 to 90 (mean 25.38± SD 25.97) months. Etiology of bacterial infection was established using the Centers for Disease Control criteria. Results - Bacterial infection was the most common cause of infection after heart transplantation; diagnosis was difficult. Infection sites were skin, mucous, membranes, soft tissue, surgical scar, pericardial and pleural spaces, soft tissue around heart pacing devices, urinary tract; bacteremias and endocarditis were also found. All bacterial agents recovered were fully identified Conclusion - Bacterial infections are the most common infections in the first month after heart transplantation. They are important and also common after the treatment of the rejection episodes. Rapid diagnosis and adequate treatment are essential to prevent morbidity and death

ABSTRACT

Objetivo - analisar ocorrências, agentes etiológicos, aspectos clínico e taxa de óbitos referentes à endocardite infecciosa (EI) diagnosticada em pacientes submetidos a transplante cardíaco. Métodos - cem doentes foram observados consecutivamente após transplante cardíaco. O período de seguimento variou de 3 a 90 (média 25,38 +/- 25,97) meses. O reconhecimento da EI levou em conta critérios usados no Serviço de Epidemiologia e Desenvolvimento de Qualidade do INCOR da FMUSP, tendo como bases as defiçöes estipuladas pelo Center for Disease Control. Cooperaram para estabelecer o diangóstico, fundamentalmente, hemocultura, ecocardiograma transtorácico ou esofágico e necrópsia. Resultados - houve reconhecimento de 6 casos, com 4 óbitos, estando presentes em 3 processos sistêmicos generalizados. Febre compareceu como manifestaçäo clínica habitual. Conclusäo - o número de óbitos afigurou-se elevado, tendo em conta a natureza das bactérias causadores, a associaçäo com afecçöes graves e o uso de fármacos mnodepressores. Considerou-se importante adotar adequados cuidados em relaçäo aos fatores de risco, efetuar precocemente o diagnóstico e institutir rapidamente o tratamento.
