ABSTRACT

Introdução: Varicela é uma doença exantemática muito frequente na população pediátrica e geralmente cursa com um quadro benigno na infância. Porém, quando coincide com uma fase de maior imaturidade imunológica, em recémnascidos de até três meses de idade, pode suscitar dúvidas quanto à sua evolução e receios na sua abordagem. Objetivo: Relatar o manejo, tratamento e a evoluçãoo de uma criança recém-nascida com varicela. Método: Trata-se de um estudo tipo relato de caso, observacional e descritivo, utilizando como fonte de dados o prontuário do paciente, registros fotográficos, além de revisões bibliográficas. Resultado: Quadro de varicela em recém-nascido, com 36 dias de vida, cuja evolução se deu ao longo dos dias com resposta satisfatória ao tratamento. Conclusão: A suspeita diagnóstica permitiu a intervenção precoce, o que culminou em desfecho favorável para o caso.(AU)


Introduction: Chickenpox is an exanthematic disease very frequent in the pediatric population and usually runs with a benign condition in childhood. However, when it coincides with a phase of greater immunological immaturity, in newborns up to three months of age, it may raise doubts about their evolution and fears in their approach. Objective: To report the management, treatment and evolution of a newborn child with chickenpox. Method: This is a case report study, observational and descriptive, using as data source the patient's medical records, photographic records, and bibliographic reviews. Result: Picture of chickenpox in newborn, with 36 days of life, whose evolution took over the days with satisfactory response to treatment. Conclusion: The diagnostic suspicion allowed early intervention, which culminated in a favorable outcome for the case..(AU)


Introducción: La varicela es una enfermedad exantemática muy común en la población pediátrica y generalmente se presenta con una condición benigna en la infancia. Sin embargo, cuando coincide con una fase de mayor inmadurez inmunológica, en recién nacidos hasta los tres meses de edad, puede plantear dudas sobre su evolución y temores en su abordaje. Objetivo: Informar el manejo, tratamiento y evolución de un recién nacido con varicela. Método: Se trata de un estudio de caso-relato, observacional y descriptivo, utilizando como fuente de datos la historia clínica del paciente, registros fotográficos y revisiones bibliográficas. Resultado: Varicela en un recién nacido de 36 días, cuya evolución se fue dando a lo largo de los días con una respuesta satisfactoria al tratamiento. Conclusión: La sospecha diagnóstica permitió una intervención precoz, lo que resultó en un resultado favorable para el caso.(AU)

ABSTRACT

The epidemic transmission of Zika virus (ZIKV) in Brazil has been identified as a cause of microcephaly and other neurological malformations in the babies of ZIKV-infected women. The frequency of adverse outcomes of Zika virus infection (ZIKVi) in pregnancy differs depending on the characteristics of exposure to infection, the time of recruitment of research participants, and the outcomes to be observed. This study provides a descriptive analysis-from the onset of symptoms to delivery-of a cohort registered as having maternal ZIKVi in pregnancy, from November 2015 to December 2016. Suspected cases were registered at a referral center for infectious and tropical diseases in Manaus, in the Amazonian region of Brazil. Of 834 women notified, 762 women with confirmed pregnancies were enrolled. Reverse-transcriptase polymerase chain reaction (RT-PCR) confirmed ZIKVi in 42.3% of the cohort. In 35.2% of the cohort, ZIKV was the sole infection identified. Severe adverse pregnancy outcomes (miscarriage, stillbirth, or microcephaly) were observed in both RT-PCR ZIKV-positive (5.0%) and ZIKV-negative (1.8%) cases (RR 3.1; 95% IC 1.4-7.3; p < 0.05), especially during the first trimester of pregnancy (RR 6.2, 95% IC 2.3-16.5; p < 0.001). Although other infectious rash diseases were observed in the pregnant women in the study, having confirmed maternal ZIKVi was the most important risk factor for serious adverse pregnancy events.

ABSTRACT

Exantema viral é considerado problema comum em regiões tropicais, afetando principalmente crianças. Diversos exantemas cutâneos estão associados a infecções por Enterovirus. Amostras biológicas provenientes de uma criança apresentando exantema generalizado foram enviadas ao Laboratório de Vírus Entéricos do Instituto Adolfo Lutz para a realização do diagnóstico laboratorial. Amostra viral isolada em RD (human rhabdomyosarcoma cells) foi submetida à reação em cadeia pela polimerase apresentando um produto de 437 pares de base, característico de gênero Enterovirus. O sorotipo echovirus 6 (E-6) foi identificado por ensaio de imunofluorescência indireta. Em adição, as amostras pareadas de soro apresentaram soroconversão para E-6. Até o momento, não há relatos do envolvimento de E-6 associado a doenças exantemáticas no Brasil, enfatizando a importância da vigilância epidemiológica para essas doenças e suas complicações.


Viral exanthems are a common problem in tropical regions, particularly affecting children. Various skin rashes have been reported in acute infections caused by Enterovirus. Biological samples from a child who presented generalized rashes were sent to the Enteric Virus Laboratory of the Adolfo Lutz Institute for laboratory diagnosis to be performed. A viral sample isolated from RD (human rhabdomyosarcoma cells) was subjected to the polymerase chain reaction and showed a 437-base pair product that was characteristic of the Enterovirus genus. Echovirus 6 (E-6) serotype was identified using the indirect immunofluorescence test. In addition, paired serum samples presented seroconversion to E-6. So far, there have not been any reports of E-6 involvement in exanthematic diseases in Brazil. Thus, the importance of epidemiological surveillance for these diseases and their complications is emphasized.
