ABSTRACT

Social determinants of health contribute to health disparities and inequities. We conducted a community forum on the topic of isolation with the objectives of (1) identifying and prioritizing key health-related issues needing attention in isolated communities in Puerto Rico; (2) developing strategies in terms of Policies, Programs, and Practices to address the community priorities we identified. We used the triangulation method for qualitative data, integrating the Colorado State University's Tri-ethnic Center Model and the Delphi Technique for a better understanding of community health needs and priorities. The five community health-related priorities identified in the community forum were: (1) access to health services (physical and mental); (2) older adults; (3) access to basic services; (4) preparedness for future disasters/emergencies; and (5) COVID-19 and access to vaccination. The Alliance Leaders and Advisory Boards understand that we will work with the priorities of preparedness for future natural disasters/emergencies and COVID-19 and access to vaccination. Fifteen strategies were developed for these priorities and were grouped into five areas that require more attention in order to reduce health disparities. Isolated communities in Puerto Rico present an intersectionality of factors that affect a wide range of health-related risks and outcomes.

ABSTRACT

Introduction: The consequences of the Covid-19 epidemic have been catastrophic for Latin America in 2021. This study explores experiences, lessons learned, and practice changes during this critical time in post-graduate medical education in Latin America. Me-thods: A panel of 53 post-graduate medical education leaders from 8 Latin American countries and Canada was invited to participate in the 2021 Latin American Medical Education Leaders Forum to share their experiences, lessons learned, and main educational practice changes given the Covid-19 pandemic scenario. Participants were selected following a snowball technique with the goal of obtaining a diverse group of experts. Small group discussions were conducted by bilingual facilitators based on a semi-structured questionnaire. The plenary session with the main conclusions of each group was recorded and fully transcribed for a thematic analysis using a framework methods approach. Results: Participants ́ profiles included 13 experienced clinician-educators, 19 program directors, and 23 deans or organizational representatives. Seven specific themes emerged. They followed a pattern that went from an initial emotional reaction of surprise to a complex collective response. The responses highlighted the value of adaptability, the application of new digital skills, a renovated residents' protagonism, the strengthening of humanism in medicine, the openness of new perspectives in wellness, and finally, an unresolved challenge of assessment in medical education in a virtual post-pandemic scenario. Conclusion: A diverse panel of medical educators from Latin America and Canada identified changes triggered by the Covid-19 pandemic that could transform postgraduate medical education in the region.

ABSTRACT

The BRICS Forum, an independent international organization encouraging commercial, political, and cultural cooperation between Brazil, Russia, India, China, and South Africa, was formed in 2011, and these countries have a significant influence on their regional affairs. These nations were hit by COVID-19 at different times, and all adopted home quarantine to reduce the spread of the virus. We present a comparative analysis of actions of psychology and potential outcomes during the COVID-19 pandemic in BRICS nations regarding five aspects: psychology in health policies, social roles of psychology, socioeconomic context, actions for the general population, and health professionals during stage 1 of the pandemic, and possible actions in stage 2. Various types of actions were taken by psychologists in BRICS, with different levels of coordinated cooperation with respective governmental and non-governmental organizations, multiple and parallel efforts from different scientific societies, and professional regulatory agencies. Scientific societies have had an important role in coordinating some of these efforts, especially because they congregate the psychologists from different parts of these countries, improving communication and access to key information. The aim of these actions varies from improving situational skills and competences to increase the accessibility of psychological services and provide psychoeducation and telepsychology. We will consider the social importance of these actions within these countries as a global opportunity for psychology to stage in a complex context involving human health. The way psychology in BRICS will face this challenging situation is likely to produce important regional influence, stimulate scientific contribution, and increase the accessibility of psychology.

ABSTRACT

ABSTRACT The case study presented, seeks to understand the corporate social responsibility (CSR), analyzing its value chain from the Model of the 31 practices of the supply chain that the World Economic Forum (2015). It is a qualitative research, with a descriptive case study approach. The objective of the work is to observe the impact of international logistics operations in a Mexican exporting SME and to analyze whether its economic significance has had a sustainable social, economic and environmental development. The results indicate a competitive advantage in product differentiation based on Porter and Kramer's (2002) concept of shared value. According to the WEF Model, the archetype is: Liberal humanist, since it emphasizes business value by 65% and weights socio-environmental aspects by 35%. However, impacts on the supply chain must be mitigated if the organization is to be identified as a socially responsible company.

ABSTRACT

The pressure for every research article to tell a clear story often leads researchers in the life sciences to exclude experiments that 'did not work' when they write up their results. However, this practice can lead to reporting bias if the decisions about which experiments to exclude are taken after data have been collected and analyzed. Here we discuss how to balance clarity and thoroughness when reporting the results of research, and suggest that predefining the criteria for excluding experiments might help researchers to achieve this balance.

ABSTRACT

Abstract In Brazil, one of the fundamental principles of the Brazilian National Health System is social participation. Through mobilization, indigenous peoples secured the publication of the law establishing the Indigenous Health Subsystem in 1999, structured in 34 Special Indigenous Health Districts. From the beginning, participation instances were organized: Local Councils, District Councils of Indigenous Health (Condisi) and the Condisi Presidents Forum (FPCondisi) This study aims to understand the formal structure and effective configuration of the social participation space of indigenous people in the construction of a differentiated health policy. A qualitative methodology was used with several sources and materials, with documentary analysis of minutes of Condisi Litoral Sul and FPCondisi meetings, legislation and with in-depth interviews with indigenous people and indigenists. The results showed that there are several ways for indigenous people to participate in health policy. It is possible to state that most of the interviewees recognizes Condisi as a space for dialogue between indigenous people and the government, but they also point out the limits of the effectiveness of this and other instances of social control. The silencing of indigenous agendas in formal participation spaces makes these people seek for other ways to lead the construction of a differentiated health policy.

ABSTRACT

OBJECTIVES: The recent development of value frameworks to inform healthcare resource allocation responds to a demand to make the decision-making process more inclusive and explicit. The objectives of the 2018 Latin American (LAtam) Health Technology Assessment International (HTAi) Policy Forum were to explore the current international experiences and to discuss the potential application of value frameworks in Latin America. METHODS: A background paper, presentations, and group discussions of Policy Forum members (43 participants, 12 LAtam countries represented) at the 2018 HTAi Policy Forum meeting informed this paper. RESULTS: Participants agreed that HTA and decision making based on more comprehensive and inclusive value frameworks could improve health system effectiveness, efficiency, sustainability, and equity; promote transparency in the decision process; sustain a more comprehensive assessment of technologies; and facilitate stakeholder participation as well as accountability of decisions. Criteria that were identified as essential to be included in a value framework for LAtam were burden of illness and severity of the disease, effectiveness and safety of the technology, quality of the evidence, cost-effectiveness, and budget impact. Potential challenges identified for the application of value frameworks in LAtam, included scarcity of human resources and delays in the assessment process. CONCLUSIONS: Forum participants agreed that the next steps should be to identify appropriate processes and methodologies, adapted to the context of each country, regarding the application of value frameworks to improve the link between HTA and decision making.

ABSTRACT

BACKGROUND: Updated urticaria guidelines recommend that patients should be assessed for disease activity, severity, control, and quality of life at baseline and follow up. Regarding treatment, guidelines consider second generation antihistamines as the cornerstone in therapy for chronic urticaria (CU), while other drugs, such as omalizumab, are conceived as second-line alternatives. In regards to omalizumab, despite advances in the management of CU, there are still open questions about timing, dosing, and objective measures for clinical response. This study was designed to portray the use of patient-reported outcomes (PROs) in chronic urticaria management, as well as the effectiveness and treatment patterns of omalizumab in CU, as seen in a real-life setting in Latin America. METHODS: This is a retrospective observational study, involving 72 Latin American patients with chronic urticaria treated with omalizumab. Patient reported outcomes and treatment patterns, response, quality of life improvement and discontinuation were analyzed. RESULTS: From the 72 patients, 91.7% (n = 66) were assessed through PROs, where urticaria control test (UCT) was the most used (79.2%; n = 57). Overall, 80.0% (n = 44) responded to omalizumab at some point of the treatment. Omalizumab 300 mg was associated with earlier response compared to lower doses. Regardless of dosage, most patients assessed with CU-Q2oL improved quality of life (80.8%; n = 21). With respect to omalizumab discontinuation, 20.8% (n = 15) patients interrupted omalizumab before the 3rd month of treatment (p = .000). CONCLUSIONS: The present study highlights how the use of PROs and omalizumab in Latin America differ from guidelines' recommendations and clinical trials. Even though most patients were initiated under omalizumab 300 mg, most of them finished with lower doses. Regardless of dosage, most patients responded to omalizumab and improved quality of life at some point during treatment. However, such features were seen earlier with omalizumab 300 mg. Regarding treatment discontinuation, one-fifth of patients interrupted omalizumab before the third month.

ABSTRACT

Abstract Introduction: Social networks are increasingly used in education due to its ease of use, familiarization, accessibility, making it possible to exchange information instantly and easily from smartphones. For this reason, a virtual forum was designed and created through a closed group of Facebook that worked in parallel with the subject. Objective: To evaluate the opinion of students in relation to the development of attitudinal, cognitive, self-regulatory and metacognitive skills through the virtual forum implemented, as well as their perception regarding the Facebook platform. Methodology: Cross-sectional descriptive study of mixed type. Quantitative with the application of a Likert opinion survey created by Andrei Fedorov (2008) and qualitative through four focus groups. 68 students participated (28 men and 40 women), who took the Integrated Diagnostic course in 2017, corresponding to 100% of them. Results: For 70% of the questions, the most selected option was -quite-; except for four of the attitudinal dimension where the most frequent was -a little-. They highlighted the Facebook platform as an excellent idea to encourage learning, offering more advantages than other platforms. The mentioned disadvantages arose from students with less participation and regular / low performance. Conclusion: The virtual forum, through Facebook, allowed to improve the attitudinal, cognitive, self-regulating and metacognitive dimensions, mainly in women and in those who participated more, obtaining better performance. It turned out to be a better virtual platform given that it is friendly, massive and instantaneous which encourages student motivation.

ABSTRACT

(AU)

The article intends to work out the questions: What is teaching? What is teaching psychoanalysis? What is teaching psychoanalysis in the Forums of the Lacanian Field? With the survey of these questions, it discusses the nuances of the concepts of transmission and teaching in the context of the text studies and Lacanian the ory for those who approach the Forums of the Lacanian Field. As a conclusion, it proposes that the teaching of psychoanalysis (in the “extensive” section) must be ethically subordinated to its transmission (in the “intensive” section). (AU)

ABSTRACT

Abstract This paper aims to describe and to analyze the foundation of Forums of the services composing the Network for Psychosocial Care in the territory of a Management District of Porto Alegre, Rio Grande do Sul, Brazil. It is a qualitative study performed with intervention-research from March 2014 to March 2015. To analyze this process, we used field diary as a tool to record the Forums and assessment reports of trainings seminars in mental health, performed with primary healthcare professionals. The Forum arrangement, with professionals of several services, defines a process that aims to join different points in the Network for Psychosocial Care, promoting interlocution among the services and co-responsibility for management-execution of the territory healthcare. From the relations emerged in the Forums, we propose to rethink work processes, questioning the practices and knowledges in health, alluding to Permanent Education in Health as an important strategy in this setting. As initial results, we think the monthly meetings provided reflection spaces and proposed construction of flows and connections among work, management, and Permanent Education, aspects considered inseparable in the public health filed.

ABSTRACT

Cancer is the leading cause of death in Puerto Rico, suggesting a need for improved strategies, programs, and resources devoted to cancer prevention. Enhanced prevention needs in Puerto Rico were initially identified in pilot studies conducted by the Ponce School of Medicine (PSM) in collaboration with the H. Lee Moffitt Cancer Center (MCC). In the current study, we used community engagement to identify specific needs in cancer prevention and education and strategies to create culturally attuned, effective cancer prevention education programs. A total of 37 participants attended a community forum and were assigned to one of three discussion groups: patients/survivors (n = 14); family/caregivers (n = 11); or healthcare providers (n = 12). Most participants were women (73 %), over 35 years of age, and a majority were married (58 %) and had a university education (81 %). The sessions were recorded and transcribed and analyzed for key themes. Participants wanted improved awareness of cancer prevention in Puerto Rico and believed cancer prevention education should start early, ideally in elementary school. Participants also stressed the importance of creating partnerships with private and government agencies to coordinate educational efforts. Suggested strategies included outreach to communities with limited resources, incorporating the testimony of cancer survivors, and utilizing social media to disseminate cancer prevention information.

ABSTRACT

In this paper we present the authors' experience of teaching a course in Ethics for Engineers, which has been delivered four times in three different universities in Spain and Chile. We begin by presenting the material context of the course (its place within the university program, the number of students attending, its duration, etc.), and especially the intellectual background of the participating students, in terms of their previous understanding of philosophy in general, and of ethics in particular. Next we set out the objectives of the course and the main topics addressed, as well as the methodology and teaching resources employed to have students achieve a genuine philosophical reflection on the ethical aspects of the profession, starting from their own mindset as engineers. Finally we offer some results based on opinion surveys of the students, as well as a more personal assessment by the authors, recapitulating the most significant achievements of the course and indicating its underlying Socratic structure.

ABSTRACT

The primary data were gathered through unsystematic observation carried out during the meetings. Descriptive/qualitative procedures were used for dealing with the data. The analyses of the collected data have shown that the undertakings boast a strong presence during the meetings and that marketing has been the pertinent demand for said undertakings. It has been concluded that the solidarity economy undertakings are being strengthened and are players that contribute in building their own story. Nevertheless, those undertakings are still strongly geared toward meeting their immediate needs, such as production flow. It has been verified that the matters related to the public and strategic debate of the Solidarity Economy of the Municipality still lack the proper space in the Forum as proposals of a movement in opposition to the current economic system.(AU)
