ABSTRACT

Capillary dried blood spot (DBS) samples facilitate field-based collection without venipuncture. This pilot study aims to evaluate the viability of creatine (Cr) and creatinine (Crt) quantification using fresh capillary serum (CrS/CrtS) and DBS samples (CrDBS/CrtDBS), using Flow Injection Analysis Mass Spectrometry (FIA - MS). Nine Olympic Athletes provided a capillary blood sample to assess CrS/CrtS and CrDBS/CrtDBS quantified by FIA - MS. No difference between CrtS (mean ± SD: 813.6 ± 102.4 µmol/L) and CrtDBS (812.4 ± 108.1 µmol/L) was observed with acceptable variance [SEM 88.7; CV 10.7%; ICC 0.57 (CI 95% 0.06 - 0.84)] and agreement [very strong (Spearman: r = 0.77; p < 0.01) or strong (Pearson: r = 0.56; p = 0.04); Bland Altman: lower (-193) and upper (+196) limits of agreement]. CrS (mean ± SD: 691.8 ± 165.2 µmol/L) was significantly different to CrDBS (2911 ± 571.4 µmol/L) with unacceptable variance [SEM 171.6; CV 27%; ICC 0.002 (CI 95% -0.02 - 0.07)] and 'weak' agreement [Spearman: r = 0.21, p = 0.47 and Pearson: r = 0.06, p = 0.84; Bland Altman lower (-3367) and upper (-1072) limits of agreement]. Crt quantification is viable using both CrtS and CrtDBS (but not for Cr and CrS/CrDBS), with the DBS tissue handling technique offering several methodological and practice facing advantages. Future work should expand upon the sample size, explore sport/discipline relevant analytes across a full competitive season, including key training, recovery and performance blocks of their periodized performance plan.

ABSTRACT

RESUMEN El ejercicio físico evita las enfermedades cardiovasculares pero cuando es intenso debe tener planificación y dosificación correctas y, de ser posible, es muy útil contar con una prueba ergométrica previa y un especialista orientador; porque también puede producir accidentes cardiovasculares en deportistas de alto rendimiento, entre otras cosas, por no cumplir con una programación bien definida con un basamento científico. La complicación más temida es la muerte súbita cardíaca, habitualmente por arritmias malignas y enfermedad isquémica. Se presenta el caso de un deportista de alto rendimiento, de 34 años de edad, en fase desentrenamiento y sin factores de riesgo cardiovascular u otros antecedentes de interés, que sufrió un infarto agudo de miocardio anterior extenso, con angina postinfarto, debido a una estenosis suboclusiva de la descendente anterior proximal. Se realizó angioplastia de rescate, con implantación de un stent farmacoactivo y el paciente evolucionó favorablemente.


ABSTRACT Physical exercise avoids cardiovascular diseases but when it is intense, it must have correct planning and dosage and, if possible, a previous ergometric test and a guidance specialist could be very useful; because it can also cause cardiovascular events in high performance athletes, among other things, for not complying with a well-defined schedule with a scientific base. The most feared complication is sudden cardiac death, usually due to malignant arrhythmias and ischemic heart disease. Here is presented the case of a 34-year-old high-performance athlete, in the detraining phase and without cardiovascular risk factors or other history of interest, who suffered an extensive acute myocardial infarction, with post-infarction angina, due to a stricture suboclusive of the proximal left anterior descending artery. A rescue angioplasty was performed, with implantation of a drug-eluting stent and the patient evolved favorably.

La osteolisis postraumática de la clavícula es una entidad de presentación rara que consiste en una osteopenia severa del extremo distal de este hueso, causada por un trauma agudo o por microtraumas a repetición. Se presenta un caso de osteolisis postraumática de la clavícula en deportista de alto rendimiento practicante de lucha grecorromana al que se realizó como tratamiento una resección del extremo distal de la clavícula. La evolución del paciente fue muy satisfactoria, recuperó íntegramente el movimiento del hombro, y el dolor desapareció. A los 2 meses de la cirugía comenzó el entrenamiento progresivo y a los 4 meses ya estaba incorporado totalmente al programa de competencias de su deporte(AU)


Post traumatic osteolysis of the clavicle is a rare presentation entity consisting of a severe osteopenia of the distal end of the bone, caused by acute trauma or microtrauma to repeat. A case of post-traumatic osteolysis of the clavicle is presented in a high performance wrestling athlete who received as treatment resection of the distal end of his clavicle. Patient outcome was very satisfactory, shoulder movement was fully recovered, and the pain disappeared. Two months after surgery, progressive training began. Four months after, he was fully incorporated into the competitions program of his sport(AU)


L'ostéolyse post-traumatique de clavicule est une affection rare consistant à une ostéopénie sévère de l'extrémité distale de cet os, causée par un traumatisme aigu ou par microtraumatismes répétés. Un cas d'ostéolyse post-traumatique de clavicule chez un athlète de lutte gréco-romaine, traité par résection de l'extrémité distale de la clavicule, est présenté. L'évolution du patient a été très satisfaisante, il a complètement récupéré le mouvement de l'épaule et la douleur a disparu. Deux mois après la chirurgie, il a commencé progressivement à entraîner, et au bout de quatre mois il s'est tout à fait réincorporé au programme de compétitions de son sport(AU)

Introducción: el control médico del entrenamiento deportivo lleva implícita la medición de parámetros hematológicos y de los índices eritrocitarios, incluido el conteo de reticulocitos, además de controles bioquímicos para velar por el estado de salud del deportista y obtener mejores resultados deportivos.Objetivo: analizar el comportamiento de las variables de los estudios hematológicos realizados a los deportistas de las preselecciones que participaron en los juegos Panamericanos Guadalajara 2011.Método: se realizó un estudio descriptivo transversal que tuvo como universo a 804 deportistas de alto nivel que integraban las preselecciones nacionales que se preparaban para constituir los equipos nacionales que participarían en los Juegos Panamericanos en Guadalajara, México, 2011. A todos se les realizó el perfil hematológico como parte del pasaporte biológico establecido para este tipo de competición. Además, a 328 se les realizó electroforesis de hemoglobina.Resultados: se encontraron cifras de hemoglobina y hematocrito significativamente más bajas en el sexo femenino. La concentración de hemoglobina corpuscular media presentó diferencias significativas por grupo de deporte. En los deportistas a quienes se les realizó electroforesis de hemoglobina se encontraron 15 AS.Conclusiones: los hallazgos de esta investigación son de gran utilidad para el diagnóstico y seguimiento de la salud de los deportistas de alto rendimiento sometidos a fuertes cargas físicas de trabajo(AU)


Introduction: medical management of sports training implies the measurement of hematological parameters, red cell indices, reticulocyte count and biochemical controls to care for the athletes´ health condition and to obtain higher sports scores. Objective: to analyze the behavior of the variables in hematological studies of athletes who participated in the presets Pan American Games Guadalajara 2011. Methods: a descriptive cross-sectional study was carried out with a universe of 804 high-performance athletes from national teams, who were training to participate in the Pan American Games in Guadalajara, Mexico, 2011. Hematological profile was performed as part of the biological passport established for this type of competition. Additionally, a hemoglobin electrophoresis was carried out in 328 of them. Results: significantly lower hemoglobin and hematocrit figures were found in females. The Mean Corpuscular Hemoglobin Concentration (MCHC) showed significant differences by group of sports. In athletes who underwent electrophoresis, Hb 15 AS were found. Conclusions: the findings of this research are useful for diagnosis and health monitoring of athletes of high-performance, under heavy physical workloads.

ABSTRACT

Se efectuó un estudio descriptivo y transversal de 124 atletas de alto rendimiento, de la provincia de Santiago de Cuba, desde febrero hasta septiembre del 2009, con vistas a caracterizar desde el punto de vista clinicoepidemiológico los afectados con trauma dentario. En la casuística prevalecieron los deportistas con algún tipo de lesión traumática en sus dientes (60,5 %) y la fractura no complicada de corona (55 dientes afectados) como la más frecuente. Tuvieron mayor afectación el incisivo central superior (88,9 %) y la arcada superior (82,8 %), así como los deportes de lucha (78,1 %) y hockey sobre césped (63,9 %). Se concluyó que 6 de cada 10 atletas presentaron alguna lesión traumática en su dentadura.


A descriptive and cross-sectional study of 124 high-performance athletes in Santiago de Cuba province was carried out from February to September, 2009, aimed at characterizing the affected athletes with dental traumatism from the clinical and epidemiological points of view. Sportsmen with some type of traumatic lesion in their teeth (60.5 %) prevailed in the case material, and the non complicated fracture of crown (55 affected teeth) prevailed as the most frequent. The superior central incisive (88.9 %) and the superior dental arcade (82.8%) were more affected, as well as the sports wrestling (78.1 %) and hockey (63.9 %). It is concluded that 6 every 10 athletes presented some traumatic lesion in their dentition.

ABSTRACT

INTRODUCTION: The goal of this retrospective study was to evaluate the safety and efficacy of ibandronate for bone marrow oedema (BMO) syndrome and stress fracture cases, and to demonstrate an additional field of therapeutic importance-the high-performance athlete. PATIENTS AND METHODS: This retrospective study included twenty-five high-performance athletes. Sixty per cent of the athletes were European soccer players and 40.0% other high-class international athletes (3 women and 22 men with an average age of 25.0±4.2), with BMO of the lower trunk or extremity diagnosed by magnetic resonance imaging (MRI). The treatment regimen consisted of high-dose vitamin D supplementation and intravenous ibandronate therapy. RESULTS: The time between the onset of pain and proper diagnosis of BMO was 106.3±104.1 days. Excellent pain reduction (pain at rest and under strain) and improved mobility was reported within the first two weeks after the first ibandronate administration by sixteen patients (64%). The time from first treatment until return to competition (RTC) was on average 102.6±65.2 days in total. If the time from onset of pain until diagnosis was within 40 days, the RTC was significantly reduced (p≤0.05) to almost 50% (63.8±48.1 days) when compared to the athletes with later diagnosis (124.4±63.2 days). CONCLUSIONS: The here-applied therapy regimen of intravenous BPs application and vitamin D supplementation in BMO syndrome has a beneficial effect for high-performance athletes. An early diagnosis and rapid treatment start can reduce the RTC significantly. An optimal bone metabolism with sufficient daily calcium and vitamin D intake is crucial and should not only be strived for the professional but also for the recreational athlete.
