ABSTRACT

Objective To investigate the preventive effect of highly effective antiretroviral therapy (HARRT) combined with SMZ-TMP on opportunistic infection (OIs) in patients with HIV/AIDS. Methods The 6-month follow-up data of 170 AIDS patients in the Department of Infectious Diseases of Nantong Third People's Hospital were analyzed retrospectively. All patients received HARRT. According to whether SMZ-TMP was used for OIs preventive treatment, the patients were divided into a prevention group (n=47) and a non-prevention group (n=123). The demographic data such as gender, age, occupation and education level of the two groups were collected. The incidence of OIs in the 12-month follow-up period of the two groups was analyzed and compared, and the single factor unconditional logistic regression analysis and multi factor unconditional logistic regression analysis were carried out for AIDS patients taking SMZ-TMP. Results The proportion of prophylactic use of SMZ-TMP in 170 AIDS patients was only 27.65%, and the incidence of OIs in the prevention group was 31.91%, which was significantly lower than that in the non-prevention group (57.72%) (χ2=9.062, P2=7.419, P2=4.473, 4.322, 4.350, 3.982, 5.656, P<0.05). Univariate and multivariate unconditional logistic regression analysis showed that the benefits of SMZ-TMP, doctors' detailed recommendation of SMZ-TMP and higher education level were all influencing factors (95% CI: 135.79-3748.36, 107.43-2954.67, 1.873-25.94). Conclusion Prophylactic use of SMZ-TMP significantly reduced the incidence of AIDS related OIs, but the proportion of SMZ-TMP use was relatively low. It is necessary to take relevant measures according to the influencing factors of taking SMZ-TMP to improve the use rate of SMZ-TMP in AIDS patients.

Resumen Introducción: la baja adherencia en el tratamiento antirretroviral de alta efectividad TARGA se asocia a peores desenlaces en salud; además de las comorbilidades infecciosas, muchos pacientes con VIH/SIDA presentan asociado psicopatologia, farmacodependencia y problemas socioeconómicos. Existe alta frecuencia de abandono a la terapia en nuestro medio. Objetivo: identificar las variables asociadas al abandono de terapia TARGA en pacientes con VIH de un hospital de tercer nivel en Colombia. Métodos y resultados estudio descriptivo corte transversal: se incluyeron 51 pacientes pertenecientes al programa durante un año; 12 pacientes (23%) tuvieron adherencia al tratamiento, 32 (62%) tenían antecedente de consumo de sustancias psicoactivas y 33 (64%) tenían asociada alguna patología psiquiátrica. Los factores que se asociaron a la no adherencia fueron la psicopatología y la pobre red de apoyo. Conclusión: En la población a estudio se documenta alta tasa de abandono a la terapia TARGA, probablemente asociado a enfermedad mental y pobre red social. (Acta Med Colomb 2018; 43: 31-36).


Abstract Introduction: the low adherence in the highly effective antiretroviral treatment HAART is associated with worse health outcomes. In addition to infectious comorbidities, many patients with HIV / AIDS have associated psychopathology, drug dependence and socioeconomic problems. There is a high frequency of abandonment of therapy in our environment. Objective: to identify the variables associated with the abandonment of HAART therapy in patients with HIV from a third level hospital in Colombia. Methods and Results: descriptive cross-sectional study: 51 patients belonging to the program during one year were included; 12 patients (23%) had adherence to treatment, 32 (62%) had a history of psychoactive substance use and 33 (64%) had some psychiatric pathology. The factors that were associated with non-adherence were psychopathology and the poor support network. Conclusion: high rate of abandonment of HAART therapy is documented in the study population, probably associated with mental illness and poor social network. (Acta Med Colomb 2018; 43: 31-36).

ABSTRACT

Objetivo: estimar el riesgo cardiovascular y la prevalencia de síndrome metabólico de un grupo de pacientes VIH positivos de la región caribe colombiana. Métodos: se llevó a cabo un estudio descriptivo de un grupo de 90 pacientes de ambos géneros, VIH positivos que asisten al programa de control de la EPS SURA. Se estimó la frecuencia síndrome metabólico, riesgo cardiovascular y el tipo de esquema de tratamiento. Resultados: 72,2% son hombres con edad media de 39.7 años (IC95%: 37.3-42.1) la edad promedio general fue de 40.9 años (IC95%: 38.7-43.0). El 5.4% eran diabéticos y 14.1% fueron hipertensos. El 87.8% recibía algún esquema de tratamiento. La frecuencia de síndrome metabólico fue 46% en pacientes en tratamiento con inhibidores de proteasa frente a un 36% en los que no usan tratamiento. Casi la mitad del grupo tenía un índice de masa corporal (IMC) en sobrepeso y obesidad (36 y 13% respectivamente). Conclusiones: se detectó un porcentaje alto de insulinorresistencia (36%) en nuestros pacientes VIH positivos. Se requieren medidas eficaces para prevenir el curso natural de esta entidad. Objective: to estimate cardiovascular risk and the prevalence of metabolic syndrome in a group of HIV-positive patients in the Colombian Caribbean region. Methods: we conducted a descriptive study of a group of 90 patients of both genders HIV positive seen in the control program of EPS SURA . Frequency of metabolic syndrome, cardiovascular risk and type of treatment schedule, were estimated. Results: 72.2% were male with an average age of 39.7 years (95% CI: 37.3-42.1) overall average age was 40.9 years (95% CI: 38.7-43.0). 5.4% had diabetes and 14.1% were hypertensive. 87.8% received a treatment regimen. The frequency of metabolic syndrome was 46% in patients treated with protease inhibitors compared to 36% in those not being treated. Almost half of the group had a body mass index (BMI) in overweight and obesesity (36 and 13% respectively). Conclusions: we found a high percentage of insulin resistance (36%) in our HIV positive patients. Effective measures are required to prevent the natural course of this entity.
