ABSTRACT

Introduction: Low 25-hydroxyvitamin D [25(OH)D] levels occur after kidney transplantation (KTx), and can be associated with increase the risk of graft loss. This longitudinal study aimed to evaluate the vitamin D status and association with biomarkers of the renal graft function after KTx. Methods: this longitudinal study included 42 patients evaluated at baseline, 3 and 6 months after KTx. Biodemographic, clinical, and biochemical parameters such as 25(OH)D and parathyroid hormone (PTH), and biomarkers of renal graft function, such as creatinine, estimated glomerular filtration rate (eGFR), and albumin/creatinine ratio (ACR), were assessed. Sun exposure was also evaluated. Patients were categorized according to their 25(OH)D levels. Results: at baseline, 25(OH)D levels < 30 ng/mL were found in 43 % patients, and 38 % of these patients failed to improve their 25(OH)D levels by 6 months after KTx. Low 25(OH)D levels occurred regardless of sun exposure. Further, 44 % patients developed albuminuria at 6 months. An increased ACR was observed in patients with 25(OH)D levels < 30 ng/mL (p = 0.002) compared to that in patients with 25(OH)D > 30 ng/mL. Additionally, 25(OH)D levels were negatively correlated with ACR at 6 months post-KTx (r = -0.444; p = 0.003). Twelve (28.6 %) patients with 25(OH)D levels < 30 ng/mL showed no eGFR recovery until 6 months after KTx. Conclusion: low vitamin D levels and increased albuminuria were observed at 6 months after KTx, even in a region with high sun exposure. The association between vitamin D status and biomarkers of renal graft function after KTx should be explored in further studies. (AU)


Introducción: los bajos niveles de 25-hidroxivitamina D [25(OH)D] ocurren después del procedimiento de trasplante de riñón (KTx) y pueden estar asociados con un aumento del riesgo de pérdida del injerto. Este estudio longitudinal tuvo como objetivo evaluar el estado de la vitamina D y la asociación con los biomarcadores de función del injerto renal después del KTx. Métodos: este estudio longitudinal incluyó a 42 pacientes que fueron evaluados al inicio del estudio, y 3 y 6 meses después del KTx. Se evaluaron los parámetros biodemográficos, clínicos y bioquímicos, como 25(OH)D y hormona paratiroidea (PTH), y los biomarcadores de función del injerto renal, como creatinina, tasa de filtración glomerular estimada (eGFR) y relación albúmina/creatinina (ACR). También se evaluó la exposición al sol. Los pacientes se clasificaron según sus niveles de 25(OH)D. Resultados: al inicio del estudio se encontraron niveles de 25(OH)D < 30 ng/ml en el 43 % de los pacientes, mientras que el 38 % de estos pacientes no lograron mejorar sus niveles de 25(OH)D a los 6 meses después del KTx. Además,los niveles de 25(OH)D se correlacionaron negativamente con la ACR a los 6 meses después del KTx (r = -0,444; p = 0,003). Doce (28,6 %) pacientes con niveles de 25(OH)D < 30 ng/ml no mostraron recuperación de la TFGe hasta 6 meses después del KTx. Conclusión: se observaron niveles bajos de vitamina D y un aumento de la albuminuria a los 6 meses después del KTx, incluso en una región con alta exposición solar. La asociación entre el estado de la vitamina D y los biomarcadores de función del injerto renal después del KTx debeexplorarse en estudios adicionales. (AU)

Conclusión: de nuestros hallazgos concluimos que la hipovitaminosis D es muy frecuente entre los pacientes en HD y que su deficiencia está asociada un deterioro del estado nutricional y peor percepción de la calidad de vida. Antecedentes: La elevada prevalencia de hipovitaminosis D en España es considerada una verdadera epidemia con importantes implicaciones para la salud por las múltiples funciones que ejerce la vitamina D tanto a nivel esquelético como extraesquelético. Para que las personas con insuficiencia o deficiencia en vitamina D alcancen los niveles séricos más adecuados, deben recibir suplementos de vitamina D. Este estudio se realizó con la finalidad de evaluar si en la práctica clínica habitual, el manejo de la hipovitaminosis D era llevada a cabo según las recomendaciones internacionales establecidas por las sociedades científicas.Métodos: Se realizaron dos rondas de circulación de un cuestionario Delphi entre un panel formado por médicos prescriptores habituales de vitamina D.Resultados: En general, los médicos del panel reconocieron la alta prevalencia de la hipovitaminosis D en España, la necesidad del cribado en los distintos grupos de riesgo y los beneficios de la suplementación en los pacientes con insuficiencia o déficit de vitamina D. Sin embargo, no se alcanzó el consenso en algunas de las aseveraciones relacionadas con los métodos de cuantificación de la vitamina D o con las recomendaciones para el manejo de la hipovitaminosis D.Conclusiones: La ausencia de acuerdo para algunos de los ítems reveló la necesidad de realizar acciones formativas destinadas a proporcionar un conocimiento adecuado y actualizado sobre las evidencias científicas y las recomendaciones para la práctica clínica de la suplementación de vitamina D. ABSTRACT

OBJECTIVE: To provide practical recommendations for the management of mineral and bone metabolism alterations in pregnancy and lactation. PARTICIPANTS: Members of the Working Group on Osteoporosis and Mineral Metabolism of the Spanish Society of Endocrinology and Nutrition. METHODS: Recommendations were formulated according to the Grading of Recommendations, Assessment, Development, and Evaluation (GRADE) system to describe both the strength of recommendations and the quality of evidence. A systematic search was carried out in Medline of the available evidence for each pathology. Papers in English with publication date until 29 February 2020 were included. A methodologist resolved the differences that arose during the process of reviewing the literature and formulating recommendations. The recommendations were discussed and approved by all members of the Working Group. CONCLUSIONS: The document establishes practical recommendations based on evidence about the management of mineral and bone metabolism disorders in pregnancy and lactation.

ABSTRACT

El propósito de este estudio fue analizar el efecto de la deficiencia de vitamina D sobre la recurrencia de vértigo y la discapacidad auto percibida en pacientes diagnosticados con Vértigo Postural Paroxístico Benigno en un hospital de tercer nivel de la Ciudad de México. Se trata de un estudio retrospectivo y descriptivo. Se revisaron expedientes electrónicos (enero 2017 ­ diciembre 2019), de pacientes previamente diagnosticados, que presentaran valores deficientes de vitamina D sérica. Se contabilizó la cantidad de episodios de vértigo reportados, así como, la puntuación obtenida en el Dizzines Handicap Inventory para determinar el nivel de discapacidad auto percibida al momento del diagnóstico y a los 6 meses de seguimiento. Los niveles de vitamina D sérica de los pacientes seleccionados (n=30), demostraban hipovitaminosis (media de 17.1 ng/mL) al momento del diagnóstico. Seis meses después y habiendo recibido tratamiento con maniobras de reposicionamiento los pacientes reportaron desde ausencia absoluta de recurrencias (7 pacientes, 23.3%) hasta 8 recurrencias (1 paciente, 3.3%) en el mismo periodo. En cuanto al nivel de discapacidad auto percibida en la evaluación inicial predominó la discapacidad moderada (60%), mientras que, en la evaluación final, la preponderante fue la discapacidad leve (73.3%). No se encontró diferencia estadísticamente significativa entre la deficiencia de vitamina D y la recurrencia de episodios de vértigo. Se concluye que los resultados obtenidos posiblemente se deben a que la insuficiencia de vitamina D es muy común en la población mexicana, incluso en individuos sanos


The purpose of this study was to analyze the effect of vitamin D deficiency on vertigo recurrence and self-perceived disability in patients diagnosed with Benign Paroxysmal Postural Vertigo in a tertiary care hospital in Mexico City. This is a retrospective and descriptive study. Electronic records (January 2017 - December 2019) of previously diagnosed patients who presented deficient values of serum vitamin D were reviewed. The number of reported episodes of vertigo was counted, as well as the score obtained in the Dizzines Handicap Inventory to determine the level of self-perceived disability at the time of diagnosis and at 6-month follow-up. The serum vitamin D levels of the selected patients (n=30) showed hypovitaminosis (mean 17.1 ng/mL) at the time of diagnosis. Six months later and having received treatment with repositioning maneuvers, the patients reported from absolute absence of recurrences (7 patients, 23.3%) to 8 recurrences (1 patient, 3.3%) in the same period. Regarding the level of self-perceived disability in the initial evaluation, moderate disability predominated (60%), while, in the final evaluation, the predominant one was mild disability (73.3%). No statistically significant difference was found between vitamin D deficiency and recurrence of vertigo episodes. It is concluded that the results obtained are possibly due to the fact that vitamin D insufficiency is very common in the Mexican population, even in healthy individuals.

Objetivo: El papel de la dieta como determinante relevante de hipovitaminosis D no está claro. El objetivo del estudio fue analizar su impacto en la prevalencia de hipovitaminosis D en población infantil sana española. Métodos: Mediante un diseño observacional se estudiaron en una muestra de población pediátrica, entre 4 y 14 años, datos demográficos, antropométricos, nutricionales, analíticos y el nivel de 25(OH) D mediante enzimo-inmuno-análisis. Se obtuvieron encuestas dietéticas mediante recordatorio de 24 horas evaluadas mediante el programa informático DietSource 3.0. La probabilidad de hipovitaminosis se analizó mediante regresión logística. Resultados: Se reclutaron 281 niños sanos con una edad media 9,0 años. La prevalencia de hipovitaminosis D (<20 ng/ml) fue de un 18,15% y la de déficit grave (<10 ng/ml) del 1,4%. La etnia, la estacionalidad, el fototipo de piel y el tiempo de exposición solar se asociaron significativamente a la presencia de hipovitaminosis D. La distribución de nutrientes no mostró diferencias entre los grupos con y sin hipovitaminosis salvo en la Piridoxina (vitamina B6) y los ácidos grasos saturados. Conclusiones: La dieta juega un papel reducido como factor de riesgo de hipovitaminosis D en población infantil sana y los factores relevantes son los relacionados con la exposición al sol. Un adecuado estilo de vida al aire libre, exposición solar libre de protectores solares y patrones dietéticos que aseguren una ingesta correcta de vitamina D y calcio siguen siendo las recomendaciones idóneas para la población general. La utilización de suplementos se debe limitar a los grupos de riesgo. (AU)


Objetives: It is not clear whether diet in the Spanish general population is also a relevant determinant of hypovitaminosis D. The objective of the study was to analyze the impact of diet on the prevalence of hypovitaminosis D in healthy children. Methods: Demographic, anthropometric, nutritional, analytical data and vitamin D (25 (OH) D) level were studied using an enzyme-immuno-analysis using an observational design in a sample of the pediatric population between 4 and 14 years old. The 24-hour reminder diet survey was evaluated with the DietSource 3.0 software. The probability of hypovitaminosis was analyzed using logistic regression. Results: 280 healthy children with a mean age of 9.0 years were recruited. The prevalence of hypovitaminosis D (<20 ng/ml) was 18.15% and that of severe deficit (<10 ng/ml) 1.4%. Ethnicity, seasonality, skin phototype, and time of sun exposure were significantly associated with the presence of hypovitaminosis D. The distribution of nutrients did not show differences between the groups with and without hypovitaminosis except for Pyridoxine B6 and saturated fatty acids. Conclusions: Diet plays a reduced role as a risk factor for hypovitaminosis D in healthy children and the relevant factors are those related to sun exposure. An adequate outdoor lifestyle, sun exposure free of sunscreens and dietary patterns that ensure a correct intake of vitamin D and calcium remain the ideal recommendations for the general population. Supplementation should be limited to risk groups. (AU)

Background and objectives: despite long hours of sunlight in Spain, vitamin D deficiency has been rising of late. We aimed to describe vitamin D deficiency and insufficiency in at-risk patients in La Rioja, a region in northern Spain. Methods: a retrospective, cross-sectional study involving 21,490 patients (74.3 % women) aged 14-105 years, who were primary health care (PHC) users in La Rioja, with an available 25-hidroxyvitamin D (25(OH)D) test result corresponding to the first request from January 2013 to October 2015. Vitamin D deficiency, insufficiency, and sufficiency were defined as 25(OH)D levels < 12 ng/mL, 12-20 ng/mL, and ≥ 20 ng/mL, respectively. Hypovitaminosis D was defined considering 2 cut-off points for 25(OH)D: < 20 ng/mL and < 30 ng/mL. Results: mean 25(OH)D levels were 18.3 (SD, 11.6) ng/mL in the whole sample, being lower in men than in women (17.6 vs 18.5 ng/mL, p < 0.001). Significantly higher 25(OH)D levels were found in the groups with 55-59, 60-64, 65-69, and 70-74 years of age (range, 19.4-20.4 ng/mL), and lower levels were found in the groups with 80-85 years and older (16.7 and 14.5 ng/mL), as compared to other groups. Two thirds of the population were vitamin D-deficient (< 12 ng/mL, 30.9 %) or insufficient (12-20 ng/mL, 32.8 %), and only 32.8 % of patients were found to be vitamin D-sufficient (> 20 ng/ml). Justificación y objetivos: a pesar de las largas horas de exposición solar en España, la deficiencia de vitamina D ha ido en aumento. Nuestro objetivo fue describir la deficiencia e insuficiencia de vitamina D en pacientes de riesgo en La Rioja, España. Métodos: estudio transversal retrospectivo en el que participaron 21.490 pacientes (74,3 % mujeres), usuarios de Atención Primaria (APS) en La Rioja, con resultado de prueba de 25-hidroxivitamina D (25(OH) D) disponible, correspondiente a las solicitudes efectuadas entre enero de 2013 y octubre de 2015. La deficiencia, insuficiencia y suficiencia de vitamina D se definieron como niveles de 25(OH)D < 12 ng/mL, = 12-20 ng/mL y ≥ 20 ng/mL, respectivamente. La hipovitaminosis D se definió considerando 2 puntos de corte de 25(OH)D: < 20 ng/mL y < 30 ng/mL. INTRODUCTION: Background and objectives: despite long hours of sunlight in Spain, vitamin D deficiency has been rising of late. We aimed to describe vitamin D deficiency and insufficiency in at-risk patients in La Rioja, a region in northern Spain. Methods: a retrospective, cross-sectional study involving 21,490 patients (74.3 % women) aged 14-105 years, who were primary health care (PHC) users in La Rioja, with an available 25-hidroxyvitamin D (25(OH)D) test result corresponding to the first request from January 2013 to October 2015. Vitamin D deficiency, insufficiency, and sufficiency were defined as 25(OH)D levels < 12 ng/mL, 12-20 ng/mL, and ≥ 20 ng/mL, respectively. Hypovitaminosis D was defined considering 2 cut-off points for 25(OH)D: < 20 ng/mL and < 30 ng/mL. Results: mean 25(OH)D levels were 18.3 (SD, 11.6) ng/mL in the whole sample, being lower in men than in women (17.6 vs 18.5 ng/mL, p < 0.001). Patients over 75 years exhibited high rates of deficiency or insufficiency across all seasons of the year. Conclusions: in La Rioja, Spain, hypovitaminosis D was highly prevalent among PHC users, and especially in older age groups and during the winter months.

BACKGROUND: Juvenile osteochondritis dissecans (JOCD) is an acquired joint disease of unknown etiology that affects skeletally immature patients and has the potential to progress to osteoarthritis. Recent studies have reported a high prevalence of vitaminD deficiency in patients with osteochondral lesions. The aim of our study was to determine serum vitaminD levels in patients diagnosed with JOCD. METHODS: Serum 25(OH)D levels of 31 patients (22 males) presenting 40 lesions (29 JOCD of the knee, and 11 of the ankle) were evaluated. The average age was 11.9±2.9years. HypovitaminosisD was defined as a value less than 30ng/mL and was divided into vitaminD insufficiency (20 to 30ng/mL) and vitaminD deficiency (<20ng/mL). RESULTS: HypovitaminosisD was present in 45.2% of the evaluated patients (32.2% insufficiency and 13% deficiency). No significant differences were found in the mean values and incidence of hypovitaminosis between those patients in which the sample was taken in warm or cold season (P=.267 and P=.875, respectively). Patients who required surgery had a higher incidence of hypovitaminosis than those treated conservatively (60% versus 31%, P=.054). There was no correlation in the incidence of hypovitaminosis with sex, location, stability of the lesion, or if the lesion was uni- or bilateral. CONCLUSION: In our series, almost half of the patients diagnosed with JOCD presented abnormal serum levels of vitaminD. A two-fold incidence of hypovitaminosis was observed in patients requiring surgical treatment compared to patients managed conservatively. The association found in this study does not imply causation, but it should be considered within the set of actions for the treatment of these injuries.

Resumen: Introducción: la hipovitaminosis D se encuentra ampliamente extendida a nivel mundial, con consecuencias clínicas a nivel óseo y extraóseo. Entre los factores que la causan se encuentran los antiepilépticos (AE). En Uruguay no se conoce su prevalencia en niños ni en pacientes que reciben AE. Objetivos: conocer la prevalencia de hipovitaminosis D de niños y adultos en un prestador de salud y compararla con la prevalencia en pacientes bajo tratamiento con AE. Método: estudio descriptivo, transversal, realizado entre marzo y diciembre de 2017. Las variables analizadas fueron: niveles de vitamina D, calcio, fósforo, fosfatasa alcalina y parathormona intacta. Se consideró insuficiencia de vitamina D niveles menores de 30 ng/ml y déficit niveles menores de 20 ng/ml. Resultados: se incluyeron 113 pacientes, 60 niños y 53 adultos. La prevalencia global de insuficiencia de vitamina D fue de 89% y déficit de 60%. En niños expuestos a AE, la media de vitamina D fue 17,5 ng/ml, y en niños no expuestos 19,6 ng/ml. En adultos la media de vitamina D fue de 18,1 en expuestos a AE y 16,9 en no expuestos. La diferencia de medias no fue estadísticamente significativa en niños ni en adultos. Se observaron niveles de calcemia significativamente descendidos en niños y adultos con AE. Conclusiones: la insuficiencia de vitamina D fue cercana a 90% y el déficit superó el 50%. No se encontraron diferencias significativas entre grupos en hipovitaminosis D, pero se observaron niveles de calcemia reducidos en los expuestos a AE. Es necesario continuar analizando los factores que la causan y sus consecuencias clínicas.


Summary: Introduction: hypovitaminosis D is a highly spread condition worldwide, with clinical consequences that affect bone directly, among other manifestations. Antiepileptic drugs are among factors that cause this deficiency. In Uruguay, there is no information about hypovitaminosis D in children or patients who receive antiepileptic drugs. Objectives: to learn about the prevalence of hypovitaminosis D in children and adults in a health institution and to compare it with the prevalence in patients receiving antiepileptic drugs. Method: descriptive, transversal study conducted from March through December, 2017. The following variables were analysed: vitamin D, calcium, phosphorous, alkaline phosphatase and intact parathyroid hormone. Vitamin D insufficiency was defined as vitamin D levels of less than 30 ng per mL and deficiency as D levels of less than 20 ng per mL. Results: 113 patients were included in the study, 60 of which were children and 53 adults. Global prevalence of vitamin D insufficiency was 89% and deficiency was 60%. In children taking antiepileptic drugs, the average vitamin D value was 17.5 ng/ml and it was 19.6 ng/ml for those not exposed to those drugs. In adults, the average vitamin D value was 18.1 in the population taking antiepileptic drugs and 16.9 in patients not taking that medication. The difference between average values was not statistically significant in children or adults. Calcemia levels observed were significantly lower in both children and adults taking antiepileptic drugs. Conclusions: vitamin D insufficiency was close to 90% and deficiency was over 50%. No significant differences were found between hypovitaminosis D groups, although reduced calcemia was observed in patients exposed to antiepileptic drugs. Further studies are necessary to analyse factors that cause this condition and its clinical consequences.


Resumo: Introdução: a hipovitaminose D está amplamente difundida em todo o mundo, com consequências clínicas a nível ósseo e extraósseo. Entre os fatores que a causam estão os medicamentos antiepilépticos (AE). No Uruguai, sua prevalência em crianças ou em pacientes adultos recebendo AE não é conhecida. Objetivos: conhecer a prevalência de hipovitaminose D em crianças e adultos em um prestador de serviços de saúde e compará-la com a prevalência em pacientes em tratamento com AE. Método: estudo transversal descritivo realizado entre março e dezembro de 2017. As variáveis analisadas foram: níveis de vitamina D, cálcio, fósforo, fosfatase alcalina e paratormona intacta. Níveis menores que 30 ng / ml e níveis de déficit menores que 20 ng / ml foram considerados como insuficiência de vitamina D. Resultados: foram incluídos 113 pacientes, 60 crianças e 53 adultos. A prevalência global de insuficiência de vitamina D foi de 89% e déficit de 60%. Em crianças expostas à AE, a média de vitamina D foi de 17,5 ng / ml e em crianças não expostas de 19,6 ng / ml. Em adultos, a média de vitamina D foi de 18,1 nos expostos ao AE e de 16,9 nos não expostos. A diferença nas médias não foi estatisticamente significativa nas crianças nem nos adultos. Níveis de cálcio significativamente diminuídos foram observados em crianças e adultos com EA. Conclusões: a insuficiência de vitamina D foi próxima a 90% e o déficit ultrapassou 50%. Não foram encontradas diferenças significativas entre os grupos na hipovitaminose D, mas níveis reduzidos de cálcio foram observados naqueles expostos a EA. É necessário continuar analisando os fatores que o causam e suas consequências clínicas.

La hipovitaminosis D es frecuente en el mundo. Según estudios realizados entre 1987 y 2015, en Argentina la prevalencia fue > 40%. En personas con infección por HIV variaría entre 20 y 90%, pero en nuestro medio no se conoce con precisión. Nuestro objetivo fue determinar la prevalencia de hipovitaminosis D en una cohorte de adultos con infección por HIV asistidos en forma ambulatoria en la ciudad de Buenos Aires. Se analizaron retrospectivamente las historias clínicas de 814 sujetos mayores de 18 años HIV positivos con al menos una determinación de vitamina D. La mediana de edad fue 44 años (rango intercuartílico 21-80), 746 (91.6%) eran hombres y 813 (99.9%) recibían tratamiento antirretroviral. Se realizó análisis uni y multivariado para determinar asociación entre hipovitaminosis D y valores de CD4, carga viral para HIV y terapia antirretroviral. La prevalencia de hipovitaminosis D fue 79.7% (insuficiencia 34.2%, deficiencia 45.5%). No se encontró asociación con el uso de efavirenz o inhibidores de la proteasa (p = 0.86 en ambos casos), con el recuento de linfocitos CD4, ni con la carga viral plasmática (p = 0.81 y 0.74, respectivamente). El presente estudio muestra que, en nuestro medio, la hipovitaminosis D es muy frecuente en personas con infección por HIV. Aun cuando no revela evidencia de relación con carga viral para HIV, estado inmune, ni tratamiento antirretroviral, es necesaria la búsqueda sistemática de hipovitaminosis D en esta población, en vista de la alta frecuencia de osteopenia y osteoporosis y el mayor riesgo de fracturas descripto en personas HIV positivas.


Hypovitaminosis D is frequent worldwide. In Argentina, according to studies conducted between 1987 and 2015, prevalence was > 40% in the general population. In people living with HIV it may vary between 20 and 90%, but the prevalence in our environment is not known. Our objective was to determine the prevalence of hypovitaminosis D in a cohort of adults with HIV infection in the city of Buenos Aires. We analyzed retrospectively medical records of 814 HIV positive subjects older than 18 years with at least one determination of vitamin D. The median age was 44 years (interquartile range 21-80), 746 (91.6%) were men, and 813 (99.9%) were on antiretroviral treatment. Univariate and multivariate analyses were performed to determine the association of hypovitaminosis D with CD4 values, viral load for HIV, and antiretroviral therapy. The present study shows that, in our environment, hypovitaminosis D is very common in people with HIV infection. Although it does not reveal evidence of a relationship with viral load for HIV, immune status, or antiretroviral treatment, the systematic search for hypovitaminosis D is mandatory in this population, taking into account its high frequency and the increased risk of osteopenia, osteoporosis and fractures, as described in people with HIV.

Hypovitaminosis D, defined by low serum levels of 25(OH)D, is a recognized worldwide public health problem. The most accepted definition considers that deficiency occurs with serum levels fall below 12 ng/ml of 25(OH)D. Long term vitamin D deficiency results in decreased bone mineralization, secondary hyperparathyroidism, increased cortical bone loss (pathogenesis of osteoporosis and hip fractures), differentiation and division of various cell types, muscle strength, diabetes type 2, blood pressure, etc. Twin- and family-based studies indicate that genetic factors influence serum 25(OH)D levels. Genetic studies have shown single-nucleotide polymorphisms (SNPs) are linked to low serum 25(OH)D concentrations through changes in the activity of the enzymes of the 1α,25(OH)2D metabolic pathway. Carriers of high genetic risk scores would need a h igher amount of vitamin D supplementation to achieve adequate serum 25(OH)D concentrations. Clinicians would not need to indicate studies to identify patients with vitamin D insufficiency of genetic origin. They should instruct their patients on their own care, to control the intake of vitamin D and the serum 25(OH)D levels until the latter are adequate. Overall, the literature reveals that the consequences of hypovitaminosis D on bone health are observed in old and infrequently in young subjects. A probable explanation for the latter is: if the rate of bone remodeling allows it, bone tissue has endogenous (genetics, hormones) and exogenous determinants (diet, physical activity) that may compensate the variables of bone health. The consequences of vitamin D deficit on bone health, has not been completely uncovered.


La hipovitaminosis D, definida por bajos niveles séricos de 25(OH)D (<12 ng/ml), es un reconocido problema de salud pública mundial. La deficiencia de vitamina D a largo plazo resulta en una disminución de la mi neralización ósea, hiperparatiroidismo secundario, pérdida de hueso cortical (patogénesis de la osteoporosis y fracturas de cadera), diferenciación y división de varios tipos de células, fuerza muscular, diabetes tipo 2, pres ión arterial, etc. Estudios genéticos han demostrado que algunos "polimorfismos de un solo nucleótido" (SNP) están relacionados con bajas concentraciones séricas de 25(OH)D a través de reducción en la actividad de las enzimas implicadas en la síntesis de 1α,25(OH)2D. Los médicos no necesitan indicar un estudio genético para identificar a la insuficiencia de vitamina D de causa genética. Bastará con instruir a los pacientes sobre su propio cuidado y controlar la ingesta de vitamina D y los niveles séricos de 25(OH)D hasta que estos últimos sean adecuados. En general, la literatura revela que las consecuencias de la hipovitaminosis D sobre la salud ósea se observan en las personas añosas y con poca frecuencia en sujetos jóvenes. Una explicación probable para esta situación es: si la tasa de remodelación ósea lo permite, el tejido óseo tiene factores endógenos (genéticos, hormonales) y exógenos (dieta, actividad física) que pueden compensar las variables de la salud ósea. Las consecuencias del déficit de vitamina D sobre la salud ósea aún no se conocen completamente.

BACKGROUND: The finding of hypovitaminosis in pregnancy D has prompted the debate about its supplementation. The objective of the study was to measure the prevalence of hypovitaminosis D in mothers and newborns. METHODS: A one-year observational study was conducted including the measuring of vitamin D levels in mothers and in the umbilical cord blood of newborns. An analysis was made of the variables as regards maternal characteristics, delivery and sun exposure. RESULTS: Values lower than 20 ng/ml were found in 64.4% of 745 mothers and 41.3% of 560 newborns, and less than 30 ng/ml in 88.7% and 67.1%, respectively. Mean levels were higher in summer-autumn than in winter-spring (21.73 and 13.70 ng / ml in mothers and 29.04 and 20.49 ng/ml in cord), and higher in the umbilical cord than in the maternal plasma. Multiple pregnancies (OR: 6.29) and non-European origin (OR: 13.09) were risk factors for maternal hypovitaminosis, while maternal supplementation (OR: 0.19), physical activity (OR: 0.57), and sun exposure (OR: 0.46) had a preventive effect. CONCLUSIONS: The high rates of hypovitaminosis support the policy of giving dietary supplements to newborns. The high level of hypovitaminosis found supports the extension of screening and supplementation to all pregnant women, and not only to those with risk factors. The greater difference between mothers and newborns in seasons of low sun exposure can be interpreted as a protective effect.

Objetivos: Comparar los niveles plasmáticos de Vitamina D (VD) en adultos en dos períodos estacionales y evaluar algunos factores que afectan a estos niveles. Pacientes y métodos: Estudio de corte transversal en el que se estudiaron adultos en dos períodos de tiempo, agosto a octubre de 2018 (grupo 1) y febrero a abril de 2019 (grupo 2). Se midieron niveles plasmáticos de VD a ambos grupos. Se obtuvieron datos demográficos y clínicos, incluyendo el uso de suplementos de VD, a través de un cuestionario autoaplicado validado. Se clasificó el nivel de VD en deficiente (< 20 ng/dl), insuficiente (21-30 ng/dl), y suficiente (> 30 ng/dl). Se consideró adulto mayor (AM) a sujetos de ≥ 65 años. Resultados: 207 pacientes (104 en el grupo 1 y 103 en grupo 2), 165 fueron mujeres. Un 31,9% (n=66) tenía niveles deficientes de VD y un 42,5% (n=88) utilizaba algún tipo de suplemento de VD. Los niveles de VD fueron mayores en el grupo 2 (media 31,2 ng/dl) respecto al grupo 1 (media 24,1 ng/dl; p<0,0001), pero esta diferencia solo fue significativa en quienes tomaban suplementos de VD. Por otro lado, el uso de suplemento de VD se asoció a niveles más altos de VD (media 33,1 ng/ml; p<0.001). Además, hubo mayores niveles en pacientes mayores de 65 años (p<0,01). No hubo diferencias estadísticamente significativas en niveles de VD según sexo ni estado nutricional. Conclusión: Se observaron mayores niveles de VD en usuarios de suplementos de esta vitamina, sin diferencias según período estacional.


Objectives:To compare plasmatic levels of vitamin D in adults in two seasonal periods and evaluate some factors affecting these levels. Patients and methods: Transversal cohort study in which adults were studied in two time periods, August to October 2018 (group 1) and February to April 2019 (group 2). Plasmatic levels of vitamin D were measured in both groups. Demographic and clinical details, including the use of vitamin D supplements, were obtained through a self-administered questionnaire. Results were classified as deficient (<20ng/dL), insufficient (21 ­ 30 ng/dL) and normal (>30 ng/dL). The elderly were defined to be the ≥65´s. Results: 207 patients (104 in group 1 and 103 in group 2), 165 were female. 31.9% (n=66) had deficient levels of vitamin D and 42,5% (n=88) used a vitamin D supplement. Vitamin D levels were higher in group 2 (median 31.2 ng/dL) compared with group 1 (median 24.1 ng/dL; p<0.0001), but this difference was significant only in those taking vitamin D supplements. La hipertensión endocraneana idiopàtica se asocia infrecuentemente con la hipovitaminosis A y D. Se presenta el caso de una paciente femenina de 8 años con visión borrosa de 24 horas y papiledema bilateral. Resonancia magnética nuclear normal. Presión de apertura de líquido cefalorraquídeo: 260 mm^O. Presentó déficit de vitamina A y D, e inició un tratamiento sustitutivo. El segundo caso corresponde a un paciente masculino de 12 años con fiebre y odinofagia de 3 días. Con antecedente de glomerulonefritis y sobrepeso. Presentaba edema bipalpebral y papiledema. Tomografia axial computada de la órbita: aumento de líquido en la vaina de ambos nervios ópticos. Resonancia magnética nuclear: aracnoidocele intraselar. Presión de apertura de líquido cefalorraquídeo: 400 mm^O. Presentó déficit de vitamina D y B6, e inició el tratamiento sustitutivo. La elevación de la presión intracraneal desencadena mecanismos de compensación que, al fallar, pueden comprometer la vida o provocar graves discapacidades neurológicas. Reconocer la causa para un enfoque terapéutico preciso es clave para disminuir la morbimortalidad asociada a esta patología.


Idiopathic endocranial hypertension is infrequently associated with hypovitaminosis A and D. The case of an 8-year-old female with 24-hour blurred vision and bilateral papilledema is presented. Nuclear magnetic resonance was normal. Opening pressure of cerebrospinal fluid: 260 mm^O. She presented vitamin A and D deficiency and started replacement therapy. The second case corresponds to a 12-year-old male with fever and odynophagia of 3 days. History of glomerulonephritis and overweight. He had bipalpebral edema and papilledema. Computed tomography scan of the orbit: increase of fluid in the sheath of both optic nerves. Nuclear magnetic resonance: intrasellar arachnoidocele. Opening pressure of cerebrospinal fluid: 400 mmH2O. He presented vitamin D and B6 deficiency and started replacement treatment. The elevation of intracranial pressure triggers compensation mechanisms that, when they fail, can compromise life or cause serious neurological disabilities. Recognizing the cause for an accurate therapeutic approach is key to reduce the morbidity and mortality associated with this pathology.

ABSTRACT

OBJECTIVE: To analyse the prevalence of hypovitaminosisD and associated factors in school children and adolescents living in a region of northern Spain. DESIGN: Cross-sectional study (convenience sampling). SETTING: Primary Health Care. PARTICIPANTS: A total of 602 Caucasian individuals (aged 3.1 to 15.4years) were included in the study. MAIN MEASUREMENTS: Prevalence of hypovitaminosisD were calculated (dependent variable). HypovitaminosisD is defined according to the US Endocrine Society criteria: deficiency (calcidiol <20ng/mL), insufficiency (calcidiol: 20-29ng/mL), and sufficiency (calcidiol ≥30ng/mL). Gender, age, body mass index, residence, and season of the year were recorded (independent variables), and their association with hypovitaminosisD was analysed by multiple regression. RESULTS: The prevalence of hypovitaminosisD was 60.4% (insufficiency: 44.6%; deficiency: 15.8%). Multivariate analysis showed that factors associated to hypovitaminosisD were being female (OR: 1.6; 95%CI: 1.1-2.3), pubertal age (OR: 1.8; 95%CI: 1.2-2.6), autumn (OR: 9.5; 95%CI: 4.8-18.7), winter (OR: 8.8; 95%CI: 4.5-17.5) and spring time (OR: 13.2; 95%CI: 6.4-27.5), living in urban areas (OR:1.6; CI95%: 1.1-2.2), and severe obesity (OR: 4.4; 95%CI: 1.9-10.3). CONCLUSIONS: There is a high prevalence of hypovitaminosisD in juvenile populations. being female, pubertal age, autumn, winter and spring seasons, severe obesity, and living in urban areas are factors associated to hypovitaminosisD. Consideration should be given to the administration of vitamin supplements and/or the increase in the ingestion of natural vitaminD dietary sources.

INTRODUCTION: Patients on haemodialysis (HD) have a high prevalence of 25-OH-vitamin D (25-OH-D)deficiency. Secondary hyperparathyroidismis a common condition in these patients, which is very important to control. 25-OH-D is involved in regulating calcium homeostasis. As such, appropriate levels of this vitamin could help to control bone mineral metabolism. OBJECTIVE: To evaluate the effect 25-OH-D repletion in HD patients with 25-OH-D deficiency (<20ng/ml) on the control of secondary hyperparathyroidism and microinflammation status. PATIENTS AND METHODS: Prospective observational study in which stable patients on HD with 25-OH-D deficiency (<20ng/ml) were treated with oral calcifediol 0.266mcg/every 2 weeks for three months. Dialysis characteristics, biochemical parameters and drug doses administered were analysed before and after the correction of the deficiency. RESULTS: Forty-five stable HD patients with a mean age of 74.08±12.49 years completed treatment. Twenty-seven patients (60%) achieved 25-OH-D levels above 20ng/ml (23 with levels>30ng/ml and 4 between 20-30ng/ml). Parathyroid hormone levels decreased in 32 of the 45 patients, 23 of which (51%) achieved a>30% decrease from baseline. In terms of concomitant treatment, we observed a significant reduction in the selective vitamin D receptor activator dose, but no changes in calcimimetic or phosphate binders administration. In terms of malnutrition-inflammation status, a decrease in C-reactive protein was noted, although other microinflammation parameters, such as activated monocytes (CD14+/CD16+ and CD 14++/CD16+) were unchanged. No changes were observed in the levels of FGF-23. CONCLUSIONS: Correcting 25-OH-D deficiency in HD patients is associated with better secondary hyperparathyroidism control with lower doses of vitamin D analogues, as well as an improvement in inflammatory status. Our results support the recommendation to determine 25-OH-D levels and correct its deficiency in these patients.

El Síndrome Metabólico (SM) es una patología heterogénea que en rangos generales se podría definir como la combinación de factores de riesgo asociados a la enfermedad cardiovascular (ECV) y diabetes, caracterizados por insulino-resistencia, hipertensión, elevada presión arterial, elevados niveles de triglicéridos (TGL) y obesidad abdominal.La hipovitaminosis D induciría perturbaciones metabólicas (hipertensión, aterogenicidad, incremento de ECV, de insulino-resistencia, de diabetes tipo II y de obesidad central) que favorecen el desarrollo de SM. El estado nutricional respecto de la vitamina D se evalúa por los niveles de 25hidroxivitamina D (25OHD), principal metabolito circulante de dicha vitamina. Diversos estudios han sugerido la existencia de una relación inversa entre los niveles de 25OHD y el desarrollo de SM. La adiposidad central, la resistencia a la insulina y aterogenicidad son importantes factores de riesgo para el desarrollo de SM. Todos ellos se asocian a la hipovitaminosis D. La suplementación con vitamina D, al aumentar los niveles de 25OHD, reduciría el riesgo de desarrollar SM, lo que tendría importantes implicancias para el individuo en particular y la Salud Pública en general. Para clarificar todos estos puntos se hace necesario realizar estudios prospectivos randomizados, controlados, a doble ciego para evaluar los efectos a largo plazo y la causalidad entre el estado nutricional de vitamina D y el riesgo de desarrollar SM.


The metabolic syndrome (MS) is an heterophasic pathology, which in general terms, could be defined as the combination of risk factors associated with cardiovascular disease (CVD) and diabetes, characterized by insulin resistance, hypertension, high blood pressure, high levels of triglycerides (TGL) and abdominal obesity. Vitamin D deficiency may induce metabolic disturbances (hypertension, atherogeny, increased CVD, insulin-resistance, type II diabetes and central obesity), which favour the development of MS. The nutritional status for vitamin D could be evaluated by serum 25-hydroxyvitamin D levels (25OHD). Several studies have suggested the existence of an inverse relationship between 25OHD levels and the increased risk of SM. Central adiposity, insulin resistance and atherogeny are important risk factors for the development of SM. All of these factors are associated with vitamin D deficiency. Supplementation with vitamin D increases 25OHD levels and could reduce the risk of developing MS. To assess the long-term effects and the causal link between the nutritional status of vitamin D and the risk of developing MS it is necessary to perform prospective randomized, controlled, double-blind studies.


A síndrome metabólica (SM) é uma patologia heterogênea que, em termos gerais, pode ser definida como a combinação de fatores de risco associados com doença cardiovascular (DCV) e diabetes, caracterizados por resistência à insulina, hipertensão, níveis altos de pressão arterial, níveis elevados de triglicérides (TGL) e obesidade abdominal. A hipovitaminose D induziria distúrbios metabólicos (hipertensão, aterogenicidade, aumento da DCV, resistência à insulina, diabetes tipo II e obesidade central), que favorecem o desenvolvimento de SM. O estado nutricional a respeito da vitamina D é avaliado pelos níveis de 25-hidroxivitamina D (25OHD), principal metabólito circulante de tal vitamina. Vários estudos têm sugerido a existência de uma relação inversa entre os níveis de 25OHD e o desenvolvimento da SM. A adiposidade central, a resistência à insulina central e aterogenicidade são fatores de risco importantes para o desenvolvimento da SM. Todos eles se associam à hipovitaminose D. A suplementação com vitamina D, aumentando os níveis de 25OHD, reduziria o risco de desenvolvimento de SM, o que teria importantes consequências para a do indivíduo em particular e a Saúde Pública em geral. Para esclarecer todos esses pontos é necessário realizar estudos prospectivos, randomizados, controlados, duplo-cego para avaliar os efeitos no longo prazo e a causalidade entre o estado nutricional de vitamina D e o risco de desenvolver SM.
