ABSTRACT

The internal iliac artery arises as a terminal extension of the common iliac artery and supplies blood to the pelvic region. This study aims to identify the anatomic variations of the internal iliac artery (IIA) in a Mexican population sample. This is a retrospective cross-sectional observational study. A total of 81 angiographies via the femoral artery approach performed on patients undergoing various medical procedures were included. Variations in the IIA branching patterns were identified by evaluating the angiographic images and grouped according to Adachi's classification into five types (I-V). A total of 139 hemipelvises were analyzed (78 right and 61 left). The frequencies of each type of variation were as follows: Type I (71.2%), Type II (10.79%), Type III (0 cases), Type IV (0.7%), Type V (12.94%), and unclassified (4.31%). The most frequent anatomical variants of the IIA in the western Mexican population sample were Type I, followed by Types V and II. Even though Type V is rare in most populations, it was the second most frequent variant in this study. Understanding the variants of the IIA branching pattern is necessary for performing invasive procedures in the pelvic region with precision and minimizing complications.

ABSTRACT

An arteriovenous fistula (AVF) is an uncommon sequela of spontaneous arterial aneurysm rupture into the adjacent venous system. We describe the case of a 74-year-old patient who underwent endovascular treatment of a right iliac AVF caused by a ruptured common iliac artery (CIA) aneurysm and a distal left CIA aneurysm. Surgery preserved the lumbar and inferior mesenteric arteries because of the need to simultaneously exclude the hypogastric arteries. Dynamic fluid balance phenomena provoked by closure of the AVF are described. The patient had a benign postoperative course with normalization of the severe hemodynamic changes presented prior to the intervention and resolution of respiratory symptoms attributed to pulmonary arterial hypertension.

Resumen: Introducción: la fusión espinal es utilizada para tratar, entre otras patologías, las enfermedades degenerativas discales. El injerto óseo autólogo de cresta ilíaca es el estándar de oro para aumentar las probabilidades de fusión; sin embargo, no está exento de complicaciones. Objetivos: investigar si los pacientes a los que se les recolecta injerto de cresta ilíaca posterior y son cegados al lado del sitio donante, pueden identificar de qué lado se tomó dicho injerto y si la intensidad de este dolor está en relación con la cantidad de injerto obtenido. Material y métodos: trabajo prospectivo, experimental, aleatorizado y comparativo, simple ciego. Se incluyeron pacientes adultos operados entre Julio de 2019 hasta Abril de 2020, a los cuales se les realizó una cirugía primaria de artrodesis lumbar posterolateral abierta instrumentada, con colocación de injerto óseo autólogo de cresta ilíaca. Se dividió a los pacientes en dos grupos aleatorizados. La cantidad de injerto a recolectar fue de acuerdo a las necesidades quirúrgicas. Se interrogó por el dolor según escala visual analógica al primer, tercer y sexto mes de la cirugía, siempre solicitando que se identifique el lado más doloroso. Resultados: se analizaron 44 pacientes (n = 23 cresta derecha, n = 21 cresta izquierda). La mayoría de los pacientes no lograron identificar el lado del cual se obtuvo el injerto óseo, con una diferencia estadística ampliamente significativa (p = 0.0001). Conclusión: la obtención de injerto óseo de cresta ilíaca es un procedimiento efectivo y seguro, que mejora las probabilidades de fusión sin aumentar la morbilidad del paciente.


Abstract: Introduction: spinal fusion is used to treat, among other pathologies, the degenerative intervertebral disc disease. Autologous iliac crest bone grafting is the golden standard treatment for increasing the rate of fusion; however, it isn't free of complications. Objectives: to investigate whether patients who have posterior iliac crest graft harvesting and are blinded to the donor site, can identify from which side the graft was harvested, and whether the intensity of this pain is related to the amount of graft obtained. Material and methods: prospective, experimental, randomized and comparative, single-blind study. Adult patients who underwent primary instrumented open posterolateral lumbar fusion with autologous iliac crest bone graft between July 2019 and April 2020 were included. Patients were divided into two randomized groups. The amount of graft to be harvested was according to surgical needs. The patients were asked about pain according to the visual analogue scale in the first, third and sixth months after surgery, always requesting that the most painful side be identified. Results: a total of 44 patients (n = 23 right crest, n = 21 left crest) were analyzed. Most patients were unable to identify the side from which the bone graft was harvested, with a statistically significant difference (p-value 0.0001). Conclusion: iliac crest bone graft harvesting is an effective and safe procedure that improves the rates of fusion without increasing the patient's morbidity.

ABSTRACT

An iliac vein aneurysm is a rare vascular anomaly, scarcely reported in the vascular literature. We present the case of a 72-year-old man with a history of a remote heart transplant complicated by severe tricuspid regurgitation and traumatic abdominal injury, who was incidentally found to have a 10-cm right common iliac vein aneurysm. Because of the size and risk of rupture, we elected to treat him with surgical iliac vein aneurysmorrhaphy. His iliac venous diameter and flow continued to be stable at subsequent follow-up.

INTRODUCTION: Scaphoid is the most fractured carpal bone, with a 5-10% nonunion rate. Treatment challenges include choosing the implant and graft that best corrects humpback deformity and carpal malalignment with higher chances of bony healing. OBJECTIVE: Compare cortico-cancellous and cancellous grafts between two groups of patients treating scaphoid nonunion with locking plates using autologous bone graft and evaluate bone healing rates and radiographic, tomographic, and functional parameters before and after surgery. METHODS: Non-randomized prospective study including 20 cases of scaphoid nonunion. Groups were divided into Group A (ten patients treated with cortico-cancellous iliac graft) and Group B (ten patients treated with cancellous iliac graft). Patients underwent pre- and postoperative radiographs, computed tomography, and functional evaluation. RESULTS: In postoperative analysis, Group A showed a statistically significant difference in intrascaphoid angle (p = 0.002) and scapholunate angle (p = 0.011) correction when comparing the pre- and postoperative periods. Group B showed a statistically significant difference in intrascaphoid angle (p = 0.002) and scapholunate angle correction (p = 0.0018), grip strength (p = 0.002), and tip pinch strength (p = 0.001) when comparing the pre- and postoperative periods. By comparing both groups, Group B showed a statistically significant difference in intrascaphoid angle correction (p = 0.002), grip strength (p = 0.002), tip pinch strength (p = 0.002), and radial deviation (p = 0.0003). There was no statistical difference when comparing bony healing between groups. CONCLUSION: Scaphoid nonunion treatment with a locking plate was effective, showing a high bony healing rate and improved carpal alignment in imaging tests for both graft types. However, results for intrascaphoid angle correction, grip strength, tip pinch strength, and radial deviation were better in Group B. LEVEL OF EVIDENCE: IV, case series.

ABSTRACT

Purpose: This case report describes a patient with a pelvic kidney associated with an aortoiliac aneurysm amenable to open repair. Methods: We chose the selective cold renal perfusion to preserve renal function. Results: Other techniques such as transient or permanent extra-anatomic bypass, extracorporeal circulation and proximal aortic double clamping are also described in this report. Conclusions: Choosing the renal perfusion technique depended on the anatomical evaluations of the kidney and the renal artery location during the preoperative period.

ABSTRACT

The treatment of choice for patients with symptomatic venous compression syndrome is venous stenting. However, this treatment has well-documented complications and, although rare, contralateral deep venous thrombosis is one of these complications. Our objective is to present a case of deep venous thrombosis of the contralateral iliac vein resulting from placement of the stent beyond the recommended position and the therapeutic challenge is to recanalize the vein with reconstruction of the iliocaval confluence.

Salmonella es una de las principales causas de gastroenteritis y bacteriemia a nivel mundial. En determinadas ocasiones, puede ser, además, el agente causal de aneurisma de la arteria ilíaca en pacientes con factores predisponentes: mayores de 60 años, diabéticos, hipertensos y sometidos a intervenciones quirúrgicas. El objetivo de la publicación fue evidenciar la presencia de aneurisma micótico por salmonella en iliaca derecha, en un paciente masculino que presentó fiebre, deposiciones líquidas y masa tumoral en región inguinal; para el cual, el diagnóstico clínico, de laboratorio, la ultrasonografía vascular y el Angiotac, ayudaron a tomar la conducta terapéutica más adecuada. Los aneurismas de iliacas de causa bacteriana no son tan infrecuentes, y el cirujano vascular debe saber qué conducta terapéutica elegir en cada caso. Los procederes endovasculares y la cirugía a cielo abierto son dos opciones que deben ser combinadas siempre con la terapia antimicrobiana(AU)


Salmonella is one of the main causes of gastroenteritis and bacteremia worldwide. On certain occasions, it can also be the causative agent of iliac artery aneurysm in patients with predisposing factors: older than 60 years, diabetics, hypertensive and undergoing surgery. The objective of the publication was to show the presence of mycotic aneurysm due to salmonella in the right iliac artery, in a male patient who presented fever, liquid stools and tumor mass in the inguinal region. Clinical and laboratory diagnosis, vascular ultrasonography and Angiotac, helped to take the most appropriate therapeutic approach. Bacterial iliac aneurysms are not so uncommon, and the vascular surgeon must know which therapeutic approach to choose in each case. Endovascular procedures and open surgery are two options that should always be combined with antimicrobial therapy(AU)

ABSTRACT

We present the case of an 87-year-old man with a ruptured right internal iliac artery aneurysm with hemoperitoneum. The right internal iliac artery aneurysm appeared to fill from the retrograde profunda femoris artery in the setting of a previously repaired abdominal aortic aneurysm with aorta-bi-iliac bypass with ligation of the bilateral internal iliac arteries. Abdominal computed tomography revealed an aneurysm of the right internal iliac artery measuring 8.9 cm, with filling through the collateral vessels. Open repair was performed, leading to complete exclusion of the aneurysm with no perioperative complications.

El dolor abdominal de alto riesgo es una condición común en los servicios de emergencia y está asociado a una alta morbilidad y mortalidad, si no se diagnostica y trata de manera rápida y precisa. Un hombre con síntomas de cólico nefrítico se presentó en emergencia con dolor abdominal intenso y deterioro clínico. Después de una tomografía, se encontró una imagen aneurismática en la arteria iliaca común primitiva izquierda. El paciente fue trasladado a sala de operaciones para una reparación quirúrgica, pero falleció. El dolor abdominal de alto riesgo requiere un abordaje diagnóstico integral y tratamiento individualizado para prevenir complicaciones graves. El aneurisma de la arteria iliaca complicado es una causa potencialmente grave de dolor abdominal en hombres fumadores de edad avanzada con antecedentes de hipertensión y aterosclerosis.


Life-Threatening abdominal pain is a common condition in emergency departments and it is associated with high morbidity and mortality, if not promptly and accurately diagnosed and treated. A man with symptoms of renal colic presented to the emergency room with severe abdominal pain and clinical deterioration. After a CT scan, an aneurysmatic image was found in the left primitive iliac artery. The patient was taken to an operating room for surgical repair but died. Life-Threatening abdominal pain requires a comprehensive diagnostic approach and individualized treatment to prevent serious complications. The complicated iliac artery aneurysm is a potentially serious cause of abdominal pain in elderly male smokers with a history of hypertension and atherosclerosis.

Introducción: El síndrome de May-Thurner es una enfermedad cuya prevalencia real es desconocida. Suele ser asintomática y las manifestaciones clínicas aparecen en el contexto de las complicaciones asociadas a la enfermedad. La aproximación diagnóstica se realiza por medio de imágenes, donde se identifican alteraciones patognomónicas que permiten descartar diagnósticos diferenciales. Se ha demostrado que la COVID-19 genera un estado protrombótico, que en contexto del síndrome de May-Thurner puede derivar en complicaciones tromboembólicas. Objetivo: Comparar la clínica y la posible relación de la enfermedad tromboembólica venosa en el curso de la COVID-19 en pacientes con SMT. Caso clínico: Mujer de 24 años, secretaria de profesión y residente en Bucaramanga. Presentó un cuadro clínico de 20 días de evolución que inició con rinorrea hialina, tos ocasional y mialgias; cinco días después manifestó fiebre no cuantificada y tos con expectoración hemoptoica. Conclusiones: Dentro de la fisiopatología de la infección por SARS-CoV-2 se desarrollan mecanismos procoagulantes, lo cual incrementa el riesgo de eventos trombóticos en pacientes con o sin factores de riesgo(AU)


Introduction: May-Thurner syndrome is a disease whose actual prevalence is unknown. It is usually asymptomatic and clinical manifestations appear in the context of complications associated with the disease. The diagnostic approach is performed by imaging, where pathognomonic alterations are identified to rule out differential diagnoses. It has been demonstrated that COVID-19 generates a prothrombotic state, which particularly in patients with May-Thurner syndrome would lead to thromboembolic complications. Objective: To compare the clinical and possible relationship of venous thromboembolic disease in the course of COVID-19 in patients with May-Thurner syndrome. Clinical case: 24-year-old woman, secretary by profession and resident in Bucaramanga, Colombia. She presented a clinical picture of 20 days of evolution that began with hyaline rhinorrhea, occasional cough and myalgias; five days later she manifested unquantified fever and cough with hemoptotic expectoration. Conclusions: Within the pathophysiology of SARS-CoV-2 infection, procoagulant mechanisms develop, which increases the risk of thrombotic events in patients with or without risk factors(AU)

ABSTRACT

BACKGROUND: Takayasu arteritis (TA) is a chronic inflammatory arteritis affecting large vessels, predominantly the aorta and its main branches. Diagnosis is usually delayed in most patients owing to the absence of typical clinical manifestations in the acute phase until the development of substantial arterial symptoms. The purpose of this case report is to describe abdominal pain being an uncommon symptom in TA. LEARNING OBJECTIVE: The clinical presentation of TA can be non-specific and can have serious consequences if the diagnosis is delayed, helping the clinicians who can treat with such patients (emergency medicine, cardiologist, or medical personnel).

ABSTRACT

Isolated external iliac artery aneurysm is a rare occurrence. These aneurysms have varied presentations depending on size and proximity. Both open surgical and endovascular modalities can be used for treatment depending upon presentation, aneurysmal anatomy, and patient condition. Preservation of at least one internal iliac artery is important to prevent post-repair hypogastric ischemia. There are no previous reports of IgG4-related disease (IgG4-RD) as etiology of these aneurysms. A 32-year-old male patient presented with a left lower abdominal lump and was found to have a left external iliac artery aneurysm on computed tomography angiography. The patient underwent iliofemoral bypass with an 8 mm polyester graft. Histopathological examination of the aneurysm wall suggested IgG4-RD. The patient fulfilled the 2020 Revised Comprehensive Diagnostic Criteria for IgG4-RD. An 18-Fluorodeoxyglucose-Positron Emission Tomography scan performed in the postoperative period showed no active disease, hence medical therapy was not instituted. The patient is doing well at 1 year.


O aneurisma isolado da artéria ilíaca externa é uma ocorrência rara. Esses aneurismas têm apresentações variadas, dependendo do tamanho e da proximidade. Ambas as modalidades cirúrgicas aberta e endovascular podem ser usadas para o tratamento, dependendo da apresentação, anatomia do aneurisma e condição do paciente. A preservação de pelo menos uma artéria ilíaca interna é importante para prevenir isquemia hipogástrica pós-reparação. A doença relacionada à imunoglobulina G4 (IgG4-RD) nunca havia sido encontrada como etiologia desse aneurisma. Um paciente do sexo masculino de 32 anos que apresentava um nódulo no abdome inferior esquerdo foi diagnosticado com aneurisma da artéria ilíaca externa esquerda na angiotomografia computadorizada. O paciente foi submetido a bypass iliofemoral com enxerto de poliéster de 8 mm. O exame histopatológico da parede do aneurisma era indicativo de IgG4-RD. O paciente cumpriu os Critérios Abrangentes Revisados ​​para IgG4-RD de 2020. A tomografia por emissão de pósitrons com 18-fluorodesoxiglicose no pós-operatório não evidenciou doença ativa, não sendo instituída terapia medicamentosa. Após seguimento de 1 ano, o paciente está bem.

RESUMEN La rotura espontánea de la vena ilíaca es una causa poco frecuente de hemorragia retroperitoneal exanguinante. Generalmente está asociada con un traumatismo y las lesiones iatrogénicas durante la cirugía de hemiabdomen inferior o pelviana. Ocurre con frecuencia del lado izquierdo y afecta indistintamente a la vena ilíaca común como a la externa. Con el objetivo de destacar las características clínicas e imagenológicas de esta infrecuente entidad, presentamos un caso fatal de rotura espontánea de la vena ilíaca externa izquierda. El tratamiento se basa en la reparación quirúrgica abierta o, en casos seleccionados, en la colocación de prótesis por vía endovascular. Es necesario un alto índice de sospecha, ya que el diagnóstico y el tratamiento tempranos son fundamentales para mejorar las altas tasas de morbimortalidad que conlleva esta entidad.


ABSTRACT Spontaneous rupture of the iliac vein is a rare cause of fatal retroperitoneal hemorrhage that is generally associated with trauma and iatrogenic injury during lower abdominal or pelvic surgery. It usually occurs in the left common and external iliac veins. We report a fatal case of spontaneous rupture of the left external iliac vein to emphasize the clinical and imaging characteristics of this rare condition. Treatment is based on open surgical repair or endovascular stenting in selected cases. High level of suspicion is essential for early diagnosis and treatment to improve the associated morbidity and mortality.

Resumen Los riñones ectópicos pélvicos solitarios asociados a degeneración aneurismática de las arterias ilíacas y la aorta abdominal son eventos muy raros. Los enfoques quirúrgicos para la corrección de aneurismas con compromiso renal son un desafío por la falta de consenso actual, en especial cuando cursan con riñones ectópicos pélvicos solitarios funcionales asociados. Por tal motivo, una de las estrategias que ha demostrado buenos resultados a corto y mediano plazo es la nefroprotección en frío, la cual puede ser usada con seguridad en estos pacientes. En el caso que se presenta, se evidenció una ectasia aórtica con aneurismas ilíacos bilaterales asociados con un riñón ectópico pélvico solitario en un paciente masculino de 75 años, con hipertensión arterial y dislipidemia. Se realizó una reconstrucción aortoilíaca bilateral y una reconstrucción de la arteria hipogástrica, además de reimplante de la arteria renal ectópica bajo irrigación renal en frío, sin complicaciones y preservación de la función renal.


Abstract Solitary pelvic ectopic kidneys associated with aneurysmal degeneration of the iliac arteries and abdominal aorta are very rare events. Surgical approaches for the correction of aneurysms with renal involvement are challenging due to the lack of current consensus, especially when they are associated with functional solitary pelvic ectopic kidneys. For this reason, one of the strategies that has shown good results in the short and medium term is cold nephroprotection, which can be used safely in these patients. In the present case, aortic ectasia with bilateral iliac aneurysms associated with a solitary pelvic ectopic kidneys was evidenced in a 75-year-old male patient with arterial hypertension and dyslipidemia. Bilateral aortoiliac reconstruction was performed with hypogastric artery reconstruction and reimplantation of the ectopic renal artery under cold renal irrigation, without complications and preservation of renal function.

Se dividió a los pacientes en dos grupos aleatorizados. La cantidad de injerto a recolectar fue de acuerdo a las necesidades quirúrgicas. Se interrogó por el dolor según escala visual analógica al primer, tercer y sexto mes de la cirugía, siempre solicitando que se identifique el lado más doloroso. RESULTADOS: se analizaron 44 pacientes (n = 23 cresta derecha, n = 21 cresta izquierda). La mayoría de los pacientes no lograron identificar el lado del cual se obtuvo el injerto óseo, con una diferencia estadística ampliamente significativa (p = 0.0001). CONCLUSIÓN: la obtención de injerto óseo de cresta ilíaca es un procedimiento efectivo y seguro, que mejora las probabilidades de fusión sin aumentar la morbilidad del paciente.

ABSTRACT

ABSTRACT Objective: Demonstrate the feasibility of endoscopic assisted L5S1 intraforaminal lumbar interbody fusion (iLIF) through a transiliac approach. Methods: Ten transiliac iLIF and ten supra iliac iLIF were performed bilaterally at L5S1 in five randomly selected fresh-frozen human cadavers. The following measurements were recorded: distances from the transiliac track to the iliac crest, posterior superior iliac spine, and superior gluteal neurovascular bundle; pelvic parameters; approach angles; cage's lateral and AP center point ratio (CPR); percentage of the cage crossing the midline in the AP and lateral views. Endplate integrity was assessed through endoscopic visualization. In addition, the facet joint, sacroiliac joint, iliolumbar ligament, and exiting and traversing nerve roots were checked for integrity through anatomic dissection. Results: In the transiliac technique, the axial and coronal approach angles were significantly decreased by 13.5º (95% CI -15.5; -11.5; p value<0.001) and 13.2º (95% CI -15.3; -11.1; p value<0.001), respectively, the sagittal approach angle was significantly increased by 5.4º (95% CI 1.8,8.9; p-value = 0.008), and the AP CPR was significantly higher (MD 0.16; 95% CI 0.12,0.20; p value<0.001). The percentage of the cage crossing the AP view's midline was increased by 31.6% (95% CI 19.8,43.4; p value<0.001). The integrity of endplates, facet joints, sacroiliac joints, iliolumbar ligament, and exiting and traversing nerve roots was maintained. Conclusion: L5S1 transiliac iLIF is a feasible surgical technique. It allows a more centrally placed interbody cage in the coronal plane without compromising the anterior position in the lateral plane. The integrity of the major anatomic structures at risk was preserved. Evidence Level III: A case-control study.


Resumo: Objetivo: Demonstrar a viabilidade da fusão intersomática lombar assistida por endoscopia (iLIF) em L5S1 através de abordagem transilíaca. Métodos: Dez iLIF por via transilíaca e dez iLIF por via suprailíaca foram realizados bilateralmente em L5S1 em cinco cadáveres selecionados aleatoriamente. Foram registadas as seguintes medidas: Distâncias da via transilíaca até a crista ilíaca, crista ilíaca póstero-superior e feixe neurovascular do glúteo superior; parâmetros pélvicos; ângulos da abordagem; relação do ponto central lateral e AP do cage (CPR); percentagem do cage cruzando a linha média nas incidências AP e perfil. A integridade das placas vertebrais foi avaliada através de visualização endoscópica. Foi verificada através de dissecção anatómica a integridade das articulações facetárias, sacroilíacas, ligamento iliolombar e raízes de L5 e S1. Resultados: Na técnica transilíaca, os ângulos de abordagem axial e coronal foram significativamente menores em 13,5º (CI 95% -15,5;-11,5; p<0,001) e 13,2º (CI 95% -15,3;-11,1; p<0,001 ), respectivamente, o ângulo de abordagem sagital aumentou significativamente em 5,4º (CI 95% 1,8,8,9; p = 0,008), e o AP CPR foi significativamente maior (MD 0,16; CI 95% 0,12,0,20; p <0,001). A percentagem do cage cruzando a linha média em AP foi superior em 31,6% (CI 95% 19,8,43,4; valor p<0,001). Se evaluó la integridad de las placas vertebrales por visualización endoscópica. Se comprobó la integridad de las articulaciones facetarias, las articulaciones sacroilíacas, el ligamento iliolumbar y las raíces de L5 y S1 mediante disección anatómica. Resultados: En la técnica transilíaca, los ángulos de abordaje axial y coronal fueron significativamente menores en 13,5° (IC 95% -15,5;-11,5; p<0,001) y 13,2º (IC 95% -15,3;-11,1); p<0,001 ), respectivamente, el ángulo de aproximación sagital aumentó significativamente en 5,4º (IC 95% 1,8,8,9; p = 0,008), y el AP CPR fue significativamente mayor (MD 0,16; IC 95% 0,12,0,20; p <0,001). El porcentaje del cage que cruzaba la línea media en AP era mayor en un 31,6% (IC 95% 19,8,43,4; valor p <0,001). Se mantuvo la integridad de las placas vertebrales, las articulaciones facetarias, las articulaciones sacroilíacas, el ligamento iliolumbar y las raíces de L5 y S1. Conclusión: La iLIF transilíaca L5S1 es una técnica quirúrgica viable. Permite colocar el cage más centrado en el plano coronal sin comprometer la posición anterior en el plano sagital. Se preservó la integridad de las principales estructuras anatómicas en riesgo. Nivel de evidencia III: Estudio de casos y controles.

Resumen ANTECEDENTES: La endometriosis de la pared abdominal implica la coexistencia de tejido endometrial en la superficie peritoneal parietal; la incidencia reportada es de 0.03 a 3.5%. Su causa aún no está debidamente esclarecida. CASO CLÍNICO: Paciente de 35 años, con antecedentes ginecoobstétricos de: tres embarazos, tres cesáreas, última cinco años previos a la intervención, en la que se practicó una histerectomía obstétrica indicada por sangrado transoperatorio. El padecimiento actual se inició 24 horas previas a su ingreso a Urgencias, con dolor espontáneo en la fosa iliaca derecha, de difícil relación con los ciclos menstruales debido al antecedente quirúrgico, acompañado de aumento de volumen y náuseas. En la exploración inicial se identificaron dos masas intraabdominales que se confirmaron en la tomografía computada, situadas por encima de la aponeurosis. Se procedió a la intervención quirúrgica para extirpación de ambas masas. El estudio histopatológico reportó: tumores compatibles con endometriosis. CONCLUSIÓN: La endometriosis es un padecimiento con alta prevalencia en el mundo, no así en su ubicación en la pared abdominal. A pesar de que aún no se conoce con certeza su causa, se sabe que la inoculación directa (muchas veces debida a un procedimiento ginecológico quirúrgico) y la proliferación celular tienen participación relevante en su origen.


Abstract BACKGROUND: Abdominal wall endometriosis is the coexistence of endometrial tissue on the parietal peritoneal surface with a reported incidence of 0.03 to 3.5%. Its cause is not well understood. CLINICAL CASE: 35-year-old female patient with a gyneco-obstetric history of: three pregnancies, three cesarean sections, last five years prior to surgery, in which an obstetric hysterectomy was performed, indicated by transoperative bleeding. The current presentation began 24 hours before her admission to the emergency department with spontaneous pain in the right iliac fossa, difficult to relate to menstrual cycles due to her surgical history, accompanied by increased volume and nausea. Initial examination revealed two intra-abdominal masses, confirmed by computed tomography, located above the aponeurosis. Surgery was performed to remove both masses. Histopathologic examination revealed tumors compatible with endometriosis. CONCLUSION: Endometriosis is a very common disease in the world, but not in the abdominal wall. Although its cause is still not known with certainty, it is known that direct inoculation (often due to gynecologic surgery) and cell proliferation play a relevant role in its origin.

ABSTRACT: Objective: To perform an analysis of the anatomy of the great vessels relevant to the access for anterior lumbar interbody fusion (ALIF), determining the level of their bifurcation, the distance between the iliac vessels at L5-S1, the morphological configuration of the left iliac vein and the presence of fatty tissue between the vessel and the disc. Methods: Two hundred magnetic resonance imaging (MRI) scans of the lumbar spine of patients (18-80 years old) were evaluated using axial, coronal, and sagittal cuts at levels L1-S1 in T2 weighting. The interiliac distance was defined as the measurement between the left iliac vein and the right iliac artery. The presence of fatty tissue was defined as the identification of space between the vessel and the disc. Vessel morphology was divided into oval and flat. Results: The population's average age was 49.6 years, with 52% being female. The average interiliac distance at L5-S1 was 27.48mm. The bifurcation of the aorta artery was identified at the level of L4 in 56.3%, as well as the confluence of the iliac veins (37.2%). The left iliac vein was identified as oval in 69% of patients and flat in 31% of patients. Fat tissue was evidenced in 60.5% of the exams. Conclusion: As a routine preoperative examination and surgical planning, lumbar MRI is fundamental in investigating the anatomy regarding anterior approach surgeries, allowing an effective assessment of the relationships between the great vessels and the lumbar spine. RESUMO: Objetivo: Realizar uma análise da anatomia dos grandes vasos relevantes ao acesso para fusão intersomática lombar anterior (ALIF), determinando o nível de sua bifurcação, a distância entre os vasos ilíacos em L5-S1, a configuração morfológica da veia ilíaca esquerda e a presença de tecido gorduroso entre o vaso e o disco. Métodos: duzentos exames de ressonância magnética (RM) da coluna lombar de pacientes (18-80 anos) foram avaliados, utilizando cortes axiais, coronais e sagitais nos níveis L1-S1, na ponderação T2. A distância interilíaca foi definida como a medida entre a veia ilíaca esquerda e artéria ilíaca direita. A presença de tecido gorduroso foi definida como identificação de espaço entre o vaso e o disco. A morfologia do vaso foi dividida em oval e plana. Resultados: A idade média da população foi de 49,6 anos, sendo 52% mulheres. A distância média interilíacas em L5-S1 foi 27,48 mm. A bifurcação da artéria aorta foi identificada ao nível de L4 em 56,3%. A confluência das veias ilíacas também foi mais frequente ao nível de L4, representando 37,2%. A veia ilíaca esquerda foi identificada com o formato oval em 69% e plana em 31% dos pacientes. Tecido gorduroso foi evidenciado em 60,5% dos exames. Conclusão: Como rotina no exame pré-operatório e no planejamento cirúrgico, a RM lombar tem fundamental importância na investigação da anatomia visando cirurgias de abordagem anterior, pois permite uma avaliação eficaz das relações entre os grandes vasos e a coluna lombar. Métodos: Se evaluaron 200 imágenes de resonancia magnética (RM) de la columna lumbar de pacientes (18-80 años) mediante cortes axiales, coronales y sagitales en los niveles L1-S1, en ponderación T2. La distancia interilíaca se definió como la medida entre la vena ilíaca izquierda y la arteria ilíaca derecha. La presencia de tejido graso se definió como la identificación de espacio entre el vaso y el disco. La morfología de los vasos se dividió en ovalados y planos. Resultados: La edad media de la población fue de 49,6 años, de los cuales 52% eran mujeres. La distancia interilíaca media en L5-S1 fue de 27,48 mm. La bifurcación de la arteria aorta se identificó a nivel de L4 en 56,3%, así como la confluencia de las venas ilíacas (37,2%). La vena ilíaca izquierda se identificó como ovalada en 69% y plana en 31%. Se evidenció tejido graso en 60,5% de los exámenes. ABSTRACT

Abstract Isolated external iliac artery aneurysm is a rare occurrence. These aneurysms have varied presentations depending on size and proximity. Both open surgical and endovascular modalities can be used for treatment depending upon presentation, aneurysmal anatomy, and patient condition. Preservation of at least one internal iliac artery is important to prevent post-repair hypogastric ischemia. There are no previous reports of IgG4-related disease (IgG4-RD) as etiology of these aneurysms. A 32-year-old male patient presented with a left lower abdominal lump and was found to have a left external iliac artery aneurysm on computed tomography angiography. The patient underwent iliofemoral bypass with an 8 mm polyester graft. Histopathological examination of the aneurysm wall suggested IgG4-RD. The patient fulfilled the 2020 Revised Comprehensive Diagnostic Criteria for IgG4-RD. An 18-Fluorodeoxyglucose-Positron Emission Tomography scan performed in the postoperative period showed no active disease, hence medical therapy was not instituted. The patient is doing well at 1 year.


Resumo O aneurisma isolado da artéria ilíaca externa é uma ocorrência rara. Esses aneurismas têm apresentações variadas, dependendo do tamanho e da proximidade. Ambas as modalidades cirúrgicas aberta e endovascular podem ser usadas para o tratamento, dependendo da apresentação, anatomia do aneurisma e condição do paciente. A preservação de pelo menos uma artéria ilíaca interna é importante para prevenir isquemia hipogástrica pós-reparação. A doença relacionada à imunoglobulina G4 (IgG4-RD) nunca havia sido encontrada como etiologia desse aneurisma. Um paciente do sexo masculino de 32 anos que apresentava um nódulo no abdome inferior esquerdo foi diagnosticado com aneurisma da artéria ilíaca externa esquerda na angiotomografia computadorizada. O paciente foi submetido a bypass iliofemoral com enxerto de poliéster de 8 mm. O exame histopatológico da parede do aneurisma era indicativo de IgG4-RD. O paciente cumpriu os Critérios Abrangentes Revisados ​​para IgG4-RD de 2020. A tomografia por emissão de pósitrons com 18-fluorodesoxiglicose no pós-operatório não evidenciou doença ativa, não sendo instituída terapia medicamentosa. Após seguimento de 1 ano, o paciente está bem.
