ABSTRACT

Medical leadership remains integral to the health system amidst a growing burden of ill health and disease, rising patient expectations and medical and technological advancements. The study objectives were to (a) provide a perspective through a rapid review of medical director roles and responsibilities in public and private hospital settings across several Organisation for Economic Co-operation and Development (OECD) and Non-Organisation for Economic Co-operation and Development countries, and (b) provide recommendations on how health system performance could be strengthened. A rapid review of Medical Director job descriptions in public and private hospitals was carried out. Medical Directors are influential leaders in organisational decision-making and quality improvement; however, their role has shifted from clinical oversight to several managerial and leadership roles. We report some variation in their role and responsibilities, in the 'intensity of job requirements' and 'complexity of managing resources' dimensions. The changing expectations of medical directors and the variation in their roles and responsibiliteis may contribute to inefficiencies and misalignment within health systems. There may be a need to pursue reform to assure alignment with health system objectives, albeit reform may require different approaches to meet the needs of different health systems. Further research is needed to explore how reform of medical directors' roles and responsibilities can be quantified to demonstrate improvement within health systems.

ABSTRACT

Labor and employment are important issues in social life. The demand for online job searching and searching for labor regulations in legal documents, particularly regarding the policy for unemployment benefits, is essential. Nowadays, each function has some programs for its working. However, there is no program that combines both functions. In practice, when users seek a job, they may be unemployed or want to transfer to another work. Thus, they are required to search for regulations about unemployment insurance policies and related information, as well as regulations about workers working smoothly and following labor law. Ontology is a useful technique for representing areas of practical knowledge. This article proposes an ontology-based method for solving labor and employment-related problems. First, we construct an ontology of job skills to match curriculum vitae (CV) and job descriptions (JD). In addition, an ontology for representing labor law documents is proposed to aid users in their search for legal labor law regulations. These ontologies are combined to construct the knowledge base of a job-searching and labor law-searching system. In addition, this integrated ontology is used to study several issues involving the matching of CVs and JDs and the search for labor law issues. A system for intelligent resume searching in information technology is developed using the proposed method. This system also incorporates queries pertaining to Vietnamese labor law policies regarding unemployment and healthcare benefits. The experimental results demonstrate that the method designed to assist job seekers and users searching for legal labor documents is effective.

ABSTRACT

PHENOMENON: Despite the nearly universal presence of chief residents within U.S. and Canadian residency programs and their critical importance in graduate medical education, to our knowledge, a comprehensive synthesis of publications about chief residency does not exist. An understanding of the current state of the literature can be helpful to program leadership to make evidence-based improvements to the chief residency and for medical education researchers to recognize and fill gaps in the literature. APPROACH: We performed a scoping review of the literature about chief residency. We searched OVID Medline, PsycINFO, ERIC, and Web of Science databases through January 2023 for publications about chief residency. We included publications addressing chief residency in ACGME specialties in the U.S. and Canada and only those using the term "chief resident" to refer to additional responsibilities beyond the typical residency training. We excluded publications using chief residents as a convenience sample. We performed a topic analysis to identify common topics among studies. FINDINGS: We identified 2,064 publications. We performed title and abstract screening on 1,306 and full text review on 208, resulting in 146 included studies. Roughly half of the publications represented the specialties of Internal Medicine (n = 37, 25.3%) and Psychiatry (n = 30, 20.5%). Topic analysis revealed six major topics: (1) selection of chief residents (2) qualities of chief residents (3) training of chief residents (4) roles of chief residents (5) benefits/challenges of chief residency (6) outcomes after chief residency. INSIGHTS: After reviewing our topic analysis, we identified three key areas warranting increased attention with opportunity for future study: (1) addressing equity and bias in chief resident selection (2) establishment of structured expectations, mentorship, and training of chief residents and (3) increased attention to chief resident experience and career development, including potential downsides of the role.

ABSTRACT

INTRODUCTION: This paper discusses the current medical sonographer status in the United Arab Emirates (UAE). The paper explores the current education, skills, job description, and challenges towards developing sonography professions and a sonographer framework. METHODS: The study employed a quantitative descriptive research approach using a cross-sectional study design to collect and analyse data from sonographers in the UAE. Data was collected through a structured survey that included sections addressing demographic information, education, skills, current practices, and challenges. RESULTS: The data showed a relatively even distribution across different age groups, with a higher proportion falling in the 31-40 age range. Females constitute a slightly higher percentage (55.9 %) than males (44.1 %). The study found that sonographers have the necessary skills and knowledge to interpret the scan results and provide a formal report. A significant percentage of participants taught ultrasound to others and participated in research activities. The survey responses indicate difficulty in reporting sonograms. CONCLUSIONS: The study found that sonographers possess the necessary expertise and skills to pass on their knowledge to trainees and colleagues. The participants expressing a desire to increase their knowledge and skills indicate that sonographers recognised that continuous professional development is crucial for staying updated with advancements in the field. The participant's "Advanced Practitioners" title recommendation suggests a desire for professional recognition, career progression, and specialisations in sonography. Effective interdisciplinary collaboration is important for providing quality patient care. IMPLICATION FOR PRACTICE: The paper emphasises the importance of recognising sonography as a vital healthcare profession. It aims to raise awareness about the crucial role of sonographers in patient care. The study also stresses the need for a well-defined career progression framework to guide professional growth and acknowledgement.

ABSTRACT

Fundamento: el proyecto de vida profesional constituye una formación psicológica compleja que debe ser desarrollada y evaluada durante las acciones que se realizan en los procesos sustantivos universitarios: formación, investigación y extensión universitaria. Objetivo: evaluar la efectividad del procedimiento para la formación del proyecto de vida profesional en estudiantes de Medicina. Métodos: se realizó una investigación cualitativa de tipo prexperimental en la Universidad de Ciencias Médicas de Holguín, Facultad de Ciencias Médicas "Mariana Grajales Coello", desde enero 2021 hasta diciembre 2022. Se utilizaron como métodos teóricos el análisis-síntesis, la inducción-deducción y la modelación. Los métodos empíricos aplicados fueron la observación y el instrumento Exploración del proyecto de vida profesional; así como el prexperimento para evaluar su efectividad. Resultados: se aportó un procedimiento para la formación del proyecto de vida profesional en estudiantes de la carrera Medicina a implementarse durante los procesos sustantivos universitarios. El prexperimento mostró su efectividad para alcanzar el objetivo. Conclusiones: el procedimiento presentado incrementa la calidad de la formación inicial de estudiantes de la carrera Medicina. A través de su evaluación se constató que perfecciona el proceso pedagógico y favorece el proyecto de vida profesional.


Background: the professional life project constitutes a complex psychological training that must be developed and evaluated during the actions carried out through core university processes: training, research and university extension. Objective: to evaluate the effectiveness of the procedure for the formation of the professional life project in medical students. Methods: a pre-experimental quantitative research was carried out at "Mariana Grajales Coello" University of Medical Sciences from Holguín, Faculty of Medical Sciences, from January 2021 to December 2022. Analysis-synthesis, induction-deduction were used as theoretical methods. The empirical methods applied were observation, the Exploration of the professional life project instrument; as well as the pre-test, post test and modeling to evaluate its effectiveness. Results: a procedure was provided for the formation of the professional life project in Medicine students to be implemented trough core university processes. The pre-test and post test showed its effectiveness in achieving the objective. Conclusions: the procedure presented increases the quality of the initial training of medical students. Through its evaluation, it was found that it improves the pedagogical process and favors the professional life project.

ABSTRACT

Midwives are professionals who fulfill maternal and child health needs. In Mongolia, midwives were unable to transfer their knowledge and skills to the next generation midwives last few decades. The details of their experiences and the comprehensive aspects of continuing professional development (CPD) are still unclear. This study aimed to assess the current status of midwives in clinical practice through an online symposium. Relevant information was collected from presentations, question-and-answer sessions, and questionnaires. It was found that CPD has unclear training plans, no specialized training, and with them having little experience with CPD. Newly graduated midwives do not have an educational program. As of the current status, midwifery services are not provided at the clinical site in the scope of midwifery job descriptions. This study also discusses the situation of low status and salary in midwifery. Strengthening the system of midwifery CPD like development of the educational program is needed.

INTRODUCTION: Prior studies have demonstrated inconsistent development and utilisation of radiographers in the reporting of radiographs, and there is ongoing consideration of the level at which such radiographers should be educated to and operating at. This study aimed to explore and evaluate expectation and utilisation of radiographers currently, or training in, reporting in projection radiography across one integrated care system (ICS). METHODS: A multi-method approach was utilised, with document analysis of projection radiography reporting role job descriptions and person specifications and an online survey of managers and clinical leads. A single ICS in the north of England formed the setting for the study. RESULTS: This study demonstrated variation in implementation and utilisation of the role across trusts within the ICS. Inconsistencies in scope, expected underpinning education and role activity were identified. Radiographers autonomously reporting in projection radiography were titled advanced practitioners, however are not expected to achieve national educational standards for such roles and are not empowered to work at this level of practice by their employers. It was acknowledged that staffing pressures hinder appropriate role utilisation and reporting capacity. CONCLUSION: Inconsistent development and utilisation of radiographers in such roles may hamper collaboration and service delivery across a network. Identifying variation and working towards role standardisation could promote cross-organisational working and improve career progression opportunities. IMPLICATIONS FOR PRACTICE: Scoping the reporting radiographer workforce may assist and guide future imaging service and workforce planning.

AIM: The aim of this study is to profile the contemporary advanced clinical practitioner (ACP) role through standardized document sets. DESIGN: Documentary analysis of job descriptions (JDs), person specification and advertisements. DATA SOURCES: England based jobs advertised on NHS jobs website from 22 January to 21 April 2021. RESULTS: A toatal of 143 trainee and qualified ACP roles were identified. A wide range of sectors and specialities were represented from across all English regions. The most common roles were urgent care, emergency medicine and primary care. Most qualified roles were agenda for change band 8A, although this did vary across specialities. Many roles were restricted to a small number of professions, notably nursing, physiotherapy and paramedic. Inconsistent role titles were noted. A lack of understanding of regulation across different professions was noted. CONCLUSION: The ACP role has become an accepted across healthcare providers in England. Implementation remains varied across specialities and organizations. Eligibility criteria may relate to professional bias. IMPLICATIONS FOR THE PROFESSION AND/OR PATIENT CARE: ACP roles are expanding but this may be at the detriment to advanced nursing posts. Inconsistency in role eligibility suggests some professional bias exists. IMPACT: This was scoping of ACP roles across England using job advertisements. ACP roles are common across sectors and specialities but eligibility varies. The research will have impact on those looking to recruit to ACP roles as well as those refining JDs. REPORTING METHOD: No EQUATOR guideline exists for document analysis. PATIENT OR PUBLIC CONTRIBUTION: No Patient or Public Contribution. Purpose: One of the hospital management issues that has not been studied much during the COVID-19 Pandemic is the relationship between health workers' performance, job satisfaction and remuneration. This study aims to examine the relationship between remuneration, job satisfaction and the employee performance during 2019-2021. Materials and Methods: This study applied an employee satisfaction survey at a General Academic Hospital between 2019-2021. The population and samples were 716 employees. The data collection were based on the personnel database, remuneration database, and the annual Employee Satisfaction Survey Database for the period 2019-2021 at General Academic Hospital of Dr. Soetomo, Surabaya, Indonesia. Results: The results of the correlation test between Employee Satisfaction, Remuneration, and Performance based on employee performance objectives showed an insignificant positive correlation between the remuneration variable and satisfaction based on The Job Itself; very weak significant positive correlation between remuneration variable and Satisfaction based on Pay; a very weak significant positive correlation between the remuneration variable and Satisfaction based on Promotion; very weak significant positive correlation between remuneration variable and satisfaction based on supervision; significant positive correlation between remuneration variable and satisfaction based on coworkers; There is a significant positive correlation between remuneration and performance variables. Conclusion: The correlation between remuneration and employee satisfaction based on the Job Description Index shows that the components of the job itself, and coworkers have a positive but not significant relationship, while the components of pay, promotion, and supervision have a positive and significant relationship. Employees satisfaction with performance achievement have a positive and significant relationship, especially job satisfaction based on pay and supervision, but there is also a positive and insignificant relationship related to job satisfaction based on the job itself, promotion, and co-workers.

ABSTRACT

OBJECTIVES: Automatic job coding tools were developed to reduce the laborious task of manually assigning job codes based on free-text job descriptions in census and survey data sources, including large occupational health studies. The objective of this study is to provide a case study of comparative performance of job coding and JEM (Job-Exposure Matrix)-assigned exposures agreement using existing coding tools. METHODS: We compared three automatic job coding tools [AUTONOC, CASCOT (Computer-Assisted Structured Coding Tool), and LabourR], which were selected based on availability, coding of English free-text into coding systems closely related to the 1988 version of the International Standard Classification of Occupations (ISCO-88), and capability to perform batch coding. We used manually coded job histories from the AsiaLymph case-control study that were translated into English prior to auto-coding to assess their performance. We applied two general population JEMs to assess agreement at exposure level. Percent agreement and PABAK (Prevalence-Adjusted Bias-Adjusted Kappa) were used to compare the agreement of results from manual coders and automatic coding tools. RESULTS: The coding per cent agreement among the three tools ranged from 17.7 to 26.0% for exact matches at the most detailed 4-digit ISCO-88 level. The agreement was better at a more general level of job coding (e.g. 43.8-58.1% in 1-digit ISCO-88), and in exposure assignments (median values of PABAK coefficient ranging 0.69-0.78 across 12 JEM-assigned exposures). Based on our testing data, CASCOT was found to outperform others in terms of better agreement in both job coding (26% 4-digit agreement) and exposure assignment (median kappa 0.61). CONCLUSIONS: In this study, we observed that agreement on job coding was generally low for the three tools but noted a higher degree of agreement in assigned exposures. The results indicate the need for study-specific evaluations prior to their automatic use in general population studies, as well as improvements in the evaluated automatic coding tools.

ABSTRACT

RATIONALE: Self-management of a chronic illness is a struggle for many patients. There is substantial evidence that patients are not as successful as they and their providers would like. Considering patient self-management through an innovative and diverse lens could help patients, providers and the health care system to consider novel changes to improve success. AIMS AND OBJECTIVES: To provide a complete view of patient work by utilizing the human resources management practice of job analysis to develop an initial job description for patients suffering from chronic illness. METHODS: Study design was descriptive qualitative with analysis aiming to identify those reoccurring ideas from the data. Thirty patients, with at least one chronic illness, in three focus groups were assembled for this study. Verbatim recordings and notes were used to categorize the data provided. Five coders analyzed the data independently, and jointly met to discuss the themes identified. Demographic data was collected via surveys. RESULTS: Patients with chronic illness engage in five primary 'job' duties including self-care (actions taken to effectively manage physical and psychological symptoms), managing relationships (effective management of relationships with employers, family, friends and providers to ensure the best outcomes), managing resources (understanding and managing finances, health and prescription medication insurance), coordination (managing and scheduling visits to providers) and research/education (gathering information and conducting research relative to all aspects of managing one's illness). Relevant knowledge, skills, abilities (KSAs) and other resources were identified that are necessary for patients to effectively perform the five job duties. CONCLUSIONS: Creating a job description for the 'job' of patient is a crucial step in understanding the work chronically ill patients undertake. Knowing the duties, their associated tasks and KSAs, and resources required to perform those tasks enables patients and their providers and advocates to better identify ways to assist, relieve and encourage these patients in order to maximize patient success.

ABSTRACT Objective: to reflect on the potentialities and challenges of entrepreneurial activity in the early-stage stage of nurses, considering the analogies between the profile of Brazilian nursing and business entrepreneurs in the country. Method: this is a theoretical reflection articulated around the concept of phases of entrepreneurial activity. From this starting point, common variables from the Nursing Profile survey and the Global Entrepreneurship Monitor Brazil Report were selected, such as sex, age group, education level, family income and occupation. Taking into account a parallel between variables, reflection on the challenges and potential of entrepreneurial nurses in the nascent and new entrepreneurial activity phases was ordered. Results: it is possible that nascent and new entrepreneurial nurses are predominantly women, specialized with the possibility of income compatible with traditional jobs. Specialization, professional experience and professional legislation are strengths, while sex is a challenge. Conclusion: there are both potentialities and challenges. For nascent and new entrepreneurs to emerge, entrepreneurial education, promotion of gender equality, work and income policies are particularly relevant.


RESUMEN Objetivo: reflexionar sobre las potencialidades y desafíos de la actividad empresarial en la etapa inicial de los enfermeros frente a las analogías entre el perfil de la enfermería brasileña y los empresarios del país. Método: reflexión teórica articulada en torno al concepto de fases de la actividad emprendedora. A partir de ese punto de partida, fueron seleccionadas variables comunes de la Encuesta Perfil de Enfermería y del Informe Global Entrepreneurship Monitor Brasil, entre las que se encuentran género, grupo etario, nivel educativo, renta familiar y ocupación. Teniendo en cuenta un paralelismo entre las variables, se ordenó la reflexión sobre los desafíos y potencialidades de los enfermeros emprendedores en las fases de actividad emprendedora naciente y nueva. Resultados: es posible que las enfermeras nacientes y nuevas emprendedoras sean predominantemente mujeres, especializadas, con posibilidad de ingresos compatibles con los trabajos tradicionales. La especialización, la experiencia profesional y la legislación profesional son potencialidades, mientras que el género es un desafío. Conclusión: hay potencialidades y desafíos. Para el surgimiento de emprendedores emergentes y nuevos, la educación emprendedora, la promoción de la igualdad de género, las políticas de trabajo e ingresos son particularmente relevantes.


RESUMO Objetivo: refletir sobre potencialidades e desafios da atividade empreendedora em estágio inicial de enfermeiros face à analogias entre o perfil da enfermagem brasileira e de empreendedores de negócios no país. Método: reflexão teórica articulada em torno do conceito de fases de atividade empreendedora. Deste ponto de partida foram selecionadas variáveis comuns da pesquisa Perfil da Enfermagem e do Relatório do Global Entrepreneurship Monitor Brasil, sendo sexo, faixa etária, nível de escolaridade, renda familiar e ocupação. Levando em conta um paralelo entre as variáveis, ordenou-se a reflexão sobre os desafios e potencialidade de enfermeiros empreendedores nas fases de atividade empreendedora nascente e nova. Resultados: é possível que enfermeiros empreendedores nascentes e novos sejam predominantemente mulheres, especializadas, com possibilidade de renda compatível com postos tradicionais de trabalho. A especialização, a experiência profissional e a legislação profissional são potencialidades; enquanto que o sexo, um desafio. Conclusão: há tanto potencialidades quanto desafios. Para que empreendedores nascentes e novos surjam são relevantes, sobretudo, a educação empreendedora, promoção da igualdade de gênero, políticas de trabalho e renda.

Fundamento: la evaluación del desempeño profesional y la competencia constituye el eje fundamental del modelo cubano para el análisis y evaluación de la calidad en la educación y práctica médicas. Objetivo: diseñar un modelo de evaluación de la competencia investigativa del licenciado en Imagenología. Métodos: se realizó una investigación de desarrollo en los centros de atención secundaria del municipio Holguín desde 2021 a 2022. Se emplearon métodos teóricos: histórico-lógico, análisis-síntesis, sistémico-estructural-funcional y modelación; y empíricos: análisis documental y cuestionario a licenciados en Imagenología. Para la valoración del modelo se empleó el criterio de 10 especialistas con reconocido prestigio en los centros de atención. Los indicadores fueron la pertinencia, utilidad, factibilidad. Resultados: se constataron insuficiencias en la competencia investigativa evidenciadas en el desempeño profesional. Se elaboró un modelo de evaluación compuesto por dos subsistemas: teórico funcional y contextualización; de sus relaciones deviene una nueva cualidad que se enuncia en la mejora del desempeño profesional investigativo para la solución de problemas tecnológicos y biomédicos. Conclusiones: los dos subsistemas que componen el modelo de evaluación de la competencia investigativa tienen una relación de coordinación. Los especialistas lo valoraron como pertinente, útil y factible de ser aplicado.


Background: the evaluation of professional performance and competence constitutes the fundamental axis of the Cuban model for the analysis and evaluation of quality in medical education and practice. Objective: to design a model for evaluating the research competence of the graduate in Imaging. Methods: a development investigation was carried out in the secondary care centers of the Holguín municipality from 2021 to 2022. Theoretical methods were used: historical-logical, analysis-synthesis, systemic-structural-functional and modeling; and empirical: documentary analysis and questionnaire to graduates in Imaging. To evaluate the model, the criteria of 10 specialists with recognized prestige in care centers were used. The indicators were relevance, usefulness, feasibility. Results: insufficiencies in the researching competence were found, evidenced in professional performance. An evaluation model composed of two subsystems was developed: functional theory and contextualization; from their relationships becomes a new quality that is expressed in the improvement of professional researching performance for the solution of technological and biomedical problems. Conclusions: the two subsystems that make up the research competence evaluation model have a coordination relationship. The specialists valued it as pertinent, useful and feasible to be applied.

Fundamento: la evaluación del impacto en el contexto de la educación superior es el proceso mediante el cual se valoran los cambios que ocurren en el egresado por influencia del programa, un tiempo después de aplicadas las acciones formativas. Objetivo: evaluar el impacto de la especialidad Oftalmología en el desarrollo docente de sus egresados en la Facultad de Ciencias Médicas "Mariana Grajales Coello" de Holguín. Métodos: se realizó un estudio de evaluación con enfoque mixto y de corte transversal en el campo del posgrado académico, en el período enero 2021-junio 2022. Se aplicaron métodos teóricos y empírico: cuestionario para obtener información sobre la percepción de los egresados respecto al impacto de la especialidad en su preparación docente. El procesamiento incluyó análisis de frecuencias relativas, índice de posición, la correlación elemento escala y el índice de Cronbach para la confiabilidad. Resultados: las subdimensiones aportes al dominio de las categorías de la didáctica, preparación en el trabajo metodológico, preparación en la organización y dirección del trabajo docente calificaron de Alto el impacto de la especialidad en la preparación docente de los egresados. Además, hubo 47 % de participación en la ejecución de actividades docentes de pregrado y posgrado después de haber cursado la especialidad. Conclusiones: la percepción de los egresados evidenció el alto impacto de la especialidad Oftalmología en su preparación docente, componente esencial en la educación médica para la calidad del proceso docente educativo.


Background: impact evaluation in the context of higher education is the process through which the changes that occur in the graduate due to the influence of the program are assessed, sometime after the training actions have been applied. Objective: to assess the impact of the Ophthalmology specialty on the teaching development of its graduates at "Mariana Grajales Coello" Faculty of Medical Sciences in Holguín. Methods: an evaluation study was carried out with a mixed and cross-sectional approach in the field of academic postgraduate studies, from January 2021 toJune 2022. Theoretical and empirical methods were applied: questionnaire to obtain information on the perception of graduates regarding the impact of the specialty on their teaching preparation. The processing included analysis of relative frequencies, position index, scale element correlation and Cronbach's index for reliability. Results: the sub dimensions contributions to the mastery of the categories of didactics, preparation in methodological work, preparation in the organization and direction of teaching work rated the impact of the specialty on the teaching preparation of graduates as High. In addition, there was 47% participation in the execution of undergraduate and graduate teaching activities after having completed the specialty. Conclusions: the perception of the graduates evidenced the high impact of the Ophthalmology specialty on their teaching preparation, an essential component in medical education for the quality of the educational teaching process.

RESUMO Objetivo Caracterizar o perfil do fonoaudiólogo brasileiro com formação especializada em motricidade orofacial (MO). Métodos Estudo descritivo e exploratório, transversal, de abordagem quantitativa, realizado entre setembro 2022 e fevereiro 2023, com parceria do Conselho Federal de Fonoaudiologia. Participaram 87 fonoaudiólogos com formação em MO. Resultados Evidenciou-se predominância feminina, faixa etária entre 41 e 50 anos e tempo de formação especializada em MO entre 16 e 20 anos; maioria com título de especialista e da Região Sudeste do Brasil, atuando em consultórios/ambulatórios e/ou clínicas, com ações interdisciplinares com Medicina e Odontologia, abrangendo adultos e crianças, principalmente na rede privada. Prevaleceram os domínios da MO quanto à reabilitação de estruturas moles e hábito oral e maior uso de tecnologias: leve - acolhimento e assistência; leve-dura - avaliação e terapia e dura - fotobiomodulação. Conclusão Os fonoaudiólogos brasileiros com formação especializada em MO, participantes do estudo, são, em sua maioria, mulheres com mais de 41 anos de idade, que detêm titulação de especialista, com práticas nos vários domínios e níveis de tecnologia da área, principalmente em diagnóstico e reabilitação dos distúrbios miofuncionais orofaciais.


ABSTRACT Purpose To characterize the profile of Brazilian speech therapists with specialized training in orofacial myology (OM). Methods Descriptive and exploratory, cross-sectional study, with a quantitative approach, carried out between September 2022 and February 2023, in partnership with CFFa. Eighty-seven speech therapists with specialized training in OM participated. Results showed female predominance, age between 41 and 50 years, and time of specialized training in OM between 16-20 years; most with specialist titles in the Southeast of Brazil. Most work in offices/outpatient clinics and/or clinics with interdisciplinary actions with Medicine and Dentistry; seeing adults and children, mainly in a private sector. As to OM domains there was a predominance of rehabilitation of soft structures and oral habits, and most use technologies: light - reception and assistance, light-hard - evaluation and therapy, and hard - photobiomodulation (40.2%). Conclusion The Brazilian speech therapists with specialized training in OM participating in the study are mostly women over 41 years old, who hold the title of specialists, and carry out practices in various domains and levels of technology in the area, with a predominance of diagnosis and rehabilitation of oromyofunctional disorders.

Objective: To define the sociodemographic and professional profile of nursing professorsat the Nursing School of the University of São Paulo who acted as managers at the University Hospital of the University of São Paulo, who experienced the Teaching-Assistance Integration process,between 1978 to 2015. Method: Descriptive and exploratory study, with retrospective collection ofsociodemographic data and components referring to the professional trajectory of 11 participants.For the characterization, a tabulated data base has been elaborated in Microsoft Excel, in which itwas possible to analyze the profiles by means of simple frequencies. Results: The majority declaredthemselves white; catholic; from the State of São Paulo, Brazil; most have training in nursing; havebetween 1 and 5 years of experience in nursing care and in the management of the University Hospital of the University of São Paulo. Conclusion: The sociodemographic and professional profile ofnursing professors who acted as managers and teachers, gave possibilities to constitute essentialunits to define the reality of the study participants, in order to clarify and concatenate with the individual and social structures. Objetivos: Conocer que tipos de enfermedades profesionales se declararon en 2021 en CREPROSS y con qué sectores están relacionados en la provincia de Salamanca. Material y Métodos: Se analiza mediante un estudio descriptivo los casos de enfermedad profesional de la provincia de Salamanca. Resultados: El número de enfermedades profesionales declaradas en el año 2021 en la provincia de Salamanca fue de 105. Los trabajadores tenían entre 26 y 65 años. Los sectores según código CNAE donde se declararon el mayor número de enfermedades profesionales fueron en la sección C con 35 casos (36.75%) y la sección la sección G con 29 casos (30.45 %). Las enfermedades profesionales más declaradas fueron el STC, la epicondilitis lateral y la tenosinivitis de al estiloides radial. Conclusiones: Los datos obtenidos pueden ser un buen punto de partida para realizar estudios más específicos sobre intervenciones para la prevención y promoción de la salud en estos sectores. (AU)


Objective: To identify which types of occupational diseases were declared during 2021 in CEPROSS and wich sectors they are related to in the province of Salamaca. Material and Method: The cases of occupational disease in the province of Salamanca are analysed by means of a descriptive study. Results: The number of occupational diseases registered in 2021 in the province of Salamanca was 105. The workers were between 26 and 65 years old. The specific sectors according to the CNAE code where the highest number of occupational diseases were reported were in section C with 35 cases (36.75%) and section G 29 cases (30.45%). The most declared occupational disease were CTS, lateral epicondylitis and radial styloid tenosynovitis. Conclusions: The data obtained can be a good starting point for more specific studies on prevention and health promotion interventions in these sectors. (AU)

Resumo Os secretários municipais de saúde são atores estratégicos na construção e governança do SUS. Neste estudo, foi realizada uma análise comparativa do perfil dos secretários municipais ao longo de três décadas (1996, 2006 e 2017), com base em dados de três inquéritos transversais realizados em âmbito nacional, por meio de questionários estruturados direcionados para todos os secretários do país. A análise evidenciou aumento da participação feminina, maior diversidade étnica e racial e maior qualificação profissional dos gestores, além da renovação do perfil geracional, mas ainda com predomínio de homens brancos e importantes diferenças regionais. Foi observado avanço da democratização desses espaços de gestão, mas permanece o desafio de tornar a máquina pública mais representativa do conjunto da sociedade brasileira.


Resumen: Les secretarios municipales de salud son actores estratégicos en la construcción del SUS (Sistema Único de Salud) y participan activamente en la gobernanza del sistema. El análisis comparativo del perfil de los secretarios municipales a lo largo de tres décadas (1996, 2006 y 2017) se realizó utilizando la base de datos de tres encuestas transversales realizadas a nivel nacional con cuestionarios estructurados dirigidos a todos los secretarios municipales de salud del país. El aumento de la participación femenina, la mayor diversidad en términos étnicos y raciales, mayor cualificación profesional de los gestores, además de la renovación del perfil generacional, reitera el avance de la democratización de estos espacios de gestión. A pesar de la renovación del perfil durante la construcción del SUS, queda el desafío de hacer que la máquina pública sea más representativa de la sociedad brasileña en su conjunto.


Abstract Municipal health secretaries are strategic actors in the construction of the Brazilian national health system - SUS, actively participating in the system's governance. The comparative analysis of the profile of municipal secretaries over three decades (1996, 2006, and 2017) was carried out using the database of three cross-sectional surveys carried out at the national level with structured questionnaires aimed at all municipal health secretaries in the country. The increase in female participation, greater diversity in ethnic and racial terms, greater professional qualification of managers, and the renewal of the generational profile, reiterates the advance of the democratization of these management spaces. Despite the renewal of the profile during SUS's construction, the challenge remains to make the public machine more representative of Brazilian society as a whole.

ABSTRACT

PURPOSE: To identify the current ambiguous status of general medicine (GM) and assess current problems and weaknesses for further development of GM. PATIENTS AND METHODS: This study adopted an observational design. GM practitioners were selected from the mailing lists of two primary professional associations for Japanese GM doctors. We included physicians currently working in a GM department, those self-described as GM doctors, and those board-certified in family medicine or general internal medicine. Respondents replied to survey questions about their sociodemographic data, job descriptions, and the requisite skills for practice. GM doctors' job description and required skills were categorized into "clinical," "management," "education," and "research." Participants (n = 971) were compared based on job descriptions and important skills in each category by facility type, size, and position. RESULTS: "Clinical" was indicated as the most important category for both job description and important skills, followed by "management," "education," and "research." For job description details, "follow-up outpatient" (35.6%) ranked first for "clinical," and "resident education" (57.3%) ranked first for "education." By facility type and size, job description and important skills decreased for clinical and management categories as facility sizes increased; the opposite was true for "education." "Research" was generally rated low. By position, no significant difference was found in effort or importance given to research. CONCLUSION: This study is the first survey on GM physicians across Japan. The results show that while Japanese GM physicians focus on and place importance on clinical practice, they are less involved in research and do not consider research skills to be important. The challenge for the future development of GM lies in research.
