ABSTRACT

Marital distress increases the risk of morbidity and mortality by altering psychological and biological health, including cardiovascular and endocrine function. Numerous interventions have been developed to mitigate marital distress. However, most of these interventions had very specific content and were limited to certain segments of the population. This study was conducted to evaluate the effectiveness of a marital relationship skills training intervention on the marital adjustment of married people in Ethiopia. We conducted a randomized-controlled trial study on 80 married people with low marital satisfaction. We used STATA 14 statistical software to process the data. The results of an Independent sample's t test analysis showed that marital relationship skills training had a significant impact on the marital adjustment of distressed couples (p = 0.001, 95% confidence interval: 0.79-1.10). A marital relationship skills training intervention can effectively improve marital adjustment and can be used as a treatment for marital distress.

ABSTRACT

This paper provides a critical analysis and synthesis of the current status and emerging developments in contemporary couple therapy. Its narrative centers on the evolution of couple therapy into a prominent intervention modality and coherent body of practice. The review begins with the consideration of the field's strong empirical underpinnings derived from research on couple therapy and basic relational science. Couple therapy comprises the widely accepted method for reducing relationship distress and enhancing relationship quality. Moreover, both as a stand-alone intervention and in conjunction with other treatment formats, couple-based interventions have garnered considerable empirical support for their effectiveness in addressing a broad spectrum of specific relational dysfunctions as well as individual emotional and physical health problems. We highlight the convergence of methods through common factors, shared strategies, and remarkably similar arrangements across approaches. Our review also points to key differences among approaches, the importance of recognizing respective strengths and limitations linked to these differences, and building on differences across models when selecting and tailoring interventions for a given couple. The discussion concludes with a consideration of recent trends in the field including the impact of telehealth and related digital technologies, the expansion of specific treatments for specific problems and diverse populations, the interface of couple therapy with relationship education, and enduring challenges as well as new opportunities addressing broader systemic and global dynamics.

ABSTRACT

Background: One of the most important causes of marital dissatisfaction is the lack of life skills. This study aimed to compare the effects of training self-awareness and problem-solving skills on marital satisfaction of married women. Methods: This study was a randomized field trial conducted in Gorgan since October 2019 to September 2020. A total of 105 married women were allocated to one of two interventions or a control group based on triplex block randomization. Data collection tools were the Persian version of the self-awareness questionnaire, problem-solving questionnaire and Four ENRICH Couple Scales. The participants completed the questionnaires before (T1), immediately (T2), and one month after the intervention completion (T3). The first intervention group received self-awareness skills training and the second received problem-solving skills training one session per week for four weeks. The control group did not receive any training. Data were analyzed using One-way ANOVA, Kruskal-Wallis, Chi-square, Fishers exact test, exact test, repeated measure ANOVA, and Bonferroni test, using SPSS 16. A P value <0.05 was considered significant. Results: The mean score of ENRICH Scales in the three groups, immediately and one month after the intervention, were not statistically significant (P>0.05). However, in the problem-solving group, the scale of marital satisfaction was significantly different at T2 and T3 (31.40±4.55, 33±5.21, P=0.008). In the self-awareness training group at T1 and T3, the mean of the conflict resolution scale (32.60±63.28, 33.94±5.74, P=0.03) was significantly different. Conclusion: The results showed that problem-solving skills training was more effective in increasing the score of marital satisfaction compared to self-awareness skills training.Trial Registration Number: IRCT20190721044290N1.

BACKGROUND: Divorce rates have increased during the last decade, leading to a greater focus of marital scholars on the importance of understanding couple-maintaining strategies within marital life. Distresses in couples are attributable to difficulties controlling felt, experienced, and expressed emotions; thus, emotion dysregulation is a core stressor in couples with maladaptive responses. OBJECTIVE: The aim of the study was to evaluate the effect of Dialectical Behaviour Therapy (DBT) on outpatient couples to treat emotion dysregulation. METHODS: We recruited 20 couples with marital distress in which partners presented emotion dysregulation. We offered the couples the opportunity to join a couple DBT group at their convenience and based on the immediate availability of treatment slots. We measured the treatment efficacy using psychometric tools (the Difficulties in Emotion Regulation Scale (DERS) and the Dyadic Adjustment Ccale (DAS) at baseline and after DBT therapy. RESULTS: Both male and female partners presented significant improvements in marital adjustment DAS and emotion regulation scores. Female partners showed significantly greater amplitude changes in both scales. Female partners showed significant improvement in most DERS subscales (except the GOALS subscale); on the other hand, male partners showed significant improvements in impulse, awareness, strategies, and clarity subscales. We found significant improvements in most DAS subscales in both sexes; only affectional expression remained unchanged before and after therapy. CONCLUSION: DBT for couples is an effective approach to treat emotion dysregulation.

BACKGROUND: Quality of life (QOL) and marital contentment, especially marital satisfaction, are important aspects of life. These items are more important in couples involved in HIV due to the present social stigma among this population considering women more vulnerable. The aim of this study was to determine the QOL and marital contentment status among seroconcordant and serodiscordant HIV couples compared to non-HIV ones. METHODS: In this cross-sectional study, 66 serodiscordant, 74 seroconcordant, and 70 non-HIV couples who referred to Lavan High-risk Behavior Counseling Center, Shiraz during September 2017 and December 2019 were studied. QOL and marital contentment were assessed by World Health Organization Quality of Life-BRIEF (WHOQOL BREF) and ENRICH questionnaire, respectively. Chi-square test for qualitative variables, independent T-test and ANOVA followed by LSD post hoc test for quantitative variables were performed. All statistical analyses were performed using SPSS 19.0, and P<0.05 was set as the significant level. RESULTS: The score of QOL questionnaire was significantly higher in non-HIV couples than serodiscordant and seroconcordant groups (P<0.001). There was no significant difference among seroconcordant and serodiscordant groups (P=0.99), and infected males vs. females (P=0.13). Non-HIV couples had significantly higher marital contentment in comparison to serodiscordant and seroconcordant groups (P<0.001). No difference was detected among seroconcordant and serodiscordant groups (P=0.81) although more contentment was observed among the males (P=0.01). CONCLUSION: Our study revealed that QOL and marital contentment were different among non-HIV and HIV infected couples. Besides, marital contentment was higher among males than female's in infected patients.

ABSTRACT

This study explored the change that unfolded when parents resolved their coparenting dissatisfaction during an Integrative Brief Systemic Intervention (IBSI) for parent couples. We conducted a task analysis (Greenberg, 2007) to build a model of resolving coparenting dissatisfaction. We compared a postulated model of change (rational model) based on theoretical and clinical assumptions to the observations of the actual change process that couples experienced in an IBSI (empirical analysis). The empirical analysis was conducted on six IBSI therapy cases (three exhibiting positive development and three exhibiting no development). We defined positive development in IBSI as moving from coparenting dissatisfaction to coparenting satisfaction. The final rational-empirical model included six steps that facilitated the resolution of coparenting dissatisfaction. This study contributes to deepening the knowledge of how coparenting may change during marital therapy.

ABSTRACT

OBJECTIVE: Treatment for posttraumatic stress disorder (PTSD) is a commonly sought mental health service among military service members and veterans (SM/Vs). Such treatment is typically individually-based, despite many SM/Vs reporting a desire for greater partner involvement in treatment. This review examined couple-based treatments for PTSD among SM/Vs and their romantic partners. METHOD: A database search conducted in July, 2018 yielded 167 studies, of which 16 (10%) met inclusion criteria. Brief intervention summaries, effect sizes, and distress change scores (where applicable) are reported. RESULTS: The 16 studies tested 7 interventions, which showed a reduction in self-rated and clinician-rated PTSD symptoms with large effect sizes observed in most studies. Relationship outcomes also improved for SM/Vs and their partners, with effect sizes ranging from small-to-medium for SM/Vs and small-to-large for partners. CONCLUSIONS: Couple-based interventions show success in reducing PTSD symptoms and improving relationship outcomes, offering several alternatives to individual-based interventions among partnered SM/Vs.

ABSTRACT

While it is known that client factors account for the largest proportion of outcome variance across treatment modalities, little is known about how clients' characteristics affect the process and effectiveness of couple therapy. To further knowledge in this area, we created a brief, practice-friendly measure, the Expectation and Preference Scales for Couple Therapy (EPSCT). Three self-report scales assess clients' Outcome expectations (e.g., I expect our relationship to improve as a result of couple therapy) and role expectations for Self (e.g., I expect to listen to my partner's concerns) and Partner (e.g., I expect my partner to blame me). Three Cognitive-Behavioral, Emotionally Focused, and Family Systems preference scales use a forced-choice format to measure the comparative strength of respondents' preferences for interventions broadly reflective of each approach. A large item pool was developed from relevant literature and clinical experience and refined based on face and content analyses with two panels of experienced couple therapists and researchers. Across four studies with 1,175 participants, the scales' internal consistency reliabilities were similar and their construct validity was supported with confirmatory factor analyses and significant correlations with several established measures, including expectation measures developed for individual psychotherapy and measures of attitudes toward professional help seeking and valuing personal growth. Across all studies, participants had stronger role expectations for themselves than their partners, although gender effects differed by sample. We discuss how to use the 15-item EPSCT in clinical practice and in future research as a predictor of couple therapy processes and outcomes.


Aunque se sabe que los factores de los clientes representan la mayor parte de la variación en los resultados a través de las modalidades de tratamiento diversas, no se sabe mucho acerca de cómo las características de los clientes afectan el proceso y efectividad de la terapia de pareja. Para avanzar el conocimiento en este campo, hemos creado una breve medida favorable a la práctica, las Escalas de expectativas y preferencias para terapia de pareja (EPSCT en inglés). Tres escalas de autoinforme evalúan las expectativas de resultados de los clientes (p.ej. Espero que nuestra relación mejore como resultado de la terapia de pareja) y las expectativas de roles para sí mismo (p.ej. Espero escuchar las preocupaciones de mi pareja) y para la pareja (p.ej. Espero que mi pareja me culpe). Tres escalas de preferencias cognitivo-conductiva, enfocada en emociones y de sistemas familiares emplean un formato de elección forzada para medir la fortaleza comparativa de las preferencias de los interrogados por intervenciones que reflejan cada enfoque a grandes rasgos. Se elaboró un conjunto amplio de ítems a partir de la literatura y experiencia clínica pertinentes y se refinó a base de análisis de apariencia y contenido con dos paneles de terapistas de pareja e investigadores experimentados. En la totalidad de cuatro estudios con 1175 participantes, las fiabilidades de coherencias internas fueron similares y su validez de constructo fue respaldada por análisis factoriales confirmatorios y correlaciones significativas con varias medidas ya establecidas, incluyendo medidas de expectativas desarrolladas para psicoterapias individuales y medidas de actitudes hacia la búsqueda de ayuda profesional y la valoración del crecimiento personal. En todos los estudios, los participantes mostraron expectativas de roles más fuertes para sí mismos que para sus parejas, aunque hubo diferencias en los efectos de género por muestra. Discutimos cómo usar el EPSCT con sus 15 ítems en la práctica clínica e investigaciones futuras como predictor de procesos y resultados de la terapia de pareja.

ABSTRACT

Substance abuse is a family disease that adversely impacts both the user and the user's family. The family can act as a risk factor for the development of substance abuse among children and adults. The family can also be involved in therapy to either help the recovery process or prevent substance abuse. Marital and family therapy have been found to be effective in reducing the severity of substance use, lowering marital and family conflict, improving family communication and cohesion as well as effective parenting practices. Behavioural Couples Therapy has been found to have good empirical support for bringing about the desired changes in both substance abuse and marital relationship. While targeting entire families, the most common evidenced based family interventions are Brief Strategic Family Therapy, Multidimensional Family Therapy, Family Behaviour Therapy, Functional Family Therapy and Community Reinforcement Programme. Marital and family therapy have to be sensitive to gender and culture. Effective use of marital and family therapy requires adequate training to equip practitioners in adequately treating not only substance use disorders and family pathology, but also in treating co-morbid mental health conditions.

BACKGROUND: Infertile couples only think of having children during their sexual intercourse, and their constant concern about this issue increases their stress level. Psychosocial and social stress leads to decreased life satisfaction, increased marital problems, and reduced sexual confidence. This study aims to determine the effect of enrichment program on marital and sexual satisfaction as well as marital intimacy among infertile couples. MATERIALS AND METHODS: This randomized controlled clinical trial was conducted on 50 infertile couples in 2013 in Hamedan. The marital relationship enrichment program was taught to the experimental group during seven 90 minutes sessions. Enrich marital satisfaction, Linda Berg sexual satisfaction, and marital intimacy questionnaires were completed by both groups in 3 pretest steps immediately after the end of training sessions, and 8 weeks later. The results were analyzed in STATA11 software using t test, Chi-square, ANCOVA, RM-ANOVA, and Bonferroni post-hoc test. To check the data normality, Kolmogorov-Smirnov test was used. P<0.05 was considered significant. RESULTS: Comparison of mean scores related to pretest on the one hand and immediately after the test in 8 week later on the other hand showed marital relationship enrichment program significantly increased marital and sexual satisfaction (P<0.001). Also, mean score of marital intimacy immediately after the test (P=0.04) and 8 weeks after the test (P<0.001) significantly increased in comparison with the pretest under the influence of the program. CONCLUSION: Enrichment training can increase marital intimacy and also marital and sexual satisfaction in infertile couples (Registration Number: IRCT201604299014N97).

Resumen El objetivo de este estudio fue reducir los niveles de celos en seis mujeres, estudiantes universitarias, con edades entre los 18 y 35 años de edad, que presentaban celos patológicos. El proceso terapéutico utilizado se basó en la guía de intervención cognitivo comportamental para el manejo de los celos en la relación de pareja. La intervención tuvo una duración de dos meses y se desarrolló en siete sesiones, bajo un diseño AB y con seguimiento a los 6 meses a dos de las consultantes. Los resultados muestran una disminución en el puntaje de la Escala Interpersonal de Celos. Se encontraron diferencias significativas en los resultados globales de la escala en la aplicación pre y pos test (p<,05), así como en la frecuencia de conductas asociadas al problema. Se discuten las bondades y limitaciones de la intervención.


Abstract The aim of this study was to reduce the level of jealousy in six female college students with ages ranging from 18 to 35 years who presented pathological jealousy. The therapeutic process used was based on the cognitive-behavioral intervention guide for jealousy management in couple relationships. Under an AB design, the intervention had a timespan of two months and was developed throughout seven sessions. A follow-up was performed six months later for two of the six subjects. The results show a decrease in Interpersonal Jealousy Scale score and significant differences were found in the global results of the IJS scores in pre- and post-application of the intervention (p<,05). Other differences were seen in the frequency of behavior associated to problematic jealousy. The strengths and limitations of the intervention are discussed.

Objetivo: O objetivo deste trabalho foi verificar a relação entre a satisfação conjugal e a depressão pós-parto e o impacto destas variáveis nos níveis de satisfação conjugal e depressão futuros, em uma amostra brasileira. Estudos revelam que o conflito conjugal é um antecedente importante no desenvolvimento da depressão e que a depressão pós-parto influencia significativamente a satisfação conjugal. Métodos: Este estudo longitudinal com 99 mulheres brasileiras utilizou análises de regressão múltipla para verificar a associação entre a satisfação conjugal e a depressão. A sintomatologia depressiva foi avaliada pelo instrumento Self-Reporting Questionnaire (SRQ-20). A satisfação conjugal foi verificada a partir de itens sobre a percepção de satisfação conjugal das participantes. Os dados foram coletados 4 meses e 2 anos após o parto. Os dados longitudinais possibilitaram a análise cronológica da interação entre as duas variáveis. Resultados e Conclusões: Os resultados indicam que o modelo teórico que estabelece a relação entre conflito conjugal e depressão é também apropriado para casais brasileiros, sugerindo a utilização potencial da terapia de casal no tratamento da depressão pós-parto no Brasil.


Objective: This study examined the relationship between marital satisfaction and postpartum depression as well as the impact of postpartum depression and marital satisfaction on later relational characteristics among Brazilian couples. Studies have shown that marital satisfaction is an important antecedent to depression and that postpartum depression has an influence on marital satisfaction. Methods: A longitudinal study using multiple regression to test the association between marital satisfaction and depression was conducted among Brazilian women. Depressive symptomatology was assessed using the Self-Reporting Questionnaire 20 and marital satisfaction using an instrument developed by the researchers to test the individuals' perception of their marital quality. Data were collected at 4 months and 2 years after childbirth. The longitudinal data allowed researchers to chronologically analyze the interaction of key variables. Results and Conclusions: Results showed that the model established between marital satisfaction and depression is appropriate for Brazilian couples as well as for postpartum women. This study highlights the potential impact of marital treatment on postpartum depression in Brazil.

Feedback to therapists based on systematic monitoring of individual therapy progress reliably enhances therapy outcome. An implicit assumption of therapy progress feedback is that clients unlikely to benefit from therapy can be detected early enough in the course of therapy for corrective action to be taken. To explore the possibility of using feedback of therapy progress to enhance couple therapy outcome, the current study tested whether weekly therapy progress could detect off-track clients early in couple therapy. In an effectiveness trial of couple therapy, 136 couples were monitored weekly on relationship satisfaction and an expert derived algorithm was used to attempt to predict eventual therapy outcome. As expected, the algorithm detected a significant proportion of couples who did not benefit from couple therapy at Session 3, but prediction was substantially improved at Session 4 so that eventual outcome was accurately predicted for 70% of couples, with little improvement of prediction thereafter. More sophisticated algorithms might enhance prediction accuracy, and a trial of the effects of therapy progress feedback on couple therapy outcome is needed.

BACKGROUND: Anxiety disorders may affect nearly one in four persons and may cause significant impairment of interpersonal relationships including marital relationships. The effect of the disorder on the spouse and the impact of including the spouse in therapy are not well studied. AIM: To determine if Cognitive Behavioural Therapy (CBT) improves the quality of life of participants with anxiety disorders and if marital adjustment of couples with anxiety disorders can be improved with Behavioural Marital Therapy (BMT), relative to standard care of pharmacotherapy and psychoeducation. METHODS: An open label randomised controlled trial. Participants were randomly assigned to CBT+BMT or standard of care. Final assessments were carried out at 3.5 months after baseline. Quality of life was assessed using the WHOQOL-Bref instrument and Marital adjustment was measured using a marital quality scale. Chi-square test, student's t-test and Analysis of Variance (ANOVA) and Effect sizes with Cohen's d were used to compare differences between groups. RESULTS: Clinically meaningful effect sizes for the CBT+ BMT intervention were evident for the marital adjustment scores among participants (d=0.63) and their spouses (d=1.29), and for the psychological (d=0.84), social (d=0.72) and environmental (d=0.52) domains of the WHOQOL of participants and psychological (d=0.86), social (d=0.32) and environmental domains (d=1.01) of the WHOQOL of spouses of participants. CONCLUSION: CBT for the partner with anxiety disorder and BMT for couples with anxiety disorders and marital discord and involvement of the spouse in the therapy will be a useful addition to the management of a couple where one partner has an anxiety disorder.

ABSTRACT

O diálogo na terapia conjugal e familiar (TCF) é, por definição, diálogo de múltiplos atores: na terapia conjugal, do terapeuta com dois parceiros da mesma geração; na terapia familiar, com um mínimo de dois membros da família de, ao menos, duas gerações diferentes. Isso representa importante desafio aos terapeutas, já que implica que a conversação é, com frequência, carregada de tensões e pode ter consequências dramáticas para os membros da família na vida real. Na primeira parte do artigo desenvolvemos uma base conceitual, em que nos apoiamos na literatura cujo objeto é a TCF, assim como na discussão conceitual no campo ao redor do conceito do não-saber. Questionamos a eficiência terapêutica das intenções igualitárias do terapeuta e a visão idealista do diálogo naturalmente curativo ou sanador implícita nesse conceito. Destacamos, sobretudo, a validade de alguns dos conceitos de Mikhail Bahktin (endereçamento, responsividade...) para nos dirigir às complexidades específicas do encontro da TCF. Em seguida, apresentamos um estudo de caso no qual nos limitamos a uma microanálise dos primeiros minutos de uma primeira sessão de terapia conjugal. Na terceira parte do artigo, discutimos o que aprendemos desse caso, confrontando-o com alguns conceitos e ideias centrais desenvolvidas na primeira parte. Dessa forma, concluímos que o terapeuta conjugal e familiar não pode escapar da posição desconfortável de assumir a responsabilidade de encontrar caminhos para contribuir ativamente para um dialogo frutífero que absolutamente não é um dado natural, mas, ao contrário, um projeto.(AU)


Dialogue in marital and family therapy (MFT) is by definition a multi-actor dialogue: in marital therapy it is a dialogue of the therapist with two partners of the same generation, and in family therapy it is a dialogue with at least two family members of at least two different generations. This raises important challenges for therapists as this means that the conversation is often tension filled and can have dramatic real life consequences for the family members. In the first part of the paper we develop a conceptual frame in which we rely on the MFT literature on the subject as well as on the conceptual discussion in the field around the concept of not-knowing. We question the therapeutic effectiveness of the therapist’s egalitarian intentions and the concept’s implicit idealistic view of the naturally healing dialogue. We especially highlight the validity of some of the concepts of Mikhail Bahktin (addressivity, responsivity, …) to address the specific complexities of the MFT encounter. Then we present a case study in which we will limit ourselves to a micro-analysis of the first minutes of a first session of a marital therapy. In the third part of the paper we discuss what we learned from the case-study against the background of some of the central concepts and ideas we developed in the first part. This leads us to the conclusion that the marital and family therapist can not escape the uncomfortable position of having a responsibility in finding ways to actively contribute to a fruitful dialogue that is not a natural given at all, but rather a project.(AU)

ABSTRACT

A modificação da interação entre os parceiros e o treino de resolução de problemas foram aspectos importantes desde o início do movimento da terapia comportamental de casal. Entretanto, o treino de comunicação sempre se destacou por possibilitar que os parceiros mudem o processo central de negociação do relacionamento, de enfrentamento de problemas e de convivência em geral. Destarte, esse aspecto é potencialmente forte e capaz de influenciar outros fatores relevantes na terapia de casal. Analisando detalhadamente como oficinas de comunicação foram conduzidas desde o início histórico da terapia comportamental de casal, percebe-se quanto esse procedimento continua atual dentro da filosofia contextualista da nova onda da terapia comportamental integrativa de casal. O presente artigo destaca assim o treino de comunicação como ingrediente essencial da abordagem comportamental de casal.


Changes in behavioral exchanges between partners and problem-solving training have been important aspects since the beginning of the movement in behavioral marital therapy. Communication training stands out because it provides the opportunity for partners to change the central process of relationship negotiations and their mode in coping with problems and their relationships in general. Communication training has a potentially powerful influence on other relevant factors that comprise marital therapy. Analyzing how communication work-shops were conducted since the historical beginning of behavioral marital therapy, it may be observed how much this procedure remains updated and compatible with the contextual philosophy of the contemporary wave of integrative behavioral marital therapy. Current essay argues that communication training must be considered an essential ingredient in behavioral marital therapy.
