Desdobramentos psicopatológicos da identificação projetiva na parte bebê do self / Psychopathological evolution of projective identification in the baby part of the self

RESUMO: Discutiremos os desdobramentos psicopatológicos na zona arcaica da subjetividade quando a identificação projetiva perde sua dimensão de comunicação primitiva, assumindo um caráter radical de evacuação e prejudicando a rêverie materna. Estudaremos as experiências precoces do psiquismo englobadas na parte bebê do self, fundamentais aos processos de simbolização primária. Suas falhas se manifestam na clínica através de intensos sofrimentos capitaneados por angústias primitivas, como as de abandono e de separação, que acarretam respostas radicais, tais como a fragmentação e a despersonalização. Apresentaremos um fragmento clínico para ilustrar a forma de desorganização psíquica própria às falhas na parte bebê do self.

Abstract: We will discuss the psychopathological evolution in the archaic zone of subjectivity when the projective identification loses its primitive communication dimension, assuming a radical role of evacuation and damaging the maternal rêverie. We will study the early experiences of the psyche embedded in the baby part of the self, fundamental to the processes of primary symbolization. Their failures are manifested in the clinic through intense sufferings captained by primitive anxieties, such as abandonment and separation, which lead to severe responses such as fragmentation and depersonalization. We will present a clinical fragment to illustrate the archaic forms of psychic disorganization proper to failures in the baby part of the self.

La autora nos habla de aquellos pacientes que acuden a consulta con miedo al derrumbe psíquico. A través de diferentes autores (Freud, Winnicott, Bion, Ferenczi, Botella y Green), trata de dar respuesta a los interrogantes metapsicológicos y clínicos que nos plantean en la clínica. Presenta un caso en el que podemos observar la angustia desintegradora primitiva de una mujer y cómo en el análisis se pone en marcha un equivalente del proceso de recordar de todo lo irrepresentable en ella, de aquello que ha sido vivenciado, pero no representado, evidenciándose una rêverie materna narcisista que ha dejado a la paciente en un estado de desamparo que se reactiva ante diferentes situaciones y que se va modificando a través del proceso psicoanalítico. The author talks about those patients who come to her office fearing psychic collapse. By means of different authors (Freud, Winnicott, Bion, Ferenczi, Botella and Green) she attempts to answer the metapsychological and clinical questions that these patients raise for us in treatment. She presents a case in which we may observe one woman’s primitive disintegrative anxiety and how in analysis a process is set in motion comparable to that of remembering everything that is unrepresentable in her, a process of remembering that which has been experienced but not represented, revealing a narcissistic maternal rêverie that has left the patient in a state of helplessness which is reactivated in different situations and is gradually modified during the process of analysis. L’auteur nous parle de ces patients qui viennent consulter par crainte d’un effondrement psychique. Grâce à différents auteurs (Freud, Winnicott, Bion, Ferenczi, Botella et Green). Elle tente de donner des réponses aux questions métapsychologiques et cliniques qui nous sont posées à la clinique. Elle présente un cas dans lequel on peut observer l’angoisse désintégrative primitive d’une femme et comment dans l’analyse se met en marche un équivalent du processus de mémorisation de tout ce qui est non représentable en elle, de ce qui a été vécu mais non représenté, mettant en évidence une rêverie maternelle narcissique qui a laissé la patiente dans un état d’impuissance qui se réactive devant différentes situations et qui est modifié par le processus psychanalytique. O inferno de Ana / Ana's hell

Trata-se de um estudo, ilustrado através de um caso clínico, sobre a parte psicótica e não psicótica da personalidade e sobre a função continente do terapeuta. O caso se refere a uma adolescente já bastante frágil por sua história de vida pregressa, que, a partir de um fator externo, passa a viver com o predomínio da parte psicótica da personalidade. A reflexão sobre a oscilação entre a parte psicótica e a não psicótica da personalidade e suas manifestações é ampliada em dois aspectos: (a) a forma de comunicação do paciente e (b) o papel do terapeuta diante de situações em que se identifica o predomínio do funcionamento da parte psicótica da personalidade. Para desenvolver a compreensão do caso, o autor baseia-se em conceitos de Wilfred Bion e de outros teóricos das relações de objeto(AU)

This is a study about the psychotic and non-psychotic sides of the personality and the therapist’s container function, shown through a clinical case. The case refers to a teenager, already very fragile because of her early life history, who started living predominantly with the psychotic side of her personality, due to an external factor. The reflection on the fluctuation between the psychotic and non-psychotic personality sides and their manifestations is extended in two aspects: (a) the patient’s way of communication and (b) the therapist’s role in situations where the predominance of the functioning of the personality's psychotic side is identified. To develop an understanding of the case, the author bases himself on the concepts of Wilfred Bion and other theorists of object relations(AU)
