ABSTRACT

Abstract Objective: This study sought to investigate the relationship between clinical response to nonsurgical periodontal therapy (NSPT) and serum changes in leukocyte count, fasting blood glucose, hemoglobin, hematocrit, creatinine, and uric acid in kidney transplant recipients (KTR). Methodology: A prospective study was performed on 20 KTRs. Periodontal and serum data were collected before and 90 days after NSPT, and delta values (Δ = after NSPT - before) were calculated. Periodontal assessment included periodontal probing depth (PPD), clinical attachment level (CAL), and bleeding on probing (BOP). Patients were classified based on the presence of periodontitis and then categorized into stages. Results: Patients showed a reduction in the percentage of sites with PPD≥3mm, PPD≥4 mm and BOP, after NSPT. There was a direct correlation between the deltas of leukocyte count and CAL ≥3 mm (r=0.645, P=0.002) and BOP (r=0.663, P=0.001), and the deltas of uric acid and CAL ≥3 mm (r=0.562, P=0.010). Conclusion: A good clinical response to NSPT may affect the reduction of serum levels of leukocyte count and uric acid, suggesting a beneficial effect on systemic health in KTR.

ABSTRACT

Controlled release devices (CRD) have been widely studied regarding their application in periodontal therapy. Therefore, the present study aims to evaluate the use and effectiveness of controlled-release devices in nonsurgical periodontal therapy through a systematic review. The research was carried out in six different databases, namely: Online Medical Literature Search and Analysis System (Medline-PubMed), Web of Science, Science Direct, Scopus, Scielo, and Google Scholar. The descriptors "Delayed-Action Preparation," "Therapeutics," and "Periodontitis," were used with their EntryTerms and connected through the Boolean operators AND and OR. A total of 2847 studies were found, and after applying the Preferred Reporting Items for Systematic Reviews and Meta-Analyses checklist flowchart, 34 were selected using the eligibility criteria. After that, the qualitative results were tabulated and the risk of bias in each of them was evaluated. It can be observed that the use of CRD presents itself as a successful alternative for adjuvant treatment to periodontal therapy, a fact due to its availability and local concentration in the crevicular fluid. However, further clinical research is still needed to develop devices that are effective, with an easy and quick application, as well as available at a good cost-benefit ratio.

ABSTRACT

The purpose of this article was to assess the effectiveness of ozone therapy as an adjunct to mechanical therapy in periodontitis patients. Thirty-two patients diagnosed with generalized periodontitis were selected, with a total of 655 teeth examined. Each patient's mouth was divided into four quadrants (the split-mouth model) to be randomly treated with four sessions of gaseous ozone or air. The following clinical variables were recorded: the gingival index, the periodontal clinical attachment loss, the Miller's mobility index and the clinical improvements, as assessed through the visual analog scale (VAS). In addition, the microorganisms were qualitatively compared. After four weeks of treatment, the teeth of the ozone-treated quadrants showed statistically significant reductions in the gingival index and an improvement in the clinical attachment (p < 0.0001). The same treatment also significantly improved mobility by between 70% and 86% compared to the control group (p < 0.0001). Statistically significant differences were also recorded for the VAS (p < 0.0001). In the qualitative study of the subgingival flora, significant differences were observed (p < 0.0001). The overall results of this trial support the view that ozone treatment is effective and well tolerated in cases of generalized chronic periodontitis.

ABSTRACT

BACKGROUND: The aim of this systematic review with meta-analysis was to analyze the effects of intra-pregnancy nonsurgical periodontal therapy on periodontal inflammatory biomarkers and adverse pregnancy outcomes. METHODS: On June 5, 2017, we searched PubMed, Cochrane, SCOPUS, Web of Science, LILACS, ProQuest, Open Grey, and Google Scholar databases. Randomized clinical trials in which pregnant women with chronic periodontitis underwent nonsurgical periodontal therapy, compared with an untreated group, tested for inflammatory biomarkers, and followed till delivery were included. Primary outcomes were preterm birth, low birth weight, and preeclampsia. Meta-analysis was performed with 5.3.5 version of Review Manager software. RESULTS: We found 565 references in the databases, 326 after duplicates removal, 28 met criteria for full text reading, and 4 met eligibility criteria for quantitative and qualitative synthesis. Intra-pregnancy nonsurgical periodontal therapy improved periodontal clinical parameters (periodontal pocket depth, clinical attachment level, and bleeding on probing) and reduced biomarker level from gingival crevicular fluid (GCF), and some from blood serum; however, it did not influence biomarker level from umbilical cord blood. Meta-analysis showed tendency for reduction of the risk of preterm birth before 37 weeks for treated group (risk ratio (RR) = 0.54, 95% CI 0.38-0.77; p = 0.0007; inconsistency indexes (I2) 32%) but did not show any difference for low birth weight occurrence (RR = 0.78, 95%CI 0.50-1.21; p = 0.27; I2 41%). No included study considered preeclampsia as a gestational outcome. CONCLUSIONS: These results demonstrated that the intra-pregnancy nonsurgical periodontal therapy decreased periodontal inflammatory biomarker levels from gingival crevicular fluid and some from serum blood, with no influence on inflammatory biomarker level from cord blood, and it did not consistently reduce adverse gestational adverse outcome occurrence. Introducción: en el tratamiento de las enfermedades periodontales contamos con la terapia periodontal no quirúrgica, la cual ha sido avalada científicamente mostrando su efectividad. El principal objetivo de este artículo es demostrar la efectividad de la terapia periodontal no quirúrgica evidenciada en múltiples estudios con relación a las indicaciones, aspectos microbiológicos, efectos enlos tejidos y la importancia de la terapia de mantenimiento una vez finalizado el tratamiento. Métodos: se hizo una revisión con relación al tema en los últimos años teniendo como patrón los conceptos clave periodontales. Resultados: la terapia periodontal no quirúrgica (TPNQ) no es un procedimiento que pueda y deba realizarse en un corto tiempo y en pocas citas, el tiempo para su ejecución se amplía lo necesario en especial para lograr una limpieza y regularización de las raíces lo más completa posible. Conclusión: varios autores reportanque la reducción de la microbiota se mantuvo entre 14 y 180 días, luego de la terapia, esto justifica las citas periódicas de mantenimientoperiodontal y estos señalan que el aspecto crítico de la terapia no es la escogencia entre un procedimiento quirúrgico o no quirúrgico, sino la limpieza detallada y completa por el profesional y el buen nivel de higiene bucal por parte del paciente.


Introduction: In the treatment of periodontal diseases, we can rely on nonsurgical periodontal therapy, which has been scientifically proven as its effectiveness has been recognized. The main objective of this article is to demonstrate the usefulness of nonsurgical periodontal therapy, as evidenced in several studies that serve as guidelines, as well as microbiological aspects, effects on tissues and theimportance of maintenance therapy after treatment completion. Methods: a review was conducted in order to revise this topic considering key periodontal concepts as a guideline. Results: nonsurgical periodontal therapy (NSPT) is a procedure that cannot and should not be carried out in a few sessions; the time required for its completion is extended as necessary in order to achieve cleaning and adjustmentof the roots as completely as possible. Conclusions: Several authors have reported that reduction of microbiota remains between 14 and 180 days after therapy. This explains the regular periodontal maintenance appointments. They also indicate that the critical aspect of this therapy is not the choice between a surgical or a nonsurgical procedure, but a detailed and thorough cleaning by the dental professionaland the good level of oral hygiene achieved by the patient.
