ABSTRACT

Advanced clinical practitioners (ACPs) are integral to modern healthcare, providing high-quality, evidence-based care to patients. While ACPs show some development in clinical practice, leadership, and education, challenges persist in the research pillar. Trainee and qualified ACPs often have difficulties initiating their research journey and may feel uncertain about where to start. Existing studies have explored and emphasised the importance of ACPs developing and maintaining their research skills, yet there remains a gap in understanding how they can effectively demonstrate their research capabilities. Therefore, this article aims to help trainees and qualified ACPs on how to evidence their research pillar capabilities.

ABSTRACT

BACKGROUND: Nurses are vital partners in the development of pediatric neurocritical care (PNCC) programs. Nursing expertise is acknowledged to be an integral component of high-quality specialty patient care in the field, but little guidance exists regarding educational requirements to build that expertise. We sought to obtain expert consensus from nursing professionals and physicians on curricular priorities for specialized PNCC nursing education in pediatric centers across the United States. METHODS: We used a modified Delphi study technique surveying a multidisciplinary expert panel of nursing professionals and physicians. Online surveys were distributed to 44 panelists over three rounds to achieve consensus on curricular topics deemed essential for PNCC nursing education. During each round, panelists were asked to rate topics as essential or not essential, as well as given opportunities to provide feedback and suggest changes. Feedback was shared anonymously to the panelist group throughout the process. RESULTS: From 70 initial individual topics, the consensus process yielded 19 refined topics that were confirmed to be essential for a PNCC nursing curriculum by the expert panel. Discrepancies existed regarding how universally to recommend topics of advanced neuromonitoring, such as brain tissue oxygenation; specialized neurological assessments, such as the serial neurological assessment in pediatrics or National Institutes of Health Stroke Scale; and some disease-based populations. Panelists remarked that not all centers see specific diseases, and not all centers currently employ advanced neuromonitoring technologies and skills. CONCLUSIONS: We report 19 widely accepted curricular priorities that can serve as a standard educational base for PNCC nursing. Developing education for nurses in PNCC will complement PNCC programs with targeted nursing expertise that extends comprehensive specialty care to the bedside. Further work is necessary to effectively execute educational certification programs, implement nursing standards in the field, and evaluate the impact of nursing expertise on patient care and outcomes.

ABSTRACT

Addiction nurses are highly skilled providers of holistic care and ensuring workforce sustainability is key to providing quality care to a traditionally marginalised group of healthcare consumers. The aim of this study was to explore perceived stigma towards the addiction nursing speciality, addiction nursing (also known as alcohol and other drug nursing) and its impact on workforce sustainability, retention and recruitment. Secondary analysis of qualitative interview data with nurses (n = 50) and survey data (n = 337) was conducted as part of a workforce mapping exercise in 2019. COREQ reporting guidelines were used. After structural coding was applied, three themes emerged: stigma experienced by clients of alcohol and other drug treatment services, stigma experienced by addiction nurses and a lack of awareness of the specialty of addiction nursing itself. Participants overwhelmingly felt that these forms of stigma made addiction nursing less attractive to new entrants, particularly new nurses and posed a threat to the sustainability of the specialty. The findings from this study indicate that urgent attention is required to address stigma towards individuals who use alcohol and other drugs, and the nurses providing care for them. Furthermore, creating awareness of the addiction nursing specialty is paramount to ensure workforce sustainability and to improve care for individuals who use alcohol and other drugs. Beyond addiction nurses, our results indicate that stigma towards other specialties (such as mental health nursing) is a substantive barrier to workforce sustainability.

ABSTRACT

While numbers of men in the nursing profession have slowly increased, men in female-dominated specialty areas have not changed. Male nurses and nursing students encounter gender bias and discrimination in certain specialty nursing environments. This has implications for the quality of care provided, parental engagement, and job satisfaction. By diversifying the nursing workforce, there is a potential to enhance patient comfort, improve satisfaction, and promote a more inclusive, creative, and patient-focused health care environment.

ABSTRACT

La propuesta que se presenta es fruto, por un lado, de los resultados del proyecto “IPACAT22: Consenso sobre los aspectos clave del modelo de la práctica avanzada en Cataluña”, realizado con la participación de 24 enfermeras representantes de asociaciones científicas enfermeras o multidisciplinares de Cataluña. Por otro lado, es también fruto del apasionante, relevante y a veces difícil debate entre los miembros del equipo con diferentes trayectorias profesionales y distintos puntos de vista que enriquecen el objetivo común. Es oportuno presentar los resultados del debate, basados en el rigor, y los resultados de los proyectos de investigación por la mejora y el avance de la enfermera de práctica avanzada. Este debate también desempeña un rol importante en el entorno profesional para ejercer un liderazgo efectivo en las organizaciones y en el sistema de salud. Estamos convencidas de que este documento puede ser un punto de partida para configurar el modelo de la enfermera de práctica avanzada en nuestro contexto.(AU)


The proposal presented is the result, on one hand, of the outcomes of the Project “IPACAT22: Consensus on the key aspects of the model for advanced practice in Catalonia”, prepared with the participation of 24 nurses representing multidisciplinary or nursing scientific associations of Catalonia. On the other hand, it is also the result of the exciting, relevant, and sometimes difficult discussion among the team members with different professional careers and point of views which enhance the common objective. It is appropriate to present the outcomes of this discussion, based on rigour and results of the research projects for the improvement and progress of advanced practice nurses. This discussion also represents a major role in the professional setting, to conduct an effective leadership in organizations and the health system. We are convinced that this document can be a starting point to set up the model for the advanced practice nurse in our setting.(AU)

ABSTRACT

Background: Nurses have a rich history in performing their duty both domestically and internationally in response to a disaster. Comprising the largest proportion of the healthcare workforce, nurses possess a unique opportunity to inform disaster planning and management. With the ongoing threat from COVID-19 and continuing conflict, humanitarian aid needs, epidemics and natural disasters; the capacity of nurses to continue to respond in times of global need is unparalleled. Aims: The aim of this paper is to explore the developments in the field of disaster nursing. Mapping key changes in policy, practice and outcomes. Methods: A qualitative interpretive historical review was conducted to examine core developments in the history of disaster nursing, examining key organisations (e.g. World Health Organization, International Council of Nurses), national and international policies and historical accounts. Results: 29 articles were analysed, and politics, strategic perspectives and nursing identity ('sense of duty' and roles) emerged from the literature. The influence of professionalisation and public health/health promotion emerged next. A total of 10 articles refer to disaster nursing specifically, of which 4 of these are reports/policy. Conclusions: Nurses have spent centuries building the trust and legitimacy of the profession. Disaster nursing goes beyond the expectations of a registered nurse. The responsibilities of a disaster nurse encompass wider community health promotion, critical decision-making beyond the individual patient, resilience and ethical challenges. Whilst significant advancements have emerged in the last 30 years, further research, and representation of the profession at a strategic and political level could enhance the effectiveness of nurses' roles in the 4 phases of disaster response: mitigation, preparation, response and recovery.

OBJECTIVES: Professional identity plays an important role in the long-term development of nurses, and it will change when public health emergency occurs. The objective of this study is to investigate the factors associated with the professional identity of nursing undergraduates in the epidemic of COVID-19. METHODS: A cross-sectional survey design with convenience sampling was used. A total of 3,875 nursing undergraduates were recruited from seven universities across China from March to April 2020. A general information questionnaire was used to collect students' information, and the Professional Identity Questionnaire for Nurse Students was used to survey their professional identity during the early and later stages of the first wave of the COVID-19 epidemic. RESULTS: The score of professional identity in the later stage (59.49 ± 12.41) was higher than that in the early stage (56.96 ± 12.61). The stepwise regression indicated that several factors were associated with professional identity, including gender, residential area, major, impact of the epidemic on intention to work after graduation, reasons for choosing nursing major and students' scores of professional identity in early stage. CONCLUSIONS: Nursing educators can utilize the positive impact of responding to public health emergencies to increase the professional identity of students. Meanwhile, educators should give those students with lower professional identity more targeted education to cultivate their professional identity after the occurrence of public health emergencies.

ABSTRACT

The establishment of nursing specialties that started in 1953 has contributed decisively to the quality of the medical care provided by the National Health System. Nowadays, a large number of nurses have an official specialist qualification achieved through different means of access established by Royal Decree on nursing specialties. Furthermore, the training programmes must continue to develop in order to complete the catalogue established under the aforementioned regulation. It should be pointed out that many specialists face difficulties in carrying out professional practice according to their education and training, due to the lack of job descriptions and identification of competences during recruitment processes and access to job positions, needed for appropriate professional performance. Therefore, it is essential to create a catalogue for specific job positions for each of the specialties to meet the expectations of professional nursing and enhance progress in quality patient care.

OBJECTIVE: To analyse the distribution of advanced competences in specialist nurses and advanced practice nurses and to evaluate their association with some characteristics of their professional profile. METHOD: Multicentre analytical cross-sectional study. Nurses who worked as advanced practice nurses and specialist nurses were included. Their level of perceived advanced competences was measured, as well as sociodemographic and professional characterization variables. RESULTS: A total of 277 nurses participated (149 practised as advanced practice nurses and 128 as specialists), with an average of 13.88 (11.05) years as a specialist and 10.48 (5.32) years as an advanced practice nurse. In the sample, 28.8% had a master's or doctorate level qualification, 50.2% worked in Primary Care, 24.9% in hospitals and 22.7% in Mental Health. The self-perceived global level was high in the different competences, the lowest dimensions being research, evidence-based practice, quality and safety management and leadership and consulting. The advanced practice nurses obtained a higher level of competence globally and in the dimensions of leadership and consulting, interprofessional relations, care management, and health promotion. There were no differences based on experience or possession of a master's degree or doctorate. In the advanced practice nurses, the practice context did not influence competence levels, although in the specialist nurses it did, in favour of those practicing in Mental Health. CONCLUSIONS: Specialist and advanced practice nurses have different competences that should be adequately managed for the development of advanced and specialist nursing services.

Resumo Objetivo Analisar o conceito de Prática Avançada de Enfermagem e elucidar os elementos-chave: atributos, antecedentes e consequências. Métodos O referencial metodológico para o estudo foi o modelo de análise conceitual de Walker e Avant (2011), estruturado mediante uma revisão integrativa da literatura conduzida nos meses de maio e junho de 2017. A busca foi realizada nas bases de dados Scopus, PubMed, CINAHL, Web of Science, Science Direct, Cochrane e Lilacs, considerando estudos que trataram do termo e que foram publicados em português, inglês ou espanhol. Resultados A amostra foi composta por 33 estudos conduzidos em oito países entre os anos 2000 e 2016, sendo 56% publicado no quinquênio 2011-2016. Foram identificados oito atributos: Preparação educacional em nível de mestrado ou doutorado, e especialização em área clínica; Prática baseada em evidências; Habilidade de desenvolver raciocínio clínico e pensamento crítico; Alto nível de autonomia; Avaliação avançada e ampla; Liderança; Capacidades diagnóstica, gerencial e administrativa; Promoção do ensino às outras enfermeiras. Foram identificados os antecedentes e consequências e foi desenvolvida uma definição do conceito. Conclusão Diante da expansão da ideia de enfermagem avançada pelo mundo, particularmente na América Latina, recomendam-se outros estudos relacionados à Prática Avançada de Enfermagem. É necessária uma compreensão aprofundada das questões na prática de implementação no Brasil para ajudar a determinar o conceito futuro de PAE para o país. Este estudo contribui a essa compreensão ao definir o conceito de PAE com seus antecedentes, atributos e consequências.

Se efectuaron búsquedas en las bases Scopus, PubMed, CINAHL, Web of Science, Science Direct, Cochrane y Lilacs, buscando estudios que abordaran el término, publicados en portugués, inglés o español. Resultados La muestra consistió en 33 estudios realizados en ocho países y datados entre 2000 y 2016, con un 56% publicado en el quinquenio 2011-2016. Se identificaron ocho atributos: Preparación educativa a nivel de maestría o doctorado y especialización en el área clínica; Práctica basada en la evidencia; Capacidad para aplicar juicio y pensamiento crítico; Alto nivel de autonomía; Evaluación avanzada e integral; Liderazgo; Capacidad de diagnóstico, gestión y administración; y Enseñanza a otras enfermeras. Se identificaron antecedentes y consecuencias, y se construyó una definición del concepto. Conclusión Considerando que la noción de enfermería avanzada está expandiéndose a nivel mundial, especialmente en América Latina, se recomiendan nuevos estudios relacionados a la Práctica Avanzada de Enfermería. Es necesario comprender a fondo las cuestiones relativas a la práctica de su aplicación en Brasil para permitir determinar el concepto de la futura red de protección de la PAE en el país. Este estudio contribuye a esa comprensión al definir el concepto de PAE, incluyendo sus antecedentes, atributos y consecuencias.


Abstract Objective Was to analyze the concept of advanced practice nursing and elucidate the essential elements: attributes, antecedents and consequences. Methods The methodological reference for the study was the Walker and Avant (2011) concept analysis model, structured by an integrative review of the literature conducted in May and June, 2017. The Scopus, PubMed, CINAHL, Web of Science, Science Direct, Cochrane and Lilacs databases were searched for studies that addressed the term and that were published in Portuguese, English or Spanish. Results The sample consisted of 33 studies conducted in eight countries and dated from 2000 to 2016, with 56% published in the five-year period of 2011-2016. Eight attributes were identified: Educational preparation at the master's or doctoral level, and specialization in clinical area; Evidence-based practice; Ability to exercise judgement and critical thinking; High level of autonomy; Advanced and comprehensive assessment; Leadership; Capacity for diagnosis, management and administration; and Teaching other nurses. The antecedents and consequences were identified, and a definition of the concept was constructed. Conclusion Considering that the idea of advanced nursing is expanding worldwide, especially in Latin America, other studies related to Advanced Nursing Practice are recommended. An in-depth understanding of the implementation practice issues in Brazil is necessary, to aide in determining the forthcoming APN concept for the country. This study contributes to that understanding by defining the concept of APN, including its antecedents, attributes, and consequences.

Introdução: O enfermeiro de prática avançada é o profissional que desenvolveu uma base de conhecimento especializado, habilidades complexas para tomada de decisão, competências clínicas com alto nível de autonomia e integra em sua atuação profissional a pesquisa, a educação, a prática assistencial direta e a gestão. A Atenção Primária à Saúde (APS) brasileira é um cenário com grande potencial para desenvolvimento de práticas avançadas de enfermagem. Objetivo: Construir um perfil de competências para a prática avançada de enfermagem no contexto da Atenção Primária à Saúde brasileira. Método: estudo exploratório e de abordagem qualitativa, que no referencial teórico-metodológico adotou as seis etapas propostas por Zarifian para construção de competências. Foram os sujeitos da pesquisa: egressos do mestrado profissional na Atenção Primária no Sistema Único de Saúde e da residência uniprofissional em Atenção Básica na Saúde da Família, ambos da Escola de Enfermagem da Universidade de São Paulo. Realizou-se uma oficina de trabalho para elencar ações com potencial para prática avançada de enfermagem, a partir de um mapa com ações desempenhadas pelos enfermeiros na APS. O material empírico resultante foi submetido à análise de discurso para posterior construção das competências. Foram seguidos todos os preceitos éticos. Resultados: Participaram do estudo oito sujeitos, sendo dois egressos do mestrado profissional e seis da residência profissional. Os sujeitos reconhecem que a prática avançada de enfermagem amplia as ações e prática clínica dos enfermeiros e que a residência profissional é um modelo potente para formação de práticas avançadas de enfermagem. Foram identificadas 29 ações com potencial para a prática avançada do enfermeiro na APS. As competências construídas com base nessas ações foram: prática clinica ampliada, integralidade do cuidado, autonomia para tomada de decisões complexas, ser referência para a formação continua de profissionais de enfermagem e no desenvolvimento de pesquisa. Conclusão: elaborou-se um perfil de competências para a formação enfermeiros de práticas avançadas para a APS. Estudos posteriores serão necessários para ampliar e aprofundar essas competências, com o envolvimento de outros sujeitos, como gestores, apoiadores técnicos e profissionais da Atenção Primária.


Introduction: Advanced practice nurses are professionals who acquired specialized know-how, complex skills for decision making, and clinical competences with a high level of autonomy. These nurses also incorporate research, education, care practice and management to their professional performance. Brazilian primary health care is a setting with great potential for the development of advanced practice nursing. Objective: To build a profile of competences for advanced practice nursing in the Brazilian primary health care context. Method: This is an exploratory study, using a qualitative approach, whose theoretical-methodological framework adopted the six stages proposed by Zarifian for the construction of competences. The study subjects were graduates of the professional master program in Primary Health Care at the Unified Health System, and the uniprofessional residency program in Primary Health Care at the Family Health Strategy, both in the School of Nursing, at the University of São Paulo. A workshop was conducted to choose actions with a potential for developing advanced practice nursing, based on a map with actions developed by primary health care nurses. The resulting empirical material was submitted to discourse analysis for later construction of the competences. All ethical principles were observed. Results: Eight subjects participated in the study, of whom two had graduated in the professional master program and six in the professional residency program. The subjects recognized that advanced practice nursing improves the actions and clinical practice of nurses and that the professional residency program is a powerful model for teaching advanced practice nursing. A total of 29 actions with a potential for developing advanced practice nursing in primary health care were identified. The following competences were built based on these actions: increased clinical practice, care comprehensiveness, autonomy for making complex decisions, being a reference for the permanent education of nursing professionals and in the development of research. Conclusion: The authors created a profile of competences for the education of advanced practice nursing in primary health care. Further studies are needed to broaden these competences, with the participation of other subjects such as managers, technical supporters and primary health care professionals.

ABSTRACT Objective: Reflect on the role of advanced practice nursing (APN) in the context of Brazilian primary health care. Method: analysis of the main scientific productions and of the discussion on the implementation of APN. Results: there are favorable areas for the work of APN in primary health care, especially in the family health strategy. Professional master's degree and nurse residency programs constitute powerful training spaces. As a challenge, standardized definition of the roles of advanced practice nurses and of minimum training for work are necessary. Final considerations: researches that contribute to understanding the perspectives, facilitators, and barriers for APN are necessary, as well as determining the population's health needs and expectations concerning the role of these professionals.


RESUMEN Objetivo: Reflejar sobre el papel de la enfermería con prácticas avanzadas (EPA) en el contexto de la atención primaria de salud brasileña. Método: Análisis de las principales producciones científicas y de la discusión sobre la implantación de la EPA. Resultados: Hay campos de actuación favorables para la EPA en la atención primaria de salud, en especial en la estrategia salud de la familia. La existencia de maestría profesional y programas de residencia en enfermería se configuran como potentes espacios de formación. Como reto es necesaria la definición estandarizada de los papeles de la enfermería con prácticas avanzadas y de formación mínima para actuación. Consideraciones finales: Son necesarias las investigaciones que colaboran para la comprensión de perspectivas, facilitadores y barreras de la EPA, así como la identificación de necesidades de salud de la población y de las expectativas del papel de esos profesionales.

Método: análise das principais produções científicas e da discussão sobre a implantação da EPA. A existência de mestrado profissional e programas de residência em enfermagem se configuram como potentes espaços de formação. ABSTRACT Qualitative descriptive and exploratory research, performed with 20 nursing professionals working in a general radiology unit and surgery center with the "C-arm" equipment, and the hemodynamics unit of a southern Brazilian teaching hospital. Aim was to analyze the dimensions of the work process for radiological technology nurse professionals. This paper made clear that radiological technologies are outside nursing, which leads to the invisibility of ionizing radiation, leading to inappropriate attitudes about radiological protection, which can be related to harm in professionals, caused by lack of knowledge about this issue. Among radiological nursing work process dimensions, the care dimension emerged. The necessity of implementing a continuous educational program in services which utilize radiological technologies was emphasized, thereby promoting, for patients and for nursing professionals, safer environments and integral care.


RESUMEN Investigación cualitativa, descriptiva y exploratoria realizada con 20 profesionales de enfermería que actúan en los sectores de radiología general, centro quirúrgico, con el equipamiento de arco en "C" y el sector de hemodinamia de un hospital-escuela al sur de Brasil. El objetivo fue analizar las dimensiones del proceso de trabajo de los profesionales de enfermería que actúan con las tecnologías radiológicas. Ese estudio evidenció que las tecnologías radiológicas son ajenas a la enfermería, lo que induce a invisibilidad de la radiación ionizante, implicando en prácticas incorrectas de protección que puede relacionarse con desgastes en los trabajadores provocados por poco conocimiento acerca del asunto. Dentro de las dimensiones del proceso de trabajo en enfermería radiológica, la dimensión cuidado sobresalió. Apúntase la necesidad de implementación de un programa de educación permanente en los servicios que utilizan las tecnologías radiológicas para que proporcionen, para los pacientes y los profesionales de enfermería, ambientes más seguros y una asistencia integral.


RESUMO Pesquisa qualitativa, descritiva e exploratória realizada com 20 profissionais de enfermagem que atuam nos setores de radiologia geral, centro cirúrgico com utilização de equipamento de arco em "C" e hemodinâmica de um hospital-escola do Sul do Brasil. Objetivo de analisar as dimensões do processo de trabalho dos profissionais de enfermagem que atuam com as tecnologias radiológicas. O estudo evidenciou que as tecnologias radiológicas são externas à enfermagem, induzindo à invisibilidade da radiação ionizante, o que implica em práticas incorretas de proteção, que podem se relacionar com desgastes nos trabalhadores, provocados pelo pouco conhecimento sobre o assunto. Dentre as dimensões do processo de trabalho na enfermagem radiológica, a dimensão cuidado sobressaiu-se. Aponta-se a necessidade de implantação de um programa de educação permanente nos serviços que utilizam as tecnologias radiológicas para que se proporcionem, aos pacientes e aos trabalhadores de enfermagem, ambientes mais seguros e uma assistência integral.

ABSTRACT

Identificar a produção da enfermagem de Centro Cirúrgico em português. Método: Revisão integrativa da literatura dos estudos primários indexados na base de dados Lilacs, no idioma português, utilizando o descritor ?enfermagem em Centro Cirúrgico?, publicados entre 2003 a2013, utilizando um instrumento disponível na literatura, avaliação crítica dos estudos primários incluídos, análise e síntese descritiva dos resultados da revisão. Resultados: A amostra foi constituída de 47 artigos, divididos nas categorias: assistência e segurança ao paciente (n=14), educação (n=10), gestão e administração (n=10), saúde do trabalhador (n=5), campos de atuação para o enfermeiro (n=4), comunicação e ética (n=3) e assistência aos familiares (n=1). ABSTRACT

The aim of this study is to describe the factors hindering and facilitating the implementation of the advanced practice registered nurses role at Finnish university hospitals, and to examine the implications for its future development. A descriptive qualitative approach, using thematic individual interviews, was conducted in 2011 with a sample of 11 advanced practice registered nurses. The data were analysed using qualitative content analysis. The advanced practice registered nurses role barriers had an impact on the role development needs. In turn, the facilitating factors helped encounter the challenges of the role, therefore having an impact on both the current role achievement, as well as contributing to the future role development. The factors hindering and facilitating the advanced practice registered nurses role need to be acknowledged to support the role implementation and planning of the future of the role.

ABSTRACT

Attracting and retaining nurses in HIV care is essential to treatment success, preventing the spread of HIV, slowing its progression, and improving the quality of life of people living with HIV. Despite the wealth of studies examining HIV care, few have focused on the factors that influenced nurses' choices to specialize in HIV care. We examined the factors that attracted and retained eight nurses currently working in HIV care in two large Canadian cities. Participants were primarily women between the ages of 20 and 60 years. Interviews were conducted between November 2010 and September 2011 using interpretive description, a qualitative design. Factors that influenced participants to focus their careers in HIV care included both attracting factors and retaining factors. Although more research is needed, this exploration of attracting and retaining factors may motivate others to specialize in HIV nursing, and thus help to promote adequate support for individuals suffering from the disease.

ABSTRACT

Since nursing became an university degree in 1977, there have been several regulations to develop specialties, all of them agreeing on the need to include skills in research. Indeed, the relevance of acquiring these skills in all current disciplines has led to Royal Decree 99/2011, which regulates the official PhD courses, and recognises specialist nurses as qualified to access PhD studies. Nowadays, students from six of the seven specialties included in the Royal Decree 450/2005 on nursing specialties, are performing their training. The acquisition of research skills is seen as an opportunity and a challenge. However, the organizational structure of training facilities (multiprofessional teaching units) and the incorporation of nurses as clinical tutors, who initiated this teaching activity, deserve special attention to ensure the correct acquisition of research skills in the training of specialist nurses.
