ABSTRACT

Objetivo: Identificar na literatura os principais fatores contribuintes para ocorrência de Eventos Adversos na Atenção Primária à Saúde. Métodos: Trata-se de uma revisão integrativa. A partir da formação da questão norteadora, foi realizada uma pesquisa nas bases de dados: MEDLINE, PubMed, BDENF, SciELO e LILACS. Ao final, foram incluídos 15 artigos. Resultados: 7 dos artigos traziam fatores ligados a medicação como principal, 3 traziam prática e procedimentos, 2 organização e gestão, 1 comunicação e 2 outros fatores. Conclusão: Percebeu-se que erros relacionados a medicação e diagnóstico são os mais comuns nas instâncias de saúde. A falha na comunicação entre funcionários e entre os serviços de saúde, além de fatores organizacionais e diagnósticos tardios também foram apontadas, sendo boa parte, evitável. A subnotificação foi comumente relatada nos artigos, revelando que os dados existentes possam não estar condizentes com a realidade. (AU)


Objective: To identify in the literature the main contributing factors for the occurrence of Adverse Events in Primary Health Care. Methods: This is an integrative review. Based on the formation of the guiding question, a research was carried out in the databases: MEDLINE, PubMed, BDENF, SciELO and LILACS. At the end, 15 articles were included. Results: 7 of the articles included factors related to medication as main, 3 had practice and procedures, 2 organization and management, 1 communication and 2 other factors. Conclusion: Errors related to medication and diagnosis are the most common in health instances. The failure to communicate between employees and between health services, in addition to organizational factors and late diagnoses were also pointed out, being mostly avoidable. Underreporting was commonly reported in the articles, revealing that existing data may not be consistent with reality. (AU)


Objetivo: Identificar en la literatura los principales factores contribuyentes para la ocurrencia de Eventos Adversos en la Atención Primaria a la Salud. Métodos: Se trata de una revisión integrativa. A partir de la formación de la cuestión orientadora, se realizó una investigación en las bases de datos: MEDLINE, PubMed, BDENF, SciELO y LILACS. Al final, se incluyeron 15 artículos. Resultados: 7 de los artículos traían factores ligados à la medicación como principal, 3 traían práctica y procedimientos, 2 orgnización y gestión, 1 comunicación y 2 otros factores. Conclusión: Se dio cuenta que errores relacionados con la medicación y el diagnóstico son los más comunes en las instancias de salud. La falla en la comunicación entre funcionarios y entre los servicios de salud, además de factores organizacionales y diagnósticos tardío también fueron señalados, siendo buena parte, evitable. La subnotificación fue comúnmente relatada en los artículos, revelando que los datos existentes pueden no estar en concordancia con la realidad. (AU)

Os impactos dos danos associados aos cuidados em saúde sobre os sistemas de saúde enunciam que a conscientização acerca da sua gravidade é urgente. Os Cadernos Ibero-Americanos de Direito Sanitário, na vanguarda em nosso país, dedicam um número para aprofundar as reflexões sobre os aspectos jurídico-sanitários da Segurança do Paciente, reconhecendo que é um dos temas mais importantes da área da Saúde e do Direito.


The impact of health care-related harms on health care systems demonstrates the urgent need for awareness of their severity. The Iberoamerican Journal of Health Law, pioneer in Brazil, dedicates an issue to deepen the reflection on the legal-sanitary aspects of patient safety, recognizing that it is one of the essential issues in the field of health and law.


Los impactos de los daños asociados con la atención de la saluden los sistemas de salud indican que es urgente tomar conciencia de su gravedad. Los Cuadernos Iberoamericanos de Derecho Sanitario, a la vanguardia en Brasil, dedican un número a profundizar en las reflexiones sobre los aspectos jurídico-sanitarios de laseguridad del paciente, reconociendo que es uno de los temas más importantes en el ámbito de la salud y el derecho.

ABSTRACT

Introducción: El evento adverso en salud ha generado múltiples estrategias de prevención. En el campo de rehabilitación, específicamente en fisioterapia, su reporte, análisis y efectos se han descrito en múltiples artículos. Objetivo: determinar los efectos de los eventos adversos reportados en consulta de fisioterapia, sus factores de aparición, causas y determinantes aportados en salud para generar mejores estrategias desde su enfoque en prevención y seguridad del paciente. Métodos: Se realizó una búsqueda sistemática de información de artículos científicos en seis bases de datos: Pubmed, Scopus, Proquest, pedro, Science Direct y Scholar, con las palabras clave efecto adverso manejo del riesgo y fisioterapia seleccionándose finalmente 44 de 1 145 artículos iniciales para la realización de este estudio. Desarrollo: Se clasificaron los artículos en tres categorías: factores intrínsecos (14 artículos, 34,1 por ciento), factores extrínsecos (24 artículos, 58,6 por ciento) y sistemas de salud (3 artículos, 7,3 por ciento), en los que se encontraron reportes de eventos adversos de fisioterapia en unidad de cuidado intensivo, terapia manual y tele rehabilitación. Conclusiones: Desde la perspectiva de la fisioterapia se debe tener más participación en estrategias propias y multidisciplinarias que promuevan la seguridad de los pacientes atendidos en rehabilitación, además de continuar con la presentación de estudios que exploren otros campos de acción de la fisioterapia y la incidencia de eventos adversos en estos(AU)


Introduction: The occurrence of adverse events in health care has prompted the development of many prevention strategies. In the field of rehabilitation, particularly in physiotherapy, their report, analysis and effects have been described in a large number of articles. Objective: Determine the effects of the adverse events occurring in physiotherapy sessions, their order of appearance, causes and health determinants contributed to develop better prevention and patient safety strategies. Methods: A systematic search for scientific articles was conducted in six databases: PubMed, Scopus, ProQuest, Pedro, ScienceDirect and Google Scholar, using the key terms adverse effect, risk management and physiotherapy. A total 1 145 articles were retrieved, from which 44 were selected for the study. Conclusions: More active participation should be achieved in physiotherapy-specific and multidisciplinary strategies fostering the safety of patients attending rehabilitation services, as well as in the continuing conduct of studies exploring other areas of physiotherapy and the incidence of adverse events therein(AU)

ABSTRACT

Litigation related to medical liability has a great impact on Italian healthcare expenditure. Recently, many Italian Regions have adopted a "self-insurance system" and, in Sicilian Hospitals, were established the Claims Management Committees (CMC) to provide the direct management of claims. Here the experience of a Sicilian University Hospital CMC was described to analyze the claims features and their outcomes providing evidence on CMC usefulness. The analysis involved claims for compensation received during 4 years, using data obtained by a retrospective analysis of claims database created by Forensic Medicine Service. Claims data, obtained from the insurance broker, were used to perform the statistical comparison. During the examined period a total of 377 claims were received by CMC, respectively 63.6% for professional liability and 36.4% for other causes (damages not related to medical malpractice). The prevalence of complaints about malpractice regarded surgery. The CMC had expressed an opinion on 120 claims related to malpractice with the percentages of admission or rejection of liability respectively of 55% and 45%. The statistical analysis revealed a greater number of lawsuits in the Insurance system and, moreover, the CMC higher probability to reach the amicable settlement of litigations. CMC provides specific data on claims trend and economic expenditure, demonstrating its usefulness for analysis and monitoring the causes of patients/people damage. It is a tool for medical malpractice risk assessment and prevention. It can encourage the amicable settlement and prevention of civil action. It seems to be an efficient system to reduce the health liability costs.
