ABSTRACT

Los problemas de salud mental durante estos periodos, representan una de las situaciones más complejas a las que puede enfrentarse el personal de salud. Este deberá evaluar los diagnósticos concomitantes, abordajes, posibles complicaciones del trastorno psiquiátrico en la madre y el riesgo en el desarrollo fetal, además posibles complicaciones asociadas al uso de psicofármacos durante el embarazo y la lactancia, así como sus efectos en el establecimiento del apego inicial con la hija e hijo. Resulta importante que el personal de salud tenga siempre presente que cualquier mujer independientemente de su edad, nivel socioeconómico, cultural y étnico, puede desarrollar un trastorno mental durante el periodo perinatal. La salud mental de las madres es indispensable para el desarrollo, crecimiento y estabilidad emocional de hijas e hijos, muchas mujeres sufren y experimentan emociones por cambios inevitables en el embarazo, parto y puerperio. En los presentes lineamientos se entenderá por salud mental perinatal el estado de la salud mental de la mujer antes, durante y después del embarazo, así como, la salud mental del recién nacido y el vínculo que se establece entre madre e hijo, por tanto, debe darse seguimiento y vigilancia hasta los dos años posteriores al parto

ABSTRACT

Patient navigation (PN) aims to improve timely access to healthcare by helping patients to "navigate" complex service provision landscapes. PN models have been applied in diverse healthcare settings including perinatal mental health (PMH). However, the practice models and implementation of PN programs vary widely, and their impact on engagement with PMH services has not been systematically investigated. This systematic narrative review study aimed to (1) identify and describe existing PMH PN models, (2) understand their effectiveness in improving service engagement and clinical outcomes, (3) review patient and provider perceptions, and (4) explore facilitators and barriers to program success. A systematic search of published articles/reports describing PMH PN programs/service delivery models targeting parents in the period from conception to 5 years postpartum was conducted. In total, 19 articles describing 13 programs were identified. The analysis yielded a number of commonalities and differences across program settings, target populations, and the scope of the navigator role. While there was promising evidence to support the clinical efficacy and impact on service utilization of PN programs for PMH, the current evidence base is sparse. Further research evaluating the efficacy of such services, and facilitators and barriers to their success, is warranted.


La meta de Navegación del Paciente (PN) es mejorar el acceso a tiempo a servicios de cuidado de salud por medio de ayudar a los pacientes a "navegar" los complejos esquemas de provisión de servicios. Los modelos PN han sido aplicados en diversos escenarios de cuidados de salud incluyendo la salud mental perinatal (PMH). Sin embargo, los modelos de la práctica e implementación de programas PN varían ampliamente, y su impacto en la participación de los servicios PMH no ha sido sistemáticamente investigada. Este estudio de revisión narrativa sistemática se propuso 1) identificar y describir modelos PMH PN existentes, 2) comprender su eficacia para mejorar la participación en el servicio y resultados clínicos, 3) examinar las percepciones de pacientes y proveedores, y 4) explorar factores facilitadores y barreras al éxito del programa. Se llevó a cabo una sistemática investigación de artículos/reportes publicados que describen modelos que proveen programas/servicios de PMH PN con enfoque en los padres en el período desde la concepción hasta los 5 años posteriores al parto. En total, se identificaron 19 artículos que describían 13 programas. Los análisis dieron como resultado un número de puntos comunes y diferencias a través de la composición de los programas, la población a la cual se dirigían, y el ámbito del papel del navegador. A pesar de que se observó una evidencia prometedora para apoyar la efectividad clínica y el impacto sobre la utilización del servicio de programas PN para PMH, la base actual de la evidencia es escasa. Es necesaria una posterior investigación para evaluar la efectividad de tales servicios, y puntos que los faciliten o barreras al éxito de éstos.


La Navigation du Patient (abrégé ici NP en français) a pour but d'améliorer l'accès rapide aux soins de santé en aidant les patients à « naviguer ¼ un paysage complexe d'offre de services. Les modèles NP ont été appliqués dans divers contextes de soins de santé y compris la santé mentale périnatale (SMP en français ici). Cependant les modèles de pratique et de mises en place de programmes NP varient grandement, et leur impact sur l'engagement avec des services SMP n'a pas encore été examiné systématiquement. Cette étude narrative systématique s'est donnée pour but de 1) identifier et décrire les modèles NP existants, 2) comprendre leur efficacité à améliorer d'engagement du service et ses résultats cliniques, 3) passer en revue les perceptions du patient et du prestataire, et 4) explorer ce qui facilite et fait obstacle au succès du programme. Une recherche systématique d'articles/rapports publiés décrivant des modèles de prestation de NP SMP visant des parents dans la période de la conception à 5 ans postpartum a été faite. En tout 19 articles décrivant 13 programmes ont été identifiés. L'analyse a produit un nombre de points communs et de différences au travers des contextes des programmes, des populations ciblées et de la portée du rôle de navigateur. Bien qu'il y ait des preuves promettantes soutenant l'efficacité clinique et l'impact de l'utilisation de services des programmes NP pour la SMP la base de preuves actuelle est éparse. Des recherches supplémentaires évaluant l'efficacité de tels services ainsi que les facteurs de facilitation et les barrières au succès sont nécessaires.

ABSTRACT

INTRODUCTION: This study examined sleep disturbances in first-time fathers from the third trimester of their partner's pregnancy to 2 months postpartum to determine prevalence, incidence, and persistence of sleep disturbances and identify associated determinants. METHODS: Men expecting their first child were recruited from local prenatal classes and university-affiliated obstetric clinics. During their partner's third trimester of pregnancy and 2 months postpartum, 459 men completed standardized online self-report questionnaires measuring sociodemographics, lifestyle, and psychosocial variables and sleep quality. RESULTS: Disturbed sleep (Pittsburgh Sleep Quality Index [PSQI] global score >5) increased from 29.6% during the third trimester to 44.7% at 2 months postpartum. The incidence of poor sleep at 2 months postpartum was 33.7%. Among men with disturbed sleep at the antenatal assessment, 70.6% continued to have sleep disturbances at 2 months postpartum. An increase in depressive symptoms and higher parenting stress was independently associated with onset and persistence of disturbed sleep at 2 months postpartum. CONCLUSIONS: Sleep is compromised in expectant and new fathers. Strategies aimed at improving sleep, depressed mood, and managing the challenges of parenting may be important components to include in prenatal interventions aimed at enhancing the transition to parenthood and infant development.

ABSTRACT

Introdução. O estresse dos pais devido à internação de um filho em Unidade de Terapia Intensiva Neonatal (UTIN) pode ter consequências físicas e emocionais importantes nos cuidadores. Objetivos. Analisar as relações entre o estresse dos pais e seus níveis de satisfação com o atendimento. Método. A amostra foi composta por 32 pais que tiveram seus filhos internados em UTIN. Os cuidadores preencheram um questionário sociodemográfico além dos instrumentos PSS: NICU e EMPATHIC-N. Os dados foram analisados a partir de estatística descritiva e inferencial. Resultados. Observou-se um estresse moderado a partir do instrumento PSS:NICU, especialmente nos domínios "Alteração no papel de pai/mãe" e "Número total de experiências" (p < 0,05). Também se detectou correlação positiva entre a idade dos pais e o nível geral de estresse (r = 0,34; p < 0,05). A satisfação dos pais apresentou níveis elevados em todos os domínios. Conclusão. Os dados indicaram que quanto maior a satisfação na avaliação das equipes, menor o estresse dos pais no ambiente da UTIN.


Introduction. Parents' stress due to the hospitalization of a child in a Neonatal Intensive Care Unit (NICU) can have important physical and emotional consequences on caregivers. Goals. To analyze the relationship between parents' stress and their levels of satisfaction with care. Method. The sample consisted of 32 parents who had their children hospitalized in NICUs. Caregivers completed a sociodemographic questionnaire in addition to the PSS instruments: NICU and EMPATHIC-N. Data were analyzed from descriptive and inferential statistics. Results. Moderate stress was observed from the PSS: NICU instrument, especially in the areas "Change in the role of father and mother" and "Total number of experiences" (p < 0.05). A positive correlation was also found between the parents' age and the general level of stress (r = 0.34, p < 0.05). Parents' satisfaction was high in all areas. Conclusion. The data indicated that the higher the satisfaction in the evaluation of the teams, the lower the stress of the parents in the NICU environment.

La identificación, investigación y manejo de la hipoglicemia en la infancia y el periodo perinatal continúan siendo una emergencia clínica debido a que la falla en la identificación oportuna puede conducir a un daño neurológico e incluso la muerte. Los avances en biología molecular y las técnicas diagnósticas han ayudado a comprender los mecanismos implicados en la homeostasis del metabolismo de la glucosa y, de esta manera, tener una mejor aproximación clínica del paciente pediátrico con hipoglicemia.En este número de la revista MEDICINA & LABORATORIO se presenta una revisión sobre el enfoque diagnóstico de la hipoglicemia en pediatría como paso inicial de la educación médica continua para actualizar al lector sobre los avances en la identificación y caracterización adecuada de las causas subyacentes.No obstante, establecer las causas de hipoglicemia es el punto de inicio necesario para definir y diseñar un plan de manejo apropiado que prevenga las complicaciones secundarias al daño neuronal por el bajo aporte de glucosa.Es importante tener presente que la evaluación y el manejo de la hipoglicemia en pediatría presenta particularidades propias. En niños, la hipoglicemia persistente casi siempre se debe a defectos congénitos o adquiridos en la regulación de la secreción de insulina, deficiencia del cortisol o la hormona de crecimiento o defectos en el metabolismo de la glucosa, el glucógeno y los ácidos grasos.

A partir do atendimento de um caso de tentativa de infanticídio, buscou-se discutir algumas das condições para a construção da função materna. Entre as possíveis formas de se abordar um caso em pesquisa, optou-se pela construção de caso, privilegiando um olhar que parte da clínica, na qual entendemos fundar-se a descoberta psicanalítica em sua vocação de retroalimentar a teoria. Foram privilegiados três eixos: a experiência corporal, o lugar do sujeito e o laço social. Para tal, trabalharam-se estes aspectos: o percurso histórico antecedente ao que se entende hoje por maternidade, incluindo-se a psicanálise no seu início, os diferentes discursos sobre o corpo que desembocam no discurso médico, conforme proposto por Clavreul, e as questões do laço social na constituição da função materna, a partir do conceito de contrato narcisista de Piera Aulagnier. Dessa forma, pôde-se apontar como a função materna é atravessada pela lógica dessubjetivante da contemporaneidade e, ao mesmo tempo, reafirma-se a escuta do sujeito como condição para a superação dos impasses da atualidade.(AU)


From the discussion of a case of attempted infanticide, we tried to discuss some of the conditions for the construction of the maternal role. Among the possible ways to approach a case in research, we chose to build a case, favoring a look that starts from the clinic, in which we understand is the base of the psychoanalytic discovery in its vocation to provide feedback for theory. Three main aspects were privileged: a bodily experience, the place of the subject and the social bond. To this end, these aspects have been worked up: the historical background antecedent to what is meant by motherhood today, including psychoanalysis in its early stages, the different discourses on the body leading into the medical discourse as proposed by Clavreul, and issues of the social bond in the constitution of the maternal role, from the concept of narcissistic contract of Piera Aulagnier. Thus, one could point out how the maternal role is crossed by the deconstructive logic of the subjective of contemporaneity and at the same time reaffirming the listening to the subject as a condition for overcoming the impasses of the present.(AU)

ABSTRACT

Rectal temperature, heart and respiratory rates, and the course of some blood parameters were monitored in calves during perinatal and neonatal periods. The study was carried out on eight Limousine calves. From all subjects, rectal temperature, heart rate, and respiratory rate were measured. Blood samples were taken at the same hour (9am) from the external jugular vein, and then centrifuged and stored until analyses. By means of an UV spectrophotometer, the following blood parameters were assessed for each subject: total protein, total cholesterol, triglycerides, glucose, non-esterified fatty acids (NEFA), urea, creatinine, cholinesterase, lactate dehydrogenase (LDH), hydroxybutirate, glutamate pyruvate transaminases (GPT), glutamate oxalacetate transaminases (GOT), direct and total bilirubin, magnesium, iron, potassium, phosphorus, chloride and sodium. The results showed a significant effect of days of life (P<0.05) only on total cholesterol, creatinine and GOT during the first week of life and a significant effect of days of life on rectal temperature, heart rate, respiratory rate, total cholesterol, NEFA, creatinina, and GOT during the first month of life. A correlation among individual values for postnatal age (days of life) and heart rate in calves during the first month of life was observed. In conclusion, modifications of studied parameters could be attributed to functional development of calves in neonatal period and contribute to the knowledge of adaptation processes in calf during the first week and the first month of life resulting useful for the diagnosis and treatment of any neonatal diseases.


Realizou-se o monitoramento da temperatura retal das frequências cardíaca e respiratória e de alguns parâmetros sanguíneos em oito bezerros da raça Limousine, durante o período perinatal e neonatal. Paralelamente às mensurações da temperatura retal e das frequências cardíaca e respiratória (9h), foram pesquisados, por meio de espectrofotômetro de UV, proteína total, colesterol total, triglicérides, glicose, ácidos graxos não esterificados (NEFA), ureia, creatinina, colinesterose, lactato desidrogenase (LDH), hidroxibutirato, glutamato piruvato transaminases (GOT), direta e total bilirrubina, magnésio, fósforo, cloretos e sódio. Os resultados mostraram efeito significativo dos dias de vida (P<0,05) somente sobre o colesterol total, creatinina e GOT, durante a primeira semana e um significante efeito de dias de vida sobre a temperatura retal, as frequências cardíaca e respiratória, colesterol total, NEFA, creatina e GOT durante o primeiro mês de vida. Observou-se correlação entre os valores individuais para dias de vida e frequência cardíaca durante o primeiro mês de vida. Conclui-se que as modificações desses parâmetros podem ser atribuídas ao desenvolvimento dos bezerros no período neonatal, contribuindo para o conhecimento do processo de adaptação, em bezerros, durante a primeira semana e o primeiro mês de vida.

ABSTRACT

A síndrome megabexiga microcólon hipoperistaltismo intestinal (SMMHI) é uma doença rara, de provável herança genética, com prognóstico reservado, já no primeiro ano de vida. Relata-se um caso de SMMHI em criança do sexo feminino, atualmente com 16 anos de idade, com o objetivo de ressaltar o prognóstico favorável, não esperado, com sobrevida até a adolescência, de uma síndrome rara. Chama-se a atenção para a possibilidade de estabelecer o diagnóstico no período perinatal e para a importância de realizar o estudo urodinâmico.
