ABSTRACT

Describir la gestión de la calidad de cuidados en enfermería y la seguridad del paciente. Metodología: La investigación se desarrolla desde el enfoque cuantitativo con una metodología descriptiva con diseño no experimental, la cual se apoya en el análisis documental y bibliográfico. Se organizó un proceso investigativo en donde la población de estudio, se basó primordialmente en documentos escritos como tesis, artículos científicos. Lo analítico­sintético, radicó en descomponer mentalmente el tema estudiado en diversos elementos para poder conseguir nuevos conocimientos. En conclusión: La enfermería es el profesional que está en el centro de los cuidados directo de las personas, familias y comunidad, prestando la colaboración al equipo de salud, desde una acción dinámica y responsable donde resaltan los valores humanos y brindar bienestar a la población con calidad y constancia. Desde los pacientes leves hasta los más críticos cumpliendo con los protocolos de cuidados para su pronta recuperación.


To describe the management of quality of care in nursing and patient safety. Methodology: The research is developed from the quantitative approach with a descriptive methodology with non-experimental design, which is supported by documentary and bibliographic analysis. A research process was organized in which the study population was based primarily on written documents such as theses and scientific articles. The analytical-synthetic aspect was based on mentally decomposing the subject studied into different elements in order to obtain new knowledge. In conclusion: Nursing is the professional who is at the center of the direct care of people, families and community, providing collaboration to the health team, from a dynamic and responsible action where human values stand out and provide welfare to the population with quality and constancy. From the mildest to the most critical patients, complying with the care protocols for their prompt recovery.

ABSTRACT

Objetivo: Compreender a percepção, atitudes e práticas de trabalhadores de um hospital universitário frente a resíduos sólidos de saúde (RSS). Métodos: Realizou-se, por meio de metodologia descritiva e qualitativa, entrevista com profissionais de saúde em 2019 em um hospital universitário do município de Bagé, situado na Região da Campanha do Estado do Rio Grande do Sul, Brasil. Desenvolveram-se um total de quatro questões norteadoras: (1) conhecimento sobre resíduos sólidos hospitalares; (2) identificação de resíduos sólidos hospitalares; (3) rotina no serviço de saúde ao manusear resíduos sólidos hospitalares; e (4) entendimento sobre danos provocados por resíduos sólidos hospitalares, sendo as falas dos depoentes (n=18) gravadas, com posterior degravação para análise de conteúdo. Emergiram as categorias: Entendimento sobre Resíduos Sólidos Hospitalares; Etapas do Gerenciamento de RSS; Dificuldades e Limitações; e Risco do Descarte Inapropriado. Resultados: Os dados revelaram fragilidades sobre a temática quanto conceito, gestão e riscos; não havendo clareza sobre a logística dada aos RSS; há falta de conhecimento sobre a categorização dos distintos tipos e o gerenciamento de RSS, evidenciando o não entendimento do correto descarte dos RSS gerados no contexto hospitalar, sinalizando para a necessidade de treinamento sobre o tema. Conclusão:Compreenderam-se as dificuldades sobre o conhecimento e entendimento dos entrevistados do presente estudo acerca da temática, impactando em suas atitudes e em suas práticas.


Objective: To understand university hospital workers' perception, attitudes and practices in relation to solid health-care waste (SHW). Methods: An interview with health professionals was carried out using a descriptive and qualitative methodology in 2019 at a university hospital in the city of Bagé, located in the Campanha Region of the State of Rio Grande do Sul, Brazil. A total of four guiding questions were formulated: (1) knowledge about solid hospital waste; (2) identification of solid hospital waste; (3) routine in the health service when handling solid hospital waste; and (4) understanding of damage caused by solid hospital waste, with the interviewees' reports (n=18) being recorded and transcribed verbatim for content analysis. The following categories emerged: Understanding Solid Hospital Waste; SHW Management Steps; Difficulties and Limitations; and Risk of Inappropriate Disposal. Results: The data revealed weaknesses on the subject in terms of concept, management and risks; there is no clarity about the logistics involving SHW; there is a lack of knowledge about the categorization of the different types and the management of SHW, highlighting the lack of understanding of the correct disposal of the SHW generated in the hospital context and signaling the need for training on the subject. Conclusion: The difficulties related to the interviewees' knowledge and understanding of the subject were understood and impact their attitudes and practices.


Objetivo: Comprender la percepción, actitudes y prácticas de trabajadores de un hospital universitario frente a residuos sólidos de salud (RSS). Métodos: Fue realizado, por medio de metodología descriptiva y cualitativa, encuesta con profesionales de salud en 2019 en un hospital universitario del municipio de Bagé, ubicado en la Región de la Campaña del Estado del Rio Grande del Sur, Brasil. Fueron desarrolladas un total de cuatro cuestiones norteadoras: (1) conocimiento sobre residuos sólidos hospitalarios; (2) identificación de residuos sólidos hospitalarios; (3) rutina en el servicio de salud al manejar residuos sólido hospitalarios; y (4) entendimiento sobre daños provocados por residuos hospitalarios, siendo las hablas de los deponentes (n=18) grabadas, con posterior transcripción literal para análisis de contenido. Surgieron las categorías: Entendimiento sobre Residuos Sólidos Hospitalarios; Etapas de Gestión de RSS; Dificultades y Limitaciones; y Riesgo de Eliminación Inapropiado. Resultados: Los datos mostraron debilidad sobre la temática cuanto al concepto, gestión y riesgos; no habiendo claridad sobre la logística dada a los RSS; hay falta de conocimiento sobre la categorización de los distintos tipos y la gestión de RSS, evidenciando el no entendimiento de la correcta eliminación de los RSS generados en el contexto hospitalario, señalando para la necesidad de entrenamiento sobre el tema. Conclusión: Se comprendieron las dificultades sobre el conocimiento y entendimiento de los encuestados del presente estudio acerca de la temática, impactando en sus actitudes y en sus prácticas.

ABSTRACT

El personal de salud (PS) está más expuesto que la población general a enfermar por SARS-CoV-2, por lo que debe utilizar elementos de protección personal (EPP) en todas las atenciones. El PS de Clínica Las Condes (CLC) que requirió licencia médica (LM) por COVID-19 durante el periodo de estudio, fue evaluado por Organismo Administrador según Ley 16.744, categorizando cada caso de contacto estrecho (CE) o contagio, en común o laboral. Además, fue contactado en su totalidad por equipo de RRHH y de IAAS, para conocer estado de salud, factores de riesgo y evolución. El objetivo fue caracterizar los casos y CE de PS que tuvieron LM, relacionándolas con diferentes eventos ocurridos en la institución y la comunidad durante el periodo comprendido entre el 12 de febrero y el 31 de julio. Un 21% del PS requirió LM en el periodo y un 12,97% presentó infección confirmada por PCR. En cuanto a la distribución por sexo y edad de casos y CE, esta no difiere de la distribución observada en el total de funcionarios de CLC. Se observa que, en los casos, la mayoría corresponde a categoría intrahospitalaria, en cambio los CE, la mayor parte correspondió a comunitario. En relación con casos confirmados intrahospitalarios, destaca que el primer caso ocurrió 63 días después del primer paciente hospitalizado en CLC con diagnóstico de COVID-19, y cuando ya estaba instalada la epidemia en la Región Metropolitana (RM) de Santiago , lo que refleja la efectividad de las medidas de prevención adoptadas al interior de CLC.


Health care workers (HCW) are at higher risk to get sick from SARS-CoV-2 than general population, so they must use personal protective equipment (PPE) in all care situations. The HCW at Clinica las Condes (CLC) that required a sick leave (SL) during the study period was evaluated by the Administrative Institution according to Law 16.744, which categorized each case of close contact (CC) or confirmed case, as community-acquired (CA) or health care-associated (HCA). In addition, all of them were traced by the HR and Infection Control team, to find out health status, risk factors and evolution. The aims were to characterize the confirmed cases and CC, their association with key events that occurred in the institution and the community between February 12th and July 31th. 21%

ABSTRACT Objective: Evaluate, from the literature, the effectiveness of the implementation of the multimodal strategy for health professionals compliance with Hand Hygiene and its sustainability over time. Method: Integrative review, with a view to answering the following question: "Is the implementation of the multimodal strategy effective in health professionals compliance with Hand Hygiene and can it be sustained over time?". The MEDLINE, SCOPUS, LILACS and CINAHL databases were used to retrieve the primary articles. Results: Twenty-five studies were analyzed. Among the components of the multimodal strategy, three need to be better worked: health education, feedback from practices and management involvement. Although it needs to focus more on its five elements, interventions based on the multimodal strategy have favored HH compliance and its long-term sustainability. Conclusion: The strategy proved to be effective for HH compliance, especially when all integrating components are adequately addressed.


RESUMEN Objetivo: Evaluar, a partir de la literatura, la eficacia de la implementación de la estrategia multimodal en la adhesión de los profesionales de salud a la Higiene de las Manos y su sostenibilidad a lo largo del tiempo. Método: Revisión integradora, para responder a la siguiente pregunta: "¿La implementación de la estrategia multimodal es eficaz en la adhesión de los profesionales de la salud a la Higiene de las Manos y puede ser sostenida a lo largo del tiempo?" Se utilizaron las bases de datos MEDLINE, SCOPUS, LILCAS y CINAHL para recuperar los artículos primarios. Resultados: Se analizaron 25 estudios. Entre los componentes de la estrategia multimodal, tres necesitan ser mejor trabajados: educación en salud, feedback de prácticas e involucramiento de la gestión. Aunque necesite un mayor enfoque de sus cinco elementos, las intervenciones basadas en la estrategia multimodal favorecieron la adhesión a la HM y su sostenibilidad a largo plazo. Conclusión: La estrategia se mostró eficaz en la adhesión a la HM, en particular cuando todos los componentes integradores están adecuadamente contemplados.

Método: Revisão integrativa, com vistas a responder a seguinte questão: "A implementação da estratégia multimodal é eficaz na adesão dos profissionais de saúde à Higiene das Mãos e pode ser sustentada ao longo do tempo?". Utilizaram-se as bases de dados MEDLINE, SCOPUS, LILACS e CINAHL para recuperar os artigos primários. Resultados: Foram analisados 25 estudos. Dentre os componentes da estratégia multimodal, três precisam ser melhor trabalhados: educação em saúde, feedback de práticas e envolvimento da gestão. Embora necessite maior enfoque dos seus cinco elementos, as intervenções baseadas na estratégia multimodal favoreceram a adesão à HM e sua sustentabilidade em longo prazo. Conclusão: A estratégia se mostrou eficaz para adesão à HM, em especial, quando todos componentes integradores são adequadamente contemplados.

Purpose The purpose of this paper is to add a little piece to the research on boundary work and inter-occupational cooperation by addressing two questions: how do actors perform boundary work in an inter-occupational cooperation project that seeks to improve the personnel health work in a hospital setting? What impact does the boundary work have on such cooperation in the personnel health project? Design/methodology/approach The study is based on individual, in-depth interviews and participative observations of focus group discussions conducted at a regional municipal organization in Sweden. Respondents are hospital line managers, experts and strategists in the HR departments, and experts from the internal occupational health service. Findings The concepts on boundary work, which include closing/opening boundary strategies, provide the framework for the empirical illustrations. The cooperation runs smoothly in the rehabilitation work because of an agreed upon process in which the professionals' jurisdictions are preserved through closing strategies. Illness prevention and health promotion are not areas of inter-occupational cooperation because the stronger actors use closing strategies. While the weaker actors, who try to cooperate, use opening boundary strategies in these areas, they are excluded or marginalized. Research limitations/implications The empirical investigation concerns one cooperation project and was completed at one data collection point. Originality/value No similar study of boundary work and inter-occupational cooperation in a hospital setting is available despite the frequency of this professional group configuration in practice. A more inclusive concept of professionalism may facilitate the study of boundary work and inter-occupational cooperation among actors with different professional authority.

ABSTRACT

O presente artigo tem por objetivo analisar a satisfação de trabalhadores de saúde mental que atuam em Centros de Atenção Psicossocial (Caps). A pesquisa é de natureza qualitativa. O instrumento de coleta de dados foi a entrevista semiestruturada, aplicada a 19 trabalhadores de três Caps em Fortaleza (Ceará). O tratamento do material empírico baseou-se na análise de conteúdo com ênfase em eixos temáticos. Os resultados revelaram os determinantes de (in)satisfação presentes no cotidiano desses trabalhadores. As relações estabelecidas com os usuários foram referidas como principal causa de satisfação, enquanto as condições de trabalho e o salário se constituem nos principais motivos de insatisfação. Além desses aspectos, emergiram consequências da (in)satisfação no trabalho no campo particular, social e organizacional da vida dos trabalhadores dos Caps, particularmente na saúde física e mental. Por fim, apontam para a urgência de implementação de estratégias, por parte da administração pública, que visem à desprecarização do trabalho em saúde e, mais particularmente, em saúde mental, tendo em vista a redução dos danos eventualmente causados pelo trabalho.


The scope of this article is to analyze satisfaction in the workplace of mental healthcare professionals who serve in Psychosocial Care Centers (Caps). The research is of a qualitative nature and the data-collecting medium was semistructured interviews with 19 workers of three Caps in Fortaleza, in the Northern Brazilian State of Ceará. The treatment of the empirical material was based upon the analysis of content with an emphasis on the thematic bias. The results revealed the determinants of (dis)satisfaction present in the daily routine of these workers. The relationships established with the users were singled out as the main source of satisfaction, whereas the work and wage conditions were the main motives for dissatisfaction. In addition to these aspects, consequences of (dis)satisfaction at work in the private, social and organizational field of the workers' life in the Caps were revealed, mainly in physical and mental health. Lastly, they emphasized the urgent need for implementation - on the part of public administration - of strategies that seek to reduce the precariousness of healthcare work, especially in mental health, with a view to mitigating damages potentially caused by such work.
