ABSTRACT
Objetivo: Relatar a construção e implantação de painel de bordo, desenvolvido por enfermeiros e profissionais da tecnologia da informação, para gerenciamento do Protocolo de Prevenção de Lesão por Pressão. Métodos: Trata-se de um relato de experiência sobre a construção e implantação de painel de bordo informatizado para gerenciamento de protocolo em um hospital privado universitário, localizado no interior do estado de São Paulo. Resultados: A construção do painel de bordo foi dividida nas seguintes etapas: revisão e atualização do protocolo, construção do modelo eletrônico e implementação. A divulgação foi realizada pela Comissão de Prevenção de Lesão por Pressão. Conclusão: O painel de bordo possibilitou a visualização rápida e em tempo real dos riscos dos pacientes, intervenções propostas e efetividade das medidas de prevenção, além de promover a integração e empoderamento dos profissionais na gestão do cuidado. Objective: To report the construction and implementation of a dashboard, developed by nurses and information technology professionals, to manage the Pressure Injury Prevention Protocol. Methods: This is an experience report on the construction and implementation of a computerized dashboard for protocol management in a private university hospital, located in the interior of the state of São Paulo. Results: The construction of the dashboard was divided into the following steps: review and update of the protocol, construction of the electronic model and implementation. The Pressure Injury Prevention Commission disclosed the tool. Conclusion: The dashboard enabled the quick and real-time visualization of patient risks, proposed interventions and effectiveness of prevention measures, in addition to promoting the integration and empowerment of professionals in the management of care. Objetivo: Informar la construcción e implementación de un panel, desarrollado por enfermeras y profesionales de tecnologías de la información, para gestionar el Protocolo de Prevención de Lesiones por Presión. Métodos: Se trata de un informe de experiencia sobre la construcción e implementación de un panel computarizado para la gestión del protocolo en un hospital universitario privado, en el interior del estado de São Paulo. Resultados: La construcción del panel se dividió en los siguientes pasos: revisión y actualización del protocolo, construcción del modelo electrónico e implementación. La divulgación fue realizada por la Comisión de Prevención de Lesiones por Presión. Conclusión: El panel permitió la visualización rápida y en tiempo real de los riesgos del paciente, las intervenciones propuestas y la efectividad de las medidas de prevención, además de promover la integración y el empoderamiento de los profesionales en la gestión del cuidado. AIMS AND OBJECTIVES: To analyse the process of elaborating social representations about pressure injury preventive measures by the nursing team (nurses and nurse technicians) and how this process relates to preventive practices for hospitalized patients. DESIGN: Qualitative study, with the application of the theory of social representations in its procedural methodological approach. METHODS: The study was carried out in an inpatient clinic of a public hospital in the state of Rondônia, Brazil. Totally, 28 nursing professionals in the medical clinic sectors who had worked directly with patient care for more than 6 months participated. The data were collected between July and September 2021 via in-depth interviews with the application of a semi-structured instrument. Analysis was carried out with the help of ALCESTE software, which performed a lexicographic analysis, and also via thematic analysis. The COREQ guided the presentation of the research report. RESULTS: The social representations were developed based on the professionals' symbolic beliefs about the visibility/invisibility of the results of applying preventive care. These symbolic constructions mobilized positive and negative feelings among the nursing team, which guided the classification of prevention practices as being of greater or lesser priority among other care activities. There were favourable attitudes among professionals, which included applying prevention measures in their daily routines, and unfavourable attitudes of non-adherence to the institution's protocol for preventing pressure injuries. CONCLUSIONS: The nursing team's perception of pressure injury prevention is influenced by symbolic, affective, values, and social dimensions. Non-adherence behaviours are attributed to the belief in the invisibility of prevention outcomes, resulting in a reluctance to implement preventive measures. RELEVANCE TO CLINICAL PRACTICE: Understanding the subjective logic that explains the thinking and actions of the nursing team suggests the need to incorporate discussions on beliefs, values, sentiments, and attitudes of nursing professionals into educational programs on pressure injury prevention. ABSTRACT
OBJECTIVE: This study aimed to analyse the nutritional factors and pressure injury (PI) risk in hospitalised patients post-stroke. METHOD: The research employed a descriptive observational method in which patients ≥18 years of age were followed for six days. Nutritional evaluation was based on anthropometric and dietary factors. The nutritional risk was assessed via anthropometric measurements, Braden nutrition subscale and daily dietary intake. PI risk was evaluated through the Braden Scale. The Wilcoxon test, paired t-test, and Kruskal-Wallis test were applied and corrected with Bonferroni correction or analysis of variance, followed by the post hoc Tukey test. RESULTS: During their hospital stays, the participating 59 patients had an increase in sensory perception (p=0.02) and nutrition (p=0.005) scores. It was observed that patients at high risk of PI did not meet daily nutritional recommendations for calories, proteins, carbohydrates, lipids and micronutrients (zinc, selenium and copper) compared with patients at low-to-moderate risk. Weight (p<0.001), body mass index (p<0.001), calf (p=0.01) and arm (p=0.04) circumferences, and subscapular (p=0.003) and triceps (p<0.001) skinfolds decreased during the six days of hospitalisation. CONCLUSION: From the findings of this study, it was concluded that nutritional factors, such as unmet recommended daily nutritional requirements of macronutrients and micronutrients, and nutritional status are associated with a higher risk of developing a PI.

OBJECTIVE: This article aims to present a narrative review of current literature about the anatomical characteristics of the scalp as well as current practices in the management of surgical, traumatic and pressure injuries in the scalp, which are common in neurosurgery practice. METHOD: We searched PubMed for publications and book chapters in English from 2011 to 2021. We also included commonly referenced papers that we considered relevant to the subject with publication before these dates. We used the search terms 'laceration,' and/or 'neurosurgery' and/or, 'pressure injury,' and/or 'craniotomy,' and/or 'surgical incision' in combination with 'scalp,' and/or 'wound care.' We also searched the reference lists of publications identified by the search strategy and selected those that we judged relevant. RESULTS: We pre-selected 52 articles that covered various aspects of anatomy, pathophysiology, scalp wound management, or general wound care that we considered applied to the anatomical region of our interest. After abstract review, we selected 34 articles that met our search criteria and were included in our review. CONCLUSION: There is limited evidence regarding classification and care of scalp wounds. As a result, many of the current practices for scalp wound management are based on evidence derived from studies involving different anatomical regions, not considering its particular anatomy, vasculature and microbiome. Further research is needed for more comprehensive and effective protocols for the management of scalp injuries. However, this present review proposes responses to the identified gaps concerning the management of scalp wounds.

BACKGROUND: Pressure injuries (PIs) are a major problem for healthcare providers, impacting both care costs and patients' quality of life, although they are predominately preventable. These injuries are especially present in Intensive Care Units (ICUs) as a result of the severity of the clinical conditions of patients in this unit. AIM: To develop a simplified version of the Braden scale by removing two of the most subjective subscores-Nutrition and Sensory Perception-in an attempt to reduce the chance of errors by the nursing team during the application of the scale. STUDY DESIGN: A cross-sectional study was conducted on data collected from patients admitted to the ICU of a private Brazilian tertiary hospital. The resulting data consisted of 5194 patients, 6353 hospital admissions, and 6974 ICU stays. The overall prevalence of PI was 1.09%. RESULTS: The T-test showed that both the Braden and the simplified Braden scores were significantly different between patients with and without PI (p < .001). Patients who developed PIs scored lower than those who did not. The area under the Receiver Operating Characteristic curve of the Braden Scale was 74.21% (95% CI: 68.61%-79.8%) and of the simplified scale was 72.54% (95% CI: 66.87%-78.22%). The Positive Predictive Value of the Braden Scale was 3.17% when interpolated at the same sensitivity as the simplified scale (47.37%), which achieved 3.26%. CONCLUSIONS: By removing two of the six subscores of the Braden scale we propose a new tool for identifying patients at risk of developing PI in a more objective and fast way. Our results show that classification performance had little negative impact. RELEVANCE TO CLINICAL PRACTICE: A simplified, less subjective scale allows for more precise and less time-consuming risk classification.

ABSTRACT Objective: to assess the effect of implementing a quality improvement project on the process of pressure injury prevention in an adult Intensive Care Unit. Method: a quality improvement project for the pressure injury prevention process was carried out in an adult Intensive Care Unit of a public hospital from November 2022 to July 2023. It was developed following the steps of an improvement cycle. The quality level of pressure injury prevention was measured before and after the interventions, using six quality criteria. Data collection for the first assessment was conducted in March 2023, retrospectively, referring to November and December 2022 and January 2023. Quality reassessment occurred in July 2023, also retrospectively, referring to April, May, and June 2023. Interventions included changes in records related to assistance in pressure injury prevention and education/awareness of the team on pressure injury prevention. Results: the initial quality assessment showed that the compliance level of pressure injury prevention was low, with virtually all criteria showing rates below 50%. After the interventions, there was an increase in compliance with almost all criteria. Conclusion: the use of a quality improvement project enabled the improvement of the pressure injury prevention process and contributed to the scientific community by corroborating the effectiveness of these projects in implementing pressure injury prevention programs, as well as prompting reflection on the multifactorial nature involved in this preventive process.
RESUMEN Objetivo: evaluar el efecto de la implementación de un proyecto de mejora de la calidad en el proceso de prevención de lesiones por presión en una Unidad de Cuidados Intensivos para adultos. Método: se llevó a cabo un proyecto de mejora de la calidad del proceso de prevención de lesiones por presión en una Unidad de Cuidados Intensivos para adultos de un hospital público, entre noviembre de 2022 y julio de 2023. Este proyecto se desarrolló siguiendo las etapas de un ciclo de mejora. El nivel de calidad en la prevención de lesiones por presión se midió antes y después de las intervenciones, utilizando seis criterios de calidad. La recopilación de datos para la primera evaluación se realizó en marzo de 2023, retrospectivamente, para los meses de noviembre y diciembre de 2022 y enero de 2023. La reevaluación de la calidad se llevó a cabo en julio de 2023, también retrospectivamente, para los meses de abril, mayo y junio de 2023. Las intervenciones incluyeron cambios en los registros relacionados con la asistencia en la prevención de lesiones por presión y la educación/concientización del equipo sobre la prevención de lesiones por presión. Resultados: la evaluación inicial de la calidad mostró que el nivel de conformidad en la prevención de lesiones por presión era bajo, con casi todos los criterios presentando tasas inferiores al 50%. Después de las intervenciones, hubo un aumento en la conformidad en casi todos los criterios. Conclusión: el uso de un proyecto de mejora de la calidad permitió mejorar el proceso de prevención de lesiones por presión y contribuyó con la comunidad científica, al corroborar la eficacia de estos proyectos en la implementación de programas de prevención de lesiones por presión, así como para promover la reflexión sobre los múltiples factores involucrados en este proceso preventivo.
RESUMO Objetivo: Avaliar o efeito da implementação de um projeto de melhoria da qualidade no processo de prevenção de lesão por pressão numa Unidade de Terapia Intensiva adulto. Método: Projeto de melhoria da qualidade do processo de prevenção de lesão por pressão, realizado em uma Unidade de Terapia Intensiva adulto, de um hospital público, no período de novembro/2022 a julho/2023. Foi desenvolvido seguindo as etapas de um ciclo de melhoria. O nível de qualidade da prevenção de lesão por pressão foi medido antes e depois das intervenções, utilizando seis critérios de qualidade. A coleta de dados da primeira avaliação foi realizada em março/2023, de forma retrospectiva, referente aos meses de novembro e dezembro/2022 e janeiro/2023. A reavaliação de qualidade ocorreu em julho de 2023, também de forma retrospectiva, referente aos meses de abril, maio e junho/2023. As intervenções incluíram mudanças nos registros relacionados à assistência na prevenção de LP e educação/sensibilização da equipe sobre prevenção de lesão por pressão. Resultados: A avaliação inicial da qualidade mostrou que o nível de conformidade de prevenção de lesão por pressão era baixo, com praticamente todos os critérios apresentando taxas inferiores a 50%. Após as intervenções, houve aumento na conformidade de quase todos os critérios. Conclusão: A utilização de um projeto de melhoria de qualidade possibilitou a melhora do processo de prevenção de lesão por pressão e contribuiu com a comunidade científica, ao corroborar a eficácia destes projetos na implementação de programas de prevenção lesão por pressão, bem como incitou a reflexão acerca da multifatorialidade envolvida neste processo preventivo.

ABSTRACT Objective: To observe the therapeutic effect of gentiopicroside, as the main component of Gentianaceae, on wounds in pressure injury (PI) model rats and explore its mechanism. Method: Male Sprague Dawley rats were randomly divided into control group, model group and gentiopicroside groups (50, 100 and 200 mg·kg-1·d-1 for 9 consecutive days). The mice's skeletal muscle fibroblast line NOR-10 cells were collected after being treated with gentiopicroside (0.2~5.0 M) and basic fibroblast growth factor receptor 1 (bFGFR1) inhibitor (5.0 M SU5402) for 7 days. Results: Compared to the model group, the gentiopicroside groups showed significantly increased wound healing rates, reduced inflammatory cells in the wound tissues, and significantly increased expression levels of proliferating cell nuclear antigen (PCNA) and bFGFR1, accompanied by increased proliferation of new myofibroblasts. Gentiopicroside upregulated the mRNA expression of bFGFR1 and PCNA in NOR-10 cells in a dose-dependent manner; however, SU5402 reversed the effect of gentiopicroside. Conclusion: Gentiopicroside may promote myofibroblast proliferation by upregulating the expression of bFGFR1 and PCNA and ultimately accelerating the healing of PI wounds.
RESUMO Objetivo: Observar o efeito terapêutico do gentiopicrosídeo como principal componente das Gentianáceas em feridas de lesão por pressão (LP) em modelos de ratos e explorar seu mecanismo. Métodos: Ratos Sprague Dawley machos foram divididos aleatoriamente em grupo controle, grupo modelo e grupos gentiopicrosídeo (50, 100 e 200 mg·kg-1·d-1 por 9 dias consecutivos). As células NOR-10 da linha de fibroblastos do músculo esquelético de camundongos foram coletadas após serem tratadas com gentiopicrosídeo (0,2~5,0 μM) e inibidor do receptor 1 do fator de crescimento fibroblástico básico (bFGFR1) (5.0 μM SU5402) por 7 dias. Resultados: Em comparação com o grupo modelo, os grupos gentiopicrosídeo apresentaram taxas de cicatrização de feridas significativamente maiores, menos células inflamatórias nos tecidos da ferida e níveis de expressão de antígeno nuclear de proliferação celular (PCNA) e bFGFR1 significativamente maiores, acompanhados por aumento da proliferação de novos miofibroblastos. O gentiopicrosídeo regulou positivamente a expressão de mRNA de bFGFR1 e PCNA em células NOR-10 de maneira dependente da dose, enquanto o SU5402 reverteu o efeito do gentiopicrosídeo. Conclusão: O gentiopicrosídeo pode promover a proliferação de miofibroblastos, suprarregulando a expressão de bFGFR1 e PCNA e, em última análise, acelerando a cicatrização de feridas de LP.

Método: Se dividieron aleatoriamente ratas macho Sprague Dawley en grupo control, grupo modelo y grupos gentiopicrósido (50, 100 y 200 mg·kg-1·d-1 durante 9 días consecutivos). Se recogieron células NOR-10 de la línea de fibroblastos de músculo esquelético de ratón después de ser tratadas con gentiopicrósido (0.2~5.0 μM) y un inhibidor del receptor 1 del factor de crecimiento de fibroblastos básico (bFGFR1) (5.0 μM SU5402) durante 7 días. Resultados: En comparación con el grupo modelo, los grupos de gentiopicrósido mostraron tasas de curación de heridas significativamente más altas, menos células inflamatorias en los tejidos de la herida y niveles de expresión significativamente mayores del antígeno nuclear de proliferación celular (PCNA) y bFGFR1, acompañados de una mayor proliferación de nuevos miofibroblastos. El gentiopicrósido podría regular positivamente la expresión de ARNm de bFGFR1 y PCNA en células NOR-10 de manera dependiente de la dosis, sin embargo, SU5402 revirtió el efecto del gentiopicrósido. Conclusión: El gentiopicrósido puede promover la proliferación de miofibroblastos al aumentar la expresión de bFGFR1 y PCNA y, en última instancia, acelerar la cicatrización de las heridas de LP.

BACKGROUND: Although preventable, pressure injuries are commonly observed in the hospital setting and are estimated to be the third most costly condition, after cancer and cardiovascular disease. AIM: Nurses play a crucial role in the prevention and management of pressure injuries, however, published evidence on the effectiveness of educational interventions, directed specifically at nurses in the hospital environment, is scarce. METHOD: The authors retrieved published studies on the subject from selected databases (Pubmed/Medline, Embase, Web of Science and the Cochrane Library) in a number of languages (Portuguese, English, French and Spanish). The search yielded randomised controlled trials, as well as quasi-experimental and comparative studies. FINDINGS: In total, 11 studies were selected. The outcomes analysed, following some type of educational intervention, included the attitudes and knowledge of the nursing professionals, as well as the incidence of pressure injuries. CONCLUSION: The present study demonstrated that different educational strategies can help prevent pressure injuries in the hospital environment.

Resumo: Pesquisa de desenvolvimento metodológico cujo objetivo foi desenvolver curso massivo aberto on-line (MOOC) para enfermeiros sobre prevenção e tratamento da lesão por pressão na pessoa idosa, à luz da Teoria de Margaret Newman. Trata-se de pesquisa metodológica, aplicada à elaboração e validação de curso on-line, pautada nas cinco etapas do modelo ADDIE de design instrucional. A primeira etapa envolveu análise, com a definição de conteúdo a partir de revisão integrativa de literatura: utilização das Teorias de enfermagem no cuidado às pessoas idosas com lesão por pressão. A segunda etapa proporcionou desenho do curso, com definição de objetivos de aprendizagem, atividades, duração e ferramentas do curso. A etapa três foi representada pela criação e edição dos recursos educacionais abertos. Na etapa quatro, ocorreu a hospedagem do conteúdo e mídias no ambiente virtual escolhido. A quinta etapa incluiu a validação de conteúdo e aparência por juízes experts. Como resultados, emergiram três produções científicas em formato de revisão integrativa: 1) estratégias de continuidade do cuidado de enfermagem, ações preventivas e de tratamento da lesão por pressão em pessoas idosas; 2) Avaliação e fatores de risco para desenvolvimento de lesão por pressão em pessoas idosas; 3) Prevenção e tratamento de lesões por pressão em pessoas idosas. Foram criados 24 produtos técnicos em formato de material didático, contemplados por: quatro (4) vídeos de apresentação: apresentação das autoras, apresentação do curso, apresentação dos módulos 1 e 2; seis (6) videoaulas, três (3) para o módulo 1 e três (3) para o módulo 2. Oito (8) infográficos divididos entre módulo 1 e módulo 2; dois (2) ebooks módulo 1 e módulo 2; um (1) estudo de caso; um (1) podcast e um (1) Curso no modelo MOOC Lesão por pressão na pessoa idosa: cuidado de enfermagem à luz da Teoria de Margaret Newman. O curso MOOC desenvolvido é potente para fortalecer a assistência de enfermagem aos idosos portadores de lesão por pressão. Além disso trata-se de estratégia de ensino inovadora para estudantes da área de ciências da saúde. A aplicação de forma sistematizada do referencial teórico deste estudo, a Teoria da expansão da consciência de Margaret Newman, evidenciou-se no processo formativo, uma vez que a teorista discute a enfermagem como profissão e que o processo formativo é estabelecido pela identidade e responde por suas práticas. Ressalta-se a relevância do estudo por se tratar do desenvolvimento de um curso teórico, no modelo MOOC, gratuito, disponível em uma plataforma confiável e aberta, em que o cursista pode organizar-se para o cumprimento das atividades conforme a sua disponibilidade de carga horária.
Abstract: Methodological development research whose objective was to develop a massive open online course (MOOC) for nurses on the prevention and treatment of pressure injuries in the elderly, in light of Margaret Newman's Theory. This is methodological research, applied to the development and validation of an online course, based on the five stages of the ADDIE model of instructional design. The first stage involved analysis, with the definition of content based on an integrative literature review: use of nursing theories in the care of elderly people with pressure injuries. The second stage provided the course design, defining learning objectives, activities, duration and course tools. Step three was represented by the creation and editing of open educational resources. In step four, the content and media were hosted in the chosen virtual environment. The fifth stage included content and appearance validation by expert judges. As results, three scientific productions emerged in the format of an integrative review: 1) nursing care continuity strategies, preventive actions and treatment of pressure injuries in elderly people; 2) Assessment and risk factors for the development of pressure injuries in elderly people; 3) Prevention and treatment of pressure injuries in elderly people. 24 technical products were created in teaching material format, comprising: four (4) presentation videos: presentation of the authors, presentation of the course, presentation of modules 1 and 2; six (6) video lessons, three (3) for module 1 and three (3) for module 2. Eight (8) infographics divided between module 1 and module 2; two (2) e-books module 1 and module 2; one (1) case study; one (1) podcast and one (1) Course using the MOOC model Pressure injuries in the elderly: nursing care in light of Margaret Newman's Theory. The MOOC course developed is powerful in strengthening nursing care for elderly people with pressure injuries. Furthermore, it is an innovative teaching strategy for students in the area of health sciences. The systematic application of the theoretical framework of this study, Margaret Newman's Consciousness Expansion Theory, was evident in the training process, since the theorist discusses nursing as a profession and that the training process is established by identity and responds to their practices. The relevance of the study is highlighted as it concerns the development of a theoretical course, in the MOOC model, free of charge, available on a reliable and open platform, in which the course participant can organize themselves to carry out the activities according to their availability. hourly.

Wound care is a complex procedure and the related research may include many variables. Deficiencies in the sample inclusion and exclusion criteria may limit the generalizability of randomized controlled trials (RCTs) for wound patients in the real world. This study aimed to evaluate deficiencies in reporting the inclusion and exclusion criteria and the characteristics of patients in RCTs of pressure injuries (PI) therapeutic interventions. We conducted a systematic methodological review in which 40 full text RCTs of PI treatment interventions published in English, from 2008 to 2020, were identified. Data on the general characteristics of the included RCTs and data about inclusion/exclusion criteria and characteristics of patients were collected. The inclusion/exclusion criteria were categorized into five domains (definition of disease, precision, safety, ethical/legal and administrative). Study duration (in weeks) was 8.0 (quartile 1: 2.0; quartile 3: 48.0); only 5.0% of the trials mentioned race, skin colour or ethnicity, and 37.5% reported the duration of the wound. Only 9 (22.5%) studies reported the drugs that the included patients were using and 10 (25.0%) RCTs reported adverse events. The presence of the five domains was observed only in 12.5% of RCTs and only 12 (30.0%) had the precision domain. Much more research is required in systematic assessments of the external validity of trials because there is substantial disparity between the information that is provided by RCTs and the information that is required by clinicians. We concluded that there are deficiencies in reporting of data related to inclusion/exclusion criteria and characteristics of patients of RCTs assessing PI therapeutic interventions.

BACKGROUND: Wheelchair positioning systems can prevent postural deficits and pressure injuries. However, a more effective professional follow-up is needed to assess and monitor positioning according to the specificities and clinical conditions of each user. OBJECTIVE: This study aims to present the concept of an electronic system embedded in a motorized wheelchair, based on the Internet of Things (IoT), for automated positioning as part of a study on wheelchairs and telemonitoring. METHODS: We conducted a mixed methods study with a user-centered design approach, interviews with 16 wheelchair users and 66 professionals for the development of system functions, and a formative assessment of 5 participants with descriptive analysis to design system concepts. RESULTS: We presented a new wheelchair system with hardware and software components developed based on coparticipation with singular components in an IoT architecture. In an IoT solution, the incorporation of sensors from the inertial measurement unit was crucial. These sensors were vital for offering alternative methods to monitor and control the tilt and recline functions of a wheelchair. This monitoring and control could be achieved autonomously through a smartphone app. In addition, this capability addressed the requirements of real users. CONCLUSIONS: The technologies presented in this system can benefit telemonitoring and favor real feedback, allowing quality provision of health services to wheelchair users. User-centered development favored development with specific functions to meet the real demands of users. We emphasize the importance of future studies on the correlation between diagnoses and the use of the system in a real environment to help professionals in treatment.

OBJECTIVE: To compare the predictive capacity of the current risk assessment scale for pressure ulcers in intensive care (EVARUCI), translated into Brazilian Portuguese, using the Braden scale. METHOD: This cross-sectional study collected prospective data from adult patients in three intensive care units. The receiver operating characteristic (ROC) and precision-recall curve (PR curve) were used to analyse the predictive capacity for pressure injury (PI) using both predictive values and odds ratios (ORs). RESULTS: The incidence of PIs in the study sample of 324 patients was 14.2%. The area under the ROC curve was 0.807 for EVARUCI and 0.798 for the Braden scale. At a cutoff point of 10 on the EVARUCI scale, sensitivity was 69.6%; specificity 78.4%; positive predictive value 34.8%; and OR 8.3. At a cutoff point of 11 on the Braden scale, sensitivity was 76.1%; specificity 75.9%; positive predictive value 34.3%; and OR 10. The area under the PR curve was 0.396 for the EVARUCI scale and 0.348 for the Braden scale, reflecting a smaller area for both. The F1 score value was 0.476 with 37.5% precision and 65.2% recall for the EVARUCI scale, and 0.473 with 34.3% precision and 76.1% recall for the Braden scale. CONCLUSION: The EVARUCI scale predictive capacity was similar to that of the Braden scale. However, the precision of both scales was low for the accurate prediction of patients at risk of developing PIs.

Objetivo: Desenvolver um aplicativo móvel que utilize a Escala de Glamorgan para prever o risco de lesões por pressão em pacientes pediátricos, no cuidado à beira do leito. Método: Trata-se de um estudo metodológico para o desenvolvimento de um aplicativo móvel visando a predição do risco de lesão por pressão pela Escala de Glamorgan, com base no referencial metodológico de Cook & Dupras. Resultados: Seguindo as etapas estabelecidas pelo referencial metodológico, concluímos o desenvolvimento do aplicativo móvel em saúde intitulado "LPP - Escala de Glamorgan". O aplicativo é composto por cinco abas que fornecem informações relevantes sobre a avaliação e prevenção de lesões por pressão. Conclusão: O aplicativo foi desenvolvido conforme as etapas estabelecidas no referencial metodológico. Além disso, foi incluída uma aba específica para facilitar a aplicação rápida e intuitiva da Escala de Glamorgan por enfermeiros durante o atendimento à beira do leito.(AU)

Method: This is a methodological study for the development of a mobile application aimed at predicting the risk of pressure injury by the Glamorgan Scale, based on the methodological framework of Cook & Dupras. Results: Following the steps established by the methodological framework, we completed the development of the mobile health application entitled "LPP - Glamorgan Scale". The application consists of five tabs that provide relevant information on the assessment and prevention of pressure injuries. Conclusion: The application was developed according to the steps established in the methodological framework. In addition, a specific tab was included to facilitate the quick and intuitive application of the Glamorgan Scale by nurses during bedside care.(AU)
Objetivo: Desarrollar una aplicación móvil que utilice la Escala de Glamorgan para predecir el riesgo de lesiones por presión en pacientes pediátricos en cuidados de cabecera. Método: Se trata de un estudio metodológico para el desarrollo de una aplicación móvil dirigida a predecir el riesgo de lesión por presión mediante la Escala de Glamorgan, basado en el marco metodológico de Cook & Dupras. Resultados: Siguiendo los pasos establecidos por el marco metodológico, completamos el desarrollo de la aplicación móvil de salud titulada "LPP - Escala de Glamorgan". La aplicación consta de cinco pestañas que proporcionan información relevante sobre la evaluación y prevención de las lesiones por presión. Conclusión: La aplicación se desarrolló siguiendo los pasos establecidos en el marco metodológico. Además, se incluyó una pestaña específica para facilitar la aplicación rápida e intuitiva de la Escala de Glamorgan por parte del personal de enfermería durante los cuidados a pie de cama.(AU)

Método: revisão integrativa da literatura coletada em fevereiro/2023 através do Portal de Periódicos da Coordenação de Aperfeiçoamento de Pessoal de Nível Superior. Foram incluídos artigos nos idiomas inglês e português, originais, relacionados ao tema e disponíveis na íntegra com acesso gratuito, publicados entre os anos de 2013 até 2023. Como critérios de exclusão: artigos que não atendam o objeto de estudo, duplicados, literatura cinzenta e não originais. Resultados: incluidos 6 artigos que evidenciam medidas de prevenção da lesão por pressão que surtiram efeitos postivos, e avaliar o conhecimento dos profissionais de enfermagem a respeito do tema. Conclusões: O estudo poderá contribuir para melhoria da qualidade de vida dos pacientes através do conhecimento dos profissionais de enfermagem acerca das tecnologias disponíveis para melhor atender e assim reduzir casos de lesão por pressão.(AU)
Objective: To describe the risk factors and nursing care for the prevention of pressure injury in the hospital context. Method: integrative literature review collected in February/2023 through the Portal of Periodicals of the Coordination for the Improvement of Higher Education Personnel. Articles were included in English and Portuguese, original, related to the theme and available in full with free access, published between 2013 and 2023. Exclusion criteria: articles that do not meet the object of study, duplicates, gray literature and non-original. Results: 6 articles were included that show measures to prevent pressure injury that had positive effects, and evaluate the knowledge of nursing professionals on the subject. Conclusions: The study may contribute to improving the quality of life of patients through the knowledge of nursing professionals about the technologies available to better serve and thus reduce cases of pressure injury.(AU)

Resultados: Se incluyeron 6 artículos que muestran medidas para prevenir las lesiones por presión que tuvieron efectos positivos, y evalúan el conocimiento de los profesionales de enfermería sobre el tema. COVID-19 has emerged as a global pandemic leading to an increase in hospitalization and intensive care unit (ICU) admissions worldwide. Due to severe acute respiratory distress syndrome (ARDS), many patients require prone positioning, which is associated with increased pressure ulcer/injury (PU/PI) incidence. COVID-19 pathophysiology may favor the occurrence of PU/PI due to hypoxemia, inflammatory status, and vasculopathy. This study aimed to compare the incidence of PU/PI in ICU patients before and during the COVID-19 pandemic. A retrospective cohort study was conducted at a university hospital in Brazil. Data from the medical charts of every adult patient admitted to ICU from March to July 2019 and the same period in 2020 were collected. The group from 2019 included 408 patients admitted due to multiple causes, and the group from 2020 included 229 patients admitted due to COVID-19 infection. The incidence of PU/PI was significantly higher in patients admitted in 2020 compared to 2019 (62,5 vs. 33,8%, respectively). Also, PU/PI location and severity have been different between groups, with the patients with COVID-19 (2020 group) more exposed to stage 3, 4, and non-stageable lesions, as well as more PU/PI on face skin and other less common locations. In conclusion, the COVID-19 pandemic has highlighted a higher PU/PI incidence. ICU patients were older during the pandemic, had higher body mass index and comorbidities, and needed more invasive medical devices and pronation. The occurrence of PU/PI was also associated with prolonged hospitalization and mortality.

OBJECTIVE: To identify the main ventilatory support medical devices related to the occurrence of pressure injuries in hospitalised adults, as well as the most frequent anatomical localisations of these injuries. METHODS: The Integrative review was registered at Open Science Framework as per DOI 10.17605/OSF.IO/P3NTZ. Two independent reviewers, in May 2022, searched the databases: PubMed, Embase, Cochrane Database of Systematic Reviews, LILACS and CINAHL; no language or publication year restriction. The review question was: What are the ventilatory support medical devices that cause PIs in hospitalised adults more often? The terms for searching the database were: "adult," "noninvasive ventilation," "artificial respiration." and "pressure injury," and their synonyms. This review followed the PRISMA checklist. RESULTS: The final sample was 21 articles. Oxygen nasal catheters caused up to 40.7% of the pressure injuries by ventilatory support devices identified, all in the ears. Noninvasive mechanical ventilation oronasal masks presented an incidence of pressure injuries of 63.3%. The nasal bridge was the site most affected by this mask. The Set of Holders for Insight® endotracheal tubes was the device that caused the most pressure injuries, with an incidence of 75%, affecting lip commissure. Tracheostomy cannula accounted for 18.2% of the pressure injuries related to ventilatory support devices; all lesions were in the neck. CONCLUSION: The ventilatory support devices causing pressure injuries and the most affected sites were, respectively, nasal catheters, ears; masks, nasal bridge; endotracheal tubes, lip commissures; tracheostomy cannulas, neck. RELEVANCE FOR CLINICAL PRACTICE: Knowing which respiratory devices cause pressure injuries more often in hospitalised adults and which anatomical localisations are more likely to be affected is fundamental for adopting preventive measures and reducing the occurrence of this problem. RESUMO: Objetivo: medir a taxa de adesão ao registro eletrônico da escala Munro no perioperatório. Método: estudo de coorte retrospectivo baseado na análise de prontuários no ano de 2022, em hospital privado, filantrópico de grande porte no município de São Paulo -BR. Os dados de preenchimento foram extraídos em um relatório em Excel e analisados quanto ao preenchimento da escala. Resultados: 22.743 registros de escala Munro foram analisados, correspondendo a 11.892 prontuários eletrônicos. A taxa de adesão foi de 98,45% na fase pré-operatória, 61,73% na intraoperatória e 90,14% na pós-operatória, sendo a taxa média de 83,44%. As falhas de registro estavam relacionadas à ausência ou preenchimento parcial de itens da escala e execução da fase intra ou pós-operatória sem avaliação da fase anterior. Conclusão: este estudo contribui para a prática profissional, mostrando a necessidade de intervenções educativas e melhorias de processos no intraoperatório para aumentar a adesão à avaliação de risco.
ABSTRACT Objective: To measure the rate of adherence to the electronic recording of the Munro scale in the perioperative period. Method: Retrospective cohort study based on the analysis of medical records in 2022 in a large private philanthropic hospital in the city of São Paulo -BR. The completion data was extracted into an Excel report and analyzed for compliance with the scale. Results: 22,743 Munro scale records were analyzed, corresponding to 11,892 electronic medical records. The adherence rate was 98.45% in the preoperative phase, 61.73% in the intraoperative phase, and 90.14% in the postoperative phase, with an average rate of 83.44%. Failures to record were related to the absence or partial completion of items on the scale and carrying out the intraor post-operative phase without evaluating the previous phase. Conclusion: This study contributes to professional practice by showing the need for educational interventions and improvements in intraoperative processes to increase adherence to risk assessment.
RESUMEN Objetivo: Medir la tasa de adherencia al registro electrónico de la escala Munro en el periodo perioperatorio. Método: Estudio de cohorte retrospectivo basado en el análisis de historias clínicas en el año 2022, en un gran hospital privado filantrópico de la ciudad de São Paulo -BR. Los datos de cumplimentación se extrajeron en un informe de Excel y se analizaron en función de si se había cumplimentado la escala. Resultados: Se analizaron 22.743 registros de la escala Munro, correspondientes a 11.892 historias clínicas electrónicas. La tasa de adherencia fue del 98,45% en la fase preoperatoria, del 61,73% en la intraoperatoria y del 90,14% en la postoperatoria, con una tasa media del 83,44%. Los fallos en el registro estaban relacionados con la ausencia o cumplimentación parcial de los ítems de la escala y con la realización de la fase intra o postoperatoria sin evaluar la fase anterior. Conclusión: Este estudio contribuye a la práctica profesional al mostrar la necesidad de intervenciones educativas y mejoras en los procesos intraoperatorios para aumentar el cumplimiento de la evaluación de riesgos.

RESUMO: Objetivo: elaborar e validar uma escala para avaliação dos recursos humanos e materiais na perspectiva da prevenção de lesões por pressão nas enfermarias de clínica médica e cirúrgica. Método: estudo metodológico conduzido em três etapas: revisão integrativa; elaboração; e validação mediante técnica Delphi, contando com oito enfermeiros especialistas em dermatologia de diferentes estados do Brasil, no período de novembro de 2018 a janeiro de 2019. Estes avaliaram 32 itens, referentes aos recursos humanos e materiais, nos cenários de pacientes de cuidado intermediário e alta dependência. Para validação, utilizou-se o índice de validade de conteúdo mínimo de 0,80. Resultados: no cenário de cuidado intermediário, todos os itens atingiram 0,77 na primeira fase, e 0,93 na segunda fase. No de alta dependência, atingiram 0,74 na primeira fase, e 0,84 na segunda fase. Conclusão: a escala permitirá avaliação do diagnóstico situacional das enfermarias na perspectiva da prevenção de lesão por pressão.
ABSTRACT Objective: To develop and validate a scale for evaluating human and material resources from the perspective of preventing pressure injuries in medical and surgical wards. Method: Methodological study was conducted in three stages: integrative review, elaboration, and validation using the Delphi technique, with eight nurses specializing in dermatology from different states in Brazil from November 2018 to January 2019. They assessed 32 items relating to human and material resources in intermediate care and high-dependency patients. A minimum content validity index of 0.80 was used for validation. Results: In the intermediate care scenario, all items reached 0.77 in the first phase and 0.93 in the second phase. In high dependency, they reached 0.74 in the first phase and 0.84 in the second phase. Conclusion: The scale will allow the assessment of the situational diagnosis of the wards from the perspective of pressure injury prevention.
RESUMEN Objetivo: desarrollar y validar una escala para evaluar los recursos humanos y materiales desde la perspectiva de la prevención de las lesiones por presión en las salas médicas y quirúrgicas. Método: estudio metodológico realizado en tres etapas: revisión integradora; elaboración; y validación mediante la técnica Delphi, con ocho enfermeros especializadas en dermatología de diferentes estados de Brasil, de noviembre de 2018 a enero de 2019. Estos evaluaron 32 ítems relativos a los recursos humanos y materiales en los escenarios de pacientes de cuidado intermedio y de alta dependencia. Para la validación se utilizó un índice de validez de contenido mínimo de 0,80. Resultados: En el escenario de cuidados intermedios, todos los ítems alcanzaron 0,77 en la primera fase y 0,93 en la segunda. En la alta dependencia, alcanzaron 0,74 en la primera fase y 0,84 en la segunda. Conclusión: La escala permitirá evaluar el diagnóstico situacional de las salas desde la perspectiva de la prevención de las lesiones por presión.

Introdução: As lesões por pressão (LPP) correspondem a danos na pele ou em tecidos moles encobertos, geralmente em áreas de proeminências ósseas, ou associado ao uso de dispositivo médico ou outro tipo de artefato. Diante disso, a enfermeira deve desenvolver sua prática, baseada em evidências científicas e priorizar a prevenção da formação das lesões na pele. Objetivo: Relatar a experiência de estudantes de Enfermagem sobre o desenvolvimento de ações de promoção do cuidado seguro relacionado à prevenção de LPP em pacientes internados em um hospital público pediátrico na Bahia. Metodologia: Trata-se de um estudo de caráter descritivo, do tipo relato de experiência, realizado por seis estudantes de Enfermagem em um hospital estadual pediátrico localizado no estado da Bahia, no período de novembro 2022. Principais resultados: Percebeu-se que existem profissionais de enfermagem que possuem interesse em obter mais conhecimento sobre o assunto, porém ainda há lacunas de conhecimento por parte da equipe sobre alguns assuntos abordados, tais como a escala de Braden e a utilização do colchão caixa de ovo para todas as faixas etárias. Foi possível perceber que os passos para prevenir a lesão por pressão não são cumpridos por parte da equipe e isso pode dificultar a prevenção das lesões de forma efetiva. Conclusão: Recomenda-se, portanto, que as unidades assistenciais, juntamente com suas respectivas coordenadoras, realizem ações contínuas de educação permanente, a fim de contribuir para uma melhor qualificação profissional, promovendo, acima de tudo, a segurança do paciente.
Introduction: Pressure lesions (PPL) correspond to damage to the skin or covert soft tissue, usually in areas of bony prominence, or associated with the use of a medical device or other type of artifact. Therefore, nurses should develop their practice, based on scientific evidence and prioritize the prevention of skin lesion formation. Objective: To report the experience of nursing students on the development of actions to promote safe care related to the prevention of PPL in patients admitted to a pediatric public hospital in Bahia. Methodology: This is a descriptive study, of the type of experience report, conducted by six nursing students in a pediatric state hospital located in the state of Bahia, in the period of November 2022. Main results: It was noticed that there are nursing professionals who have an interest in obtaining more knowledge about the subject, but there are still gaps in knowledge on the part of the team on some subjects addressed, such as the Braden scale and the use of the egg box mattress for all age groups. It was possible to notice that the steps to prevent pressure injury are not met by the team and this can hinder the prevention of injuries effectively. Conclusion: It is recommended, therefore, that the care units, together with their respective coordinators, carry out continuous actions of continuing education, in order to contribute to a better professional qualification, promoting, above all, patient safety.
Introducción: Las lesiones por presión (LPP) corresponden a lesiones en la piel o tejidos blandos encubiertos, generalmente en zonas de prominencia ósea, o asociadas al uso de un dispositivo médico u otro tipo de artefacto. Por ello, el personal de enfermería debe desarrollar su práctica, basándose en la evidencia científica y priorizando la prevención de la formación de lesiones cutáneas. Objetivo: Relatar la experiencia de estudiantes de enfermería sobre el desarrollo de acciones para promover cuidados seguros relacionados a la prevención de PPL en pacientes internados en un hospital público pediátrico de Bahía. Metodología: Se trata de un estudio descriptivo, del tipo relato de experiencia, realizado por seis estudiantes de enfermería en un hospital público pediátrico localizado en el estado de Bahía, en el período de noviembre de 2022. Principales resultados: Se observó que hay profesionales de enfermería que tienen interés en obtener más conocimientos sobre el tema, pero todavía hay lagunas de conocimiento por parte del equipo en algunos temas abordados, como la escala de Braden y el uso del colchón caja de huevos para todos los grupos de edad. Se pudo notar que los pasos para prevenir lesiones por presión no son cumplidos por el equipo y esto puede dificultar la prevención de lesiones de forma efectiva. Conclusiones: Se recomienda, por lo tanto, que las unidades asistenciales, junto con sus respectivos coordinadores, realicen acciones continuas de formación continuada, con el fin de contribuir a una mejor cualificación profesional, promoviendo, sobre todo, la seguridad del paciente.

Este estudo teve por objetivo avaliar o risco de desenvolvimento de lesão por pressão em pacientes internados na unidade de oncologia. Trata-se de estudo descritivo, analítico, transversal e de natureza quantitativa aprovado pelo Comitê de Ética da Universidade Federal de Alfenas sob parecer nº 5.916.419. A população de interesse foi composta por pacientes em tratamento oncológico de ambos os sexos, com idade acima de 18 anos e com tempo de internação superior a 24 horas. A coleta de dados ocorreu no período de março a maio de 2023, em uma unidade de internação oncológica. Foram utilizados o instrumento de caracterização sociodemográfica e clínica e a escala de Braden. Para análise estatística descritiva e inferencial foi utilizado o software Statistical Package for the Social Science (SPSS). Participaram 100 pacientes. A média da pontuação da Escala de Braden foi de 17,71 pontos, o que representa baixo risco de desenvolvimento de lesão por pressão. O presente estudo permitiu avaliar o risco de desenvolvimento de lesão por pressão em pacientes internados na unidade de oncologia por meio da Escala de Braden, sendo que, para esta amostra, o risco foi classificado como baixo.
This study aimed to evaluate the risk of developing pressure injury in patients admitted to the oncology unit. This is a descriptive, analytical, cross-sectional study of a quantitative nature approved by the Ethics Committee of the Federal University of Alfenas under opinion nº 5.916.419. The population of interest was composed of patients in oncological treatment of both sexes, aged over 18 years and with hospitalization time greater than 24 hours. The data collection took place in the period from March to May 2023, in an oncology hospitalization unit. The instrument of sociodemographic and clinical characterization and Braden's scale were used. The Statistical Package for the Social Science (SPSS) software was used for descriptive and inferential statistical analysis. 100 patients participated. The average Braden's score was 17.71 points, representing a low risk of developing pressure injury. The present study allowed to evaluate the risk of developing pressure injury in patients admitted to the oncology unit by means of the Braden scale, and for this sample, the risk was classified as low.

Se trata de un estudio descriptivo, analítico, transversal y cuantitativo, aprobado por el Comité de Ética de la Universidad Federal de Alfenas con arreglo al dictamen no 5 916 419. La población de interés se compone de pacientes sometidos a tratamiento oncológico de ambos sexos, mayores de 18 años y con tiempo de internación superior a 24 horas. La recolección de datos tuvo lugar entre marzo y mayo de 2023, en una unidad de internamiento para enfermos de cáncer. Se utilizó el instrumento de caracterización sociodemográfica y clínica y la escala Braden. El paquete estadístico para el programa de ciencias sociales (SPSS) se utilizó para el análisis estadístico descriptivo e inferencial. Había 100 pacientes. La puntuación media en la escala de Braden fue de 17,71 puntos, lo que representa un bajo riesgo de desarrollar lesión a presión. Este estudio ha permitido evaluar el riesgo de desarrollar lesión a presión en pacientes de la unidad oncológica a través de la Escala Braden, y para esta muestra el riesgo se ha clasificado como bajo.
