ABSTRACT

Preventing a luteinizing hormone (LH) surge is a major concern in controlled ovarian stimulation (COS). Several strategies have been developed over the years, including protocols with Gonadotrophin Releasing Hormone agonists and antagonists. More recently Progestin Primmed Ovarian Stimulation (PPOS) has shown to be equally effective in pituitary suppression, with comparable clinical and laboratorial outcomes. This is the case of a 34 year old female, with a previous diagnosis of primary infertility due to tubal factor and high ovarian reserve markers. The initial plan was to perform IVF/ICSI. followed by fresh blastocyst transfer. The chosen COS strategy was to use Alfacorifolitropin 150mg (Elonva®) and Cetrorelix acetate 0,25mg (Cetrotide®) in a flexible pituitary suppression protocol. However, because of elevated risk for Ovarian Hyper-stimulation Syndrome (OHSS) detected during ultrasound and hormonal monitoring, in order to diminish financial burden and to have a more patient friendly protocol, we switched cetrorelix acetate to oral dydrogesterone. COS was successful and resulted in 24 retrieved oocytes (16 metaphase 2 oocytes) without any premature LH peak. No OHSS symptoms occurred. Our main goal with this case report is to reinforce the feasibility and efficacy of this innovative approach, especially in patients aiming for a fresh embryo transfer, who present alert sings of OHSS during the stimulation. Developing friendlier and cheaper protocols in assisted reproduction makes the treatment more accessible and affordable.

As neoplasias mamárias são as mais frequentes nas fêmeas da espécie canina, sendo sua maioria de caráter maligno. Dentre as principais causas dessas neoplasias, destacam-se o fator genético, a idade, castração tardia, obesidade e o uso de anticoncepcionais. Embora o uso de métodos contraceptivos a base de fármacos em cadelas e gatas ser extremamente prejudicial, podendo acarretar vários problemas como infecções uterinas e câncer de mama, no Brasil sua comercialização em estabelecimentos veterinários é consideravelmente alta, talvez por ser considerado por muitos tutores como uma forma mais barata de controlar a natalidade, quando comparada com a ovariohisterectomia. Foram entrevistados os responsáveis/funcionários de 50 estabelecimentos veterinários na cidade de Descalvado SP e municípios da região por meio de questionários sobre a comercialização dos anticoncepcionais avaliando o percentual de estabelecimentos que fazem prescrição dessas medicações, e a forma como os mesmos são prescritos. 90% deles disseram comercializar algum tipo de fármaco contraceptivo, 50% responderam que a venda é feita sempre que o tutor solicita o uso do medicamento, 26,7% só indicam o uso antes de cada cio e 8,9% em caso de gestação indesejada; 77% administram a mesma dose independente do peso do animal; 16% dos entrevistados disseram não orientar os tutores sobre os possíveis riscos e complicações


Mammary neoplasms are the most frequent cancer in bitches and mostly of them are malignant. Among the main causes of these neoplasms are the genetic factor, age, late ovariohysterectomy, obesity and contraceptive use. Although the use of contraceptives in female dogs and cats is extremely harmful and can lead to various problems such as uterine infections and mammary cancer, in Brazil its marketing in veterinary establishments is considerably high, perhaps because many tutors consider it a cheaper way to birth control as compared to castration. The sales profile of contraceptives in veterinary establishments in the Sao Paulo state was evaluated in relation to the percentage of establishments that prescribe these medications as well as the way they are prescribed. Fifty veterinary establishments were interviewed through questionnaires, 90% said they sell the product, 50% answered that the sale is made whenever the owner requests the use of the drug, 26.7% only indicate use before each estrus and 8,9% in case of unwanted pregnancy; 77% administer the same dose regardless of the animal's weight; About the sterilization´s indication, 87% of respondents said they indicated; 16% said they did not advise tutors about the possible risks and complications of administering these contraceptives. Despite all the risks of contraceptives, their sale is still very common in veterinary establishments due to the affordable price, often without the guidance of an appropriate professional explaining about the risk to animal health.

ABSTRACT

To compare the reproductive performance after TAI in ewes synchronized with mid (12 or 13) or long (14-16 d) interval prostaglandin (PG) or progesterone plus eCG (P4-eCG) based protocols, 440 multiparous Corriedale ewes were synchronized with two PG injections administered 12-16 d apart (PG12, PG13, PG14, PG15 and PG16 respectively), or P4-eCG (MAP sponges 14 d and eCG). Cervical TAI (Day 0) was performed with fresh semen. It was evaluated the ovulated ewes (OE, %) and the ovulation rate (OR) on Day 8 by trans-rectal ultrasonography, the rate of non-return to service between Days 13 and 21 by painted rams, and the pregnancy rate, prolificacy and fecundity on Day 60 by trans-abdominal ultrasonography. No significant differences were found in OE among groups (P>0.05), but P4-eCG achieved higher OR (P<0.05) compared to PG protocols, without differences among them (P>0.05). Similar NRR-21, pregnancy and fecundity were observed among PG15 (64.3, 62.9 and 84.3), PG16 (59.7, 59.7 and 77.8) and P4-eCG (70.3, 66.2 and 95.9), but higher compared to PG12 (42.5, 39.7 and 52.1) and PG13 group (44.0, 40.0 and 48.0, respectively; P<0.05). PG14 achieved intermediate results compared to other groups. No differences were found in prolificacy among groups (P>0.05), except PG13 that was lower compared to P4-eCG (P<0.05). In conclusion, long interval between PG injections (15 or 16 d) determined better reproductive outcome that mid interval (12 or 13 d), equating the P4-eCG based protocol after cervical TAI with fresh semen during the breeding season in sheep.

O acetato de medroxiprogesterona (MA) é administrado em cadelas e gatas para supressão do estro. Porém, seu uso na gestação pode levar à distocia, salientando a importância em discutir os seus prováveis mecanismos de ação. Avaliou-se o caso de uma gata com secreção vaginal purulenta e não desencadeamento do parto, após aplicação do MA na gestação. Foi diagnosticada mace ração fetal e realizada ovariosalpingohisterectomia (OSH), com resolução da afecção. Os mecanismos pelos quais os progestágenos promovem a quiescência uterina ainda não são completamente conhecidos. Em outras espécies sugere-se ação nos níveis intracelulares de cálcio e de relaxina; nos receptores para ocitocina e na síntese de prostaglandina. No caso relatado, acredita-se que o MA inibiu a contração uterina e a abertura cervical desencadeou a maceração fetal.

However, its use during pregnancy can lead to dystocia, emphasizing the importance of discussing their possible mechanisms of action. It was evaluated the case of a cat with purulent vaginal discharge and not onset the birth, after application of MA during pregnancy. Was diagnosed fetal maceration and performed ovariosalpingohisterectomy (OSH), with resolution of the disease. The mechanisms by which the incorrect administration of MA leads to not trigger parturition in cats are not fully understood. In other species suggest action on the intracellular calcium levels and relaxin; the receptors for oxytocin and prostaglandin synthesis. In the reported case, it is believed that MA inhibited uterine contraction and cervical opening triggered fetal maceration.


El acetato de medroxiprogesterona (MA) se administra a los perros y gatos para la supresión del estro. Sin embargo, su uso durante el embarazo puede causar distocia, haciendo hincapié en la importancia de discutir sus posibles mecanismos de acción. Evaluado el caso de un gato con la secreción vaginal purulenta y no desencadenar el nacimiento, después de la aplicación del MA en el embarazo. Fue diagnosticado maceración fetal y ovariosalpingohisterectomy (OSH) celebrada el trabajo, con la resolución de la enfermedad. Los mecanismos por los que la progesterona promueve reposo uterino no están todavía completamente conocidos. En otras especies sugieren acción sobre los niveles de calcio intracelular y relaxina; los receptores de la oxitocina y la síntesis de prostaglandinas. En nuestro caso, se cree que MA inhibió la contracción uterina y la abertura cervical desencadena la maceración fetal.

Know the mechanism of action, main indication and possible side effects may help during the choice of the contraceptive method.


Apesar da ovariosalpingohisterectomia (OSH) ou ovariectomia serem os métodos contraceptivos de escolha para gatas domésticas, o manejo reprodutivo de animais geneticamente valiosos ou que ainda serão utilizados para reprodução requer métodos contraceptivos seguros e reversíveis. Diversos métodos contraceptivos reversíveis já foram utilizados em gatas entre eles os progestágenos, os andrógenos, os imunocontraceptivos e os agonistas do GnRH. Apesar de eficientes, muitos desses contraceptivos estão associados a efeitos colaterais sérios. Conhecer o mecanismo de ação, as principais indicações e os possíveis efeitos colaterais auxilia na escolha do contraceptivo a ser utilizado.
