ABSTRACT

La planificación preoperatoria resulta fundamental en la cirugía de escoliosis idiopática del adolescente (EIA) para determinar los niveles a fusionar y realizar el procedimiento con mayor precisión y diligencia. Sin embargo, su realización protocolizada no está tan extendida entre los especialistas en formación. El objetivo de este artículo es describir en detalle el método de planificación preoperatoria en la EIA empleado en una unidad especializada de raquis infantil y del adolescente de un centro de referencia, así como, la logística y la táctica, apoyado en un software gratuito de medición digital semiautomática y planificación. Se muestran 3 casos representativos de diferentes deformidades vertebrales, intervenidos mediante una artrodesis vertebral posterior tras una planificación preoperatoria según el método descrito. Este método resulta muy adecuado para el cirujano en formación al aunar las ventajas de los métodos tradicionales y los más modernos, y ser sencillo, de bajo coste, accesible, reproducible y con carácter formativo.(AU)

Describimos un método de planificación digital, entre lo tradicional y lo tecnológico, que conserva su esencia educativa, permite comprender la fractura e individualizar la osteosíntesis. Material y métodos: Tras el análisis inicial de la fractura y características del paciente, se realizan diferentes mediciones en las imágenes de Rx y TC con un programa de imagen médica digital. Estas imágenes se copian en un programa de presentación (Microsoft® PowerPoint o Keynote ©Apple Inc.), en el que se reproducen con el puntero del ordenador los principales fragmentos y líneas de fractura. A continuación, estos se mueven a una posición reducida y se representan gráficamente los implantes para la fijación interna junto con un guion de la estrategia quirúrgica. Resultados: Mostramos 4 casos de diferentes tipos de fracturas intervenidas mediante reducción y osteosíntesis tras una planificación preoperatoria según el método descrito. Se detallan los puntos básicos para la planificación quirúrgica, logística, táctica y los resultados radiológicos postoperatorios de cada caso. Conclusiones: A pesar del auge de programas informáticos avanzados de planificación, los métodos tradicionales con lápiz y papel siguen siendo fundamentales, más aún para el traumatólogo en formación. El método de planificación digital descrito resulta muy adecuado para este objetivo, al aunar las ventajas de ambos métodos: sencillez, accesibilidad, rapidez, bajo coste, reproducibilidad, carácter formativo y eficacia y por posibilitar la simulación, correcciones y la reutilización de casos.(AU)

However, there are barriers that radiologists should be aware of prior to implementing Artificial Intelligence in daily practice. Barriers include regulatory compliance, ethical issues, data privacy, cybersecurity, AI training bias, and safe integration of AI into routine practice. In this article, we summarize the issues and the impact on clinical radiology.

La inteligencia artificial (IA) ofrece la posibilidad de cambiar la práctica de la radiología clínica en todo el mundo. Sin embargo, existen dificultades que los radiólogos deben conocer antes de aplicar la inteligencia artificial en la práctica diaria. Estas dificultades incluyen cuestiones de cumplimiento de la legislación, cuestiones éticas, aspectos relacionados con la privacidad de los datos y la ciberseguridad, el sesgo de aprendizaje automático y la integración segura de la IA en la práctica habitual. En este artículo, resumimos estas cuestiones y su repercusión en la radiología clínica.(AU)

Introducción: Cuba es un país tropical, asediado por disimiles fenómenos naturales, que cada vez son más frecuentes y agresivos; la confección de una multimedia como material de apoyo en la disciplina Preparación para la Defensa, tiene gran impacto desde el punto de vista económico y social. Objetivo: confeccionar una multimedia educativa sobre desastres naturales, que apoye la docencia de la asignatura para la adecuada preparación de estudiantes de las ciencias médicas. Métodos: se realizó una investigación de desarrollo tecnológico, en la Universidad de Ciencias Médicas de Santiago de Cuba, en el período de enero a mayo de 2018. El universo estuvo constituido por 110 estudiantes pertenecientes al quinto año de la carrera de Medicina. Se realizó una validación del producto mediante el método Delphi. Resultados: se confeccionó una multimedia educativa para la preparación de estudiantes de las ciencias médicas ante situaciones de desastres, se obtuvieron valoraciones de muy adecuado en la totalidad de los indicadores, se evidenció un aumento del nivel de conocimientos de los estudiantes, se constató un 49,1 por ciento de aprobados antes de aplicar la multimedia y un 93,6 percent después de aplicarla. Conclusiones: la multimedia permitió la incorporación de nuevos conocimientos acerca de los diferentes tipos de desastres en estudiantes de las ciencias médicas, se consideró una herramienta efectiva a generalizar en la docencia(AU)


Introduction: Cuba is a tropical country, beset by dissimilar natural phenomena that are becoming more frequent and aggressive; the preparation of a multimedia as support material in the discipline Preparation for Defense has a great impact from the economic and social point of view. Objective: to prepare an educational multimedia about natural disasters that supports the teaching of the subject for the adequate preparation of students of medical sciences. Methods: a technological development research was carried out at the University of Medical Sciences of Santiago de Cuba, in the period from January to May 2018. The universe was made up of 110 students belonging to the fifth year of the Medicine career. Product validation was performed using the Delphi method. Theoretical and empirical methods were used. Results: an educational multimedia was prepared for the preparation of students of the medical sciences in situations of disasters, very adequate assessments were obtained in all of the indicators, an increase in the level of knowledge of the students was evidenced, confirming 49,1 percent of approved before applying multimedia and 93,6 percent after applying. Conclusions: multimedia allowed the incorporation of new knowledge about the different types of disasters in students of medical sciences, being considered an effective tool to generalize in teaching(AU)

ABSTRACT

Se decidió elaborar una multimedia educativa y didáctica para el aprendizaje y la correcta autopreparación en la asignatura Educación Física, como vía de solución a las insuficiencias detectadas en los estudiantes de primer y segundo años de la carrera de medicina. Esta se diseñó con la herramienta Mediator 9.0 y se estructuró en módulos que vinculan textos e imágenes, lo cual incentiva el aprendizaje y agiliza el estudio de los contenidos de la asignatura. Finalmente se logró una multimedia educativa con la integración de los contenidos de la asignatura en un solo material didáctico, muy fácil de usar en el trabajo individual, que mejora el interés y la motivación de los estudiantes, al mismo tiempo que estandariza e interrelaciona materias establecidas en el programa con otras asignaturas propias de la medicina.


It was decided to elaborate an educational and didactics multimedia for the learning and correct self-preparation in the Physical Education subject, as solution to the inadequacies detected in the students of first and second years of the medicine career. It was designed with the Mediator 9.0 tool and it was structured in modules that link texts and images, which motivates the learning and speeds up the study of the subject contents. Finally they managed to get an educational multimedia that integrates the contents of the subject in a single didactic material, very easy to use in the individual work, which improves the interest and motivation of the students, at the same time that it standardizes and interrelates established matters in the schedule with other subjects characteristic of the medicine.

La integración de imágenes médicas es el proceso de registrarlas y fusionarlas con el fin de conseguir una mayor cantidad de información diagnóstica. En este trabajo se realiza un análisis para la integración de imágenes obtenidas a través de la tomografía axial computarizada y la resonancia magnética, para lo cual se desarrolló una herramienta en el programa Matlab, donde se implementa el registro mediante características equivalentes; además, los pares de imágenes son comparados por varias reglas de fusión, con vistas a identificar el mejor algoritmo en el que la imagen fusionada resultante contenga la mayor cantidad de información de las representaciones originales(AU)


The medical images integration is the process of register and fuse them with the purpose of getting a higher quantity of diagnostic information. In this work an analysis for images integration obtained through the computerized axial tomography and the magnetic resonance is carried out, for which a tool in the Matlab program was developed, where the registration by means of equivalent characteristic is implemented; also, the couples of images are compared by several fusion rules, aimed at identifying the best algorithm in which the resulting image fused contains the highest quantity of information of the original representations(AU)

ABSTRACT

The 24-hour Dietary Recall is considered an efficient method to collect diet data and is also widely used in population food intake monitoring. However, it requires highly qualified interviewers, which hinders its use for large samples. The objective of this study was to develop a structured and automated 24-hour Dietary Recall (R24ec) to evaluate schoolchildren and adolescent food intake, with both online and offline modes. The foods chosen for the R24ec were pre-defined from a secondary database that represents the age range of the interviewee. Using a computer, the respondents described all foods and beverages they consumed the day before the interview. The R24ec includes six pre-defined meals, incorporated in questions that contextualize the daily routines and time of consumption on the day before the interview. It also contains a list of food divided into food groups and subgroups. During the interview, non-listed items were recorded in the blanks provided for this purpose. The R24ec also includes a food photographic atlas containing different types of food; it was used as a visual aid so that the interviewees could select portion sizes. The structured 24-hour Dietary Recall was successfully developed. It is posted at the University of Sao Paulo website: www.fsp.usp.br/nutrisim.


El uso del Recordatorio de 24 horas es un buen método para recolectar datos sobre dieta y se aplica al acompañamiento alimentario de la población. Este método, sin embargo, requiere encuestadores altamente entrenados, lo que dificulta su uso en investigaciones a gran escala. El objetivo de este estudio fue elaborar un Recordatorio de 24 horas estructurado, automatizado y computarizado (R24ec) para evaluación del consumo alimentario de niños y adolescentes; el Recordatorio puede ser completado de manera online y offline Los alimentos presentes en el R24ec fueron pre-definidos a partir de una base de datos secundaria, representativa de las edades estudiadas. Los entrevistados deben describir todos los alimentos y bebidas consumidos el día anterior a la entrevista, a través del uso de una PC. El R24ec contiene seis comidas pre-definidas, incluye preguntas que contextualizan las rutinas y los lugares visitados el día anterior. Contiene, además, una lista de alimentos dividida en grupos y subgrupos alimentares. Pueden adicionarse nuevos ítems, usando el menú ?ítems no encontrados?. Se desarrolló, además, un atlas fotográfico que ilustra diferentes tipos de alimentos y que ayuda visualmente a los entrevistados a seleccionar los tamaños de las porciones ingeridas. El Recordatorio de 24 horas estructurado fue desarrollado con éxito y el mismo está hospedado en el sitio web www.fsp.usp.br/ nutrisim de la Universidade de São Paulo.


O Recordatório de 24 horas é considerado um ótimo método para coletar dados dietéticos e é muito utilizado no acompanhamento alimentar da população. Porém requer entrevistadores altamente treinados, o que dificulta o seu uso em pesquisas com grandes amostras. O objetivo deste estudo foi desenvolver um Recordatório de 24 horas estruturado e computadorizado (R24ec) de autopreenchimento para avaliação do consumo alimentar de crianças e adolescentes nos modos on-line e off-line. Os alimentos escolhidos para o R24ec foram pré-definidos a partir de um banco de dados secundário representativo para esta faixa etária. Os entrevistados devem descrever todos os alimentos e bebidas consumidos no dia anterior à entrevista através de um computador. O R24ec foi desenvolvido contendo seis refeições pré-definidas, embutidas em questões que contextualizam as rotinas e locais do dia anterior. Contém ainda uma lista de alimentos dividida em grupos alimentares e subgrupos. Durante a entrevista, itens não listados podem ser adicionados em um grupo de ?itens não encontrados?. Desenvolveu-se ainda um atlas fotográfico contendo diferentes tipos de alimentos, que serve como auxílio visual, nele os entrevistados podem selecionar tamanhos de porções. O Recordatório de 24 horas estruturado foi desenvolvido com sucesso e está hospedado no site www.fsp.usp.br/nutrisim da Universidade de São Paulo.

ABSTRACT

La hemodiálisis es el proceder que permite mantener con vida a los pacientes con insuficiencia renal crónica terminal y se realiza a través de un riñón artificial. La necesidad de unidades nefrológicas más cerca de los lugares donde residen los afectados, demanda el entrenamiento y capacitación del personal, razón por la cual se implementó el sistema de autoentrenamiento para el manejo de riñones artificiales (SARA), producto que describe, entre otros aspectos, las funciones y las precauciones a tener en cuenta con el uso de dicho riñón. Se presentan aspectos concluyentes de 4 años de explotación del SARA y se compara con el nuevo sistema, el RENTREN, cuya implementación permite elevar el nivel científico de todos los que laboran en el Servicio de Hemodiálisis; también se incluye un simulador que posibilita al usuario evaluar su dominio de todo el proceso(AU)


Hemodyalisis is the procedure that allows to maintain alive the patients with terminal chronic renal failure and is carried out through an artificial kidney. The necessity of nephrologic units nearer to the places where the affected live, demands the training of the staff, reason why the self-training system was implemented for the use of artificial kidneys (SARA), a product which describes, among other aspects, the functions and the cares to keep in mind with the use of this kidney. Conclusive aspects of 4 years of exploitation of SARA are presented and it is compared with the new system, the RENTREN, which implementation allows to elevate the scientific level of all those who work in the Hemodyalisis Service; a simulator is also included that facilitates the user to evaluate the domain on the whole process(AU)

ABSTRACT

Se realizó una investigación descriptiva y retrospectiva, basada en la metodología de utilización de medicamentos del tipo prescripción-indicación, para describir la prescripción, elaboración y dispensación de los remedios homeopáticos en el Hospital General Docente "Dr. Juan Bruno Zayas Alfonso" de Santiago de Cuba, desde enero hasta diciembre del 2008, cuyas irregularidades fueron determinadas durante el diagnóstico inicial y eran causadas por el desconocimiento de los profesionales sobre esta modalidad terapéutica. Como respuesta a las deficiencias encontradas se confeccionó una multimedia que aporta los elementos teórico-metodológicos para recetar, confeccionar y administrar estos medicamentos alternativos, lo cual representa la incorporación y actualización de nuevos conocimientos en el modo de actuación de los especialistas médicos y farmacéuticos.


A descriptive and retrospective investigation based on the prescription-indication drug use methodology to describe the prescription, manufacturing and distribution of the homeopathic remedies was carried out in "Dr. Juan Bruno Zayas Alfonso" General Hospital in Santiago de Cuba, from January to December, 2008. Irregularities were determined during the initial diagnosis and were caused by the lack of knowledge about this therapeutic modality by the professionals. As a response to the detected deficiencies a multimedia was developed which provides the theoretical-methodological elements prescribing, manufacturing and controlling these alternative drugs, which represents the incorporation and updating of new knowledge in the performance of the medical specialists and pharmacists.

Dirigido a estudiantes de medicina, médicos en formación, residentes de la especialidad de bioquímica clínica, docentes de la materia, investigadores y profesionales de la salud en general, facilita comprender y ejercitar lo relacionado con las enzimas en la asignatura de Bioquímica, si bien puede ser utilizado para otros contenidos, previa adecuación de los temas. Designed for medical students, doctors in training, and residents in the specialty of clinical biochemistry, teachers of this subject, researchers and health professionals in general, it facilitates the understanding and teaching of all aspects related to the enzymes in the biochemistry subject, although it can be used for other contents with previous adequacy of the topics. It also includes videos, curiosities, updated reference materials and a glossary of terms, reason why it is an auxiliary learning resource, which can integrate knowledge, characterized by being simple and easy to navigate; advantages appreciated by students and teachers
