ABSTRACT

Posttraumatic malunion with combination of angular, rotational, and shortening deformity of the proximal phalanx may cause scissoring of a finger and impairment of hand function. Cosmetic disfigurement and severe dysfunction of fingers require surgical correction, most often via open corrective osteotomies and rigid fixation with a plate or screws. However, such an approach requires a longer incision, inevitably results in a scar, and has a higher potential for extensor tendon adhesion. Also, abruption of the periosteum and plating of the phalange requires longer bone healing time. Thus, we devised a technique of minimally invasive correction of phalangeal malunion using an external mini-fixator. We presented representative three cases of malunited fractures of phalanges treated with the Ilizarov mini-fixator in adolescence and review reports of similar cases. The usage of Ilizarov mini-fixator provided excellent outcomes for posttraumatic malunion of three fingers.

ABSTRACT

INTRODUCTION: A rare case of proximal phalange fractures has been presented in this study. PRESENTATION OF CASE: A 29-year-old male patient from Turkey was admitted to the orthopedics and traumatology clinic with a complaint of left hand pain, which had persisted for 10days. He described a rotational trauma that had occurred right after hitting his hand on the wheel of a car as a result of an in-car traffic accident 10days ago. Radiological work-ups were requested. In the anteroposterior (AP) graph of the case in which the fracture line could not be observed in the oblique graph, unicondylar fractures in the proximal section of the second and third proximal phalanges have been observed. Surgical treatment was not planned because 10days had passed since the trauma. In the control graphs taken after 3 weeks, healing at the fracture site was observed, and he was referred to physiotherapy after the removal of the splint. A good result was observed after physiotherapy. DISCUSSION: Because of the extension of tendons is important over the bone, the goal of the treatment is not only to heal the fracture, but also to preserve the sliding mechanism of these tendons. Regarding to lack of soft tissue trauma, favored joint movements after the healing of the fracture can be achieved more easily with conservative treatment; however, the fracture must be closely followed up. CONCLUSION: Condylar fractures of proximal phalanges those nondisplaced can be conservatively treated with closed methods.

ABSTRACT

O cisto ósseo aneurismático é uma lesão benigna bastante comum caracterizada por ser expansiva, radioluzente, multiloculada e excêntrica, podendo acometer qualquer osso. O presente artigo relata o caso de uma criança com dor e aumento do volume no quarto dedo da mão direita há três semanas, decorrente de um cisto ósseo aneurismático. Os autores destacam as características de imagens na radiografia, tomografia computadorizada e ressonância magnética, com posteriorcorrelação cirúrgica.


Aneurysmal bone cyst is a common benign lesion characterized by its insuflative, radiolucent, excentric and multiloculated pattern. It also can occur in any kind of bone. The present article report case of a child with pain and swelling in the fourth right finger forthree weeks caused by an aneurysmal bone cyst. The authors highlightthe characteristics of images in radiography, computed tomographyand magnetic resonance imaging, with subsequent surgical correlation.
