ABSTRACT

Background: Hand hygiene is recognized as the leading measure to prevent the cross-transmission of microorganisms and to reduce the incidence of healthcare-associated infections. Objectives: To assess the knowledge and practice of hygiene among dental health workers even in the wake of Lassa fever. Method: This questionnaire-based descriptive cross-sectional study was carried out in four public hospitals in Edo state. All data were collected using a self-administered structured questionnaire after obtaining ethical clearance. Results: 108 of the 120 questionnaires distributed were filled and returned giving a response rate of 90.0%. Overall assessment of respondents' knowledge of hand hygiene revealed that 41.7% of the respondents possessed a good knowledge of hand hygiene, 37.0% had excellent knowledge, 17.6% had moderate and 3.7% had a weak knowledge of hand hygiene. Overall assessment of the practice of hand hygiene showed that the hand hygiene practice of 58.3% of respondents was fair while 37.0% practiced hand hygiene poorly. Only a few (5.6%) respondents had good hand hygiene practices. Conclusion: The outbreak of Lassa fever does not seem to positively influence the practice of hand hygiene among the respondents. While knowledge of hand hygiene is satisfactory in this study, the practice still leaves much to be desired.

ABSTRACT

By the end of 2020, a total of 34 multifunctional Assistive Technology Resources Centers (ATR centers) were set up in 22 counties and cities in Taiwan. This study examines the perceptions of the users and their caregivers of the government-established ATR centers in Taiwan and examines the impact on the reputation of the public institution. The research framework and hypotheses were developed by examining the factors of "service convenience", "center-related factors", "justice", and "perceived value" and using "perceived value" as a mediating variable. The data were collected through a questionnaire survey, and the structural equation model was used to test the model and verify the hypotheses. Research data was collected in various townships in Yunlin County A total of 320 questionnaires were collected. Of these respondents, 22% were aged 51-60. All the research hypotheses were positively and significantly verified. Of these, justice was the most important factor affecting the value of the ATR center's services compared to convenience and center-related factors. Of convenience, service value and justice, service value was the most important factor affecting the perceived reputation of the public institution. According to the findings of this study, it is beyond expectation that the convenience of ATR is not the main factor influencing the service value, but rather the perceived justice is the most important factor. Therefore, ATR should be prioritized from the perspective of the service recipient, especially the perceived justice of the service, in order to best enhance the value of the service and improve the reputation of the public institution.

ABSTRACT

Este trabalho teve como objetivo avaliar o controle metabólico e a prevalência de complicações em pacientes com diagnóstico de diabetes mellitus tipo 1 (DM1) atendidos em uma clínica privada e em um ambulatório público em Joinville ­ SC. Realizou-se um estudo descritivo e retrospectivo, onde foram analisados 64 prontuários de pacientes com diagnóstico de DM1, sendo 25 da rede pública (RP) e 39 da privada (RPV). Observamos que na RP a idade média ao diagnóstico do DM1 foi 16,2 (±7,63) contra 12,8 (±9,80) anos na RPV, que foram realizadas em média 1,9 (±1,2) consultas nos últimos 12 meses na RP e 3,0 (±1,3) na RPV. Em relação à presença de complicações microvasculares, havia informação da presença de retinopatia em 8 de 48 pacientes (16,7%), de nefropatia em 12 de 54 pacientes (22,2%) e neuropatia em 8 de 50 pacientes (16%). Quanto ao controle metabólico, apenas 2 pacientes (8%) de RP e 10 (25%) da RPV apresentavam HbA1c < 7,0%. A maioria dos pacientes, tanto da RP quanto da RPV, atingiu a meta de colesterol total (78,3 e 86,8%), que define bom controle clínico e metabólico do diabetes tipo 1, segundo a Associação Americana de Diabetes (ADA). Os dados mostram a dificuldade de se atingir um bom controle glicêmico, independente do local de atendimento (público ou privado), embora o grupo RPV tenha apresentado um maior percentual de pacientes que atingiram as metas de bom controle clínico e metabólico do diabetes tipo 1 definidas pela ADA, inclusive menor índice de tabagismo e etilismo.


This study aims to evaluate the prevalence of complications and metabolic control in patients diagnosed with Type 1 Diabetes Mellitus (T1D) treated in a private clinic and a public outpatient clinic in Joinville - Santa Catarina. A descriptive and retrospective study was carried out, where 64 medical records of patients diagnosed with T1D were analyzed, 25 of which were treated in the public outpatient clinic and 39 in the private clinic. Mean age at diagnosis of T1D in the public institution was 16.2 (± 7.63) years versus 12.8 (± 9.80) years in the private institution, patients had 1.92 (± 1.22) medical appointments in the last 12 months in the public institution and 3 (± 1.32) in the private institution. We assessed the presence of microvascular complications, there was data about retinopathy in 8 of 48 patients (16.7%), nephropathy in 12 of 54 patients (22.2%) and neuropathy in 8 of 50 patients (16%). Most patients did not reach a good glycemic control: only 2 patients (8%) from public institution and 10 (25%) from private institution showed HbA1c < 7.0%. The majority of patients from public and private institution reached the American Diabetes Association (ADA) goals for total cholesterol (78.3 and 86.8%). This data confirms the difficulty of achieving good glycemic control regardless of the institution (public or private), although the private group has shown best results in clinical control with a higher proportion in patients that reached the ADA goals, including the lowest number of smokers and alcohol use.

BACKGROUND: Substance abuse is causing increasing threats to the stability of young minds, from teenage to the older youths and is an issue of public health concern in Nigeria. This study was carried out to determine the prevalence of drug abuse among students of tertiary institutions in Ekiti State. METHODOLOGY: A comparative cross-sectional survey was carried out among students in tertiary institutions in Ekiti state with participants selected from Year 1 to Year 4. Data collection tool was the WHO questionnaire (STASSIS). Data was analysed to obtain descriptive and inferential data, Kruskal-Wallis test and chi square were used for analysis of variance and test of association. RESULT: The majority of the students reported low use of all the drugs (64-90.1%) though moderate to high use were reported for: alcohol 35.1%, marijuana 15.7%, heroine 15.3%, tobacco 13.8% and cocaine 10.2%. There was a significant association between student's moderate to high use and university type. Students in private universities reported more use, especially heroine. Difference in substance use across the four years surveyed was significant with final year students (400 level) showing highest use. CONCLUSION: Tertiary students in Ekiti state are low users of psychoactive substances with a prevalence of 60-91%. Moderate to high prevalence of 10-31% was reported with alcohol as the substance with highest usage. Substance use varied significantly with university type with highest prevalence in the private institution and students at different academic levels with final year students being the highest users.

To verify the feasibility of key performance indicators (KPI) evaluating model of public institution based on the analyzed results of KPI of Institute of Chinese Materia Medica of China Academy of Chinese Medical Sciences. Establishing KPI evaluating model according to scientific research characteristics of public institution. Indicators of flexibility can guarantee the dynamic adjustment with strategic goals of public institutions, and the accurancy and convenience of data collection. The KPI model of Institute of Chinese Materia Medica of China Academy of Chinese Medical Sciences has showed the justic and effecient for the work performance to motivate the staff. The KPI evaluating model has showed the good operability.

ABSTRACT

O artigo versa sobre as relações entre trabalho, saúde do trabalhador e movimentos sociais em instâncias de controle social e o Serviço Social nessas relações, após 1993 no Brasil. Apresenta novas políticas públicas nesse campo e mediações que determinaram infl exões e retrocessos no controle social democrático, estimulando a cultura autoritária patrimonialista em suas instâncias e em um serviço público de saúde pesquisado. Conclui sobre a necessidade de assessoria democrática participativa entre intelectuais, técnicos e população, exemplificada em práticas de Serviço Social, com apresentação de suas bases teóricas e procedimentos operacionais.


The article is about the relation among work, workers' health and social movements in social control jurisdiction, and Social Services in these relations in Brazil after 1993. It presents new public policies in this field and mediations which have led to setbacks and variations in democratic social control, stimulating the authoritarian wealthy culture in their bodies and in a public health service searched. The article presents the conclusion that it is necessary participatory democratic advice among intellectuals, technicians and population, as exemplified in the practices of Social Services, with the presentation of their theoretical grounds and operating procedures.

Este trabalho propõe uma reflexão acerca de alguns efeitos do discurso burocrático num funcionamento institucional na clínica da saúde mental. Trata-se de um exercício que exige muitos caminhos a percorrer. Contudo, tentamos indicar alguns pontos deste tema de maior pertinência, uma vez que um pressuposto fundamental da ética psicanalítica é a indissociabilidade entre clínica e política. Para a sustentação deste debate, acompanharemos os estudos de Arendt (1979; 1999; 2006) e as reflexões de Lacan (1998a; 1998b;1991), pois possibilitam inferir que determinadas formas de funcionamento institucional podem conter dispositivos de perversidade, estando em causa o encobrimento do sujeito e estratégias de dominação do Outro, e permitem fazer uma aproximação entre o discurso burocrático e uma modalidade de perversidade. Quando os dispositivos institucionais são transformados em normas rígidas, prevalecendo a impessoalidade e a desautorização da construção de um saber, o paciente as obedecendo subservientemente, se encontra numa posição autônomata, cronificando seu padecimento. Uma das direções clínicas para estas formas de funcionamento pode ser o trabalho com os dejetos institucionais. Ou seja, parece ser um recurso clínico fundamental a escuta daquilo que retorna como efeito de um discurso automatizado, circunscrito na impessoalidade, podendo, assim, indicar palavras que possam estabelecer uma dimensão subjetiva.(AU)


This work considers to reflect concerning some effect on the bureaucratic speech in an institucional functioning in the clinic of the mental health. It is about a reflection that demands many ways to pass. However, we will try to indicate some points of this theme of greater relevancy, as soon as the main assumption of the psychoanalysis ethics is the individible between clinic and politics. For sustentation of this debate, we will follow the studies of Arendt (1979; 1999; 006) the presented reflections of Lacan (1998a; 1998b; 1991), make possible to infer that some forms of institutional functioning they can contain perversity devices, being in cause the covering of the personal and strategies of domination of the Other, as support for an approach between the bureaucratic speech and a modality of perversity. When the institucional arrangements are transformed into rigid rules, personality and prevailing of the construction of knowledge, the patient obeying subserviently comes into an automata position, chronical his suffering. One of the directions for these clinical forms of operation may be working with institutional waste. That seems to be a key clinical feature to listen to what the of returns of an automated speech, confined in impersonal and may thus indicate words that can make a subjective dimension.(AU)
