ABSTRACT

Low-energy vertebral fractures pose a diagnostic challenge for the radiologist due to their often-inadvertent nature and often subtle imaging semiology. However, the diagnosis of this type of fractures can be decisive, not only because it allows targeted treatment to prevent complications, but also because of the possibility of diagnosing systemic pathologies such as osteoporosis or metastatic disease. Pharmacological treatment in the first case has been shown to prevent the development of other fractures and complications, while percutaneous treatments and various oncological therapies can be an alternative in the second case. Therefore, it is necessary to know the epidemiology and typical imaging findings of this type of fractures. The objective of this work is to review the imaging diagnosis of low-energy fractures, with special emphasis on the characteristics that should be outlined in the radiological report to guide a specific diagnosis that favours and optimizes the treatment of patients suffering of low energy fractures.

ABSTRACT

Las fracturas vertebrales de baja energía suponen un reto diagnóstico para el radiólogo debido a su naturaleza, frecuentemente inadvertida, y a su semiología en imagen, a menudo sutil. Sin embargo, el diagnóstico de este tipo de fracturas puede resultar determinante, no solo por permitir realizar un tratamiento dirigido que evite complicaciones, sino también por la posibilidad de diagnosticar patologías sistémicas como la osteoporosis o la enfermedad metastásica. El tratamiento farmacológico en el primer caso ha demostrado evitar el desarrollo de otras fracturas y complicaciones, mientras que los tratamientos percutáneos y las diversas terapias oncológicas pueden ser una alternativa en el segundo caso. Por lo tanto, es preciso conocer la epidemiología y los hallazgos por imagen de este tipo de fracturas. El objetivo de este trabajo es revisar el diagnóstico por imagen de las fracturas de baja energía, con especial énfasis en las características que deben reseñarse en el informe radiológico para orientar a un diagnóstico específico que favorezca y optimice el tratamiento de los pacientes que padecen este tipo de fracturas.(AU)

ABSTRACT

The radiological diagnosis of osteoporotic vertebral fractures (OVFs) is of major importance considering its therapeutic and prognostic implications. Both radiologists and clinicians have the opportunity to diagnose OVFs in daily clinical practice due to the widespread use of spine and chest radiography. However, several studies have reported an under-diagnosis of OVFs, particularly by a lack of consensus on the diagnostic criteria. Therefore, up-to-date knowledge of the most relevant approaches for the diagnosis of OVFs is necessary for many physicians. This article aims to review the most commonly used classification systems in the diagnosis of OVFs based on conventional radiography. We discuss their rationale, advantages and limitations, as well as their utility according to the context. This review will provide a concise yet useful understanding of the typology of OVFs, their clinical significance and prognosis. Finally, we include anatomical variations that can be confused with OVFs by non-experts.

ABSTRACT Objective: to investigate the frequency of recurrent lumbar disk herniation (rLDH) and evaluated risk factors of rLDH in Russian population. Methods: this was a retrospective clinical study. From January 2015 to March 2022, 218 patients having single-level LDH at three institutes were included in this clinical study and who were then observed for a minimum of 5 years postoperatively. All the patients or relatives gave informed consent to participate in this study. The levels of disk herniation were L4-L5 in 132 cases (60.5 %), and L5-S1 in 86 cases (39.4 %). Results: The rLDH group was composed of 31 male and 12 female, whose ages ranged from 18 to 57 years (34.8±9.5 years). The non-rLDH group was composed of 97 male and 78 female, whose ages ranged from 19 to 73 years (47.5±9.8 years). According to the constructed binary logistic model, body mass index (p=0.021), current smoking (p=0.017), stage of disk degeneration (p=0.043), facet tropism (p=0.037), disk height index (p=0.018) and apparent diffusion coefficient (p=0.009) are significantly associated with incidence of rLDH. Conclusions: patients with these risk factors should be paid more attention for prevention of rLDH. Level of Evidence III; Retrospective Study.


RESUMO Objetivo: investigar a frequência de hérnia de disco lombar recorrente (rLDH) e avaliar os fatores de risco de rLDH na população russa. Métodos: este foi um estudo clínico retrospectivo. De janeiro de 2015 a março de 2022, 218 pacientes com LDH de nível único em três institutos foram incluídos neste estudo clínico e que foram observados por um período mínimo de 5 anos no pós-operatório. Todos os pacientes ou familiares deram consentimento informado para participar deste estudo. Os níveis de hérnia de disco foram L4-L5 em 132 casos (60,5%) e L5-S1 em 86 casos (39,4%). Resultados: o grupo rLDH foi composto por 31 homens e 12 mulheres, cujas idades variaram de 18 a 57 anos (34,8±9,5 anos). O grupo não-LDH foi composto por 97 homens e 78 mulheres, cujas idades variaram de 19 a 73 anos (47,5±9,8 anos). De acordo com o modelo logístico binário construído, índice de massa corporal (p=0,021), tabagismo atual (p=0,017), estágio de degeneração do disco (p=0,043), tropismo facetário (p=0,037), índice de altura do disco (p=0,018) e o coeficiente de difusão aparente (p=0,009) estão significativamente associados à incidência de rLDH. Conclusões: pacientes com esses fatores de risco devem receber mais atenção para prevenção de rLDH. Nível de evidência III; Estudo Retrospectivo.

Todos los pacientes o familiares dieron su consentimiento informado para participar en este estudio. Los niveles de hernia discal fueron L4-L5 en 132 casos (60,5%) y L5-S1 en 86 casos (39,4%). Resultados: el grupo rLDH estuvo compuesto por 31 hombres y 12 mujeres, cuyas edades oscilaron entre 18 y 57 años (34,8±9,5 años). El grupo no HDH estaba formado por 97 hombres y 78 mujeres, cuyas edades oscilaban entre 19 y 73 años (47,5±9,8 años). Según el modelo logístico binario construido, índice de masa corporal (p=0,021), tabaquismo actual (p=0,017), estadio de degeneración discal (p=0,043), tropismo facetario (p=0,037), índice de altura del disco (p =0,018) y el coeficiente de difusión aparente (p=0,009) se asocian significativamente con la incidencia de rLDH. Conclusiones: Los pacientes con estos factores de riesgo deberían recibir más atención para prevenir la rLDH. Nivel de evidencia II; Estudio Retrospectivo.

BACKGROUND: Functional endoscopic sinus surgery might lead to dangerous complications. Studying and analysing preoperative CT scans provides surgeons with a precise knowledge of their patient's anatomy, thus reducing the risk of potential complications. Checklists highlighting key anatomical areas have been published and proven useful. However, none of these are widely accepted or systematically used in daily practice. OBJECTIVE: In this paper, the rhinology group of the Young-Otolaryngologists of the International Federations of Otorhinolaryngological Societies (YO-IFOS) aim to create and validate a new checklist designed to be fast and user friendly for daily practice. METHODS: Two CT sinonasal scans were selected as test cases. Forty otolaryngologists were selected from five tertiary referral hospitals. It was a cross-sectional study; each participant was their own control. All participants completed a questionnaire after the analysis of both CT scans to prevent learning bias. The evaluation included ten items critical in endoscopic sinus surgery according to previous publications. RESULTS: There were 80 evaluations. There was a significant increase in the number of correctly identified critical structures with the use of the checklist (p=.009). There was a statistically significant difference in low- experience evaluators, while it was not statistically significant for experienced surgeons. The most unanswered structures were suprabullar recess, dangerous v2 nerve, anterior ethmoid artery, dangerous vidian nerve and Onodi cell. The most wrongly identified structures were Keros type, septal deviation and cribiform middle turbinate. CONCLUSION: The YO-IFOS radiological checklist has proven a useful tool for correctly studying sinonasal anatomical variations. There is a clear learning component in the use of the checklist although it does not in any way exempt specialists from thorough study of sinonasal anatomy. Given the risk-benefit ratio, we strongly suggest the routine use of the checklist to systematically assess CT-scans prior to endoscopic sinonasal surgery.

Objetivo: La cirugía endoscópica nasosinusal (CENS) tiene complicaciones peligrosas. El estudio peroperatorio de la tomografía nasosinusal otorga un conocimiento preciso de la anatomía del paciente, reduciendo así el riesgo de complicaciones. Se han publicado listas de comprobación para cirugía nasosinusal, y estas han demostrado su utilidad. En este trabajo, desde el grupo de rinología de Young-Otolaryngologists of the International Federations of Oto-rhino-laryngological Societies (YO-IFOS), se diseñó y validó una lista de comprobación con el objetivo de ser amplia, cómoda y práctica para uso en la práctica clínica diaria.MétodosSe seleccionaron 2 tomografías nasosinusales como caso problema. Cuarenta otorrinolaringólogos fueron reclutados de 5 centros de tercer nivel. Se diseñó un estudio cruzado, por lo que cada participante fue su propio control. Todos los participantes completaron una evaluación tras el análisis de ambos casos con el objetivo de evitar sesgo de aprendizaje.ResultadosSe completó un total de 80 evaluaciones. El uso del checklist supuso una mejoría en el número de variantes peligrosas identificadas (p=0,009). La diferencia fue estadísticamente significativa para evaluadores poco experimentados, pero no para los experimentados. Las variantes con mayor número de respuestas en blanco fueron el receso suprabullar, el nervio V2, la arteria etmoidal anterior, el nervio vidiano y la celda de Onodi. Las estructuras con mayor número de error fueron Keros, desvío septal e inserción del cornete medio en lámina cribosa. (AU)

ABSTRACT

Liver radioembolization is an emerging treatment against liver primary and secondary tumours. The whole procedure of radioembolization involves different health care specialists with different expertise. During the fractionation and infusion phases, the personnel manipulates high activities of 90Y. In our centre, the number of radioembolization treatments per year is increasing; the aim of this study is to monitor the dose to the operators and to estimate the radiological risk for the operators involved in the RE. At present, two medical devices are approved: Sir-Sphere® and Therasphere™, both loaded with 90Y. The dosimeters used were TLDs placed over the fingertips, for a total of 4 dosimeters for each phase; the selected dose descriptor was Hp0.07. The study concerned 17 patients affected by malignant hepatic lesions, treated from September 2017 to March 2018. We performed 27 procedures: 10 fractionations (with Sir-Sphere®) and 17 infusions to the patients (10 with Sir-Spheres®, 7 with Theraspheres™). For fractionation phase, the average activity of each preparation was 3.34 GBq, the average value of Hp0.07 was 0.50mSv. For infusion phase, the average activity was 1.51 GBq for Sir-Sphere® and 2.10 GBq for Theraspheres™, the average value of Hp0.07 was 0.10mSv. No significant differences were found between senior (Hp0.07 = 0.08mSv) and young operators (Hp0.07 = 0.09mSv), respectively. Similarly, no significant differences were found between the right and left hand, with the same average value of Hp0.07 (0.01mSv). In conclusion, the results are encouraging, since fingertips reported doses very low. The handling of 90Y microspheres and the radioembolization procedure can be carried out under safe conditions.

ABSTRACT

Most of the patients who overcome the SARS-CoV-2 infection do not present complications and do not require a specific follow-up, but a significant proportion (especially those with moderate / severe clinical forms of the disease) require clinicalradiological follow-up. Although there are hardly any references or clinical guidelines regarding the long-term follow-up of post-COVID-19 patients, radiological exams are being performed and monographic surveillance consultations are being set up in most of the hospitals to meet their needs. The purpose of this work is to share our experience in the management of the post-COVID-19 patient in two institutions thathave had a high incidence of COVID-19 and to propose general follow-uprecommendations from a clinical and radiological perspective.

INTRODUCTION: Real-time reverse-transcription polymerase chain reaction (RT-PCR) is the gold standard for establishing the diagnosis of coronavirus disease 2019 (COVID-19). Chest computed tomography (CCT), as a diagnostic complement, classifies tomographic findings according to the COVID-19 Reporting and Data System (CO-RADS). OBJECTIVE: To determine CCT sensitivity and specificity for COVID-19 diagnosis. METHODS: We reviewed RT-PCR results, as well as their respective CCTs. All CCTs were classified according to CO-RADS. CCT sensitivity and specificity were evaluated using the RT-PCR result that was closest to that of CCT as reference. RESULTS: We included 412 patients, out of whom 277 were males (46-70 years) and 130 were females (45-71 years); with 181 negative and 231 positive tests, a sensitivity of 92.15% and specificity of 79.32% were obtained. Mortality increased after six hospitalization days, in males and in CO-RADS 4, 5 and 6 in comparison with CO-RADS 1, 2 and 3. CONCLUSIONS: Early diagnosis plays a decisive role in the prognosis of SARS-CoV-2-associated pneumonia. Although RT-PCR is current gold standard, false negatives are common; for this reason, CCT helps to confirm suspected cases, even at early stages. This imaging technique is an accessible and fundamental study for classification, diagnosis and prognosis.


INTRODUCCIÓN: La reacción en cadena de la polimerasa con transcripción inversa en tiempo real (RT-PCR) es el estándar de oro para establecer el diagnóstico de enfermedad por coronavirus 2019 (COVID-19). La tomografía computarizada de tórax (TCT), como complemento diagnóstico, clasifica los hallazgos tomográficos de acuerdo con el sistema CO-RADS (COVID-19 Reporting and Data System). OBJETIVO: Determinar la sensibilidad y especificidad de la TCT para el diagnóstico de COVID-19. MÉTODOS: Consultamos los resultados de RT-PCR, así como sus respectivas TCT. Todas las TCT se clasificaron de según CO-RADS. Se evaluó sensibilidad y especificidad de la TCT utilizando el resultado de RT-PCR más cercano de TCT como referencia. RESULTADOS: Incluimos 412 pacientes, incluyendo 277 hombres (46-70 años) y 130 mujeres (45-71 años), con 181 pruebas negativas y 231 positivas; obteniendo sensibilidad del 92.15% y especificidad del 79.32%. La mortalidad aumentó después de seis días de hospitalización, en el sexo masculino y en CO-RADS 4, 5 y 6 en comparación con CO-RADS 1, 2 y 3. CONCLUSIONES: El diagnóstico temprano es decisivo en el pronóstico de la neumonía SARS CoV 2. Aunque la RT-PCR es el estándar de oro actual, los falsos negativos son frecuentes, por lo que la TCT ayuda a confirmar los casos sospechosos, incluso en etapas tempranas. Este estudio de imagen es un estudio accesible y fundamental para la clasificación, diagnóstico y pronóstico.

ABSTRACT

The goal of this study was to describe the gastrointestinal transit technique in the Boa constrictor amarali. For that purpose, we obtained simple radiographic images of seven serpents, subsequently administering a 25mL/kg dose of barium sulfate and establishing a radiographic sequence at the following times: 5 minutes; 1, 2, 3, 6, 9, 24, 48, 72 and 96 hours, extending to 120 and 126 hours for one animal. The mean esophageal transit was 26.71±19.48 hours; the mean gastric filling time was 28.57±27.22 minutes and the emptying time was 60±12 hours; the mean filling time of the contrast medium in the small intestine was 3±2.16 hours and the emptying time was 97±15.65 hours. We also obtained the mean filling time of the large intestine, which was 40±11.31 hours. We found that the mean passage time of the contrast medium through the cranial gastrointestinal tract - until the complete elimination of barium sulfate from the small intestine -was 97±15.65 hours. In addition to determining the gastrointestinal transit time, the technique used allowed for the morphological identification of the alimentary canal.(AU)


O objetivo deste trabalho foi descrever a técnica de trânsito gastrointestinal em Boa constrictor amarali. Para tanto, foram obtidas radiografias simples de sete serpentes, e, subsequentemente a essas, foi administrado sulfato de bário na dose de 25mL/kg. A partir disso, foi estabelecida a sequência radiográfica nos seguintes tempos: cinco minutos; uma, duas, três, seis, nove, 24, 48, 72 e 96 horas, e em um animal estendeu-se para 120 e 126 horas. O trânsito esofágico médio foi de 26,71±19,48 horas, o tempo médio de preenchimento gástrico foi de 28,57±27,22 minutos, e o esvaziamento de 60±12 horas; a média de tempo de enchimento do meio de contraste no intestino delgado foi de 3±2,16 horas e, por fim, o esvaziamento foi de 97±15,65 horas, e ainda obteve-se o tempo médio de enchimento do intestino grosso igual a 40±11,31 horas. Desse modo, verificou-se que a média do tempo de passagem do contraste pelo trato gastrointestinal cranial - até a completa eliminação do sulfato de bário do intestino delgado - foi de 97±15,65 horas. Além da determinação do tempo de trânsito gastrointestinal, a técnica empregada permitiu a identificação morfológica do canal alimentar.(AU)

Resumen: Antecedentes: Hallux valgus (HV) es una desviación en valgo de la articulación metatarsofalángica y en varo del primer metatarsiano. La cirugía pretende reestablecer la congruencia articular, corregir la deformidad y disminuir el dolor. Existe poca literatura de los resultados en población geriátrica. El propósito de este estudio es demostrar que la técnica quirúrgica abierta de Lelièvre y Vidalot logra resultados radiográficos aceptables en dicha población. Material y métodos: 68 pacientes entre 60 y 85 años intervenidos con técnica Lelièvre y Vidalot por HV moderado a severo de Isham, entre Enero 2014 y Enero 2017. Se evaluaron las radiografías preoperatoria, postoperatoria inmediata y a los seis meses, con radiometría convencional para esta afección; todas las mediciones se realizaron en sistema digital. Los datos se ingresaron en Microsoft Excel. Los valores de p < 0.05 se consideraron estadísticamente significativos. Resultados: De 68 pacientes (54 mujeres y 14 hombres), edad media 68.0 años, 12 presentaron afectación bilateral; 28 segundo dedo en garra. El ángulo metatarsofalángico con una media preoperatoria fue 36.6o, incrementado en 100% de los casos de ángulo intermetatarsal 1-2, existiendo una correlación p < 0.05 entre ángulo intermetatarsal y aumento del ángulo metatarsofalángico. Los resultados postquirúrgicos inmediato y a seis meses presentaron una correlación p = 0.3107. Conclusiones: El tratamiento de HV moderado a severo en nuestra población geriátrica bajo técnica Lelièvre y Vidalot logra una corrección aceptable demostrada radiográficamente en el postoperatorio inmediato y a los seis meses, esto traduce una opción viable de tratamiento quirúrgico en dicha población.


Abstract: Background: Hallux valgus (HV) defines a valgo deviation from the metatarsophalangeal joint and in a varo of the first metatarsal. The surgery aims to restore joint congruence, correct deformity and decrease pain. There is little literature on the results in the geriatric population. The purpose of this study is intended to demonstrate that the open surgical technique of Lelièvre and Vidalot achieves acceptable radiographic results in that population. Material and methods: 68 patients between 60 and 85 years of age were involved with Lelièvre Technique and Vidalot for moderate to severe HV of Isham, between January 2014 and January 2017. Immediate postoperative and six month radiographs were evaluated with conventional radiometry for this condition; All measurements were made in digital system. The data was entered in Microsoft Excel. p values < 0.05 were considered statistically significant. Results: Out of 68 patients (54 women and 14 men), average age 68.0 years. 12 had bilateral concern. 28 second finger claw. The metatarsphasopalangic angle with a preoperative mean was 36.6o, increased by 100% of cases of intermetatarsal angle 1-2, with a correlation p < 0.05 between intermetatarsal angle and metatarsal angle increase. Immediate and six-month post-surgical results with a correlation of p = 0.3107. Conclusions: The treatment of moderate to severe HV in our geriatric population under technique Lelièvre and Vidalot achieves an acceptable correction demonstrated radiographically in the immediate postoperative period and at six month, this translates a viable option of surgical treatment in these population.

ABSTRACT

Introducción: La aplicación del método de la matriz de riesgo para la evaluación del riesgo radiológico en la medicina permite identificar de manera proactiva debilidades en las etapas del proceso y hacer un plan de acciones de mejora para la seguridad y calidad. Objetivo: Evaluar los riesgos radiológicos de la radiosinoviortesis y el tratamiento mielosupresor con Fósforo-32 de la policitemia vera. Método: Se utilizó el método de matriz de riesgo y se realizó el análisis y tratamiento de los riesgos radiológicos por medio del código cubano SECURE-MR-FMEA 3.0. Resultados: El 17 por ciento del riesgo alto se eliminó con las medidas adicionales adoptadas; predominaron las consecuencias medias para los trabajadores y el público, 30 por ciento y el 14 por ciento, respectivamente. Las defensas más importantes fueron: levantamiento radiológico inicial de las áreas del departamento; revisión independiente del proyecto con las regulaciones de seguridad aplicables; inspección de trabajos de construcción civil y montaje de equipos antes de iniciar la operación del departamento; capacitación de los médicos nucleares en los tratamientos; existencia de protocolos de tratamiento; análisis de lecciones aprendidas de incidentes radiológicos; levantamiento radiológico periódico de las áreas del servicio y procedimiento de emergencia para reducir la dosis en órganos críticos en caso de administración errónea de radiofármacos. Se creó una base de datos de incidentes utilizada como referencia para el modelo utilizado. El factor humano fue la causa mayor de los sucesos radiológicos analizados (88 por ciento). Conclusiones: Estos resultados facilitan la toma de decisiones para el mejor desempeño de la radiosinoviortesis y el tratamiento de la policitemia vera con Fósforo-32 en Cuba. Se sugiere elaborar el plan de mejora de la seguridad con especial atención a las operaciones de administración del radiofármaco en ambos casos.(AU)


Introduction: The application of risk matrix for ionizing radiation medicine allow identify in proactive way the weakness of the process' step, which implies in the design of safety and quality improvement plan for this. Method: Risk matrix method applied for radiosynoviorthesis and the myelosupressor treatment with Phosphorus-32 of polycythemia vera. The Cuban code SECURE-MR-FMEA 3.0 is used. Results: It was eliminated the 17 percent of the high risk with additional measures, and the medium consequences for workers and public are 30 percent and 14 percent, respectively. The most important identified safety measures were the initial radiological monitoring from different nuclear medicine department areas; the project revision based on the applicable safety regulations; a survey of civil construction works and equipment assembly before work began; training of nuclear medicine doctors in related aspects of nuclear medicine treatments; existence of treatment protocols; the analysis of learned lessons from radiological incidents; the periodical radiological monitoring from different services areas and the emergency procedure for the cases of mistake in the radiopharmaceuticals administration. Human factor was the major cause in analyzed radiological events (88 percent). Conclusions: These results facilitate taking decisions for the best performance of radiosynoviorthesis and the myelosupressor treatment with Phosphorus-32 of polycythemia vera in Cuba. It is recommended to elaborate the safety improvement plan from these and focussing in the radiopharmaceutical administration operations in both cases.(AU)

It is presented an adult patient treated during the childhood in the Pediatric Hospital ¨Eliseo Noel Caamaño¨, of Matanzas, with the association of both entities, to observe his current anatomic, functional and radiological status. The aim was studying the patient´s clinical radiological evolution through the time. Abstract Objective: Community-acquired pneumonia is an important cause of morbidity in childhood, but the detection of its causative agent remains a diagnostic challenge. The authors aimed to evaluate the role of the chest radiograph to identify cases of community-aquired pneumonia caused by typical bacteria. Methods: The frequency of infection by Streptococcus pneumoniae, Haemophilus influenzae, and Moraxella catarrhalis was compared in non-hospitalized children with clinical diagnosis of community acquired pneumonia aged 2-59 months with or without radiological confirmation (n = 249 and 366, respectively). Infection by S. pneumoniae was diagnosed by the detection of a serological response against at least one of eight pneumococcal proteins (defined as an increase ≥2-fold in the IgG levels against Ply, CbpA, PspA1 and PspA2, PhtD, StkP-C, and PcsB-N, or an increase ≥1.5-fold against PcpA). Infection by H. influenzae and M. catarrhalis was defined as an increase ≥2-fold on the levels of microbe-specific IgG. Results: Children with radiologically confirmed pneumonia had higher rates of infection by S. pneumoniae. The presence of pneumococcal infection increased the odds of having radiologically confirmed pneumonia by 2.8 times (95% CI: 1.8-4.3). The negative predictive value of the normal chest radiograph for infection by S. pneumoniae was 86.3% (95% CI: 82.4-89.7%). There was no difference on the rates of infection by H. influenzae and M. catarrhalis between children with community-acquired pneumonia with and without radiological confirmation. Conclusions: Among children with clinical diagnosis of community-acquired pneumonia submitted to chest radiograph, those with radiologically confirmed pneumonia present a higher rate of infection by S. pneumoniae when compared with those with a normal chest radiograph.


Resumo Objetivo: Avaliar o papel do raios X de tórax na identificação de casos de pneumonia adquirida na comunidade (PAC) causada por agentes bacterianos. Métodos: A frequência de infecção por Streptococcus pneumoniae, Haemophilus influenzae e Moraxella catarrhalis em crianças com PAC não hospitalizadas foi comparada com a presença de confirmação radiológica da pneumonia (n = 249 crianças com pneumonia radiologicamente confirmada e 366 crianças com raios X de tórax normal). Infecção por S. pneumoniae foi diagnosticada com base na resposta sorológica a pelo menos uma dentre oito proteínas pneumocócicas investigadas (aumento ≥ 2 vezes nos níveis de IgG em relação a Ply, CbpA, PspA1 e 2, PhtD, StkP-C e PcsB-N ou aumento≥ 1,5 vez em relação aPcpA). Infecção por H. influenzae e M. catarrhalis foi definida por aumento ≥ 2 vezes nos níveis de IgG específica a antígenos de cada agente. Resultados: Crianças com pneumonia radiologicamente confirmada apresentaram maior taxa de infecção pelo pneumococo. Além disso, a presença de infecção pneumocócica foi um fator preditor de pneumonia radiologicamente confirmada, o que aumenta sua chance de detecção em 2,8 vezes (IC 95%: 1,8-4,3). O valor preditivo negativo do raios X normal para a infecção por S. pneumoniae foi 86,3% (IC95%: 82,4%-89,7%). Não houve diferença nas frequências de infecção por H. influenzae e M. catarrhalis entre crianças com PAC com ou sem confirmação radiológica. Conclusão: Crianças com diagnóstico clínico de PAC submetidas a um raios X de tórax que apresentam confirmação radiológica têm maior taxa de infecção por S. pneumoniae comparadas com as crianças com raios X normal.

ABSTRACT

A nefropatia induzida por contraste (NIC) é uma doença de caráter agudo, secundária à administração intravascular de meios de contraste iodado (MCI). Dentre os mecanismos fisiopatológicos desta enfermidade destacam-se a vasoconstrição intrarrenal prolongada, consequente redução da perfusão renal, hipóxia e isquemia medulares, associada ao dano tubular renal devido à citotoxicidade do contraste. Frente à existência de poucas informações relacionadas a estes mecanismos na literatura médico-veterinária, objetivaram-se comparar os efeitos renais da administração intravenosa de MCI não iônicos de diferentes osmolaridades, em grupos de cães com fatores de risco para o desenvolvimento da NIC, por meio das avaliações ultrassonográficas modo B, Doppler colorido, de amplitude e pulsado, pareada aos exames laboratoriais, a fim de estimar indiretamente o potencial nefrotóxico de cada contraste. Constituíram-se dois grupos de acordo com o MCI utilizado: o grupo GIH [11 cães receberam iohexol (baixa osmolaridade)] e o grupo GID [sete cães receberam iodixanol (isosmolar)]. Administrou-se a dose de 600mgI/kg/IV em ambos. Avaliaram-se os seguintes aspectos renais antes da administração do MCI (momento basal) e após 1h30min, 24 horas e 48 horas: morfometria (comprimento e volume), morfologia, ecogenicidade cortical e perfusão renais e resistência vascular intrarrenal (índices hemodinâmicos de resistividade e pulsatilidade). Realizou-se ainda exame de urina e se mensuraram as razões gama-glutamil transferase:creatinina (GGT:C) e proteína:creatinina (RPC) urinárias e a concentração sérica de creatinina. Os grupos apresentaram comportamentos similares para comprimento, volume, RPC, exame de urina e creatinina sérica. Em relação ao índice de pulsatilidade (IP), os grupos apresentaram comportamentos não similares, mas sem diferenças significantes entre o momento basal e os demais. Para o índice de resistividade (IR) e a razão GGT:C urinária, os grupos revelaram comportamentos não similares e se constataram aumentos significantes do IR e da razão GGT:C urinária no período de 1h30min após a administração do contraste, somente para o grupo que recebeu iohexol. Concluiu-se que o IR pode ser utilizado para monitorar a hemodinâmica intrarrenal, visto que junto com a razão GGT:C urinária, demonstrou a existência de maior potencial nefrotóxico do iohexol, quando comparado ao iodixanol. Dessa forma, considera-se o uso do iodixanol, opção favorável para cães com fatores de risco para o desenvolvimento da NIC.(AU)


Contrast-induced nephropathy (CIN) is an acute disease, secondary to intravascular administration of iodinated contrast media (ICM). The most important mechanisms of this nephropathy are intrarenal prolonged vasoconstriction, medular hypoxia, and ischemia associated with renal tubular damage due to contrast cytotoxicity. Owing to the limited information available in veterinary literature regarding these mechanisms this study aims to compare the renal effects of intravenous administration of two nonionic ICM of different osmolarities in groups of dogs with risk factors for CIN development, by using a B-mode, color, power- and pulsed-wave Doppler ultrasonography, and other laboratory tests, in order to indirectly estimate the nephrotoxic potential of each contrast. The following two groups were established according to the nonionic ICM used: the GIH group [11 dogs administered iohexol (low osmolarity)] and the GID group [seven dogs administered iodixanol (iso-osmolarity)]. Both the groups were administered the same dose (600mgI/kg/IV). The following renal aspects were evaluated before administration of ICM (baseline) and after 1h30min, 24h, and 48h: renal morphometry (length and volume), renal morphology, cortical echogenicity, renal perfusion, and intrarenal vascular resistance (resistive and pulsatility indices); in addition, urinalysis was performed, and urinary gamma-glutamyl transferase:creatinine ratio (GGT:C), urinary protein:creatinine ratio (UPC), and serum creatinine were also measured. Both groups showed similar characteristics with respect to the length, volume, UPC ratio, urinalysis, and serum creatinine levels. No similarity was observed with respect to the pulsatility index (PI) in both the groups and there were no significant differences between baseline and 1h30min, 24h and 48h time points. With respect to the IR and urinary GGT:C, both groups showed no similarity, and significant increases were observed in the resistive index (RI) and urinary GGT:C only in the GIH group, 1h30min after contrast administration. In conclusion, RI can be used to monitor intrarenal hemodynamics, and along with the urinary GGT:C, revealed that iohexol had higher nephrotoxic potential than iodixanol. Thus, iodixanol is considered a favorable option for dogs with risk factors for CIN development.(AU)

Resumen:El síndrome del niño agredido, constituye uno de los casos que se valoran en la Sección Clínica Médico Forense, donde se deben tomar en cuenta elementos objetivos que permitan establecer la relación de causalidad entre los hallazgos evidenciados en las valoraciones con los documentos médicos o estudios radiológicos realizados al menor, ya que en la mayoría de las veces la historia brindada por el adulto a cargo no explica o no es compatible con las lesiones evidenciadas. En el presente artículo se expone un caso en el cual los estudios radiológicos fueron claves para poder catalogar el caso como síndrome de niño agredido; así mismo la importancia y relevancia que tienen la realización de estos estudios.fhernandezar@poder-judicial.go.cr


Abstract:The child syndrome attacked, is one of the cases that are valued at the Forensic Medical Clinic, where you must take into account objective evidence to establish a causal link between the findings evidenced in the ratings with medical documents or radiologic studies made the child, since in most of the time the story provided by the adult in charge does not explain or is not compatible with evidenced injuries. In this article a case in which imaging studies were key to classify the case as exposed assaulted baby syndrome; and the importance and relevance that are conducting these studies.

RESUMOObjetivos:A coorte Sarar é composta por pacientes portadores de artrite reumatoide (AR) e artrite idiopática juvenil (AIJ) submetidos a artroplastias de quadril e joelho no hospital Sarah-Brasília. O objetivo deste estudo foi avaliar fatores clínicos e laboratoriais associados à atividade de doença, capacidade funcional e dano radiológico em pacientes com AR, participantes dessa coorte.Métodos:Estudo transversal, com coleta de dados em revisão de prontuário.Resultados:Foram incluídos 32 pacientes, com tempo médio de início da doença de 240 meses. Dezenove pacientes foram submetidos a ATJ e 17, a ATQ. Foi encontrada correlação positiva entre dose máxima de metotrexato (MTX) durante a evolução e Clinical Disease Activity Index (CDAI) (R = -0,46, p = 0,011) e negativa com Simplified Erosion and Narrowing Score (SENS) (R = -0,58, p = 0,004). Valores de SENS foram maiores nos pacientes com fator reumatoide (FR) (p = 0,005) e anticorpo antipeptídeo cíclico citrulinado 3 (anti-CCP3) positivo (p = 0,044), nos com maiores títulos de FR (p = 0,037) e anti-CCP3 (p = 0,025) e menores nos pacientes com história familiar de AR (p = 0,009). Valores de HAQ foram maiores em pacientes mais idosos (p = 0,031). Na regressão linear múltipla, somente “dose máxima de MTX” e “história familiar” permaneceram com associação significativa com SENS (r2= 0,73, p < 0,001 para ambas as variáveis). No modelo que avaliou CDAI, apenas “dose máxima de MTX” permaneceu com associação significativa (r2 = 0,35, p = 0,016).Conclusão:Na coorte Sarar, fatores clínicos e laboratoriais estiveram relacionados à atividade de doença, capacidade funcional e dano radiológico, semelhantemente a estudos que avaliaram pacientes com menor tempo de doença.

The objective of this study was to evaluate clinical and laboratory factors associated with disease activity, functional capacity and radiological damage in rheumatoid arthritis patients, participants in this cohort.Methods:Cross-secal study, with data collection achieved from medical records review.Results:Thirty-two patients were included, with a mean time of disease onset of 240 months. Nineteen patients underwent total knee and 17 total hip arthroplasty. There was a positive correlation between maximum dose of methotrexate and Clinical Disease Activity Index (R = −0.46, p = 0.011), and a negative one with Simplified Erosion and Narrowing Score (R = −0.58, p= 0.004). Simplified Erosion and Narrowing Score values were higher in patients with rheumatoid factor (p = 0.005) and anti-cyclic citrullinated peptide antibody 3 positivity (p = 0.044), in those with higher rheumatoid factor (p = 0.037) and anti-cyclic citrullinated peptide antibody 3 (p = 0.025) titers, and lower in patients with family history of rheumatoid factor (p = 0.009). Health Assessment Questionnaire values were higher in older patients (p = 0.031). In multiple linear regression, only “maximum dose of methotrexate” and “family history” remained with significant association with Simplified Erosion and Narrowing Score (r2 = 0.73, p < 0.001 for both variables). In the model evaluating “Clinical Disease Activity Index” only “maximum dose of methotrexate” remained significantly associated (r2 = 0.35, p = 0.016).Conclusion:In the Sarar cohort, clinical and laboratory factors were related to disease activity, functional capacity and radiological damage, similar to studies evaluating patients with lower disease duration.

ABSTRACT

Meios de contraste iodado podem promover efeitos hemodinâmicos relacionados à vasoconstrição intrarrenal prolongada e redução da perfusão, predispondo à hipóxia e isquemia medular. Alterações de resistência vascular renal podem representar os primeiros sinais de mudança funcional desse órgão. A técnica Doppler pulsado é considerada acessível, não invasiva e permite avaliar a dinâmica vascular dos rins, por meio da aferição dos índices de resistividade (IR) e pulsatilidade (IP). Contudo, na espécie canina, a aquisição de traçados espectrais pode ser penosa devido às dificuldades de varredura e captação de sinal Doppler, sobretudo em relação ao rim direito, devido à sua localização dorsocranial na cavidade abdominal, o que prolonga substancialmente a realização do exame. O objetivo deste estudo é comprovar que a avaliação Doppler pulsado das artérias intrarrenais do rim esquerdo de cães representa a repercussão hemodinâmica renal da administração intravenosa de meios de contraste iodado não sendo necessária a realização do exame nos dois rins. Foram avaliados ambos os rins de seis cadelas adultas em quatro momentos distintos: antes da infusão intravenosa do contraste radiológico e após 1,5 horas, 24 horas e 48 horas, por meio da análise subjetiva da morfologia, ecogenicidade cortical e grau de perfusão renais e análise objetiva do comprimento, volume e resistência vascular intrarrenais (IR e IP). Os parâmetros avaliados ao modo B e Doppler dos rins direito e esquerdo não apresentaram diferenças estatisticamente significativas entre si em cada momento avaliado. Assim, constatou-se que o exame ultrassonográfico Doppler pulsado do rim esquerdo representou a repercussão hemodinâmica renal da aplicação intravenosa de meios de contraste iodado, desde que morfometria, morfologia, ecogenicidade cortical e perfusão de ambos os rins fossem consideradas semelhantes na abordagem ultrassonográfica inicial...


Iodinated contrast media can provide hemodynamic effects related to prolonged intrarenal vasoconstriction and reduced perfusion, predisposing to hypoxia and medular ischemia. Changes in the renal vascular resistance may represent the first sign of impaired renal function. Pulsed wave Doppler sonography is low cost, non-invasive and allows renal hemodynamic evaluation by resistivity (RI) and pulsatility (PI) indices. However in dogs, spectral Doppler waveform may be hard to obtain due to scanning difficulties and lack of signal Doppler. This fact occurs mostly in the right kidney because of its cranial and dorsal position in the abdominal cavity and may prolong the exam. The purpose of this study is to prove that the pulsed wave Doppler interrogation of the canine left kidney intrarenal arteries represent the renal hemodynamic effects of intravenous administration of iodinated contrast media and is not necessary to perform the exam in both kidneys. Right and left kidneys of six adult dogs were evaluated at four different times: before intravenous contrast infusion and after 1.5 hours, 24 hours and 48 hours, subjectively by using renal morphology, cortical echogenicity and renal perfusion, and objectively by using renal length, renal volume and intrarenal vascular resistance (RI and PI). No significant differences were noted between the parameters assessed by B mode and Doppler sonography of the right and left kidney in each evaluated moment. Thus, it was noted that pulsed wave Doppler sonographic evaluation of the left kidney represented the renal hemodynamic response of intravenous iodinated contrast media administration, only if both kidneys presented similar morphometry, morphology, cortical echogenicity, and perfusion during the first assessment...
