ABSTRACT

Las alteraciones cervicales son un problema multifactorial que afecta a la sociedad moderna. Posturas viciosas, traumatismos y defectos congénitos relacionados con la columna cervical pueden desarrollar inestabilidad, pinzamiento radicular, cervicoartrosis y cervicalgias. Objetivo. Relacionar el uso de dispositivos móviles con las alteraciones cervicales en estudiantes universitarios. Materiales y métodos. Estudio descriptivo, observacional, que se realizó entre los meses de mayo y julio del 2023, cuya muestra fue de 172 estudiantes universitarios que se obtuvo aplicando la fórmula para el cálculo muestral de poblaciones conocidas, mediante un muestreo no probabilístico. Se utilizó el test goniométrico para medir el rango articular, el test postural para identificar las alteraciones posturales, la técnica de palpación para identificar dolor inespecífico, prueba de resistencia para los músculos flexores (NFMET) y extensores (NEET), por último, se realizó la prueba de Spurling para identificar casos de radiculopatías. Resultados. Aunque las relaciones estadísticas no fueron consistentes, se observó que quienes utilizaron más tiempo los teléfonos celulares (87,0%) mostraron más limitaciones cervicales que los usuarios menos frecuentes (73,5%). La movilidad articular fue limitada en el 84,3% de la población, especialmente en varones (93,5%); la resistencia muscular normal en extensión fue más prevalente en el caso de los hombres (84,9%), mientras que la resistencia alterada en flexión fue más prevalente en mujeres (94,9%). Conclusiones. Según los resultados obtenidos en esta investigación, no se encontró suficiente evidencia para determinar una relación estadísticamente significativa (P˃0,05) entre las alteraciones cervicales y el uso de teléfonos celulares, aunque se observó una mayor limitación en el caso de quienes más tiempo utilizaban el dispositivo móvil.


Cervical disorders are a multifactorial problem affecting modern society. Vicious postures, trauma and congenital defects related to the cervical spine can develop instability, radicular impingement, cervicoarthrosis and cervicalgia. Objective. To relate the use of mobile devices with cervical disorders in university students. Materials and methods. Descriptive, observational study carried out between May and July 2023, with a sample of 172 university students obtained by applying the formula for the sample calculation of known populations, by means of non-probabilistic sampling. The goniometric test was used to measure joint range, the postural test to identify postural alterations, the palpation technique to identify non-specific pain, resistance test for flexor (NFMET) and extensor (NEET) muscles, and finally, the Spurling test was performed to identify cases of radiculopathy. Results. Although the statistical relationships were not consistent, it was observed that those who used cell phones longer (87.0%) showed more cervical limitations than less frequent users (73.5%). Joint mobility was limited in 84.3% of the population, especially in men (93.5%); normal muscular endurance in extension was more prevalent in men (84.9%), while impaired endurance in flexion was more prevalent in women (94.9%). Conclusions. According to the results obtained in this investigation, there was not enough evidence to determine a statistically significant relationship (P˃0.05) between cervical alterations and cell phone use, although a greater limitation was observed in the case of those who used the mobile device the longest.


Os distúrbios cervicais são um problema multifatorial que afeta a sociedade moderna. Posturas viciosas, traumas e defeitos congênitos relacionados à coluna cervical podem levar a instabilidade, impacto radicular, cervicoartrose e cervicalgia. Objetivo. Relacionar o uso de dispositivos móveis com distúrbios cervicais em estudantes universitários. Materiais e métodos. Estudo descritivo, observacional, realizado entre maio e julho de 2023, com uma amostra de 172 estudantes universitários obtida pela aplicação da fórmula para o cálculo de amostras de populações conhecidas, por meio de amostragem não probabilística. Foram utilizados o teste goniométrico para medir a amplitude articular, o teste postural para identificar alterações posturais, a técnica de palpação para identificar dores inespecíficas, o teste de resistência para músculos flexores (NFMET) e extensores (NEET) e o teste de Spurling para identificar casos de radiculopatia. Resultados. Embora as relações estatísticas não tenham sido consistentes, observou-se que aqueles que usavam telefones celulares por mais tempo (87,0%) apresentavam mais limitações cervicais do que os usuários menos frequentes (73,5%). A mobilidade articular foi limitada em 84,3% da população, especialmente no sexo masculino (93,5%); a resistência muscular normal em extensão foi mais prevalente no sexo masculino (84,9%), enquanto a resistência prejudicada em flexão foi mais prevalente no sexo feminino (94,9%). Conclusões. De acordo com os resultados obtidos nesta pesquisa, não houve evidências suficientes para determinar uma relação estatisticamente significativa (P˃0,05) entre os distúrbios cervicais e o uso de telefones celulares, embora tenha sido observada uma limitação maior no caso daqueles que usaram o dispositivo móvel por períodos mais longos.

ABSTRACT

Objetivos El objetivo es estudiar la relación entre los parámetros estructurales, de remodelado, de sincronía y de función sistólica del ventrículo izquierdo (VI) mediante gSPECT. Obtener los valores de corte del VI para el índice de la forma telediastólica (iFTD), el índice de la forma telesistólica (iFTS), el índice de excentricidad (iEX) y la velocidad máxima de vaciado (VMV). Desarrollar un nuevo índice para evaluar diferentes patrones de función sistólica del VI. Material y métodos Analizamos prospectivamente 238 pacientes (edad: 63,4±13 años) estudiados mediante gSPECT de esfuerzo-reposo (grupo-control, n=148; pacientes con infarto de miocardio [IM] previo, n=90). Estudio aprobado por el Comité de Ética del Hospital (PR[AG]168.2010). Resultados En el grupo-control, el índice del volumen telediastólico (iVTD) y el iEX influyeron en el iFTD (r2: 0,52, p<0,001). El iVTD, la VMV, ser varones y el iEX (r2: 0,44; p<0,001) influyeron en el iFTS. El iFTD, iFTS, la fracción de eyección VI (FEVI) y el volumen de eyección sistólica (r2: 0,62; p<0,001) influyeron en el iEX. La FEVI, la frecuencia cardíaca, el ancho de banda (AB) y la desviación estándar influyeron (r2: 0,76; p<0,001) en la VMV. Los valores de corte para iFTD, iFTS, iEX y VMV fueron 0,59, 0,42, 0,87 y −3,3 respectivamente. La VMV, el iFTS y el AB fueron los parámetros mejor relacionados con los pacientes con IM previo (AUC: 0,89), y sumados a la FEVI permitieron obtener distintos patrones de función sistólica (índice PERRS). Conclusiones Los parámetros de remodelado, sincronía y función sistólica del VI deben interpretarse simultáneamente, ya que esto permite obtener distintos patrones de función sistólica del VI (AU)

Conclusions The remodeling, synchrony and the systolic function parameters of the LV should be interpreted together (PERRS-index). In this way, we obtain different patterns of LV systolic function (AU)

ABSTRACT

Objetivos: 1) Explorar las principales características de la transición de la unidad de cuidados intensivos de acuerdo a la experiencia vivida de los pacientes y 2) identificar la terapéutica enfermera para facilitar la transición de los pacientes desde la unidad de cuidados intensivos a la unidad de hospitalización. Metodología: Análisis secundario de los hallazgos de un estudio cualitativo descriptivo sobre la experiencia de los pacientes ingresados en una UCI durante la transición a la unidad de hospitalización, en base a la teoría de las transiciones de enfermería. Los datos para el estudio primario se generaron de 48 entrevistas semiestructuradas de pacientes que habían sobrevivido a una enfermedad crítica en 3 hospitales universitarios de tercer nivel. Resultados: Se identificaron 3 temas principales durante la transición de los pacientes de la unidad de cuidados intensivos a la unidad de hospitalización: 1) naturaleza de la transición de la UCI, 2) patrones de respuesta y 3) terapéutica enfermera. La terapéutica enfermera incorpora la información, educación y promoción de la autonomía del paciente; además del apoyo psicológico y emocional. Conclusiones: La teoría de las transiciones como marco teórico ayuda a comprender la experiencia de los pacientes durante la transición de la UCI. La terapéutica enfermera de empoderamiento integra las dimensiones dirigidas a satisfacer las necesidades y expectativas de los pacientes durante la misma.(AU)

Methodology: Secondary analysis of the findings of a descriptive qualitative study on the experience of patients admitted to an ICU during the transition to the inpatient unit, based on the nursing transitions theory. Data for the primary study were generated from 48 semi-structured interviews of patients who had survived critical illness in three tertiary university hospitals. Results: Three main themes were identified during the transition of patients from the intensive care unit to the inpatient unit: 1) nature of ICU transition, 2) response patterns and 3) nursing therapeutics. Nurse therapeutics incorporates information, education and promotion of patient autonomy; in addition to psychological and emotional support. Conclusions: Transitions theory as a theoretical framework helps to understand patients’ experience during ICU transition. Empowerment nursing therapeutics integrates the dimensions aimed at meeting patients’ needs and expectations during ICU discharge.

ABSTRACT

OBJECTIVE: To determine the impact of switching from the predictive low glucose suspend (PLGS) system to the advanced hybrid Tandem Control-IQ system on glucometrics and glycosylated haemoglobin (HbA1c) at one year. To assess the impact on the quality of life perceived by parents. METHOD: Prospective study in 71 patients aged 6-18 years with type 1 diabetes (DM1), in treatment with PLGS, who switched to an advanced hybrid system. Glucometric data were collected before the change, at 4 and 8 weeks, and at one year of use; HbA1c before the change and after one year. The Diabetes Impact and Devices Satisfaction (DIDS) questionnaire was used at weeks 4 and 8. RESULTS: An increase in time in range (TIR) was observed with a median of 76% (P<.001) at 4 weeks, which was maintained after one year (+8% in the total group). Overall, 73.24% of patients achieved a TIR above 70%. The subgroup with an initial TIR of less than 56% increased it by 14.4%. After one year there was a 0.3% reduction in HbA1c. Level 1 hypoglycaemia, level 1 and level 2 hyperglycaemia, mean glucose (GM) and coefficient of variation (CV) decreased. Auto mode stayed on 97% of the time and no dropouts occurred. Caregivers had a perception of better glycaemic control and less need to monitor blood glucose variations during the night. None of them would switch back to the previous system and they feel safe with the new system. CONCLUSIONS: The Tandem Control-IQ advanced hybrid system was shown to be effective one year after its implementation with improvement in all glucometric parameters and HbA1c, as well as night-time rest in caregivers.

ABSTRACT

Objetivo: el estiramiento muscular se aplica con el propósito de aumentar el arco de movimiento. Para este propósito se usan diferentes técnicas, entre ellas: la facilitación neuromuscular propioceptiva (FNP) y el estiramiento dinámico, pero aún no se conoce cuál de ellas es más efectiva. El objetivo de esta investigación fue estimar la eficacia de la técnica sostener relajar en comparación con el estiramiento dinámico sobre la flexibilidad de los isquiotibiales en adultos jóvenes sanos. Metodología: ensayo clínico controlado aleatorizado, con enmascaramiento de dos brazos. En el estudio participaron un total de 32 voluntarios adultos jóvenes con edades entre los 18 y 30 años y fueron aleatorizados a dos grupos de intervención FNP (n = 16), dinámico (n = 16). La flexibilidad de la articulación de la rodilla se evaluó mediante el ángulo poplíteo a través de la goniometría. Resultados: los resultados de las medias edad, peso, talla e índice masa corporal son equivalentes, lo que indica grupos comparables entre sí. No se hallaron diferencias entre los grupos de estiramiento en los promedios ajustados -2.09 (IC95% -7.05 a 2.87). Conclusiones: tanto la técnica dinámica como la FNP aumentaron el arco de movimiento de rodilla, ambas técnicas de estiramiento mostraron diferencias con la línea de base. No se encontraron diferencias entre los grupos de estiramiento.


Objective: Muscle stretching is applied with the purpose of increasing range of motion. Different techniques are used for this purpose, including proprioceptive neuromuscular facilitation (PNF) and dynamic stretching, but it is still unclear which one is more effective. The objective of this research was to estimate the efficacy of the hold-relax technique compared to dynamic stretching on hamstring flexibility in healthy young adults. Methodology: Randomized controlled trial with blinding, consisting of two intervention arms. The study included a total of 32 young adult volunteers aged 18 to 30 years, who were randomized into two intervention groups: PNF (n = 16) and dynamic stretching (n = 16). Knee joint flexibility was assessed using the popliteal angle measured with a goniometer. Results: The mean age, weight, height, and body mass index were found to be equivalent, indicating comparable groups. No significant differences were found between the stretching groups in the adjusted means, with a mean difference of -2.09 (95 % CI -7.05 to 2.87). Conclusions: Both dynamic stretching and PNF demonstrated an increase in knee range of motion, with both stretching techniques showing differences from the baseline. No significant differences were found between the stretching groups.


Finalidade: O alongamento muscular é aplicado com a finalidade de aumentar a ampli-tude de movimento. Para tanto, diversas técnicas são utilizadas, dentre elas: facilitação neuromuscular proprioceptiva (FNP) e alongamento dinâmico, mas ainda não se sabe qual delas é mais eficaz. O objetivo desta investigação foi estimar a eficáciada técnica segurar-relaxar em comparação com o alongamento dinâmico na flexibilidade dos isquiotibiais em adultos jovens saudáveis. Metodologia: Ensaio clínico randomizado, cego, controlado por dois braços. Um total de 32 voluntários adultos jovens com idades entre 18 e 30 anos participaram do estudo e foram randomizados para dois grupos de intervenção FNP (n = 16), dinâmico (n = 16). A flexibilidade da articulação do joelho foi avaliada por meio do ângulo poplíteo por goniometria. Resultados: os resultados das médias de idade, peso, altura e índice de massa corporal são equivalentes, o que indica grupos comparáveis entre si. Não foram encontradas dife-renças entre os grupos de alongamento nas médias ajustadas - 2,09 (IC 95 % -7,05 a 2,87). Conclusões: tanto a técnica dinâmica quanto o PNF aumentaram a amplitude de mo-vimento do joelho, ambas as técnicas de alongamento apresentaram diferenças com a linha de base. Não foram encontradas diferenças entre os grupos de alongamento.

ABSTRACT

Objective Evaluation of glucometrics in the first week of ICU stay and its association with outcomes. Design Prospective observational study. Setting Mixed ICU of teaching hospital. Patients Adults initiated on insulin infusion for 2 consecutive blood glucose (BG) readings ≥180mg/dL. Main variables of interest Glucometrics calculated from the BG of first week of admission: hyperglycemia (BG>180mg/dL) and hypoglycemia (BG<70mg/dL) episodes; median, standard deviation (SD) and coefficient of variation (CV) of BG, glycemic lability index (GLI), time in target BG range (TIR). Factors influencing glucometrics and the association of glucometrics to patient outcomes analyzed. Results A total of 5762 BG measurements in 100 patients of median age 55 years included. Glucometrics: hyperglycemia: 2253 (39%), hypoglycemia: 28 (0.48%), median BG: 169mg/dL (162–178.75), SD 31mg/dL (26–38.75), CV 18.6% (17.1–22.5), GLI: 718.5 [(mg/dL)2/h]/week (540.5–1131.5) and TIR 57% (50–67). Diabetes and higher APACHE II score were associated with higher SD and CV, and lower TIR. On multivariate regression, diabetes (p=0.009) and APACHE II score (p=0.016) were independently associated with higher SD. Higher SD and CV were associated with less vasopressor-free days; lower TIR with more blood-stream infections (BSI). Patients with higher SD, CV and GLI had a higher 28-day mortality. On multivariate analysis, GLI alone was associated with a higher mortality (OR 2.99, p=0.04). Conclusions Glycemic lability in the first week in ICU patients receiving insulin infusion is associated with higher mortality. Lower TIR is associated with more blood stream infections (AU)


Objetivo Evaluación de la glucometría en la primera semana de estancia en la UCI y su asociación con los resultados. Diseño Estudio observacional prospectivo. Ámbito UCI mixta de hospital docente. Pacientes Adultos que iniciaron una infusión de insulina para dos lecturas consecutivas de glucosa en sangre (GS) ≥180mg/dl. Principales variables de interés Glucometría calculada a partir de la GS de la primera semana de ingreso: episodios de hiperglucemia (GS >180mg/dl) e hipoglucemia (GS <70mg/dl); mediana, desviación estándar (DE) y coeficiente de variación (CV) de GS, índice de labilidad glucémica (ILG), tiempo en el rango objetivo de GS (TIR). Resultados Se incluyeron un total de 5.762 GS en 100 pacientes con una mediana de edad de 55años. Glucometría: hiperglucemia: 2.253 (39%), hipoglucemia: 28 (0,48%), mediana GS: 169mg/dl, DE 31mg/dl, CV 18,6%, ILG: 718,5 [(mg/dl)2/h]/semana, TIR 57%. La diabetes y una puntuación APACHEII más alta se asociaron con una DE y un CV más altos y una TIR más baja. En la regresión multivariada, la diabetes (p=0,009) y la puntuación APACHEII (p=0,016) se asociaron de forma independiente con una DE más alta. La DE y el CV más altos se asociaron con menos días sin vasopresores; menor TIR, con más infecciones del torrente sanguíneo (ITS). En el análisis multivariado, el ILG solo se asoció con una mayor mortalidad (OR: 2,99, p=0,04). Conclusiones La labilidad glucémica en la primera semana en pacientes de UCI que reciben infusión de insulina se asocia con mayor mortalidad. Una TIR más baja se asocia con más ITS (AU)

ABSTRACT

Introducción: El rodillo de espuma o Foam Roller (FR) es un instrumento de liberación miofascial autoinducida, para aplicarpresión de forma directa sobre la musculatura diana. FR es ampliamente empelado por deportistas como herramienta deauto-masaje. Objetivo: Evaluar la evidencia actual sobre el impacto del FR, sobre el sistema musculoesquelético, en deportistas, tratandode identificar los mecanismos que influyen sobre los tejidos miofasciales.Material y método: Basándonos en las directrices de los Elementos de Información Preferidos para Revisiones Sistemáticas yMetaanálisis (PRISMA), revisamos sistemáticamente estudios indexados en Web of Science, Cochrane y PubMed, para evaluarlos efectos del FR en el rango articular de movimiento (ROM), la flexibilidad, la fuerza y el dolor muscular de inicio retardado(DOMS) en deportistas de alto rendimiento. Se incluyeron artículos originales publicados desde el 2018 hasta el 30 de septiembre de 2022, con diseño de ensayo controlado o pre-post intervención, en los que se comparó la intervención de FR conun grupo control. Se utilizó la escala PEDro para evaluar de la calidad metodológica.Resultados: Entre los 141 registros identificados en la búsqueda, un total de 10 estudios cumplieron los criterios de inclusióny exclusión. En general, el uso de FR, en los deportistas de alto rendimiento, mostró mejoras significativas sobre el ROM yflexibilidad, y efectos notablemente beneficiosos sobre el DOMS y la fuerza, sin efectos adversos en el tejido miofascial. El FRpuede actuar mejorando la arquitectura tisular miofascial, atenuando el efecto inflamatorio y nociceptivo. Conclusión: El uso FR, parece seguro, es un instrumento efectivo para la mejora de las cualidades físicas de movilidad, fuerzay flexibilidad, y disminuir el DOMS incrementando del rendimiento deportivo.(AU)


Introduction: The Foam Roller (FR) is a self-induced myofascial release instrument to apply pressure directly on the targetmusculature. FR is widely used by athletes as a self-massage tool. Objective: We evaluate the current evidence on the impact of FR on the musculoskeletal system in athletes, trying to identifythe mechanisms that influence myofascial tissues.Material and method: Based on the Preferred Reporting Item Guidelines for Systematic Reviews and Meta-Analyses (PRISMA),we systematically reviewed studies indexed in Web of Science, Cochrane, and PubMed to evaluate the effects of FR on jointrange of motion (ROM), flexibility, strength, and delayed onset muscle soreness (DOMS) in high-performance athletes. Originalarticles published from 2018 through September 30, 2022, with controlled trial or pre-post intervention design, in which theFR intervention was compared to a control group, were included. The PEDro scale was used to assess methodological quality. Results: Among the 141 records identified in the search, a total of 10 studies met the inclusion and exclusion criteria. Ingeneral, the use of FR, in high performance athletes, showed significant improvements on ROM and flexibility, and markedlybeneficial effects on DOMS and strength, with no adverse effects on myofascial tissue. FR may act by improving myofascialtissue architecture, attenuating the inflammatory and nociceptive effect. Conclusion: The use of FR seems to be safe; it is an effective tool for the improvement of the physical qualities of mobility,strength, and flexibility, and to decrease DOMS and increase sports performance.(AU)

ABSTRACT

INTRODUCTION: Improvements in continuous glucose monitoring (CGM) in recent years have changed the treatment of type 1 diabetes (T1D) by permitting the automation of glucose control. The Minimed 780G advanced hybrid closed-loop (ACHL) system adapts basal infusion rates and delivers auto-correction boluses in order to achieve a user-decided glucose target (100, 110 or 120mg/dL). This study set out to evaluate the effectiveness of the Medtronic 780G system in real-life conditions over 6 months. MATERIALS AND METHODS: Prospective study that included T1D subjects previously treated with insulin pump without CGM (pump group) or with sensor-augmented pump with predictive low-glucose suspend (SAP-PLGS group) who started with the Minimed 780G system. Sensor and pump data from baseline, and at 1, 3 and 6 months were downloaded and HbA1c was recorded at baseline and at 6 months. RESULTS: Fifty T1D subjects were included; 25 were previous SAP-PLGS 640G users and 25 used 640G without CGM. 66% were female, 48.6 (40-57) years of age with 20 (12-31.5) years of diabetes duration. Time in range (TIR) improved in the total cohort from baseline to 6 months (69% (57.7-76) vs. 74% (70-82); p=0.01 as did HbA1c (7.6% (7.1-7.8) vs. 7.0% (6.8-7.5); p<0.001), with improvement in times <54, >180 and >250mg/dL. Outcomes at 6 months did not differ between groups, although the SAP-PLGS subjects were prone to hypoglycaemia and the pump group mainly presented suboptimal metabolic control. CONCLUSION: The AHCL Medtronic Minimed 780G system achieves and maintains good glycaemic control over 6 months in real-life conditions in different profiles of T1D subjects.

ABSTRACT

Flamenco dance is a performing art which is based on footwork technique where the foot and ankle play an important role.The objective of this study was to investigate the effect of ankle active range of motion on external load and its efficacy as apredictor during a flamenco footwork technique, with consideration of accelerometer positions and dance proficiency. Twelveflamenco dancers composed of 6 professional and 6 amateurs participated voluntarily in this study for whom no significantdifferences were detected regarding age, mass or height. Participants completed a 15-second Zap-3 footwork test at a speedof 160 bpm (beats per minute), 180 bpm and as fast as they could. Triaxial accelerometers were positioned at the dominantankle, 5th lumbar vertebrae and 7th cervical vertebrae to calculate accumulated PlayerLoad and uniaxial PlayerLoad of the3 planes (anteroposterior, mediolateral and vertical) for each speed level. Percentage contributions were also calculated. Theeffect of dorsiflexion on the external load of the dominant ankle of both professional and amateur dancers existed only in theanteroposterior axis while dorsiflexion was related to the external load at the 7th cervical vertebrae and only amateurs wereaffected. Plantarflexion only affected the uniaxial contribution of the vertical-axis of professional dancers. These programswould be applied to develop a technique feedback system for the flamenco dancer to follow their own model with respectto the ideal. This would allow intervention in the prevention of overuse injuries in flamenco dance artists.(AU)


El baile flamenco es un arte en el que el zapateado tiene un papel muy relevante. El objetivo de este estudio fue investigar elefecto del rango de movimiento activo del tobillo sobre la carga externa y su eficacia como predictor durante la realizaciónde un zapateado flamenco, en función de las posiciones del acelerómetro y el dominio técnico de los participantes. Un totalde doce bailaoras de flamenco, 6 profesionales y 6 amateurs, participaron voluntariamente en este estudio y en los que nose encontraron diferencias significativas respecto a edad, peso o estatura. Los participantes realizaron un test de zapateadodenominado Zap-3 durante 15 segundos a una velocidad de 160 pulsos por minuto, 180 y tan rápido como pudieron. Secolocaron acelerómetros triaxiales en el tobillo del pie dominante, en la 5ª vértebra lumbar y en la 7ª vértebra cervical paracalcular la PlayerLoad acumulada y la PlayerLoad uniaxial de los 3 planos (anteroposterior, medio-lateral y vertical) en funciónde cada nivel de velocidad, así como sus contribuciones porcentuales. Solamente se ha encontrado relación entre la flexióndorsal del tobillo dominante y la carga externa en el eje anteroposterior, tanto en profesionales como amateurs, mientras quea nivel de la 7ª vértebra cervical sólo se ha encontrado relación entre la dorsiflexión y la carga externa en el grupo de amateurs.Respecto a la flexión plantar solo se ha encontrado relación a nivel uniaxial con el eje vertical de los bailarines profesionales.Estos programas podrían servir de ayuda a desarrollar un sistema de retroalimentación de la técnica para que el practicantede baile flamenco pueda seguir su propio modelo respecto al ideal. Esto permitiría intervenir en la prevención de las lesionespor sobreuso en los artistas de baile flamenco.(AU)

ABSTRACT

Objetivo Evaluar la fiabilidad intraexaminador e interexaminador de la aplicación Clinometer para medir el rango de movimiento (RDM) de la cadera en pacientes con artrosis de cadera y su correlación con el goniómetro universal. Métodos Se diseñó un estudio de medidas repetidas en el que se incluyeron 35 pacientes con artrosis de cadera. Se registró el RDM de rotación interna, externa, flexión y extensión de cadera mediante la aplicación Clinometer por 2 examinadores independientes. Se calculó la fiabilidad intraexaminador e interexaminador utilizando el coeficiente de correlación intraclase, y se calculó el error estándar de medición y el cambio mínimo detectable. Se utilizó el coeficiente de correlación de Pearson para correlacionar la aplicación Clinometer con el goniómetro universal. Resultados La aplicación Clinometer mostró una fiabilidad excelente tanto intraexaminador (coeficiente de correlación intraclase: 0,82-0,96) como interexaminador (coeficiente de correlación intraclase: 0,81-0,95) para todos los RDM de la cadera valorados. La correlación entre este instrumento y el goniómetro universal mostró ser fuerte para todos los RDM de la cadera (r>0,70). Conclusión La aplicación Clinometer ha mostrado unos niveles de fiabilidad excelentes para la medición del RDM de rotación interna, externa, flexión y extensión de la cadera en pacientes con artrosis y presenta una fuerte correlación con el goniómetro universal (AU)


Objective To evaluate the test–retest and inter-rater reliability of the application Clinometer for measuring the hip range of motion (ROM) in patients with hip osteoarthritis and its correlation with the universal goniometer. Methods A blinded repeated measured was carried out with 35 patients with hip osteoarthritis. Hip internal rotation, external rotation, flexion and extension ROM were measured using Clinometer app by 2 independents raters. The intraclass correlation coefficient was used to calculate the test–retest reliability and inter-rater reliability. The standard error measurement and minimal detectable change were also calculated. The Pearson's correlation coefficient was used to correlate the app with the universal goniometer. Results Clinometer application achieved excellent test–retest reliability (intraclass correlation coefficient: 0.82-0.96) and inter-rater reliability (intraclass correlation coefficient: 0.81-0.95) in all the ROMs assessed. The correlation analysis with the universal goniometer showed strong correlation values (r>0.70) in all the ROMs assessed. Conclusion Clinometer application presented excellent test–retest reliability and inter-rater reliability for the measurement of hip ROM in patients with hip osteoarthritis. The smartphone application showed a strong correlation compared to universal goniometer (AU)

ABSTRACT

Objetivo Comparar los efectos de un estiramiento pasivo del músculo cuádriceps frente a un autoestiramiento en la flexibilidad de los músculos del muslo y el rango de movimiento (ROM) de la cadera en jugadores de fútbol. Métodos Se diseñó un ensayo clínico aleatorizado en el que se incluyeron 34 jugadores de fútbol amateur con déficit de flexibilidad en el músculo cuádriceps. Los jugadores fueron aleatorizados en dos grupos (grupo estiramiento o grupo autoestiramiento). Cada participante recibió una única sesión de estiramiento pasivo o autoestiramiento del cuádriceps en su pierna dominante. Se valoró la flexibilidad del cuádriceps mediante el test de Ely, la flexibilidad de los isquiotibiales mediante el Passive Knee Extension test, y ROM de flexión y extensión de la cadera. Resultados Ambos grupos mostraron un aumento estadísticamente significativo de la flexibilidad del cuádriceps, de la flexibilidad de los isquiotibiales y del ROM de extensión de la cadera sin diferencias significativas entre ellos (p>0,05). El grupo estiramiento mostró unos tamaños del efecto grandes en la flexibilidad y el ROM de extensión (d>0,8), siendo superiores a los registrados en el grupo autoestiramiento. Conclusiones La flexibilidad de los músculos cuádriceps e isquiotibiales, así como el ROM de extensión de cadera aumentaron tras el estiramiento y el autoestiramiento del músculo cuádriceps. Los tamaños del efecto mostrados por el grupo estiramiento fueron superiores a los del grupo autoestiramiento (AU)


Objective To compare the effects of quadriceps passive stretching or quadriceps self-stretching in muscle flexibility and hip range of motion (ROM) in football athletes. Methods A randomized clinical trial was carried out. Thirty-four football athletes with lack of flexibility in the quadriceps muscle were included and randomized in two groups (Stretching group or self-stretching group) and received a single session of quadriceps passive stretching or quadriceps self-stretching in the dominant lower limb. The outcome variables were: quadriceps flexibility measured with the Ely's test, hamstring flexibility measured with the Passive Knee Extension test and hip flexion and extension ROM. Results Both groups achieved a statistically significant improvement of quadriceps flexibility, hamstrings flexibility and hip extension ROM without statistically significant differences between them (p>0.05). The stretching group reported large effect sizes in muscle flexibility and hip extension ROM (d>0.8), and the effect sizes of the stretching group were higher than the self-stretching group. Conclusions Flexibility of the quadriceps and hamstring muscles as well as hip extension ROM increased after stretching and self-stretching of the quadriceps muscle. The effect sizes shown by the stretching group were higher than those of the self-stretching group (AU)

OBJECTIVE: Evaluation of glucometrics in the first week of ICU stay and its association with outcomes. DESIGN: Prospective observational study. SETTING: Mixed ICU of teaching hospital. PATIENTS: Adults initiated on insulin infusion for 2 consecutive blood glucose (BG) readings ≥180mg/dL. MAIN VARIABLES OF INTEREST: Glucometrics calculated from the BG of first week of admission: hyperglycemia (BG>180mg/dL) and hypoglycemia (BG<70mg/dL) episodes; median, standard deviation (SD) and coefficient of variation (CV) of BG, glycemic lability index (GLI), time in target BG range (TIR). Factors influencing glucometrics and the association of glucometrics to patient outcomes analyzed. RESULTS: A total of 5762 BG measurements in 100 patients of median age 55 years included. Glucometrics: hyperglycemia: 2253 (39%), hypoglycemia: 28 (0.48%), median BG: 169mg/dL (162-178.75), SD 31mg/dL (26-38.75), CV 18.6% (17.1-22.5), GLI: 718.5 [(mg/dL)2/h]/week (540.5-1131.5) and TIR 57% (50-67). Diabetes and higher APACHE II score were associated with higher SD and CV, and lower TIR. On multivariate regression, diabetes (p=0.009) and APACHE II score (p=0.016) were independently associated with higher SD. Higher SD and CV were associated with less vasopressor-free days; lower TIR with more blood-stream infections (BSI). Patients with higher SD, CV and GLI had a higher 28-day mortality. On multivariate analysis, GLI alone was associated with a higher mortality (OR 2.99, p=0.04). CONCLUSIONS: Glycemic lability in the first week in ICU patients receiving insulin infusion is associated with higher mortality. Lower TIR is associated with more blood stream infections.

Objetivo: El objetivo principal de este trabajo es determinar la variación de movilidad pre y postquirúrgica del primer radio. Pacientes y métodos: El diseño corresponde a un estudio prospectivo de series de casos en pacientes sometidos a cirugía de hallux valgus. Se valoró en 13 pies la movilidad del primer radio con el medidor del primer radio, la flexo-extensión de la primera articulación metatarsofalángica, el ángulo de Clarke y el “Arch Index” en pedigrafías, los ángulos hallux valgus e intermetatarsal I-II en radiografías dorsoplantares en carga, y la postura general del pie mediante el “Foot Posture Index” antes y después de ser sometidos a cirugía de hallux valgus. Todo ello con un periodo de seguimiento de 6 meses. Resultados: El rango total de movilidad del primer radio pasó de 11.74 ± 1.88 mm a 6.72 ± 2.20 mm; y el de la primera articulación metatarsofalángica pasó de 79.62° ± 22.33° a 48.69° ± 18.69° tras la cirugía. Los ángulos radiográficos de hallux valgus y ángulo intermetatarsal disminuyeron una media de 19.07° ± 8.20° y 7.23° ± 2.97°, respectivamente. El ángulo de Clarke cambió de 41.31° ± 10.79° a 38.31° ± 11.00°. El “Arch Index” cambió de 0.24 ± 0.06 a 0.25 ± 0.05. Y el “Foot Posture Index” pasó de 6.70 ± 3.33 de media antes de la cirugía a 5.46 ± 3.86 tras la misma. Conclusión: En los participantes incluidos en este estudio se observó una disminución generalizada de todas las variables examinadas tras la cirugía, a excepción de los parámetros valorados en las pedigrafías, que no llegaron a presentar diferencias relevantes.(AU)


Objectives: The main aim of this work is to determine the variation of first ray mobility before and after surgery. Patients and methods: The design corresponds to a prospective case series of patients operated of hallux valgus. A total of 13 feet have been included in this study. Measurements of the first ray mobility were obtained using the first ray mobility measurer. Furthermore, we assess the flexo-extension of the first metatarsophalangeal joint. The Clarke’s angle and the Arch Index were evalued on weightbearing footprints. The hallux valgus angle and intermetatarsal I-II angle were made on weightbearing dorsal-plantar projection radiographs. And finally, the general posture of the foot was quantified using the Foot Posture Index. All this with pre and post measurements of hallux valgus surgery and with a follow-up period of 6 months. Results: The total range of motion of the first ray decreased from 11.74 ± 1.88 mm to 6.72 ± 2.20 mm; and the range of motion of the first metatarsophalangeal joint underwent from 79.62° ± 22.33° to 48.69° ± 18.69° after surgery. The hallux valgus angle and intermetatarsal I-II angle decreased by a mean of 19.07° ± 8.20° and 7.23° ± 2.97°, respectively. The Clarke’s angle changed from 41.31° ± 10.79° to 38.31° ± 11.00°. The Arch Index changed from 0.24 ± 0.06 to 0.25 ± 0.05. And the Foot Posture Index went from 6.70 ± 3.33 before surgery to 5.46 ± 3.86 after it. Una per sona empoderada es aquella que es ca paz de deliberar, solucionar sus contra tiempos y complacer sus necesidades, con control y juicio crítico sobre su vida y su salud. Para que consigan estas habi lidades, nosotros como profesionales tenemos la responsabilidad de informar y formar a los pacientes para que mejoren su conocimiento y habilidades en el au tocuidado de sus enfermedades crónicas (1). A propósito de un caso clínico abor da, desde una teoría de rango medio, como podemos abordar a este tipo de pacientes para un correcto mantenimien to de su autocuidado (AU)

Las caídas son un problema de salud pública de primer orden, y su pre vención es algo clave en el manejo de las mismas. El paciente protagonista del caso clínico ha tenido una caída casual en el baño de su domicilio lo que le ha provocado una fractura de cadera y, por tanto, una pérdida de independencia y calidad de vida. Para su valoración se utilizó la Teoría de Rango Medio de “Los síntomas desagradables” cuya autoría lidera la enfermera y doctora en sociolo gía Elisabeth Lenz y teniendo a Linda Pugh, Renee Milligan y Andreu Gift como colaboradoras. Tras esto, se realiza un plan de cuidados individualizado utilizan do el lenguaje estandarizado enfermero NANDA, NOC y NIC. Finalmente, el pa ciente es capaz de conseguir una mejora en su salud a través de la adhesión al plan y de nuestra intervención desde el ámbito familiar y comunitario (AU)

A care plan is made through the standardized languages NANDA, NOC and NIC. ABSTRACT Introduction In competitive sports like Tae Kwon Do, the body remains in constant disequilibrium and displacement. In this state, the limbs generate and transmit energy through the muscles of the abdominal core, which is the main link in the human kinetic energy chain. Objective Explore the effect of abdominal core strength on improving flexibility in taekwondo athletes. Methods Thirty male athletes were selected, with a minimum sport time limit of 4 years, and a mean age of 21±1 years. They were randomly divided into three groups, A, B and C, with 10 people in each group. Group A participated in stable abdominal core strength training, 1 hour daily, 3 times a week, for a total of 12 weeks. Results Intra-group comparisons: There were extremely significant differences between groups A, B and C; extremely significant differences were found between the first and second and third times in group A; when compared, the first, second and third times in group B also showed modifications. Conclusion Stable and unstable core strength training can improve flexibility; after training, flexibility gains from unstable abdominal core strength training are more lasting. RESUMO Introdução Em esportes competitivos como o Tae Kwon Do, o corpo permanece em constante desequilíbrio e deslocamento. Neste estado, os membros geram e transmitem energia através dos músculos do centro abdominal, que é o elo principal da cadeia de energia cinética humana. Objetivo Explorar o efeito da força do centro abdominal na melhoria da flexibilidade dos atletas de taekwondo. Métodos Foram selecionados 30 atletas masculinos, com limite de tempo esportivo mínimo de 4 anos, e idade média de 21±1 anos. Foram divididos aleatoriamente em três grupos, A, B e C, com 10 pessoas em cada grupo. O grupo A participou do treinamento estável da força do centro abdominal, 1 hora diária, 3 vezes por semana, por um total de 12 semanas. Resultados Comparações intra-grupo: Houve diferenças extremamente significativas entre os grupos A, B e C; foram encontradas diferenças extremamente significativas entre a primeira e segunda e terceira vez no grupo A; quando comparados, a primeira, segunda e terceira vezes do grupo B também apresentaram modificações. Conclusão O treinamento estável e instável da força do centro abdominal pode melhorar a flexibilidade; após o treinamento, os ganhos de flexibilidade do treinamento instável da força do centro abdominal são mais duradouros. RESUMEN Introducción En los deportes de competición como el Tae Kwon Do, el cuerpo permanece en constante desequilibrio y desplazamiento. En este estado, las extremidades generan y transmiten energía a través de los músculos del núcleo abdominal, que es el principal eslabón de la cadena de energía cinética humana. Objetivo Explorar el efecto de la fuerza del núcleo abdominal en la mejora de la flexibilidad en atletas de taekwondo. Métodos Se seleccionaron 30 atletas masculinos, con un tiempo mínimo de práctica deportiva de 4 años, y una edad media de 21±1 años. Se dividieron aleatoriamente en tres grupos, A, B y C, con 10 personas en cada grupo. El grupo A participó en un entrenamiento de fuerza abdominal estable, 1 hora diaria, 3 veces por semana, durante un total de 12 semanas. Resultados Comparaciones intragrupo: Hubo diferencias extremadamente significativas entre los grupos A, B y C; se encontraron diferencias extremadamente significativas entre el primer y el segundo y tercer tiempo del grupo A; cuando se compararon, el primer, el segundo y el tercer tiempo del grupo B también mostraron modificaciones. Conclusión El entrenamiento de la fuerza del núcleo estable e inestable puede mejorar la flexibilidad; después del entrenamiento, las ganancias de flexibilidad del entrenamiento de la fuerza del núcleo abdominal inestable son más duraderas. ABSTRACT Introduction: The quality of flexibility greatly impacts the performance of aerobics athletes. It is believed that stretching by proprioceptive neuromuscular facilitation (PNF) can contribute to this aspect by maximizing active joint range motion. Objective: Explore the influences of PNF stretching on the physical fitness of aerobics athletes. Methods: 24 professional aerobics athletes were selected, and randomly distributed into two groups with no statistical discrepancies in physical indices. PNF stretching exercise was performed on aerobics athletes in the experimental group, while no training intervention was performed in the control group. Physical fitness was assessed before and after the nine weeks of the intervention. Results: After PNF stretching training, the lower limb vertical score in the experimental group was from 6.58±0.19 to 8.20±0.47, and the transverse pronation score changed from 5.95±0.35 to 7.09±0.61. The vertical score in the control group was from 6.44±59.89 to 6.83±0.37 after the experiment; the transverse pronation penetration score was from 5.78±0.37 to 6.58±0.33. Conclusion: PNF stretching can effectively improve athletes' physical fitness, both statically and dynamically, generating gains in range of motion and providing favorable conditioning for overall competitive performance. RESUMO Introdução: A qualidade da flexibilidade tem um grande impacto sobre o desempenho dos atletas de aeróbica, e acredita-se que o alongamento por facilitação neuromuscular proprioceptiva (PNF) possa contribuir neste quesito com a maximização da amplitude de movimento articular ativa. Objetivo: Explorar as influências do alongamento PNF sobre a aptidão física dos atletas de aeróbica. Métodos: Foram selecionadas 24 atletas profissionais de aeróbica, aleatoriamente distribuídas em dois grupos sem discrepâncias estatísticas nos índices físicos. Foi realizado o exercício de alongamento PNF em atletas de aeróbica no grupo experimental, enquanto nenhuma intervenção de treinamento foi realizada no grupo de controle. A aptidão física foi avaliada antes e após as nove semanas da intervenção. Resultados: Após o treinamento de alongamento PNF, a pontuação de membros inferiores vertical no grupo experimental foi de 6,58±0,19 para 8,20±0,47, e a pontuação da pronação transversal mudou de 5,95±0,35 para 7,09±0,61. A pontuação vertical no grupo de controle foi de 6,44±59,89 para 6,83±0,37 depois do experimento; a pontuação da penetração transversal de pronação foi de 5,78±0,37 para 6,58±0,33. Conclusão: O alongamento PNF pode efetivamente melhorar a aptidão física das atletas, tanto estática quanto dinâmica, gerando ganhos à amplitude de movimento proporcionando um condicionamento favorável ao desempenho competitivo geral. RESUMEN Introducción: La calidad de la flexibilidad tiene un gran impacto en el rendimiento de los atletas de aeróbic, y se cree que el estiramiento por facilitación neuromuscular propioceptiva (PNF) puede contribuir en esta cuestión con la maximización de la amplitud articular activa de movimiento. Objetivo: Explorar las influencias del estiramiento PNF en la condición física de atletas de aeróbic. Métodos: Fueron seleccionados 24 atletas profesionales de aeróbic, distribuidos aleatoriamente en dos grupos sin discrepancias estadísticas en los índices físicos. Se realizaron ejercicios de estiramiento PNF en los atletas de aeróbic del grupo experimental, mientras que en el grupo de control no se realizó ninguna intervención de entrenamiento. Se evaluó la aptitud física antes y después de las nueve semanas de intervención. Resultados: Tras el entrenamiento de estiramiento PNF, la puntuación vertical de las extremidades inferiores en el grupo experimental fue de 6,58±0,19 a 8,20±0,47, y la puntuación de pronación transversal cambió de 5,95±0,35 a 7,09±0,61. La puntuación vertical en el grupo de control fue de 6,44±59,89 a 6,83±0,37 después del experimento; la puntuación de penetración de pronación transversal fue de 5,78±0,37 a 6,58±0,33. Conclusión: El estiramiento PNF puede mejorar eficazmente la condición física de los atletas, tanto estática como dinámica, generando ganancias en el rango de movimiento y proporcionando un acondicionamiento favorable para el rendimiento competitivo general. ABSTRACT Introduction: Aerobic gymnastics is a sport that involves a sense of beauty, rhythm and dance, with high artistic expressiveness, characteristics that have favored a strong population adherence. It is believed that the specific flexibility exercise can add a greater visual impact to their artistic movements, raising their competitive performance. Objective: Verify the impacts of lower limb flexibility exercise on the performance of aerobics athletes. Methods: Six aerobics athletes were selected to perform an 8-week experiment, adopting the PNF methodology for training. The impacts on lower limb flexibility were measured before and after the experiment, these data were analyzed and studied statistically. Results: The results of the long jump in the physical quality index of the athletes increased by 0.1209 m; the added value in the two-minute, 10-m backstroke test was 1.46. The movement performance indexes showed expressive increases: flexion increased by 2.62; right angle support increased by 0.78; split leg jump increased by 1.95; standing jump with eyes closed and the foot balance quality index was 4.63. Conclusion: Lower limb flexibility exercise can positively impact the performance of aerobic gymnastics athletes. Level of evidence II; Therapeutic studies - investigation of treatment outcomes.


RESUMO Introdução: A ginástica aeróbica é um esporte que envolve senso de beleza, ritmo e dança, com alta expressividade artística, características que favoreceram uma forte adesão populacional. Acredita-se que o exercício específico de flexibilidade possa adicionar um maior impacto visual aos seus movimentos artísticos, elevando o desempenho competitivo. Objetivo: Verificar os impactos do exercício de flexibilidade dos membros inferiores sobre o desempenho dos atletas de aeróbica. Métodos: Seis atletas de aeróbica foram selecionados para realizar uma experiência de 8 semanas, adotando a metodologia PNF para o treinamento. Os impactos sobre a flexibilidade dos membros inferiores foram aferidos antes e depois do experimento, esses dados foram analisados e estudados estatisticamente. Resultados: Os resultados do salto em distância no índice de qualidade física dos atletas aumentaram em 0,1209 m; o valor agregado no teste de dois minutos e 10 m de recuo foi de 1,46. Os índices de desempenho do movimento apresentaram aumentos expressivos: de flexão teve um aumento de 2,62; apoio em ângulo reto aumentou 0,78; salto de pernas divididas aumentou 1,95; salto de pé com os olhos fechados e o índice de qualidade de equilíbrio dos pés foi de 4,63. Conclusão: O exercício de flexibilidade para membros inferiores pode impactar positivamente o desempenho dos atletas de ginástica aeróbica. RESUMEN Introducción: La gimnasia aeróbica es un deporte que envuelve un sentido de belleza, ritmo y danza, con alta expresividad artística, características que favorecieron una fuerte adhesión de la población. Se cree que el ejercicio específico de la flexibilidad puede agregar un mayor impacto visual a sus movimientos artísticos, elevando el desempeño competitivo. Objetivo: Verificar los impactos del ejercicio de flexibilidad de los miembros inferiores en el desempeño de atletas de aeróbica. Métodos: Seis atletas de aeróbic fueron seleccionados para realizar un experimento de 8 semanas, adoptando la metodología PNF para el entrenamiento. Los impactos en la flexibilidad de los miembros inferiores fueron medidos antes y después del experimento, estos datos fueron analizados y estudiados estadísticamente. Resultados: Los resultados del salto de longitud en el índice de la calidad física de los atletas aumentaron en 0,1209 m; el valor añadido en la prueba de dos minutos y 10 m de espalda fue de 1,46. Los índices de rendimiento de movimiento mostraron aumentos expresivos: de flexión tuvo un aumento de 2,62; apoyo en ángulo recto aumentó 0,78; salto de pierna dividida aumentó 1,95; salto de pie con los ojos cerrados y el índice de calidad de equilibrio de los pies fue de 4,63. Conclusión: El ejercicio de flexibilidad de los miembros inferiores puede influir positivamente en el rendimiento de los atletas de gimnasia aeróbica.
