ABSTRACT

This is the case report of a woman who started to write and read from right to left after anterior cerebral artery stroke, affecting the left supplementary motor area. No cases were found in the literature with exactly the same characteristics. She has been able to read and write faster after rehabilitation approach at Sarah Network of Rehabilitation Hospitals, in the Belo Horizonte city unit, Brazil, despite the maintenance of the inversion. She returned to her previous activities in an adaptive way. It was discussed how the dysfunction in this cerebral area and its connections may disturb the reading strategy and direction.


Relato do caso de uma mulher que passou a escrever e ler da direita para a esquerda após um acidente vascular encefálico isquêmico de artéria cerebral anterior, acometendo área motora suplementar esquerda. Não foram encontrados casos na literatura exatamente com as mesmas características. Durante a participação da paciente no programa de reabilitação neurológica da Rede Sarah de Hospitais de Reabilitação, unidade Belo Horizonte, foram observados ganhos na agilidade de leitura e escrita, ainda que mantendo a inversão, e retorno às suas atividades de forma adaptada. Realizou-se discussão de como o comprometimento dessa área e de suas conexões pode perturbar a estratégia de leitura e sua direção.

RESUMO Objetivo Caracterizar o conhecimento, habilidades, opiniões e principais barreiras percebidas por fonoaudiólogos, da área de linguagem infantil no Brasil, a respeito da prática baseada em evidências (PBE). Método O estudo foi conduzido entre agosto de 2021 e julho de 2022 por meio de um questionário online. Além de dados sociodemográficos e de campo de trabalho, foram considerados 22 itens relacionados à PBE e subdivididos em "conhecimento", "habilidades", "opinião" e "barreiras". Cada item apresentava cinco opções de resposta (discordo totalmente, discordo, não estou decidido, concordo, concordo totalmente). Ao total 122 fonoaudiólogos que atuam na área da linguagem infantil responderam ao questionário. Suas respostas foram descritas pela porcentagem de distribuição de frequência. O tempo de formação e o nível de domínio do inglês foram considerados para comparar o padrão de distribuição das respostas. Resultados Apesar da maioria dos fonoaudiólogos reportar ter aprendido as bases da PBE em sua formação acadêmica, há fragilidades em seu conhecimento e falta de domínio das estratégias de busca e avaliação crítica dos artigos científicos. Ainda que a maioria concorde que a aplicação da PBE é necessária para a prática fonoaudiológica e considere precisar aumentar o uso de evidências científicas em sua prática diária, são apontadas como barreiras a falta de artigos, dificuldades relacionadas à aplicação prática de resultados científicos e falta de apoio coletivo entre os colegas. Conclusão Este estudo alerta a comunidade acadêmica para a urgência de se considerar a PBE no contexto da Fonoaudiologia brasileira.


ABSTRACT Purpose To characterize the knowledge, skills, opinions, and main barriers perceived by speech-language pathologists, in child language in Brazil, regarding evidence-based practice (EBP). Methods The study was conducted between August 2021 and July 2022 using an online questionnaire. In addition to sociodemographic and field data, 22 items related to EBP were considered and subdivided into "knowledge", "skills", "opinion" and "barriers". Each item had five response options (strongly disagree, disagree, not decided, agree, strongly agree). A total of 122 speech-language pathologists who work with child language answered the questionnaire. Their responses were described by the percentage of frequency distribution. The time since graduation and the level of proficiency in English were considered to compare the distribution pattern of the answers. Results Although most speech-language pathologists report having learned the basics of EBP in their academic training, there are weaknesses in their knowledge and lack of mastery of search strategies and critical evaluation of scientific articles. Although most agree that EBP's use is necessary for speech-language practice and consider the need to increase the use of scientific evidence in their daily practice, the lack of articles, difficulties related to the practical application of scientific results and lack of collective support among colleagues are identified as barriers. Conclusion This study alerts the academic community to the urgency of considering EBP in the context of Brazilian Speech-Language Pathology.

ABSTRACT

RESUMO O Transtorno do Espectro Autista (TEA) é classificado pelo Manual de Diagnóstico e Estatístico de Transtornos Mentais (DSM-5) como um Transtorno do Neurodesenvolvimento, sendo caracterizado, principalmente, por alterações na comunicação social e pela presença de um repertório restrito de atividades e interesses. Na literatura, há muitos estudos sobre autismo, fala e linguagem, mas poucos correlacionando fala e autismo. Este estudo teve como finalidade realizar um estudo de caso que abordou autismo, fala e PROMPT (Pontos para a Reestruturação de Objetivos Fonéticos e Oro-Musculares) e mensurou a melhora da fala no participante com autismo, utilizando o método. Foram definidas palavras-alvo para toda a intervenção, conforme o Sistema de Observação e Análise (SOA) e Hierarquia Motora de Fala (HMF), que fazem parte da avaliação de PROMPT. Após a avaliação, o participante foi atendido por 16 sessões, uma vez semanalmente. Após análise dos dados, observou-se melhora em todos os aspectos que foram delineados de acordo com a avaliação no pré-tratamento, como controle fonatório, controle mandibular, controle lábio-facial e controle lingual, como também no movimento sequenciado, apesar deste não ser o objetivo traçado na avaliação. Também foi possível mensurar melhora no número de vocábulos adequados, número de fonemas adequados, porcentagens de consoantes corretas - revisado (PCC-R) e inteligibilidade.


ABSTRACT Autism Spectrum Disorder (ASD) is classified by Diagnostic and Statistical Manual of Mental Disorders (DSM-5) as a neurodevelopmental disorder, whose characteristics are mainly deficits in social communication and a restricted range of interests. There are several studies about autism, speech, and language in the literature, but few correlate speech and autism. This study aims to carry out a case study that will address autism, speech, and PROMPT (Restructuring Oral Muscular Phonetic Targets) and also to describe the speech improvement in the participant with autism using the method. The target words were defined for the entire intervention according to the System Analysis Observation (SAO) and Motor Speech Hierarchy (MSH), which are parts of the PROMPT evaluation. After the evaluation, the participant was attended for 16 sessions, once weekly, with the objective of improving their speech. After analyzing the data, it was possible to observe improvement in all aspects outlined according to the pre-treatment evaluation of the method such as phonatory control, mandibular control, lip-facial control and lingual control as well as in the sequenced movement although this was not the aim outlined in the evaluation. It was also possible to measure the improvement of an adequate number of words, an adequate number of phonemes, percentages of correct consonants - revised (PCC-R), and intelligibility.

ABSTRACT

BACKGROUND: There has been a growing focus on functional communication interventions for primary progressive aphasia (PPA). These interventions aim to support individuals to participate in life situations. One such intervention, communication partner training (CPT) aims to change conversation behaviours in both the person with PPA and their communication partner (CP). CPT has a growing evidence base in stroke aphasia; however, these programmes are not designed to meet the needs of people with progressive communication difficulties. To address this, the authors developed a CPT program entitled Better Conversations with PPA (BCPPA) and undertook a pilot trial to establish for a future full trial; predicted recruitment rates, acceptability, an assessment of treatment fidelity and an appropriate primary outcome measure. METHODOLOGY: This was a single-blind, randomised controlled pilot study comparing BCPPA to no treatment, delivered across 11 National Health Service Trusts in the UK. A random sample of eight recordings of local collaborators delivering the intervention were analysed to examine fidelity. Participants completed feedback forms reporting on acceptability. Pre- and post-intervention measures targeted conversation behaviours, communication goals and quality of life. RESULTS: Eighteen people with PPA and their CPs (9 randomised to BCPPA, 9 randomised to no treatment) completed the study. Participants in the intervention group rated BCPPA positively. Treatment fidelity was 87.2%. Twenty-nine of 30 intervention goals were achieved or over-achieved and 16 of 30 coded conversation behaviours demonstrated change in the intended direction. The Aphasia Impact Questionnaire was identified as the preferred outcome measure. CONCLUSION: The first randomised controlled UK pilot study of a CPT program for people with PPA and their families demonstrates BCPPA is a promising intervention. The intervention was acceptable, treatment fidelity high and an appropriate measure identified. Results of this study indicate a future RCT of BCPPA is feasible. ABSTRACT

OBJECTIVE: The Padovan-Method Neurofunctional Reorganization® is a promising approach in speech therapy treating neurodevelopmental disorders with traumatic or congenital origin. Its use is based on a long-time experience of certified therapists. However, its efficacy and safety has not been assessed in a systematic review. This report aims to gain evidence for the use of the therapy method. The review was registered (PROSPERO: CRD42020156124). METHODS AND ANALYSIS: Guidelines of PRISMA, the Cochrane Collaboration Handbook, MECIR, and GRADE were followed. General databases (Cochrane Library, PubMed, AWMF, Anthromedics, etc.) and further 38 databases including grey literature were searched. Hand search was done additionally and contact to experts used to retrieve unpublished manuscripts. All trials investigating the effect of the method in comparison to either no intervention, alternative as state of the art, or placebo intervention in English, Portuguese, and German language were included. No restriction regarding study design was applied. Data related to the intervention outcome and the study method was extracted and analyzed independently. Risk of bias was assessed using ROBINS-I for non-RCTs, adherence to CARE-Guidelines was analyzed for case series or reports, and keeping the Declaration of Helsinki was checked for all items. Results are presented both in evidence profiles and summary of findings tables according to GRADE. RESULTS: Amongst 98 records assessed for eligibility, four studies and 14 case reports were identified with a total of n = 196 participants. Duration of reported interventions was between 2 days and 2 years. Microcephalia, down-syndrome, unspecified neurological disorders, and myofunctional disorders were main conditions of the patients with neurodevelopmental disorders. Only indirect overlapping of operationalized criteria was found. Conclusions are therefore limited. CONCLUSION: The Padovan-Method® is a holistic therapy approach claiming its feasibility to a large group of disorders making a proof of efficacy difficult. An application of therapy according to the Padovan-Method® by trained therapists might be considered by clinicians (weak recommendation) and a contribution to a relief of symptoms or improvements of condition of named conditions might be gained. Therefore, development and validation of therapy protocols and further investigation are required.

ABSTRACT

The Rehabilitation Treatment Specification System (RTSS) was developed as a systematic way to describe rehabilitation treatments for the purpose of both research and practice. The RTSS groups treatments by type and describes them by 3 elements: the treatment (1) ingredients and (2) the mechanisms of action that yield changes in the (3) target behavior. Adopting the RTSS has the potential to improve consistency in research, allowing for better cross-study comparisons to strengthen the body of research supporting various treatments. Because it is still early in its development, the RTSS has not yet been widely implemented across different rehabilitation disciplines. In particular, aphasia recovery is one area of rehabilitation that could benefit from a unifying framework. Accordingly, this article is part of a series where we illustrate how the RTSS can be applied to aphasia treatment and research. This article more specifically focuses on examining the neurobiological mechanisms of action associated with experimental aphasia therapies, including brain stimulation and pharmacologic intervention, as well as more traditional behavioral therapy. Key elements of the RTSS are described, and 4 example studies are used to illustrate how the RTSS can be implemented. The benefits of a unifying framework for the future of aphasia treatment research and practice are discussed.

Introdução: O treinamento auditivo é um método de intervenção utilizado para aprimorar o desempenho das habilidades auditivas alteradas, de modo que, pode promover melhora no processamento da informação sonora. Objetivo: Verificar a eficácia da estimulação do processamento auditivo central na reabilitação dos transtornos fonológicos. Método: Foi realizado um estudo de caso de seis crianças com idades entre 6:00 e 7:11 de idade, com diagnóstico de transtorno fonológico. Todas as crianças passaram por avaliação fonoaudiológica, com avaliação específica da fonologia por meio da Avaliação Fonológica da Criança (Yavas, Hernandorena e Lamprecht, 2002) e pela avaliação das habilidades de processamento temporal por meio dos testes de ordenação e resolução temporal, pré e pós terapia. Todos os sujeitos passaram por 25 sessões de terapia, sendo que três sujeitos foram submetidos à terapia puramente fonológica (grupo de estudo 1 - GE1), e as outras três crianças receberam terapia fonológica associada ao treinamento auditivo computadorizado (grupo de estudo 2 - GE2), com o software Escuta Ativa. Resultados: Não houve diferença estatisticamente significativa quando comparados os resultados do desempenho em tarefas concernentes a percepção do sistema fonológico e do processamento temporal nos mesmos testes (conjunto de tarefas com habilidades auditivas) aplicados pré e pós terapia do GE1 com o GE2. Contudo, houve melhora no desempenho dos testes de processamento temporal em GE2. Conclusão: A terapia fonológica combinada ao treinamento auditivo computadorizado em crianças com transtorno fonológico não evidenciou diferença estatisticamente diferente, porém demonstrou influenciar no aumento do desempenho dos testes de processamento temporal destas crianças quando comparado ao uso isolado da terapia fonológica.


Introduction: The auditory training is an intervention method used to improve the performance of auditory skills, aiming to enhance the processing of sound information. Objective: To assess the effectiveness of central auditory processing stimulation in the rehabilitation of phonological disorders. Method: A case study of six children aged between 6:00 and 7:11 diagnosed with phonological disorder. All children underwent speech-language assessment, with specific assessment of phonology through the Child's Phonological Assessment (Yavas, Hernandorena e Lamprecht, 2002) and temporal processing skills through the temporal ordering and resolution tests, before and after therapy. All subjects underwent 25 therapy sessions, with three subjects undergoing only phonological therapy (study group 1 - SG1), and the other three went through phonological therapy associated with computerized auditory training (study group 2 - SG2), using the software Escuta Ativa. Results: There was no statistically significant difference when comparing the results of performance in tasks concerning the perception of the phonological system and the temporal processing in the same tests (set of tasks of listening skills) applied before and after therapy of SG1 with SG2. However, there was an improvement in the performance of temporal processing tests for SG2. Conclusion: Phonological therapy combined with computerized auditory training in children with phonological disorders did not show a statistically different difference, however it did influence the increase in the performance of temporal processing tests in these children when compared to the isolated use of phonological therapy.


Introducción: el entrenamiento auditivo es un método de intervención utilizado para perfeccionar el desempeño de las habilidades auditivas alteradas, de modo que, puede promover mejora en el procesamiento de la información sonora. Objetivo: verificar la eficacia de la estimulación del procesamiento auditivo central en la rehabilitación de los trastornos fonológicos. Método: Fue realizado un estudio de casos de seis niños con edades de 6:00 a 7:11 años, con diagnóstico de trastorno fonológico. Todos los niños pasaron por evaluación fonoaudiológica, con evaluación específica de fonología por medio de la Evaluación Fonológica del Niño (Yavas, Hernandorena e Lamprecht, 2002) y por la evaluación de las habilidades de procesamiento temporal por medio de los test de coordinación y resolución temporal, pre y post terapia. Todos los sujetos pasaron por 25 sesiones de terapia, siendo que tres sujetos fueron sometidos a la terapia puramente fonológica (grupo de estudio 1 - GE1),y los otros tres niños recibieron terapia fonológica asociada al entrenamiento auditivo computarizado (grupo de estudio 2 - GE2), con el software Escuta Ativa. Resultados: No hubo diferencia estadísticamente significativa cuando comparados los resultados del desempeño en tareas concernientes a la percepción del sistema fonológico y del procesamiento temporal en los mismos tests(conjunto de tareas con habilidades auditivas)aplicados pre y post terapia del GE1 con el GE2. Sin embargo, se observó mejora en el desempeño de las pruebas de procesamiento temporal en GE2. Conclusión: La terapia fonológica combinada con el entrenamiento auditivo computarizado en niños con Trastorno Fonológico no evidenció diferencia estadísticamente diferente, sin embargo demostró influir en el aumento del desempeño de las pruebas de procesamiento temporal de estos niños cuando comparado al uso aislado de la terapia fonológica.

ABSTRACT

Infants usually say their first word at the age of 12 months; subsequently, within the next 6-12 months, they develop a vocabulary of approximately 50 words, along with the ability to make two-word combinations. However, late talkers (LTs) demonstrate delayed speech in the absence of hearing impairments, cognitive developmental issues or relevant birth history. The prevalence of late language emergence (LLE) in toddlers is reported to be 10-15%. Studies of LTs are both theoretically and clinically significant. Early diagnosis and clinical intervention may result in relatively stable speech capabilities by the early school years. The present article aimed to review both theoretical and empirical studies regarding LLE within the process of first language acquisition, as well as methods for the early diagnosis of delayed speech in children and the authors' own clinical and theoretical recommendations.

ABSTRACT Purpose: to describe the speech therapy aspects of patients treated by the palliative care team in a hospital. Methods: an observational and cross-sectional study, performed from medical records of patients treated under Palliative Care Program, in a hospital, from July to September 2018. Information from the anamnesis and speech-language assessments, which were analyzed by frequency measures, were collected. Results: the sample was composed by 41 medical records, including 25 males and 16 females, with an average age of 61.2 years and hospitalization average time of 20.7 days. Oral feeding was present in 73% of the sample. It was observed that 24% of the patients had impaired expressive language, 56% had reduced maximum phonation times and 34% showed altered mobility phonoarticulatory organs. For swallowing, 22% showed difficulty in some consistency. A nutritional feeding was verified in 74% of the sample and the remaining was making use of comfort feeding. In relation to assistance, 46% of the sample was under management, 7% in therapy, and the remaining did not have follow-up indication. Conclusion: relevant alterations to orofacial motricity, voice, language and swallowing were found in patients under palliative care.


RESUMO Objetivo: caracterizar aspectos fonoaudiológicos de pacientes atendidos pela equipe de cuidados paliativos em complexo hospitalar. Métodos: estudo transversal, observacional, realizado a partir de dados de prontuários de pacientes internados pelo Programa de Cuidados Paliativos de um hospital, no período entre julho e setembro de 2018. Foram coletadas informações da anamnese e avaliações fonoaudiológicas, as quais foram analisadas por medidas de frequência para as variáveis categóricas e medidas de tendência central e dispersão para as variáveis contínuas. Resultados: a amostra foi composta por 41 prontuários, referentes a 25 homens e 16 mulheres, com média de idade de 61,2 anos e tempo médio de internação de 20,7 dias. A alimentação oral esteve presente em 73% da amostra. Observou-se que 24% dos pacientes apresentavam linguagem expressiva prejudicada, 56% apresentaram tempos máximos de fonação reduzidos e 34% mobilidade de órgãos fonoarticulatórios alterada. Quanto à deglutição, 22% apresentaram dificuldade em alguma consistência. Dieta para nutrição foi verificada em 74% da amostra e o restante fazia uso de dieta para conforto. Sobre a assistência, 46% da amostra estava em gerenciamento, 7% em terapia, demais sem acompanhamento. Conclusão: foram encontradas alterações pertinentes à motricidade orofacial, voz, linguagem e deglutição em pacientes em cuidados paliativos.

ABSTRACT

El labio fisurado se define como una anomalía congénita de la cabeza y el cuello, observándose una mayor incidencia en personas del sexo masculino. El tratamiento por excelencia es la intervención quirúrgica, en la búsqueda de una adecuada funcionalidad y un buen resultado estético, para cumplir con este objetivo es necesario el trabajo interprofesional, en el cual participa el fonoaudiólogo, durante el proceso de evaluación e intervención terapéutica. El objetivo de la revisión narrativa fue abordar principalmente los procesos de evaluación e intervención fonoaudiológica en niños con labio fisurado. Para su desarrollo fue realizada una búsqueda de información en las bases de datos PubMed, Scopus y Lilacs, posteriormente fue realizado un análisis crítico de la información. En la presente revisión narrativa se proponen realizar un recuento sobre aspectos relacionados con la historia de la clasificación del labio fisurado, factores etiológicos, procesos de intervención quirúrgica realizada en los niños con labio fisurado, que permitan comprender el proceso de la evaluación clínica fonoaudiológica en las áreas de deglución, lenguaje, habla, voz y las propuestas de intervención terapéutica en niños con labio fisurado de acuerdo con las evidencias presentadas en la literatura.


Cleft lip is defined as a congenital anomaly of head and neck, with a higher incidence in males. The treatment par excellence is surgical intervention seeking to achieve functionality and the best aesthetic result) for this the interprofessional work is necessary, in which the speech pathologist participates on the evaluation process and therapeutic intervention in swallowing, speech and language. The objective of this narrative review was to primarily address the processes of evaluation and speech therapy intervention in children with cleft lip. For its development, a search of information in PubMed, Scopus and Lilacs databases, subsequently a critical analysis of the information was made. This narrative review proposed to make a synthesis of the history, the etiological factors and the surgical intervention processes that are performed in children with cleft lip, this allows to understand the actions and importance of the speech pathologist's work in the processes of evaluation with specific protocols that involve all areas; swallowing, language, speech, voice, hearing and speech pathologist therapy intervention in children with cleft lip, based on the evidence that allows to acknowledge positive results, being promising for this population the implementation of summer camps

RESUMO Objetivo Verificar a efetividade da intervenção fonoaudiológica grupal no comportamento comunicativo de indivíduos com diagnóstico de esquizofrenia. Método Trata-se de um estudo quase experimental, quantitativo analítico-exploratório. Foram incluídos usuários de um Centro de Atenção Psicossocial III (CAPS III) com diagnóstico de esquizofrenia, divididos em 2 grupos: Grupo Experimental (GE), compondo o Grupo de Intervenção Fonoaudiológica (GIF) e Grupo Controle (GC). O comportamento comunicativo foi avaliado através da Bateria MAC Breve. O GIF foi realizado em 2 sessões semanais, totalizando 24 sessões. Após esse período, os indivíduos foram reavaliados. A análise ocorreu por meio dos Testes não paramétricos de Mann Whitney e o Teste de Correlação de Pearson. Resultados Participaram 19 indivíduos, de ambos os sexos, com idade entre 19 e 59 anos, escolaridade mínima de 5 anos, sendo que 14 participaram do GE e 5 do GC. No GE, foi possível observar que houve melhora no comportamento comunicativo após a intervenção fonoaudiológica em todas as tarefas avaliadas, exceto na tarefa de Escrita. Já no GC, não foram observadas alterações significativas comparando a avaliação e a reavaliação após 12 semanas. Conclusão A intervenção fonoaudiológica grupal foi efetiva, utilizando a comunicação como instrumento de socialização e contribuindo para a melhoria das condições de vida de indivíduos com diagnóstico de esquizofrenia.


ABSTRACT Purpose To verify the effectiveness of the speech language intervention in the communicative behavior in group of individuals diagnosed with schizophrenia. Methods This is a semi-experimental, quantitative analytical-exploratory study. Users of a Psychosocial Care Center III (CAPS III) with a diagnosis of schizophrenia were included, divided into 2 groups: Experimental Group (EG), comprising the Speech Therapy Intervention Group (STIG) and Control Group (CG). The communicative behavior was evaluated through the Brief MAC Battery. The STIG was performed in 2 weekly sessions, during 12 weeks, totalizing 24 sessions. After this period, individuals were reassessed. Data were analyzed through Mann Whitney non-parametric Test, and Pearson's Correlation Test. Results A total of 19 individuals of both sexes participated, who are between 19 and 59 years old with a minimum schooling of 5 years, 14 participating in EG and 5 in CG. In the EG, it was possible to observe that there was improvement in the communicative behavior after the speech language intervention in all the tasks evaluated, except in the writing task. In CG, no significant changes were observed comparing evaluation and reevaluation after 12 weeks. Conclusion The speech-language intervention in group was effective as a socialization tool and contributing to the improvement of the living conditions of these people with schizophrenia.

RESUMO Com o crescimento da população idosa, evidencia-se um aumento da incidência de doenças comuns a essa faixa etária, como as demências. Esforços para aprimorar a qualidade dos cuidados em saúde a esses pacientes, incluindo os cuidados fonoaudiológicos, têm crescido. Este estudo objetiva avaliar a efetividade e a aplicabilidade do aplicativo Talk Around It como estratégia complementar na terapia fonoaudiológica em um caso de distúrbio cognitivo da comunicação. O participante foi avaliado pré e pós-terapia fonoaudiológica por meio de protocolos padronizados de avaliação da linguagem. A terapia teve como foco principal a redução de anomias. O aplicativo Talk Around It foi selecionado como recurso terapêutico complementar para alcançar esse objetivo. Na avaliação pós-terapia observou-se melhora ou manutenção dos escores dos subtestes da Bateria Montreal-Toulouse de Avaliação da Linguagem-Brasil: Nomeação oral (substantivos, verbos e total), Fluência verbal semântica e ortográfica e Discurso narrativo oral (unidade de informação e cenas). A avaliação funcional das habilidades comunicativas não se modificou de modo consistente após a intervenção O recurso tecnológico empregado com estratégias terapêuticas convencionais, durante três meses, conduziu a melhoras da habilidade de acesso lexical no caso estudado. Seu uso na prática clínica em pacientes com demência em fase leve mostrou-se possível.


ABSTRACT With the aging of the population, there is an increase in the incidence of common diseases to this age group, such as dementias. Efforts to improve the quality of health care for these patients, including speech-language therapy, have grown. This study aims to evaluate the effectiveness and applicability of the Talk Around It as a complementary strategy in the language therapy of a patient with cognitive communication disorder. The participant was evaluated before and after speech therapy through standardized language assessment protocols. The main focus of therapy was the reduction of anomies. The Talk Around It application has been selected as a complementary therapeutic resource to achieve this goal. In the post-therapy evaluation improvement or maintenance of the Montreal-Toulouse Battery for Language Assessment-Brazil Battery subtests scores was observed: Oral naming (nouns, verbs and total), Semantic and Orthographic verbal fluency and Oral narrative discourse (information unit and scenes). Functional assessment of communicative skills has not changed consistently after the intervention. The technological resource used with conventional therapeutic strategies, during three months, led to improvements in the lexical access ability in the case studied. It use in clinical practice in patients with mild dementia has proved possible.

RESUMO Objetivos Revisar sistematicamente os protocolos e/ou avaliações que contribuem para o diagnóstico de apraxia de fala na infância (AFI) e classificá-los de acordo com a dimensão clínica avaliada. Estratégia de pesquisa Estudo de revisão sistemática da literatura nas bases de dados MEDLINE (acessado via PubMed), LILACS, Scopus e SciELO, com os descritores Apraxias, Childhood apraxia of speech, Evaluation, Assessment, Validation Studies, Evaluation Studies, Language Therapy, Rehabilitation of Speech and Language Disorders, Child e Child, Preschool. Critérios de seleção A busca nas bases de dados foi conduzida por três pesquisadores independentes. Foram incluídos estudos que avaliavam, de forma clara, sujeitos com suspeita ou diagnóstico de AFI. Os revisores realizaram a coleta de dados no que diz respeito às características metodológicas, intervenções e desfechos dos estudos, por meio de planilhas previamente elaboradas especificamente para o presente estudo. O dado principal coletado foi referente aos procedimentos de avaliação da AFI para crianças. Resultados A maior parte dos estudos (14 dos 21 incluídos) realizou a associação entre a avaliação de habilidades motoras e/ou articulatórias e segmentais. Cinco realizaram avaliação de todos os aspectos elencados: motor e/ou articulatória, segmental e suprassegmental e dois realizaram apenas avaliação motora e/ou articulatória. A idade dos sujeitos variou de 3 a 12 anos. Conclusão A maioria das pesquisas considerou a associação entre habilidades motoras e/ou articulatórias e segmentais para avaliação da apraxia de fala na infância. Sugere-se a realização de mais estudos, a fim de buscar evidências de validade.


ABSTRACT Purpose Systematically review the protocols and/or assessments that contribute to the diagnosis of CAS and classify them according to the clinical dimension evaluated Research strategy Study of systematic literature review in the databases MEDLINE (accessed via PubMed), LILACS, Scopus and SciELO with the descriptors Apraxias, Childhood apraxia of speech, Evaluation, Assessment, Validation Studies, Evaluation Studies, Language Therapy, Rehabilitation of Speech and Language Disorders, Child and Child, Preschool. Selection criteria The search for scientific articles in the databases was conducted by three independent researchers. Studies that clearly assessed subjects with suspected or diagnosed PIA were included. The reviewers performed data collection with regard to methodological characteristics, interventions and study outcomes using standardized forms. The main data collected was related to the assessment procedures of CAS. Results Most studies (14 of the 21 included) made an association between the assessment of motor and/or articulatory and segmental skills. Five performed an evaluation of all listed aspects: motor and/or articulatory, segmental and suprasegmental; and two underwent only motor and/or articulatory assessment. The age of the subjects in the present study ranged from 3 to 12 years. Conclusion The assessment of CAS generally involves the association between the assessment of motor and/or articulatory and segmental skills. It is suggested that further studies in order to evidence validity for the assessment of CAS.

RESUMO Pessoas com demência apresentam alterações cognitivas, de linguagem e de comportamento. As alterações da linguagem podem ser identificadas no discurso desses pacientes já nas fases iniciais. Contudo, ainda há poucas pesquisas sobre a efetividade da terapia fonoaudiológica nesses casos. Os objetivos deste estudo foram descrever as manifestações linguísticas presentes no discurso de um paciente com distúrbio linguístico-cognitivo e analisar a efetividade da terapia fonoaudiólogica. Foi realizado um estudo de caso único longitudinal, com participante com demência mista em fase leve à moderada, submetida à avaliação pré e pós-terapia fonoaudiológica. Foram utilizados os subtestes de Compreensão oral e de Discurso narrativo oral da Bateria MTL-Brasil e o teste The Dog Story. Na avaliação, a paciente apresentou alteração grave da compreensão oral e alteração nas dimensões micro e macrolinguística do discurso, obtendo pontuação 2/10 no índice de avaliação do discurso oral. Após a terapia, a paciente obteve resultados dentro do esperado nos subtestes de Compreensão oral (palavras, frases e texto) e de Discurso narrativo oral (número de palavras e unidades de informação) da Bateria MTL-Brasil. No teste The Dog Story, a participante obteve melhor desempenho na produção do discurso em relação aos itens alteração lexical e complexidade sintática (dimensão microlinguística) e macroproposição (dimensão macrolinguística), alcançando escore de 5/10 no índice. É possível afirmar que a terapia fonoaudiológica foi efetiva a curto prazo para esta paciente com distúrbio linguístico-cognitivo e alterações no nível discursivo da linguagem.


ABSTRACT People with dementia present cognitive, language and behavioral impairments. The language deficits can be identified in the speech of these patients in the early stages. However, there are still few studies about the effectiveness of speech-language therapy in these cases. This study aimed to describe the language manifestations observed in the speech of a patient with a linguistic-cognitive disorder and to analyze the effectiveness of speech-language therapy. A longitudinal single-case study with a patient with mild to moderate mixed dementia was carried out. The patient underwent pre- and post-speech-language therapy evaluation with the Oral comprehension and the Narrative discourse subtests of the MTL-Brazil Battery and The Dog Story test. We observed severe impairments of the oral comprehension and deficits involving the micro and macrolinguistic dimensions of the discourse, with a score of 2/10 in the oral discourse evaluation index. After the speech-language therapy, the patient achieved adequate performance in the Oral Comprehension subtests (words, sentences and text) and the Oral Narrative discourse (number of words and information units) of the MTL-Brazil Battery. In The Dog Story test, the participant obtained better performance in the discourse production regarding the items lexical change and syntactic complexity (microlinguistic dimension) and macroproposition (macrolinguistic dimension), achieving a score of 5/10 on the index. It is possible to state that the speech-language therapy was effective in the short term for this patient with linguistic-cognitive disorder and impairments in the discursive level of the language.

RESUMO Objetivo Investigar a eficácia de uma proposta de intervenção baseada em leitura compartilhada de histórias para a compreensão leitora, narrativa escrita e leitura/escrita de palavras. Método 44 crianças divididas em dois grupos, segundo o desempenho destas em uma prova de compreensão leitora - G1: crianças com dificuldade em compreensão leitora; G2: sem dificuldades. Todas as crianças foram avaliadas em relação à leitura e escrita de palavras isoladas, compreensão leitora de frases e narrativa escrita. Após avaliação as crianças do G1 participaram de um programa de intervenção (15 encontros) que estimulava a leitura compartilhada de histórias. Após a intervenção todas as crianças (G1 e G2) foram reavaliadas. Os resultados foram comparados intra e intergrupos, sendo aplicados testes estatísticos pertinentes, adotando o nível de significância menor que 0,05. Resultados Após o programa de intervenção as crianças do G1 apresentaram melhora significativa nas variáveis avaliadas. Na análise intergrupos, no momento da avaliação, G1 era diferente do G2 em leitura/escrita, compreensão leitora, capacidade julgar corretamente os verbos e em coerência textual. Na reavaliação, G1 se igualou ao G2 em coerência textual e, se aproximou do G2 em leitura/escrita de apalvras isoladas e compreensão leitora. Conclusão A leitura compartilhada de histórias promoveu o desenvolvimento das habilidades de baixo e alto nível de leitura e escrita, sendo mais eficaz para as estratégias de alto nível (exemplo, narrativa escrita). Sugere-se que, para questões ortográficas, um programa específico para este aspecto seja associado.


ABSTRACT Purpose to investigate the effectiveness of an intervention proposal based on shared story reading for reading comprehension, written narrative and word reading/writing. Methods 44 children were divided into two groups according to their performance in a reading comprehension test - The first group, G1, consisted of children with reading comprehension difficulty, and the second, G2, consisted of children who did not have difficulties. All children were evaluated regarding reading/writing isolated words, reading comprehension of sentences, and written narrative. After this evaluation, G1 children participated in an intervention program (15 meetings) that stimulated shared story reading. After this intervention, all children were reevaluated. Intra- and intergroup data were analyzed statistically by applying appropriate statistical tests, with the level of significance set at >0.05%. Results after the intervention program, G1 children showed significant improvement in the variables evaluated. In the intergroup analysis, at the time of evaluation, G1 differed from G2 in reading/writing, reading comprehension, ability to correctly judge the verbs, and in textual coherence. At reassessment, G1 matched G2 in textual coherence and approached G2 in reading/writing isolated words and reading comprehension. Conclusion shared story reading promoted the development of low and high level reading and writing skills, being more effective for high-level strategies (example: written narrative). It is suggested that a specific program for orthographic questions be associated.

ABSTRACT

National guidance recommends the involvement of speech and language therapists in intensive care particularly for those requiring tracheostomy and ventilation. However, the role of speech and language therapists is poorly understood especially in the context of critical care. This article aims to increase awareness of the background training and skills development of speech and language therapists working in this context to demonstrate their range of specialist abilities. Speech and language therapists support and enhance the process of laryngeal weaning alongside the rehabilitation of speech and swallowing as part of the multidisciplinary team. Examples are provided of the types of interventions that are used and technological innovations that may enhance rehabilitation of oropharyngeal impairments.

BACKGROUND: Alteration in tongue mobility requires rehabilitation work through oral motor exercises. These exercises can be integrated with computer games to increase the patient's motivation during treatment. OBJECTIVE: To investigate the influence of tongue mobility on children's motor performance in a computer game reliant on lingual movements. METHODS: A cross-sectional descriptive observational study was carried out with 16 children with altered tongue mobility and 16 children with normal tongue mobility. The subjects were between 8 and 12 years of age. They underwent a clinical evaluation of the tongue and performed an activity using an intra-oral joystick controlled by the tongue to play a simple computer game. The game consisted of targets appearing on the screen that the participants had to reach by moving the joystick control rod. Afterward, the participants answered a feedback questionnaire. Motor performance in the game, measured by the number of reached targets and by the time to reach the targets, was compared between groups and across directions of tongue movement and order of appearance of the targets. RESULTS: The group with altered tongue mobility presented a higher time to reach the target in the downward direction and in the first and last 12 targets and a lower number of targets reached in the left direction, upward direction, and in the first 12 targets than the control group. The direction of the movement influenced tongue performance in both groups. CONCLUSION: Children with altered tongue mobility exhibited a worse performance than those with normal tongue mobility.

Abstract Introduction: The patient's evolution in the audiology and speech-language clinic acts as a motivator of the therapeutic process, contributing to patient adherence to the treatment and allowing the therapist to review and/or maintain their clinical therapeutic conducts. Electrophysiological measures, such as the P300 evoked potential, help in the evaluation, understanding and monitoring of human communication disorders, thus facilitating the prognosis definition in each case. Objective: To determine whether the audiology and speech-language therapy influences the variation of P300 latency and amplitude in patients with speech disorders undergoing speech therapy. Methods: This is a systematic review with meta-analysis, in which the following databases were searched: Pubmed, ScienceDirect, SCOPUS, Web of Science, SciELO and LILACS, in addition to the gray literature bases: OpenGrey.eu and DissOnline. The inclusion criteria were randomized or non-randomized clinical trials, without language or date restriction, which evaluated children with language disorders undergoing speech therapy, monitored by P300, compared to children without intervention. Results: The mean difference between the latencies in the group submitted to therapy and the control group was −20.12 ms with a 95% confidence interval of −43.98 to 3.74 ms (p = 0.08, I 2 = 25% and p value = 0.26). The mean difference between the amplitudes of the group submitted to therapy and the control group was 0.73 uV with a 95% confidence interval of −1.77 to 3.23 uV (p = 0.57, I 2 = 0% and p value = 0.47). Conclusion: The present meta-analysis demonstrates that speech therapy does not influence the latency and amplitude results of the P300 evoked potential in children undergoing speech therapy intervention.


Resumo Introdução: A evolução do paciente na clínica fonoaudiológica atua como fator motivador do processo terapêutico, contribui para a sua adesão ao tratamento e possibilita ao terapeuta a revisão e/ou a manutenção de suas condutas. As medidas eletrofisiológicas, como o potencial evocado P300, auxiliam na avaliação, na compreensão e no monitoramento dos distúrbios da comunicação humana, facilitam, dessa forma, a definição do prognóstico de cada caso. Objetivo: Determinar se a terapia fonoaudiológica influencia na variação da latência e da amplitude do P300 em pacientes com distúrbio de linguagem submetidos à terapia fonoaudiológica. Método: Revisão sistemática com metanálise, na qual foram feitas buscas nas seguintes bases de dados: Pubmed, ScienceDirect, Scopus, Web of Science, SciELO e Lilacs, além das bases de literatura cinzenta: OpenGrey.eu e DissOnline. Foram considerados critérios de inclusão: ensaios clínicos aleatórios ou não, sem restrição de idiomas ou data, que submeteram crianças com distúrbio de linguagem à terapia fonoaudiológica, monitoradas pelo P300, comparadas a crianças sem intervenção. Resultados: A diferença média entre as latências do grupo submetido à terapia e do grupo controle foi de -20,12 ms com intervalo de confiança 95% entre -43,98 e 3,74 ms (p = 0,08; I2 = 25% e o valor de p = 0,26). A diferença média entre as amplitudes do grupo submetido à terapia e do grupo controle foi de 0,73 uV com intervalo de confiança de 95% entre -1,77 e 3,23 uV (p = 0,57; I2 = 0% e o valor de p = 0,47).
