ABSTRACT

In order to promote the innovative application of Sanjiao theory and Yingwei theory, this paper tries to apply the ''Sanjiao-Yingwei'' Qi transformation theory to the treatment of tumor diseases, integrating it with T cell exhaustion mechanism to elaborate on its scientific connotation and using network pharmacology and bioinformatics to elucidate the correlation between the anti-tumor mechanism of ''Sanjiao-Yingwei'' Qi transformation and T cell exhaustion. The ''Sanjiao-Yingwei'' Qi transformation function is closely related to the immunometabolic ability of the human body, and the ''Sanjiao-Yingwei'' Qi transformation system constitutes the immunometabolic exchange system within and outside the cellular environment. Cancer toxicity is generated by the fuzzy Sanjiao Qi, and the long-term fuzzy Sanjiao Qi is the primary factor leading to T cell exhaustion, which is related to the long-term activation of T cell receptors by the high tumor antigen load in the tumor microenvironment. Qi transformation malfunction of the Sanjiao produces phlegm and collects stasis, which contributes to T cell exhaustion and is correlated with nutrient deprivation, lipid accumulation, and high lactate levels in the immunosuppressed tumor microenvironment, as well as with the release of transforming growth factor-β and upregulated expression of programmed death receptor-1 by tumor-associated fibroblasts and platelets in the tumor microenvironment. Ying and Wei damage due to Sanjiao Qi transformation malfunction is similar to the abnormal manifestations such as progressive loss of exhausted T cell effector function and disturbance of cellular energy metabolism. Guizhi decoction, Shengming decoction, and Wendan decoction can correct T cell exhaustion and exert anti-tumor effects through multi-target and multi-pathways by regulating ''Sanjiao-Yingwei'' Qi transformation, and hypoxia inducible factor-1α (HIF-1α) may be one of the main pathways to correct T cell exhaustion. It was found that HIF-1α may be one of the important prognostic indicators in common tumors by bioinformatics. The use of the ''Sanjiao-Yingwei'' Qi transformation method may play an important part in improving the prognosis of tumor patients in clinical practice.

Insomnia induced by mental retardation is related to the synergistic dysfunction of "shaoyang, sanjiao and haishi (the period of the day from 9 pm to 11 pm) " in pathogenesis. Based on "trinity theory of shaoyang, sanjiao and haishi ", the acupoints of hand and food shaoyang meridians on the head and face (Fengchi [GB 20], Sizhukong [TE 23] and Jiaosun [TE 20]) and those on the four limbs (Sanyangluo [TE 8], Zhongzhu [TE 3], Qiuxu [GB 40] and Xiaxi [GB 43]) are selected to regulate the pivot function of shaoyang. Front-mu and back-shu points (Sanjiaoshu [BL 22], Danzhong [CV 17] and Tianshu [ST 5]), as well as auricular points (Gan [CO12], Dan [CO11] and Sanjiao [CO17]) are chosen to promote qi circulation of shaoyang and sanjiao meridians. In association with the relevant knowledge of time acupuncture at haishi, the circadian rhythm of sleep is set up and the appropriate sleep pattern is developed.

ABSTRACT

Review of historical literature showed that the understanding of the indicated disease location of Modified FuMai Decoction (复脉汤加减方) has evolved from the upper jiao (焦) to the middle and lower jiao. Initially, it is used for the treatment of yin deficiency of both yin and yang in the upper jiao, changes to supplement stomach and produce fluids in the middle jiao, and is used to protect yin, clear the pathogens, conslidate yin and subdue yang so as to store the true yin of lower jiao. The unchanging principle of Fumai Decoction modifications is nourishing yin, while the changing aspects are determining the secondary treatment methods based on disease location of sanjiao, concomitant disease natures, internal injury or external contraction, warm disease or cold damage, thereby choosing the corresponding added or subtracted herbs, and providing reference for the application of classical formulas.

"Unblocking fu organs" is one of the essential principles of Ma's warm moxibustion technique, characterized as "dredging" and "harmonizing" for either deficiency or excess condition. Under the guidance of this therapeutic thought, the acupoints for moxibustion are mainly selected from the middle and lower parts of the body. Regarding the therapeutic approach, the acupoint prescription for moxibustion should be formed in line with warming and promoting circulation of fu organs; the moxibustion degree should be specially considered, in which, the mild moxibustion is recommended to induce promoting action; and the systematic moxibustion technique should be the root for dredging fu organs and regulating zang organs. Ma's mild moxibustion technique stresses on removing the obstruction of fu organs and emphasizes promoting the qi activity of sanjiao (triple energizer) and regulating the balance of five zang organs.

ABSTRACT

Ovarian cancer has the highest mortality among the three major gynecological malignancies. Peritoneal metastasis, intestinal obstruction and then death is the main outcome pattern of advanced ovarian cancer. Traditional Chinese medicine maintenance treatment of advanced ovarian cancer is effective, but the theoretical basis needs to be further improved. Under the guidance of the membranous Sanjiao theory and combined with clinical experience, this paper traces the origin, focuses the location of membranous Sanjiao of ovarian cancer peritoneal metastasis and the role of membranous Sanjiao dysfunction in the occurrence and metastasis of ovarian cancer. Besides, the paper puts forward that the main strategies for the maintenance treatment of traditional Chinese medicine on ovarian cancer are ordering the Qi movement of Sanjiao, regulating and harmonizing Qi and blood, and clearing away the latent toxin. From the perspective of membranous Sanjiao theory, the theory enriches the traditional Chinese medicine theory of ovarian cancer and provides new ideas for the treatment of ovarian cancer.

ABSTRACT

Background: Vascular dementia (VD) is the second most common type of dementia after Alzheimer's disease, but there is a lack of definitive treatment for VD. Acupuncture treatment is effective in improving the cognitive impairment and behavioral capacity of patients with VD. In recent years, more studies indicated that peripheral inflammation and abnormal peripheral immune function may aggravate neuroinflammation and cognitive dysfunction. However, there are few studies about the acupuncture and the abnormal peripheral immune function of VD. Also, few studies concern the regulating effect of acupuncture on peripheral immunity of patients with VD. Objective: The aim of this study was to explore the effect of the "sanjiao" acupuncture method on peripheral immunity of patients with mild to moderate VD. Methods: A total of 30 patients with VD were involved in the acupuncture group (AG), which was treated with the "sanjiao" acupuncture method once a day for six times a week and lasted for 12 weeks, and 30 healthy elderly people were assigned to the normal group (NG), which had no treatment. The distribution of lymphocyte subsets and the levels of some inflammatory cytokines in the peripheral blood of subjects were evaluated using the flow cytometry (FCM) and the enzyme-linked immunosorbent assay (ELISA). Results: A total of 60 subjects were involved in this study, while 58 subjects completed the entire trial. Before treatment, the levels of CD3+ T, CD4+ T cells, CD4+/CD8+, Tregs, B cells, IFN-γ, and IL-10 in patients with VD were significantly decreased compared with the normal group (all P < 0.05 or P < 0.01). The level of TNF-α in peripheral blood of patients with VD was significantly increased (P < 0.01). After acupuncture treatment, the levels of CD3+ T, CD4+ T cells, and IFN-γ were significantly increased (all P < 0.05 or P < 0.01). The level of TNF-α was significantly decreased (P < 0.01). The proportion of Tregs was increased (P < 0.01), but it was still lower than that of the normal group (P < 0.05). Conclusion: The acupuncture method can increase the proportion of CD3+, CD4+ T cells, and Tregs in peripheral blood of patients with VD. And, it reduces the levels of pro-inflammatory factor TNF-α, which achieves the anti-inflammatory effects and immunostimulation. It suggests that acupuncture can improve the peripheral immune dysfunction of patients with VD by regulating the distribution of lymphocyte subsets and the levels of inflammatory cytokines. ABSTRACT

OBJECTIVE: To observe the effect of Sanjiao acupuncture(triple energizer acupuncture)on the small G protein guanosine triphosphate enzyme subfamily protein RhoA/Rho kinase (ROCK) pathway in Alzheimer's disease mice, and explore its effect on learning and memory function and neurosynaptic plasticity. METHODS: Forty SAMP8 senile dementia mice were randomly divided into model, Sanjiao acupuncture (acupuncture), non acupoint acupuncture (non-acupoint) and fasudil groups, with 10 mice in each group, another 10 SAMR normal aging mice were selected as normal aging group. Mice in the acupuncture group were treated with acupuncture intervention on "Danzhong"(CV18), "Zhongwan"(CV13), "Qihai"(BL24) and bilateral "Xuehai"(SP10) and "Zusanli" (ST36). Mice in the non-acupoint group were treated with acupuncture at each of the left and right non-acupoints under the ribs and mice in the fasudil group were intraperitoneally injected with fasudil (25 mg/kg). The mice in each group were given medicine or acupuncture on the second day after grouping for 28 continuously days, once a day. Morris water maze test was used to test the learning and memory ability of mice. HE staining was used to observe the pathological changes of neurons in hippocampus. The number of hippocampal neuron dendritic spine was detected by FD fast Golgi staining kit. The contents of ß-amyloid 42 (Aß42) and phosphorylated tau protein (p-tau) in hippocampus were detected by ELISA. Western blot was used to detect the protein relative expression levels of RhoA, ROCK, F-actin and p-cofilin in hippocampus. RESULTS: Compared with those in the normal aging group, the hippocampal neurons of the model group were disorderly arranged, the number of neuron was reduced, the escape latency, hippocampal Aß42 and p-tau contents, RhoA and ROCK protein expressions increased (P<0.05), the number of crossing the original platform, the number of neuronal dendritic spines, expressions of F-actin and p-cofilin decreased (P<0.05). After the interventions, there was no statistically significant difference in the above indicators in the non-acupoint group relevant to the model group (P>0.05). The acupuncture group and fasudil group improved the hippocampal pathological damage. The escape latency, hippocampal Aß42 and p-tau contents, the expressions of RhoA and ROCK protein decreased (P<0.05), and the number of crossing the original platform, the number of hippocampal neuron dendritic spines, expressions of F-actin and p-cofilin increased (P<0.05) in both of the acupuncture and fasudil groups in contrast to the model and non-acupoint groups. Compared with the acupuncture group, there was no significant difference in the above indicators in the fasudil group (P>0.05). CONCLUSION: Sanjiao acupuncture may inhibit the activation of the RhoA/ROCK pathway, so as to improve the learning and memory function of AD mice, increase the number of hippocampal neuron dendritic spines, and promote synaptic plasticity.

ABSTRACT

Objective:To investigate the renoprotective effects that Sanjiao Qushi prescription ameliorates cationic bovine serum albumin (C-BSA) induced membranous nephropathy(MN) in mice model and its influence on nuclear factor erythroid-2-related factor-2(Nrf2)/heme oxygenase-1(HO-1) signaling pathway. Method:Sixty female BALB/c mice were randomly divided into the normal group(<italic>n</italic>=10) and the model group(<italic>n</italic>=50). The mice in the model group received C-BSA injection via tail vein (6.5 mg·kg<sup>-1</sup>). The mice that were successfully modeled were randomized into the model group, the low dose Sanjiao Qushi prescription group(3.71 g·kg<sup>-1</sup>), the high dose Sanjiao Qushi prescription group(7.42 g·kg<sup>-1</sup>) and benazepril hydrochloride group(1.3 mg·kg<sup>-1</sup>). And they were administered with the corresponding medicine by gavage once a day for four consecutive weeks. 24 hour-urine protein quantitation were performed before C-BSA injection and after C-BSA injection as well as the medicine gavage. When the treatment was finished, all of the mice were sacrificed and the biochemical indicators such as serum creatinine(SCr), blood urea nitrogen(BUN), triglyceride(TG), total cholesterol(TC), total protein(TP) and albumin(Alb) were measured. And the renal pathological morphology changes were observed by light microscope with hematoxylineosin(HE), Masson and periodic acid-silver metheramine(PASM) staining. The deposition of immunoglobulin G(IgG) in the glomerulus was detected by fluorescence microscope. The expression of reactive oxygen species (ROS) of kidney was detected by fluorescence immunoassay. The protein expression levels of Nrf2 in cell nucleus and cytoplasm and the downstream protein factors HO-1 and NADH quinone acceptor oxidoreductase 1(NQO1) were detected by Western blot. Result:Compare to normal group, the levels of 24 hour-urine protein quantitation, TG and TC significantly increased in model group(<italic>P</italic><0.01), while TP and Alb levels significantly decreased(<italic>P</italic><0.01). The model group exhibited enlarged volume of glomerular, significantly thickened glomerular basement membrane(GBM), fuchsinophilic protein deposition and spike formation through light microscope. Immunofluorescence staining for the model group exhibited granular deposition of IgG along the capillary wall. The expression of ROS in kidney significantly increased(<italic>P</italic><0.01). The protein expression levels of Nrf2 in cell nucleus significantly increased(<italic>P</italic><0.01), while Nrf2 in cytoplasm significantly decreased(<italic>P</italic><0.01).The protein expression levels of HO-1 and NQO1 significantly increased(<italic>P</italic><0.01). Compared to model group, the levels of 24 hour-urine protein quantitation, TG and TC significantly decreased in each treated group(<italic>P</italic><0.01), TP and Alb levels significantly increased(<italic>P</italic><0.05,<italic>P</italic><0.01). The pathological damages alleviated obviously. The expression of ROS in kidney significantly decreased(<italic>P</italic><0.01). The protein expression levels of Nrf2 in cell nucleus significantly decreased(<italic>P</italic><0.01), while Nrf2 in cytoplasm significantly increased(<italic>P</italic><0.05,<italic>P</italic><0.01). The protein expression levels of HO-1 and NQO1 significantly increased(<italic>P</italic><0.01). Conclusion:Sanjiao Qushi prescription worked on MN mice possibly by regulating related proteins in the Nrf2/HO-1 signaling pathway and relieving oxidative stress, thus decreasing 24 hour-urine protein and blood lipid, increasing serum protein, and alleviating the pathological damages to protect renal function and delay progress of the disease.

ABSTRACT

OBJECTIVE: To observe the therapeutic effect of "Tiaoyi Sanjiao" (regulating and tonifying the triple energizer)acupuncture and moxibustion in the treatment of cancer-related fatigue of advanced non-small cell lung cancer and observe its influence on lymphocyte count. METHODS: The cancer-related fatigue patients with advanced non-small cell lung cancer who met the inclusive criteria were randomly divided into 2 groups, with 77 cases in each group. Patients in the control group were treated with conventional Chinese and Western medicine. In the treatment group, on the base of the treatment as the control group, "Tiaoyi Sanjiao" acupuncture and moxibustion was applied(mild moxibustion at Danzhong [CV17], Zhongwan[CV12], Qihai[CV6], bilateral Zusanli[ST36], and acupuncture at bilateral Xuehai[SP10], Waiguan[SJ5], Taichong[LR3]). KPS score, Piper scale, percentage and absolute count of lymphocyte subsets of the two groups before and after treatment were observed. RESULTS: Compared with pre-treatment, the KPS scores of both groups were significantly increased after treatment (P<0.05); the behavioral dimension score of the Piper scale, and the percentage of B cells of the control group decreased significantly(P<0.05); the behavioral dimension, emotional dimension, sensory dimension scores, and total score of the Piper scale in the treatment group were significantly reduced(P<0.05), while the percentage of CD3+T cells and absolute counts of CD3+T cells, CD4+T cells and CD8+T cells of the treatment group were significantly increased (P<0.05). After treatment, in comparison with the control group, behavioral dimension score of the Piper scale in the treatment group decreased significantly(P<0.05); the percentage and absolute counts of B cells and CD4+T cells, the absolute count of CD3+T cells in the treatment group were up-regulated (P<0.05). CONCLUSION: "Tiaoyi Sanjiao" acupuncture and moxibustion can significantly alleviate the fatigue state and improve the quality of life in patients with advanced non-small cell lung cancer, which may be achieved by regulating the number of lymphocytes and improving immune function.

To discuss the pathogenesis of sanjiao and gynecological diseases, and to briefly analyze the theory of "regulate sanjiao acupuncture", the method of selecting acupoints and operating methods. In order to use the "regulate sanjiao" as the rule, Baihui (GV 20), Danzhong (CV 17), Zhongwan (CV 12), Tianshu (ST 25), Qihai (CV 6), Guanyuan (CV 4), Zhongji (CV 3), Zigong (EX-CA 1), Xuehai (SP 10), Sanyinjiao (SP 6), Baliao were selected as the main acupoints to cooperate with syndrome differentiation for the treatment of gynecological diseases, such as infertility, perimenopausal syndrome. "Regulate sanjiao acupuncture" is effective in the treatment of infertility and perimenopausal syndrome, which is worthy of clinical promotion and application.

This article reviewed the relevant research on concept, substance, functions, and clinical application of Sanjiao during recent years. The study believed that Sanjiao is the "big viscera" integrating functions of viscus;"Sanjiao is invisible" means it couldn't be described; "Sanjiao is visible" theory has made certain achievements;Sanjiao is secondary channel for running of primordial qi; Qi transformation of lower-jiao is the foundation of blood, essence, bones and brain; The function of Sanjiao is the function of autonomic nervous system; The dysfunction of qi transformation and the disorder of qi activity of Sanjiao are generally deemed as pathogenesis of various diseases. This study also pointed out that researchers should pay more attention to physiological characteristics and functions of Sanjiao, with a purpose to help guide clinical practice.

[Objective] To elucidate the original meaning, connotation and clinical significance of SanJiao.[Methods] Dig out the connotation of Sanjiao theory by means of genetic research methods, it's found that the Sanjiao includes not noly the Fu Sanjiao and the Sanjiao divided by parts of the body, but also hand Sanjiao and foot Sanjiao, pointing out emphatically the foot Sanjiao's along route and acupoints,diagnosis site, disease syndrome and treatment,and it expounds the physiology and pathology of Sanjiao.[Results]The Sanjiao Fu is visible stria connecting tissue,which has the storage and diarrhea function of Fu,every place will be smooth if the Sanjiao is smooth.The Sanjiao divided by parts of the body is unified in stomach,it's actually one Sanjiao.The ministerial fire of Sanjiao roots in the stomach,it is responsible for the Yang Qi of the entire body.The ministerial fire is responsible for digesting food and generating and transformating Qi of Ying and Wei,the upper side of Sanjiao is called upper burner,also called hand Sanjiao,being responsible for the heart and lung.It has 5 functions,supplying and distributing Wei Qi, governing the skin and muscle,passing through Ying Qi and Wei Qi,regulating channel of human body,being responsible for the function of striae.The lower side of Sanjiao is called lower burner,also called foot Sanjiao, being responsible for the gasification of the renal bladder.excreting two urines,conducting dross,it involves small intestine, large intestine, bladder and kidney.[Conclusion]Sanjiao is visible and nameable,it is rooted in the spleen and stomach, located in the muscle.The Sanjiao Fu is striae between the muscle.The ministerial fire of Sanjiao is responsible for the Yang Qi of the entire body,the middle burner is responsible for digesting food and generating and transformating Qi of Ying and Wei,the upper burner is responsible for the heart and lung,the lower burner is responsible for the bladder meridian.

ABSTRACT

OBJECTIVE:To observe the efficacy and safety of Sanjiao fuzheng mixture combined with concurrent chemoradiotherapy of paclitaxel and nedaplatin in the treatment of local advanced non-small cell lung cancer (NSCLC). METHODS:A total of 68 patients with local advanced NSCLC selected from our hospital during Jan. 2015 to Jan. 2017 were divided into control group and observation group according to random number table,with 34 cases in each group. Control group was given Paclitaxel injection 135 mg/m2 intravenously,d1+Nedaplatin for injection 75 mg/m2 intravenously,d3,21 d as a treatment course,for 2 courses;routine fractionated intensity modulated radiation therapy,2 Gy each time,5 times a week,60-70 Gy in total;given 2 cycles of primary chemotherapy continuously after radiotherapy. Observation group was additionally given Sanjiao fuzheng mixture 250 mL/d,divided into 3 times,till the end of treatment,on the basis of control group. Clinical efficacies were observed in 2 groups. The levels of nutritional indexes (BMI,PAB,ALB,Hb) and tumor markers (SCC-Ag,CEA,TK1, CYFRA21-10) before and after treatment were observed. The occurrence of ADR were recorded. RESULTS:There was no statistical significance in the total effective rate between 2 groups (observation group 82.35% vs. control group 73.53%)(P>0.05). After treatment,the levels of BMI,PAB,ALB and Hb in 2 groups were significantly lower than before treatment,but the observation group was significantly higher than the control group. The levels of SCC-Ag,CEA,TK1 and CYFRA21-1 in 2 groups were significantly lower than before treatment,and the observation group was significantly lower than the control group,with statistical significance (P<0.05). The incidence of Ⅲ-Ⅳ degree aleucocytosis,Ⅰ-Ⅱ degree hemoglobin reduction and thrombocytopenia in observation group were significantly lower than control group, with statistical significance (P<0.05). CONCLUSIONS:The efficacy of Sanjiao fuzheng mixture combined with concurrent chemoradiotherapy of paclitaxel and nedaplatin is similar to that of concurrent chemoradiotherapy of paclitaxel and nedaplatin for localadvanced NSCLC,which can improve nutritional status significantly,and reduce the incidence of ADR.

In the clinical practice, Professor Jiang Liangduo, a national senior Chinese medicine doctor, has created the theory of "sanjiao meridian stasis" from the theory of meridian dialectics and from the overall state. In this paper, the traditional Chinese medicine and Western medicine clinical characteristics of sanjiao meridian stasis theory which is often used by Professor Jiang Liangduo in the treatment of out-patient syndrome differentiation, were first studied and summarized to investigate its inherent regularity. First, the source of data and research methods were introduced, and then the Traditional Chinese Medicine Inheritance Support System was used with the method of data mining to retrospectively analyze the disease characteristics of Chinese and Western medicine in 279 patients with sanjiao meridian stasis diagnosed by Professor Jiang in 2014. Then the following main conclusions were made after research: sanjiao meridian stasis was more common in women as well as young and middle-aged population. Often manifested by prolonged treatment course, red tongue with yellowishfur, with good correlation between modern Western medicine diagnosis and TCM differentiation syndrome. The symptoms of sanjiao meridian stasis syndrome are mostly of heat syndromes, and middle-aged patients are the most common patients with stasis and stasis of sanjiao. Related information of Western medicine diagnosis can help to diagnose the "sanjiao meridian stasis".

ABSTRACT

Abdominal obesity is a core factor in and necessary condition of diagnosing metabolic syndrome. Abdominal fat contains two parts: subcutaneous fat and visceral fat. Both are related to hypertension, impaired fasting blood glucose, diabetes and metabolic syndrome. Current studies often focus on visceral fat, but subcutaneous fat accumulation also has an inseparable relationship. Physical and chemical changes in abdominal fat cells result in abdominal fat thickening and density increase and cause changes in secretion of adipokines in this process. The common action of the two aspects leads to a number of metabolic problems. The sanjiao dysfunction brings about abdominal accumulation of phlegm, heat and stagnant turbidity and infiltration of them into blood vessels, which are the basic pathogenesis of metabolic syndrome. Abnormal Dai Meridian function plays a key role in this process. Unblocking and regulating the Dai Meridian as a core means, promoting functional activities of qi in the whole body and especially in the middle jiao and restoring the sanjiao function help the clinical treatment of metabolic syndrome.

ABSTRACT

Objective To investigate the clinical efficacy of acupuncture treatment for tinnitus by point selection under the theory of “kidney and Sanjiao communication”. Method Fifty-three tinnitus patients were randomly allocated to two groups. The treatment group of 26 patients received acupuncture and the control group of 27 patients, medication. The course of treatment was four weeks. The Visual Analogue Scale (VAS) score and the Tinnitus Handicap Inventory (THI) score were recorded in the two groups before and after treatment. The improving effects on tinnitus symptoms were compared between the two groups. Result After two weeks of treatment, there was a statistically significant difference in the THI score (P0.05) between the treatment and control groups. After four weeks of treatment, both the VAS and THI scores were lower in the treatment group than in the control groups (P<0.05). After four weeks of treatment, the total efficacy rate was 92.3% in the treatment group and 81.5% in the control group; there was a statistically significant difference between the two groups (P<0.05). Conclusion The theory of “kidney and Sanjiao communication” can well guide clinical acupuncture treatment of tinnitus and improve the symptoms of tinnitus and the quality of life in the patients. Its therapeutic effect is superior to that of medication.
