Resumo Introdução A hipertensão arterial e o diabetes mellitus são enfermidades crônicas que necessitam de cuidados contínuos e uso adequado de medicamentos para seu efetivo controle. A estrutura existente nas farmácias influencia no acesso, dispensação e armazenamento dos medicamentos e interfere na qualidade final da atenção. Objetivo Avaliar a estrutura das farmácias das Unidades de Saúde da Família no cuidado à hipertensão arterial e diabetes mellitus em Pernambuco, 2016. Método Estudo avaliativo do componente estrutura do modelo proposto por Donabedian. Entrevistou-se o responsável pela farmácia de uma amostra probabilística das equipes de saúde da família com questões sobre estrutura física, insumos e recursos humanos. Realizaram-se análises descritiva e de associação para verificar diferenças entre capital e interior. Resultados Mais de 90% das farmácias funcionavam nos turnos de manhã e tarde. Apenas 5,7% na capital e 2,7% no interior possuíam farmacêutico como responsável pelo serviço; essa atividade era desempenhada por outros profissionais, a maioria de nível médio e sem capacitação. A capital possuía maior variedade de medicamentos, contudo eles não eram suficientes para a demanda. Conclusão Identificaram-se problemas na estrutura das farmácias, principalmente na capacitação de recursos humanos e disponibilidade de insumos com poucas diferenças entre capital e interior.


Abstract Background Hypertension and diabetes mellitus are chronic diseases that require continuous care and the adequate use of medicines for effective control. The existing structure of pharmacies influences the access, dispensing, and storage of medicines and influence the quality of care. Objective To evaluate the structure of the pharmacies in the family health units in the care of arterial hypertension and diabetes mellitus in Pernambuco, 2016. Method This study is an evaluation of the structure component according to a model proposed by Donabedian. The person responsible for pharmacy from a probabilistic sample of family health teams was interviewed with questions about the physical structure, supplies, and human resources. A descriptive and association analysis was carried out to verify the differences between capital and interior. Results More than 90% of the pharmacies were operating in the mornings and afternoon shifts. Only 5.7% of the pharmacies in the capital and 2.7% in the interior had a pharmacist as responsible for service; this activity was performed by other professionals, most of whom were middle-level and without qualification. The capital had a greater variety of medicines; however, the quantities were insufficient for demand. Conclusion Problems in the pharmacies structure were identified, mainly in the training of human resources and supplies' availability with few differences between capital and interior.

ABSTRACT

STUDY OBJECTIVES: To characterize the balance of clinical and academic responsibilities of clinical track pharmacy faculty in the United States and evaluate organizational structures that promote satisfactory balance between these responsibilities. DESIGN: Prospective cross-sectional survey. SETTING: A 22-item online survey was developed and distributed via Qualtrics software. PARTICIPANTS: Clinical faculty members of the American College of Clinical Pharmacy Adult Medicine, Ambulatory Care, Cardiology, Critical Care, Gastrointestinal/Liver/Nutrition, Immunology/Transplantation, Infectious Disease, and Pediatrics Practice and Research Networks (PRNs) were invited to participate via the PRN electronic mailing list. MEASUREMENTS AND MAIN RESULTS: The survey comprised questions related to demographics, organizational structure, and balance of clinical and academic responsibilities. A total of 344 participants responded to some or all of the survey questions. The demographics were relatively equally balanced between faculty at state and private academic institutions, academic rank, and practice setting. Expected and actual effort allocations were similar for each of the clinical and academic responsibilities, with direct patient care and clinical teaching representing more than 50% effort allocation cumulatively. Clinical faculty at state institutions devoted a larger proportion of time to clinical service, whereas clinical faculty at private institutions devoted a greater proportion of time to didactic teaching. When asked about time constraints, 157 (69.8%) of the 225 survey participants responding to this question did not believe they had sufficient time to fulfill their nonclinical academic needs. Clinical faculty who were provided "protected time" away from clinical service had a significantly more favorable opinion of this question. CONCLUSION: Most of the clinical track pharmacy faculty indicated that they have insufficient time to fulfill their nonclinical academic responsibilities. Provision of protected time may alleviate some of these time constraints.

ABSTRACT

Este estudo teve como objetivo descrever as características das dimensões de estrutura e de processos dos Serviços de Atenção à Saúde da Pessoa Ostomizada (SASPO) no Estado de Minas Gerais, Brasil. Foi uma pesquisa transversal e descritiva realizada na Rede Estadual de Atenção à Saúde das Pessoas Ostomizadas no Estado de Minas Gerais, em 2011. Foram analisados critérios relacionados à estrutura e aos processos de 28 unidades de SASPO distribuídas em 13 macrorregiões de saúde. Os dados foram obtidos por meio de questionários estruturados com base nas normas governamentais para o serviço de referência ambulatorial para os estomizados. Foi realizada a distribuição de frequências para a análise descritiva dos dados. Os resultados mostraram que 72% dos SASPO têm estrutura para o cadastro dos pacientes e a dispensação de dispositivos e que 40% das unidades correspondem a serviços do tipo II. Quanto aos processos, 96% das unidades contavam com a assistência de enfermeiros, porém 52% deles sem capacitação. Percebeu-se ainda que esses profissionais estão mais envolvidos com as ações relacionadas à operacionalização do programa do que à assistência clínica. Conclui-se que os SASPO de Minas Gerais contam com estrutura e processos relacionados ao cadastro, controle e dispensação de equipamentos coletores e dispositivos.


This study aimed to describe the characteristics of the structure and processes dimensions of ostomized patients' Health Care Services (SASPO) in the State of Minas Gerais, Brazil. It's a cross-sectional and descriptive survey carried out in 28 SASPO of Minas Gerais in 2011. Criteria related to structure and processes were analyzed in these 28 health care units distributed in 13 health macro-regions. The data were obtained from structured questionnaires based on governmental regulations for reference service to ambulatory of the ostomized patients. Frequency distribution was used for the descriptive analysis. The results showed that 72% of the units have a structure able to register the patients and dispense devices, and that 40% of them correspond to the service classified as type II. Regard to the processes, 96% of the units relied on nurses, but 52% of them had no qualification. It was also observed that these professionals are more involved in actions related to the program implementation than in health care. It was concluded that the SASPO of Minas Gerais have structure and processes related to registration, control and dispensing of collector bags and devices.

ABSTRACT

O objetivo do presente estudo foi avaliar os indicadores de estrutura e processo empregados na implantação do serviço de Atenção Farmacêutica na rede Farmácia Popular do Brasil. Para tanto, um estudo piloto descritivo e avaliativo foi conduzido em novembro de 2009 em uma filial da rede Farmácia Popular do Brasil em Aracaju-SE. Os indicadores de estrutura e de processo foram avaliados por meio da aplicação da versão modificada do instrumento Behavioural Pharmaceutical Care Scale. Esta ferramenta foi composta de 34 itens distribuídos em 14 domínios, com escore mínimo de 16 e máximo de 175 pontos. Cada domínio foi avaliado por duas farmacêuticas-pesquisadoras (autorelato) e por dois farmacêuticos-auditores (observação direta). A pontuação média geral dos avaliadores foi de 144,25 ± 6,75, com variação de 138 a 153 pontos. Todos os domínios relacionados à estrutura do serviço e a maioria do processo apresentaram escores elevados (≥ 80%). As principais falhas foram detectadas no domínio referência a outros profissionais de saúde (56,3%), sobretudo, no encaminhamento a outros farmacêuticos 1,00 ± 0,00 (0,0%) e no envio de cartas ao médico 1,50 ± 0,58 (12,5%). Os resultados apontam que estrutura e processo empregados neste estudo piloto mostraram-se satisfatórios, indicando o potencial do serviço de Atenção Farmacêutica adotado para contribuir na elaboração de um modelo de implantação deste serviço para a rede Farmácia Popular do Brasil. No entanto mais estudos são necessários.


The aim of the present study was to evaluate indicators of the structure and process used in the implantation of a pharmaceutical care service of the drugstore chain People’s Pharmacy of Brazil. A descriptive, evaluative study was conducted in November 2009 at a branch of the People’s Pharmacy of Brazil in the city of Aracaju (state of Sergipe). Structure and process indicators were evaluated by applying the modified version of the Behavioral Pharmaceutical Care Scale, which is composed of 34 items divided among 14 subscales, the score of which ranges from 16 to 175 points. Evaluations were performed by two pharmacist researchers (self-reports) and two pharmacist auditors (direct observations). The overall mean score attributed by the evaluators was 144.25 ± 6.75 (range: 138 to 153). All subscales related to the structure of the service and most of the subscales related to the process achieved high scores (≥ 80%). The main flaws regarded referrals and consultations with other health professionals (56.3%), especially referrals to other pharmacists (1.00 ± 0.00; 0.0%) and sending letters to doctors (1.50 ± 0.58; 12.5%). The findings of this pilot study demonstrate satisfactory structure and process, indicating the potential of the pharmaceutical care service to contribute to the development of a model for the deployment of this service to other branches of the People’s Pharmacy of Brazil. However, further studies are needed.

O presente estudo pretende dar a conhecer, por um lado, o grau de satisfação dos usuários reais e potenciais em relação à oferta de atividades físico-desportivas de tempo livre da comunidade universitária de La Rioja (Espanha) e, por outro, constatar que atributos, referidos às instalações, recursos e atividades, se associam a dita satisfação. Na pesquisa se detecta que as infra-estruturas e sua manutenção (conservação, dimensões, cobertura, número de espaços físico-desportivos, limpeza e iluminação) são os atributos externos que mais determinam a satisfação dos usuários, tanto reais como potenciais


Satisfaction with physical and sport based free time activities. The concrete example of the univesitarian community of La Rioja (Spain). The following study intends, on the one hand, to show the level of satisfaction linked to the offer of physical and sport based free time activities in the univesitarian community of La Rioja (Spain) as well as, on the other hand, to check all the attributes (facilities, courses and activities) directly linked to this satisfaction. The study shows that the facilities and their maintenance (the upkeep, the size, the numbers of physical education areas, the cleanliness and the lighting) are at the origin of the satisfaction of potential and actual users
