ABSTRACT

BACKGROUND AND OBJECTIVE: Risankizumab - a humanized monoclonal antibody that targets the p19 subunit of IL-23 - has been recently approved to treat moderate-to-severe plaque psoriasis. Real-world data based on a representative pool of patients are currently lacking. OBJECTIVE: To assess the mid- and long-term safety and efficacy profile of risankizumab in patients with moderate-to-severe psoriasis in the routine clinical practice. METHODS: This was a retrospective and multicenter study of consecutive psoriatic patients on risankizumab from April 2020 through November 2022. The primary endpoint was the number of patients who achieved a 100% improvement in their Psoriasis Area and Severity Index (PASI) (PASI100) on week 52. RESULTS: A total of 510 patients, 198 (38.8%) women and 312 (61.2%) men were included in the study. The mean age was 51.7±14.4 years. A total of 227 (44.5%) study participants were obese (body mass index [BMI] >30kg/m2). The mean baseline PASI score was 11.4±7.2, and the rate of patients who achieved PASI100 on week 52, 67.0%. Throughout the study follow-up, 21%, 50.0%, 59.0%, and 66% of the patients achieved PASI100 on weeks 4, 16, 24, and 40, respectively. The number of patients who achieved a PASI ≤2 was greater in the group with a BMI ≤30kg/m2 on weeks 4 (P=.04), 16 (P=.001), and 52 (P=.002). A statistically significantly greater number of patients achieved PASI100 in the treatment-naïve group on weeks 16 and 52 (P=.001 each, respectively). On week 16 a significantly lower number of participants achieved PASI100 in the group with psoriatic arthropathy (P=.04). Among the overall study sample, 22 (4.3%) patients reported some type of adverse event and 20 (3.9%) discontinued treatment. CONCLUSIONS: Risankizumab proved to be a safe and effective therapy for patients with moderate-to-severe psoriasis in the routine clinical practice.

ABSTRACT

OBJECTIVE: To determine the predictive value of Cognitive Assessment, Symptom Severity, Personal Control and Self-Efficacy on decision making in the experience of Acute Coronary Syndrome symptoms. METHOD: Quantitative study of cross-sectional analytical design, a probabilistic sampling was carried out for 256 participants diagnosed with coronary syndrome in three health institutions. The effects between the independent variables Cognitive Assessment, Symptom Severity, Personal Control, Self-Efficacy and the dependent Decision-Making were analyzed. Using inferential statistics, a Generalized Linear Regression Model was carried out, which allowed establishing the causal relationships between the variables. RESULTS: Two predictive models were obtained between decision making and cognitive evaluation, in which personal control, severity of symptoms, sex and context were significant. Self-efficacy was not reported as a predictor variable. The values ​​of the independent variables showed a behavior directly proportional to the Decision Making score. CONCLUSION: A verification of the conceptual model for the management of symptoms was carried out.

Background and objective The treatment of psoriasis should not only focus on skin affectations but also weigh the parameters for health-related quality of life (HRQoL), thereby tackling the concept of cumulative life course impairment (CLCI) and treating the patient from a holistic perspective. The CRYSTAL study aimed to characterize psoriasis with real-word data from Spanish clinical practice in patients with moderate to severe disease who received continuous systemic treatment for at least 24 weeks by using the absolute Psoriasis Area and Severity Index (PASI) score and its correlation to HRQoL. Material and methods This was a non-interventional, cross-sectional study conducted in 30 centers in Spain, with 301 patients between the ages of 18 and 75 years. The study collected data regarding current treatment and absolute PASI and their relationship to HRQoL using the Dermatology Life Quality Index (DLQI), to activity impairment using the Work Productivity and Activity Impairment (WPAI) questionnaire, and to treatment satisfaction. Results The mean (SD) age was 50.5 (12.5) years, with a duration of disease of 14 (14.1) years. The mean (SD) absolute PASI reported was 2.3 (3.5), with 28.7% of patients presenting with PASI from >1 to ≤3 and 22.6% with PASI>3. Higher PASI scores were associated with higher DLQI (p<0.001) and WPAI scores and lower levels of treatment satisfaction (p<0.001). Conclusions These data indicate that achieving lower absolute PASI values may correlate not only with better HRQoL but also with better work productivity and treatment satisfaction (AU)

Background: There are serious doubts as to whether parental strictness, one of the two main dimensions of parental style, can be a negative or positive component of parenting in traditional societies. Method: Parenting style (authoritarian, authoritative, indulgent, and neglectful) was captured from strictness and warmth dimensions and child maladjustment was assessed with problems of self-esteem and self-concept (academic, social, emotional, family, and physical) studied worldwide. The sample was composed of 1,282 Chinese participants (676 females, 52.7%), 581 adolescent children (age ranging from 12-18 years, 45.3%), and 701 young adult children (age ranging from 19-31 years, 54.7%). A 4 × 2 × 2 factorial MANOVA was applied for all outcomes using parenting style, sex, and age as the independent variables. Results: The statistical analysis plainly indicated that authoritarian (strictness but not warmth) and neglectful (neither strictness nor warmth) parenting styles were associated with higher maladjustment in terms of lower self-esteem and self-concept scores. Indulgent (not strictness but warmth) and authoritative (strictness and warmth) parenting were positive parenting styles acting as protective factors against self-esteem and self-concept problems. The authoritative style (strictness and warmth), but not the authoritarian parenting style (strictness but not warmth), was the most positive parenting for academic self-concept, but only among adolescents. Conclusions: Interestingly, completely contrary to expectations that authoritarian parenting might be a positive parenting in traditional societies, present findings suggest that the authoritarian style might be a negative parenting related to child maladjustment. Antecedentes: Existen serias dudas sobre si la severidad parental, una de las dos dimensiones principales del estilo parental, puede ser un componente negativo o positivo de la socialización en las sociedades tradicionales. Método: El estilo parental (autoritario, autorizativo, indulgente y negligente) se evaluó a partir de las dimensiones de severidad y afecto, y el desajuste de los hijos por medio de problemas de autoestima y autoconcepto (académicos, sociales, emocionales, familiares y físicos) estudiados en todo el mundo. La muestra estaba compuesta por 1,282 participantes chinos (676 mujeres, 52.7%), 581 hijos adolescentes (de 12 a 18 años, 45.3%) y 701 hijos adultos jóvenes (de 19 a 31 años, 54.7%). Se aplicó un MANOVA factorial 4 × 2 × 2 para todos los criterios utilizando el estilo parental, el sexo y la edad como variables independientes. Resultados: El análisis estadístico indicó claramente que el estilo parental autoritario (severidad sin afecto) y el negligente (ni severidad ni afecto) estaban relacionados con un mayor desajuste, como indican las menores puntuaciones de autoestima y autoconcepto. El estilo indulgente (afecto sin severidad) y el autorizativo (severidad y afecto) fueron estilos parentales positivos que actuaron como factores protectores contra los problemas de autoestima y autoconcepto. El estilo autorizativo (severidad y afecto), pero no el autoritario (severidad sin afecto), fue el más positivo para el autoconcepto académico, pero sólo en los adolescentes. Conclusiones: En contra de las expectativas de que el estilo parental autoritario podría ser positivo para la socialización en las sociedades tradicionales, los presentes resultados sugieren que la socialización autoritaria es un estilo parental negativo relacionado con problemas de desajuste de los hijos. Introducción: La caries dental es una enfermedad multifactorial que prevalece durante la infancia y adolescencia. Objetivo: Determinar la prevalencia, severidad y gravedad de la caries dental en los adolescentes. Métodos: Se realizó un estudio descriptivo de corte transversal en adolescentes pertenecientes al municipio Plaza de la Revolución, La Habana, Cuba, en el período de octubre de 2019 a octubre de 2022. La muestra que se empleó fue de tipo aleatoria estratificada de 969 adolescentes. Para medir la prevalencia de la caries se utilizó el COP-D, para la severidad y gravedad el sistema ICDAS. Los datos primarios se procesaron con los programas informáticos Statistica 6.1. Se realizó prueba de ji al cuadrado por el programa Epidat y los resultados se describieron mediante cifras frecuenciales y porcentuales. Resultados : El índice de COP-D fue de 1,92; en el grupo de 18-19 años el COP-D fue de 1,96. Con relación a la severidad de las caries, el 19,2 % presentó caries código 1; el grupo de 18-19 años mostró un 18,9 % de afectación por caries, relacionadas con la gravedad de las lesiones; los mayores porcentajes se presentaron en las lesiones leves con un 35,2 %. Conclusiones: La prevalencia de la caries dental fue baja. El índice de COP-D ligeramente mayor en el grupo de 18-19 años. En cuanto a la severidad, predominaron las lesiones de caries código 1 y 2; las mayores proporciones fueron leves.


Introduction: Dental cavities is a multifactorial disease prevalent during childhood and adolescence. Objective: To determine the prevalence and severity of dental cavities in adolescents. Methods: A descriptive cross-sectional study was carried out in adolescents belonging to the Plaza de la Revolución municipality, Havana, Cuba, in the period from October 2019 to October 2022. The sample used was a stratified random sample of 969 adolescents. The COP-D was used to measure the prevalence of cavities, and the ICDAS system was used for severity and severity. The primary data were processed with Statistica 6.1 software. The chi-square test was performed with the Epidat program and the results were described by frequency and percentage figures. Results: The COP-D index was 1.92; in the group aged 18-19 years the COP-D was 1.96. In relation to cavities severity, 19.2% presented code 1 cavities; the 18-19 years age group showed 18.9% cavities involvement, related to the severity of the lesions; the highest percentages were presented in mild lesions with 35.2%. Conclusions: The prevalence of dental cavities was low. The rate of COP-D was slightly higher in the 18-19 years age group. In terms of severity, cavities code 1 and 2 lesions predominated; the highest proportions were mild.

Introducción: la periodontitis es una enfermedad infecciosa multifactorial asociada a un biofilm de microorganismos patógenos. Objetivo: el objetivo del trabajo fue establecer la prevalencia de Porphyromonas gingivalis en pacientes con periodontitis y relacionarla con la severidad de la enfermedad. Material y métodos: participaron 45 pacientes, sistémicamente saludables, con edades entre 35 y 65 años. El grado de periodontitis se definió según los criterios de Papapanou y colaboradores. Como grupo control, se incluyeron 20 sujetos de ambos sexos sin periodontitis y sin enfermedades sistémicas. Se tomaron muestras de fluido gingival en dos sitios más profundos. Porphyromonas gingivalis se detectó por PCR (reacción en cadena de la polimerasa). Resultados: la frecuencia relativa de periodontitis fue de 13.3% grado I, 46.7% grado II y 40% grado III. El sexo masculino presentó periodontitis grado III 72.2% y grado II 52.3%. El grado I se registró con mayor frecuencia en el sexo femenino, 66.7%. La prevalencia de Porphyromonas gingivalis en la población con periodontitis fue de 44.4%. Se obtuvieron diferencias estadísticamente significativas entre los grados de severidad de periodontitis y la presencia de Porphyromonas gingivalis (p = 0.0002, α = 5%). Conclusión: la periodontitis predominó en el sexo masculino. La prevalencia de Porphyromonas gingivalis en la población con periodontitis crónica fue de 44.4% y su presencia está relacionada con la severidad (AU)


Introduction: periodontitis is a multifactorial infectious disease associated with a biofilm of pathogenic microorganisms. The prevalence of Porphyromonas gingivalis in the population with periodontitis was 44.4%. Statistically significant differences were obtained between the degrees of severity of periodontitis and the presence of Porphyromonas gingivalis (p = 0.0002, α = 5%). Conclusion: periodontitis predominated in males. The prevalence of Porphyromonas gingivalis in the population with chronic periodontitis was 44.4% and its presence is related to severity (AU)

RESUMEN Objetivos. Determinar la asociación entre el grado de severidad de la infección por COVID-19 durante el embarazo y la rotura prematura de membranas pretérmino (RPMP) en un hospital nivel III de Perú. Materiales y Métodos. Estudio transversal, analítico y observacional en mujeres mayores de 18 años con diagnóstico de infección por COVID-19 en el embarazo durante el 2020-2022. Se recogieron variables clínicas y obstétricas. Para el análisis descriptivo se realizaron las pruebas de Chi Cuadrado y exacta de Fisher, y para el análisis multivariado, se calculó la razón de prevalencia mediante regresión de Poisson en modelos crudos y ajustados. Todas las pruebas estadísticas se realizaron considerando un valor de p<0,05 como significativo y con un nivel de confianza de 95%. Resultados. Se analizaron los datos de 163 gestantes con COVID-19, de las cuales el 9,2% tuvieron RPMP, todas fueron casos sintomáticos. Los casos leves de COVID-19 tuvieron 1,10 veces la probabilidad de presentar RPMP (RPa=1,10; IC95%: 1,02−1,18) y los casos moderados/severos tuvieron 1,64 veces esta probabilidad (RPa=1,64; IC95%: 1,43−1,87), en comparación con los casos asintomáticos. Conclusiones. Se identificó que un mayor grado de severidad de la infección por COVID-19 durante el embarazo se asoció a una mayor probabilidad de tener RPMP.


ABSTRACT Objectives. To determine the association between the degree of severity of COVID-19 infection during pregnancy and preterm premature rupture of membranes (PPROM) in a level III hospital in Peru. Materials and Methods. Cross-sectional, analytical and observational study in women older than 18 years diagnosed with COVID-19 infection during pregnancy, between the years 2020 and 2022. Clinical and obstetric variables were collected. The chi-square and Fisher's exact tests were used for the descriptive analysis. For the multivariate analysis, we calculated the prevalence ratio by using Poisson regression in crude and adjusted models. All statistical tests were performed considering a value of p<0.05 as significant and with a confidence level of 95%. Results. We analyzed data from 163 pregnant women with COVID-19, of which 9.2% had PPROM; all were symptomatic cases. Mild COVID-19 cases were 1.10 times more likely to have PPROM (RPa=1.10; 95%CI: 1.02-1.18) and moderate/severe cases were 1.64 times more likely (RPa=1.64; 95%CI: 1.43-1.87), compared to asymptomatic cases. Conclusions. We identified that a higher degree of severity of COVID-19 infection during pregnancy was associated with a higher probability of having PPROM.

Fundamento el asma bronquial es una de las enfermedades crónicas de mayor prevalencia en la infancia. Las guías para el diagnóstico y tratamiento se rigen según severidad, nivel de control y calidad de vida, sin embargo, no existen evidencias que demuestren su asociación estadística. Objetivo medir el grado de asociación entre la severidad del asma bronquial, nivel de control y calidad de vida en niños. Métodos estudio de corte transversal, realizado en el periodo enero-diciembre de 2022, en una muestra de 189 pacientes con diagnóstico confirmado de asma bronquial. Se analizaron las variables: edad, sexo, severidad, nivel de control y calidad de vida. Se calcularon estadígrafos, frecuencia absoluta, porcentaje, media aritmética y desviación estándar. Para medir asociación se utilizó la prueba de Chi cuadrado (p<0,01) y para evaluar la fuerza de la asociación el coeficiente de V-Cramer. Resultados la edad media resultó 9,17 años (DE ± 0,67). Predominaron los varones, representativos del 53,54 %. Prevalecieron el asma intermitente (39,15 %), el nivel de control parcial (59,79 %) y la calidad de vida alta (50,26 %). Al relacionar severidad con nivel de control, el estadígrafo de asociación indicó X2=190,461 (VC= 0,662); en el caso de severidad y calidad de vida, X2=252,673 (VC=0,762); y entre nivel de control y calidad de vida, X2=66,733 (VC=0,401). Conclusiones existió asociación fuerte entre severidad del asma bronquial-nivel de control y entre la severidad del asma-calidad de vida; así como asociación moderada entre nivel de control-calidad de vida.


Foundation bronchial asthma is one of the most prevalent chronic diseases in childhood. The guidelines for diagnosis and treatment are governed by severity, level of control and quality of life, however, there is no evidence that demonstrates its statistical association. Objective to measure the association degree between the severity of bronchial asthma, control level and quality of life in children. Methods cross-sectional study, carried out from January to December 2022, in a sample of 189 patients with a bronchial asthma confirmed diagnosis. The analyzed variables were: age, sex, severity, level of control and quality of life. Statisticians, absolute frequency, percentage, arithmetic mean and standard deviation were calculated. The Chi-square test (p<0.01) was used to measure association and the V-Cramer coefficient was used to evaluate the strength of the association. Results the mean age was 9.17 years (SD ± 0.67). Males predominated, representing 53.54%. Intermittent asthma (39.15%), partial control level (59.79%) and high quality of life (50.26%) prevailed. When relating severity with level of control, the association statistician indicated X2=190.461 (VC= 0.662); in the case of severity and quality of life, X2=252.673 (VC=0.762); and between level of control and quality of life, X2=66.733 (VC=0.401). Conclusions there was a strong association between severity of bronchial asthma-level of control and between severity of asthma-quality of life; as well as a moderate association between level of control-quality of life.

RESUMEN La psoriasis es una enfermedad crónica de la piel mediada por el sistema inmunológico con una base genética y patogénica compleja, que frecuentemente conduce a comorbilidades significativas y una reducción en la calidad de vida. Su prevalencia varía a nivel global y muestra una tendencia creciente con el tiempo. Comorbilidades como la artritis psoriásica, enfermedades cardiovasculares y problemas de salud mental complican aún más la carga de la psoriasis. Las opciones de tratamiento van desde terapias tópicas hasta agentes sistémicos, siendo los agentes biológicos prominentes en los últimos años. Sin embargo, la seguridad y eficacia de estos tratamientos se evalúan continuamente a través de datos del mundo real. La vitamina D ha llamado la atención como un posible objetivo terapéutico debido a su papel en la regulación inmunológica y la función de barrera de la piel. Esta revisión tiene como objetivo evaluar la eficacia de la suplementación oral de vitamina D en mejorar la gravedad de la psoriasis. Después de una búsqueda bibliográfica, se encontró que la psoriasis es una condición multifacética con significativas implicaciones globales. Los agentes biológicos han transformado su manejo, y la suplementación oral de vitamina D es un camino prometedor para una mayor exploración. Un enfoque integral centrado en el paciente que tenga en cuenta las comorbilidades y los resultados a largo plazo es crucial para optimizar el cuidado de la psoriasis. Se necesita más investigación para comprender completamente el papel de la vitamina D en la psoriasis y su potencial como intervención terapéutica.


ABSTRACT Psoriasis is a chronic immune-mediated skin disease with a complex genetic and pathogenic basis, often leading to significant comorbidities and a reduced quality of life. Its prevalence varies globally and exhibits an increasing trend over time. Comorbidities such as psoriatic arthritis, cardiovascular diseases, and mental health issues further compound the burden of psoriasis. Treatment options range from topical therapies to systemic agents, with biologics playing a prominent role in recent years. However, the safety and efficacy of these treatments are continuously assessed through real-world data. Vitamin D has gained attention as a potential therapeutic target due to its role in immune regulation and skin barrier function. This review aims to evaluate the efficacy of oral vitamin D supplementation in ameliorating the severity of psoriasis. After bibliographic search, it was found that psoriasis is a multifaceted condition with significant global implications. Biologics have transformed its management, and oral vitamin D supplementation is a promising avenue for further exploration. A comprehensive, patient-centered approach that considers comorbidities and long-term outcomes is crucial for optimizing psoriasis care. Further research is needed to fully understand the role of vitamin D in psoriasis and its potential as a therapeutic intervention.

Introducción: La miastenia gravis (MG) es una enfermedad autoinmune mediada por anticuerpos. El cuadro clínico se caracteriza por debilidad muscular fluctuante y fatigable, con frecuente afectación de músculos fonodeglutorios y respiratorios. Dada la severidad que implica el compromiso respiratorio en la MG, su evaluación rutinaria es esencial.Nuestro objetivo fue identificar un marcador semiológico útil en la pesquisa del compromiso respiratorio en pacientes con MG.Métodos: Se realizó un trabajo observacional en pacientes con diagnóstico de MG. Los pacientes fueron evaluados con test de cuenta máxima, pico flujo espiratorio (PEF), cuestionario de disnea modificado (mMRC) y valoración de fuerza del cuello. Los resultados de estos parámetros fueron correlacionados con la medición de CVF (capacidad vital forzada) y presiones bucales estáticas máximas (PiMáx y PeMáx). Resultados:Cuarenta y cinco pacientes con MG fueron incluidos, dos pacientes tenían MGFA grado I, 35 grado II, siete grado III y uno grado IV al momento de la evaluación. Se halló una correlación positiva entre el test de cuenta máxima y la CVF (r = 0,57, p = 0,000), y entre el PEF y la CVF (r = 0,76, p = 0,000). El grado de disnea, según el mMRC, mostró una correlación negativa con la CVF (r =-0,31 p = 0,03). A su vez, el PEF correlacionó con la PeMáx de forma positiva, estadísticamente significativa (r = 0,51, p = 0,002).Conclusiones: El PEF, el test de cuenta máxima y el mMRC fueron útiles para evaluar la función respiratoria en pacientes con MG.(AU)

Considering the severity of respiratory involvement in MG, routine evaluation ofrespiratory function is essential.The aim of this study was to identify a useful clinical marker of respiratory involvement inpatients with MG. Methods: We performed an observational study of patients with MG. All cases were evaluatedwith the single-breath counting test, peak expiratory flow (PEF), a modified Medical ResearchCouncil dyspnoea scale (mMRC), and a neck strength assessment. The results of these parameters were correlated with forced vital capacity (FVC), maximal inspiratory pressure (MIP), andmaximal expiratory pressure (MEP). Results: The study included 45 patients with MG: 2 patients classified as grade I on the Myasthenia Gravis Foundation of America classification at the time of evaluation, 35 classified asgrade II, 7 classified as grade III, and one classified as grade IV. Positive correlations were foundbetween single-breath counting test scores and FVC values (r = 0.57, p = .000), and betweenPEF and FVC values (r = 0.76, p = .000). Severity of dyspnoea according to the mMRC scaleshowed a negative correlation with FVC values (r = -0.31, p = .03). PEF also showed a significantcorrelation with MEP (r = 0.51, p = .002). Fundamento la COVID-19 es causada por el virus del SARS-Cov-2 y presenta una amplia sintomatología, tanto en su fase aguda como en su fase crónica. Entre los sistemas que afecta se encuentra el Nervioso, debido a su mecanismo neurotrópico. Objetivo determinar el riesgo de complicaciones neurológicas asociadas a la severidad de la COVID-19 en pacientes adultos. Métodos se realizó un estudio descriptivo y transversal, que incluyó a 143 pacientes positivos a COVID-19 atendidos en el Hospital San Vicente de Paúl, de Ibarra, Ecuador, durante el año de 2021. Se analizaron las complicaciones neurológicas y la severidad de la COVID-19. Como medida de asociación para dichas variables, se utilizó Test de Fisher (p = ≤ 0,05) y se realizó un análisis bivariado. Resultados el 54 % de los pacientes presentó complicaciones neurológicas del Sistema Nervioso Central, mientras que el 46 % fue del Sistema Nervioso Periférico; y fue la severidad leve-moderada la de mayor frecuencia (41 %), y la alteración auditiva la que mayor probabilidad estadística tuvo de presentarse (OR= 74,968; IC: 95 %). La tasa de letalidad en estos pacientes fue de 7,1 %; y en aquellos con complicaciones neurológicas, de 8,4 %. Conclusión las secuelas neurológicas con mayor probabilidad de presentarse fueron la alteración auditiva y la alteración del gusto, en pacientes con severidad leve y severidad grave, respectivamente; además de polineuropatía en pacientes con severidad crítica, la cual también se presentó en pacientes fallecidos.


Background COVID-19 is caused by the SARS-Cov-2 virus and presents a wide range of symptoms, both in its acute phase and in its chronic phase. Among the systems that it affects is the Nervous, due to its neurotropic mechanism. Objective to determine the risk of neurological complications associated with the COVID-19 severity in adult patients. Methods a descriptive and cross-sectional study was carried out, which included 143 positive patients for COVID-19 treated at the San Vicente de Paúl Hospital, in Ibarra, Ecuador, during 2021. Neurological complications and the severity of the COVID-19 disease were analyzed. As a measure of association for these variables, the Fisher Test was used (p = ≤ 0.05) and a bivariate analysis was performed. Results 54% of the patients presented neurological complications of the Central Nervous System, while 46% were of the Peripheral Nervous System; and mild-moderate severity was the most frequent (41%), and hearing impairment had the highest statistical probability of occurring (OR= 74.968; CI: 95%). The case fatality rate in these patients was 7.1%; and in those with neurological complications, 8.4%. Conclusion the neurological sequelae most likely to occur were hearing impairment and taste impairment, in patients with mild severity and serious severity, respectively; in addition to polyneuropathy in patients with critical severity, which also occurred in deceased patients.

ABSTRACT

Objetivos. Determinar la asociación entre los síntomas de la infección por la COVID-19 y los resultados materno-perinatales adversos en gestantes de un hospital de referencia. Materiales y métodos. Estudio transversal analítico en gestantes en el tercer trimestre, hospitalizadas por la COVID-19 en el área de ginecología y obstetricia de un hospital general de Lima, en el 2020. Se recogieron variables clínicas y obstétricas. Para el análisis descriptivo se empleó la prueba exacta de Fisher y Chi Cuadrado, y para hallar la asociación entre las variables de interés se usó la regresión de Poisson, con un intervalo de confianza al 95% (IC95%). Resultados. Se incluyeron a 272 gestantes, de ellas el 50,3% tuvieron síntomas de infección. El 35,7% de las gestantes y el 16,5% de los recién nacidos tuvieron un resultado adverso. Tener síntomas de la infección por la COVID-19 aumentó el riesgo de complicaciones maternas en su conjunto (RP= 2,32 IC95%: 1,61-3,34), de ruptura prematura de membranas (RP= 2,73 IC95%: 1,51-4,94) y de preeclampsia (RP= 2,73 IC95%: 1,51-4,94). De igual forma, aumentó el riesgo de complicaciones perinatales en su conjunto (RP= 2,51 IC95%: 1,34-4,68) y de sufrimiento fetal agudo (RP= 2,99 IC95%: 1,07-8,38). Conclusiones. Los síntomas de la infección por la COVID-19 aumentan el riesgo de resultados materno-perinatales adversos.


Objectives. To determine the association between symptoms of COVID-19 infection and adverse maternal-perinatal outcomes in pregnant women from a referral hospital. Materials and methods. Analytical cross-sectional study of women in the third trimester of pregnancy hospitalized due to COVID-19 in the gynecology and obstetrics area of a general hospital in Lima during 2020. Clinical and obstetric variables were collected. Fisher's exact test and Chi-square test were used during the descriptive analysis. Poisson regression was used to find the association between the variables of interest, with a 95% confidence interval (95%CI). Results. A total of 272 pregnant women were included, 50.3% of whom had symptoms of infection. Of these, 35.7% of the pregnant women and 16.5% of the newborns had an adverse outcome. Having symptoms of COVID-19 infection increased the risk of maternal complications as a whole (PR= 2.32 95%CI: 1.61-3.34), premature rupture of membranes (PR= 2.73 95%CI: 1.51-4.94) and preeclampsia (PR= 2.73 95%CI: 1.51-4.94). Similarly, symptoms of COVID-19 infection increased the risk of perinatal complications as a whole (PR= 2.51 95%CI: 1.34-4.68) and acute fetal distress (PR= 2.99 95%CI: 1.07-8.38). Conclusions. The presence of symptoms of COVID-19 infection increase the risk of adverse maternal-perinatal outcomes.

ABSTRACT

Objetivo: identificar factores asociados a severidad y mortalidad por COVID-19. Material y métodos: estudio observacional multicéntrico retrospectivo de pacientes ingresados por COVID-19 en centros de una ciudad de la República Dominicana. Se reportaron datos demográficos, clínicos, terapéuticos y de laboratorio para su comparación entre pacientes con enfermedad severa y no severa, fallecidos y dados de alta. Resultados: los factores asociados a mayor riesgo de severidad y mortalidad fueron el sexo masculino, la hipertensión arterial, la cantidad de comorbilidades, el uso de fármacos previos, niveles elevados de marcadores inflamatorios, linfopenia, neutrofilia, entre otros. Mientras que el uso de inhibidores de la enzima convertidora de angiotensina se asoció a menor mortalidad. Conclusiones: la identificación de los factores asociados a progresión severa y mortal por COVID-19 contribuye a la toma temprana de decisiones relacionadas a la hospitalización y a pautas de manejo médico diferencial en quienes conllevan mayor riesgo de desenlaces adversos.


Objective: To identify factors associated with severity and mortality by COVID-19. Material and methods: Retrospective multicenter observational study of patients admitted with COVID-19 in health centers in a city of the Dominican Republic. Demographic, clinical, therapeutic and laboratory data were reported for comparison between patients with severe and non-severe disease, deceased and discharged. Results: Factors significantly associated with higher risk of severity and mortality were male sex, hypertension, number of comorbidities, drugs' used previously, high levels of inflammatory markers, lymphopenia, neutrophilia, among others. While the use of angiotensin converting enzyme inhibitors was associated with lower mortality. Conclusions: Identification of factors associated with severe and fatal progression of COVID-19 contributes to early clinical decision-making related to hospitalization and differential medical management in those at increased risk of adverse outcomes.

Introducción. Las eritrocitosis patológicas en la altura afectan al 10 % de la población constituyéndose una causa importante de morbilidad por enfermedades no transmisibles. Suscita emergente categorizar la severidad de estas eritrocitosis para estimar su evolución y tratamientos adecuados. Objetivo. Estratificar el riesgo de las eritrocitosis patológicas en la altura considerando parámetros de severidad que sean de utilidad clínica en el pronóstico y tratamiento. Material y métodos. Estudio transversal retrospectivo de 283 historias clínicas de pacientes con eritrocitosis patológica de altura (EPA) o eritrocitosis secundaria (ES), residentes en altura (>3600 m s. n. m) y diagnosticados entre gestiones 2000 a 2021. Se identificó características clínico-laboratoriales diferenciales respecto del diagnóstico, respuesta al tratamiento y evolución de pacientes. Se planteó 3 niveles de estratificación de riesgo (bajo, intermedio, alto) considerando variaciones en síntomas de hiperviscosidad, eritropoyetina, complicaciones y comorbilidades. Resultados. 194 pacientes correspondieron al grupo de riesgo bajo, 67 al riesgo intermedio y 22 al riesgo alto. Riesgo bajo conllevó Epo 30 mUI/ml (30-100 mUI/ml), tratamiento con atorvastatina-aspirina, respuestas parciales y pronóstico regular, concerniendo pacientes con ES asociada a patologías pulmonares leves. Riesgo alto reflejó Epo >100 mUI/ml, inclusión de hidroxiurea al tratamiento con atorvastatina, menor respuesta y pronóstico desfavorable, incumbiendo pacientes >60 años con ES asociada a patologías pulmonares crónicas severas o complicaciones por eritrocitosis. Conclusiones. Contar con niveles de riesgo para las eritrocitosis patológicas permite conjeturar su pronóstico y optimizar decisiones terapéuticas.


Introduction. Pathological erythrocytoses at altitude affect 10% of the population, representing an important cause of morbidity from non-communicable diseases. Categorizing the severity of such erythrocytoses to estimate their evolution and suitable treatments becomes emergent. Objective. To stratify the risk of the pathological erythrocytoses at high altitude considering severity parameters useful for prognosis and treatment. Material and methods. Retrospective cross-sectional study that included 283 medical records of patients with Chronic Mountain Sickness-erythrocytosis (CMS-e) or Secondary Erythrocytosis (SE), inhabitants at high altitude (>3600 m a. s. l.) diagnosed between 2000 to 2021. Differential clinical-laboratory characteristics regarding the diagnosis, response to treatment and evolution of patients were identified. Three risk groups (low, intermediate, high) were raised, considering variations about hyperviscosity symptoms, erythropoietin levels, complications, and comorbidities. Results. 194 patients corresponded to the low-risk group, 67 to the intermediaterisk and 22 to the high-risk. Low-risk group involved Epo 30 mIU/ml (30-100 mIU/ ml), treatment with atorvastatin-aspirin, partial responses and favorable prognosis, concerning patients with SE attributed to mild lung diseases. High-risk reflected Epo >100 mIU/ml, inclusion of hydroxyurea to the treatment with atorvastatin, poor response and unfavorable prognosis, involving patients >60 years of age with SE attributed to severe and chronic lung diseases or complications due to erythrocytosis. Conclusion. Counting on a risk stratification for pathological erythrocytosis at high altitude allows to assess the prognosis and optimize therapeutic decisions.

El sistema ungueal conforma una compleja unidad anatómica fundamental en el ser humano, susceptible de sufrir numerosas alteraciones debido a procesos patológicos, tanto sistémicos como locales. El estudio de la patología ungueal es un tema de especial interés, sin embargo, en el ámbito clínico todavía resulta complicado analizar parámetros como área concreta de extensión, anatomía comprometida o porcentaje de afectación, dificultando poder realizar una valoración objetiva y establecer pautas efectivas de tratamiento, debiendo recurrir, en numerosas ocasiones, al examen histopatológico. En este sentido, los índices de medición se posicionan como una herramienta que permitiría valorar estos factores de forma numérica, reproducible y simple. El objetivo de este estudio fue identificar las herramientas de medición de las diferentes patologías ungueales descritas en la literatura. Se realizó una revisión de la literatura por pares en cuatro bases de datos atendiendo a criterios del Critical Appraisal Skills Programme España (CASPe). Los hallazgos describieron la existencia de 12 herramientas de medición de patología ungueal. Las conclusiones señalaron, finalmente, el Nail Psoriasis Severity Index o índice NAPSI como método más utilizado para valorar la severidad de la psoriasis ungueal, pese a los reportes obtenidos en sus modificaciones posteriores. En cuanto a la onicomicosis, el Scoring Clinical Index for Onychomycosis o índice SCIO resultó ser el método objetivo más útil en la práctica clínica. Para la paroniquia crónica, el Índice de paroniquia crónica se distinguió como una buena alternativa a las opciones de valoración previas (AU)


The nail system forms a complex anatomical unit fundamental in humans, susceptible to numerous alterations due to both systemic and local pathological processes. The study of nail pathology is a subject of special interest, however, in the clinical setting, it is still difficult to analyse parameters such as specific area of extension, anatomy involved or percentage of involvement, making it difficult to carry out an objective assessment and establish effective treatment guidelines, having to resort, on numerous occasions, to histopathological examination. In this sense, measurement indices are positioned as a measurement tool that would allow these factors to be assessed in a numerical, reproducible and simple way. The aim of this study was to identify measurement tools for the different nail pathologies described in the literature. A peer-review of the literature was conducted in four databases according to Critical Appraisal Skills Programme España (CASPe) criteria. The findings described the existence of 12 nail pathology measurement tools. Conclusions finally pointed out the Nail Psoriasis Severity Index or NAPSI index as the most widely used method to asses the severity of nail psoriasis, despite reports of subsequent modifications. For onychomycosis, the Scoring Clinical Index for Onychomycosis or SCIO index appeared to be the most useful objective method in clinical practice. For chronic paronychia, the Chronic Paronychia Index standed out as a good alternative to previous assessment options (AU)

Objetivo: investigar a correlação da carga de trabalho de enfermagem e índice de gravidade em pacientes com COVID-19, em Unidade de Terapia Intensiva Adulto. Método: estudo transversal, retrospectivo, realizado entre fevereiro e outubro de 2020, com dados de 93 pacientes com COVID-19. A análise da gravidade foi realizada segundo o Acute Physiology Age and Chronic Health Evaluation e a carga de trabalho pelo Nursing Activities Score. Resultados: prevaleceram idosos, sexo masculino, com uma ou mais comorbidades, com internação superior a sete dias. A média do Nursing Activities Score admissional foi de 74,2 pontos e o índice de gravidade de 24,47, com letalidade de 66,6%. Identificada correlação significante entre carga de trabalho e gravidade do paciente (0,5132; p<0,0001). Conclusão: carga de trabalho de enfermagem correlacionou-se moderadamente com a gravidade dos pacientes com COVID-19. Os resultados deste estudo podem auxiliar no dimensionado de enfermagem e prevenção de eventos adversos.


Objective: to investigate the correlation between nursing workload and severity index in patients with COVID-19 in an Adult Intensive Care Unit. Method: this retrospective, cross-sectional study was conducted between February and October 2020, with data from 93 patients with COVID-19. Severity analysis was performed according to the Acute Physiology Age and Chronic Health Evaluation method and workload, by the Nursing Activities Score. Results: patients were predominantly elderly, male, with one or more comorbidities, and hospitalized for more than seven days. The average Nursing Activities Score at admission was 74.2 and the severity index was 24.47, with 66.6% lethality. Significant correlation was found between workload and patient severity (0.5132; p<0.0001). Conclusion: nursing workload correlated moderately with severity of patients with COVID-19. The study findings can help in scaling nursing staffs and preventing adverse events.

Método: estudio transversal, retrospectivo, realizado entre febrero y octubre de 2020, con datos de 93 pacientes con COVID-19. El análisis de la severidad se realizó de acuerdo con la Acute Physiology Age and Chronic Health Evaluation y la carga de trabajo de acuerdo con el Nursing Activities Score. Resultados: predominó el adulto mayor, del sexo masculino, con una o más comorbilidades, hospitalizado desde hace más de siete días. El Nursing Activities Score medio al ingreso fue de 74,2 puntos y el índice de gravedad de 24,47, con una letalidad del 66,6%. Se identificó una correlación significativa entre la carga de trabajo y la gravedad del paciente (0,5132; p<0,0001). Conclusión: la carga de trabajo de enfermería se correlacionó moderadamente con la gravedad de los pacientes con COVID-19. Los resultados de este estudio pueden auxiliar en el dimensionamiento de la enfermería y la prevención de eventos adversos.
